Abstract:Background: The relationship between online health communities (OHCs) and offline medical care is unclear because both provide physician-patient interaction services and channels. Taking advantage of information and communication technology, patients have been using OHCs widely. However, some physical medical resources (such as hospital beds and medical devices) cannot be replicated by information and communication technologies. Therefore, it is worth studying how offline medical resources affect physician-patient interactions in OHCs and how OHCs help to solve resource scarcity and the uneven distribution of traditional medical treatment. Objective: This study aimed to support the notion that physician-patient consultations in OHCs are influenced by the objective distribution of offline health care capital (accessibility and availability) and to provide suggestions for the allocation of medical resources in practice through the judicious use of offline and online channels. Methods: The empirical data in this study were collected from both online and offline channels. The offline data include 9 years (2006-2014) of medical resource statistics of 31 provincial administrative regions in mainland China. Moreover, data regarding the geolocation-based physician-patient interaction network in the OHC were also collected. The online data come from one of China's largest OHCs. We obtained 92,492 telephone consultation records of 6006 physicians using an automatic web crawler program. Social network analysis was used to visualize the descriptive statistics of the offline geolocation-based physician-patient interaction network in the OHC. A regression model with a squared variable was applied to analyze online and offline empirical data to further test our hypothesis. Two types of robustness tests were used to increase the reliability of the test results of the initial model. Results: The results of our social network analysis show that there is a uniform geographic distribution of patients who use OHCs, whereas the physician relies more on geographic advantage (eg, a higher medical resource capability). Moreover, the empirical results of the regression model support the notion that physician-patient telephone consultations are positively influenced by physicians' online contributions (β contribution =.210; P <.001) and capital availability (β bed =.935; P =.07), and, interestingly, spatial accessibility has an inverted U–shaped effect (β distance =.199; P <.001 and β distance 2 =–.00449; P =.008). The results indicate that the use of OHCs, although constrained by offline medical resources, provides a channel for offline resources to flow from areas with high availability to those with low availability. Conclusions: This study explores the relationship between online and offline channels by investigating online physician-patient interactions and offline medical resources. In particular, this study analyzes the impact of offline channels on online channels and verifies the possibility of OHC capital use shifting from a high-availability area to a low-availability area. In addition, it provides a theoretical and practical basis for understanding the interaction of online and offline channels of medical care.

Abstract:Background: Internet medical treatment, also known as telemedicine, represents a paradigm shift in health care delivery. This contactless model allows patients to seek medical advice remotely, often before they physically visit a doctor's clinic. Herein, physicians are in a relatively passive position, as patients browse and choose their health care providers. Although a wealth of experience is undoubtedly a draw for many patients, it remains unclear which specific facets of a doctor's credentials and accomplishments patients prioritize during their selection process. Objective: Our primary aim is to delve deeper into the correlation between physicians' static characteristics-such as their qualifications, experiences, and profiles on the internet-and the number of patient visits they receive. We seek to achieve this by analyzing comprehensive internet hospital data from public hospitals. Furthermore, we aim to offer insights into how doctors can present themselves more effectively on web-based platforms, thereby attracting more patients and improving overall patient satisfaction. Methods: We retrospectively gathered web-based diagnosis and treatment data from the First Affiliated Hospital of Guangxi Medical University in 2023. These data underwent rigorous analysis, encompassing basic descriptive statistics, correlation analyses between key factors in doctors' internet-based introductions, and the number of patient consultation visits. Additionally, we conducted subgroup analyses to ascertain the independence of these vital factors. To further distill the essence from these data, we used nonnegative matrix factorization to identify crucial demographic characteristics that significantly impact patient choice. Results: The statistical results suggested that there were significant differences in the distribution of consultation volume (P<.001), and the correlation analysis results suggested that there was a strong correlation between the two groups of data (ρ=0.93; P<.001). There was a correlation between the richness of a profile and popularity (P<.001). Patients were more interested in physicians with advanced titles, doctoral degrees, social activities, and scientific achievements (P<.001) as well as other institutional visit experiences (P=.003). More prosperous social activities, scientific achievements, experiences of other institutional visits, and awards were more common among people with advanced professional titles. Doctoral degrees remained attractive to patients when data were limited to senior physicians (P<.001). Patients trusted the medical staff with advanced titles, social activities, scientific achievements, and doctoral degrees (P<.001). Conclusions: Patient preferences for choosing a health care provider differed significantly between free and paid consultations. Notably, patients tended to trust doctors with advanced professional titles more and were more likely to seek out those with doctoral qualifications over other professional ranks. Additionally, physicians who actively participated in social events and scientific endeavors often had an advantage in attracting new patients. Given these insights, doctors who invest in enhancing their personal and professional experiences within these domains are likely to see increased popularity and patient satisfaction.

