Novel low-dose protocols using cone beam computed tomography in dental medicine: a review focusing on indications, limitations, and future possibilities

Abstract:Objectives: A narrative review on the potential use of low-dose protocols for cone beam computed tomography (CBCT) was conducted to identify indications and their relevance for various dental disciplines. Materials and methods: Google Scholar was searched using the words "low-dose CBCT". Reviews, consensus papers, clinical studies, and experimental studies were eligible for the initial screening process, but for data extraction only original articles were selected. Similar search procedures were then performed with the additional search words "pedo," "ortho," "endo," "implant," "perio," and "oral surgery." Furthermore, references of included articles were examined to identify further relevant articles. Results: After screening, 27 publications remained for the data extraction process. Low-dose protocols have been reported for specialties such as pediatric dentistry (evaluating orofacial clefts, periapical lesions, impacted teeth, and autotransplantation), orthodontics (cephalometric analysis and interim assessment of treatment results), endodontics (detecting root fractures, resorptions and periapical bone loss), implant dentistry (planning implant insertion, evaluating peri-implant fenestration and dehiscence), periodontology (assessing periodontal structures), and oral and maxillofacial surgery (assessing mandibular third molars and TMJs). Nevertheless, most of the literature available is related to non-clinical studies. Furthermore, there is a lack of position statements or guidelines from authoritative bodies regarding the use of low-dose protocols in dental medicine. Conclusions: Low-dose protocols for CBCT imaging seem to have potential in various disciplines in dental medicine ranging from pediatric dentistry to oral and maxillofacial surgery. Dose reduction is usually achieved by mAs reduction, use of partial rotations, reduced number of projections, and larger voxel sizes, but seldom by kV reduction. Clinical relevance: Albeit low-dose protocols have potential to result in a reduction of dose exposure for 3D imaging due to dental indications, there is a need to more clearly specify indications and limitations to avoid indiscriminate use of standard and high-dose CBCT scans in the future on the lines of ALARA/ALADA principles.
Cone beam computed tomography in implant dentistry: recommendations for clinical use

Abstract:BackgroundIn implant dentistry, three-dimensional (3D) imaging can be realised by dental cone beam computed tomography (CBCT), offering volumetric data on jaw bones and teeth with relatively low radiation doses and costs. The latter may explain why the market has been steadily growing since the first dental CBCT system appeared two decades ago. More than 85 different CBCT devices are currently available and this exponential growth has created a gap between scientific evidence and existing CBCT machines. Indeed, research for one CBCT machine cannot be automatically applied to other systems.MethodsSupported by a narrative review, recommendations for justified and optimized CBCT imaging in oral implant dentistry are provided.ResultsThe huge range in dose and diagnostic image quality requires further optimization and justification prior to clinical use. Yet, indications in implant dentistry may go beyond diagnostics. In fact, the inherent 3D datasets may further allow surgical planning and transfer to surgery via 3D printing or navigation. Nonetheless, effective radiation doses of distinct dental CBCT machines and protocols may largely vary with equivalent doses ranging between 2 to 200 panoramic radiographs, even for similar indications. Likewise, such variation is also noticed for diagnostic image quality, which reveals a massive variability amongst CBCT technologies and exposure protocols. For anatomical model making, the so-called segmentation accuracy may reach up to 200 μm, but considering wide variations in machine performance, larger inaccuracies may apply. This also holds true for linear measures, with accuracies of 200 μm being feasible, while sometimes fivefold inaccuracy levels may be reached. Diagnostic image quality may also be dramatically hampered by patient factors, such as motion and metal artefacts. Apart from radiodiagnostic possibilities, CBCT may offer a huge therapeutic potential, related to surgical guides and further prosthetic rehabilitation. Those additional opportunities may surely clarify part of the success of using CBCT for presurgical implant planning and its transfer to surgery and prosthetic solutions.ConclusionsHence, dental CBCT could be justified for presurgical diagnosis, preoperative planning and peroperative transfer for oral implant rehabilitation, whilst striving for optimisation of CBCT based machine-dependent, patient-specific and indication-oriented variables.

CBCT vs other imaging modalities to assess peri-implant bone and diagnose complications: a systematic review.

