Abstract:Tumor angiogenesis is considered an important prognostic factor. With an increasing emphasis on imaging evaluation of the tumor microenvironment, dynamic contrast enhanced-computed tomography (DCE-CT) has evolved as an important functional technique in this setting. Yet many questions remain as to how and when these functional measurements should be performed for each agent and tumor type, and what quantitative models should be used in the fitting process. In this study, we evaluated the variations of perfusion measurement on DCE-CT for rectal cancer patients from (1) different tracer kinetic models, (2) different scan acquisition lengths, and (3) different scan intervals. A total of seven commonly used models were studied: the adiabatic approximation to the tissue homogeneity (AATH) model, adiabatic approximation to the homogeneity tissue with fixed transit time (AATHFT) model, the Tofts model (TM), the extended Tofts model (ETM), Patlak model, Logan model, and the model-free deconvolution method. Akaike's information criterion was used to identify the best fitting model. The interchangeability of different models was further evaluated using Bland-Altman analysis. All models gave comparable blood volume (BV) measurements except the Patlak method. While for the volume transfer constant (K-trans) estimation, AATHFT, AATH, and ETM generated reasonable agreement among each other but not for the other models. Regarding the blood flow (BF) measurement, no two models were interchangeable. In addition, the perfusion parameters were compared with four acquisition times (45, 65,85, and 105s) and four temporal intervals (1, 2, 3, and 4s). No significant difference was observed in the volume transfer constant (K-trans), BV, and BF measurements when comparing data acquired over 65s with data acquired over 105s using any of the DCE models in this study. Yet increasing the temporal interval led to a significant overestimation of BF in the deconvolution method. In conclusion, the perfusion measurement is indeed model dependent and the image acquisition/processing technique is dependent. The radiation dose of DCE-CT was an average of 1.5-2 times an abdomen/pelvic CT, which is not insubstantial. To take the DCE-CT forward as a biomarker in oncology, prospective studies should be carefully designed with the optimal image acquisition and analysis technique.
Factors affecting the deceased organ donation rate in the Chinese community: an audit of hospital medical records in Hong Kong.

Abstract:INTRODUCTION The number of actual donors per million population is the most commonly used metric to measure organ donation rates worldwide. It is deemed inadequate, however, because it does not take into account the potential donor pool. The aim of this study was to determine the true potential for solid organ donation from deceased brain-dead donors and the reasons for non-donation from potential donors in the Chinese community. METHODS Medical records of all hospital deaths between 1 January and 31 December 2014 at a large regional hospital in Hong Kong were reviewed. Those who were on mechanical ventilation with documented brain injury and aged ≤75 years were classified as possible organ donors. The reasons why some potential organ donors did not become utilised organ donors were recorded and evaluated. RESULTS Among 3659 patient deaths, 121 were classified as possible organ donors. The mean age of the possible organ donors was 59.4 years and 72.7% of them were male. The majority (88%) were from non-intensive care units. Of the 121 possible organ donors, 108 were classified as potential organ donors after excluding 13 unlikely to fulfil brain death criteria. Finally 11 patients became actual organ donors with an overall conversion rate of 10%. Reasons for non-donation included medical contra-indication (46%), failure to identify and inform organ donation coordinators (14%), failure of donor maintenance (11%), brain death diagnosis not established (18%), and refusal by relatives (11%). CONCLUSIONS It is possible to increase the organ donation rate considerably by action at different stages of the donation process. Ongoing accurate audit of current practice is necessary.
Patient-Specific Characterization of Breast Cancer Hemodynamics Using Image-Guided Computational Fluid Dynamics