Abstract:Background: Web-based medical services have become an effective supplement to traditional services in hospitals and an essential part of medical services. Studies have shown that web-based medical services are useful for shortening the delayed admission time and for enhancing the treatment effect from the service continuity perspective. However, the specific measures that patients and physicians should take to improve service continuity remain unknown. Objective: Based on the information richness theory and continuity of care, this study investigates the dynamic effects of information continuity and interpersonal continuity on physician services online. Methods: Data of 7200 patients with 360 physicians covering complete interaction records were collected from a professional web-based platform in China. Content analysis was performed to recognize matching patients and physicians, and least square regression analysis was performed to obtain all empirical results. Results: Empirical results showed that in the short term, information continuity (including offline experience, medical records, and detailed information) influences physicians' web-based services, and their influences show heterogeneity. Moreover, if a patient's online physician is the same physician who he/she has visited offline, we find that interpersonal continuity is important for service. In the long term, information continuity and interpersonal continuity positively improve service continuity by facilitating repeat purchases. Conclusions: Overall, our findings not only shed new light on patient behavior online and cross-channel behavior but also provide practical insights into improving the continuity of care in online health communities.

The effect of interactive factors on online health consultation review deviation: An empirical investigation

Abstract:Background: One-to-one online consultation is a common type of online health consultation. In choosing doctors for these consultations, patients rely on online reviews. Yet the deviation between online doctor reviews and the true quality of doctor-provided online services calls the usefulness of online doctor reviews into question, and the methods for reducing this deviation via doctor-patient communication remain unclear. Objectives: The purpose of this study is to test the effects of interactive factors on online doctor review deviation and to further explore deviation across doctor specialties in the context of one-to-one online health consultations. Methods: We collect our data from a well-known Chinese online health consultation platform. The dataset includes 60,693 one-to-one online health consultation communication flows and corresponding online doctor reviews. We construct an online doctor review deviation matrix and use logistic regression and multinomial logistic regression models to examine the effects of interactive factors on online doctor review deviation. Results: Our findings indicate that, in the context of a one-to-one online health consultation, a quicker response time and a lower response-question ratio could reduce deviation in online doctor reviews. Single modalities, such as the use of voice messages and uploading of photos, could reduce online doctor review deviation, especially in terms of patient overestimation. Medical information, including structural medical history and prescription information, could decrease online doctor review deviation. Moreover, the use of voice messages in surgery patient treatment can reduce online doctor review deviation more than in internal medicine. Conclusion: Interaction frequency, message delivery methods, and medical information can influence the deviation of online doctor reviews. Furthermore, the effects of voice messages vary across doctor specialties. This study offers theoretical and practical implications for the design of online health consultation platforms and the usage of online doctor reviews.
Provision of Paid Web-Based Medical Consultation in China: Cross-Sectional Analysis of Data From a Medical Consultation Website

