Abstract:Adolescent idiopathic scoliosis (AIS) is a structural spinal deformity mainly in the coronal plane and is among the most frequent deformities in children, adolescents, and young adults, with an overall prevalence of 0.47–5.2%. The Cobb angle is an objective measure to determine the progression of deformity and plays a critical role in the planning of surgical treatment. However, existing studies suggested that Cobb angle measurement is susceptible to inter- and intra-observer variability, as well as a high variability in the definition of the end vertebra. In this study, we proposed an automatic method for the spine vertebrae segmentation using Deeplab V3+, a powerful tool that has shown success in the image segmentation of other anatomical regions but spine, and Cobb angle measurement. The segmentation performance was compared to existing mainstay neural networks. Compared to U-Net, Residual U-Net and Dilated U-Net, our method using Deeplab V3+ showed the best performance in the Dice Similarity Coefficient (DSC), accuracy, sensitivity and Jaccard Index. An excellent correlation in the final Cobb angle calculation was achieved between the smallest distance point (SDP) method and two experts (> 0.95), with a small error in the angle estimation compared (MAE < 3°). The proposed method could provide a potential tool for the automatic estimation of the Cobb angle to improve the efficiency and accuracy of the treatment workflow for AIS.

Abstract:Scoliosis is a common spinal deformity that seriously affects patients’ physical and mental health. An accurate Lenke classification is greatly significant for evaluating and treating scoliosis. Currently, the clinical diagnosis mainly relies on manual measurement; however, using computer vision assists with an intelligent diagnosis. Due to the complex rules of Lenke classification and the characteristics of medical imaging, the fully automated Lenke classification of scoliosis remains a considerable challenge. Herein, a novel Lenke classification method for scoliosis using X-rays based on segmentation networks and adaptive shape descriptors is proposed. Three aspects of our method should be noted in comparison with the previous approaches. We used Unet++ to segment the vertebrae and designed a post-processing operation to improve the segmentation effect. Then, we proposed a new shape descriptor to extract the shape features for segmented vertebrae in greater detail. Finally, we proposed a new Lenke classification framework for scoliosis that contains two schemes based on Cobb angle measurement and shape classification, respectively. After rigorous experimental evaluations on a public dataset, our method achieved the best performance and outperformed other sophisticated approaches.
Validation of Artificial Intelligence in the Classification of Adolescent Idiopathic Scoliosis and the Compairment to Clinical Manual Handling

Abstract:Objective: The accurate measurement of Cobb angles is crucial for the effective clinical management of patients with adolescent idiopathic scoliosis (AIS). The Lenke classification system plays a pivotal role in determining the appropriate fusion levels for treatment planning. However, the presence of interobserver variability and time-intensive procedures presents challenges for clinicians. The purpose of this study is to compare the measurement accuracy of our developed artificial intelligence measurement system for Cobb angles and Lenke classification in AIS patients with manual measurements to validate its feasibility. Methods: An artificial intelligence (AI) system measured the Cobb angle of AIS patients using convolutional neural networks, which identified the vertebral boundaries and sequences, recognized the upper and lower end vertebras, and estimated the Cobb angles of the proximal thoracic, main thoracic, and thoracolumbar/lumbar curves sequentially. Accordingly, the Lenke classifications of scoliosis were divided by oscillogram and defined by the AI system. Furthermore, a man-machine comparison (n = 300) was conducted for senior spine surgeons (n = 2), junior spine surgeons (n = 2), and the AI system for the image measurements of proximal thoracic (PT), main thoracic (MT), thoracolumbar/lumbar (TL/L), thoracic sagittal profile T5-T12, bending views PT, bending views MT, bending views TL/L, the Lenke classification system, the lumbar modifier, and sagittal thoracic alignment. Results: In the AI system, the calculation time for each patient's data was 0.2 s, while the measurement time for each surgeon was 23.6 min. The AI system showed high accuracy in the recognition of the Lenke classification and had high reliability compared to senior doctors (ICC 0.962). Conclusion: The AI system has high reliability for the Lenke classification and is a potential auxiliary tool for spinal surgeons.