Abstract:The overall goal of this study is to employ quantitative magnetic resonance imaging (MRI) data to constrain a patient-specific, computational fluid dynamics (CFD) model of blood flow and interstitial transport in breast cancer. We develop image processing methodologies to generate tumor-related vasculature-interstitium geometry and realistic material properties, using dynamic contrast enhanced MRI (DCE-MRI) and diffusion weighted MRI (DW-MRI) data. These data are used to constrain CFD simulations for determining the tumor-associated blood supply and interstitial transport characteristics unique to each patient. We then perform a proof-of-principle statistical comparison between these hemodynamic characteristics in 11 malignant and 5 benign lesions from 12 patients. Significant differences between groups (i.e., malignant versus benign) were observed for the median of tumor-associated interstitial flow velocity ( P = 0.028 ), and the ranges of tumor-associated blood pressure (P = 0.016) and vascular extraction rate (P = 0.040). The implication is that malignant lesions tend to have larger magnitude of interstitial flow velocity, and higher heterogeneity in blood pressure and vascular extraction rate. Multivariable logistic models based on combinations of these hemodynamic data achieved excellent differentiation between malignant and benign lesions with an area under the receiver operator characteristic curve of 1.0, sensitivity of 1.0, and specificity of 1.0. This image-based model system is a fundamentally new way to map flow and pressure fields related to breast tumors using only non-invasive, clinically available imaging data and established laws of fluid mechanics. Furthermore, the results provide preliminary evidence for this methodology's utility for the quantitative characterization of breast cancer.
An Optimal Mass Transport Model for the Analysis of DCE-MRI and its Application to Breast Cancer Treatment Response

Abstract:Purpose: Dynamic contrast-enhanced MR imaging (DCE-MRI) is widely deployed in cancer care and research, but the methods conventionally used to quantify contrast agent kinetics do not account the cross-voxel movement characterized by advection and diffusion. We hypothesized that unbalanced optimal mass transport could be used to quantify and visualize such contrast agent flows across tumor volumes. Methods: We developed a computational fluid dynamics model termed the unbalanced regularized optimal mass transport (urOMT) model. We tested the urOMT on a multi-institutional dataset of 153 longitudinal DCE-MRI scans from 39 breast cancer patients treated with neoadjuvant chemotherapy (NACT.) Results: The urOMT model can quantify dynamic fluid transport properties such as net speed, flux and rates of contrast entering and leaving the tumor (influx and efflux). The urOMT model can also visualize the trajectories and directions of net fluid flows. Quantitative metrics from urOMT exhibited distinct patterns that may be relevant to predicting pathological complete response (pCR) to NACT. Conclusion: The urOMT model can be used to estimate and visualize local fluid flow in DCE-MRI breast cancer images. Model-based estimates of flux, influx and efflux should be tested as potential predictive imaging biomarkers to measure treatment effectiveness in patients treated with NACT. The urOMT model in principle has applicability to other cancer imaging use cases, but this will require further testing.

Abstract:Purpose: Technical developments in both magnetic resonance imaging (MRI) and computed tomography (CT) have helped to reduce scan times and expedited the development of dynamic contrast-enhanced (DCE) imaging techniques. Since the temporal change of the image signal following the administration of a diffusible, extracellular contrast agent (CA) is related to the local blood supply and the extravasation of the CA into the interstitial space, DCE imaging can be used to assess tissue microvasculature and microcirculation. It is the aim of this review to summarize the biophysical and tracer kinetic principles underlying this emerging imaging technique offering great potential for non-invasive characterization of tumour angiogenesis. Methods: In the first part, the relevant contrast mechanisms are presented that form the basis to relate signal variations measured by serial CT and MRI to local tissue concentrations of the administered CA. In the second part, the concepts most widely used for tracer kinetic modelling of concentration-time courses derived from measured DCE image data sets are described in a consistent and unified manner to highlight their particular structure and assumptions as well as the relationships among them. Finally, the concepts presented are exemplified by the analysis of representative DCE data as well as discussed with respect to present and future applications in cancer diagnosis and therapy. Results: Depending on the specific protocol used for the acquisition of DCE image data and the particular model applied for tracer kinetic analysis of the derived concentration-time courses, different aspects of tumour angiogenesis can be quantified in terms of well-defined physiological tissue parameters. Conclusions: DCE imaging offers promising prospects for improved tumour diagnosis, individualization of cancer treatment as well as the evaluation of novel therapeutic concepts in preclinical and early-stage clinical trials.

