Abstract:Introduction: Although musical hallucinations do not tend to be accompanied by delusions, occasionally patients persistently accuse others of being responsible for causing the music they perceive, sometimes with severe social consequences such as frequently calling the police or moving house. In this study we seek to broaden our understanding of this rare type of musical hallucination that comes with secondary delusions and lack of insight, and to explore associations, underlying mechanisms, and treatment possibilities. Methods: The present study is part of a cohort study on musical hallucinations carried out in the Netherlands from 2010 through 2023. Participants underwent testing with the aid of the MuHa Questionnaire, Launay-Slade Hallucinations Scale (LSHS), Schizotypal Personality Questionnaire (SPQ), Hamilton Depression Rating Scale (HDRS), and Mini Mental State Examination (MMSE). Additionally, they underwent a brain MRI, electroencephalogram, and audiological testing. Results: Five patients out of a group of N = 81 (6%) lacked insight and presented with secondary delusions regarding the perceived music. They were all female, of advanced age, and hearing-impaired, and were diagnosed with cognitive impairment. In three patients (60%), risperidone was started. This had a positive effect on the hallucinations and secondary delusions. Conclusion: The pathophysiological process underlying musical hallucinations is multifactorial in nature. We consider cognitive impairment the most likely contributing factor of the secondary delusions and lack of insight encountered in our patients, and antipsychotics the most beneficial treatment. On the basis of these small numbers, no definite conclusions can be drawn, so further research is needed to elucidate the underlying mechanisms and to develop evidence-based treatment methods for people experiencing this rare and debilitating combination of symptoms. Since the black box warning of risperidone cautions against the use of this drug in elderly persons with dementia, a proper comparison with the efficacy and safety of other antipsychotics for this group is paramount.

Abstract:BACKGROUND Musical hallucinations (MH) involve the false perception of music in the absence of external stimuli which links with different etiologies. The pathomechanisms of MH encompass various conditions. The etiological classification of MH is of particular importance and offers valuable insights to understand MH, and further to develop the effective treatment of MH. Over the recent decades, more MH cases have been reported, revealing newly identified medical and psychiatric causes of MH. Functional imaging studies reveal that MH activates a wide array of brain regions. An up-to-date analysis on MH, especially on MH comorbid psychiatric conditions is warranted. AIM To propose a new classification of MH; to study the age and gender differences of MH in mental disorders; and neuropathology of MH. METHODS Literatures searches were conducted using keywords such as “music hallucination,” “music hallucination and mental illness,” “music hallucination and gender difference,” and “music hallucination and psychiatric disease” in the databases of PubMed, Google Scholar, and Web of Science. MH cases were collected and categorized based on their etiologies. The t -test and ANOVA were employed (P < 0.05) to compare the age differences of MH different etiological groups. Function neuroimaging studies of neural networks regulating MH and their possible molecular mechanisms were discussed. RESULTS Among the 357 yielded publications, 294 MH cases were collected. The average age of MH cases was 67.9 years, with a predominance of females (66.8% females vs 33.2% males). MH was classified into eight groups based on their etiological mechanisms. Statistical analysis of MH cases indicates varying associations with psychiatric diagnoses. CONCLUSION We carried out a more comprehensive review of MH studies. For the first time according to our knowledge, we demonstrated the psychiatric conditions linked and/or associated with MH from statistical, biological and molecular point of view.

Abstract:Musical hallucination is a rare perceptual phenomenon wherein individuals hear music in the absence of external auditory stimuli. This phenomenon occurs across diverse medical conditions and can be triggered by some drugs. The underlying mechanism of drug-induced hallucination is unknown. This study explores drug-induced musical hallucination through a literature review, aiming to investigate its pathophysiology and potential treatment modalities. A literature search was conducted until January 2024 using databases PubMed, WorldCat, Google Scholar, and DOAJ, with keywords "drugs induced musical hallucination" or "drugs" combined with "musical hallucination." The search yielded 24 articles which met inclusion criteria, encompassing 27 cases. The average patient age was 58.3 years, with 67.9% females. Prevalent conditions among cases included hearing impairments, psychiatric disorders, cancers, and neurodegenerative conditions. Common trigger drugs comprised antidepressants, opioids, anti-Parkinson drugs, ketamine, and voriconazole. Musical hallucination descriptions varied widely, and 6 patients reported concurrent visual hallucinations. The onset of symptoms ranged from 75 min to 240 days. Treatment strategies included termination of trigger drugs, dosage reduction, alteration of administration routes or formula, switching to similar drugs, or addition of antidepressants, sedatives, or atypical antipsychotic medications. Musical hallucinations completely disappeared in 24/27 (88.9%) patients but continued in 3/27 (11.1%) patients. The current study concludes that drug-induced musical hallucination may arise from altering neurotransmitter/receptor balance and intricate interactions between trigger drugs and underlying conditions.

