Abstract:In this paper, a detailed method for computing the transient voltage distribution in underground cables is described. The proposed model is based on RLCG lumped frequency-dependent parameters for time-domain analysis. The Numerical Laplace Transform technique in s-complex plane is used. The method is proposed for underground cable systems working at high frequencies (HFs). The real lossy earth return path is computed by means of the novel Machado's technique successfully applied for transients for first time. Numerical results confirm very good agreement with reference solutions taken from the literature. The proposed numerical technique is specially recommended for computing very fast transients as a better alternative than those available in commercial packages.
Clinical Characteristics, Diagnosis, and Predictors of Neurosyphilis Patients with Human Immunodeficiency Virus Co-Infection

Abstract:ABSTRACT:This study aimed to compare between the clinical and laboratory characteristics of neurosyphilis and those of syphilis in human immunodeficiency virus (HIV) positive and explore the risk factors associated with the occurrence of neurosyphilis in the HIV infected.In-patients diagnosed with HIV and syphilis co-infection who underwent a lumbar puncture and completed cerebrospinal fluid (CSF) examination were divided into neurosyphilis group and syphilis group. The demographic characteristics, symptoms and signs, and laboratory tests of the 2 groups were comparatively analyzed. Logistic regression analysis was used to explore the risk factors associated with the occurrence of neurosyphilis.Among 81 patients, 33 patients were assigned to the neurosyphilis group, and 48 to the syphilis group. There were no significant differences in the age, gender, marital status, acquired immunodeficiency syndrome course, opportunistic infections, serum HIV viral load, and history of syphilis treatment. The difference in HIV transmission route between the 2 groups was statistically significant (P = .010), and the patients from the neurosyphilis group were mainly infected via heterosexual contact. The proportion of serum toludine red unheated serum test (TRUST) titer ≥1:16 in the neurosyphilis group were 78.8%, which was significantly higher compared to the syphilis group (48.9%). The level of CSF white blood cell count, CSF protein, and CSF HIV viral load in the neurosyphilis group were significantly higher than those of the syphilis group. The proportion of patients with neurological symptoms and signs in the neurosyphilis group was significantly higher compared to the syphilis group (P < .001). Multivariate logistic regression analysis showed that heterosexual contact transmission route, not received antiretroviral therapy, lower CD4 cell count and higher serum TRUST titer, untreated with syphilis, and neurological symptoms and signs were risk factors associated with the occurrence of neurosyphilis.The serum TRUST titer, CSF white blood cell count, CSF protein level, CSF HIV viral load, and the percentage of neurological symptoms and signs in the neurosyphilis group were higher. Heterosexual transmission route, not received antiretroviral therapy, and untreated with syphilis prompted the possibility of neurosyphilis occurrence.

Abstract:Sexually transmitted infectious diseases could affect a variety of organs, generating significant symptomatology. In the elderly population, infectious causes for vision problems are not generally considered. We present the case of an elderly patient with blurred vision and darkening of visual fields. He underwent an unsuccessful biopsy of the temporal artery as his vision disturbances presented also with episodic headaches. He was found to have an elevated rapid plasma reagin (RPR) and venereal disease research laboratory (VDRL) test in his cerebrospinal fluid (CSF) analysis. He was treated for ocular syphilis with a total resolution of his vision loss.

Abstract:Introduction: Syphilis, a bacterial infection caused by Treponema pallidum, can affect a wide variety of organ systems of its host. We aim to present the cases of two patients who presented to an urban, tertiary care, academic Emergency Department with ocular symptoms who underwent imaging, laboratory, and specialist evaluation and were ultimately diagnosed with ocular syphilis. Case report: The first patient is a 46-year-old female who presented to the Emergency Department with painless complete vision loss in a unilateral eye. Her exam was significant for bilateral papilledema, and further workup revealed a retinal detachment in the affected eye. Rapid plasma reagin (RPR) titers resulted positive with 1:128 and a reactive fluorescent treponemal antibody (FTA). Further workup including lumbar puncture and magnetic resonance imaging were unable to be obtained due to patient refusal and multiple discharges against medical advice. The second patient is a 38-year-old female with a history of intravenous drug use who presented for bilateral circumferential peripheral vision loss with central sparing. The examination showed bilateral papilledema. Lumbar puncture was performed with normal intracranial pressure. RPR titers resulted positive with 1:128. MRI and ophthalmology evaluation did not reveal any other etiology or explanation for papilledema. Conclusion: This case series highlights the importance of considering syphilis as a possible cause of ocular symptoms in high risk patient populations and the need for prompt and appropriate treatment given the increasing prevalence of syphilis worldwide.

