Abstract:Objective To summarize the results of surgical management of different types of transposition of great arteries with different procedures.Methods Two hundred and seventy-five patients with transposition of great arteries and other congenital heart defects (ventricular septal defect,pulmonary artery stenosis,atrial septal defect,patent ductus arteriosus,interrupted inferior vana cava,persistent left superior vena cava,coronary artery anomalies and coarctation of the aortic arch) were surgically corrected.Different procedures,such as arterial switch operation,bidirectional cavo-pulmonary anastomosis,Rastelli procedure,Fontan operation and Senning operation,were adopted according to different malformations.Clinical data was reviewed and postoperative survival was evaluated.Results There were 14 cases of early deaths (5.09％) in this group with 10 cases occurred after arterial switch operation (10/243,4.12％) and 4 cases following other procedures (4/32,12.50％).Surviving patients following Glenn/modified Fontan procedures had significant improvement in oxygen saturation.Patients who undertook arterial switch operation and Senning operations could grow and develop normally.Conclusions Complete transposition of great arteries can be treated with a variety of procedures according to the combined anomalies.Satisfactory results can be achieved in neonates with arterial switch operation.

Abstract:Background: Arterial switch operation (ASO) has been optimal for children with transposition of great arteries (TGA) in either simple or complex form with an excellent survival rate. This operation was introduced late in China, but there has been a decreasing mortality in recent years. Optimizing the postoperative management has been essential to improve the survival rate after ASO. This study summarizes the experience in the management of the postoperative complications after ASO. Methods: Twenty-eight infants with TGA underwent ASO from January 2004 through December 2006. These patients aged 1-70 days (median, 6 days) had a body weight of 3.36±0.57 kg on average. Before operation, continuous intravenous infusion of prostaglanding E1 was routinely used to keep the ductus open with a SpO2 of 75%-90%. Ten patients required tracheal intubation and mechanical ventilation. Two patients underwent emergency ASO under general anesthesia and low-flow cardiopulmonary bypass or hypothermic circulatory arrest. All the 28 patients were further treated with modified ultrafiltration and delayed sternal closure after cessation of cardiopulmonary bypass. After ASO, they were on circulatory and respiratory support, with antibiotics, nutritional supplement in the ICU. Results: The patients had a cardiopulmonary bypass time of 167±32 minutes and an aortic crossclamping time of 101±24 minutes. Delayed sternal closure was carried out on 3.63±1.49 days after ASO. They had a mechanical ventilation of 5.89±3.02 days and an ICU stay of 10.12±3.25 days. There were 5 deaths after ASO with a mortality rate of 17.9%. Fifteen patients developed low cardiac output syndrome. In 12 patients presenting with cardiac arrhythmias, 9 had paroxysmal supraventricular tachycardia, 1 had frequent ventricular premature beats, 1 had ventricular tachycardia, and 1 had ventricular fibrillation. One patient suffered from pulmonary hypertensive crisis. Three patients with major bleeding and tamponade required emergency mediastinal exploration and finally survived. There were 6 patients with ventilatorassociated pneumonia, 6 with delayed incision healing, and 1 with chylothorax. Conclusions: The complications after ASO are common and complicated. Understanding of the physiological characteristics of infants and the pathophysiological changes, and optimizing postoperative treatment help to improve the survival rate after ASO.

