Abstract:Continuous glucose monitoring (CGM)-derived metrics have been used to accurately assess glycemic variability (GV) to facilitate management of diabetes mellitus, yet their relationship with diabetic peripheral neuropathy (DPN) is not fully understood. We performed a systematic review and meta-analysis to evaluate the association between GV metrics and the risk of developing DPN. Nine studies totaling 3,649 patients with type 1 and type 2 diabetes mellitus were included. A significant association was found between increased GV, as indicated by metrics including standard deviation (SD) with OR and 95% CI of 2.58 (1.45-4.57), mean amplitude of glycemic excursions (MAGE) with OR and 95% CI of 1.90 (1.01-3.58), mean of daily difference (MODD) with OR and 95% CI of 2.88 (2.17-3.81) and the incidence of DPN. Our findings support a link between higher GV and an increased risk of DPN in patients with diabetes. These findings highlight the potential of GV metrics as indicators for the development of DPN, advocating for their inclusion in diabetes management strategies to potentially mitigate neuropathy risk. Longitudinal studies with longer observation periods and larger sample sizes are necessary to validate these associations across diverse populations.
The value of multiple neurophysiological tests in the diagnosis of diabetic peripheral neuropathy

Abstract:Objective To evaluate the value of united nerve electrophysiological tests in the diagnosis of diabetic peripheral neuropathy (DNP).Methods The quantitative temperature threshold (QTT), including the cold sensation threshold (CST) , thermal sensation threshold (WST), cold pain threshold (CPT) and thermal pain threshold (HPT), sympathetic skin response (SSR) and nerve conduction velocity (NCV) were measured for 85 diabetic patients.Results The abnormal rate of QTT was 84.71％ , significantly higher than that of SSR and NCV (56.47％ and 31.76％ respectively).However, no significant difference was found in the abnormal rate of QTT between the DPN asymptomatic group and DPN symptomatic group (78.85％ and 93.94％ respectively).There was significant difference in the abnormal rate of SSR (48.08％ and 69.70％ respectively) and the abnormal rate of NCV (19.23％ and 51.52％ respectively) between the above two groups(P ＜ 0.05).There was no difference in the abnormal rate of QTT for patients with short or long course of disease (77.77％ and 89.80％ respectively), but significant difference in the abnormal rate of SSR (44.44％ and 65.31％ respectively) and the abnormal rate of NCV (19.44％ and 40.82％ respectively) Conclusion The abnormal rate of QTT was highest in detecting the diabetic patients, and it is not related to clinical symptoms or disease course.However, the abnormal rates of SSR and NCV were related to clinical symptoms and course.It is more sensitive to diagnose DPN using united electrophysiological tests of QTT, SSR and NCV.
Glycemic Variability and Diabetic Neuropathy in Young Adults With Type 1 Diabetes

Abstract:Background: Glycemic variability (GV) may attribute to the pathogenesis of diabetic neuropathy. The aim of this cross-sectional study was to investigate the association between GV and distal symmetric polyneuropathy (DSPN) and cardiovascular autonomic neuropathy (CAN) in a Danish population of young adults with type 1 diabetes.Methods: Young adults between 18 and 24 years with type 1 diabetes were included in this cross-sectional study. CAN was assessed by cardiovascular autonomic reflex tests (CARTs) and heart rate variability (HRV). DSPN was assessed by light pressure, pain and vibration perception, electrochemical skin conductance, sural nerve conduction velocity (SNCV), and amplitude potential (SNAP). GV were obtained by continuous glucose monitoring including coefficient of variation (CV), SD, continuous overall net glycemic action (CONGA), and mean amplitude of glucose excursions (MAGE).Results: The study comprised 133 young adults (43.6% males), mean age of 22 years (SD 1.6). Unadjusted, higher CV was associated with a decreased risk of sural nerve conduction (P = 0.03), abnormal SNAP (P = 0.04) and incidents of definite CAN (P = 0.04). Likewise, higher CONGA was associated with increasing incidents of subclinical DSPN (P = 0.03), abnormal SNAP (P = 0.01), and SNCV (P = 0.02). However, both associations were not statistically significant in the fully adjusted model. Higher MAGE was associated with slightly increasing measures of HRV (P = 0.03) but only when fully adjusted. When correcting for multiple tests significance was lost. A significant association was found between HbA1c and measures of both DSPN (P < 0.02) and HRV (P < 0.03) in fully adjusted models.Conclusions: No significant associations between GV and diabetic neuropathy were found after adjusting for risk factors and multiple tests. This suggests that GV may not be a risk factor for diabetic neuropathy in young adults with type 1 diabetes. However, long-term effects of GV excursions may still play a role in the pathogenic mechanisms leading to neuropathy in later life.

