Abstract:BACKGROUND AND PURPOSE: MR imaging provides critical information about fetal brain growth and development. Currently, morphologic analysis primarily relies on manual segmentation, which is time-intensive and has limited repeatability. This work aimed to develop a deep learning?based automatic fetal brain segmentation method that provides improved accuracy and robustness compared with atlas-based methods. MATERIALS AND METHODS: A total of 106 fetal MR imaging studies were acquired prospectively from fetuses between 23 and 39?weeks of gestation. We trained a deep learning model on the MR imaging scans of 65 healthy fetuses and compared its performance with a 4D atlas-based segmentation method using the Wilcoxon signed-rank test. The trained model was also evaluated on data from 41 fetuses diagnosed with congenital heart disease. RESULTS: The proposed method showed high consistency with the manual segmentation, with an average Dice score of 0.897. It also demonstrated significantly improved performance (P?<?.001) based on the Dice score and 95% Hausdorff distance in all brain regions compared with the atlas-based method. The performance of the proposed method was consistent across gestational ages. The segmentations of the brains of fetuses with high-risk congenital heart disease were also highly consistent with the manual segmentation, though the Dice score was 7% lower than that of healthy fetuses. CONCLUSIONS: The proposed deep learning method provides an efficient and reliable approach for fetal brain segmentation, which outperformed segmentation based on a 4D atlas and has been used in clinical and research settings.

Abstract:Fetal brain MRI has become an important tool for in utero assessment of brain development and disorders. However, quantitative analysis of fetal brain MRI remains difficult, partially due to the limited tools for automated preprocessing and the lack of normative brain templates. In this paper, we proposed an automated pipeline for fetal brain extraction, super-resolution reconstruction, and fetal brain atlasing to quantitatively map in utero fetal brain development during mid-to-late gestation in a Chinese population. First, we designed a U-net convolutional neural network for automated fetal brain extraction, which achieved an average accuracy of 97%. We then generated a developing fetal brain atlas, using an iterative linear and nonlinear registration approach. Based on the 4D spatiotemporal atlas, we quantified the morphological development of the fetal brain between 23 and 36 weeks of gestation. The proposed pipeline enabled the fully automated volumetric reconstruction for clinically available fetal brain MRI data, and the 4D fetal brain atlas provided normative templates for the quantitative characterization of fetal brain development, especially in the Chinese population.

Abstract:Magnetic resonance imaging (MRI) is a prevailing method of modal infant brain tissue analysis that precisely segments brain tissue and is vitally important for diagnosis, remediation, and analysis of early brain development. To achieve such segmentation is challenging, particularly for the brain of a six-month-old, owing to several factors: poor image quality; isointense contrast between white and gray matter and the simple incomplete volume consequence of a tiny brain size; and discrepancies in brain tissues, illumination settings, and the vagarious region. This article addresses these challenges with a fuzzy-informed deep learning segmentation network that takes T1- and T2-weighted MRIs as inputs. First, a fuzzy logic layer encodes input to the fuzzy domain. Second, a volumetric fuzzy pooling (VFP) layer models the local fuzziness of the volumetric convolutional maps by applying fuzzification, accumulation, and defuzzification on the adjacency feature map neighborhoods. Third, the VFP layer is employed to design the fuzzy-enabled multiscale feature learning module to enable the extraction of brain features in different receptive fields. Finally, we redesign the Project & Excite module using the VPF layer to enable modeling uncertainty during feature recalibration, and a comprehensive training paradigm is used to learn the ideal parameters of every building block. Extensive experimental comparative studies substantiate the efficiency and accuracy of the proposed model in terms of different evaluation metrics to solve multimodal infant brain segmentation problems on the iSeg-2017 dataset.

