Abstract:Introduction Foot ulceration and its complications are one of the major complications attributing to the hospital admission, with overall prevalence of about 4 -10% and even higher in geriatric population (5-10%).With the lifetime risk of 15%,it contributes as a major factor of morbidity and not infrequently leading to severe adverse outcomes, such as amputation and life threatening sepsis.The study was aimed at analyzing the prevalence of diabetic foot infection,its complications and outcomes of different treatment modalities available in a tertiary care center.Material and Methods A prospective observational study of 12 month duration was conducted among 100 patients having a history of diabetes and diagnosed with diabetic foot ulcer undergoing treatment in the surgery department of a tertiary care center in Jammu.The period of study was Feb 2022 to Feb 2023.Prior to the study an informed written consent and approval of the ethics committee was obtained. Results The incidence of diabetic foot ulcers peaks at the 5th decade of life with the mean age in our study being 53.17±10.2 years.A predominance in the male population (m:f ratio=2.57:1) and higher risk in the lower socio-economic strata was also noticed. The most common presenting complaint being ulcer (96%) and discharge (67%).Culture positivity was seen in 80% of the patients with Staph.Aureus being the most common causative organism. The main stray of treatment involved debridement and amputation (31%) followed by Split thickness skin grafting (30%). No adverse events were noted during the course of study. Conclusion Being a menace in developing countries,diabetes mellitus,a disease with life threatening and physically incapacitating consequences which not only increases the patient morbidity but also substantially increases the economic burden. Provision of precise counseling and a multidisciplinary line of action can lessen the complication and amputation rate, and is therefore recommended, along with the incorporation of diabetic foot management guidelines into the routine practice and teaching

Background: Diabetic Foot Ulcer (DFU) is a severe complication of diabetes mellitus and is associated with high rates of morbidity and mortality. The incidence of DFUs and amputation disproportionately impact Black and Hispanic populations. DFU is also associated with a high rate of hospital readmissions and prolonged length of stay (LOS). The goal of this study was to examine 1-year mortality data, LOS and readmission rate. Methods: This study was a retrospective chart review of 1348 patients admitted to three hospitals in our system in the Bronx from 6/1/16 to 5/31/21. Data was extracted from the EPIC® electronic health records, based on the diagnoses ICD 10 codes, E10.621 and E11.622. Inclusion criteria was patients admitted with an active DFU regardless of other admission diagnosis, age 18 years or above. The index admission was defined as the first admission in our medical records within the date range. Patients were excluded if their index admission was not accessible through our EMR. Simple logistic regression and multiple logistic regression were used to analyze the association between variables of interest and mortality at index admission and at 1-year, LOS and re admissions. Results: Of the 650 eligible patients, the mean age was 59 + 13 years, 70% of patients were male, and majority were non-White (88%). Mortality was 1% during the index admission and the risk increased to 6% at 1 year after the index admission. Readmission with an active DFU from the index admission occurred in 15% of patients within 30-days, 32% of patients within 90 days, and 55% of patients within 1 year. The mean length of stay was 9 +7.5 days. Discussion: Our study in an urban region has identified high rates of mortality and costs associated with DFU, which has been reported in many settings. This patient population is high risk for poor outcomes and targeted programs are needed to reduce mortality and provide care to reduce readmissions. Future studies that explore the challenges of DFU health care and that test interventions are needed to improve DFU related health outcomes. Presentation: 6/2/2024

Abstract:Diabetes is one of the most prevalent epidemic metabolic disorders, responsible for a significant amount of physical, psychological and economic loss in human society. Diabetic foot ulcer (DFU) is one of the extreme pathophysiological consequences of diabetes. Bacterial infection is the most important cause of chronic DFU. Bacterial species or their biofilms show multidrug resistance, which complicates DFU and consequently leads to amputation of the infected part. Since the Indian population comprises diverse ethnic and cultural groups, this could influence the aetiology of diabetic foot infections and bacterial diversity. We reviewed 56 articles published from 2005 to 2022 on the microbiology of DFU and extracted the data on study location, number of patients analysed in the study, pathophysiological complications, age of the patients, sex of the patient, type of bacteria, type of infection (mono or polymicrobial), predominant bacteria (Gram-positive or Gram-negative), predominant isolates and multiple drug resistance (tested or not). We analysed data and described aetiological trends in diabetic foot infections and bacterial diversity. The study revealed that Gram-negative bacteria are predominant as compared to Gram-positive bacteria in individuals with diabetes with DFU in India. Escherichia coli, Pseudomonas aeruginosa, Klebsiella sp. and Proteus sp. were the most predominant Gram-negative bacteria, while Staphylococcus aureus and Enterococcus sp. were the major Gram-positive bacteria in DFU. We discuss bacterial infections in DFU in the context of bacterial diversity, sampling methods, demography and aetiology.

