Abstract:PURPOSE:To study the killing effects on osteosarcoma cells of cinobufacini and cisplatin in combination and the related mechanisms so as to explore the chemotherapeutic method with integrated traditional Chinese and Western medicines.METHODS:Cinobufacini and cisplatin were applied to OS732 cells singly or jointly and survival rates were measured by MTT assay. Changes in cellular shape were observed with inverted phase contrast and fluorescence microscopy and apoptosis rates were analyzed with flow cytometry (FCM). Immunocytochemistry were used to examine the Fas expression of OS732 cells.RESULTS:The combination of cinobufacini and cisplatin had the effect of up-regulating Fas expression and inducing apoptosis. The survival rate of combined application of 100 μg/ml cinobufacini and 1 μg/ml cisplatin on OS-732 cells was significantly lower than with either of the agents alone (p<0.01). Changes in cellular shape and apoptotic rates also indicated the apoptosis-inducing effects of combined application were much enhanced.CONCLUSION:The combination of cinobufacini and cisplatin demonstrated strong killing effects on OS-732 cells which might be related to up-regulation of Fas expression.
[Phase III Clinical Study of Zoledronic Acid in the Treatment of Pain Induced by Bone Metastasis from Solid Tumor or Multiple Myeloma].

Abstract:OBJECTIVETo evaluate the efficacy and safety of zoledronic acid in the treatment of bone pain in patients with bone metastasis from solid tumor or multiple myeloma.METHODSA randomized, double-blind, double-simulated and multi-center phase III clinical trail with pamidronate as control was conducted. Patients with moderate to severe bone pain (VAS > 50 mm) induced by solid tumor or multiple myeloma were randomized to receive intravenous zoledronic acid 4 mg or pamidronate 90 mg. Then the change of VAS and urinary NTX/Cr and CTX/Cr were observed in two groups.RESULTSFrom July 2005 to September 2006, 228 patients with bone pain induced by bone metastasis from 15 cancer centers were randomize into two groups: 116 patients in zoledronic acid group and 112 patients in pamidronate group. The VAS value was decreased gradually after treatment in these two groups. Significant improvement in bone pain after treatment were observed both in zoledronic acid group and the control group when compared with baseline VAS on D8 (-11.77% vs. -10.87%), D15 (-24.60% vs. -21.06%) and D28 (-32.37% vs. -31.26%) (P< or =0.0001), but no significant difference existed between two groups (P =0.6587). Compared with baseline, urine NTX/Cr and CTX/Cr level were decreased rapidly after treatment in both groups, the nadir was on D8, the median decreased on D28, which was -36.9% vs. -32.1% for NTX/Cr (P = 0.7922) and -63.2% vs. -47.9% for CTX/Cr (P =0.834). The frequently observed adverse events were pyrexia (19.0% vs. 31.3%), vomiting (6.0% vs. 8.9%), nausea (4.3% vs. 4.5%), fatigue (3.4% vs. 2.7%) and constipation (2.6% vs. 1.8%) in the two groups. Compared with baseline, the serum creatinine level was not significantly increased throughout the study.CONCLUSIONIntravenous injection of 4 mg zoledronic acid can significantly reduce bone pain and bone resorption marker in urine in the Chinese patients with bone metastasis from solid tumor or multiple myeloma, which is tolerable and also comparable to pamidronate in the efficacy and safety.