Abstract:Fetal brain tissue segmentation is essential for quantifying the presence of congenital disorders in the developing fetus. Manual segmentation of fetal brain tissue is cumbersome and time-consuming, so using an automatic segmentation method can greatly simplify the process. In addition, the fetal brain undergoes a variety of changes throughout pregnancy, such as increased brain volume, neuronal migration, and synaptogenesis. In this case, the contrast between tissues, especially between gray matter and white matter, constantly changes throughout pregnancy, increasing the complexity and difficulty of our segmentation. To reduce the burden of manual refinement of segmentation, we proposed a new deep learning-based segmentation method. Our approach utilized a novel attentional structural block, the contextual transformer block (CoT-Block), which was applied in the backbone network model of the encoder-decoder to guide the learning of dynamic attentional matrices and enhance image feature extraction. Additionally, in the last layer of the decoder, we introduced a hybrid dilated convolution module, which can expand the receptive field and retain detailed spatial information, effectively extracting the global contextual information in fetal brain MRI. We quantitatively evaluated our method according to several performance measures: dice, precision, sensitivity, and specificity. In 80 fetal brain MRI scans with gestational ages ranging from 20 to 35 weeks, we obtained an average Dice similarity coefficient (DSC) of 83.79%, an average Volume Similarity (VS) of 84.84%, and an average Hausdorff95 Distance (HD95) of 35.66 mm. We also used several advanced deep learning segmentation models for comparison under equivalent conditions, and the results showed that our method was superior to other methods and exhibited an excellent segmentation performance.

Abstract:Automated quantification of brain tissues on MR images has greatly contributed to the diagnosis and follow-up of neurological pathologies across various life stages. However, existing solutions are specifically designed for certain age ranges, limiting their applicability in monitoring brain development from infancy to late adulthood. This retrospective study aims to develop and validate a brain segmentation model across pediatric and adult populations. First, we trained a deep learning model to segment tissues and brain structures using T1-weighted MR images from 390 patients (age range: 2–81 years) across four different datasets. Subsequently, the model was validated on a cohort of 280 patients from six distinct test datasets (age range: 4–90 years). In the initial experiment, the proposed deep learning-based pipeline, icobrain-dl, demonstrated segmentation accuracy comparable to both pediatric and adult-specific models across diverse age groups. Subsequently, we evaluated intra- and inter-scanner variability in measurements of various tissues and structures in both pediatric and adult populations computed by icobrain-dl. Results demonstrated significantly higher reproducibility compared to similar brain quantification tools, including childmetrix, FastSurfer, and the medical device icobrain v5.9 ( p -value< 0.01). Finally, we explored the potential clinical applications of icobrain-dl measurements in diagnosing pediatric patients with Cerebral Visual Impairment and adult patients with Alzheimer's Disease.

Abstract:<p>Accurate segmentation of white matter, gray matter and cerebrospinal fluid from neonatal brain MR images is of great importance in characterizing early brain development. Deep-learning-based methods have been successfully applied to neonatal brain MRIs with superior performance if testing subjects were acquired with the same imaging protocols/scanners as training subjects. However, for the testing subjects acquired with different imaging protocols/scanners, they cannot achieve accurate segmentation results due to large appearance/pattern differences between the testing and training subjects. Besides, imaging artifacts, like head motion, which are inevitable during the imaging acquisition process, also pose a challenge for the segmentation methods. To address these issues, in this paper, we propose a harmonized neonatal brain MR image segmentation model that harmonizes testing images acquired by different protocols/scanners into the domain of training images through a cycle-consistent generative adversarial network (CycleGAN). Meanwhile, the artifacts can be largely alleviated during the harmonization. Then, a densely-connected U-Net based segmentation model trained in the domain of training images can be applied robustly for segmenting the harmonized testing images. Comparisons with existing methods illustrate the better performance of the proposed method on neonatal brain MR images from cross-sites, a grand segmentation challenge, as well as images with artifacts.</p>