Abstract:Background: Magnetic resonance imaging (MRI) is a primary non-invasive imaging modality for tumor segmentation, leveraging its exceptional soft tissue contrast and high resolution. Current segmentation methods typically focus on structural MRI, such as T<sub>1</sub>-weighted post-contrast-enhanced or fluid-attenuated inversion recovery (FLAIR) sequences. However, these methods overlook the blood perfusion and hemodynamic properties of tumors, readily derived from dynamic susceptibility contrast (DSC) enhanced MRI. This study introduces a novel hybrid method combining density-based analysis of hemodynamic properties in time-dependent perfusion imaging with deep learning spatial segmentation techniques to enhance tumor segmentation.Methods: First, a U-Net convolutional neural network (CNN) is employed on structural images to delineate a region of interest (ROI). Subsequently, Hierarchical Density-Based Scans (HDBScan) are employed within the ROI to augment segmentation by exploring intratumoral hemodynamic heterogeneity through the investigation of tumor time course profiles unveiled in DSC MRI.Results: The approach was tested and evaluated using a cohort of 513 patients from the open-source University of Pennsylvania glioblastoma database (UPENN-GBM) dataset, achieving a 74.83% Intersection over Union (IoU) score when compared to structural-only segmentation. The algorithm also exhibited increased precision and localized predictions of heightened segmentation boundary complexity, resulting in a 146.92% increase in contour complexity (ICC) compared to the reference standard provided by the UPENN-GBM dataset. Importantly, segmenting tumors with the developed new approach uncovered a negative correlation of the tumor volume with the scores in the Karnofsky Performance Scale (KPS) clinically used for assessing the functional status of patients (-0.309), which is not observed with the prevailing segmentation standard.Conclusions: This work demonstrated that including hemodynamic properties of tissues from DSC MRI can improve existing structural or morphological feature-based tumor segmentation techniques with additional information on tumor biology and physiology. This approach can also be applied to other clinical indications that use perfusion MRI for diagnosis or treatment monitoring.

Abstract:We developed and tested the feasibility of computational fluid modeling (CFM) based on dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) for quantitative estimation of interstitial fluid pressure (IFP) and velocity (IFV) in patients with head and neck (HN) cancer with locoregional lymph node metastases. Twenty-two patients with HN cancer, with 38 lymph nodes, underwent pretreatment standard MRI, including DCE-MRI, on a 3-Tesla scanner. CFM simulation was performed with the finite element method in COMSOL Multiphysics software. The model consisted of a partial differential equation (PDE) module to generate 3D parametric IFP and IFV maps, using the Darcy equation and Ktrans values (min−1, estimated from the extended Tofts model) to reflect fluid influx into tissue from the capillary microvasculature. The Spearman correlation (ρ) was calculated between total tumor volumes and CFM estimates of mean tumor IFP and IFV. CFM-estimated tumor IFP and IFV mean ± standard deviation for the neck nodal metastases were 1.73 ± 0.39 (kPa) and 1.82 ± 0.9 × (10−7 m/s), respectively. High IFP estimates corresponds to very low IFV throughout the tumor core, but IFV rises rapidly near the tumor boundary where the drop in IFP is precipitous. A significant correlation was found between pretreatment total tumor volume and CFM estimates of mean tumor IFP (ρ = 0.50, P = 0.004). Future studies can validate these initial findings in larger patients with HN cancer cohorts using CFM of the tumor in concert with DCE characterization, which holds promise in radiation oncology and drug-therapy clinical trials.

Abstract:Background: Glioblastoma (GBM) is the deadliest and most common brain tumor in adults, with poor survival and response to aggressive therapy. Limited access of drugs to tumor cells is one reason for such grim clinical outcomes. A driving force for therapeutic delivery is interstitial fluid flow (IFF), both within the tumor and in the surrounding brain parenchyma. However, convective and diffusive transport mechanisms are understudied. In this study, we examined the application of a novel image analysis method to measure fluid flow and diffusion in GBM patients. Methods: Here, we applied an imaging methodology that had been previously tested and validated in vitro, in silico, and in preclinical models of disease to archival patient data from the Ivy Glioblastoma Atlas Project (GAP) dataset. The analysis required the use of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI), which is readily available in the database. The analysis results, which consisted of IFF flow velocity and diffusion coefficients, were then compared to patient outcomes such as survival. Results: We characterized IFF and diffusion patterns in patients. We found strong correlations between flow rates measured within tumors and in the surrounding parenchymal space, where we hypothesized that velocities would be higher. Analyzing overall magnitudes indicated a significant correlation with both age and survival in this patient cohort. Additionally, we found that neither tumor size nor resection significantly altered the velocity magnitude. Lastly, we mapped the flow pathways in patient tumors and found a variability in the degree of directionality that we hypothesize may lead to information concerning treatment, invasive spread, and progression in future studies. Conclusions: An analysis of standard DCE-MRI in patients with GBM offers more information regarding IFF and transport within and around the tumor, shows that IFF is still detected post-resection, and indicates that velocity magnitudes correlate with patient prognosis.