Abstract:Objective To observe the clinical combination effect of Jinlong Capsule (金龙胶囊, JLC) and transcatheter arterial chemoembolization (TACE) on the patients with primary hepatic carcinoma (PHC) and JLC’ s influence on serum osteopontin (OPN) expression and elucidate the correlation between the serum OPN level and curative effect of JLC and TACE. Methods A total of 98 patients with PHC were observed in a randomized controlled trial (RCT). They were assigned to the Chinese medicine (CM) group (53 patients who were treated with TACE and JLC) and the intervention group (45 patients who were treated with TACE only). The serum OPN levels were measured before and after treatment by quantitative sandwich enzyme-linked immunosorbent assay (ELISA). Forty healthy people were assigned to the control group. The clinical efficacy was observed and Karnofsky score (KPS) was graded. Results The clinical efficacy of the CM group (60.38%) was better than that of the intervention group (40.00 %), and the KPS (84.35±12.19) was higher than the intervention group (69.86± 11.58) ( P <0.05). The serum OPN levels before and after treatment in the patients with PHC were significantly elevated compared with those in the control group ( P <0.01). After treatment, the OPN levels in CM group (117.69 <±78.50) were significantly lower compared with those in intervention group (151.09±83.90, P <0.05). The OPN levels of responders were remarkably lowered than the non-responders after treatment, and the level of OPN in the CM group was lower than the intervention group ( P <0.05). Conclusions The short-term clinical efficacy and the quality of life of patients with PHC can be improved by combining JLC with TACE. The serum OPN levels in PHC patients can reflect the curative effect of treatment and the prognosis of the disease.

Abstract:Background. Cinobufacini capsule, an anticancer traditional Chinese patent medicine, has been widely used as adjunctive treatment to platinum-based chemotherapy in patients with advanced NSCLC. Purpose. To evaluate the efficacy and safety of cinobufacini capsule combined with first-line platinum-based chemotherapy for advanced NSCLC. Study Design. A systematic review and meta-analysis of eight outcome measures selected for this study were performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Methods. A comprehensive literature search was conducted in 7 electronic databases to identify all the relevant randomised controlled trials. Cochrane handbook 5.1.0 was applied to evaluate the quality of included trials, and the RevMan 5.3 and Stata 15.1 software were used to combine the trials for data analysis and assess the publication bias. Results. From the 19 studies reviewed, a total of 1,564 patients were included. Compared with first-line platinum-based chemotherapy alone, cinobufacini capsule combined with chemotherapy showed significant effects in improving ORR (RR = 1.49, 95% CI (1.33, 1.66)), 1-year survival rate (RR = 1.44, 95% CI (1.28, 1.63)), and 2-year survival rate (RR = 1.78, 95% CI (1.42, 2.22)), raising the percentages of CD3(+) cells (SMD = 1.25, 95% CI (1.05, 1.45)), CD4(+) cells (SMD = 1.52, 95% CI (1.33, 1.71)), and ratio of CD4(+)/CD8(+) (SMD = 1.36, 95% CI (1.17, 1.54)), and reducing chemotherapy toxicity including leukopenia (RR = 0.61, 95% CI (0.51, 0.72)), thrombocytopenia (RR = 0.52, 95% CI (0.41, 0.67)), and vomiting (RR = 0.79, 95% CI (0.70, 0.88)). Conclusion. Cinobufacini capsule may increase the therapeutic effectiveness, improve cellular immune function, and reduce the toxicity of first-line platinum-based chemotherapy in patients with NSCLC. These results require confirmation by further rigorously designed randomised controlled trials (RCTs).

Abstract:Objective We conducted this paper to decipher the efficacy of the combined chemotherapy of zoledronic acid and pamidronate in treating bone metastases from nonsmall cell lung cancer (NSCLC) and the effects on pain stress and bone metabolic indices. Methods Patients with bone metastases from NSCLC were allocated into Group A and Group B. Patients in the Group A were administrated with pamidronate combined chemotherapy and patients in the Group B were administrated with zoledronic acid combined chemotherapy. The efficacy, pain symptom scores, quality of life scores, serum inflammatory factor, serum bone metabolic indices, serum pain stress indicators, and the occurrence of adverse effects were compared in patients of the two groups. Results The total effective rate of treatment was higher in the Group B than in the Group A. After treatment, reduced Numerical Rating Scale scores and elevated Karnofsky Performance Score score, reduced serum levels of N‐terminal mid‐fragment of osteocalcin, N‐terminal propeptide of type I procollagen, bone‐specific alkaline phosphatase, and type I collagen hydroxyl terminal peptide β special sequence, reduced serum levels of C‐reactive protein, procalcitonin, tumor necrosis factor‐α, and interleukin‐6, as well as decreased levels of bradykinin, substance P, neuropeptide Y, and β‐endorphin were found in the Group B versus the Group A. No notable difference was witnessed in the rate of adverse reactions between the Group A and the Group B. Conclusion Zoledronic acid combined with chemotherapy can effectively treat bone metastases of NSCLC and improve pain stress and bone metabolic status, which has value that can be promoted and applied in clinical treatment.