Abstract:AIM The objective of this systematic review was to evaluate the diagnostic value of CBCT compared with 2D imaging and clinical gold standard techniques in peri-implant bone defect detection and measurement. MATERIALS AND METHODS Literature search was performed using MEDLINE, Embase and Web of Science databases up to July 2017. Clinical, ex vivo, in vitro and animal studies that assessed and measured peri-implant bone defects using different imaging modalities were included in this review. Two reviewers performed data extraction and qualitative analysis. The methodological quality of each study was reviewed using the QUADAS-2 tool. RESULTS The initial search revealed 2849 unique papers. Full-text analysis was performed on 60 articles. For the present review, nine studies were considered eligible to be included for qualitative analysis. CBCT performed similar to intraoral radiography in mesiodistal defect detection and measurements. Additional buccolingual visualisation and volumetric and morphological assessment of peri-implant bone defects are major advantages of 3D visualisation with CBCT. Nevertheless, one must be aware of metal artefacts masking osseointegration, shallow bony defects and other peri-implant radiolucencies, thus impeding early diagnosis of intrabony lesions. CONCLUSIONS The present review did not provide evidence to support the use of CBCT as standard postoperative procedure to evaluate peri-implant bone. Up to date, we are clinically forced to remain with intraoral radiography, notwithstanding the inherent limitations related to restricted field of view and two-dimensional overlap. A 3D imaging approach for postoperative implant evaluation is crucial, making further development of an optimised and artefact-free CBCT protocol indispensable.

Abstract:Optimization of device‐specific low‐dose CBCT protocols for orofacial cleft imaging. Objective The aim of this study was to present optimized device‐specific low‐dose cone‐beam computed tomography (CBCT) protocols with sufficient image quality for pre‐surgical diagnostics and three‐dimensional (3D) modelling of cleft defects. Methods Six paediatric skulls were acquired, and an artificial bony cleft was created. A high‐resolution CBCT scan acted as a reference standard (Accuitomo 170, Morita, Kyoto, Japan) for comparing eight low‐dose protocols of Newtom VGi‐evo (QR Verona, Cefla, Verona, Italy), which included Eco and Regular protocols with different field of views (FOVs). Delineation of lamina dura, cementoenamel junction (CEJ), trabecular bone and bony bridge were assessed. A 3D model of the defect was also evaluated. Result The dose area product of low‐dose protocols ranged from 31 to 254 mGy*cm2. Despite the dose difference of up to eight times between applied protocols, trabecular bone and CEJ exhibited appropriate image quality in all scans. However, Regular small FOV protocols (5 × 5 and 8 × 5 cm2), for both lamina dura and bony bridge, demonstrated a significant improvement in image quality compared to Eco FOV counterparts. Based on 3D defect analysis, no significant difference existed between low‐dose protocols and the reference standard. Conclusion The findings highlight the possibility of achieving a considerable reduction (up to eight times) in the radiation dose using low‐dose CBCT protocols while maintaining sufficient image quality for assessing anatomical structures and 3D modelling in cleft cases.

[Mechanical responses of the sphincter of oddi and duodenum preparations to certain autonomic drugs and transmural stimulation].

Abstract:Exogenously applied acetylcholine causes contractions, depending on the concentration. The contraction was competitively antagonized by atropine and potentiated by physostigmine. On the other hand, exogenously applied phenylephrine, isoproterenol and norepinephrine resulted in relaxation in both preparations. These relaxations were competitively antagonized by adrenergic alpha and beta blocking agents and their combination, respectively. In both preparations, nicotine caused a transient contraction followed by a relaxation. Pretreatment with physostigmine potentiated the contraction in all cases. In 9 out of 22 of the sphincter preparations, and all those of the duodenum, atropine inhibited the contraction caused by nicotine and converted it to a relaxation. In the remaining 13 cases of the atropine-treated sphincter preparations, however, the nicotine-induced response became triphasic: a relaxation followed by a transient slight contraction and a relaxation. The contraction was significantly inhibited by phentolamine or guanethidine. The relaxation caused by nicotine in the presence of atropine was unaffected by propranolol alone, or its combina-tion with phentolamine, bretylium and guanethidine. In both sphincter and duodenum preparations, transmural stimulation caused a transient contraction followed by a relaxation and inhibition of automaticity. Threshold intensity of stimulation was approximately 10V in both preparations. The maximum contractile tension induced by transmural stimulation increased with stimulation intensity until a plateau was attained at 40V. The increase of maximum tension was also seen with stimulation frequency and pulse duration. The time to peak, duration and tension of contraction and the subsequent relaxation of transmural response were compared between the sphincter and duodenum preparations. No difference was observed in any parameter of the responses. Mechanical responses to transmural stimulation and certain autonomic drugs were compared on the isolated sphincter of Oddi and the duodenum from rabbit. Nicotine response in the presence of atropine was the only qualitative difference observed.
Radiation dose in non-dental cone beam CT applications: a systematic review