Abstract:IntroductIon Complete transposition of the great arteries (TGAs) is a common cyanotic congenital heart defect, with an incidence rate of 0.02%, accounting for 5–7% of congenital heart diseases.[1] After birth, infants with TGA largely rely on ventricular septal defect (VSD), atrial septal defect (ASD), and patent ductus arteriosus (PDA) to sustain life. Abstract:Dextro-transposition of the great arteries (D-TGA) is the second most common cyanotic heart disease, accounting for 5-7% of all congenital heart defects (CHDs). It is characterized by ventriculoarterial (VA) connection discordance, atrioventricular (AV) concordance, and a parallel relationship with D-TGA. As a result, the pulmonary and systemic circulations are separated [the morphological right ventricle (RV) is connected to the aorta and the morphological left ventricle (LV) is connected to the pulmonary artery]. This anomaly is included in the group of developmental disorders of embryonic heart conotruncal irregularities, and their pathogenesis is multifactorial. The anomaly's development is influenced by genetic, epigenetic, and environmental factors. It can occur either as an isolated anomaly, or in association with other cardiac defects. The typical concomitant cardiac anomalies that may occur in patients with D-TGA include ventriculoseptal defects, patent ductus arteriosus, left ventricular outflow tract obstruction (LVOTO), mitral and tricuspid valve abnormalities, and coronary artery variations. Correction of the defect during infancy is the preferred treatment for D-TGA. Balloon atrial septostomy (BAS) is necessary prior to the operation. The recommended surgical correction methods include arterial switch operation (ASO) and atrial switch operation (AtrSR), as well as the Rastelli and Nikaidoh procedures. The most common postoperative complications include coronary artery stenosis, neoaortic root dilation, neoaortic insufficiency and neopulmonic stenosis, right ventricular (RV) outflow tract obstruction (RVOTO), left ventricular (LV) dysfunction, arrhythmias, and heart failure. Early diagnosis and treatment of D-TGA is paramount to the prognosis of the patient. Improved surgical techniques have made it possible for patients with D-TGA to survive into adulthood.

Abstract:OBJECTIVE:To evaluate transesophageal echocardiography (TEE)-guided periventricular device closure for treatment of ventricular septal defects (VSD) in children.METHODS:The Amplatzer device was applied, the size of which was 1 or 2 mm larger than the VSD size assessed by TEE. Via a small lower sternotomy, the device was introduced through right ventricular surface to close the VSD under continuous TEE guidance without cardiopulmonary bypass.RESULT:The procedures were successful in 6 cases, including 4 cases with muscular VSD and 2 cases with perimembranous VSD. The diameter of VSD was 3 mm to 10 mm and one case had pulmonary hypertension. One case of multi-muscular VSD with transposition of the great arteries failed to the treatment because the mitral open was interfered by the device. One case had bundle branch block in ICU stay and returned to normal without special therapy. The average operation time was 30 min and blood loss was 5 approximately 20 ml, no patients required blood transfusion; the average length of ICU stay was 24 h. Patients were discharged 1 to 2 d postoperatively. At a median follow-up for 12 months, all patients were asymptomatic and had no residual ventricular shunts.CONCLUSION:With the guide of TEE, intra-operative hybrid therapy is a safe and effective method for closure of muscular and perimembranous VSD without cardiopulmonary bypass in children.

Abstract:Premature closure of the ductus arteriosus (PCDA) and transposition of the great arteries (TGA) cause persistent pulmonary hypertension of the newborn (PPHN). We present a case of a newborn who demonstrated d-TGA with ventricular septal defect (VSD) and pulmonary stenosis (PS) complicated by PCDA. The neonate showed severe cyanosis resistant to resuscitation soon after birth, and was diagnosed with d-TGA with VSD by echocardiography. PPHN was also suspected based on physical symptoms and results of echocardiography. The neonate was given inhaled nitric oxide, prostaglandin E1, and catecholamines under sedation, and underwent a balloon atrial septostomy (BAS). His condition gradually improved, and he was extubated on day 7, but his pulmonary subvalvular stenosis gradually worsened and pulmonary blood flow was markedly decreased. A second BAS was performed on day 27 and he showed no improvement. Blalock-Taussig shunt surgery was performed on day 34, which markedly improved his condition. The co-existence of d-TGA and PCDA is generally a lethal state. In our patient, an increase in pulmonary blood flow during the fetal period was restricted because of PS and outlet flow from the left ventricle to the right ventricle via the VSD. This restricted blood flow through the ductus arteriosus, which led to narrowing of the DA. At the same time, damage to and constrictive changes of the pulmonary vessels were prevented. The ductus arteriosus should be carefully evaluated to exclude PCDA in cases of d-TGA. The presence of both VSD and PS may be a prognostic factor in such cases.