Abstract:<b>KEY POINTS</b> ➢ Studies suggest a substantial minority of healthy older adults have hallucinatory experiences, in line with existing evidence on hallucinations in other age groups, though it is still unclear if hallucination prevalence increases or declines with age in older cohorts.➢ Stigma attached to both hallucinations and ageing leads to considerable under-reporting of these experiences in healthy older adults and may negatively bias how professionals, family members, and the public respond.➢ Why and when hallucinations in healthy older adults remit, persist, or progress to other clinical disorders remains poorly understood.➢ Current evidence points to a range of factors associated with hallucinations in older adults including decline in sensory or cognitive functioning, poor sleep, and psychosocial stressors (e.g., social isolation, loneliness, and bereavement), highlighting the need for accurate assessment and tailored interventions. Hallucinations, though common in youth and younger adults, are not the preserve of these age groups. Accumulating evidence shows that hallucinatory experiences are also present at surprisingly high rates in healthy older adults in the general community. Furthermore, stigma and misunderstanding of hallucinations, together with ageism, may lead to under-reporting of these experiences by older adults, and misdiagnosis or mismanagement by health and mental health practitioners. Consequently, improved public and professional knowledge is needed about the nature and significance of hallucinations with advancing age. The purpose of this review is to provide a comprehensive overview, and critical analysis, of research on the prevalence, psychosocial, and neurobiological factors associated with hallucinations in people aged 60 years and over. To the best of our knowledge, this is the first review of its kind in the literature. The evidence supports a dynamic conceptualization of hallucinations, in which the emergence of hallucinations is viewed as a balance between the sensory, cognitive, or social impairments accompanying advancing age and the degree to which compensatory processes elicited by these impairments are successful. We briefly summarize the implications of the literature for aged care services and interventions, and stress that far more studies are needed in this important field of research.

Abstract:Australian &New Zealand Journal of Psychiatry, Ahead of Print. Objective:Auditory hallucinations (hearing voices) have been associated with a range of altered cognitive functions, pertaining to signal detection, source-monitoring, memory, inhibition and language processes. Yet, empirical results are inconsistent. Despite this, several theoretical models of auditory hallucinations persist, alongside increasing emphasis on the utility of a multidimensional framework. Thus, clarification of current evidence across the broad scope of proposed mechanisms is warranted.Method:A systematic search of the Web of Science, PubMed and Scopus databases was conducted. Records were screened to confirm the use of an objective behavioural cognitive task, and valid measurement of hallucinations specific to the auditory modality.Results:Auditory hallucinations were primarily associated with difficulties in perceptual decision-making (i.e. reduced sensitivity/accuracy for signal-noise discrimination; liberal responding to ambiguity), source-monitoring (i.e. self–other and temporal context confusion), working memory and language function (i.e. reduced verbal fluency). Mixed or limited support was observed for perceptual feature discrimination, imagery vividness/illusion susceptibility, source-monitoring for stimulus form and spatial context, recognition and recall memory, executive functions (e.g. attention, inhibition), emotion processing and language comprehension/hemispheric organisation.Conclusions:Findings were considered within predictive coding and self-monitoring frameworks. Of concern was the portion of studies which – despite offering auditory-hallucination-specific aims and inferences – employed modality-general measures, and/or diagnostic-based contrasts with psychologically healthy individuals. This review highlights disparities within the literature between theoretical conceptualisations of auditory hallucinations and the body of rigorous empirical evidence supporting such inferences. Future cognitive investigations, beyond the schizophrenia-spectrum, which explicitly define and measure the timeframe and sensory modality of hallucinations, are recommended.