Abstract:BackgroundRadiation-induced health risks are broadly questioned in the literature. As cone beam computed tomography (CBCT) is increasingly used in non-dental examinations, its effective dose needs to be known. This study aimed to review the published evidence on effective dose of non-dental CBCT for diagnostic use by focusing on dosimetry system used to estimate dose.Materials and methodsA systematic review of the literature was performed on 12 November 2017. All the literature up to this date was included. The PubMed and web of science databases were searched. Studies were screened for inclusion based on defined inclusion and exclusion criteria according to the preferred reporting items for systematic reviews.ResultsFifteen studies met the inclusion criteria and were included in our review. Thirteen and two of them examined one and two anatomical areas, respectively. The anatomical areas were: ear (6), paranasal sinuses (4), ankle (3), wrist (2), knee (1), and cervical spine (1). Effective dose was estimated by different methods: (i) RANDO phantom associated with thermoluminescent dosimeters (6), metal oxide semiconductor field-effect transistor dosimeters (3), and optically stimulated luminescent dosimeters (1). (ii) Scanner outputs, namely computed tomography dose index (1) and dose area product (2). (iii) Monte Carlo simulations (2).ConclusionCBCT of extremities, cervical spine, ears and paranasal sinuses was found to be a low-dose volumetric imaging technique. Effective doses varied significantly because of different exposure settings of CBCT-units and different dosimetry systems used to estimate dose.

Assessment of cone-beam CT technical image quality indicators and radiation dose for optimal STL model used in visual surgical planning

Abstract:Objectives: The aim of this study was to identify cone-beam computed tomography (CBCT) protocols that offer an optimal balance between effective dose (ED) and 3D model for orthognathic virtual surgery planning, using CT as a reference, and to assess whether such protocols can be defined based on technical image quality metrics. Methods: Eleven CBCT (VISO G7, Planmeca Oy, Helsinki, Finland) scan protocols were selected out of 32 candidate protocols, based on ED and technical image quality measurements. Next, an anthropomorphic RANDO SK150 phantom was scanned using these 11 CBCT protocols and 2 CT scanners for bone quantity assessments. The resulting DICOM (Digital Imaging and Communications in Medicine) files were converted into Standard Tessellation Language (STL) models that were used for bone volume and area measurements in the predefined orbital region to assess the validity of each CBCT protocol for virtual surgical planning. Results: The highest CBCT bone volume and area of the STL models were obtained using normal dose protocol (F2) and ultra-low dose protocol (J13), which resulted in 48% and 96% of the mean STL bone volume and 48% and 95% of the bone area measured on CT scanners, respectively. Conclusions: The normal dose CBCT protocol "F2" offered optimal bone area and volume balance for STL. The optimal CBCT protocol can be defined using contrast-to-noise ratio and modulation transfer function values that were similar to those of the reference CT scanners'. CBCT scanners with selected protocols can offer a viable alternative to CT scanners for acquiring STL models for virtual surgical planning at a lower effective dose.
A comprehensive study on the relationship between image quality and imaging dose in low-dose cone beam CT