Abstract:Neurosyphilis, a rare manifestation of <i>Treponema pallidum</i> spreading into the central nervous system, is a critical differential diagnosis due to its potentially severe neurologic effects if left untreated. We present a case of a 37-year-old woman who complained of bilateral vision loss and headaches originally concerning for idiopathic intracranial hypertension and uncontrolled diabetes. Comprehensive evaluations eventually led to a neurosyphilis diagnosis. Ophthalmologic examination revealed multifactorial visual symptoms with diabetic retinopathy contributing alongside neurosyphilis. Treatment was started promptly at the time of diagnosis and resulted in improvement in visual symptoms. This case emphasizes the importance of syphilis screening in appropriate age ranges, even in low-prevalence areas. Recognition of neurosyphilis as a potential diagnosis can impact patient outcomes and highlights the need for continued vigilance to identify the disease.

Abstract:A man in his 50s presented with a 3-week history of painless blurry vision. The ocular examination showed decreased visual acuity and 3+ bilateral papilloedema. A CT of the brain without contrast revealed a 5 mm left subdural haematoma. Anti-treponemal IgG antibodies were positive, and a reflex rapid plasma regain (RPR) was >1:64. HIV serology was negative. Ophthalmology and infectious diseases agreed that the presentation was consistent with ocular syphilis. Cerebrospinal fluid (CSF) examination revealed an elevated CSF protein of 52 mg/dL and CSF Venereal Disease Research Laboratory (VDRL) of 1:1. Penicillin was started. The patient developed a Jarisch-Herxheimer reaction soon after. He had a fever, rash and worsening headaches due to the enlargement of subdural haematoma for which he underwent a burr hole drainage. Vision improved after completing penicillin therapy but did not recover fully. The CSF VDRL became non-reactive and serum RPR titre decreased to 1:8 3 months later.

Abstract:Epidemiological trends have demonstrated re-emergence of neurosyphilis in the twenty-first century. As prevalence rises in clinical practice, neurosyphilis must be considered in the differential diagnosis even if initial diagnostic workup is unrevealing, especially in patients with human immunodeficiency virus (HIV). Co-infection of neurosyphilis and HIV can result in atypical presentations. In this report, we discuss a challenging diagnosis of neurosyphilis in a man with HIV who presented with atypical imaging findings and initially negative cerebrospinal fluid (CSF) nontreponemal testing. Our patient underwent repeated CSF evaluation and a comprehensive diagnostic workup, including brain biopsy, to arrive at the appropriate diagnosis. He received antibiotic treatment with excellent outcome. We review typical imaging features of neurosyphilis and highlight other neurological diseases that may mimic these radiographic findings. We discuss CSF testing and interpretation in this high-risk patient population.

Abstract:Neurosyphilis (NS) is an important component of central nervous system diseases among HIV-infected patients. However, its characteristics are not very clear. A retrospective analysis of clinical and laboratory findings was performed in 92 NS patients with HIV infection from a tertiary hospital in Shanghai, China. The patients had a median age of 38 years and a median CD4 count of 198cells/L. In all, 44.6% (41/92) were diagnosed as asymptomatic NS (ANS), 23.9% (22/92) as syphilitic meningitis, 17.4% (16/92) as cerebrovascular NS, and 14.1% (13/92) as parenchymal syphilis. A quarter of patients (23/92) complicated with ocular syphilis (OS), 60.9% (14/23) of which were ANS. The serum tolulized red unheated serum test (TRUST) titers were 1:8 in 15 patients (16.3%), 1:16-1:128 in 51 patients (55.4%), and 1:256 in 26 patients (28.3%). Sixty-nine patients (75.0%) had both cerebrospinal fluid (CSF) TRUST and Treponema pallidum particle assay reactive. CSF pleocytosis and protein elevation were found in 58.7% and 53.3% of patients, respectively. Syphilitic meningitis was more likely to present with CSF pleocytosis than ANS (P=.001), cerebrovascular NS (P<.001), and parenchymal NS (P<.001). The proportion of patients with CSF elevated protein was lower in ANS group than that in syphilitic meningitis (P=.003), cerebrovascular NS (P=.001), and parenchymal NS groups (P=.025), and was higher in sero-TRUST titers 1:8 group than that in 1:16-1:128 (P=.01) and 1:256-1:1024 groups (P=.005).This study revealed that ANS was the most common clinical type of NS in HIV-infected patients, which should be considered in HIV and syphilis co-infection patients without neurologic symptoms, especially in those with OS. Different patterns of NS might have different CSF features which may also vary with sero-TRUST titers.