Maturation, reproduction and recruitment of the sand smelt, Atherina boyeri Risso, 1810 (Pisces:Atherinidae) in the estuary of Mala Neretva River (southeastern Adriatic, Croatia)

Abstract:Characteristics of maturation and recruitment of the sand smelt, Atherina boyeri, from the Mala Neretva River estuary in the southeastern Adriatic, Croatia in 2001/2, are presented. The average gonadosomatic index (GSI) during a 12-month period was 1.59% for females and 2.1% for males. The highest value was in April, 5.4% for females and 4.7% for males. The smallest mature female was 5.2 cm total length. Fifty percent of the females were mature at 7.75 cm total length. New juveniles began to recruit in June and July and later appeared in fewer numbers. In general, the GSI was lower than in other sand smelt populations. The reasons are difficult to determine but might be partly explained by the high and unpredictable temperature and salinity variations in the researched area that have led to previously described disturbances of the sand smelt life cycle, as in feeding and a high incidence of spinal deformities.
Phase II trial of zoledronic acid combined with androgen-deprivation therapy for treatment-naïve prostate cancer with bone metastasis

Abstract:Background: The efficacy of zoledronic acid in patients with treatment-naïve prostate cancer is unclear. We conducted a phase II study to investigate the benefits of combined zoledronic acid and androgen deprivation therapy in treatment-naïve prostate cancer with bone metastasis. The primary endpoint was skeletal-related event-free survival at 24 months. Methods: Subjects were treatment-naïve patients with histologically confirmed adenocarcinoma of the prostate and radiological evidence of bone metastasis. Treatment consisted of bicalutamide 80 mg daily, goserelin acetate 10.8 mg every 12 weeks, and zoledronic acid 4 mg every 4 weeks. Zoledronic acid was continued for 24 months. Results: Of the patients enrolled between July 2008 and April 2010, 52 were included in the analyses. The median age of the patients was 72 years. The median baseline prostate-specific antigen level was 249.4 ng/mL. The median follow-up period was 33.3 months. The 24-month skeletal-related event-free survival rate was 84.4 % (95 % confidence interval 71.2-91.9). The median time to prostate-specific antigen progression was 25.9 months (95 % confidence interval 14.7-36.3). The median overall survival time was not reached. Improvement in pain or maintenance of no pain during the first 12 weeks was observed in 70 % of patients and the extent of bone disease was decreased in 10 % of patients at 12 months. Grade 3 osteonecrosis of the jaw was observed in three patients (5.8 %). Conclusion: Zoledronic acid concomitant with androgen deprivation therapy as initial treatment in patients with treatment-naïve prostate cancer with bone metastasis resulted in an encouraging skeletal-related event-free survival rate at 24 months.

Abstract:Background Bortezomib (BZM), alone or in combination with other chemotherapies, has displayed strong anticancer effects in several cancers. The efficacy of the combination of BZM and mitoxantrone (MTX) in treating prostate cancer remains unknown. Methods Anticancer effects of combination of BZM and MTX were determined by apoptosis and proliferation assay in vivo and in vitro. Expression of β-Catenin and its target genes were characterized by western blot and Real-time PCR. Results BZM significantly enhanced MTX-induced antiproliferation in vivo and in vitro. Mice administered a combination of BZM and MTX displayed attenuated tumor growth and prolonged survival. BZM significantly attenuated MTX-induced apoptosis. Moreover, the combination of BZM and MTX contributed to inhibition of the Wnt/β-Catenin signaling pathway compared to monotherapy. Conclusions This study demonstrates that BZM enhances MTX-induced anti-tumor effects by inhibiting the Wnt/β-Catenin signaling pathway in prostate cancer cells.
Significance of baseline bone markers on disease progression and survival in hormone-sensitive prostate cancer with bone metastasis