Abstract:While compressed sensing (CS) based reconstructions have been developed for low-dose CBCT, a clear understanding on the relationship between the image quality and imaging dose at low dose levels is needed. In this paper, we qualitatively investigate this subject in a comprehensive manner with extensive experimental and simulation studies. The basic idea is to plot image quality and imaging dose together as functions of number of projections and mAs per projection over the whole clinically relevant range. A clear understanding on the tradeoff between image quality and dose can be achieved and optimal low-dose CBCT scan protocols can be developed for various imaging tasks in IGRT. Main findings of this work include: 1) Under the CS framework, image quality has little degradation over a large dose range, and the degradation becomes evident when the dose < 100 total mAs. A dose < 40 total mAs leads to a dramatic image degradation. Optimal low-dose CBCT scan protocols likely fall in the dose range of 40-100 total mAs, depending on the specific IGRT applications. 2) Among different scan protocols at a constant low-dose level, the super sparse-view reconstruction with projection number less than 50 is the most challenging case, even with strong regularization. Better image quality can be acquired with other low mAs protocols. 3) The optimal scan protocol is the combination of a medium number of projections and a medium level of mAs/view. This is more evident when the dose is around 72.8 total mAs or below and when the ROI is a low-contrast or high-resolution object. Based on our results, the optimal number of projections is around 90 to 120. 4) The clinically acceptable lowest dose level is task dependent. In our study, 72.8mAs is a safe dose level for visualizing low-contrast objects, while 12.2 total mAs is sufficient for detecting high-contrast objects of diameter greater than 3 mm.

Abstract:We prove an Omitting Types Theorem for certain algebraizable extensions of first order logic without equality studied in [SAI 00] and [SAY 04]. This is done by proving a representation theorem preserving given countable sets of infinite meets for certain reducts of ω- dimensional polyadic algebras, the so-called G polyadic algebras (Theorem 5). Here G is a special subsemigroup of (ω, ω o) that specifies the signature of the algebras in question. We state and prove an independence result connecting our representation theorem to Martin's axiom (Theorem 6). Also we show that the countable atomic G polyadic algebras are completely representable (Corollary 16) contrasting results on cylindric algebras. Several related results are surveyed.
Dental cone beam computed tomography: justification for use in planning oral implant placement

Abstract:Intra-oral and panoramic radiographs are most frequently used in oral health care. Yet, the inherent nature of jaws and teeth renders three-dimensional diagnosis essential, especially in relation to oral surgery. Nowadays, this can be accomplished by dental cone beam computed tomography, which provides high-quality images at low radiation doses and low costs. Nonetheless, the effective dose ranges of cone beam computed tomography machines may easily vary from 10 to 1000 μSv, this being equivalent to two to 200 panoramic radiographs, even for similar presurgical indications. Moreover, the diagnostic image quality varies massively among available machines and parameter settings. Apart from the radiodiagnostic possibilities, dental cone beam computed tomography may offer a vast therapeutic potential, including opportunities for surgical guidance and further prosthetic rehabilitation via computer-aided design/computer-aided manufacturing solutions. These additional options may definitely explain part of the success of cone beam computed tomography for oral implant placement. In conclusion, dental cone beam computed tomography imaging could be justified for oral implant-related diagnosis, planning and transfer to surgical and further prosthetic treatment, but guidelines for justification and cone beam computed tomography optimization remain mandatory.

Abstract:Since its adoption more than two decades ago in the field of dentistry, three-dimensional (3D) cone-beam computed tomography (CBCT) has rapidly gained popularity as an imaging tool for three-dimensional visualization, diagnosis, and treatment simulation. The objective of this two-part study was to assess landmark identification, as well as the diagnostic value of images obtained using an ultra-low-dose reduced projection (sparse) views algorithm Adaptive-Steepest-Descent-Projection-Onto-Convex-Sets (ASD-POCS) applied to existing dental CBCT data. ASD-POCS was developed for CBCT studies, producing multiple CBCT data files with 12.5% (39), 25% (76), and 50% (150) of the original number of projection views (which is in direct proportion with the lowering of radiation dose). Assessment of 2D landmark identification derived from CBCT data at different projection views was conducted. Assessment of diagnostic quality was studied by evaluating radiographs of various projection views on a visual analog scale by different dental specialists. In conclusion, this study found no statistically significant differences in the quality of images at 25% (76) projection views as compared to 100% (300) projection views. The implementation on ASD-POCS algorithm by the CBCT manufacturers in the future may be beneficial for clinicians in reducing the amount of patient radiation exposure when obtaining routine orthodontic diagnostic radiographs. Evaluating Micro-computed Tomography in Dental Implant Osseointegration: A Systematic Review and Meta-analysis

