Abstract:Diagnosis and treatment of urological tumors, relying on auxiliary data such as medical imaging, while incorporating individual patient characteristics into treatment selection, has long been a key challenge in clinical medicine. Traditionally, clinicians used extensive experience for decision-making, but recent artificial intelligence (AI) advancements offer new solutions. Machine learning (ML) and deep learning (DL), notably convolutional neural networks (CNNs) in medical image recognition, enable precise tumor diagnosis and treatment. These technologies analyze complex medical image patterns, improving accuracy and efficiency. AI systems, by learning from vast datasets, reveal hidden features, offering reliable diagnostics and personalized treatment plans. Early detection is crucial for tumors like renal cell carcinoma (RCC), bladder cancer (BC), and Prostate Cancer (PCa). AI, coupled with data analysis, improves early detection and reduces misdiagnosis rates, enhancing treatment precision. AI's application in urological tumors is a research focus, promising a vital role in urological surgery with improved patient outcomes. This paper examines ML, DL in urological tumors, and AI's role in clinical decisions, providing insights for future AI applications in urological surgery.

Abstract:With the rapid increase in computer processing capacity over the past two decades, machine learning techniques have been applied in many sectors of daily life. Machine learning in therapeutic settings is also gaining popularity. We analysed current studies on machine learning in robotic urologic surgery. We searched PubMed/Medline and Google Scholar up to December 2023. Search terms included "urologic surgery", "artificial intelligence", "machine learning", "neural network", "automation", and "robotic surgery". Automatic preoperative imaging, intraoperative anatomy matching, and bleeding prediction has been a major focus. Early artificial intelligence (AI) therapeutic outcomes are promising. Robot-assisted surgery provides precise telemetry data and a cutting-edge viewing console to analyse and improve AI integration in surgery. Machine learning enhances surgical skill feedback, procedure effectiveness, surgical guidance, and postoperative prediction. Tension-sensors on robotic arms and augmented reality can improve surgery. This provides real-time organ motion monitoring, improving precision and accuracy. As datasets develop and electronic health records are used more and more, these technologies will become more effective and useful. AI in robotic surgery is intended to improve surgical training and experience. Both seek precision to improve surgical care. AI in ''master–slave'' robotic surgery offers the detailed, step-by-step examination of autonomous robotic treatments.

Abstract:Artificial intelligence (AI) techniques like artificial neural networks, Bayesian belief networks and neurological fuzzy systems are widely adopted in urology.AI approach are found to be more exploratory as well as accurate in the terms of prediction compared to conventional statistics. The model is complex mathematical based models derived from working of human brain. A detailed study conducted in the field of urology using a technique resulted in finding new dynamic applications in the field of neurological cancer treatment. The results of machine learning techniques and implementation were focused on handling as well as prediction using artificial neural networks for the purpose of diagnosis, prognosis and treatment of cancer. Different techniques of AI depending on respective characteristics, where found suitable for different tasks.Also,the lack of transparency in neural networks was also overcome using neuro-fuzzy systems.

Abstract:Recently, advances in machine learning and neural network technology have allowed artificial intelligence (AI) to further promote guidance of clinical diagnosis, treatment and resource expenditures. In genitourinary cancers, AI has made huge progress in improving the diagnosis and treatment of prostate, kidney and bladder cancers. Numerous studies have developed methods to utilize neural networks to automate prognostic prediction, treatment plan optimization and patient follow-up education. Obviously, AI guidance could markedly reduce the subjectivity of diagnosis and treatment management of genitourinary cancers. However, although the application of AI in cancer treatment has become a research hotspot in modern technology, there still exist obvious limitations of AI management when compared with real-world clinical strategies. Therefore, this article summarized the current advantages and disadvantages of AI to provide novel insights for the future application of AI in the precision, personalized diagnosis and treatment, and long-term management of both patients and urologists.