Abstract:Background: Diabetic foot ulcer (DFU) is one of the most serious complications of diabetes and frequently necessitates amputation, which lowers quality of life with the distressing prospect of re-amputation and increased mortality. The available classifications for a standardized approach to diabetic foot ulceration, though crucial in predicting clinical outcomes, have their limitations. Aims and Objectives: This study examined the reliability of the University of Texas (UT) classification system in predicting outcomes of diabetic foot in a tertiary center in Kerala, South India. Materials and Methods: In this prospective observational study, 98 DFU patients at Jubilee Mission Medical College and Research Institute, Thrissur, from December 2019 to June 2022 were categorized according to UT classification, and the treatment with outcomes for 6 months was recorded. Results: Among the total population, 24.4% of diabetic feet achieved healing without amputation. We observed an increasing incidence of impairment of healing (P=0.018) and in the number of amputations in Grades 1, 2, and 3 (P=0.000). At 6 months, 52% of diabetic feet, including those who underwent amputation, healed completely. Conclusion: DFU in a lower grade but higher stage according to the UT classification showed impaired healing and higher amputation rates as this system includes infection and ischemia. This classification allows reliable categorization and enables re-allocation of grade and stage of diabetic foot during treatment and follow-up, thereby facilitating effective management and predicting outcome.

Abstract:Background: Diabetic foot is one of major complication in diabetes patients with unfavorable outcome. Survival study in outpatients is limited and factors related are inconsistent. Survival and its modifiable risk factors should be identified early since the foot at risk status to reduce amputation/mortality in type 2 diabetes mellitus (T2DM). Objective: The aims of this study were to investigate survival probability for amputation or mortality, compare different ulcer risk classification, and figure out the relation of status of ulcer risk, age, gender, diabetes duration, body mass index, fasting plasma glucose, HbA1C, and LDL with amputation or mortality. Methods: This is a retrospective cohort study of 487 T2DM subjects who visited internal medicine outpatient clinic in Fatmawati General Hospital since January-December 2016. Status of ulcer risk and risk factors were extracted from medical record and lower-extremity amputation or mortality was observed in 3 years from baseline. Result: Three years overall survival is 85.7% (SE 0.17). Patients with high risk for foot ulcer have survival probability of 80.2% (SE 0.027), which is lower compared to non-high risk for foot ulcer with survival probability of 91.8% (SE 0.019). Patients with high risk for foot ulcer (aHR 2.386 [95% CI 1.356-4.20]; P = .003), aged ≥60 years old (aHR 2.051 [95% CI 1.173-3.585]; P = .012), and HbA1C ≥7% (aHR 2.022 [95% CI 1.067-3.830]; P = .031) were independently associated with amputation or mortality. Conclusion: T2DM patients with high risk for foot ulcer have lower survival probability and higher risk for amputation or mortality in 3 years compared to patients with non-high risk for foot ulcer. Status of ulcer risk, age ≥60 years, and HbA1C ≥7% were associated with amputation or mortality in 3 years observation.

Abstract:To perform a retrospective survey of the clinical features and clinical courses of diabetic foot patients with amputations hospitalized in the Second Affiliated Hospital of Zhejiang University from 2007 to 2011.According to the database from the medical records department in our hospital 36 cases of diabetic patients were selected out of 805 cases of amputees. The clinical information was recorded in detail. With the contact information provided in the medical record, telephone interviews were conducted for each patient and their family to complete the record for the current disease progression and the patient's treatment process. Among all of the patients with hospitalized amputations, 36/805(4.47%) were diabetic foot patients with amputations, accounting for 36/273(13.19%) of non-traumatic amputations. The average age of the diabetic patients with first-time amputations was 62 years, the average BMI was greater than 24.5kg/m(2), and the waist-to-hip ratio was greater than 0.92. Among the 22 patients who completed the follow-up interviews, 17 had only small amputations. Diabetic amputation has become a common clinical problem. The patients were mainly elderly, overweight or obese. Amputations at the toe level still accounted for a large proportion of the patients. Overall, the follow-up interviews indicated that the patients lacked good understanding of and concern for their own disease, with unclear control of blood sugar and no standardized control method. Helping the patients to understand the disease and paying attention to health education may play a role in the prevention and treatment of diabetes and diabetic foot disease.