Abstract:Graphical abstract Text The gold standard electromyography system diagnosis of diabetic polyneuropathy can be replicated using two simple quantitative tests: DPNCheckTM and coefficient of variation of R‐R intervals. By combining these tests, we have developed an estimation formula with excellent diagnostic performance. The use of DPNCheck and electrocardiogram would simplify the diagnosis of diabetic polyneuropathy, making it more accessible, reproducible and reliable. Aims/Introduction This study aimed to investigate the diagnostic potential of two simplified tests, a point‐of‐care nerve conduction device (DPNCheckTM) and a coefficient of variation of R‐R intervals (CVR‐R), as an alternative to traditional nerve conduction studies for the diagnosis of diabetic polyneuropathy (DPN) in patients with diabetes. Materials and Methods Inpatients with type 1 or type 2 diabetes (n = 167) were enrolled. The study population consisted of 101 men, with a mean age of 60.8 ± 14.8 years. DPN severity was assessed using traditional nerve conduction studies, and differentiated based on Baba's classification (BC). To examine the explanatory potential of variables in DPNCheckTM and CVR‐R regarding the severity of DPN according to BC, a multiple regression analysis was carried out, followed by a receiver operating characteristic analysis. Results Based on BC, 61 participants (36.5% of the total) were categorized as having DPN severity of stage 2 or more. The multiple regression analysis yielded a predictive formula with high predictive power for DPN diagnosis (estimated severity of DPN in BC = 2.258 – 0.026 × nerve conduction velocity [m/s] – 0.594 × ln[sensory nerve action potential amplitude (μV)] + 0.528In[age(years)] – 0.178 × ln[CVR‐R], r = 0.657). The area under the curve in receiver operating characteristic analysis was 0.880. Using the optimal cutoff value for DPN with severer than stage 2, the predictive formula showed good diagnostic efficacy: sensitivity of 83.6%, specificity of 79.2%, positive predictive value of 51.7% and negative predictive value of 76.1%. Conclusions These findings suggest that DPN diagnosis using DPNCheckTM and CVR‐R could improve diagnostic efficiency and accessibility for DPN assessment in patients with diabetes.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Aims</h3>Continuous glucose monitoring (CGM)-derived time-in-range (TIR) of 3.9-10 mmol/L is associated with diabetic retinopathy in type 2 diabetes (T2DM), but its relationship to peripheral nerve function has not been previously investigated. To explore the association between the TIR and nerve conduction study parameters in patients with T2DM, we performed a cross-sectional analysis.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3>A total of 740 patients with T2DM were enrolled in this study. All of the participants were divided into tertiles according to the TIR (TIR low: ≤53%; TIR medium: 54-76%; TIR high: ≥77%). Composite Z-scores of nerve conduction velocity (CV), latency, and amplitude were calculated. The linear correlation between the TIR and composite nerve function Z-score was evaluated and risk assessment was analysed using binary logistic regression.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3>The composite Z-score of the CV and amplitude increased with higher TIR and the composite Z-score of latency significantly decreased as the TIR tertiles increased (all P trend<0.05). After adjusting for age, diabetes duration, height, weight and other confounding factors, higher TIR was associated with a higher composite Z-score of CV (β=0.230, P<0.001), amplitude (β=0.099, P=0.010), and lower composite Z-score of latency (β=-0.172, P<0.001). The risk of TIR tertiles and low composite Z-score of CV remained significant even after adjustment of HbA1c (TIR medium: OR=0.48, P=0.001; TIR high: OR=0.41, P<0.001).<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3>Higher TIR tertiles were independently associated with better peripheral nerve function. CGM-derived TIR may be a promising approach to screen patients for further assessment of possible diabetic peripheral neuropathy.