Abstract:BACKGROUND: Web-based medical consultation, which has been adopted by patients in many countries, requires a large number of doctors to provide services. However, no study has provided an overall picture of the doctors who provide online services.OBJECTIVE: This study sought to examine doctors' participation in medical consultation practice in an online consultation platform. This paper aimed to address the following questions: (1) which doctors provide Web-based consultation services, (2) how many patients do the doctors get online, and (3) what price do they charge. We further explored the development of market segments in various departments and various provinces.METHODS: This study explored the dataset including all doctors providing consultation services in their spare time on a Chinese Web-based consultation platform. We also brought in statistics for doctors providing offline consultations in China. We made use of Bonferroni multiple comparison procedures and z test to compare doctors in each group.RESULTS: There are 88,308 doctors providing Web-based consultation in their spare time on Haodf, accounting for 5.25% (88,308/1,680,062) of all doctors in China as of September 23, 2017. Of these online doctors, 49.9% (44,066/88,308) are high-quality doctors having a title of chief physician or associate chief physician, and 84.8% (74,899/88,308) come from the top, level 3, hospitals. Online doctors had an average workload of 0.38 patients per doctor per day, with an online and offline ratio of 1:14. The average price of online consultation is ¥32.3. The online ratios for the cancer, internal medicine, ophthalmology-otorhinolaryngology, psychiatry, gynecology-obstetrics-pediatrics, dermatology, surgery, and traditional Chinese medicine departments are 16.1% (2,983/18,481), 4.4% (16,231/372,974), 6.3% (8,389/132,725), 9.5% (1,600/16,801), 5.8% (12,955/225,128), 18.0% (3,334/18,481), 10.8% (24,030/223,448), and 3.8% (8,393/22,3448), respectively. Most provinces located in eastern China have more than 4000 doctors online. The online workloads for doctors from most provinces range from 0.3 to 0.4 patients per doctor per day. In most provinces, doctors' charges range from ¥20 to ¥30.CONCLUSIONS: Quality doctors are more likely to provide Web-based consultation services, get more patients, and charge higher service fees in an online consultation platform. Policies and promotions could attract more doctors to provide Web-based consultation. The online submarket for the departments of dermatology, psychiatry, and gynecology-obstetrics-pediatrics developed better than other departments such as internal medicine and traditional Chinese medicine. The result could be a reference for policy making to improve the medical system both online and offline. As all the data used for analysis were from a single website, the data might be biased and might not be a representative national sample of China.

Evaluation of methods to improve detection of Escherichia coli O157:H7 in fresh produce by multiplex polymerase chain reaction.

Abstract:Multiplex polymerase chain reaction (PCR) analysis was used to detect two genes encoding Shiga-like toxins (stx1 and stx2) and a universal Escherichia coli gene (gadA/B) in fresh produce spiked with E. coli O157:H7. Current U.S. Food and Drug Administration procedures for the analysis of fresh produce include the use of the rinsate from an initial rinse for the analysis of several potential pathogens, including E. coli O157:H7. In this study, several procedures were evaluated for their ability to increase the sensitivity of PCR analysis of rinsates from 15 types of produce. The procedures evaluated included the preliminary clarification and concentration of templates by centrifugation and the treatment of templates with compounds reported to facilitate nucleic acid amplification, including polyvinlypolypyrrolidone (PVPP), nonfat dry milk (NFDM), and InstaGene. The preliminary concentration of rinsates resulted in moderate improvements in detection sensitivity. The use of PVPP-treated templates in PCR reaction mixtures did not further improve sensitivity, but the inclusion of NFDM-treated templates increased sensitivity by an order of magnitude for 12 rinsates. The incorporation of InstaGene also improved the detection capability of the analysis; this procedure yielded the strongest gel bands for eight rinsates. However, for four other rinsates, the use of this reagent decreased sensitivity; these four rinsates were those for the produce varieties with the largest surface areas and were the most turbid rinsates. The use of facilitating compounds to block PCR inhibition may enable an analysis for Shiga toxin-producing E. coli in fresh produce to be completed in 1 to 2 days, rather than the 5 days required for current methods.
Patient Activeness During Online Medical Consultation in China: Multilevel Analysis

