A Postreconstruction Harmonization Method for Multicenter Radiomic Studies in PET

Abstract:Several reports have shown that radiomic features are affected by acquisition and reconstruction parameters, thus hampering multicenter studies. We propose a method that, by removing the center effect while preserving patient-specific effects, standardizes features measured from PET images obtained using different imaging protocols. Methods: Pretreatment 18F-FDG PET images of patients with breast cancer were included. In one nuclear medicine department (department A), 63 patients were scanned on a time-of-flight PET/CT scanner, and 16 lesions were triple-negative (TN). In another nuclear medicine department (department B), 74 patients underwent PET/CT on a different brand of scanner and a different reconstruction protocol, and 15 lesions were TN. The images from department A were smoothed using a gaussian filter to mimic data from a third department (department A-S). The primary lesion was segmented to obtain a lesion volume of interest (VOI), and a spheric VOI was set in healthy liver tissue. Three SUVs and 6 textural features were computed in all VOIs. A harmonization method initially described for genomic data was used to estimate the department effect based on the observed feature values. Feature distributions in each department were compared before and after harmonization. Results: In healthy liver tissue, the distributions significantly differed for 4 of 9 features between departments A and B and for 6 of 9 between departments A and A-S (P < 0.05, Wilcoxon test). After harmonization, none of the 9 feature distributions significantly differed between 2 departments (P > 0.1). The same trend was observed in lesions, with a realignment of feature distributions between the departments after harmonization. Identification of TN lesions was largely enhanced after harmonization when the cutoffs were determined on data from one department and applied to data from the other department. Conclusion: The proposed harmonization method is efficient at removing the multicenter effect for textural features and SUVs. The method is easy to use, retains biologic variations not related to a center effect, and does not require any feature recalculation. Such harmonization allows for multicenter studies and for external validation of radiomic models or cutoffs and should facilitate the use of radiomic models in clinical practice.
Impact of feature harmonization on radiogenomics analysis: Prediction of EGFR and KRAS mutations from non-small cell lung cancer PET/CT images

Abstract:OBJECTIVE: To investigate the impact of harmonization on the performance of CT, PET, and fused PET/CT radiomic features toward the prediction of mutations status, for epidermal growth factor receptor (EGFR) and Kirsten rat sarcoma viral oncogene (KRAS) genes in non-small cell lung cancer (NSCLC) patients.METHODS: Radiomic features were extracted from tumors delineated on CT, PET, and wavelet fused PET/CT images obtained from 136 histologically proven NSCLC patients. Univariate and multivariate predictive models were developed using radiomic features before and after ComBat harmonization to predict EGFR and KRAS mutation statuses. Multivariate models were built using minimum redundancy maximum relevance feature selection and random forest classifier. We utilized 70/30% splitting patient datasets for training/testing, respectively, and repeated the procedure 10 times. The area under the receiver operator characteristic curve (AUC), accuracy, sensitivity, and specificity were used to assess model performance. The performance of the models (univariate and multivariate), before and after ComBat harmonization was compared using statistical analyses.RESULTS: While the performance of most features in univariate modeling was significantly improved for EGFR prediction, most features did not show any significant difference in performance after harmonization in KRAS prediction. Average AUCs of all multivariate predictive models for both EGFR and KRAS were significantly improved (q-value < 0.05) following ComBat harmonization. The mean ranges of AUCs increased following harmonization from 0.87-0.90 to 0.92-0.94 for EGFR, and from 0.85-0.90 to 0.91-0.94 for KRAS. The highest performance was achieved by harmonized F_R0.66_W0.75 model with AUC of 0.94, and 0.93 for EGFR and KRAS, respectively.CONCLUSION: Our results demonstrated that regarding univariate modelling, while ComBat harmonization had generally a better impact on features for EGFR compared to KRAS status prediction, its effect is feature-dependent. Hence, no systematic effect was observed. Regarding the multivariate models, ComBat harmonization significantly improved the performance of all radiomics models toward more successful prediction of EGFR and KRAS mutation statuses in lung cancer patients. Thus, by eliminating the batch effect in multi-centric radiomic feature sets, harmonization is a promising tool for developing robust and reproducible radiomics using vast and variant datasets.