Abstract:Rationale and Objectives The success of dental implants is heavily dependent on the implant's efficient integration with the surrounding bone, a process known as osseointegration. This integration is influenced by a range of individual characteristics of each patient, as well as the accuracy of diagnostic imaging techniques. Micro-computed tomography (micro-CT) is capable of capturing detailed three-dimensional images with high resolution. It may offer superior capabilities for assessing the connections between bones and implants compared to older techniques such as cone-beam computed tomography (CBCT) and intraoral radiography. This systematic review will discuss the uses of micro-CT in dental implant osseointegration, compare it to other radiography methods, and discuss it's effect on patients, especially those with previous history of periodontal disease. Materials and Methods This research performed an extensive search across multiple databases, choosing papers based on specific criteria for inclusion and exclusion. The study focus was on using micro-CT to evaluate bone osseointegration. Meta-analyses were conducted to measure the impact of micro-CT on bone loss and the success rates of implants, while comparing various surgical procedures and depths of implantation. This systematic review is registered in PROSPERO with the registration number CRD42023482747. Results The result of this study comprised 28 papers, incorporating a meta-analysis of 8. It emphasized the exceptional spatial resolution of micro-CT, which enables accurate assessments of bone volume and density, crucial factors for implant success. Micro-CT, when compared to CBCT and intraoral radiography, provides more comprehensive information, but it faces limitations due to its elevated expenses and radiation exposure. The analysis also indicated that micro-CT could be particularly advantageous in tailored therapy planning, particularly for patients with impaired conditions. Systemic factors like diabetes or periodontal disease can cause bone conditions. Conclusion Micro-CT is a highly advanced imaging technique that offers extensive information about dental implants, which is essential for evaluating osseointegration. Although micro-CT has several limitations, it has the capacity to improve clinical outcomes by providing more accurate diagnoses and tailored implant techniques. Subsequent investigations should focus on assessing its cost-effectiveness and establishing protocols to mitigate radiation exposure.

Abstract:Cone Beam Computed Tomography (CBCT) is a valuable imaging technique in oral and maxillofacial surgery (OMS) that can help direct a surgeon’s approach to a variety of conditions. A 3-dimensional analysis of head and neck anatomy allows practitioners to plan appropriately, operate with confidence, and assess results post-operatively. CBCT imaging has clear indications and limitations. CBCT offers the clinician 3-dimensional and multi-planar views for a more accurate diagnosis and treatment without the financial burden and radiation exposure of conventional computed tomography (CT) scans. Furthermore, CBCT overcomes certain limitations of 2-dimensional imaging, such as distortion, magnification, and superimposition. However, CBCT lacks the detailed depiction of soft tissue conditions for evaluation of pathologic conditions, head and neck infections, and temporomandibular joint (TMJ) disc evaluation. This review evaluates the evidence-based research supporting the application of CBCT in the various fields of oral and maxillofacial surgery, including dentoalveolar surgery, dental implants, TMJ, orthognathic surgery, trauma, and pathology, and will assess the value of CBCT in pre-operative assessment, surgical planning, and post-operative analysis when applicable. Additionally, the significant limitations of CBCT and potential areas for future research will be discussed.
Evaluation of Root Resorption after Comprehensive Orthodontic Treatment Using Cone Beam Computed Tomography (CBCT): a Meta-Analysis

Abstract:Background: Orthodontic treatment can result in root resorption (RR). Traditional two-dimensional (2D) data exhibit magnification, deformation and positioning problems. Cone beam computed tomography (CBCT) contains more accurate three-dimensional (3D) information. This study identified and qualified the extent and location of root resorption using cone beam computed tomography (CBCT) after comprehensive orthodontic treatment. Methods: Studies comparing the RR before and after comprehensive orthodontic treatment using CBCT were identified using electronic searches of databases, including Cochrane, PubMed, EMBASE, China National Knowledge Infrastructure (CNKI) and Web of Science, and manual searches in relevant journals and the reference lists of the included studies until Oct 25, 2017. The extraction of data and the risk of bias evaluation were conducted by two investigators independently. The methodological quality of the included studies was assessed using the methodological index for non-randomized studies (MINORS). Studies that reported the length and volume of teeth were used for quantitative analyses. Results: Twelve studies were included in the meta-analysis. The length of all teeth after intervention was significantly shorter than that before treatment (MD = 0.80, 95% CI 0.56, 1.03, P < 0.00001). The sequence of RR from heaviest to lightest was maxillary lateral incisors, maxillary central incisors, mandibular anterior teeth, and maxillary canines. Studies were divided into two subgroups based on the use of tooth extraction. Root shortening after treatment was observed in both groups, and extraction caused more root resorption than was observed in the non-extraction group. Conclusions: There were different degrees of root resorption after orthodontics, but it was clinically acceptable. Root resorption established in CBCT research was less serious and more accurate than that observed in the two-dimensional research. Current evidence suggests that root length and volume were reduced after orthodontic treatment. The order of the amount of RR was maxillary lateral incisors, maxillary central incisors and mandibular anterior teeth. Most of the articles were complicated by different confounding factors. Therefore, more high-quality clinical trials are needed to determine the risk factors of root resorption and optimal protocols for treatment and to draw more reliable conclusions.