Non-invasive imaging of interstitial fluid transport parameters in solid tumors in vivo

Abstract:In this paper, new and non-invasive imaging methods to assess interstitial fluid transport parameters in tumors in vivo are developed, analyzed and experimentally validated. These parameters include extracellular volume fraction (EVF), interstitial fluid volume fraction (IFVF) and interstitial hydraulic conductivity (IHC), and they are known to have a critical role in cancer progression and drug delivery effectiveness. EVF is defined as the volume of extracellular matrix per unit volume of the tumor, while IFVF refers to the volume of interstitial fluid per unit bulk volume of the tumor. There are currently no established imaging methods to assess interstitial fluid transport parameters in cancers in vivo. We develop and test new theoretical models and imaging techniques to assess fluid transport parameters in cancers using non-invasive ultrasound methods. EVF is estimated via the composite/mixture theory with the tumor being modeled as a biphasic (cellular phase and extracellular phase) composite material. IFVF is estimated by modeling the tumor as a biphasic poroelastic material with fully saturated solid phase. Finally, IHC is estimated from IFVF using the well-known Kozeny-Carman method inspired by soil mechanics theory. The proposed methods are tested using both controlled experiments and in vivo experiments on cancers. The controlled experiments were performed on tissue mimic polyacrylamide samples and validated using scanning electron microscopy (SEM). In vivo applicability of the proposed methods was demonstrated using a breast cancer model implanted in mice. Based on the controlled experimental validation, the proposed methods can estimate interstitial fluid transport parameters with an error below 10% with respect to benchmark SEM data. In vivo results demonstrate that EVF, IFVF and IHC increase in untreated tumors whereas these parameters are observed to decrease over time in treated tumors. The proposed non-invasive imaging methods may provide new and cost-effective diagnostic and prognostic tools to assess clinically relevant fluid transport parameters in cancers in vivo.
Comprehensive characterization of tumor therapeutic response with simultaneous mapping cell size, density, and transcytolemmal water exchange

Abstract:Early assessment of tumor therapeutic response is an important topic in precision medicine to optimize personalized treatment regimens and reduce unnecessary toxicity, cost, and delay. Although diffusion MRI (dMRI) has shown potential to address this need, its predictive accuracy is limited, likely due to its unspecific sensitivity to overall pathological changes. In this work, we propose a new quantitative dMRI-based method dubbed EXCHANGE (MRI of water Exchange, Confined and Hindered diffusion under Arbitrary Gradient waveform Encodings) for simultaneous mapping of cell size, cell density, and transcytolemmal water exchange. Such rich microstructural information comprehensively evaluates tumor pathologies at the cellular level. Validations using numerical simulations and in vitro cell experiments confirmed that the EXCHANGE method can accurately estimate mean cell size, density, and water exchange rate constants. The results from in vivo animal experiments show the potential of EXCHANGE for monitoring tumor treatment response. Finally, the EXCHANGE method was implemented in breast cancer patients with neoadjuvant chemotherapy, demonstrating its feasibility in assessing tumor therapeutic response in clinics. In summary, a new, quantitative dMRI-based EXCHANGE method was proposed to comprehensively characterize tumor microstructural properties at the cellular level, suggesting a unique means to monitor tumor treatment response in clinical practice.