Abstract:Purpose: This study evaluated the baseline patient characteristics associated with the time to biochemical progression and overall survival in patients who participated in a phase II trial on zoledronic acid combined with the initial androgen-deprivation therapy for treatment-naïve bone-metastatic prostate cancer. Methods: Patients received zoledronic acid 4 mg intravenously every 4 weeks for up to 24 months, concomitantly started with bicalutamide 80 mg orally every day and goserelin acetate 10.8 mg subcutaneously every 12 weeks. Results: A total of 53 Japanese patients were enrolled between July 2008 and April 2010, and 52 patients were evaluable. Median follow-up period was 41.6 months. Updated median time to biochemical progression was 25.9 months (95 % confidence interval 14.5-49.9). Higher serum bone-specific alkaline phosphatase was an independent risk factor for time to biochemical progression based on multivariate analysis (hazard ratio 6.51; 95 % confidence interval 2.71-15.62; P < 0.001). Median time to biochemical progression for patients with serum bone-specific alkaline phosphatase level higher than 26 μg/L was 12.7 months. Multivariate analysis indicated that higher serum C-terminal telopeptide of type I collagen independently increased the risk of death (hazard ratio 9.62; 95 % confidence interval 2.11-43.89; P = 0.003). Median overall survival for patients with serum C-terminal telopeptide of type I collagen level higher than 8.0 ng/ml was 31.1 months. Conclusions: Baseline bone markers can be useful as predictors for disease progression and survival time in patients with bone metastasis from treatment-naïve prostate cancer treated with upfront zoledronic acid concomitantly started with androgen-deprivation therapy.
Long term zoledronic acid during androgen blockade for prostate cancer

Abstract:Objectives: To evaluate the effect of zoledronic acid on androgen deprivation therapy in patients with hormone-sensitive prostate cancer by measuring the percentage change in lumbar-spine bone mineral density (BMD) at 12 and 24 months. Materials and methods: An open-label, multicenter, randomized, two-phase study was conducted in patients with hormone-sensitive prostate cancer (N = 200) receiving 10.8 mg goserelin acetate with or without zoledronic acid (4 mg intravenously) every 3 months. In phase I, patients were randomized to goserelin acetate alone or goserelin acetate plus zoledronic acid for 12 months. In phase II, patients receiving goserelin acetate plus zoledronic acid continued treatment for up to a total of 24 months, whereas patients receiving goserelin acetate alone were randomized to goserelin acetate alone or goserelin acetate plus zoledronic acid for an additional 12 months. Lumbar-spine, femoral-neck, and total-hip BMD were assessed at 6, 12, and 24 months. Additional assessments included height change, laboratory studies, bone scans, radiographs, and computed tomography scans. Results: Significant BMD differences between patients receiving goserelin acetate alone and goserelin acetate plus zoledronic acid were observed at the 12-month (p <or= .01 for each site) and 24-month (p < .05 for each site) assessments. Initiating zoledronic acid after 12 months of goserelin acetate alone provided BMD benefits but was insufficient to completely restore BMD. Combining goserelin acetate and zoledronic acid was generally well tolerated. Conclusions: Two years of zoledronic acid is well tolerated and can prevent bone loss in patients with prostate cancer undergoing androgen deprivation therapy.
A multicenter randomized, double-blind and double dim comparative studies on zoledronic acid and pamidronate disodium: Phase II trials from 12 centers for treating bone metastasis pain