Abstract:To accurately analyze changes of anatomical structures in longitudinal imaging studies, consistent segmentation across multiple time-points is required. Existing solutions often involve independent registration and segmentation components. Registration between time-points is used either as a prior for segmentation in a subsequent time point or to perform segmentation in a common space. In this work, we propose a novel hybrid convolutional neural network (CNN) that integrates segmentation and registration into a single procedure. We hypothesize that the joint optimization leads to increased performance on both tasks. The hybrid CNN is trained by minimizing an integrated loss function composed of four different terms, measuring segmentation accuracy, similarity between registered images, deformation field smoothness, and segmentation consistency. We applied this method to the segmentation of white matter tracts, describing functionally grouped axonal fibers, using N=8045 longitudinal brain MRI data of 3249 individuals. The proposed method was compared with two multistage pipelines using two existing segmentation methods combined with a conventional deformable registration algorithm. In addition, we assessed the added value of the joint optimization for segmentation and registration separately. The hybrid CNN yielded significantly higher accuracy, consistency and reproducibility of segmentation than the multistage pipelines, and was orders of magnitude faster. Therefore, we expect it can serve as a novel tool to support clinical and epidemiological analyses on understanding microstructural brain changes over time.

Abstract:Accurate segmentation of fetal brain magnetic resonance images is crucial for analyzing fetal brain development and detecting potential neurodevelopmental abnormalities. Traditional deep learning-based automatic segmentation, although effective, requires extensive training data with ground-truth labels, typically produced by clinicians through a time-consuming annotation process. To overcome this challenge, we propose a novel unsupervised segmentation method based on multi-atlas segmentation, that accurately segments multiple tissues without relying on labeled data for training. Our method employs a cascaded deep learning network for 3D image registration, which computes small, incremental deformations to the moving image to align it precisely with the fixed image. This cascaded network can then be used to register multiple annotated images with the image to be segmented, and combine the propagated labels to form a refined segmentation. Our experiments demonstrate that the proposed cascaded architecture outperforms the state-of-the-art registration methods that were tested. Furthermore, the derived segmentation method achieves similar performance and inference time to nnU-Net while only using a small subset of annotated data for the multi-atlas segmentation task and none for training the network. Abstract:Deformable registration is fundamental to longitudinal and population-based image analyses. However, it is challenging to precisely align longitudinal infant brain MR images of the same subject, as well as cross-sectional infant brain MR images of different subjects, due to fast brain development during infancy. In this paper, we propose a recurrently usable deep neural network for the registration of infant brain MR images. There are three main highlights of our proposed method. (i) We use brain tissue segmentation maps for registration, instead of intensity images, to tackle the issue of rapid contrast changes of brain tissues during the first year of life. (ii) A single registration network is trained in a one-shot manner, and then recurrently applied in inference for multiple times, such that the complex deformation field can be recovered incrementally. (iii) We also propose both the adaptive smoothing layer and the tissue-aware anti-folding constraint into the registration network to ensure the physiological plausibility of estimated deformations without degrading the registration accuracy. Experimental results, in comparison to the state-of-the-art registration methods, indicate that our proposed method achieves the highest registration accuracy while still preserving the smoothness of the deformation field. Knowledge-Guided Prompt Learning for Lifespan Brain MR Image Segmentation

Abstract:Automatic and accurate segmentation of brain MR images throughout the human lifespan into tissue and structure is crucial for understanding brain development and diagnosing diseases. However, challenges arise from the intricate variations in brain appearance due to rapid early brain development, aging, and disorders, compounded by the limited availability of manually-labeled datasets. In response, we present a two-step segmentation framework employing Knowledge-Guided Prompt Learning (KGPL) for brain MRI. Specifically, we first pre-train segmentation models on large-scale datasets with sub-optimal labels, followed by the incorporation of knowledge-driven embeddings learned from image-text alignment into the models. The introduction of knowledge-wise prompts captures semantic relationships between anatomical variability and biological processes, enabling models to learn structural feature embeddings across diverse age groups. Experimental findings demonstrate the superiority and robustness of our proposed method, particularly noticeable when employing Swin UNETR as the backbone. Our approach achieves average DSC values of 95.17% and 94.19% for brain tissue and structure segmentation, respectively. Abstract:To better understand early brain development in health and disorder, it is critical to accurately segment infant brain magnetic resonance (MR) images into white matter (WM), gray matter (GM), and cerebrospinal fluid (CSF). Deep learning-based methods have achieved state-of-the-art performance; h owever, one of the major limitations is that the learning-based methods may suffer from the multi-site issue, that is, the models trained on a dataset from one site may not be applicable to the datasets acquired from other sites with different imaging protocols/scanners. To promote methodological development in the community, the iSeg-2019 challenge (http://iseg2019.web.unc.edu) provides a set of 6-month infant subjects from multiple sites with different protocols/scanners for the participating methods. T raining/validation subjects are from UNC (MAP) and testing subjects are from UNC/UMN (BCP), Stanford University, and Emory University. By the time of writing, there are 30 automatic segmentation methods participated in the iSeg-2019. In this article, 8 top-ranked methods were reviewed by detailing their pipelines/implementations, presenting experimental results, and evaluating performance across different sites in terms of whole brain, regions of interest, and gyral landmark curves. We further pointed out their limitations and possible directions for addressing the multi-site issue. We find that multi-site consistency is still an open issue. We hope that the multi-site dataset in the iSeg-2019 and this review article will attract more researchers to address the challenging and critical multi-site issue in practice.