Abstract:Abstract Adolescent idiopathic scoliosis is the most common spinal disorder in adolescents with a prevalence of 0.5–5.2% worldwide. The traditional methods for scoliosis screening are easily accessible but require unnecessary referrals and radiography exposure due to their low positive predictive values. The application of deep learning algorithms has the potential to reduce unnecessary referrals and costs in scoliosis screening. Here, we developed and validated deep learning algorithms for automated scoliosis screening using unclothed back images. The accuracies of the algorithms were superior to those of human specialists in detecting scoliosis, detecting cases with a curve ≥20°, and severity grading for both binary classifications and the four-class classification. Our approach can be potentially applied in routine scoliosis screening and periodic follow-ups of pretreatment cases without radiation exposure.

As a result, many individuals are not aware that they may have scoliosis until the symptoms become quite severe, significantly affecting the physical and mental well-being of patients. Traditional screening methods for scoliosis demand significant physician effort and require unnecessary radiography exposure; thus, implementing large-scale screening is challenging. The application of deep learning algorithms has the potential to reduce unnecessary radiation risks as well as the costs of scoliosis screening.MethodsThe data of 247 scoliosis patients observed between 2008 and 2021 were used for training. The dataset included frontal, lateral, and back upright images as well as X-ray images obtained during the same period. We proposed and validated deep learning algorithms for automated scoliosis screening using upright back images. The overall process involved the localization of the back region of interest (ROI), spinal region segmentation, and Cobb angle measurements.ResultsThe results indicated that the accuracy of the Cobb angle measurement was superior to that of the traditional human visual recognition method, providing a concise and convenient scoliosis screening capability without causing any harm to the human body.ConclusionsThe method was automated, accurate, concise, and convenient. It is potentially applicable to a wide range of screening methods for the detection of early scoliosis.

Abstract:Purpose Adolescent idiopathic scoliosis (AIS) is a three-dimensional spine deformity governed by lateral curvature and axial vertebral rotation (AVR). Estimating AVR is important for treatment decisions and the prediction of AIS progression. However, manual AVR measurements have intra-observer and inter-observer errors, and therefore this paper proposes an automatic AVR measurement method to address the low precision issue of manual measurement. Method We develop an improved feature extraction module for vertebral landmark detection and pedicle segmentation. The improved coordinate convolution layer combined Polarized Self-Attention mechanism is applied in the feature extraction module to help the High-resolution Network to extract coordinate information. Based on vertebral landmark detection and pedicle segmentation, we propose an automatic AVR measurement algorithm to estimate the AVR. Results The mean radial error (MRE) of vertebral landmark detection is 2.70 mm pixels, and the dice coefficient of pedicle segmentation is 72.45%. Compared with the original model, the MRE decreases by 2.13 mm, and the dice coefficient improves by 4.61%. For the AVR measurement results, the testing set contains 37 spine radiographs, including 481 vertebrae and 962 AVR measurements (481 vertebrae by two observers). The average measurement error is lower than 5°, which is within the standard of clinical measurement error. Conclusion The results demonstrate that the proposed network performs well in vertebral landmark detection and pedicle segmentation, and the proposed AVR measurement is adopted for clinic diagnosis of AIS. Significance Our method achieves automatic AVR measurement, reducing the error introduced by manual measurement and improving the efficiency of orthopedists.

Abstract:Scoliosis, characterized by spine deformity, is most common in adolescent idiopathic scoliosis (AIS). Manual Cobb angle measurement limitations underscore the need for automated tools. This study employed a vertebral landmark extraction method and Feedforward Neural Network (FNN) to predict scoliosis progression in 79 AIS patients. The novel intervertebral angles matrix format showcased results. The mean absolute error for the intervertebral angle progression was 1.5 degrees, while the Pearson correlation of the predicted Cobb angles was 0.86. The accuracy in classifying Cobb angles ( 45°) was 0.85, with 0.65 sensitivity and 0.91 specificity. The FNN demonstrated superior accuracy, sensitivity, and specificity, aiding in tailored treatments for potential scoliosis progression. Addressing FNNs' over-fitting issue through strategies like "dropout" or regularization could further enhance their performance. This study presents a promising step towards automated scoliosis diagnosis and prognosis.