Abstract:Background: The cone-beam computed tomography (CBCT) technology has continuously evolved since its appearance in oral medicine in the early 2000s. Objective: To present recent advances in CBCT in oral medicine: i) selection of recent and consensual evidence-based sources, ii) structured summary of the information based on an iterative framework and iii) compliance with ethical, public health and patient-centered concerns. Main Findings: We will focus on technological advances, such as sensors and reconstruction algorithms used to improve the constant quality of the image and dosimetry. CBCT examination is now performed in almost all disciplines of oral medicine: currently, the main clinical disciplines that use CBCT acquisitions are endodontics and oral surgery, with clearly defined indications. Periodontology and ear, nose and throat medicine are more recent fields of application. For a given application and indication, the smallest possible field of view must be used. One of the major challenges in contemporary healthcare is ensuring that technological developments do not take precedence over admitted standards of care. The entire volume should be reviewed in full, with a systematic approach. All findings are noted in the patient’s record and explained to the patient, including incidental findings. This presupposes the person reviewing the images is sufficiently trained to interpret such images, inform the patient and organize the clinical pathway, with referrals to other medical or oral medicine specialties as needed. Conclusion: A close collaboration between dentists, medical physicists, radiologists, radiographers and engineers is critical for all aspects of CBCT technology.

Tooth segmentation by low-dose CBCT for orthodontic treatment planning : Explorative ex vivo validation

Abstract:Purpose: Three-dimensional imaging has become an increasingly important component of orthodontics. Associated with this, however, is a higher radiation exposure for patients. New cone-beam computed tomography (CBCT) devices have been developed that can provide low-dose CBCT (LD-CBCT). We hypothesized that LD-CBCT is as precise and reproducible as standard high-dose CBCT (HD-CBCT) in segmenting roots and crowns as well as measuring tooth length. Methods: HD-CBCT and LD-CBCT scans were taken of four human cadaveric heads. Thirty single-rooted teeth were segmented twice by one investigator. The length of each tooth was also measured. Lin's concordance correlation coefficient (CCC) was calculated to assess the agreement of HD-CBCT and LD-CBCT measurements and the intraclass correlation coefficient (ICC) was calculated to assess intrarater reliability. Analyses were supported by Bland-Altman plots. Results: Volume measurements obtained using HD-CBCT were significantly higher than those obtained using LD-CBCT (p < 0.001). CCC was 0.975 (95% confidence interval [CI] = 0.956-0.986) indicating excellent agreement between the two modalities. Intrarater reliability between the two sets of LD-CBCT and HD-CBCT volume measurements was excellent (ICC = 0.998, 95%CI = 0.995-0.999 [HD-CBCT], ICC = 0.997, 95%CI = 0.992-0.998 [LD-CBCT]). CCC for tooth length measurements was 0.991 (95% CI = 0.983-0.995), indicating excellent agreement between HD-CBCT and LD-CBCT. Intrarater reliabilities between the two sets of tooth length measurements were also excellent for both methods (ICC = 0.998, 95%CI = 0.995-0.999 [HD-CBCT], ICC = 0.997, 95%CI = 0.992-0.998 [LD-CBCT]). Conclusions: Within the limitations of this experimental setting, LD-CBCT is as valid as HD-CBCT for measuring tooth length. Regarding the volume differences, in vivo studies are required to determine their clinical relevance.