Abstract:Objectives Diabetic foot ulcers (DFU) are a leading cause of morbidity and mortality which disproportionately impacts underserved populations. This study seeks to provide data regarding the rates and outcomes of amputation in patients admitted with DFU in our health system which cares for an ethnically diverse and underserved population. Methods This retrospective study examined the electronic medical records of adult patients hospitalized with DFU at three hospitals in our health system between 06/01/2016 and 05/31/2021. Results Among 650 patients admitted for DFU, 88% self-identified as non-White race. Male sex (OR 0.62), low BMI (OR 0.98), and history of smoking (OR 1.45) were significantly associated with amputation during the study period. Higher erythrocyte sedimentation rate (OR 1.01), C-reactive protein (OR 1.05), white blood cells (OR 1.11), and low albumin (OR 0.41) were found to be significantly associated with amputation versus no amputation during admission. The amputation risk during the index admission for DFU was 44%. Conclusion Our study identified a high DFU-related amputation risk (44%) among adult patients who were mostly Black and/or Hispanic. The significant risks factors associated with DFU amputation included male sex, low BMI, smoking as well as high levels inflammation or low levels of albumin during admission. Many of these patients required multi-disciplinary care and intravenous antibiotic therapy, requiring a longer length of stay and a high rate of readmission.

Abstract:Abstract: Diabetic foot ulcers (DFUs) remain important sequelae of diabetes (DM) which cause debilitating effects on the sufferer. The evolution of some aspects of epidemiology and the current clinical impact of DFUs was examined. Methods: A single-center prospective observational study. Study subjects were consecutively recruited. Results: Total medical admissions during the study period were 2288, 350 were DM related, out of these 112 were admitted for DFU. 32% of total DM admissions were for DFU. The mean age of the study subjects is 58 ± 11.0 range is from 35 years to 87 years. Males were slightly predominant (51.8%). Most of them were actively employed (92%), and the majority were in the 55 to 64 years age category. Most of them had not been diabetic for longer than 8 years (61%). The mean duration of DM is 8.32±7.27 years. The mean duration of ulcer at presentation was 72.0±138.13 days. The majority of the patients (80.3%) presented with severe (grades 3 to 5) ulcers, Wagner grade four was the most predominant. Regarding clinical outcome, 24 (24.7%) had an amputation, 3 of which were minor. The factor that was associated with amputation was concomitant heart failure - OR 6.00 CI 0.589-61.07, 0.498-4.856. Death occurred in 16 (18.4%). The factors associated with mortality were severe anemia OR 2.00 CI 0.65 - 6.113, severe renal impairment requiring dialysis OR 3.93 CI 0.232-66.5, concomitant stroke OR 8.42 CI 0.71-99.6, and peripheral arterial disease- OR 18.33 CI 2.27 -147 p-value- 0.006. Conclusion: The hallmark of DFU in this report is late presentation, it accounted for a significant proportion of the total medical admissions, although the case fatality of DFU reduced from previous reports from the center, mortality, and amputation rates are still unacceptably high. Concomittant heart failure was a factor of amputation. Mortality was associated with severe anemia, renal impairment and peripheral arterial disease.