Abstract:Scoliosis, a prevalent condition characterized by abnormal spinal curvature leading to deformity, requires precise assessment methods for effective diagnosis and management. The Cobb angle is a widely used scoliosis quantification method that measures the degree of curvature between the tilted vertebrae. Yet, manual measuring of Cobb angles is time-consuming and labor-intensive, fraught with significant interobserver and intraobserver variability. To address these challenges and the lack of interpretability found in certain existing automated methods, we have created fully automated software that not only precisely measures the Cobb angle but also provides clear visualizations of these measurements. This software integrates deep neural network-based spine region detection and segmentation, spine centerline identification, pinpointing the most significantly tilted vertebrae, and direct visualization of Cobb angles on the original images. Upon comparison with the assessments of 7 expert readers, our algorithm exhibited a mean deviation in Cobb angle measurements of 4.17 degrees, notably surpassing the manual approach's average intra-reader discrepancy of 5.16 degrees. The algorithm also achieved intra-class correlation coefficients (ICC) exceeding 0.96 and Pearson correlation coefficients above 0.944, reflecting robust agreement with expert assessments and superior measurement reliability. Through the comprehensive reader study and statistical analysis, we believe this algorithm not only ensures a higher consensus with expert readers but also enhances interpretability and reproducibility during assessments. It holds significant promise for clinical application, potentially aiding physicians in more accurate scoliosis assessment and diagnosis, thereby improving patient care.

Abstract:Introduction Scoliosis is characterized by an abnormal curvature of the spine in the coronal plane. Idiopathic scoliosis is the most prevalent type, though specific causes are sometimes identifiable. Genetic factors significantly influence adolescent idiopathic scoliosis (AIS), which is diagnosed through clinical and radiographic evaluations, primarily using the Cobb angle to measure curvature severity. The classification of scoliosis severity ranges from mild scoliosis, where sometimes the absence of pain is encountered, to moderate and severe, which is usually associated with lancinating pain. Early onset and high progression rates in idiopathic scoliosis are indicative of poorer prognoses. Methods The study analyzed 197 radiographic images from a private clinic database between December 2023 and April 2024. Inclusion criteria focused on anteroposterior images of the thorax and abdomen, excluding unclear and non-spinal images. Manual Cobb angle measurements were performed using RadiAnt DICOM Viewer 2020.2, followed by automated measurements using the Cobb Angle Calculator software. Discrepancies led to further image processing with enhanced color contrast for improved visualization. Data were analyzed using GraphPad InStat to assess error margins between manual and automated measurements. Results Initial results indicated discrepancies between manual and automated Cobb angle measurements. Enhanced image processing improved accuracy, demonstrating the efficacy of both manual and automated techniques in evaluating spinal deformities. Statistical analysis revealed significant error margins, prompting a refined approach for minimizing measurement errors. Discussion The study highlights the importance of accurate Cobb angle measurement in diagnosing and classifying scoliosis. Manual measurements, while reliable, are time-consuming and prone to human error. Automated methods, particularly those enhanced by machine learning algorithms, offer promising accuracy and efficiency. The integration of image processing techniques further enhances the reliability of scoliosis evaluation. Conclusion Accurate assessment of scoliosis through Cobb angle measurement is crucial for effective diagnosis and treatment planning. The study demonstrates that combining manual techniques with advanced automated methods and image processing significantly improves measurement accuracy. Such an approach is intended to support better clinical outcomes. Future research should focus on refining these technologies for broader clinical applications.

Abstract:Abstract Purpose Artificial intelligence based on deep learning (DL) approaches enables the automatic recognition of anatomic landmarks and subsequent estimation of various spinopelvic parameters. The locations of inflection points (IPs) and apices (APs) in whole-spine lateral radiographs could be mathematically determined by a fully automatic spinal sagittal curvature analysis system. Methods We developed a DL model for automatic spinal curvature analysis of whole-spine lateral plain radiographs by using 1800 annotated images of various spinal disease etiologies. The DL model comprised a landmark localizer to detect 25 vertebral landmarks and a numerical algorithm for the generation of an individualized spinal sagittal curvature. The characteristics of the spinal curvature, including the IPs, APs, and curvature angle, could thus be analyzed using mathematical definitions. The localization error of each landmark was calculated from the predictions of 300 test images to evaluate the performance of the landmark localizer. The interrater reliability among a senior orthopedic surgeon, a radiologist, and the DL model was assessed using the intraclass correlation coefficient (ICC). Results The accuracy of the landmark localizer was within an acceptable range (median error: 1.7–4.1 mm), and the interrater reliabilities between the proposed DL model and each expert were good to excellent (all ICCs > 0.85) for the measurement of spinal curvature characteristics. Conclusion The interrater reliability between the proposed DL model and human experts was good to excellent in predicting the locations of IPs, APs, and curvature angles. Future applications should be explored to validate this system and improve its clinical efficiency.