Abstract:To assess the accuracy of CBCT in implant-supported prostheses and to evaluate metal artifacts with and without implants or implant prostheses. Accuracy and artifacts were assessed in the dried mandible at three points on the buccal and lingual cortical plates on the mandible's body near the crest and the base. On the buccal cortical plate, these points were labelled as A, B and C near the crest and D, E and F near the base of the body of the mandible. Similarly, points a to f were marked on the lingual cortical plate corresponding to points A to F. The study had two control groups, C0 for physical linear measurement (PLM) and C1 for radiographic linear measurement (RLM) and artifact assessment. There were seven test groups, TG 1 to 7, progressing from a single implant to implant full-arch prosthesis. For accuracy assessment, PLM was compared to RLM. CBCT artifacts were investigated in images integrated at 0.25 mm, 10 mm, and 20 mm at regions of interest on concentric circles at different intersecting angles by comparing grayscale values at C1 and TG1 to 7. The data were collected and statistically analyzed. A significant difference was observed between C0 and C1, and RLM in test groups at the superior axial plane. Similarly, PLM and test RLM in the sagittal plane at A-B, B-C, and D-E were statistically significant. A significant difference between PLM and RLM was also observed in the vertical plane at A-D, B-E, and C-F. Quantification of CBCT artifacts in the presence of implants or prostheses revealed that full-arch prostheses had the highest mean grayscale value, whereas single implants with a prosthesis had the lowest. The mean grayscale change was greatest around the implant and implant prosthesis. The mean grayscale value was maximum at 20 mm voxel integration scales (VIS) and lowest at 0.25 mm. CBCT is a clinically reliable device. Metal in implants or implant-supported prostheses prevents true assessment of the peri-implant area; therefore, lower VIS is suggested in the presence of implants or implant prostheses.

Abstract:Purpose This study aimed to assess the influence of magnetic resonance imaging (MRI) protocol on the decision‐making for the positioning of dental implants in edentulous arches in comparison to planning based on cone beam computed tomography (CBCT). Materials and Methods One phantom was scanned with CBCT and two MRI protocols (T1‐ and T2‐weighted). Two calibrated examiners performed digital implant planning (coDiagnostiX, Dental Wings), considering a digital prosthetic planning and alveolar ridge surface scan. Four implants were planned for each patient dataset, and the angular deviation between the long axis of the implants and the prosthetic planning occlusal plane was measured. Results Each examiner planned 40 implants, 12 implants with CBCT and 28 implants with MRI (16 with T1 and 12 with T2 images). Significant differences in angle deviation were observed between CBCT, T1, and T2 in the anterior region for both examiners (p = 0.009 for examiner 1 and p = 0.042 for examiner 2). Implants planned with CBCT showed significantly lower angular deviation than those based on the T1 and T2 scans for both examiners (p = 0.028 and p = 0.046 for examiner 1 and p = 0.028 and p = 0.027 for examiner 2). No significant difference in angular deviation was found between T1 and T2 for both examiners (examiner 1: p = 0.600, examiner 2: p = 0.916). Conclusion Compared to CBCT planning, the MRI protocol influenced decision‐making for anterior dental implants, with angular deviations within acceptable clinical thresholds, but further studies are needed to validate these findings.

Abstract:Abstract Objectives The current study aimed to evaluate different CBCT exposure protocols and influencing factors affecting the subjective image quality of scans taken for endodontic indications. Materials and methods Twelve extracted teeth, comprising of two sets of maxillary molars, premolars, canines and incisors, mandibular premolars, and molars, were endodontically treated, and either received a fiber or metal post. The teeth were scanned by CBCT imaging before and after root canal treatment, and after post insertion. Each scan was performed thrice, using an ultra low dose (ULD), standard (SM), and high-resolution mode (HR), respectively. Twelve observers—4 endodontists, 4 periodontists, and 4 radiologists—assessed the subjective image quality using visual analogue scales (VAS). Potential influencing factors were evaluated including acquisition mode, observer specialty, stage of treatment, type of post, and type of tooth, using one-way ANOVA and T test. Results Teeth scanned with the ULD had the highest average VAS score (72.5), followed by HR (70.2), and SM (69.0) for values pooled from all teeth and observers. CBCT acquisition mode was not a significant influencing factor on the VAS scores. Observer specialty, stage of treatment, type of post, and type of tooth were significant influencing factors. Conclusions Based on the present in vitro data, a low-dose CBCT mode seems not to negatively affect the perception of image quality. Clinical relevance The findings from this in vitro study demonstrate that a low-dose CBCT mode might have potential for diagnostics prior to or following endodontic treatment.