This article explores the transformative impact of AI in the field of uropathology, with a particular focus on its application in diagnosing, grading, and prognosticating various urological cancers. AI, especially deep learning algorithms, has shown significant potential in improving the accuracy and efficiency of pathology workflows. Abstract:Regular and Frequent checking of x-ray machines is needed to assure accurate dosage values in therapy. Theoretically, the best solution is to measure the output intensity with a Victoreen or similar dosimeter at least once weekly. Unfortunately, the nature of such instruments and measurements normally prevents delegating this task to a technician, and frequent regular checking appears to be limited to larger hospitals and teaching institutions. A more rugged and inexpensive but still accurate instrument, designed specifically for this purpose, is desirable. Abstract:<h3 class="Heading">Purpose</h3><p class="Para">The purpose of the study was to provide a comprehensive review of recent machine learning (ML) and deep learning (DL) applications in urological practice. Numerous studies have reported their use in the medical care of various urological disorders; however, no critical analysis has been made to date.</p><h3 class="Heading">Methods</h3><p class="Para">A detailed search of original articles was performed using the PubMed MEDLINE database to identify recent English literature relevant to ML and DL applications in the fields of urolithiasis, renal cell carcinoma (RCC), bladder cancer (BCa), and prostate cancer (PCa).</p><h3 class="Heading">Results</h3><p class="Para">In total, 43 articles were included addressing these four subfields. The most common ML and DL application in urolithiasis is in the prediction of endourologic surgical outcomes. The main area of research involving ML and DL in RCC concerns the differentiation between benign and malignant small renal masses, Fuhrman nuclear grade prediction, and gene expression-based molecular signatures. BCa studies employ radiomics and texture feature analysis for the distinction between low- and high-grade tumors, address accurate image-based cytology, and use algorithms to predict treatment response, tumor recurrence, and patient survival. PCa studies aim at developing algorithms for Gleason score prediction, MRI computer-aided diagnosis, and surgical outcomes and biochemical recurrence prediction. Studies consistently found the superiority of these methods over traditional statistical methods.</p><h3 class="Heading">Conclusions</h3><p class="Para">The continuous incorporation of clinical data, further ML and DL algorithm retraining, and generalizability of models will augment the prediction accuracy and enhance individualized medicine.</p>

The long-used nomograms and other classic forecasting approaches are being reconsidered considering AI's capabilities. This review emphasizes the coming integration of artificial intelligence into healthcare settings while critically examining the most recent and significant literature on the subject. This study seeks to define the status of AI and its potential for the future, with a special emphasis on how AI can transform bladder cancer diagnosis and treatment.

Abstract:Being the graft of choice, the internal mammary artery (IMA) is increasingly used for coronary artery bypass grafting. Presently, electrocautery is liberally used to mobilize both IMAs. Preservation of IMA flow surface was evaluated by scanning electron microscopy (SEM) after mobilization with either sharp dissection or electrocautery in five clinical patients undergoing coronary artery bypass surgery. Major side branches were secured with metallic clips. Specimens for SEM were harvested after heparinization but before institution of cardiopulmonary bypass from the terminal branches of both IMAs after they had been skeletonized with either mobilization method. When mobilization of the IMA was performed as a pedicle, the flow surface was well preserved in both groups. However, if a contact of the electrocautery blade with the wall of the IMA or with a metallic clip parallell to the wall was allowed, a clearly visible zone of endothelial damage, sometimes associated with mural trombus formation was observed. In conclusion, caution is needed with electrocautery. Thermal injuries of the IMA may remain undetected during surgery and cause graft occlusion later.

Abstract:Purpose of review: The aim of the present manuscript is to provide an overview on the current state of artificial intelligence (AI) tools in either decision making, diagnosis, treatment options, or outcome prediction in functional urology. Recent findings: Several recent studies have shed light on the promising potential of AI in functional urology to investigate lower urinary tract dysfunction pathophysiology but also as a diagnostic tool by enhancing the existing evaluations such as dynamic magnetic resonance imaging or urodynamics. AI may also improve surgical education and training because of its automated performance metrics recording. By bringing prediction models, AI may also have strong therapeutic implications in the field of functional urology in the near future. AI may also be implemented in innovative devices such as e-bladder diary and electromechanical artificial urinary sphincter and could facilitate the development of remote medicine. Summary: Over the past decade, the enthusiasm for AI has been rising exponentially. Machine learning was well known, but the increasing power of processors and the amount of data available has provided the platform for deep learning tools to expand. Although the literature on the applications of AI technology in the field of functional urology is relatively sparse, its possible uses are countless especially in surgical training, imaging, urodynamics, and innovative devices.

Abstract:Artificial intelligence (AI) — the ability of a machine to perform cognitive tasks to achieve a particular goal based on provided data — is revolutionizing and reshaping our health-care systems. The current availability of ever-increasing computational power, highly developed pattern recognition algorithms and advanced image processing software working at very high speeds has led to the emergence of computer-based systems that are trained to perform complex tasks in bioinformatics, medical imaging and medical robotics. Accessibility to 'big data' enables the 'cognitive' computer to scan billions of bits of unstructured information, extract the relevant information and recognize complex patterns with increasing confidence. Computer-based decision-support systems based on machine learning (ML) have the potential to revolutionize medicine by performing complex tasks that are currently assigned to specialists to improve diagnostic accuracy, increase efficiency of throughputs, improve clinical workflow, decrease human resource costs and improve treatment choices. These characteristics could be especially helpful in the management of prostate cancer, with growing applications in diagnostic imaging, surgical interventions, skills training and assessment, digital pathology and genomics. Medicine must adapt to this changing world, and urologists, oncologists, radiologists and pathologists, as high-volume users of imaging and pathology, need to understand this burgeoning science and acknowledge that the development of highly accurate AI-based decision-support applications of ML will require collaboration between data scientists, computer researchers and engineers.