Abstract:Background: Online medical consultation is an important complementary approach to offline health care services. It not only increases patients' accessibility to medical care, but also encourages patients to actively participate in consultation, which can result in higher shared decision making, patient satisfaction, and treatment adherence. Objective: This study aims to explore multilevel factors that influence patient activeness in online medical consultations. Methods: A data set comprising 40,505 patients from 300 physicians in 10 specialties was included for multilevel analysis. Patient activeness score (PAS) was calculated based on the frequency and the proportion of patient discourses to the total frequency of doctor-patient interactions. Intraclass correlation coefficients were calculated to identify between-group variations, and the final multilevel regression model included patient- and physician-level factors. Results: Patients were not equally active in online medical consultations, with PASs varying from 0 to 125.73. Patient characteristics, consultation behavioral attributes, and physician professional characteristics constitute 3 dimensions that are associated with patient activeness. Specifically, young and female patients participated more actively. Patients' waiting times online (beta=-.17; P<.001) for physician responses were negatively correlated with activeness, whereas patients' initiation of conversation (beta=.83; P<.001) and patient consultation cost (beta=.52; P<.001) in online medical consultation were positively correlated. Physicians' online consultation volumes (beta=-.10; P=.01) were negatively associated with patient activeness, whereas physician online consultation fee (beta=.03; P=.01) was positively associated. The interaction effects between patient- and physician-level factors were also identified. Conclusions: Patient activeness in online medical consultation requires more scholarly attention. Patient activeness is likely to be enhanced by reducing patients' waiting times and encouraging patients' initiation of conversation in online medical consultation. The findings have practical implications for patient-centered care and the improvement of online medical consultation services.
Exploring the Role of Online Health Community Information in Patients' Decisions to Switch from Online to Offline Medical Services

Abstract:Background: Online healthcare has become an important part of healthcare services. Patients can access unlimited information reflecting a doctor's service quality in online health communities (OHCs). This information can reduce information asymmetry between patients and doctors, and further promote patient decisions. In this study, we focus on the patient's decision to switch from a doctor's online medical service to the same doctor's offline service. Objectives: We classify OHC information into objective information and subjective information. Following this, we investigate how these two types of information influence patient decisions to switch from a doctor's online to offline medical service, and explore the nature of interaction between the two types of influence. Methods: We collected data from a leading OHC in China and constructed a longitudinal dataset to examine our research questions. To control for doctor heterogeneity and common trends across time, we leveraged fixed effects at the doctor level and monthly fixed effects in our models, respectively. Results: Our findings demonstrate a strong relationship between OHC information and patient decisions to switch from a doctor's online to offline medical service. Patients are more likely to switch from online to offline medical services with doctors whose subjective and objective information on their homepages indicate a higher quality (βSubjective = 0.001, p < 0.001; αObjective = 0.019, p < 0.002). We also observe that patients prefer doctors who do not exhibit objective information that indicates low-quality service over those who show such information on their homepages, even though the chosen doctor provides a lower-quality service (αLow_Objective = -0.032, p < 0.006). Further, we find that the influence of subjective information on patient decisions crowds out the influence of objective information. Conclusion: This study investigates the role of subjective and objective information on patient decisions to switch from a doctor's online to offline medical service and the interaction effect between these two types of information. This study provides valuable theoretical and practical implications.

Abstract:Background Online health care services are a possible solution to alleviate the lack of medical resources in rural areas, and further understanding of the related medical service pricing system would contribute to improvement of the online health care community (OHC). Although many studies have investigated the OHC, the impact of physicians’ reputations and wage levels on consulting prices in the OHC has rarely been discussed in the literature. Objective This study was designed to explore the determinants of consulting prices in the OHC. We addressed the following questions: (1) Are the prices of online health consultation services affected by wage levels at the doctor’s location? (2) How does a physician’s online and offline reputation affect their consulting prices? Methods Employing a large-scale sample of 16,008 doctors in China, we first used descriptive statistics to investigate the determinants of consulting prices in their entirety. Hierarchical linear modeling was then used to investigate the determinants of consulting prices in the OHC. Results The empirical results led to the conclusion that if doctors have more elevated clinic titles, work in higher-level hospitals, have better online reputations, and/or have made more past sales, their consulting prices will be higher. Additionally, the wage level in the city in which the doctor is working determines their opportunity cost and therefore also affects consulting prices. Conclusions The findings indicate that the characteristics of the doctor, the doctor’s online reputation, and past sales affect the consulting price. In particular, the wage level in the city affects the price of the consultation. These findings highlight that the OHC is important because it can indeed break through geographical restrictions and give rural residents the opportunity to obtain medical service from doctors in big cities. However, doctors from cities often charge higher fees because of their higher opportunity cost. The results reveal that one of the most important functions of the OHC is to reduce the medical disparity between urban and rural areas; however, planners appear to ignore the possibility that rural residents with lower incomes may not be able to afford such high medical consultation costs. Therefore, the government should consider providing incentives to encourage urban doctors to provide discounts to rural residents or directly offer appropriate subsidies.