Abstract:The Cobb angle measurement of the scoliotic spine is prone to inter- and intra-observer variations in the clinical setting. This paper proposes a deep learning architecture for detecting spine vertebrae from X-ray images to evaluate the Cobb angle automatically. The public AASCE MICCAI 2019 anterior-posterior X-ray image dataset and local images were used to train and test the proposed convolutional neural network architecture. Sixty-eight landmark features of the spine were detected from the input image to obtain seventeen vertebrae on the spine. The vertebrae locations obtained were processed to automatically measure the Cobb angle. The proposed method can measure the Cobb angle with accuracies up to 93.6% and has excellent reliability compared to clinicians’ measurement (intraclass correlation coefficient > 0.95). The proposed deep learning architecture may be used as a tool to augment Cobb angle measurement in X-ray images of patients with adolescent idiopathic scoliosis in a real-world clinical setting.

Abstract:Objectives: To quantify the magnitude of spinal deformity in adolescent idiopathic scoliosis (AIS), the Cobb angle is measured on X-ray images of the spine. Continuous exposure to X-ray radiation to follow-up the progression of scoliosis may lead to negative side effects on patients. Furthermore, manual measurement of the Cobb angle could lead to up to 10{\deg} or more of a difference due to intra/inter observer variation. Therefore, the objective of this study is to identify the Cobb angle by developing an automated radiation-free model, using Machine learning algorithms. Methods: Thirty participants with lumbar/thoracolumbar AIS (15{\deg} < Cobb angle < 66{\deg}) performed gait cycles. The lumbosacral (L5-S1) joint efforts during six gait cycles of participants were used as features to feed training algorithms. Various regression algorithms were implemented and run. Results: The decision tree regression algorithm achieved the best result with the mean absolute error equal to 4.6{\deg} of averaged 10-fold cross-validation. Conclusions: This study shows that the lumbosacral joint efforts during gait as radiation-free data are capable to identify the Cobb angle by using Machine learning algorithms. The proposed model can be considered as an alternative, radiation-free method to X-ray radiography to assist clinicians in following-up the progression of AIS.

Abstract:The current clinical gold standard for evaluating adolescent idiopathic scoliosis (AIS) is X-ray radiography, using Cobb angle measurement. However, the frequent monitoring of the AIS progression using X-rays poses a challenge due to the cumulative radiation exposure. Although 3D ultrasound has been validated as a reliable and radiation-free alternative for scoliosis assessment, the process of measuring spinal curvature is still carried out manually. Consequently, there is a considerable demand for a fully automatic system that can locate bony landmarks and perform angle measurements. To this end, we introduce an estimation model for automatic ultrasound curve angle (UCA) measurement. The model employs a dual-branch network to detect candidate landmarks and perform vertebra segmentation on ultrasound coronal images. An affinity clustering strategy is utilized within the vertebral segmentation area to illustrate the affinity relationship between candidate landmarks. Subsequently, we can efficiently perform line delineation from a clustered affinity map for UCA measurement. As our method is specifically designed for UCA calculation, this method outperforms other state-of-the-art methods for landmark and line detection tasks. The high correlation between the automatic UCA and Cobb angle (R$^2$=0.858) suggests that our proposed method can potentially replace manual UCA measurement in ultrasound scoliosis assessment.