Abstract:Background: Diabetic foot ulceration (DFU) is one of the macrovascular complications of diabetes, its prevalence was estimated to be about 8-11% of individuals with uncontrolled diabetes experience for long time. The pathogenic cascade of DFU is a combination of vascular complications of peripheral arteries, Peripheral neuropathy and the raised blood glucose makes it as a non-healing ulcer followed by secondary infection which will progress to end with leg Amputation. The recurrent nature of Diabetic foot ulcer can impact on the health status as well as the economical stability of the affected patients and are associated with the major health care consumption with high cost. Methodology: It was a Mono centered prospective observational comparative study conducted in the department of general surgery for 6 months duration. Results: Total samples of 100 diabetic foot ulcer patients were enrolled into the study, 74 (74%) were males and 26 (26%) were females. The age group between51-60 and 61-70 years was observed as the highest percentage 27% (27). Highest percentage 42% (42) of patients were none (without any comorbidities) followed by 32(32%) patients are with combined (more than one comorbidity like HTN, CKD, CAD...etc.) with utmost percentage of 43% was peripheral neuropathy. 23 patients were diagnosed with gangrene and23 patients underwent major amputation, The economic burden of diabetic foot ulcer for conservative treatment was slightly more when compared to amputated patients. Conclusion: Study concludes that, economic burden on diabetic foot ulcer in conservative patients was slightly more while compared to amputated patients by descriptive statistics even the statistical analysis shown less significant difference. Because the necessity rate for extra hospitalization and medical services was more for conservative treatment (Debridement, Fasciotomy, Incision and Drainage) when compared to amputation patients with diabetic foot ulcer. whereas in the pharmaco-economics there was not much significant difference among both amputated and conservative patients.

Abstract:Background: Foot infections are one of the most commonly observed complications in diabetic patients and are associated with high morbidity and risk of lower extremity amputation. Foot infections account for about 20% of all hospitalizations in people with diabetes and at least 50% of all non-traumatic lower-limb amputations performed annually. Objective: To identify the risk factors in patients with diabetic foot infections attending tertiary care centre. Method: It is a longitudinal prospective study in which patients attending the tertiary care centre with diabetic foot infections meeting the inclusion criteria were enrolled after obtaining the informed consent form. Results and discussion: The results of this study revealed that the overall prevalence of DFI was seen more in males (61.42%) when compared to females (38.57%). The results also showed that the risk of DFI was more with trauma(44.28%) followed by the long duration of DM (28.57%) > wound (15.71%) > uncontrolled DM & prior foot ulcer (5.7%) which indicates a lack of awareness, longer duration of DM, poor glycemic control were the main risk factors causing diabetic foot problems. Conclusion: The results suggest that lack of awareness, poor glycemic control, and long duration of diabetes were the main risk factors causing DFI. Therefore, efforts to prevent infections should be targeted at people with traumatic foot wounds especially those that are chronic and recurrent. Foot care education would be the foremost important way of dealing with this serious problem. Keywords: Morbidity, lower-extremity amputation, trauma, glycemic control.
Mortality rates in people presenting with a new diabetes-related foot ulcer: a cohort study with implications for management

Abstract:Aims/hypothesis: People with diabetes-related foot ulcers (DFUs) have high mortality rates. This analysis assesses the impact of selected risk factors on short-term mortality using a population registered in the National Diabetes Foot Care Audit (NDFA). Methods: Mortality rates at 12, 26 and 52 weeks was assessed in people with a new DFU registered by a specialist diabetes footcare service in the NDFA in England and Wales between April 2017 and March 2022. Poisson regression models were created to explore risk factors for mortality. Results: In 71,000 people registered with a new DFU, mortality rates at 12, 26 and 52 weeks was 4.2%, 8.2% and 14.4%, respectively. At 26 weeks, higher mortality rates was associated with older age (rate ratio 2.15; 95% CI 2.03, 2.28, for age ≥80 years vs age 65-79 years), certain ulcer characteristics (area ≥1 cm2 [1.50; 95% CI 1.42, 1.59], deep ulcers [1.26; 95% CI 1.18, 1.35] or hindfoot location [1.53; 95% CI 1.44, 1.62]) and recorded evidence of ischaemia in the lower limb (1.78; 95% CI 1.69, 1.88) and various comorbidities (heart failure [2.13; 95% CI 2.00, 2.26], myocardial infarction [1.45; 95% CI 1.29, 1.63], stroke [1.37; 95% CI 1.22, 1.53], renal replacement therapy [2.34; 95% CI 2.09, 2.61] and chronic kidney disease stage 3 or greater [1.20; 95% CI 1.12, 1.29]). The 26-week mortality rate exceeded 25% for 7.3% of all individuals, rising to 11.5% of those aged 65 years and older, and 22.1% of those aged 80 years and over. Conclusions/interpretation: Short-term mortality rates in people with a DFU is high. Teams managing people with DFUs should consider modifying the burdensome interventions and care required to heal such ulcers so maximising the quality of residual life, rather than focusing exclusively on healing.
Magnitude of limb loss attributable to diabetes mellitus in a tertiary institution in Nigeria