Abstract:Objective: To evaluate the clinical effect and safety profiles of zoledronic acid in the treatment of bone metastasis pain in cancer patients. Methods: A multicenter, double-blind and double-dim clinical trial on pamidronate was conducted in 237 cancer patients with bone metastasis pain from 12 centers in China. The patients were randomized into 2 groups, i.e., the experiment group and control group. Zoledronic acid (4 mg) was injected for 15 min in the experiment group and pamidronate (90 mg) i.v. within 4 h in the controls. The daily change of bone ache recorded by a visual analog scale (VAS) and the side effects were observed within 4 weeks as well. Results: The total 219 of 239 cases were eligible for final analysis, 112 from experimental group and 107 from control group were involved in the clinical response evluation. For the best clinical response within 14 days, after injection with those two biphosphanates, the complete response rate (CR rate), partial response rate (PR rate ) and clinical benefit rate was 10.08%, 53.78% and 90.75% respectively in the experiment group, compared with those data of 5.08%, 54.24% and 83.90% in the control group. There was no significant difference between the 2 groups. The daily clinical improvements of pain reduction and/or relief in the experiment group during the 5th, 7th till 12th day were absolutely better than those from the controls.There was no significant difference between the 2 groups for the consideration of pain improvement with 28 days . The medium maintain time (MMT) of complete response within 14 days were 5.51 and 3.01 days respectively between the 2 groups (P=0.0302), with the contrast of the MMT of effective responses (CR+PR) were 9.02 and 8.02 days (P>0.05). The incidence rate of the main side effects, like fever, hypocalcaemia, and fatigue was 23.94% and 27.98% respectively in the experiment group and the controls. No severe side effect was found in both groups. Conclusion: Zoledronic acid can effectively relieve bone metastasis pain. The response rate and side effect were similar to those of pamidronate.

Abstract:To examine the effect of timely zoledronic acid (ZA) treatment on clinical outcomes and health care utilization in patients with bone-metastatic prostate cancer. Patients with prostate cancer and bone metastasis were identified in a Veterans Affairs database (01/2002–09/2009). Eligible patients had no documented skeletal-related events (SREs) before the index date (that is, the first bone metastasis diagnosis date). Patients who received early ZA treatment, defined as having a ZA infusion after the index date and before any recorded SREs, were matched 1:1 on propensity score to patients not treated with bisphosphonates (BPs). Risks of SREs, hospitalization and death during the 6-month post-index period were compared between matched cohorts using Kaplan–Meier analyses. Baseline characteristics were well balanced between the matched cohorts ( n =73 per group). 6-month SRE-free survival and hospitalization-free survival were higher in patients receiving timely ZA than patients without BP treatment (91.7 versus 71.5%, P <0.01; 80.5 versus 66.3%, P =0.05, respectively). 6-month mortality risk was significantly lower in patients treated with ZA versus those without BP treatment (4.3 versus 13.8%, P =0.04). Timely ZA intervention in bone-metastatic prostate cancer patients was associated with significant reductions in 6-month risks of SREs, hospitalization and mortality, as compared with no BP treatment.
Cinobufotalin effect and mechanism on serum MMP-2, MMP-9, Beclin1, LC3-II in advanced NSCLC patients

Abstract:This study was conducted to explore cinobufotalin's effects and related mechanisms on serum MMP-2, MMP-9, Beclin1, and LC3-II in advanced non-small-cell lung cancer (NSCLC) patients. For this purpose, 150 patients with advanced NSCLC in our hospital from Jan. 2020 to Feb. 2022 were chosen as participants in the research study. Using a random number table method, the 150 patients were divided evenly into two groups - a control group (C) and an observation group (O). Group C received conventional NP regimen chemotherapy, while Group O received cinobufotalin capsules based on the control group. The follow-up was conducted for 4 months, and the differences in serum MMP-2, MMP-9, Beclin1, LC3-II and chemotherapy resistance rates were compared. Results showed that There was no statistically significant difference in MMP-2, MMP-9, Beclin1, and LC3-II levels between the two before treatment (P>0.05); 4 months later, Group O's MMP-2, MMP-9, Beclin1, and LC3-II levels were lower than those before treatment and Group C during the same period, with a statistically significant difference (P<0.05); At 4 months after treatment, the clinical efficacy of Group O was better and its ORR was higher, with a statistically significant difference (P<0.05); Using Pearson correlation analysis, a weak positive correlation was identified between MMP-2, Beclin1, LC3-II, and chemotherapy resistance (r=0.167, 0.197, 0.273, P<0.05), a positive correlation between MMP-2 and MMP-9, Beclin1, LC3-II (r=0.592, 0.852, 0.665, P<0.01), a positive correlation between MMP-9 and Beclin1, LC3-II (r=0.552, 0.472, P<0.01), and a positive correlation between Beclin1 and LC3-II (r=0.647, P<0.01). It was concluded that cinobufotalin has an inhibitory effect on the serum MMP-2, MMP-9, Beclin1, and LC3-II levels in advanced NSCLC patients, which can promote clinical efficacy improvement and reduce the risk of chemotherapy resistance by downregulating MMP-2, Beclin1, and LC3-II levels.
Effects of tibolone combined with zoledronic acid on bone density, bone metabolism, and pain in postmenopausal patients with osteoporosis