Incorporating cross-voxel exchange for the analysis of dynamic contrast-enhanced imaging data: pre-clinical results

Abstract:Tumours exhibit abnormal interstitial structures and vasculature function often leading to impaired and heterogeneous drug delivery. The disproportionate spatial accumulation of a drug in the interstitium is determined by several microenvironmental properties (blood vessel distribution and permeability, gradients in the interstitial fluid pressure). Predictions of tumour perfusion are key determinants of drug delivery and responsiveness to therapy. Pharmacokinetic models allow for the quantification of tracer perfusion based on contrast enhancement measured with non-invasive imaging techniques. An advanced cross-voxel exchange model (CVXM) was recently developed to provide a comprehensive description of tracer extravasation as well as advection and diffusion based on cross-voxel tracer kinetics (Sinnoet al2021). Transport parameters were derived from DCE-MRI of twenty TS-415 human cervical carcinoma xenografts by using CVXM. Tracer velocity flows were measured at the tumour periphery (mean 1.78-5.82μm.s-1) pushing the contrast outward towards normal tissue. These elevated velocity measures and extravasation rates explain the heterogeneous distribution of tracer across the tumour and its accumulation at the periphery. Significant values for diffusivity were deduced across the tumours (mean 152-499μm2.s-1). CVXM resulted in generally smaller values for the extravasation parameterKext(mean 0.01-0.04 min-1) and extravascular extracellular volume fractionve(mean 0.05-0.17) compared to the standard Tofts parameters, suggesting that Toft model underestimates the effects of inter-voxel exchange. The ratio of Tofts' extravasation parameters over CVXM's was significantly positively correlated to the cross-voxel diffusivity (P< 0.0001) and velocity (P= 0.0005). Tofts' increasedvemeasurements were explained using Sinnoet al(2021)'s theoretical work. Finally, a scan time of 15 min renders informative estimations of the transport parameters. However, a duration as low as 7.5 min is acceptable to recognize the spatial variation of transport parameters. The results demonstrate the potential of utilizing CVXM for determining metrics characterizing the exchange of tracer between the vasculature and the tumour tissue. Like for many earlier models, additional work is strongly recommended, in terms of validation, to develop more confidence in the results, motivating future laboratory work in this regard.
Heterogeneous Mechanical Stress and Interstitial Fluid Flow Predictions Derived from DCE-MRI for Rat U251N Orthotopic Gliomas

Abstract:Mechanical stress and fluid flow influence glioma cell phenotype in vitro, but measuring these quantities in vivo continues to be challenging. The purpose of this study was to predict these quantities in vivo, thus providing insight into glioma physiology and potential mechanical biomarkers that may improve glioma detection, diagnosis, and treatment. Image-based finite element models of human U251N orthotopic glioma in athymic rats were developed to predict structural stress and interstitial flow in and around each animal's tumor. In addition to accounting for structural stress caused by tumor growth, our approach has the advantage of capturing fluid pressure-induced structural stress, which was informed by in vivo interstitial fluid pressure (IFP) measurements. Because gliomas and the brain are soft, elevated IFP contributed substantially to tumor structural stress, even inverting this stress from compressive to tensile in the most compliant cases. The combination of tumor growth and elevated IFP resulted in a concentration of structural stress near the tumor boundary where it has the greatest potential to influence cell proliferation and invasion. MRI-derived anatomical geometries and tissue property distributions resulted in heterogeneous interstitial fluid flow with local maxima near cerebrospinal fluid spaces, which may promote tumor invasion and hinder drug delivery. In addition, predicted structural stress and interstitial flow varied markedly between irradiated and radiation-naïve animals. Our modeling suggests that relative to tumors in stiffer tissues, gliomas experience unusual mechanical conditions with potentially important biological (e.g., proliferation and invasion) and clinical consequences (e.g., drug delivery and treatment monitoring).

Abstract:Magnetic resonance imaging (MRI) is the method of choice for noninvasive studies of micrometer-scale structures in biological tissues via their effects on the time/frequency-dependent ("restricted") and anisotropic self-diffusion of water. Traditional MRI relies on pulsed magnetic field gradients to encode the signal with information about translational motion in the direction of the gradient, which convolves fundamentally different aspects-such as bulk diffusivity, restriction, anisotropy, and flow-into a single effective observable lacking specificity to distinguish between biologically plausible microstructural scenarios. To overcome this limitation, we introduce a formal analogy between measuring rotational correlation functions and interaction tensor anisotropies in nuclear magnetic resonance (NMR) spectroscopy and investigating translational motion in MRI, which we utilize to convert data acquisition and analysis strategies from NMR of rotational dynamics in macromolecules to MRI of diffusion in biological tissues, yielding model-independent quantitative metrics reporting on relevant microstructural properties with unprecedented specificity. Our model-free approach advances the state-of-the-art in microstructural MRI, thereby enabling new applications to complex multi-component tissues prevalent in both tumors and healthy brain.