Abstract:Background: Diabetes mellitus (DM) and its complications, continues to pose enormous challenge to health and financial stability. Diabetes has remained a source of national and global economic burden. It has been observed lately that incidence of diabetic foot ulcer (DFU); one of the complications of DM, is on the increase and it is contributing hugely to financial loss, morbidity and mortality among diabetic patients. This is taking a great toll on affected individuals in terms of cost of treatment, deformities sustained, number of working/productive days lost while on admission and its attendant economic implications, and ultimately mortalities recorded. The aim was to determine the magnitude of limb loss or lower extremity amputation attributable to DM over a period of 12 months in tertiary institution and to assess the clinical profile of the patients. Method: This is a retrospective study of the records of all patients that were consecutively booked for lower limb amputation and operated upon in the last 12 months at UNIOSUN Teaching Hospital, Osogbo, South-Western Nigeria. Out of these total, cases of DFU were extracted to form another study group and then studied in detail. Results: Atotal of 38 patients had lower limb amputation from July 2021 to May 2022. Twenty three of the total number of amputations had DFU with a huge proportion of 60.5%, followed by road traffic accident with 28.9% (n=11) Of the 23 diabetic cases , females were 15 with M:F ratio of 1:1.9. Mean age was 63±11.18 years. Mean duration of DM was 8.06±5.64 years. 78.3% had no foot care education and Doppler USS confirmed atherosclerosis in 100% of the patients. The commonest bacteria isolated was Proteus 34.8%. Length of hospital stay ranged between 6 weeks to 16 weeks. Outcome of admission was largely successful with 95.7% discharged and 4.3% mortality. Conclusion: DFU contributes markedly to morbidity and mortality. Long duration of DM, presence of PAD and DPN as well as advancement in age and wound infection with proteus bacterial are observed to be risk factors associated with gangrenous DFU. However, larger studies are needed to establish these factors as predictors of amputation in patient with DFU. Outcome was majorly good as majority of the patients were discharged.

Among all of the patients with hospitalized amputations, 36/805(4.47%) were diabetic foot patients with amputations, accounting for 36/273(13.19%) of non-traumatic amputations. The average age of the diabetic patients with first-time amputations was 62 years, the average BMI was greater than 24.5kg/m2, and the waist-to-hip ratio was greater than 0.92. Among the 22 patients who completed the follow-up interviews, 17 had only small amputations. Diabetic amputation has become a common clinical problem. The patients were mainly elderly, overweight or obese. Amputations at the toe level still accounted for a large proportion of the patients. Overall, the follow-up interviews indicated that the patients lacked good understanding of and concern for their own disease, with unclear control of blood sugar and no standardized control method. Helping the patients to understand the disease and paying attention to health education may play a role in the prevention and treatment of diabetes and diabetic foot disease.
Survival of Patients Following First Diagnosis of Diabetic Foot Complications: A Nationwide 15-Year Longitudinal Analysis

Abstract:Background and Aims The long-term survival in people with type 2 diabetes following first diagnosis of diabetic foot complications (FDDFC) is unclear. The object is to evaluate the mortality rate in subjects with type 2 diabetes following FDDFC and the impacts of the major cardiovascular comorbidities. Methods Nationwide data were analyzed for subjects with T2D and DFC between 2003 and 2017 according to ICD-9 coding. DFC was defined with the codes of ulcers, infections, or severe peripheral artery disease that required intervention (PAD) to mimic the real world diagnosis. Criteria of FDDFC were preceded by a period without any DFC for at least 5 years. Major cardiovascular comorbidities: established PAD and cardiovascular diseases (CVD: including coronary heart disease (CHD), stroke, or heart failure) before the index date as well as lower-extremity amputations (LEA) at the index episode were analyzed. Results Among 300,115 subjects with DFC, a total of 103,396 patients had FDDFC. The mean 5-year survival rate of these subjects was 81.05%. Using subjects without associated major cardiovascular comorbidity as baseline, the adjusted hazard ratios (aHR) were1.43 (95% confidence interval 1.38–1.49) in group PAD-/CVD+, followed by 1.70 (1.59–1.80) in PAD+/CVD- and 1.98 (1.89–2.08) in PAD+/CVD+. The aHR was further increased in patients with PAD who additionally had heart failure (3.77, 3.50–4.05), stroke (2.06, 1.95–2.18), or CHD (1.89, 1.79–2.00). Subjects with PAD rather than other CVD were associated with LEA at FDDFC. Patients with major LEA (above the ankle) at FDDFC episode had lower 5-year survival rate (65.01%) followed by those with minor LEA (72.24%) and without LEA (81.61%). Conclusions Cardiovascular comorbidity as well as LEA status at the event of FDDFCs were both associated with patient survival outcomes. Earlier identification of this large population could lead to higher survival rates.