Abstract:Purpose To evaluate the diagnostic performance of corneal confocal microscopy (CCM) in assessing corneal nerve parameters in patients with diabetic peripheral neuropathy (DPN). Methods Studies in the literature that focused on CCM and DPN were retrieved by searching PubMed, Excerpt Medica Database (EMBASE) and China National Knowledge Infrastructure (CNKI) databases. RevMan V.5.3 software was used for the meta-analysis. The results are presented as weighted mean difference (WMD) with a corresponding 95% CI. Results 13 studies with a total of 1680 participants were included in the meta-analysis. The pooled results showed that the corneal nerve fibre density, nerve branch density and nerve fibre length were significantly reduced (all p<0.00001) in the patients with DPN compared with healthy controls ((WMD=−18.07, 95% CI −21.93 to −14.20), (WMD=−25.35, 95% CI −30.96 to −19.74) and (WMD=−6.37, 95% CI −7.44 to −5.30)) and compared with the diabetic patients without DPN ((WMD=−8.83, 95% CI −11.49 to −6.17), (WMD=−13.54, 95% CI −20.41 to −6.66) and (WMD=−4.19, 95% CI −5.35 to −3.04)), respectively. No significant difference was found in the corneal nerve fibre tortuosity coefficient between diabetic patients with DPN and healthy controls (p=0.80) or diabetic patients without DPN (p=0.61). Conclusions This meta-analysis suggested that CCM may be valuable for detecting and assessing early nerve damage in DPN patients.
Screening and Prevalence of Peripheral Neuropathy in Type 2 Diabetic Outpatients: a Randomized Multicentre Survey in 12 City Hospitals of China

Abstract:Background The 10-g Semmes-Weinstein monofilament and 128-Hz tuning fork examinations are considered simple and effective screening instruments for diabetic peripheral neuropathy. This study investigated the prevalence of diabetic peripheral neuropathy of combining both methods in type 2 diabetic outpatients in Chinese urban hospitals.Methods Pressure sensation and vibration perception were randomly screened using 10-g Semmes-Weinstein monofilament and 128-Hz tuning fork in type 2 diabetic outpatients from 12 centres in 8 Chinese cities in a randomized, multicentre epidemiological study. The prevalence and treatment status of neuropathy were assessed, and relevant risk factors were analysed.Results A total of 1197 outpatients were screened and 1193 were enrolled in this study, including 1067 known cases of diabetes (KDM group) and 126 newly diagnosed cases of diabetes (NDM group). Screening results showed that the prevalence of perception defects was 17.02% in the total population, 18.28% in the known DM group and 6.35% in the new DM group, respectively. The frequency of previous diabetic peripheral neuropathy examination was only 32% in the known DM group and 31.75% in the new DM group. Thirty-two percent of known DM patients with diabetic peripheral neuropathy did not receive any treatment formerly. It also revealed that there was a region-wise difference in prevalence of diabetic peripheral neuropathy, and it increased with age and diabetic duration. Retinopathy, HbA(1c), diabetes duration and age were independent risk factors for diabetic peripheral neuropathy.Conclusions The screening and treatment rate of diabetic peripheral neuropathy is low in China. The prevalence of diabetic peripheral neuropathy in type 2 diabetic outpatients in Chinese city hospitals is 17.2%. Diabetic retinopathy is an independent risk factor for diabetic peripheral neuropathy. Copyright (C) 2010 John Wiley & Sons, Ltd.

Abstract:The aim was to assess the use of multi-frequency vibrometry (MFV) in detecting diabetic peripheral neuropathy (DPN) in type 1 diabetes in comparison to nerve conduction studies (NCS) and neurothesiometer (NT). Our objectives were to examine how VPTs correlated with NCS parameters, evaluate the efficacy of MFV in distinguishing DPN as well as to investigate whether MFV procedure could be based on fewer frequencies. Adults with type 1 diabetes with previous MFV examinations were recruited at Skåne University Hospital in Malmö, Sweden, between 2018 and 2020. Participants were examined regarding nerve function in the lower limbs through MFV, NT and NCS. A total of 66 participants (28 women and 38 men) with a median age of 50 (39 to 64) years were included in the study. Through NCS assessment, 33 participants (50%) were diagnosed with DPN. We found negative correlations between VPTs and all NCS parameters, where the strongest correlation was found between sural nerve amplitude and the 125 Hz frequency of MFV. A combination of four frequencies, two low (4 and 8 Hz) and two high (125 and 250 Hz), showed the highest classification efficacy (AUC 0.83, 95% CI 0.73–0.93). We conclude that a strong correlation exists between the sural nerve amplitude and the VPTs at 125 Hz and that VPT testing with MFV can be focused on only four frequencies instead of seven, thus shortening test time, to distinguish DPN in the lower limb.