Abstract:Objectives: Brain segmentation of infant magnetic resonance (MR) images is vitally important in studying developmental mental health and disease. The infant brain undergoes many changes throughout the first years of postnatal life, making tissue segmentation difficult for most existing algorithms. Here, we introduce a deep neural network BIBSNet (Baby and Infant Brain Segmentation Neural Network), an open-source, community-driven model that relies on data augmentation and a large sample size of manually annotated images to facilitate the production of robust and generalizable brain segmentations. Experimental Design: Included in model training and testing were MR brain images on 84 participants with an age range of 0-8 months (median postmenstrual ages of 13.57 months). Using manually annotated real and synthetic segmentation images, the model was trained using a 10-fold cross-validation procedure. Testing occurred on MRI data processed with the DCAN labs infant-ABCD-BIDS processing pipeline using segmentations produced from gold standard manual annotation, joint-label fusion (JLF), and BIBSNet to assess model performance. Principal Observations: Using group analyses, results suggest that cortical metrics produced using BIBSNet segmentations outperforms JLF segmentations. Additionally, when analyzing individual differences, BIBSNet segmentations perform even better. Conclusions: BIBSNet segmentation shows marked improvement over JLF segmentations across all age groups analyzed. The BIBSNet model is 600x faster compared to JLF and can be easily included in other processing pipelines.

Abstract:Automatic segmentation of brain MR images into white matter (WM), gray matter (GM), and cerebrospinal fluid (CSF) is critical for tissue volumetric analysis and cortical surface reconstruction. Due to dramatic structural and appearance changes associated with developmental and aging processes, existing brain tissue segmentation methods are only viable for specific age groups. Consequently, methods developed for one age group may fail for another. In this paper, we make the first attempt to segment brain tissues across the entire human lifespan (0-100 years of age) using a unified deep learning model. To overcome the challenges related to structural variability underpinned by biological processes, intensity inhomogeneity, motion artifacts, scanner-induced differences, and acquisition protocols, we propose to use contrastive learning to improve the quality of feature representations in a latent space for effective lifespan tissue segmentation. We compared our approach with commonly used segmentation methods on a large-scale dataset of 2,464 MR images. Experimental results show that our model accurately segments brain tissues across the lifespan and outperforms existing methods.