Abstract:Introduction: Globally, about 425 million people are living with diabetes mellitus. Diabetic Foot Ulcers (DFU) are one of the severe complications of poorly controlled diabetes and over the time, about 50% of DFUs become infected which may require hospitalization. Aim: To find out the DFU infection severity pattern and its predictors among T2DM patients. Materials and Methods: This cross-sectional study was conducted at Integrated Diabetes and Gestational Diabetes Clinic (IDGDC), IQ City Medical College and Multispecialty Hospital, Durgapur, West Bengal, India, among Type 2 Diabetes Mellitus (T2DM) patients from June 2018 to November 2018. Total 1534 T2DM patients attended IDGDC during data collection period of 4 months and 132 of them had diabetic foot ulcer. After taking written informed consent, detailed data were collected from 132 of study participants using predesigned, semi structured and pre tested schedule developed with the help of Infectious Disease Society of America (IDSA) and International Working Group on the Diabetic Foot (IWGDF/ IDSA) classification system. Socio-demographic characteristic like age, sex, education, residence were recorded along with clinical data like glycated haemoglobin (HbA1c), duration of diabetes, treatment modalities Anthropometric measurements were taken as per World Health Organization (WHO) guidelines. T2DM was defined and classified as per American Diabetes Association (ADA) Guidelines. DFU infection severity was classified into uninfected, mild infection, moderate infection and severe infection as per IWGDF/IDSA guidelines. Chi-square test was used to show association between categorical variable. One-way Analysis of Variance (ANOVA) with Tukey's post-hoc test was used to show association between mean HbA1c level and DFU infection severity. The p-value ≤0.5 was considered significant. Results: Proportion of DFU was found to be 8.6%. As per the IWGDF/IDSA classification of DFU infection severity was found to be moderate in 59 (44.7%) of the study participants and mild in 32 (24.2%) of the study participants. 22 (16.7%) of study subjects had severe infection and required hospitalization for optimal care. Only 14.4% of study subjects did not have DFU infection. Increasing age (p-value=0.023), rural residence (p-value=0.015), poor education (p-value=0.001), obesity (p-value=0.001), central obesity (p-value=0.001), longer duration of diabetes (p-value=0.028), and poor glycemic control (p-value=0.001) was found to be significant risk factors for severe infection in DFU. Conclusion: Routine clinical assessment of DFU infection may help in making clinical decision of treatment modalities and help in saving lower limb as well as life of people with T2DM.

Abstract:Background and aims: Diabetic Foot Ulcers (DFUs) are a significant health concern, particularly in Low- and Middle-Income Countries (LMICs). This review explores key strategies for managing DFUs in LMICs, including integrating podiatry, endocrinology, and wound care services, educating patients, promoting self-care, and preventive measures to reduce amputation rates. Methods: A comprehensive literature review was conducted, focusing on studies conducted in Low and Middle Income Countries to facilitate a qualitative analysis. The review examined the aetiology and risk factors to developing DFUs, clinical presentation, multidisciplinary management and evidence based interventions, challenges to the provision of care and future directions, all pertaining to DFUs in low and middle income countries. Results: The aetiology and risk factors contributing to the development of DFUs are complex and multifaceted. Factors such as limited access to health care, inadequate diabetes management, and socioeconomic disparities significantly influence the incidence of DFUs. Clinical presentation varies, with patients often presenting at advanced stages of the disease due to delayed or missed diagnoses. Multidisciplinary management, incorporating podiatry, endocrinology, and wound care services, has exhibited substantial promise in enhancing patient outcomes. Evidence-based interventions, including offloading techniques, wound debridement, and the use of advanced wound dressings, have proven effective in promoting ulcer healing. Conclusion: The burden of DFUs in LMICs requires comprehensive strategies. Integrating podiatry, endocrinology, and wound care services, along with patient education and self-care practices, is essential for reducing amputations and improving patients' quality of life. Regular follow-up and early detection are vital for effective DFU management, emphasizing the need for ongoing research and investment in LMIC health care infrastructure. Embracing these multidisciplinary, patient-centered approaches can effectively address the challenge of DFUs in LMICs, leading to better patient outcomes and improved quality of life.