Abstract:It is well known and documented that sensory perception decreases with age. In the elderly population, hearing loss and reduced vestibular function are among the most prevalently affected senses. Two important side effects of sensory deprivation are cognitive decline and decrease in social participation. Hearing loss, vestibular function impairment, and cognitive decline all lead to a decrease in social participation. Altogether, these problems have a great impact on the quality of life of the elderly. This is why a rehabilitation program covering all of these aspects would therefore be useful for clinicians. It is well known that long-term music training can lead to cortical plasticity. Behavioral improvements have been measured for cognitive abilities and sensory modalities (auditory, motor, tactile, and visual) in healthy young adults. Based on these findings, it is possible to wonder if this kind of multisensory training would be an interesting therapy to not only improve communication but also help with posture and balance, cognitive abilities, and social participation. The aim of this review is to assess and validate the impact of music therapy in the context of hearing rehabilitation in older adults. Musical therapy seems to have a positive impact on auditory perception, posture and balance, social integration, and cognition. While the benefits seem obvious, the evidence in the literature is scarce. However, there is no reason not to recommend the use of music therapy as an adjunct to audiological rehabilitation in the elderly when possible. Further investigations are needed to conclude on the extent of the benefits that music therapy could bring to older adults. More data are needed to confirm which hearing abilities can be improved based on the many characteristics of hearing loss. There is also a need to provide a clear protocol for clinicians on how this therapy should be administered to offer the greatest possible benefits.

The importance of music for people with dementia: the perspectives of people with dementia, family carers, staff and music therapists

Abstract:OBJECTIVES: Despite the popularity of music-based interventions in dementia care, there is a limited knowledge of how and why people with dementia find music beneficial for their well-being. A qualitative study was conducted to develop further insights into the musical experiences of people with dementia and explore the meaning of music in their lives.METHOD: Separate focus groups and interviews with (1) care home residents with dementia and their families, (2) day hospital clients with dementia, (3) care home staff, and (4) music therapists, were conducted. The findings of the thematic analysis were investigated further in the light of psychosocial factors with the aim of developing a theoretical model on music in dementia.RESULTS: Six key themes were identified. The accessibility of music for people at all stages of dementia, close links between music, personal identity and life events, the importance of relationship-building through music making were particularly highlighted as valuable. The psychosocial model of music in dementia was developed. The model revealed the importance of music to support the personal psychology of people with dementia and the social psychology of the care home environment.CONCLUSION: The effects of music go beyond the reduction of behavioural and psychological symptoms. Individual preference of music is preserved throughout the process of dementia. Sustaining musical and interpersonal connectedness would help value who the person is and maintain the quality of their life.

Developing a music-based digital therapeutic to help manage the neuropsychiatric symptoms of dementia

Abstract:The greying of the world is leading to a rapid acceleration in both the healthcare costs and caregiver burden that are associated with dementia. There is an urgent need to develop new, easily scalable modalities of support. This perspective paper presents the theoretical background, rationale, and development plans for a music-based digital therapeutic to manage the neuropsychiatric symptoms of dementia, particularly agitation and anxiety. We begin by presenting the findings of a survey we conducted with key opinion leaders. The findings highlight the value of a music-based digital therapeutic for treating neuropsychiatric symptoms, particularly agitation and anxiety. We then consider the neural substrates of these neuropsychiatric symptoms before going on to evaluate randomized control trials on the efficacy of music-based interventions in their treatment. Finally, we present our development plans for the adaptation of an existing music-based digital therapeutic that was previously shown to be efficacious in the treatment of adult anxiety symptoms.