Abstract:Ultrasound spine imaging technique has been applied to the assessment of spine deformity. However, manual measurements of scoliotic angles on ultrasound images are time-consuming and heavily rely on raters experience. The objectives of this study are to construct a fully automatic framework based on Faster R-CNN for detecting vertebral lamina and to measure the fitting spinal curves from the detected lamina pairs. The framework consisted of two closely linked modules: 1) the lamina detector for identifying and locating each lamina pairs on ultrasound coronal images, and 2) the spinal curvature estimator for calculating the scoliotic angles based on the chain of detected lamina. Two hundred ultrasound images obtained from AIS patients were identified and used for the training and evaluation of the proposed method. The experimental results showed the 0.76 AP on the test set, and the Mean Absolute Difference (MAD) between automatic and manual measurement which was within the clinical acceptance error. Meanwhile the correlation between automatic measurement and Cobb angle from radiographs was 0.79. The results revealed that our proposed technique could provide accurate and reliable automatic curvature measurements on ultrasound spine images for spine deformities.

For the X-ray images of 204 patients with idiopathic scoliosis who underwent full-spine radiography, histogram equalization of original image was performed before a flipping method was used to magnify asymmetric elements, search for the global maximum pixel value in each line, and scan local maximal pixel value, with the intersection set of two point sets being regarded as candidate anchor points. All fine anchors were fitted with cubic spline algorithm to obtain the approximate curve of the spine, and the degree of scoliosis was measured by the standardized integral area. All measured data were analyzed. In manual measurement, the Cobb angle was 11.70–25.00 (20.15 ± 3.60), 25.20–44.70 (33.89 ± 5.41), and 45.10–49.40 (46.98 ± 1.25) in the mild, moderate and severe scoliosis group, respectively, whereas the value for the standardized integral area algorithm was 0.072–0.298 (0.185 ± 0.040), 0.100–0.399 (0.245 ± 0.050), and 0.246–0.901 (0.349 ± 0.181) in the mild, moderate and severe scoliosis group, respectively. Correlation analysis between the manual measurement of the Cobb angle and the evaluation of the standardized integral area algorithm demonstrated the Spearman correlation coefficient r = 0.643 ( P < 0.001). There was a positive correlation between the manual measurement of the Cobb angle and the measurement of the standardized integral area value. Two methods had good consistency in evaluating the degree of scoliosis. ROC curve analysis of the standardized integral area algorithm to measure the degree of scoliosis showed he cutoff value of the standardized integral area algorithm was 0.20 for the moderate scoliosis with an AUC of 0.865, sensitivity 0.907, specificity 0.635, accuracy 0.779, positive prediction value 0.737 and negative prediction value 0.859, and the cutoff value of the standardized integral area algorithm was 0.40 for the severe scoliosis with an AUC of 0.873, sensitivity 0.188, specificity 1.00, accuracy 0.936, positive prediction value 1 and a negative prediction value 0.935. Using the standardized integral area as an independent variable and the Cobb angle as a dependent variable, a linear regression equation was established as Cobb angle = 13.36 + 70.54 × Standardized area, the model has statistical significance. In conclusion, t he integrated area algorithm method of machine learning can quickly and efficiently assess the degree of scoliosis and is suitable for screening the degree of scoliosis in a large dataset as a useful supplement to the fine measurement of scoliosis Cobb angle.

Abstract:Scoliosis is a three-dimensional deformity of the spine, most often diagnosed in childhood. It affects 2-3% of the population, which is approximately seven million people in North America. Currently, the reference standard for assessing scoliosis is based on the manual assignment of Cobb angles at the site of the curvature center. This manual process is time consuming and unreliable as it is affected by inter- and intra-observer variance. To overcome these inaccuracies, machine learning (ML) methods can be used to automate the Cobb angle measurement process. This paper proposes to address the Cobb angle measurement task using YOLACT, an instance segmentation model. The proposed method first segments the vertebrae in an X-Ray image using YOLACT, then it tracks the important landmarks using the minimum bounding box approach. Lastly, the extracted landmarks are used to calculate the corresponding Cobb angles. The model achieved a Symmetric Mean Absolute Percentage Error (SMAPE) score of 10.76%, demonstrating the reliability of this process in both vertebra localization and Cobb angle measurement.