Abstract:The titer and avidity of respiratory syncytial virus-specific antibodies were measured in 196 serum specimens from 93 children with an acute, laboratory-confirmed respiratory syncytial virus infection. An enzyme immunoassay method based on the ability of urea to dissociate the bound antibodies with low avidity from the antigen was used. Three patterns of immune responses were observed. Children less than 6 months of age usually had low titers of antibodies with high avidity in their acute-phase serum samples. These antibodies were concluded to be of maternal origin, since their reaction pattern was similar to that of healthy adults. During the next few weeks, a slight increase in titers with a concurrent decrease in antibody avidity was observed. All children 6 to 24 months of age had low-avidity antibodies in their acute-phase serum samples, which matured to high avidity during the follow-up. On the contrary, about half of the children greater than 24 months of age had high-avidity antibodies already in the acute-phase serum samples. We conclude that the former children were experiencing primary infections with respiratory syncytial virus and the latter were experiencing reinfections. All adults with remote immunity had antibodies with high avidity.

Abstract:Diabetic foot ulcer (DFU) is a debilitating and severe manifestation of uncontrolled and prolonged diabetes that presents as ulceration, usually located on the plantar aspect of the foot. Approximately 15% of individuals with diabetes will eventually develop DFU, and 14%-24% of them will require amputation of the ulcerated foot due to bone infection or other ulcer-related complications. The pathologic mechanisms underlying DFU are comprise a triad: Neuropathy, vascular insufficiency, and secondary infection due to trauma of the foot. Standard local and invasive care along with novel approaches like stem cell therapy pave the way to reduce morbidity, decrease amputations, and prevent mortality from DFU. In this manuscript, we review the current literature with focus on the pathophysiology, preventive options, and definitive management of DFU.

Impact of Poor Glycemic Control and Vascular Complications on Diabetic Foot Ulcer Recurrence

Abstract:Background: Diabetes mellitus is a growing global health issue that leads to serious complications like diabetic foot ulcers (DFUs), which can worsen health and increase death rates in diabetic patients. In Sorong, Southwest Papua, Indonesia, the recurrence of DFUs is a major public health problem, affecting patients' well-being and adding significant healthcare costs. This study aimed to identify the factors that contribute to DFU recurrence in this region to help improve prevention and treatment strategies. Methods: A cross-sectional study was conducted in Sorong, Indonesia, including 53 diabetic patients with recurrent DFUs. Data were collected through structured interviews, physical examinations, and laboratory tests. Risk factors such as metabolic, vascular, behavioral, and demographic elements were analyzed. Statistical analysis, including univariate, bivariate, and multivariate techniques, was performed to identify independent predictors of DFU recurrence. Results: The study found that poor glycemic control, with a mean HbA1c of 8.3±1.4, and vascular complications, such as peripheral artery disease and neuropathy, were the most significant predictors of DFU recurrence. Behavioral factors like smoking and physical inactivity, along with longer diabetes duration and previous ulcer history, also showed a strong correlation with recurrence. Logistic regression identified HbA1c levels, peripheral artery disease, and smoking habits as independent predictors. Conclusion: This study determined the need for comprehensive care that focuses on controlling blood sugar levels, managing vascular issues, and promoting healthy behaviors to prevent diabetic foot ulcer recurrence in Sorong, Southwest Papua. Future efforts should target these risk factors to lower recurrence rates and improve patient health. More research is needed to find effective ways to prevent DFU recurrence in this population.