Abstract:Objective: To explore the efficacy and safety of tibolone combined with zoledronic acid in the treatment of postmenopausal osteoporosis (PMO). Methods: We conducted a retrospective analysis of 121 PMO patients from March 2019 to July 2021. Patients were divided into two groups based on treatment regimen: an observation group (n=62) receiving zoledronic acid combined with tibolone and a control group (n=59) receiving tibolone monotherapy. We evaluated and compared therapeutic efficacy, bone mineral density, bone metabolism markers (osteocalcin, serum C-terminal telopeptide of type I collagen, and bone alkaline phosphatase), pain, knee joint function, incidence of fragility fractures, and adverse reactions. Logistic regression analysis was used to evaluate risk factors affecting treatment efficacy. Results: The observation group showed a significantly higher therapeutic effect (96.77%) compared to the control group (83.05%), and a lower incidence of fragility fractures (P=0.012). Before treatment, there were no significant differences in bone mineral density, bone metabolism markers, pain status, or knee function between the two groups (all P>0.05). However, after treatment, evaluations showed marked improvements in these parameters in both groups, with more significant enhancements observed in the observation group (all P<0.001). The incidence of adverse reactions did not significantly differ between the groups (20.97% vs 13.56%, P=0.282). Logistic regression analysis identified the use of tibolone combined with zoledronic acid as a protective factor for effective treatment. Conclusions: Tibolone combined with zoledronic acid significantly increases bone mineral density, improves bone metabolism, and reduces pain in PMO patients, with a safety profile comparable to that of monotherapy. This regimen should be considered for clinical use in treating PMO.
Clinical Comparative Study of Zoledronic Acid Versus Pamidronate Disodium in the Treatment of Pain of Skeletal Metastases Lung Cancer

Abstract:The objective of this study was to evaluate the efficacy and safety of zoledronic acid in the treatment of pain of skeletal metastases in lung cancer patients. Fifty patients were randomly assigned to a zoledronic acid group,and pamidronate disodium group. The total rates of efficacy of the zoledronic acid group and pamidronate disodium group in relieves pain of skeletal metastases were 76%(19/25) and 72%(18/25), respectively. There was no significant difference between the two groups, P=0.798. The initial time of effect was 1-11 d (median time, 5 d) in the zoledronic acid group, and 2-13 d (median time, 5 d) in the pamidronate disodium group. There was no significant difference between the two groups,P=0.702. The persistent time of effect was 2-32 days (median time, 24 days) in the zoledronic acid group, and 1-29 days (median time, 23 days) in the pamidronate disodium group. There was no significant difference between the two groups,P=0.509. In conclusion, the zoledronic acid injection, with a pronounced effect, can relieve bone pain caused by metastasis of lung cancer. The curative and adverse effects are similar to those found with pamidronate disodium.
Abiraterone Acetate for Metastatic Castration-Resistant Prostate Cancer after Docetaxel Failure: A Randomized, Double-Blind, Placebo-Controlled Phase 3 Bridging Study