Abstract:Background: To compare glycemic variability (GV) indices between patients with fibrocalculous pancreatic diabetes (FCPD) and type 2 diabetes mellitus (T2D) using continuous glucose monitoring (CGM). Methods: We measured GV indices using CGM (iProTM2 Professional CGM, Medtronic, USA) data in 61 patients each with FCPD and T2D who were matched for glycated hemoglobin A1c (HbA1c) and duration of diabetes. GlyCulator2 software was used to estimate the CGM-derived measures of GV (SD, mean amplitude of glycemic excursion [MAGE], continuous overall net glycemic action [CONGA], absolute means of daily differences [MODD], M value, and coefficient of variance [%CV]), hypoglycemia (time spent below 70 mg/dL, AUC below 70 mg/dL, glycemic risk assessment diabetes equation hypoglycemia, Low Blood Glucose Index), and hyperglycemia (time spent above 180 mg/dL at night [TSA > 180], AUC above 180 mg/dL [AUC > 180], glycemic risk assessment diabetes equation hyperglycemia, High Blood Glucose Index [HBGI], and J index). The correlation of GV indices with HbA1c, duration of diabetes, and demographic and biochemical parameters were also assessed. Results: All the CGM-derived measures of GV (SD, MAGE, CONGA, MODD, and %CV), except M value, were significantly higher in the FCPD group than in the T2D group ( P 180, AUC >180, HBGI, and J index) were significantly higher in the FCPD group than in the T2D group ( P < 0.05). The measures of hypoglycemia were not significantly different between the two groups. All the hyperglycemia indices showed a positive correlation with HbA1c in both groups. Conclusions: FCPD is associated with higher GV than is T2D. The findings of higher postprandial glycemic excursions in patients with FCPD could have potential therapeutic implications.
Glycemic variability's impact on painful diabetic peripheral neuropathy in type 2 diabetes patients

Abstract:Hyperglycemia in type 2 diabetes leads to diabetic peripheral neuropathy (DPN) and neuropathic pain, yet the association between glycemic variability and painful DPN remains insufficiently evidenced. To address this, we conducted a prospective longitudinal cohort study involving adult type 2 diabetes patients at a medical center. DPN was identified using the Michigan Neuropathy Screening Instrument (MNSI), and neuropathic pain was assessed with the Taiwan version of the Douleur Neuropathique 4 (DN4-T) questionnaire. At baseline in 2013, all participants were free of DPN and were re-evaluated in 2019 for the development of painful DPN. We measured visit-to-visit glycemic fluctuations using the coefficient of variation (CV) of fasting plasma glucose (FPG) and glycated hemoglobin (HbA1c). Patients were stratified into tertiles according to their FPG-CV and HbA1c-CV. Among the 622 participants, 267 developed DPN during the six-year follow-up. Following matching of age and sex, 210 patients without DPN and 210 with DPN (including 26 with neuropathic pain) were identified. Our findings revealed a significant association between high FPG-CV and painful DPN, with the highest tertile showing an adjusted odds ratio of 2.82 (95% confidence interval 1.04–7.64) compared to the lowest tertile. On the contrary, HbA1c-CV did not show a significant association with the risk of painful DPN. Our study indicates that higher FPG-CV is associated with an increased risk of painful DPN, supporting the role of glycemic variability in the development of painful DPN.

Correlation Between Glycemic Variability and Sudomotor Function in Patients with Type 2 Diabetes