Abstract:Background. The outcomes, therapeutic strategies, and risk factors of congenital corrected transposition of great arteries (ccTGA) unoperated before adulthood are unclear. Methods. From October 2009 to January 2018, 117 adult ccTGA patients, classified into ccTGA with intact ventricular septum, ventricular septum defect, and pulmonary valve or subpulmonary outflow tract stenosis (PS) groups, were reviewed. Statistical analysis was performed with SPSS 19.0 (IBM, Armonk, NY). Results. At the first visit, no patients suffered operation. The PS group had the least systemic atrioventricular valve regurgitation and the greatest systemic ventricular ejection fraction. All 49 patients underwent surgery. From the first visit to last follow-up, systemic ventricular ejection fraction of unoperated ccTGA decreased significantly. In the intact ventricular septum group, patients receiving systemic atrioventricular valve replacement/valvuloplasty had a significantly increased systemic ventricular ejection fraction and statistically more freedom from death and transplant than unoperated. In the ventricular septum defect group the late systemic ventricular ejection fraction of operated patients was not statistically different from their basic data at first visit. In the PS group patients receiving physiologic repair had significantly decreased systemic ventricular ejection fractions. Severe systemic atrioventricular valve regurgitation, physiologic repair, and systemic ventricular dysfunction (ejection fraction <40%) were risk factors for mortality, transplant, and congestive heart failure. Conclusions. PS protects against systemic atrioventricular valve regurgitation and ventricular dysfunction. Systemic atrioventricular valve replacement/valvuloplasty improved systemic ventricular function for ccTGA with an intact ventricular septum. Physiologic repair was not ideal for ccTGA with PS. Severe systemic atrioventricular valve regurgitation and systemic ventricular dysfunction were associated with suboptimal outcomes. (C) 2021 by The Society of Thoracic Surgeons
Integrated prenatal and postnatal management for neonates with transposition of the great arteries: thirteen-year experience at a single center

Abstract:Background: Transposition of the great arteries (TGA) is the most common cyanotic congenital heart defect in neonates but with low prenatal detection rate. This study sought to review the prenatal diagnosis, associated abnormalities, and mid-term postnatal outcomes of fetuses with TGA and investigate the integrated prenatal and postnatal management for TGA neonates. Methods: A total of 134 infants prenatally diagnosed with TGA in Guangdong Provincial People's Hospital, China, from January 2009 to December 2022 were included in the study. The prenatal ultrasound data and neonatal records were reviewed to assess the accuracy of prenatal diagnosis. Univariate and multivariate logistic and Cox analyses were used to identify risk factors associated with prognosis in such individuals. Results: The population originated from 40 cities in 10 provinces in China, with integrated antenatal and postnatal management rate reaching 94.0% (126/134) and a high accuracy rate (99.3%) of prenatal primary diagnosis. The median period of follow-up was 1.6 [interquartile range (IQR) 0.1-4.3] years. There were 3 (2.2%) postnatal deaths, 118 (88.1%) patients undergoing arterial switch operation (ASO), 3 (2.2%) undergoing Rastelli operations and 5 (3.7%) doing stage operations. Of 118 patients receiving ASO, the major morbidity occurred in 64 patients (54.2%), and right ventricular outflow tract obstruction (RVOTO) in 31 (26.3%). In the multivariate logistic analysis, gestational ages at birth (OR = 0.953, 95% CI 0.910-0.991; p = 0.025) and cardiopulmonary bypass (CPB) time (OR = 1.010, 95% CI 1.000-1.030; p = 0.038) were identified as independent risk factors associated with major morbidity. In the Cox multivariate analysis, aortic cross-clamping time (HR = 1.030, 95% CI 1.000-1.050; p = 0.017) was identified as independent risk factor associated with RVOTO. Conclusion: Earlier gestational ages at birth and longer CPB time are significantly associated with increased morbidity. Integrated prenatal and postnatal management is recommended for patients with prenatal diagnosis of TGA.