Abstract:Abstract Background Online doctor–patient consultation is a new option for orthopedic patients in China to obtain a diagnosis and treatment advice. This study explores the factors associated with online consultation to formulate operational guidelines for managing online consultations in an online medical community (OMC). Methods An empirical model was developed to identify the factors that influence online orthopedic doctor–patient consultations in an OMC while focusing on the perceived value of and perceived trust in online consultations. The moderating effects of different risk categories of orthopedic diseases were also considered. Data from 339 feedback surveys from orthopedic patients who used online consultation services and Stata software version 14.0 were used to estimate the model parameters and test the robustness of the empirical model. Results Of those who completed the feedback surveys, 53.42% were female patients, 82.27% were between 18 and 60 years old, and 61.98% sought consultations online more than 2 times per year. Model analysis demonstrated that the regression coefficients of the perceived value of and perceived trust in online consultations are 0.489 ( p < 0.01) and 0.505 ( p < 0.01), respectively. The interaction coefficient between disease risk and perceived value is 0.336 ( p < 0.01), and the interaction coefficient between disease risk and perceived trust is − 0.389 ( p < 0.01). Conclusions Orthopedic patients’ perceived value of and perceived trust in online consultations in an OMC can significantly influence their intention to seek online disease diagnosis and treatment consultations. The effects of perceived value and perceived trust on patients' intention to consult vary significantly across different disease risk categories. Therefore, enhancing the perceived value and perceived trust of orthopedic patients is an important component of OMC operation and management.

Abstract:Background: E-consultation is expected to improve the information level of patients, affect patients' subsequent judgments of medical services, and guide patients to make a reasonable medical selection in the future. Thus, it is important to understand the influence mechanism of e-consultation on patients' medical selection. Objective: This study aims to explore the changes in first-visit patients' understanding of disease and medical resources after e-consultation as well as the choice of follow-up medical services. Methods: Patients' medical selection before and after e-consultation was compared using a scenario survey. Based on the service characteristics of the e-consultation platform, representative simulation scenarios were determined, and parallel control groups were set up considering the order effect in comparison. Finally, a total of 4 scenario simulation questionnaires were designed. A total of 4164 valid questionnaires were collected through the online questionnaire collection platform. Patients' perception of disease severity, evaluation of treatment capacity of medical institutions, selection of hospitals and doctors, and other outcome indicators were tested to analyze the differences in patients' evaluation and choice of medical services before and after e-consultation. Additionally, the results' stability was tested by regression analysis. Results: In scenario 1 (mild case), before e-consultation, 14.1% (104/740) of participants considered their conditions as not serious. After e-consultation, 69.5% (539/775) of them considered their diseases as not serious. Furthermore, participants' evaluation of the disease treatment capacity of medical institutions at all levels had improved after using e-consultation. In scenario 3 (severe case), before e-consultation, 54.1% (494/913) of the participants believed their diseases were very serious. After e-consultation, 16.6% (157/945) considered their diseases were very serious. The evaluation of disease treatment capacity of medical institutions in nontertiary hospitals decreased, whereas that of tertiary hospitals improved. In both mild and severe cases, before e-consultation, all of the participants were inclined to directly visit the hospital. After e-consultation, more than 71.4% (553/775) of the patients with mild diseases chose self-treatment, whereas those with severe diseases still opted for a face-to-face consultation. After e-consultation, patients who were set on being treated in a hospital, regardless of the disease severity, preferred to select the tertiary hospitals. Of the patients with mild diseases who chose to go to a hospital, 25.7% (57/222) wanted to consult online doctors face-to-face. By contrast, 56.4% (506/897) of the severe cases wanted to consult online doctors face-to-face. Conclusions: E-consultation can help patients accurately enhance their awareness of the disease and guide them to make a more reasonable medical selection. However, it is likely that e-consultation makes online medical services centralized. Additionally, the guiding effect of e-consultation is limited, and e-consultation needs to be combined with other supporting systems conducive to medical selection to play an improved role.