Abstract:Objective To investigate the correlation between glycemic variability and sudomotor function in patients with type 2 diabetes mellitus (T2DM). Methods A total of 395 cases with T2DM in the Department of Endocrinology, Nanjing General Hospital of the PLA from March 2016 to May 2017 were enrolled. According to the electrical skin conductivities (ESC) of hands and feet measured with SUDOSCAN, a conductance analyzer, the patients were divided into two groups: 221 cases with normal sudomotor function (normal group, both ESC of hands and feet >60 μs, including 144 males, 77 females) and 174 cases with abnormal sudomotor function (abnormal group, ESC of hands or feet ≤60 μs, including 104 males and 70 females). All participants were monitored with the continuous glucose monitoring system for consecutive 72 hours. Clinical and biochemical indexes were recorded. Biochemical indexes and glycemic variability parameters were compared between the two groups. Spearman correlation analysis and the multiple linear regression analysis were adopted for statistical analysis. Results (1) Compared with the normal sudomotor function group, the abnormal group were with older age [(59±12) vs (52±13) yrs, t=-5.252, P 9.6% subgroup (r value: 0.058-0.368 vs 0.003-0.282, 0.007-0.237). The correlation coefficients between HESC, FESC, hand mean asymmetry, foot mean asymmetry and glycemic variability parameters were generally larger in male group than those in female group (r value: 0.023-0.215 vs 0.003-0.219). (4) Stepwise multiple linear regression analysis revealed that MAGE, duration of disease and creatinine were the independent risk factors affecting sudomotor function (B=-1.550, -0.758, -0.118, all P<0.05). Conclusion MAGE, duration of disease and creatinine are the independent risk factors affecting sudomotor function in patients with T2DM. Glycemic variability are more likely to affect the sudomotor function in male patients and patients with lower level of HbA1c. Abstract:Objective: To evaluate the efficacy of corneal confocal microscopy (CCM) as a non-invasive test to assess diabetic peripheral neuropathy in Chinese patients diagnosed with type 2 diabetes. Research design and methods: Diabetic distal symmetric polyneuropathy (DSPN) and its severity degrees were assessed based on the modified Toronto diagnostic criteria in 128 patients with type 2 diabetes (No DSPN [n = 49], mild DSPN [n = 43], moderate-to-severe DSPN [n = 36]) and 24 age-matched controls. CCM was also examined in all enrolled subjects. Corneal nerve fiber length (CNFL), corneal nerve branch density (CNBD) and corneal nerve fiber density (CNFD) were analyzed by Fiji imaging analysis software. The efficacy of CCM as a non-invasive test to assess diabetic peripheral neuropathy was determined. Results: CNFL was 17.99 +/- 0.66, 15.82 +/- 0.64, 14.98 +/- 0.63, and 12.49 +/- 0.93 in healthy controls, T2DM patients with no, mild, and moderate-to-severe DPN, respectively. CNFL in type 2 diabetes patients with no, mild, and moderate-to-severe DSPN demonstrated a significant reduction than in healthy controls (P = .012, .003 and < .001, respectively). CNFL in patients with moderate-to-severe DSPN was significantly shorter than in patients with no or mild DSPN (P < .001 and .004, respectively). CNBD was 41.48 +/- 3.35, 33.02 +/- 2.50, 30.91 +/- 2.33, and 18.00 +/- 2.33 in healthy controls, T2DM patients with no, mild, and moderate-to-severe DPN, respectively. CNBD in healthy control was significantly higher than in type 2 diabetes patients with no, mild, and moderate-to-severe DSPN (P = .036, 0.016 and < .001, respectively). CNBD in patients with moderate-to-severe DSPN was significantly lower than in patients with no or mild DSPN (P < .001 for both). CNFD was 35.32 +/- 1.18, 35.68 +/- 1.10, 34. 54 +/- 1.12, and 32.28 +/- 1.76 in healthy controls, T2DM patients with no, mild, and moderate-to-severe DPN, respectively. CNFD did not differ among the four groups. In an analysis that divided CNFL, CNFD and CNBD into quartiles, there were no significant differences in electromyography findings and vibration perception threshold among the 4 groups; however, significant differences were seen in the positive distribution of temperature perception measurements following CNFL and CNBD stratification (P=.001 and <.001, respectively). Glycemic Variability is an Important Risk Factor for Cardiovascular Autonomic Neuropathy in Newly Diagnosed Type 2 Diabetic Patients.

Abstract:BackgroundThe relationship between glycemic variability, another component of glycemic disorders as well as chronic sustained hyperglycemia, and cardiovascular autonomic neuropathy (CAN) has not been clarified. Our aim is to investigate the association between glycemic variability and CAN in newly diagnosed type 2 diabetic patients.MethodsEwing tests were performed in 90 newly diagnosed type 2 diabetic patients and 37 participants with normal glucose tolerance as control from May 1, 2009, through September 30, 2010. According to the scores from Ewing tests, diabetic patients were divided into two groups: without CAN (CAN−) and with CAN (CAN+). All participants underwent a 48-h to 72-h continuous glucose monitoring (CGM). Coefficient of variability of glycemia (%CV), mean amplitude of glycemic excursions (MAGE) and means of daily differences (MODD) were calculated with the CGM data.ResultsThe prevalence of CAN in patients with newly diagnosed type 2 diabetes was 22.2%. An increasing trend of glycemic variability was found from control group, CAN− group to CAN+ group. MAGE in CAN+ group was significantly higher than that in CAN− group (5.27±1.99mmol/L vs. 4.04±1.39mmol/L, P=0.001). In the Logistic regression analysis, a significant relationship was shown between MAGE and CAN [odds ratio (OR): 1.73, 95% confidence interval (CI): 1.01–2.73, P=0.018)]. The area under the receiver-operating characteristic curve for MAGE was superior to those for other dysglycemic indices in detecting CAN.ConclusionsGlycemic variability is associated with CAN in patients with newly diagnosed type 2 diabetes. Among the glycemic variability indices, MAGE is a significant indicator for detecting CAN.