Abstract:D-transposition of the great arteries (D-TGA) is the most commonly diagnosed cyanotic congenital heart disease presenting in the neonatal period. The survival after an arterial switch operation, with freedom from adverse cardiovascular events, has been reported to be as high as 93% at 25 years. However, despite excellent surgical outcomes, there continues to be significant preoperative morbidity and potential mortality due to compromise in the delivery room from foramen ovale closure requiring urgent balloon atrial septostomy for stabilization in the first minutes of life. The prenatal diagnosis of D-TGA using fetal echocardiography has aided in the perinatal management and delivery planning of these infants, lowering preoperative morbidity and mortality and preventing delivery room compromise. Fetuses with D-TGA have more highly oxygenated blood supplying the pulmonary arteries and ductus arteriosus which likely results in ductal constriction and increased pulmonary blood flow. This may be the cause of foramen ovale restriction or closure in-utero, which then increases the risk for postnatal compromise at delivery. Theories regarding the cause of the abnormal pulmonary vasculature that may be seen in D-TGA, including aorto-pulmonary collateral formation, have been proposed but to our knowledge, observation of the ultrasound findings throughout mid and late gestation describing the progression of the abnormal fetal physiology have not been previously described. We present a case of D-TGA in which serial assessment using fetal echocardiography enabled observation of the in-utero progression of disease, predicting postnatal compromise and facilitating the planning of life-saving specialized delivery room care and intervention.

Abstract:Congenitally corrected transposition of the great arteries (ccTGA) is a rare malformation with diverse morphology. We assessed features of fetuses with ccTGA and evaluated neonatal and pediatric outcomes. This was a retrospective review of fetuses with ccTGA at Birmingham Women's and Children's Hospital born from 2005 to 2019. Of thirty-six fetuses identified, six had unavailable prenatal data, one was postnatally diagnosed with isomerism and 29 fetuses were evaluated. ccTGA without associated cardiac lesions was found in 28% (8/29), ccTGA with significant VSD in 31% (9/29), ccTGA with pulmonary obstruction in 24% (7/29) and ccTGA with complex anomalies in 17% (5/29). Tricuspid regurgitation (TR) was observed in 17% (5/29) and heart block (HB) in 10% (3/29) prenatally. Six, that is 21% underwent genetic testing of which one was abnormal. Five extra-cardiac anomalies were reported prenatally and postnatally. Pregnancy was discontinued in five, of which two had moderate TR. There were thirty-one liveborn. Coarctation of the aorta was found in five postnatally but not suspected prenatally. In one, pulmonary stenosis was underestimated; otherwise, prenatal morphology was confirmed. Cardiac interventions were performed in 77% (24/31) liveborn with 39% (12/31) undergoing neonatal intervention. Overall, 6/31 liveborn died including all three with prenatal heart block and one with TR. Estimated survival for all liveborn at 1, 5 and 10 years was 87% (95% CI 76–100%), 83% (95% CI 72–98%) and 80% (95% CI 66–96%) respectively. Accurate prenatal diagnosis of ccTGA is critical for counseling. Early outcomes are favorable with 77% of liveborn undergoing surgery. Fetuses with prenatal diagnosis of complex associated abnormalities, HB and TR appear to do less well.