Abstract:Purpose: This study aimed to investigate ocular manifestations in patients with neurosyphilis and their association with general indexes. Methods: This retrospective study was conducted among patients who were hospitalized for neurosyphilis from 2012 to 2022. Clinical manifestations, laboratory tests, brain MRI, and ocular examinations were reviewed and analyzed. Results: A total of 106 neurosyphilis patients were included, of which 10 patients presented with ocular signs as their initial symptoms. The most common main complaint was reduced vision (6/10), followed by diplopia (2/10) and ptosis (2/10). The causes of vision loss included optic neuropathy (4/6) and posterior uveitis (2/6), with all six patients showing optic nerve involvement. A total of 29 (27.4%) patients exhibited pupil abnormalities. A lumbar puncture was performed on all 106 patients, and abnormal cerebrospinal fluid (CSF) findings were revealed in 101 (95.3%) patients, indicating central nervous involvement. The median white blood cell count in the CSF of the patients with pupil abnormalities was significantly higher than that of those without pupil abnormalities (14.0 vs. 6.0 cells/μl, p = 0.037). In addition, the patients with abnormal pupillary light reflex (PLR) were more likely to have multiple brain lesions compared to those with normal PLR (77.3% vs. 29.2%, p < 0.001). Conclusion: Optic nerve involvement is the main cause of vision loss in neurosyphilis. Patients with optic neuropathy or posterior uveitis should undergo prompt diagnostic evaluation for syphilis. Pupil abnormalities can serve as indicators of more severe CSF and MRI findings.

Abstract:Purpose: To report the demographic, clinical, and laboratory characteristics of ocular syphilis based on a 6-year case series study from an eye center in East-China. Methods: A total of 131 cases (191 eyes) of ocular syphilis and the annual number of total syphilis cases from January 2016 to December 2021, were included in this study. Detailed medical records including systemic and ophthalmic medical history, a complete ophthalmic examination, color fundus photography, B-type ultrasound, fundus fluorescein angiography (FFA), spectral domain optical coherence tomography (SD-OCT), laboratory tests of the serum and cerebrospinal fluid (CSF) samples, as well as visual field test and orbital or cranial MRI in cases with suspected optic neuritis or optic atrophy, were collected and analyzed. Pearson Chi-square or Fisher's exact tests was used for statistics analysis. Results: Of the 131 cases with ocular syphilis, 86 cases were in men and 6 cases had a past medical history or systemic manifestation of syphilis. HIV was found in only 2 of 70 cases undergoing serum HIV test. The average age was 54.0 years, ranging from 26–85 years. The average percentage of ocular syphilis out from the total syphilis cases was 5.1%, the average titer of serum rapid plasma regain (RPR) at presentation was 1:32, ranging from 1:1–1:512. The most predominant manifestation of ocular syphilis was posterior uveitis, followed by optic neuritis, optic atrophy, panuveitis, retinal vasculitis, and retinitis. The median of BCVA of all 191 eyes was 20/200 (ranging from no light perception to 20/20), and 20/40 (ranging from no light perception to 20/20) at presentation and final follow-up, respectively. Ocular syphilis with active inflammation responded well to penicillin therapy, no matter the initial visual acuity, ocular disease type, or RPR titers, as long as it was diagnosed early and treated properly and promptly. However, cases with optic atrophy, acute retinal necrosis, late diagnosis, permanent disruption, or loss of outer segment of photoreceptors of macular retina on SD-OCT showed poor visual improvement after therapy. Conclusions: Early diagnosis of ocular syphilis is still challenging in clinical practice and syphilis tests should be routinely performed in patients with uveitis, retinitis, optic neuritis, and optic atrophy.