The coefficient of variation of glycated hemoglobin A1c (HbA1c-CV) and the mean glycated hemoglobin A1c(HbA1c-Mean) are considered as indexes of HbA1c variability. Multivariate logistic regression analysis, the generalized linear model and ROC curve correlation analysis were used to analyze the correlation of various factors and VPT. Results: The multivariate logistic regression analysis revealed that age, systolic blood pressure (SBP), and HbA1c-CV were identified as risk factors for vibration sensation deficiency in middle-aged and elderly patients with T2DM, while estimated glomerular filtration rate (eGFR), triiodothyronine (T3), and alanine aminotransferase (ALT) were considered as protective factors. In the unadjusted generalized linear model, a significant association was observed between HbA1c-CV and VPT values. After adjusting for age, diabetic duration, SBP, homeostatic model assessment for beta-cell function (HOMA-β), ALT, eGFR, T3, 24-hour urinary protein excretion levels, and HbA1c-Mean, HbA1c-CV remained significantly correlated with VPT values on both sides. (left side, B=2.560, 95% CI 1.298~3.823; P< 0.001; right side, B=2.608, 95% CI 1.498~3.718, P< 0.001). The area under the curve (AUC) for HbA1c-CV and VSD prevalence was 0.723, with a sensitivity of 79.85%, specificity of 56.22%. Conclusion: The risk of developing VSD increases proportionally with higher HbA1c-CV levels in middle-aged and elderly patients with T2DM. Reaching and maintaining blood glucose stability is essential to the mitigation of diabetes peripheral neuropathy occurrence. The prevalence of diabetic peripheral neuropathy (DPN) 2 is remarkably high among individuals with diabetes mellitus, making it one of the most prevalent complications associated with this metabolic disorder. Due to the insidious onset and slow progression of DPN, clinical symptoms often manifest when irreversible pathological changes have already occurred in most peripheral nerves, thereby predisposing individuals to foot ulcers and an elevated risk of amputation. According to the Standards of Medical Care in Diabetes published by the American Diabetes Association (ADA), the prevalence of DPN is reported to be 13.1%. 3 DPN significantly impacting the overall quality of life for individuals with diabetes mellitus. Potential Benefits of Continuous Glucose Monitoring for Predicting Vascular Outcomes in Type 2 Diabetes: A Rapid Review of Primary Research

Abstract:(1) Background: Chronic hyperglycaemia is a cause of vascular damage and other adverse clinical outcomes in type 2 diabetes mellitus (T2DM). Emerging evidence suggests a significant and independent role for glycaemic variability (GV) in contributing to those outcomes. Continuous glucose monitoring (CGM) provides valuable insights into GV. Unlike in type 1 diabetes mellitus, the use of CGM-derived GV indices has not been widely adopted in the management of T2DM due to the limited evidence of their effectiveness in predicting clinical outcomes. This study aimed to explore the associations between GV metrics and short- or long-term vascular and clinical complications in T2DM. (2) Methods: A rapid literature review was conducted using the Cochrane Library, MEDLINE, and Scopus databases to seek high-level evidence. Lower-quality studies such as cross-sectional studies were excluded, but their content was reviewed. (3) Results: Six studies (five prospective cohort studies and one clinical trial) reported associations between GV indices (coefficient of variation (CV), standard deviation (SD), Mean Amplitude of Glycaemic Excursions (MAGE), Time in Range (TIR), Time Above Range (TAR), and Time Below Range (TBR)), and clinical complications. However, since most evidence came from moderate to low-quality studies, the results should be interpreted with caution. (4) Conclusions: Limited but significant evidence suggests that GV indices may predict clinical compilations in T2DM both in the short term and long term. There is a need for longitudinal studies in larger and more diverse populations, longer follow-ups, and the use of numerous CGM-derived GV indices while collecting information about all microvascular and macrovascular complications.
Effectiveness of Clinical Alternatives to Nerve Conduction Studies for Screening for Diabetic Distal Symmetrical Polyneuropathy: A Multi-Center Study.