Abstract:Abstract To reduce the variability of radiomics features caused by computed tomography (CT) imaging protocols through using a generative adversarial network (GAN) method. In this study, we defined a set of images acquired with a certain imaging protocol as a domain, and a total of four domains (A, B, C, and T [target]) from three different scanners was included. In data set#1, 60 patients for each domain were collected. Data sets#2 and #3 included 40 slices of spleen for each of the domains. In data set#4, the slices of three colorectal cancer groups ( n = 28, 38 and 32) were separately retrieved from three different scanners, and each group contained short-term and long-term survivors. Seventy-seven features were extracted for evaluation by comparing the feature distributions. First, we trained the GAN model on data set#1 to learn how to normalize images from domains A, B and C to T. Next, by comparing feature distributions between normalized images of the different domains, we identified the appropriate model and assessed it, in data set#2 and data set#3, respectively. Finally, to investigate whether our proposed method could facilitate multicenter radiomics analysis, we built the least absolute shrinkage and selection operator classifier to distinguish short-term from long-term survivors based on a certain group in data set#4, and validate it in another two groups, which formed a cross-validation between groups in data set#4. After normalization, the percentage of aligned features between domains A versus T, B versus T, and C versus T increased from 10.4 %, 18.2% and 50.1% to 93.5%, 89.6% and 77.9%, respectively. In the cross-validation results, the average improvement of the area under the receiver operating characteristic curve achieved 11% (3%–32%). Our proposed GAN-based normalization method could reduce the variability of radiomics features caused by different CT imaging protocols and facilitate multicenter radiomics analysis.

Abstract:Positron emission tomography (PET) image synthesis plays an important role, which can be used to boost the training data for computer aided diagnosis systems. However, existing image synthesis methods have problems in synthesizing the low resolution PET images. To address these limitations, we propose multi-channel generative adversarial networks (M-GAN) based PET image synthesis method. Different to the existing methods which rely on using low-level features, the proposed M-GAN is capable to represent the features in a high-level of semantic based on the adversarial learning concept. In addition, M-GAN enables to take the input from the annotation (label) to synthesize the high uptake regions e.g., tumors and from the computed tomography (CT) images to constrain the appearance consistency and output the synthetic PET images directly. Our results on 50 lung cancer PET-CT studies indicate that our method was much closer to the real PET images when compared with the existing methods.
Multiscale Deep-Learning Network Based Reconstruction of PET Images from Sinogram Domain

Abstract:Positron emission tomography (PET) is an indispensable medical imaging technique which can reveal the physiology activities by injecting special PET tracers. In recent years, deep learning has been widely used in PET reconstruction tasks. In this work, a multiscale direct reconstruction framework based on cGANs has been presented. First, the generator is built as two scales which will combine both global and detail information in training phase. Second, to match the generator, we tried multiscale discriminators. Each discriminator shares the same structure, but operates on different scales. Both coarse field and fine field were scored by the discriminators, so this score considered not only the global structure but also fine details. In order to verify the feasibility of the proposed framework, both simulation datasets and real SD rat dataset were utilized in our experiments. We also compared our method with U-Net and Deep-PET network, the results showed that we got more accurate and real radioactivity images on all datasets.

Abstract:Multi-center neuroimaging studies face technical variability due to batch differences across sites, which potentially hinders data aggregation and impacts study reliability.Recent efforts in neuroimaging harmonization have aimed to minimize these technical gaps and reduce technical variability across batches. While Generative Adversarial Networks (GAN) has been a prominent method for addressing image harmonization tasks, GAN-harmonized images suffer from artifacts or anatomical distortions. Given the advancements of denoising diffusion probabilistic model which produces high-fidelity images, we have assessed the efficacy of the diffusion model for neuroimaging harmonization. we have demonstrated the diffusion model's superior capability in harmonizing images from multiple domains, while GAN-based methods are limited to harmonizing images between two domains per model. Our experiments highlight that the learned domain invariant anatomical condition reinforces the model to accurately preserve the anatomical details while differentiating batch differences at each diffusion step. Our proposed method has been tested on two public neuroimaging dataset ADNI1 and ABIDE II, yielding harmonization results with consistent anatomy preservation and superior FID score compared to the GAN-based methods. We have conducted multiple analysis including extensive quantitative and qualitative evaluations against the baseline models, ablation study showcasing the benefits of the learned conditions, and improvements in the consistency of perivascular spaces (PVS) segmentation through harmonization.