Abstract:Purpose: The primary aim of this study was to explore whether intravoxel incoherent motion (IVIM) can offer a contrast-agent-free alternative to dynamic contrast-enhanced (DCE)-MRI for measuring breast tumor perfusion. The secondary aim was to investigate the relationship between tissue diffusion measures from DWI and DCE-MRI measures of the tissue interstitial and extracellular volume fractions. Materials and methods: A total of 108 paired DWI and DCE-MRI scans were acquired at 1.5 T from 40 patients with primary breast cancer (median age: 44.5 years) before and during neoadjuvant chemotherapy (NACT). DWI parameters included apparent diffusion coefficient (ADC), tissue diffusion (D t ), pseudo-diffusion coefficient (D p ), perfused fraction (f), and the product f×D p (microvascular blood flow). DCE-MRI parameters included blood flow (F b ), blood volume fraction (v b ), interstitial volume fraction (v e ) and extracellular volume fraction (v d ). All were extracted from three tumor regions of interest (whole-tumor, ADC cold-spot, and DCE-MRI hot-spot) at three MRI visits: pre-treatment, after one, and three cycles of NACT. Spearman's rank correlation was used for assessing between-subject correlations (r), while repeated measures correlation was employed to assess within-subject correlations (r rm ) across visits between DWI and DCE-MRI parameters in each region. Results: No statistically significant between-subject or within-subject correlation was found between the perfusion parameters estimated by IVIM and DCE-MRI (f versus v b and f×D p versus F b ; P=0.07–0.81). Significant moderate positive between-subject and within-subject correlations were observed between ADC and v e (r=0.461, r rm =0.597) and between D t and v e (r=0.405, r rm =0.514) as well as moderate positive within-subject correlations between ADC and v d and between D t and v d (r rm =0.619 and 0.564, respectively) in the whole-tumor region. Conclusion: No correlations were observed between the perfusion parameters estimated by IVIM and DCE-MRI. This may be attributed to imprecise estimates of fxD p and v b , or an underlying difference in what IVIM and DCE-MRI measure. Care should be taken when interpreting the IVIM parameters (f and f×D p ) as surrogates for those measured using DCE-MRI. However, the moderate positive correlations found between ADC and D t and the DCE-MRI parameters v e and v d confirms the expectation that as the interstitial and extracellular volume fractions increase, water diffusion increases.