Abstract:Aim: This study explored the possibility of developing an alternative, simple and rapid test for the screening of distal symmetrical polyneuropathy (DSPN), for use in local primary care facilities.Methods: 4023 patients with type 2 diabetes mellitus (T2DM), aged 18-80 years, from 21 hospitals in China were screened for inclusion in a prospective diagnostic assessment of DSPN, between April 2011 and March 2012. Patients underwent nerve conduction study (NCS; as the gold standard) and clinical assessments of ankle reflexes (AR) and thermal (T), vibration (V), pressure (P), and pinprick (PP) sensations.Results: Results from various combinations of these clinical assessments were compared to determine the optimal screening regimens. The Toronto Consensus Panel definition was used to confirm clinical DSPN (symptoms or signs plus abnormal NCS). 3883 of the 4023 patients (M:F, 2162:1721; mean age, 56.79 + 13.16 years) were included. No significant differences were found between the two most efficient clinical assessment combinations, AR + PP + T + V and AR + T + V (P = 0.9612). Compared to the diagnosis made using AR + T + V, the same diagnosis was reached using AR + PP + P + V + T in 100% of patients with probable clinical DSPN, 84.05% with possible clinical DSPN, and 96.84% with no DSPN. The AR + T + V assessment was also similar to 40% faster than the AR + PP + P + V + T assessment.Conclusions: Combined AR + T + V assessment is a valuable tool for screening diabetic patients for DSPN, potentially decreasing costs to local primary care facilities and making care more available to underserved populations. Abstract:Clinical studies investigating the benefit of glucose control on the progression of diabetic neuropathy (DN) have come to controversial results in patients with type 2 diabetes (T2D). This study aimed to assess associations of HbA1c levels with parameters of nerve perfusion in patients with T2D with and without DN using dynamic contrast-enhanced magnetic resonance neurography (DCE-MRN) at 3 Tesla. A total of 58 patients with T2D (20 with DN and 38 without DN) took part in this cross-sectional single-center study. Groups were matched for age, BMI, HbA1c, duration of T2D, and renal function. All patients underwent DCE-MRN with subsequent electrophysiologic and serologic testing. The extended Tofts model was used to quantify the sciatic nerve's microvascular permeability (Ktrans), volume fraction of the extracapillary extracellular space, and volume fraction of the plasma space. As a main result, we found that Ktrans correlated positively with HbA1c in patients with DN, while a negative correlation between the two parameters was found in patients without DN. Our results indicate that the effect of glucose control on the capillary permeability of peripheral nerves differs between patients with T2D with and without DN.

Abstract:Zhao et al [1] from Shanghai, China, have undertaken a detailed clinical study in a cohort of 206 asymptomatic patients with type 2 diabetes utilizing advanced in vivo nerve imaging with corneal confocal microscopy (CCM-Heidelberg HRT III RCM) and continuous glucose monitoring (CGM-iPro2 system) over 7 days. The study provides important insights into the relationship between relatively short-term glucose perturbation over 7 days and corneal nerve loss in diabetic neuropathy. Furthermore, it reinforces the role of CCM in detecting early sub-clinical nerve loss, as almost one-third of asymptomatic patients had corneal nerve fiber loss, which was independently associated with glucose time in range (TIR) (percentage of time within the glucose range of 3.9–10.0 mmol/L) but not with quartiles of hemoglobin A1c (HbA1c), an established measure of long-term glycemic control. Indeed, each 10% increase in TIR was associated with a 28.2% (95% CI: 0.595–0.866, P = 0.001) decrease in the risk of abnormal corneal nerve fiber length (CNFL). This supports recent studies showing that TIR is associated with other long-term complications of diabetes, including symptomatic diabetic neuropathy in patients with nephropathy [2] and diabetic retinopathy. Abstract:Background: This study explored the correlation between pancreatic islet α cell function, as reflected by the plasma glucagon levels, and Diabetic Peripheral Neuropathy (DPN) in patients with Type 2 Diabetes Mellitus (T2DM). Methods: A total of 358 patients with T2DM were retrospectively enrolled in this study and divided into the Non-DPN (NDPN) group (n = 220) and the DPN group (n = 138). All patients underwent an oral glucose tolerance test to detect levels of blood glucose, insulin and glucagon, and the Area Under the Curve (AUC) for Glucagon (AUCglu) was used to estimate the overall glucagon level. The Peripheral Nerve Conduction Velocity (PNCV), Amplitude (PNCA) and Latency (PNCL) were obtained with electromyography, and their Z scores were calculated. Results: There were significant differences regarding the age, disease duration, serum levels of alanine aminotransferase, aspartate aminotransferase, urea nitrogen, high-density lipoprotein, and 2h-C peptide between these two groups (p < 0.05). The NDPN group had higher glucagon levels at 30, 60 and 120 min and AUCglu (p < 0.05). The Z-scores of PNCV and PNCA showed an increasing trend (p < 0.05), while the Z-score of PNCL showed a decreasing trend (p < 0.05). The glucagon levels were positively correlated with PNCV and PNCA, but negatively correlated with PNCL, with Gluca30min having the strongest correlation (p < 0.05). Gluca30min was independently related to PNCV, PNCL, PNCA and DPN, respectively (p < 0.05). The function of pancreatic α islet cells, as reflected by the plasma glucagon level, is closely related to the occurrence of DPN in T2DM patients. Conclusion: Gluca30min may be a potentially valuable independent predictor for the occurrence of DPN.