Abstract:The purpose of this work was to assess the capability of radiomic features in distinguishing PET image regions with different uptake patterns. Furthermore, we assessed the stability of PET radiomic features with varying image reconstruction settings. An in-house phantom was designed and constructed, consisting of homogenous and heterogenous artificial phantom inserts. Four artificially constructed inserts were placed into a water filled phantom and filled with varying levels of radioactivity to simulate homogeneous and heterogeneous uptake patterns. The phantom was imaged for 80 min. PET images were reconstructed whilst varying reconstruction parameters. The parameters adjusted included, number of ordered subsets, number of iterations, use of time-of-flight and filter cut off. Regions of interest (ROI) were established by segmentation of the phantom inserts from the reconstructed images. In total seventy eight 3D radiomic features for each ROI with unique reconstructed parameters were extracted. The Friedman test was used to determine the statistical power of each radiomic feature in differentiating phantom inserts with different hetero/homogeneous configurations. The Coefficient of Variation (COV) of each feature, with respect to the reconstruction setting was used to determine feature stability. Forty three out of seventy eight radiomic features were found to be stable (COV ≤ 5%) against all reconstruction settings. To provide any utility, stable features are required to differentiate between regions with different hetro/homogeneity. Of the forty three stable features, fifteen (35%) features showed a statistically significant difference between the artificially constructed inserts. Such features included GLCM (Difference average, Difference entropy, Dissimilarity and Inverse difference), GLRL (Long run emphasis, Grey level non uniformity and Run percentage) and NGTDM (Complexity and Strength). The finding of this work suggests that radiomic features are capable of distinguishing between radioactive distribution patterns that demonstrate different levels of heterogeneity. Therefore, radiomic features could serve as an adjuvant diagnostic tool along with traditional imaging. However, the choice of the radiomic features needs to account for variability introduced when different reconstruction settings are used. Standardization of PET image reconstruction settings across sites performing radiomic analysis in multi-centre trials should be considered.

Abstract:For machine learning-based prognosis and diagnosis of rare diseases, such as pediatric brain tumors, it is necessary to gather medical imaging data from multiple clinical sites that may use different devices and protocols. Deep learning-driven harmonization of radiologic images relies on generative adversarial networks (GANs). However, GANs notoriously generate pseudo structures that do not exist in the original training data, a phenomenon known as "hallucination". To prevent hallucination in medical imaging, such as magnetic resonance images (MRI) of the brain, we propose a one-shot learning method where we utilize neural style transfer for harmonization. At test time, the method uses one image from a clinical site to generate an image that matches the intensity scale of the collaborating sites. Our approach combines learning a feature extractor, neural style transfer, and adaptive instance normalization. We further propose a novel strategy to evaluate the effectiveness of image harmonization approaches with evaluation metrics that both measure image style harmonization and assess the preservation of anatomical structures. Experimental results demonstrate the effectiveness of our method in preserving patient anatomy while adjusting the image intensities to a new clinical site. Our general harmonization model can be used on unseen data from new sites, making it a valuable tool for real-world medical applications and clinical trials.

Abstract:The reconstruction kernel in computed tomography (CT) generation determines the texture of the image. Consistency in reconstruction kernels is important as the underlying CT texture can impact measurements during quantitative image analysis. Harmonization (i.e., kernel conversion) minimizes differences in measurements due to inconsistent reconstruction kernels. Existing methods investigate harmonization of CT scans in single or multiple manufacturers. However, these methods require paired scans of hard and soft reconstruction kernels that are spatially and anatomically aligned. Additionally, a large number of models need to be trained across different kernel pairs within manufacturers. In this study, we adopt an unpaired image translation approach to investigate harmonization between and across reconstruction kernels from different manufacturers by constructing a multipath cycle generative adversarial network (GAN). We use hard and soft reconstruction kernels from the Siemens and GE vendors from the National Lung Screening Trial dataset. We use 50 scans from each reconstruction kernel and train a multipath cycle GAN. To evaluate the effect of harmonization on the reconstruction kernels, we harmonize 50 scans each from Siemens hard kernel, GE soft kernel and GE hard kernel to a reference Siemens soft kernel (B30f) and evaluate percent emphysema. We fit a linear model by considering the age, smoking status, sex and vendor and perform an analysis of variance (ANOVA) on the emphysema scores. Our approach minimizes differences in emphysema measurement and highlights the impact of age, sex, smoking status and vendor on emphysema quantification.