Abstract:BACKGROUND:As a new medical service model, the online health community can integrate various medical resources to the maximum extent and improve the accessibility and utilization rate of hospital high-quality medical resources.OBJECTIVE:Research based on the characteristics of doctors can enable doctors to display themselves on the network platform better, provide better services for patients, and improve the quality of medical services for doctors.METHOD:By crawling the characteristic data of doctors in Good Doctor Online, using dynamic analysis, correlation analysis and regression analysis, this study explores the relationships between each characteristic data and online consultation volume.RESULTS:The doctor's title and city level representing the static characteristics of the doctor have a weak impact on the doctor's online consultation volume, and the doctor's dynamic characteristics such as the number of patient completions, the number of gifts received, and the number of published articles can have a positive impact on the doctor's online consultation volume. However, the recommended heat will negatively affect the online consultation volume, and the comment text has no significant impact on the doctor's online consultation volume.CONCLUSION:Therefore, doctors should actively publicize and show their professional level and constantly optimize their dynamic characteristics, increasing the number of online consultations and thus improving their influence.

Abstract:Online medical consultation is a form of medical service that facilitates interactions between patients and doctors online, offering significant utility and value. This review aims to retrieve, screen, and analyze articles related to online medical consultations, formulating a theoretical framework and proposing future research directions. According to PRISMA guidelines, a systematic search was conducted in Web of Science, EBSCO, ScienceDirect, PubMed, and Scopus, retrieving a total of 4072 English records on 16 December 2023. After rigorous screening, 75 articles were included in this review. Among these, 8 articles focused on patients utilizing online medical consultation platforms, 5 on doctors participating in online medical platforms, 18 on patients' choice of doctors, 12 on doctors providing services, 7 on online reviews of patients, 14 on service quality for patients, 8 on rewards to doctors, and 11 on the spillover effect between online and offline services. These themes comprise the theoretical framework of the starting point, process, and outcomes of the online medical consultation system, providing a comprehensive understanding of the field and a foundation for future research.

Patient Activeness During Online Medical Consultation in China: Multilevel Analysis (Preprint)

Abstract:BACKGROUND Online medical consultation is an important complementary approach to offline health care services. It not only increases patients’ accessibility to medical care, but also encourages patients to actively participate in consultation, which can result in higher shared decision making, patient satisfaction, and treatment adherence. OBJECTIVE This study aims to explore multilevel factors that influence patient activeness in online medical consultations. METHODS A data set comprising 40,505 patients from 300 physicians in 10 specialties was included for multilevel analysis. Patient activeness score (PAS) was calculated based on the frequency and the proportion of patient discourses to the total frequency of doctor-patient interactions. Intraclass correlation coefficients were calculated to identify between-group variations, and the final multilevel regression model included patient- and physician-level factors. RESULTS Patients were not equally active in online medical consultations, with PASs varying from 0 to 125.73. Patient characteristics, consultation behavioral attributes, and physician professional characteristics constitute 3 dimensions that are associated with patient activeness. Specifically, young and female patients participated more actively. Patients’ waiting times online (β=–.17; P<.001) for physician responses were negatively correlated with activeness, whereas patients’ initiation of conversation (β=.83; P<.001) and patient consultation cost (β=.52; P<.001) in online medical consultation were positively correlated. Physicians’ online consultation volumes (β=–.10; P=.01) were negatively associated with patient activeness, whereas physician online consultation fee (β=.03; P=.01) was positively associated. The interaction effects between patient- and physician-level factors were also identified. CONCLUSIONS Patient activeness in online medical consultation requires more scholarly attention. Patient activeness is likely to be enhanced by reducing patients’ waiting times and encouraging patients’ initiation of conversation in online medical consultation. The findings have practical implications for patient-centered care and the improvement of online medical consultation services.