[Clinical characteristics and therapy of neurosyphilis in patients who are negative for human immunodeficiency virus]

Abstract:Introduction: Neurosyphilis is still a significant medical problem in developing countries and its occurrence in HIV infection is the reason for a growing number of new cases in developed countries [1-4]. Personality changes are the commonest symptom of late neurosyphilis [5]. Neurologic deficits are usually due to syphilitic vasculitis with lacunar infarctions [6]. The laboratory confirmation of neurosyphilis must depend on a cluster of tests (Venereal Disease Research Laboratories--VDRL; Fluorescent Treponemal Antibody-Absorption--FTA-ABS; Treponema Pallidum Immobilization--TPI or Nelson-Mayer). The diagnosis of active neurosyphilis also requires an inflammatory cerebrospinal fluid (CSF) test [4]. Methods: A retrospective study of hospitalized neurosyphilitic patients at the Institute of Neurology, Belgrade, Serbia, was carried out. Between 1990 and 1996 seven cases were found: five males and two females, aged from 22 to 66 years, mean 56 years. All were in tertiary stage and HIV negative. None of them reported previous veneral disease and only one reported meningitis. In all patients neurological examination, VDRL and TPI in blood and CSF, CT or MRI of the brain, Mini Mental State (MMS) test [11] and psychiatric evaluation were performed. To patients with active disease 24 million units of crystalline penicillin were administered, in divided daily doses in continuous intravenous infusions for three weeks. Results: Symptoms. Five patients (71%) had gait disturbances, four (57%) headache, three (43%) hearing loss, speech complaints, forgetfulness, mood changes, and two (29%) seizures and stroke-like episodes. Malaise, diplopia, visual loss, vertigo, loss of sphincter control and paresthesia were noticed in one patient (14%). Neurologic findings. Five patients (71%) had pupillary changes. In four (57%) of them we found hemiparesis, gait disturbances and signs of cerebellar involvement, in three (43%) hearing loss, dysarthria, paraparesis, hypesthesia for light touch, mood disorders (depression in two, hypomania in one), mild cognitive decline and tremor of the tongue and hands. Lesion of the second and the sixth cranial nerve, position sense and positive release signs were recorded in one patient (14%). Gonarthrotic changes were observed in two (29%) patients. Cerebrospinal fluid. Elevated cell count and high protein level were detected in four (57%) patients. Three had also positive oligoclonal bands. Serological tests. In six patients (86%) serological tests for syphilis were positive (VDRL and/or Nelson-Mayer Test) in serum and in three (43%) in CSF. Results in one patient were negative both regarding serum and CSF, as he was treated for secondary syphilis in his twenties (now 65 years old) presumably as a "burnt out" case with neurologic sequelae. Neuroimaging. CT and MRI of the brain showed cortical and subcortical reduction in the brain parenchyma in four (57%) and multiischaemic changes in two (29%) patients. Therapy. Crystalline penicillin was administered to five patients with no side effects. Improvement of mood and cognitive status were noted in three patients, improvement of neurologic status in two subjects and arrest of progression in two patients. Discussion: None of our patients reported previous venereal disease. They complained of impaired gait, headache, hearing loss, speech difficulties, forgetfulness, and mood changes. Pupillary changes, hemiparesis, gait disturbances and cerebellar signs, hearing loss, dysarthria, paraparesis, hypesthesia, mood disorders, mild cognitive decline and tremor of the tongue and hands were found during the examination. Pupillary signs are the most important symptoms because they can point to neurosyphilis. A two-step approach is recommended for establishing the diagnosis of syphilis with simple nontreponemal tests for screening purpose, and specific treponemal tests for its confirmation [21]. Abstract:Neurosyphilis is caused by Treponema pallidum invades the central nervous system (CNS) and can occur at any time after infection. NS includes asymptomatic neurosyphilis (ANS), symptomatic syphilitic meningitis, meningovascular neurosyphilis, ocular syphilis, otologic syphilis, intracranial gummas, general paresis, and tabes dorsalis. [ 1 ] Neurosyphilis is of particular concern in human immunodeficiency virus (HIV) infected individuals for several reasons, including (a) HIV and syphilis are transmitted in the same way, which leads to high rates of HIV and Treponema pallidum coinfection; (b) the difficulties in diagnosing neurosyphilis in these patients; (c) the potential increased risk of failing therapy for early syphilis, which may result in the development of early neurosyphilis; and (d) the possibly higher rates of treatment failure. All of these factors may contribute to long-term morbidity in patients with HIV infection. In China, there were 1.053 million people living with HIV, syphilis is the most common sexually transmitted infection among patients with HIV in China. [ 2 ] Compared with without HIV infection, neurosyphilis was estimated to occur twice in patients with coinfection. [ 3 ] From 2007 to 2017, the morbidity of syphilis showed a significant upward trend, from 15.8834/100,000 to 34.4867/100,000 in China. [ 4 ]

Ocular Manifestations of Syphilitic Uveoretinitis in Patients Co-Infected with Human Immunodeficiency Virus