Abstract:Deep networks are now ubiquitous in large-scale multi-center imaging studies. However, the direct aggregation of images across sites is contraindicated for downstream statistical and deep learning-based image analysis due to inconsistent contrast, resolution, and noise. To this end, in the absence of paired data, variations of Cycle-consistent Generative Adversarial Networks have been used to harmonize image sets between a source and target domain. Importantly, these methods are prone to instability, contrast inversion, intractable manipulation of pathology, and steganographic mappings which limit their reliable adoption in real-world medical imaging. In this work, based on an underlying assumption that morphological shape is consistent across imaging sites, we propose a segmentation-renormalized image translation framework to reduce inter-scanner heterogeneity while preserving anatomical layout. We replace the affine transformations used in the normalization layers within generative networks with trainable scale and shift parameters conditioned on jointly learned anatomical segmentation embeddings to modulate features at every level of translation. We evaluate our methodologies against recent baselines across several imaging modalities (T1w MRI, FLAIR MRI, and OCT) on datasets with and without lesions. Segmentation-renormalization for translation GANs yields superior image harmonization as quantified by Inception distances, demonstrates improved downstream utility via post-hoc segmentation accuracy, and improved robustness to translation perturbation and self-adversarial attacks.

Abstract:Clinical trials often involve comparing 2–4 doses or regimens of an experimental therapy with a control treatment. These studies might occur early in a drug development process, where the aim might be to demonstrate a basic level of proof (the so-called proof of concept (PoC) studies), at a later stage, to help establish a dose or doses that should be used in phase III trials (dose-finding), or even in confirmatory studies, where the registration of several doses might be considered. When a small number of doses are examined, the ability to implement parametric modeling is somewhat limited. As an alternative, in this paper, a flexible Bayesian model is suggested. In particular, we draw on the idea of using Bayesian model averaging (BMA) to exploit an assumed monotonic dose–response relationship, without using strong parametric assumptions. The approach is exemplified by assessing operating characteristics in the design of a PoC study examining a new treatment for psoriatic arthritis and a post hoc data analysis involving three confirmatory clinical trials, which examined an adjunctive treatment for partial epilepsy. Key difficulties, such as prior specification and computation, are discussed. A further extension, based on combining the flexible modeling with a classical multiple comparisons procedure, known as MCP–MOD, is examined. The benefit of this extension is a potential reduction in the number of simulations that might be needed to investigate operating characteristics of the statistical analysis.

Abstract:Background The kernel used in CT image reconstruction is an important factor that determines the texture of the CT image. Consistency of reconstruction kernel choice is important for quantitative CT‐based assessment as kernel differences can lead to substantial shifts in measurements unrelated to underlying anatomical structures. Purpose In this study, we investigate kernel harmonization in a multi‐vendor low‐dose CT lung cancer screening cohort and evaluate our approach's validity in quantitative CT‐based assessments. Methods Using the National Lung Screening Trial, we identified CT scan pairs of the same sessions with one reconstructed from a soft tissue kernel and one from a hard kernel. In total, 1000 pairs of five different paired kernel types (200 each) were identified. We adopt the pix2pix architecture to train models for kernel conversion. Each model was trained on 100 pairs and evaluated on 100 withheld pairs. A total of 10 models were implemented. We evaluated the efficacy of kernel conversion based on image similarity metrics including root mean squared error (RMSE), peak signal‐to‐noise ratio (PSNR), and structural similarity index measure (SSIM) as well as the capability of the models to reduce measurement shifts in quantitative emphysema and body composition measurements. Additionally, we study the reproducibility of standard radiomic features for all kernel pairs before and after harmonization. MRI to PET Cross-Modality Translation using Globally and Locally Aware GAN (GLA-GAN) for Multi-Modal Diagnosis of Alzheimer's Disease