Abstract:Objectives: To evaluate the efficacy and safety of abiraterone acetate-prednisone versus placebo-prednisone in Asian metastatic castration-resistant prostate cancer patients who have failed docetaxel-based chemotherapy. Methods: In this double-blind, phase 3 study from China, 214 patients were randomized (2:1) to abiraterone acetate 1000 mg once daily plus prednisone 5 mg twice daily and placebo plus prednisone 5 mg twice daily in 28-day treatment cycles. Results: Abiraterone acetate-prednisone treatment significantly decreased prostate-specific antigen progression risk by 49%, with longer median time to prostate-specific antigen progression of 5.55 months versus 2.76 months in the placebo-prednisone group (hazard ratio 0.506, P = 0.0001, primary end-point). There was a strong trend for improved overall survival in the abiraterone acetate-prednisone group, with a 40% decrease in the risk of death (hazard ratio 0.604, P = 0.0597); however, median survival was not reached in either group because of the short follow-up period (12.9 months) and limited number of observed death events. The prostate-specific antigen response rate was higher in the abiraterone-prednisone group (49.7%) than in the placebo-prednisone group (14.1%). A total of 37.1% patients in this group had pain progression events compared with 50.7% in the placebo-prednisone group. Abiraterone-prednisone significantly decreased the risk of pain progression by 50% (hazard ratio 0.496, P = 0.0014). The incidence of adverse events was similar between the two groups; the most common adverse events being anemia (25.9% for abiraterone-prednisone vs 22.5% for placebo-prednisone), hypokalemia (25.9% and 11.3%), bone pain (23.8% and 21.1%), hypertension (16.1% and 12.7%) and increased aspartate aminotransferase (14.7% and 15.5%), respectively. Conclusions: Abiraterone-prednisone significantly delays disease and pain progression, and prostate-specific antigen, with a favorable benefit-risk ratio in Asian metastatic castration-resistant prostate cancer patients in the post-docetaxel setting.
Zoledronic Acid Reduces the Need for Radiation to Bone in Patients with Breast or Prostate Cancer Metastatic to Bone: A Survival-Adjusted Cumulative Incidence Analysis

Abstract:8058 Background: Bone metastases are common in patients with breast or prostate cancer and cause significant pain and skeletal morbidity. The need for radiation to bone to treat pain or prevent a fracture is a significant clinical event associated with disease progression and quality-of-life decline. Therefore, radiation to bone was included in the composite endpoints describing skeletal morbidity in recent clinical trials of zoledronic acid. Herein, a statistically robust nonparametric method was used to measure the cumulative burden of radiation to bone over the entire course of follow-up to evaluate the benefit of 4-mg zoledronic acid in patients with breast or prostate cancer enrolled in 2 large, randomized, phase III trials. We have previously reported by this method that zoledronic acid significantly reduces the overall cumulative incidence of skeletal complications compared with placebo or pamidronate (Pam) in these patient populations. Methods: These trials were GCP-compliant, and patients gave written informed consent. Patients received 4 mg zoledronic acid via 15-minute infusion every 3–4 weeks for 2 years. Comparators were 90 mg Pam via 2-hour infusion (breast cancer) or placebo (prostate cancer). Survival-adjusted multiple event analysis was performed via the methods of Cook et al. (Stat Med 1997;16;911–924) and Ghosh and Lin (Biometrics 2000;56:554–562). Results: In patients with breast cancer (n = 766), 4 mg zoledronic acid was superior to 90 mg Pam and significantly reduced the cumulative mean number of administrations of radiation to bone by 35.4% at 25 months (P = .003), adding to the benefit achieved with Pam. Similarly, in patients with prostate cancer, 4 mg zoledronic acid significantly reduced the cumulative mean number of administrations of radiation to bone by 33.2% compared with placebo at 24 months (P = .034). Conclusion: This new, robust method provides a simple graphic representation of the cumulative need for radiation therapy and reveals the significant reduction in need for radiation therapy achieved with zoledronic acid in breast or prostate cancer patients. Abstract:Objective To evaluate the efficacy and safety of zoledronic acid in the treatment of pain of skeletal metastases in cancer patients. Methods A randomized double-blind, multicenter, comparative trial was performed in 210 patients with osteolytic metastases and pains. One hundred and five patients were treated with both zoledronic acid (group A), others were treated with pamidronate disodium (group B).Results The efficacy of group A and B in relieves pain of osteolytic were 71.43% and 65.72%, respectively. There was no significant difference between the two groups (P0.05). The performance status of group A is lower than that of group B on the 7th day after administration. There was significant difference between the two groups (P0.05). The adverse reaction of the group A and B were 46.76% and 52.38%, respectively. No significant difference was found between the two groups(P0.05). Conclusion Zoledronic acid was safe and effective in the treatment of pain patients caused by osteolytic metastases.