Abstract:Analyzing spinal curvatures manually is time-consuming and tedious for clinicians, and intra-observer and inter-observer variability can affect manual measurements. In this study, we developed and evaluated the performance of an automated deep learning–based computer-aided diagnosis (CAD) tool for measuring the sagittal alignment of the spine from X-ray images. The CAD system proposed here performs two functions: deep learning–based lateral spine segmentation and automatic analysis of thoracic kyphosis and lumbar lordosis angles. We utilized 322 datasets with data augmentation for learning and fivefold cross-validation. The segmentation model was based on U-Net, which has multiple applications in medical image processing. Here, we utilized parameter equations and trigonometric functions to design spinal angle measurement algorithms. The kyphosis (T4–T12) and lordosis angle (L1–S1, L1–L5) were automatically measured to help diagnose kyphosis and lordosis. The segmentation model had precision, sensitivity, and dice similarity coefficient values of 90.53 ± 4.61%, 89.53 ± 1.8%, and 90.22 ± 0.62%, respectively. The performance of the CAD algorithm was also verified with the Pearson correlation, Bland–Altman, and intra-class correlation coefficient (ICC) analysis. The proposed angle measurement algorithm exhibited high similarity and reliability during verification. Therefore, CAD can help clinicians in reaching a diagnosis by analyzing the sagittal spinal curvatures while reducing observer-based variability and the required time or effort.

Abstract:Study Design: Retrospective cohort study. Objective: To develop a machine learning (ML) model that predicts the progression of AIS using minimal radiographs and simple questionnaires during the first visit. Summary of Background Data: Several factors are associated with angle progression in patients with AIS. However, it is challenging to predict angular progression at the first visit. Methods: Among female patients with AIS treated at a single institution from July 2011 to February 2023, 1119 cases were studied. Patient data, including demographic and radiographic data based on anterior-posterior and lateral whole-spine radiographs, were collected at the first and last visits. The last visit was defined differently based on treatment plans. For patients slated for surgery or bracing, the last visit occurred just before these interventions. For others, it was their final visit before turning 18 years. Angular progression was defined as a Cobb angle greater than 25 degrees for each of the proximal thoracic (PT), main thoracic (MT), and thoracolumbar/lumbar (TLL) curves at the last visit. ML algorithms were employed to develop individual binary classification models for each type of curve (PT, MT, and TLL) using PyCaret in Python. Multiple models were explored and analyzed, with the selection of optimal models based on the area under the curve (AUC) and Recall scores. Feature importance was evaluated to understand the contribution of each feature to the model predictions. Results: For PT, MT, and TLL progression, the top-performing models exhibit AUC values of 0.94, 0.89, and 0.84, and achieve recall rates of 0.90, 0.85, and 0.81. The most significant factors predicting progression varied for each curve: initial Cobb angle for PT, presence of menarche for MT, and Risser grade for TLL. Conclusions: This study introduces an ML-based model using simple data at the first visit to precisely predict angle progression in female patients with AIS.

Abstract:Canadian Association of Radiologists Journal, Ahead of Print. Purpose: Scoliosis is a complex spine deformity with direct functional and cosmetic impacts on the individual. The reference standard for assessing scoliosis severity is the Cobb angle which is measured on radiographs by human specialists, carrying interobserver variability and inaccuracy of measurements. These limitations may result in lack of timely referral for management at a time the scoliotic deformity progression can be saved from surgery. We aimed to create a machine learning (ML) model for automatic calculation of Cobb angles on 3-foot standing spine radiographs of children and adolescents with clinical suspicion of scoliosis across 2 clinical scenarios (idiopathic, group 1 and congenital scoliosis, group 2). Methods: We retrospectively measured Cobb angles of 130 patients who had a 3-foot spine radiograph for scoliosis within a 10-year period for either idiopathic or congenital anomaly scoliosis. Cobb angles were measured both manually by radiologists and by an ML pipeline (segmentation-based approach—Augmented U-Net model with non-square kernels). Results: Our Augmented U-Net architecture achieved a Symmetric Mean Absolute Percentage Error (SMAPE) of 11.82% amongst a combined idiopathic and congenital scoliosis cohort. When stratifying for idiopathic and congenital scoliosis individually a SMAPE of 13.02% and 11.90% were achieved, respectively. Conclusion: The ML model used in this study is promising at providing automated Cobb angle measurement in both idiopathic scoliosis and congenital scoliosis. Nevertheless, larger studies are needed in the future to confirm the results of this study prior to translation of this ML algorithm into clinical practice.