Abstract:Medical imaging datasets are inherently high dimensional with large variability and low sample sizes that limit the effectiveness of deep learning algorithms. Recently, generative adversarial networks (GANs) with the ability to synthesize realist images have shown great potential as an alternative to standard data augmentation techniques. Our work focuses on cross-modality synthesis of fluorodeoxyglucose~(FDG) Positron Emission Tomography~(PET) scans from structural Magnetic Resonance~(MR) images using generative models to facilitate multi-modal diagnosis of Alzheimer's disease (AD). Specifically, we propose a novel end-to-end, globally and locally aware image-to-image translation GAN (GLA-GAN) with a multi-path architecture that enforces both global structural integrity and fidelity to local details. We further supplement the standard adversarial loss with voxel-level intensity, multi-scale structural similarity (MS-SSIM) and region-of-interest (ROI) based loss components that reduce reconstruction error, enforce structural consistency at different scales and perceive variation in regional sensitivity to AD respectively. Experimental results demonstrate that our GLA-GAN not only generates synthesized FDG-PET scans with enhanced image quality but also superior clinical utility in improving AD diagnosis compared to state-of-the-art models. Finally, we attempt to interpret some of the internal units of the GAN that are closely related to this specific cross-modality generation task.

A Novel Generative Artificial Intelligence Method for Interference Study on Multiplex Brightfield Immunohistochemistry Images

Abstract:Multiplex brightfield imaging offers the advantage of simultaneously analyzing multiple biomarkers on a single slide, as opposed to single biomarker labeling on multiple consecutive slides. To accurately analyze multiple biomarkers localized at the same cellular compartment, two representative biomarker sets were selected as assay models - cMET-PDL1-EGFR and CD8-LAG3-PDL1, where all three biomarkers can co-localize on the cell membrane. One of the most crucial preliminary stages for analyzing such assay is identifying each unique chromogen on individual cells. This is a challenging problem due to the co-localization of membrane stains from all the three biomarkers. It requires advanced color unmixing for creating the equivalent singleplex images from each triplex image for each biomarker. In this project, we developed a cycle-Generative Adversarial Network (cycle-GAN) method for unmixing the triplex images generated from the above-mentioned assays. Three different models were designed to generate the singleplex image for each of the three stains Tamra (purple), QM-Dabsyl (yellow) and Green. A notable novelty of our approach was that the input to the network were images in the optical density domain instead of conventionally used RGB images. The use of the optical density domain helped in reducing the blurriness of the synthetic singleplex images, which was often observed when the network was trained on RGB images. The cycle-GAN models were validated on 10,800 lung, gastric and colon images for the cMET-PDL1-EGFR assay and 3600 colon images for the CD8-LAG3-PDL1 assay. Visual as well as quantified assessments demonstrated that the proposed method is effective and efficient when compared with the manual reviewing results and is readily applicable to various multiplex assays.

Vitamins and immunity: II. Influence of L-carnitine on the immune system.

Abstract:Vitamin A affects the antibody responses and may affect phagocytic function and properdin levels. Pyridoxine deficiency impairs nucleic acid synthesis and depresses antibody formation, delayed hypersensitivity reactions and the ability of phagocytes to kill bacteria. Pantothenic acid deficiency impairs antibody formation. Vitamin C deficiency increases the incidence of infection, primary by a negative influence on reparative processes. Deficiencies of other vitamins either have not been sufficiently studied or have a variable effect. Moreover, even substances which for their biosynthesis require an adequate vitamin supplementation may exert immunomodulatory influences. With this respect the authors report their results on the influence of L-carnitine on the immune system. L-carnitine increases the proliferative responses of both murine and human lymphocyte following mitogenic stimulation and increase polymorphonuclear chemotaxis. Furthermore, L-carnitine, even at minimal concentrations, neutralizes the lipid induced immunosuppression.