Patients and Methods: This was a single-center cross-sectional analysis of 324 T2DM patients. All patients were separated into a group with normal NCV (NCVN) and a group with abnormal NCV (NCVA). Patients were also classified into groups with low and high UHR values based on the median UHR in this study cohort. Neurophysiological data including motor and sensory conduction velocity (MCV and SCV, respectively) were measured for all patients. Results: Relative to patients with low UHR values, those in the high UHR group presented with greater NCVA prevalence (P = 0.002). UHR remained negatively correlated with bilateral superficial peroneal nerve SCV, bilateral common peroneal nerve MCV, bilateral ulnar nerve SCV, and bilateral right median nerve MCV even after adjustment for confounding factors. UHR was identified as an NCVA-related risk factor, with a 1.370-fold increase in NCVA prevalence for every unit rise in UHR (P < 0.001). Conclusion: These results identify UHR as a risk factor associated with NCVA that was independently negatively associated with NCV among T2DM patients. Keywords: type 2 diabetes mellitus, diabetic peripheral neuropathy, peripheral nerve conduction velocity, uric acid to high-density lipoprotein cholesterol ratio Type 2 diabetes mellitus (T2DM) is an increasingly common metabolic disease such that it represents one of the most important global public health challenges in the 21st century. 1 The International Diabetes Federation (IDF) predicts that T2DM prevalence will rise to 10.9% by the year 2045, with the highest number of T2DM patients residing in China with approximately 116.4 affected individuals 2–4 Diabetic peripheral neuropathy (DPN) is a complex yet common complication of T2DM that causes chronic issues in affected patients such that it represents a pressing health issue throughout the world. DPN patients can experience pain, numbness, nociceptive hypersensitivity, and other forms of discomfort, 5,6 and the late diagnosis of this condition can lead to higher rates of diabetic foot ulcers, gangrene, and amputation procedures that can have serious negative effects on patient quality of life while also imposing a significant economic burden. 1 As DPN is generally characterized by a nonspecific insidious onset, diagnosing it at an early stage is challenging such that missed diagnoses can occur. 7 Early screening for DPN can entail analyses of nerve conduction velocity (NCV), which primarily reflects myelin sheath function, 8 and entails analyses of both motor NCV (MCV) and sensory NCV (SCV). 9–11 Serum uric acid (SUA) is the primary end product of the purine metabolism pathway. Elevated levels of SUA are associated with vascular dysfunction and irreversible damage with the potential to result in tissue ischemia and the impairment of peripheral nerve function. 12 SUA has been demonstrated to be related to the pathogenesis of DPN in several studies, albeit with some inconsistent findings. 13–15 One meta-analysis found that there is a significant association between hyperuricemia and a greater risk of DPN incidence and that DPN patients exhibit significant increases in their SUA concentrations. 14 Consistently, Zhang et al 16 observed similar results in a Mendelian randomization study. A separate cross-sectional analysis, however, revealed that low SUA levels can also serve as a risk factor for DPN development among T2DM patients, potentially influencing tibial nerve motor fiber function in a manner independent of the effects of HbA1c. 15,17 The plasma lipoprotein high-density lipoprotein cholesterol (HDL-C) exhibits beneficial antioxidant and anti-inflammatory activity.