Abstract:Transposition of the great arteries (TGA) is a serious and complicated congenital heart disease, accounting for 5% to 7% of all congenital heart malformations. Most fetuses with TGA can survive stably to delivery. After birth, their survival depends on the associated ventricular septal defect (VSD), atrial septal defect (ASD) or patent ductus arteriosus (PDA). Without treatment, approximately 30% of these infants die in the first week after birth and 90% die within the first year.[1] Arterial switch operation is now recognized worldwide as an effective treatment for TGA and good results have been achieved with this approach. ASO was performed late at our center, with a few patients and high mortality rates in the early stage. In recent years, both the number of patients and the survival rate have increased with the gradual improvement of the skills of the surgical team. More patients have been diagnosed prenatally with the development and promotion of fetal echocardiography. From pregnancy to delivery to surgery, children with TGA require monitoring with close cooperation among medical staff from multiple disciplines. In this study, the data of 87 patients who underwent ASO from January 2014 to December 2019 at our center were retrospectively summarized to analyze the major nonsurgical factors affecting patient prognosis [Table 1]. The study was approved by the Ethics Committee of Shandong Provincial Hospital.

Abstract:Complete transposition of the great arteries (TGA) is the most common cause of cyanosis in the first week of life. Prenatal diagnosis helps with counseling, planning delivery, and postnatal management for resource‐rich health services. In a resource‐limited setting, postnatal diagnosis is the norm. This work examines cases of complete TGA in one center in Cambodia without prenatal testing. Twenty‐four cases were studied over 5 years. Complex TGA was frequently seen. The majority had normal coronary anatomy and arterial switch operation (ASO) was performed in most cases with a favorable outcome.

Abstract:Background Extracorporeal membrane oxygenation (ECMO) support on D-transposition of the great arteries (D-TGA) carries formidable challenges. Methods A retrospective study was performed on pediatric patients with D-TGA supported by ECMO from July 2007 to December 2019. This study summarized the clinical experience of ECMO support in pediatric patients with D-TGA preoperative, intraoperative, and postoperative. Results Overall, 16 children with D-TGA received ECMO support during this period. Two (2 of 16) were supported before cardiac surgery, 3 (3 of 16) were supported postoperatively in the intensive care unit, and 11 (11 of 16) failed to wean off cardiopulmonary bypass. Two cases of preoperative ECMO support for patients with D-TGA with an intact ventricular septum and restrictive atrial septum due to severe hypoxemia died. In this study, D-TGA with coronary artery malformation and other complicated deformities died (8 of 14), whereas uncomplicated D-TGA without coronary artery malformation all survived (6 of 14). The wean-off rate of ECMO patients supported in D-TGA was 62.5% (10 of 16), while the 30-day survival rate was 44% (7 of 16). Conclusion Although a promising ECMO weaning rate was obtained, 30-day survival of this population was frustrating, mainly attributed to the original anatomy of coronary arteries and the concomitant deformities.

Abstract:Objective: To study the experience and efficacy of 32 patients with congenital heart disease combined with severe pulmonary hypertension. Methods:The clinical data of 32 patients were studied retrospectively. All the patients showed cyanosis after physical activity and accepted echocardiogram measure showed bi-directed shunt at the defects. The intracardiac malformation included ventricular septal defects (VSD) 22 cases, VSD with atrial septal defects (ASD) 5 cases, VSD with patent ductus arteriosus (PDA) 5 cases. Among 32 cases, 6 cases were accepted the operation on cardioplegic heart arrested, while 26 on beating heart. Using auto-pericardium repaired ASD and made a valve that allowed only right-to-left shunt. If no ASD, the valve was done at the oval fosse. Results: There were 3 cases of early surgical deaths (9.4%, 3/32). Every patient accompanied with hypoxia, and need more long mechanical support but no case needs tracheolaryngotomy. There were 2 (6. 3%, 2/32) cases of late deaths and 3 (9.4%, 3/32) cases of late heart function failure followed-up from two months to eighty-six months. The other patients (75%, 24/32) recovered well. Conclusion: On-pump with beating heart is an effective myocardial protective technique in patients with congenital heasrt disease combined with severe pulmonary hypertension. It contribute to early recovery for the patients with severe pulmonary hypertension by doing the valves on atrial septum.
Surgical Treatment of Tetralogy of Fallot with Subarterial Ventricular Septal Defect