Abstract:BACKGROUNDThe availability of standard multiparametric MRI sequences, including pre-contrast T1w (T1w-pre), post-contrast T1w (T1w-post), T2w, and FLAIR images, is essential for accurate segmentation of tumor subregions and subsequent response assessment in pediatric brain tumors (PBTs). However, clinical MRI sets are often incomplete due to imaging artifacts or inconsistent acquisition protocols across different centers. This study leverages Generative Adversarial Networks (GANs) to synthesize missing FLAIR and T1w-pre images from T2w and T1w-post images, respectively, to facilitate tumor segmentation in PBTs. METHODSWe developed two GAN models based on the pix2pix framework, trained, validated, and tested on FLAIR-T2w and T1w-post-T1w-pre pairs from a multi-histology and multi-institutional cohort of 476 subjects with PBTs from the Children's Brain Tumor Network (CBTN). The perceptual quality of the synthesized T1w-pre and FLAIR images was evaluated using the Structural Similarity Index (SSI; ideal value = 1). The robustness of the synthesized sequences to replace missing MRI sequences was evaluated by comparing the segmentation performance (Dice score) of our pre-trained autosegmentation model using all real modalities versus replacing real sequences with their synthesized counterparts. RESULTSThe T1w-pre and FLAIR synthesis GANs achieved mean SSI values of 0.95 and 0.93, respectively. Median Dice scores for segmentation of whole tumor (WT), enhancing tumor (ET), and tumor core (TC) were calculated for real sequences: 0.90, 0.83, 0.89, replacing real FLAIR with synthetic: 0.86, 0.81, 0.86, and replacing real T1w-pre with synthetic: 0.90, 0.75, 0.89. No statistical significance (p>0.05) was observed in segmentation performance for WT, TC, ET when replacing real T1w-pre with synthetic, and for ET when replacing real with synthetic FLAIR sequences. CONCLUSIONThis method achieves robust segmentation performance in limited data scenarios by synthesizing missing MRI sequences. Future work includes improving FLAIR synthesis and segmentation performance for WT and ET by implementing and comparing other GAN architectures.

Abstract:Background: Harmonization methods reduce variability between different make and models of positron emission tomography (PET) scanners. The study aims to explore harmonization strategies that lead to comparable and robust quantitative metrics in a multicenter setting. Methods: NEMA IEC Phantom data acquisition was performed for low and high spheres-to-background ratios (SBR4:1 and 10:1) on six PET/CT (computed tomography) scanners. Different reconstruction sets, including the number of sub-iterations, number of subsets, and full width at half maximum (FWHM) for each scanner, were evaluated towards optimized and harmonized reconstruction settings. Recovery coefficients (RCs) of four quantitative metrics, including standardized uptake value (SUV)max, SUVISO-50 (SUVmean in 50% isocontour), SUVpeak, and mean uptake of 10 highest concentration voxels were evaluated as RCmax, RCISO-50, RCpeak, and RC10V, representing percent difference relative to the static ground truth case as functions of sphere sizes. A set of image reconstruction parameters was proposed for harmonized reconstruction to minimize variability between scanners. The root mean square error (RMSE), curvature, and reproducibility were examined. The proposed reconstruction protocols for harmonization and standard clinical reconstruction settings were compared to each other across all scanners. Results: A significant difference (P value <0.0001) was observed in the aforementioned quantitative metrics between SBR10 and SBR4. Reconstruction parameter sets with the smallest RMSE and RC values within 10% bias were identified as the best candidate for harmonization. The coefficient of variation of the mean value of RCs (CVMRC) shows a remarkable reduction of about 28%, 26%, 32%, and 19% in harmonized reconstruction settings for MRCmax, MRCISO-50, MRCpeak, and MRC10V, respectively. CVMRC for MRC10V in the harmonized reconstruction setting was 5.9% in SBR4, while the smallest value in SBR10 belongs to MRCpeak, with a value of 5.8%. The reproducibility of RC is improved by deriving the value from ten hottest voxels and is equally reproducible with RCpeak. Compared to RCmax and RCISO-50, the variability is reduced by 18% and 22% if ten voxels are pooled. Conclusions: Harmonizing PET/CT systems with and without point spread function/time of flight (PSF/TOF) using various vendor-developed image reconstruction algorithms improves the quantification reproducibility. RC10V, likewise RCpeak, is superior to the rest of the quantitative indices in terms of accuracy and reproducibility and helpful in quantifying lesion volume below 1 mL.