Abstract:6063 Background: ZOL is the only bisphosphonate that has demonstrated efficacy for the prevention of skeletal-related events (SREs) in patients with BM in a wide range of tumor types. Recent retrospective analyses also showed that normalization of N-telopeptide of type I collagen (NTX) over 3 months by the treatment of ZOL provided a continuum of SRE risk reduction and survival benefit in patients with BM (Lipton et al. ASCO 2007). Therefore, we conducted the prospective open-label randomized phase II trial to evaluate the changes of NTX after administration of ZOL in NPC patients with BM. Methods: Newly diagnosed NPC patients with BM were randomized to receive chemotherapy of cisplatin (20mg/m2 IV, D1–5) plus FU (500mg/m2 IV, D1–5) (CF regimen, q3wks) and intravenous ZOL (4mg, q4wks, for 3months, CF+ZOL Group) or the same chemotherapy alone (CF Group). Urinary NTX was measured by ELISA method (Coleman et al. JCO 2005) at baseline and 1, 2, 3 months after administration of ZOL in all patients. Results: Sixty patients were enrolled into the study, 30 patients in each group. The median chemotherapy cycles was the same (4 and 4, respectively) in two groups. The median baseline NTX level had no difference between the two groups (75.4 vs. 95.6 nM BCE/mM creatinine, respectively p>0.05). The NTX decreased 65.9% within 1 month in CF+ZOL group, whereas NTX increased 2.61 % in CF group (p<0.01). The median NTX decrease percentage in 2,3 months after treatment were 70.8%, 86.5% in CF+ZOL Group and 15.9%, 34.5% in CF Group respectively (p<0.01, p<0.01). Conclusions: ZOL administered with chemotherapy (CF) consistently reduced NTX levels in NPC patients with BM, indicating potential benefit of ZOL may exist in this group of patients. The value of NTX reduction in NPC patients with BM need to be further studied in larger prospective randomized trials. [Table: see text] [Table: see text]

Abstract:Bone metastasis causes significant pain and morbidity and is characterized by multiple skeletal-related events (SREs), including fractures, spinal cord compression, and the requirement for radiation or surgery to bone. Analysis of such a composite endpoint provides insight into the clinical impact of the disease. However, SREs typically occur in a complex pattern. Recently, zoledronic acid (4 mg) was approved for the treatment of bone metastases secondary to all solid tumors and primary bone lesions from multiple myeloma based on significant benefits from first-event and skeletal morbidity rate analyses. However, multiple-event methods can provide information about the risk of SREs over the entire course of follow-up and address interpatient variation in event rates and temporal trends. Moreover, multiple-event methodology can adjust for survival, which has an effect on SRE incidence. Herein we present a survival-adjusted multiple-event analysis of the cumulative incidence of radiation to bone (a homogenous endpoint) and SREs (a composite endpoint) in 3 large, randomized clinical trials of zoledronic acid (4 mg) in patients with prostate cancer, lung cancer and other solid tumors, or breast cancer. In patients with prostate cancer, zoledronic acid significantly reduced the cumulative incidence of SREs compared with placebo (P = 0.002) as it did among patients with lung cancer and other solid tumors (P = 0.025). In patients with breast cancer, zoledronic acid significantly reduced the cumulative incidence of SREs compared with pamidronate 90 mg over 25 months (P = 0.050). These results are consistent with those obtained using established methods, supporting the validity of the survival-adjusted cumulative mean function for assessing benefits during bisphosphonate therapy.