Abstract:Objective To declare the unique anatomy,surgical management and prognosis of tetralogy of Fallot with subarterial ventricular septal defect (VSD). Methods From November 2002 to April 2009, 150 cases of typical tetralogy of Fallot were performed operations, 13 of which had subarterial VSD. Results Morphology and sites of right ventricular outflow obstruction in 13 cases: 2 cases isolated infundibular stenosis, 3 cases infundibular + pulmonary stenosis and 8 cases infundibular + pulmonary stenosis + valve ring stenosis. The diameter of subarterial VSD ranged from 1.5 to 3.0 cm. The proportion of aorta lying above the right ventricle varied from 30% to 50%. Twelve cases cured, one died of low cardiac output syndrome. Early postoperative mortality was 7.7% ( 1/13 ). The follow-up duration ranged from 1 month to 6 years. All cases had no cyanosis and the active ability were improved. Echo showed residual stenosis in 4 cases,residual VSD in 1 case and aortic regurgitation in 1 case. Conclusions Tetralogy of Fallot with subarterial VSD is associated with worse functional outcome. To prevent adverse outcomes, precise suturing of the distal ventricular septal patch, extensive infundibulectomy,lower threshold for transannular incision, and smaller-sized ventricular septal patch placement are warranted.

Abstract:The combination of persistent pulmonary hypertension of the newborn (PPHN) and transposition of the great arteries (TGA) has serious impacts on treatment and prognosis, often with adverse outcomes. We report the case of a male full-term newborn with TGA with intact ventricular septum and severe PPHN who died 2 h after birth; further, we examined his vascular histology. On autopsy, lung histology showed mild fibrous hypertrophy in the intima and moderate medial hypertrophy of the minimal pulmonary artery. Hypoplasia of the pulmonary artery was not detected. Pulmonary congestion was detected and pneumatization was poor. Debris was present in the alveoli. Hemosiderin deposition was detected, suggesting prenatal hemostasis or hemorrhage. Severe PPHN may have occurred because of pulmonary arterial spasm accompanying pulmonary congestion which had been in the fetal stage. A wide range of lesions can be present in the pulmonary vascular bed in TGA. The pathologies of pulmonary vascular tissues with TGA and PPHN are not uniform. <Learning objective: Neonates with transposition of the great arteries with intact ventricular septum (TGA/IVS) may have severe pulmonary hypertension. The progression of pulmonary vascular disease in TGA/IVS is unpredictable; therefore, the delivery of a fetus diagnosed with TGA/IVS should be performed at an institution in which catheterization and balloon atrial septostomy can be performed.>.

Abstract:Background: Total anomalous pulmonary venous connection (TAPVC) to left superior vena cava (LSVC) is an extremely rare congenital heart disease, and its surgical management is very challenging. Case summary: We report one such case of a 5-year-old south Asian male with double outlet right ventricle and unbalanced atrioventricular canal defect, where all the pulmonary veins were found opening into LSVC, which was then opening into the left side of the common atrium. Intraoperatively, the LSVC was transected just below the left internal jugular vein and left subclavian vein junction and left-sided bidirectional Glenn shunt done using 8 mm Dacron tube graft. Pulmonary veins were left draining through the LSVC into the common atrium. Right-sided Glenn shunt was completed as usual. Currently, the patient is year and half post-surgery and is doing well; school going on par with the peer group maintaining a room air saturation of 87%. Discussion: Here, we report a successful surgical correction of TAPVC to LSVC in a child with univentricular physiology, however due to the paucity of data and rarity of such cases, optimal surgical management is yet to be defined.

Medical physicists should seek employment elsewhere when resources are insufficient for quality patient care.