Abstract:Abstract Introduction Receptive music listening has been shown to reduce depression, anxiety, and agitation in older adults. However, unfiltered and disruptive noise can increase confusion and agitation. Yet, during hospitalization older patients often have little control over when and for how long they are exposed to music, the genre which is heard, or they may have no access to music. Furthermore, older persons have reduced ability to use modern technology to counter this and their sensory and functional impairments may further isolate them. This study aimed to assess the feasibility of offering two one-hour daily sessions of patient specific music (PSM) choices on an elderly-care ward. Methods On a district general hospital elderly-care ward a 5-day trial of offering two one-hour daily sessions of PSM, using enhanced wireless speakers optimally positioned with daily amended music playlists based on specific patient choice. Ambient noise was minimised with regular decibel monitoring. A post-intervention staff survey was completed to assess the feasibility of continuing, the perceived impact on staff and patients and potential barriers to continuation. Thematic analyses were completed on the survey. Results In the post-intervention feasibility survey (n=14) the majority of staff (86%) agreed that it was possible, implementable and the procedure easy to use. In the impact assessment (n=19) 80% of staff stated it had a positive effect on patients and 89% stated it had a positive effect on staff. Thematic analyses on impact identified benefits to: work, engagement, enjoyment, physical activity and well-being. Barriers that were identified included patient choice, repetition of music, patients unable to engage with the process and staff availability for consistent delivery. Conclusion Playing patient specific music choice was feasible and acceptable to staff with a perceived positive influence on both staff and patients. Further studies are now required to assess the impact on patient outcomes.

Characteristics of non-clinical hallucinations: A mixed-methods analysis of auditory, visual, tactile and olfactory hallucinations in a primary voice-hearing cohort

Abstract:Phenomenological studies involving hallucinations in non-clinical populations have been relatively neglected, especially within the non-auditory realm. Relevant knowledge would help further a more nuanced understanding of the psychosis continuum. Participants (N=33) were non-clinical voice-hearers, who have experienced auditory verbal hallucinations, but with no known mental health diagnosis and not taking any prescribed psychiatric medications. A comprehensive hallucinations phenomenology interview was conducted to assess physical, cognitive and emotional characteristics of hallucinatory experiences across sensory domains. Mixed methods analysis was employed. Characteristics of reported AVHs were mostly in agreement with existing knowledge, though some deviations did exist (e.g. controllability). In addition, 50%, 24% and 29% of our voice-hearing sample experienced hallucinations in the visual, tactile and olfactory domains at least once a week. In contrast, delusions and disordered thinking were rare. Qualitative thematic analysis yielded added phenomenological insights into contextual triggers as well as the content and perceived purpose(s) of multisensory hallucinations. Our findings highlight lesser-reported data that hallucinations in non-auditory domains are relatively frequent in non-clinical voice-hearers. However, other psychotic-like symptoms (i.e. delusions and thought disorder) seem less common. These insights should be considered in the context of the psychosis continuum argument.
An Individualized Music-Based Intervention for Acute Neuropsychiatric Symptoms in Hospitalized Older Adults With Cognitive Impairment: A Prospective, Controlled, Nonrandomized Trial

Abstract:Background: Neuropsychiatric symptoms are common features of dementia, and these occur in three fourths of patients on psychogeriatric inpatient units. These symptoms have traditionally been treated with pharmacological agents, but many medications are as likely to harm patients with dementia as to help them. As a result, nonpharmacological interventions are increasingly being investigated as ways to reduce these symptoms. Objective: The current study evaluated the impact of an individualized music-based intervention on agitation, negative mood, positive mood, compliance with care, need for one-on-one nursing staff intervention, and need for PRN medication. Method: Participants in this study were older adults who were admitted to a geriatric behavioral inpatient unit for acute agitation or behavioral disturbance. Twenty patients were in a treatment as usual group and 21 were in the individualized music group. Results: Agitation, negative mood, and positive mood all benefited from the music-based intervention, with resulting large effect sizes. Resisting care level also significantly benefited from the intervention, with a resulting medium effect size. Conclusion: These findings indicate that an easily implemented and reproducible music-based intervention, which is well tolerated and without adverse side effects, can be an effective way to reduce neuropsychiatric symptoms associated with dementia on a hospital unit.