Abstract:An emerging service model in online health communities (OHCs) is that of medical teams comprising multiple physicians who collaborate to offer diagnoses and recommendations to patients. Given its multiple information sources, this model has the potential to deliver high-quality services and enhance patient satisfaction. However, the effect of a wider range of information on patient satisfaction has yet to be empirically examined. Therefore, the current research aims to examine the effect of multiple sources of health-related information on the satisfaction of patients in OHCs. We construct a sample model and empirically test it using a dataset comprising 115,367 consultation records sourced from WeDoctor. The results show that responses from multiple physicians in OHC medical teams increase patient satisfaction. In addition, we explore the moderating effects of team composition and team replies. The results show that physicians with higher titles and affiliations with the same department and the same question's replies from multiple physicians all play a positive moderating role, while reply time plays a negative moderating role. This research enriches the existing literature by focusing on patient satisfaction in the context of OHCs and offers recommendations for research and practice.

Online medical consultation in China: Evidence from obesity doctors

Abstract:Objective: Online medical consultation (OMC) is increasingly used in China, but there have been few in-depth studies of consultation arrangements and fee structures of online doctors in China. This research assessed the consultation arrangements and fee structure of OMC in China by undertaking a case study of obesity doctors from four representative OMC platforms. Methods: Detailed information, including fees, waiting time and doctor information, was collected from four obesity OMC platforms and analyzed using descriptive statistical analysis. Results: The obesity OMC platforms in China shared similarities in the use of big data and artificial intelligence (AI) but differed across service access, specific consultation arrangements and fees. Big data search and AI response technologies were used by most platforms to match users with doctors and reduce doctors' pressure. The descriptive statistical analysis showed that the higher the rank of the online doctor, the higher the online fee and the longer the wait time. Through a comparison with offline hospitals, we found online doctors' fees exceeded offline hospital doctors' fees by up to 90%. Conclusions: OMC platforms can gain competitive advantages over offline medical institutions through the following measures: make fuller use of big data and AI technologies to provide users with longer duration, lower cost and more efficient consultation services; provide better user experience than offline medical institutions; use big data and fee advantages to screen doctors to match users' consultation needs instead of screening by the rank of doctors only; and cooperate with commercial insurance providers to provide innovative health care packages.

Abstract:Abstract Recent years have seen robust growth in online medical consultation platforms. These platforms allow patients to access various healthcare services provided by doctors (e.g., health assessment, diagnosis, consultation, and supervision). In China, many such platforms allow patients to give small monetary gifts to doctors as an expression of gratitude. The implicit assumption is that expensive gifts influence doctors' medical service and generate conflicts of interest but small gifts do not. However, there is little empirical evidence to support this assumption. In order to fill this gap in the literature, our study investigates whether small gifts from patients impact the quality of service provided to the gift‐givers (i.e., direct effect) and the nongivers (i.e., spillover effect). We examine three aspects of online medical service quality: (i) patient wait time, (ii) the amount of information in doctors' responses, and (iii) the degree of emotional support in doctors' responses. We find that despite the gifts' negligible monetary value, doctors who receive gifts do reciprocate to the gift‐givers by providing them with more timely responses and greater emotional support. Furthermore, after receiving the small gifts, doctors may be slower in responding to nongivers and offer them less emotional support. We also investigate whether these effects (both direct and spillover effects) vary with doctors' backgrounds, including their professional experience and geographic location. Our findings have both theoretical and practical implications for patients, online medical consultation platforms, and healthcare policy makers.