Abstract:Introduction and objective: Artificial intelligence (AI) is in full development and its implementation in medicine has led to an improvement in clinical and surgical practice. One of its multiple applications is surgical training, with the creation of programs that allow avoiding complications and risks for the patient. The aim of this article is to analyze the advantages of AI applied to surgical training in urology. Material and methods: A literary research is carried out to identify articles published in English regarding AI applied to medicine, especially in surgery and the acquisition of surgical skills. Results: Surgical training has evolved over time thanks to AI. A model for surgical learning where skills are acquired in a progressive way while avoiding complications to the patient, has been created. The use of simulators allows a progressive learning, providing trainees with procedures that increase in number and complexity. On the other hand, AI is used in imaging tests for surgical or treatment planning. Conclusion: Currently, the use of AI in daily clinical practice has led to progress in medicine, specifically in surgical training.

Abstract:Introduction: Since the significant breakthroughs in artificial intelligence (AI) algorithms, the application of AI in bladder cancer has rapidly expanded. AI can be used in all aspects of the bladder cancer field, including diagnosis, treatment and prognosis prediction. Nowadays, these technologies have an excellent medical auxiliary effect and are in explosive development, which has aroused the intense interest of researchers. This study will provide an in-depth analysis using bibliometric analysis to explore the trends in this field. Method: Documents regarding the application of AI in bladder cancer from 2000 to 2022 were searched and extracted from the Web of Science Core Collection. These publications were analyzed by bibliometric analysis software (CiteSpace, Vosviewer) to visualize the relationship between countries/regions, institutions, journals, authors, references, keywords. Results: We analyzed a total of 2368 publications. Since 2016, the number of publications in the field of AI in bladder cancer has increased rapidly and reached a breathtaking annual growth rate of 43.98% in 2019. The U.S. has the largest research scale, the highest study level and the most significant financial support. The University of North Carolina is the institution with the highest level of research. EUROPEAN UROLOGY is the most influential journal with an impact factor of 24.267 and a total citation of 11,848. Wiklund P. has the highest number of publications, and Menon M. has the highest number of total citations. We also find hot research topics within the area through references and keywords analysis, which include two main parts: AI models for the diagnosis and prediction of bladder cancer and novel robotic-assisted surgery for bladder cancer radicalization and urinary diversion. Conclusion: AI application in bladder cancer is widely studied worldwide and has shown an explosive growth trend since the 21st century. AI-based diagnostic and predictive models will be the next protagonists in this field. Meanwhile, the robot-assisted surgery is still a hot topic and it is worth exploring the application of AI in it. The advancement and application of algorithms will be a massive driving force in this field.

Abstract:This comprehensive review critically examines the transformative impact of artificial intelligence (AI) and radiomics in the diagnosis, prognosis, and management of bladder, kidney, and prostate cancers. These cutting-edge technologies are revolutionizing the landscape of cancer care, enhancing both precision and personalization in medical treatments. Our review provides an in-depth analysis of the latest advancements in AI and radiomics, with a specific focus on their roles in urological oncology. We discuss how AI and radiomics have notably improved the accuracy of diagnosis and staging in bladder cancer, especially through advanced imaging techniques like multiparametric MRI (mpMRI) and CT scans. These tools are pivotal in assessing muscle invasiveness and pathological grades, critical elements in formulating treatment plans. In the realm of kidney cancer, AI and radiomics aid in distinguishing between renal cell carcinoma (RCC) subtypes and grades. The integration of radiogenomics offers a comprehensive view of disease biology, leading to tailored therapeutic approaches. Prostate cancer diagnosis and management have also seen substantial benefits from these technologies. AI-enhanced MRI has significantly improved tumor detection and localization, thereby aiding in more effective treatment planning. The review also addresses the challenges in integrating AI and radiomics into clinical practice, such as the need for standardization, ensuring data quality, and overcoming the "black box" nature of AI. We emphasize the importance of multicentric collaborations and extensive studies to enhance the applicability and generalizability of these technologies in diverse clinical settings. In conclusion, AI and radiomics represent a major paradigm shift in oncology, offering more precise, personalized, and patient-centric approaches to cancer care.