In addition to posing a substantial burden to disease and negatively impacting quality of life, an emerging literature highlights that HL is associated with unipolar depression including among older adults. This review outlines evidence examining the HL and depression relationship as well as clinical implications for assessment and treatment of comorbid depression and HL. Although prevalence estimates of comorbid depression in HL vary, as many as 1 in 5 experience clinically relevant depression symptoms. Both cross-sectional and longitudinal studies indicate that HL is related to increased unipolar depression symptoms, although the strength of the association varies between studies. A range of methodological variations, such as inclusion age, severity of HL and assessment of depression, likely underpin this heterogeneity. Overall, however, the evidence clearly points to an association of HL with clinically relevant depression symptoms. The association with the diagnosis of major depression disorder remains less clear and under-researched. HL is also associated with a range of other poor mental health outcomes in older adults, including anxiety and suicidal ideation, and predicts poorer cognitive functioning. Accordingly, assessment and treatment of comorbid depression in HL is pertinent to promote mental well-being among older adults. Currently, evidence regarding best practice for treating depression in HL remains scant. Preliminary evidence indicates that audiological rehabilitation, including use of hearing aids, as well as community-based hearing interventions can also improve mental health. Psychological intervention that enhances communication skills and addresses coping strategies might also be beneficial for this population. Additionally, evidence suggests that online interventions are feasible and may circumvent communication difficulties in therapy associated with HL. Due to poor help-seeking among this population, an enhanced focus on specific and targeted assessment and treatment is likely necessary to ensure reduced mental health burden among older adults with HL. Persistent Interictal Musical Hallucination in a Patient With Mesial Temporal Sclerosis-Related Epilepsy: First Case Report and Etiopathological Hypothesis

Abstract:Musical hallucination is a disorder of complex sound processing of instrumental music, songs, choirs, chants, etc. The underlying pathologies include moderate to severe acquired hearing loss (the auditory equivalent of Charles Bonnet syndrome), psychiatric illnesses (depression, schizophrenia), drug intoxication (benzodiazepines, salicylate, pentoxifylline, propranolol), traumatic lesions along the acoustic pathways, and epilepsy. The hallucinations are most likely to begin late in life; 70% of patients are women. Musical hallucination has no known specific therapy. Treating the underlying cause is the most effective approach; neuroleptic and antidepressant medications have only rarely succeeded.Musical hallucination in epilepsy typically presents as simple partial seizures originating in the lateral temporal cortex. To our knowledge, no formal report of musical hallucination in the interictal state has been published before. In contrast, other interictal psychotic features are a relatively common complication, especially in patients with long-standing drug-resistant epilepsy.We describe a 62-year-old woman with a long history of mesial temporal lobe epilepsy whose musical hallucination was solely interictal. We speculate on the possible link between temporal epilepsy and her hallucination. We hypothesize that, as a result of her epileptic activity-induced damage, an imbalance developed between the excitatory and inhibitory projections connecting the mesial temporal cortex to the other auditory structures. These structures may have generated hyperactivity in the lateral temporal cortex through a "release" mechanism that eventually resulted in musical hallucination.

Abstract:Abstract Introduction Sleep-related hallucinations, encompassing visual and auditory experiences pose a complex clinical challenge. Here, we illustrate 2 cases with diverse underlying etiologies. Report of case(s) Case 1: 76-year-old male with 3 months of nocturnal visual hallucinations, occurring 3-5 nights per week. These ranged from familiar and unfamiliar faces and characters reminiscent of movies. Symptoms occurred when waking up in the middle of the night, with difficulty returning to sleep. Polysomnography (PSG) unveiled moderate obstructive sleep apnea (OSA) without findings indicative of parasomnias and no symptoms on the night of PSG. Brain MRI revealed chronic infarcts involving right basal ganglia and bilateral optic radiations. Lesions of structures that commonly cause visual hallucinations (optic lens, retina, thalamus, midbrain) were absent. The frequency of episodes decreased to 1-2 per month after initiating CPAP therapy. Potential mechanism of improvement could be PAP mediated optimization of perfusion to the ischemic penumbra. Case 2: 93-year-old male with an eight-year history of insomnia due to musical hallucinations (MH). Auditory experiences included church and semi-classical music, with 80% being familiar tunes. The patient, a former church choir member, has a history of progressive hearing loss and tinnitus. Brain MRI/MRA revealed chronic periventricular microvascular ischemic changes. Extended EEG showed no evidence of seizures. Audiometry confirmed moderate-severe bilateral sensorineural hearing loss. No response was seen to Levetiracetam, Gabapentin, Donepezil and behavioral therapy. Home sleep study revealed severe OSA. CPAP therapy did not result in improvement of MH. MH improved significantly with Lorazepam. Hypoacusis, psychiatric disorders, focal brain lesions, epilepsy and intoxication are common etiologies for MH. The etiology in this case is very likely due to hypoacusis. The hypothesis is deafferentation resulting in release phenomenon or spontaneous activity in the auditory pathway, manifesting as MH. Conclusion Sleep-related and nocturnal hallucinations warrant a comprehensive evaluation. Differential diagnoses include neurological causes, including stroke, seizure, Multiple Sclerosis, tumor, vascular lesions, and neurodegenerative disorders. Other etiologies include psychiatric disorders, medications, alcohol, otologic and ophthalmic pathologies. It is also reasonable to evaluate for comorbid OSA as appropriate treatment may decrease the frequency of episodes and improve overall sleep in some patients. Support (if any)