Abstract:Abstract Interstitial fluid flow dynamics in brain tumors are known to be an important indicator for tumor invasion into healthy tissue, as cancerous cells follow the interstitial flow towards tumor-stimulating chemokines. These flow gradients not only carry tumor-stimulating chemokines, but also carry cytokines secreted by immune response. CAR-T cells are known to also depend highly on the cytokines present in the CSF and serum. For these reasons, we have developed a novel method, Localized Convolutional Function Regression (LCFR) for measuring interstitial fluid flow, perfusion, and diffusion using dynamic contrast-enhanced MRI. LCFR works by applying the known physics of contrast agent transport, and fitting the observed contrast agent dynamics to the known physics. From LCFR, we can recover standard Tofts-Kety perfusion dynamics, including Ktrans and Vp, and also recover the underlying flow field and diffusivity. These new measurements allow for the prediction of where tumors invade, and allow for us to model how CAR-T cells move within a tumor. In a case study, we model the spatial distribution of CAR-T cells and glioblastoma cells over time, assuming that the fluid velocity field drives CAR-T spread throughout the tumor. Using multiple delivery locations, we identify the optimal catheter placement to optimally distribute CAR-T cells through the tumor, improving the 50% kill rate of simulated tumor cells by over 25% from a naive delivery strategy. Further, we collect longitudinal cytokine levels from patients undergoing CAR-T therapy, allowing us to understand how interstitial flow, CAR-T dynamics, and tumor dynamics interact with each other. We report a case study wherein a patient is longitudinally followed through their treatment with CAR-Ts. Perfusion imaging reveals that the patient experienced a steady increase in perfusion and decrease in fluid leakage into surrounding tissue. VEGF and IL12 levels are observed to increase as tumor volume increases. To better understand this patient's dynamics, we use the fluid velocity field measured by LCFR to simulate the development of CAR-T cells within the tumor, using the cytokine profile as a surrogate measurement of CAR-T population. Abstract:Here, we investigate radiomics-based characterization of tumor vascular and microenvironmental properties in an orthotopic rat brain tumor model measured using dynamic-contrast-enhanced (DCE) MRI. Thirty-two immune compromised-RNU rats implanted with human U-251N cancer cells were imaged using DCE-MRI (7Tesla, Dual-Gradient-Echo). The aim was to perform pharmacokinetic analysis using a nested model (NM) selection technique to classify brain regions according to vasculature properties considered as the source of truth. A two-dimensional convolutional-based radiomics analysis was performed on the raw-DCE-MRI of the rat brains to generate dynamic radiomics maps. The raw-DCE-MRI and respective radiomics maps were used to build 28 unsupervised Kohonen self-organizing-maps (K-SOMs). A Silhouette-Coefficient (SC), k-fold Nested-Cross-Validation (k-fold-NCV), and feature engineering analyses were performed on the K-SOMs' feature spaces to quantify the distinction power of radiomics features compared to raw-DCE-MRI for classification of different Nested Models. Results showed that eight radiomics features outperformed respective raw-DCE-MRI in prediction of the three nested models. The average percent difference in SCs between radiomics features and raw-DCE-MRI was: 29.875% ± 12.922%, p < 0.001. This work establishes an important first step toward spatiotemporal characterization of brain regions using radiomics signatures, which is fundamental toward staging of tumors and evaluation of tumor response to different treatments.

Abstract:Magnetic resonance imaging (MRI) is particularly attractive for clinical application in perfusion imaging thanks to the absence of ionizing radiation and limited volumes of contrast agent (CA) necessary. Dynamic contrast-enhanced MRI (DCE-MRI) involves sequentially acquiring T1-weighted images through an organ of interest during the passage of a bolus administration of CA. It is a particularly flexible approach to perfusion imaging as the signal intensity time course allows not only rapid qualitative assessment, but also quantitative measures of intrinsic perfusion and permeability parameters. We examine aspects of the T1-weighted image series acquisition, CA administration, post-processing that constitute a DCE-MRI study in clinical practice, before considering some heuristics that may aid in interpreting the resulting contrast enhancement time series. While qualitative DCE-MRI has a well-established role in the diagnostic assessment of a range of tumours, and a central role in MR mammography, clinical use of quantitative DCE-MRI remains limited outside of clinical trials. The recent publication of proposals for standardized acquisition and analysis protocols for DCE-MRI by the Quantitative Imaging Biomarker Alliance may be an opportunity to consolidate and advance clinical practice.
Assessing the feasibility of a new approach to measure the full spectrum of CSF dynamics within the human brain using MRI: insights from a simulation study

Abstract:Cerebrospinal fluid (CSF) dynamics are essential in waste clearance of the brain. Disruptions in CSF flow are linked to various neurological conditions, highlighting the need for accurate measurement of its dynamics. Current methods typically capture limited aspects of CSF movement or focus on a single anatomical region, presenting challenges for comprehensive analysis. This study proposes a novel approach using Displacement Encoding with Stimulated Echoes (DENSE) MRI to assess the full spectrum of CSF motion within the brain. Through simulations, we evaluated the feasibility of disentangling distinct CSF motion components, including heartbeat- and respiration-driven flows, as well as a net velocity component due to continuous CSF turnover, and tested the performance of our method under incorrect assumptions about the underlying model of CSF motion. Results demonstrate that DENSE MRI can accurately separate these components, and reliably estimated a net velocity, even when periodic physiological motions vary over time. The method proved to be robust for including low frequency components (LFO), incorrect assumption on the nature of the net velocity component and missing CSF components in the model. This approach offers a comprehensive measurement technique for quantifying CSF dynamics, advancing our understanding of the relative role of various drivers of CSF dynamics in brain clearance.