Abstract:OBJECTIVE:To investigate the clinical manifestations of syphilitic uveoretinitis in patients co-infected with human immunodeficiency virus (HIV).METHODS:Uveoretinitis patients presenting between January 2008 and December 2014 at Eye and ENT Hospital of Fudan University were collected. Patients were selected with positive serologic tests, including rapid plasma regain titer (RPR) > 1:8, treponema pallidum particle agglutination assay (TPPA) and human immunodeficiency virus (HIV). Other causes of uveoretinitis were excluded. Each patient underwent complete ophthalmologic examination including best-corrected visual acuity (BCVA), slit-lamp biomicroscopy, ophthalmoscopy, applanation tonometry and B-scan ultrasonography. Fundus fluorescein angiography were obtained in each case except for patients whose fundus was blurred with dense vitritis. Optical coherence tomography (OCT) was performed in selected patients. Thorough physical examination was performed simultaneously.RESULTS:Twenty six patients were included in this study including 24 male (92.3%) and 2 female (7.7%). The mean age at presentation was 39.3 ± 13.2 (range 20 to 63 years). RPR titres ranged from 1:32 to 1:512. There were 49 eyes altogether and ocular involvement was bilateral in 23 patients (88.5%) and unilateral in 3 patients (11.5%). Panuveitis was the most common ophthalmic presentation (n = 39, 79.6%). Only two eyes were anterior uveitis (4.1%) and 8 eyes were posterior uveitis (16.3%). Anterior chamber cells+++ and mild to severe vitreous opacities were observed. Sixteen eyes presented with neuroretinitis (32.7%), 27 eyes had vitritis (55.1%), 5 eyes had retinochoroiditis (10.2%), 5 eyes had optic neuritis (10.2%), 5 eyes had retinitis (10.2%) including 2 eyes with necrotizing retinitis (4.1%), 4 eyes had retinal vasculitis (8.2%). Two eyes (11.5%) presented with posterior placoid chorioretinitis (4.1%) and multifocal choroiditis (4.1%). Systemic manifestations were detected. Five patients had a history of skin rash (19.2%), five (19.2%) had genital ulcers, three (11.5%) had chancre, two (7.7%) suffered from headache, two (7.7%) had a fever, one (3.8%) had oral ulcers and one patient (3.8%) lost his hair.CONCLUSIONS:The clinical features are protean and lack of specificity. The most common manifestations was neuroretinitis, ususlly with vitritis.

Abstract:Syphilis is a chronic infectious disease, which dates back to the XV century and is caused by the spirochete treponema pallidum, capable of invading the central nervous system in any of its stages- Its incidence has increased in parallel to the HIV/AIDS pandemic, and the synergism between both pathologies is such. that it has become a public health problem in recent years. Here we present the case of a 31-year-old female patient, who consulted for headache associated with decreased visual acuity and provided an unenhanced head CT showing hypodense lesions in both thalamic regions, serological tests for syphilis were reactive and those for HIV were not reactive. The brain MRI with spectroscopy was reported in favor of cerebral toxoplasmosis, which was later ruled out with a study of cerebrospinal fluid. Management with penicillin G sodium IV for 6 weeks was indicated, achieving complete imaging resolution of her lesions.

Abstract:years old male, natural and coming from Rupa-Rupa (Tingo Maria-Huanuco-Peru), who since two years (2011) ago, presented fatigue and apathy, adding six months later: paresthesias, seizures without loss of consciousness and decreased muscle strength in his lower limbs, which prevents him from walking. 14 months ago, he developed quadriplegia and dysarthria and six months later, a sudden and severe decreased visual acuity (OD> OI). History of multiple hetero and homosexual relationships until 37 years. Physical examination showed general malaise, body trigger position, right eye ptosis and dyschromic skin lesions in penis and buttocks. Awake, oriented himself disoriented in time. Eyes right and left: 4 and 2 mm, respectively, decreased visual acuity. Bilateral decreased motor coordination. Stiloradial reflex: asymmetry in four extremities. Distal spastic paraplegia below both knees. FTA-Abs reactive in serum and CSF. MRI: Thickening of meninges surrounding the superior sagittal sinus. Even after treatment with penicillin and ceftriaxone, difficult walking

Abstract:With syphilis resurgence, physicians should be more vigilant to infection-induced cerebral vasculitis in high-risk patients presenting with neurological symptoms. In this case, neurosyphilis should not be missed. Thorough serologic screening and lumbar puncture are crucial for diagnosis, and further research is needed for safe and effective treatments in these populations.