Abstract:Background In the last decades, much research has shown that the phenomenon of "Hearing Voices" is not necessarily a symptom of psychiatric disorder but can also occur in the general population without causing serious mental health problems. Less attention has been paid to auditory experiences whose content is not verbal but musical. Although it has been hypothesised that both pathological and non-pathological phenomena, known as "musical hallucinations" and "musical imagery'', respectively, lie on a continuum, the boundary between these phenomena is not clear, nor why musicians experience them in a more positive way than non-musicians.

Abstract:Background Depression in elderly people is an ongoing concern, contributing to comorbidities and increasing deaths from suicide. Alternative treatments for the elderly are needed due to risks from polypharmacy and unwanted side effects. Therefore, we considered music therapy and its evidence base as a possible management modality. Aim This study evaluates how music therapy can treat depression in the elderly population, aiming to further investigate the effects that music can have on health. An analysis of over 20 research papers was carried out, looking into a range of topics varying from the monoamine hypothesis of depression to how music can affect multiple comorbidities. Method A search using accredited platforms, such as PubMed and OVID, was carried out into recent and robust research in this area, to provide a systematic review of the literature. Results The key papers showed that music was found to have a positive effect on mental aspects of health, as described by a number of psychological theories. Current pharmacological treatments for depression were often not compatible with elderly patients due to unwanted side effects and potentially dangerous results from polypharmacy; therefore, music therapy offered a safer alternative. The results also showed that music therapy can be more favourable than psychological therapy, such as CBT, due to its lower energy and concentration requirements. Conclusion Depression in the elderly is a serious illness that can lead to reduced quality of life and poor health status. Non-pharmacological treatments need to be identified and music therapy can be an accessible way of doing this, with numerous benefits demonstrated.

Abstract:Across the globe, researchers, clinicians, and caregivers are continuing to search for solutions to the myriad problems caused by Alzheimer’s disease and related dementias (ADRDs). Music is an area of focus receiving considerable attention for its potential benefits to help manage the effects of neuropsychiatric symptoms upon individuals living with ADRDs. However, attention is lacking on the potential social value of music to positively influence quality of life for both individuals living with dementias and their surrounding communities. This scoping review compiles recent reports on this potential, highlighting the social nature of music and its various uses (e.g. listening, making) to change the often tense and reactive atmosphere surrounding both the individuals living with ADRDs and their surrounding communities into comparatively joyous living environments. Databases used included PubMed, EBSCOhost’s CINAHL and PsychINFO, Cochrane Library (sub‐search conducted using ALOIS, the Specialized Register of the Cochrane Dementia and Cognitive Improvement Group (CDCIG)), ProQuest’s Biological Science Collection, the Journal of Music Therapy and Nordic Journal of Music Therapy, and Google Scholar. After screening for eligibility and inclusion, 26 documents were identified, including review articles, experimental studies, recommendations, pilot studies, conference proceedings, book chapters, and executive summaries. Overall, this review concludes that music can facilitate prosocial engagement between persons living with ADRDs and their surrounding communities, due to a complex interplay of neurobiological mechanisms largely preserved in the ADRD population. More well designed and well controlled empirical studies are necessary to clarify contexts in which music and its various uses may be helpful to improve quality of life for both the individuals suffering from ADRDs and their surrounding communities. Such results would provide further evidence‐based support for the use of music to strengthen both formal and informal patient‐centered care models.