Abstract:Magnetic resonance (MR) imaging is widely used for assessing infant head and brain development and for diagnosing pathologies. The main goal of this work is the development of a segmentation framework to create patient-specific head and brain anatomical models from MR images for clinical evaluation. The proposed strategy consists of a fusion-based Deep Learning (DL) approach that combines the information of different image sequences within the MR acquisition protocol, including the axial T1w, sagittal T1w, and coronal T1w after contrast. These image sequences are used as input for different fusion encoder–decoder network architectures based on the well-established U-Net framework. Specifically, three different fusion strategies are proposed and evaluated, namely early, intermediate, and late fusion. In the early fusion approach, the images are integrated at the beginning of the encoder–decoder architecture. In the intermediate fusion strategy, each image sequence is processed by an independent encoder, and the resulting feature maps are then jointly processed by a single decoder. In the late fusion method, each image is individually processed by an encoder–decoder, and the resulting feature maps are then combined to generate the final segmentations. A clinical in-house dataset consisting of 19 MR scans was used and divided into training, validation, and testing sets, with 3 MR scans defined as a fixed validation set. For the remaining 16 MR scans, a cross-validation approach was adopted to assess the performance of the methods. The training and testing processes were carried out with a split ratio of 75% for the training set and 25% for the testing set. The results show that the early and intermediate fusion methodologies presented the better performance (Dice coefficient of 97.6 ± 1.5% and 97.3 ± 1.8% for the head and Dice of 94.5 ± 1.7% and 94.8 ± 1.8% for the brain, respectively), whereas the late fusion method generated slightly worst results (Dice of 95.5 ± 4.4% and 93.8 ± 3.1% for the head and brain, respectively). Nevertheless, the volumetric analysis showed that no statistically significant differences were found between the volumes of the models generated by all the segmentation strategies and the ground truths. Overall, the proposed frameworks demonstrate accurate segmentation results and prove to be feasible for anatomical model analysis in clinical practice.

Abstract:Large scale early scanning of fetuses via ultrasound imaging is widely used to alleviate the morbidity or mortality caused by congenital anomalies in fetal hearts and lungs. To reduce the intensive cost during manual recognition of organ regions, many automatic segmentation methods have been proposed. However, the existing methods still encounter multi-scale problem at a larger range of receptive fields of organs in images, resolution problem of segmentation mask, and interference problem of task-irrelevant features, obscuring the attainment of accurate segmentations. To achieve semantic segmentation with functions of (1) extracting multi-scale features from images, (2) compensating information of high resolution, and (3) eliminating the task-irrelevant features, we propose a multi-scale model with skip connection framework and attention mechanism integrated. The multi-scale feature extraction modules are incorporated with additive attention gate units for irrelevant feature elimination, through a U-Net framework with skip connections for information compensation. The performance of fetal heart and lung segmentation indicates the superiority of our method over the existing deep learning based approaches. Our method also shows competitive performance stability during the task of semantic segmentations, showing a promising contribution on ultrasound based prognosis of congenital anomaly in the early intervention, and alleviating the negative effects caused by congenital anomaly.

Abstract:Most segmentation methods in child brain MRI are supervised and are based on global intensity distributions of major brain structures. The successful implementation of a supervised approach depends on availability of an age-appropriate probabilistic brain atlas. For the study of early normal brain development, the construction of such a brain atlas remains a significant challenge. Moreover, using global intensity statistics leads to inaccurate detection of major brain tissue classes due to substantial intensity variations of MR signal within the constituent parts of early developing brain. In order to overcome these methodological limitations we develop a local, semi-supervised framework. It is based on Kernel Fisher Discriminant Analysis (KFDA) for pattern recognition, combined with an objective structural similarity index (SSIM) for perceptual image quality assessment. The proposed method performs optimal brain partitioning into subdomains having different average intensity values followed by SSIM-guided computation of separating surfaces between the constituent brain parts. The classified image subdomains are then stitched slice by slice via simulated annealing to form a global image of the classified brain. In this paper, we consider classification into major tissue classes (white matter and grey matter) and the cerebrospinal fluid and illustrate the proposed framework on examples of brain templates for ages 8 to 11 months and ages 44 to 60 months. We show that our method improves detection of the tissue classes by its comparison to state-of-the-art classification techniques known as Partial Volume Estimation.

Abstract:Brain magnetic imaging(MRI) has become an effective tool for disease diagnosis, disease tracking and so on. Deep brain structures such as hippocampus plays an important role in the quantitative assessment of human brain. Therefore, it is crucial to segment these deep brain structures in a short time. In recent years, deep learning has achieved great process in image processing, especially the U-net, which is the Champion Model of 2015 ISBI Cell Tracking Challenge. In this paper, a novel structure was proposed which based on U-net as well as substitute the normal convolution block with the residual block to achieve a robust segmentation. In addition, SEblock is introduced to emphasize the correlation between channels which helps to focus on the features that are more important. The experimental results on the IBSR dataset show that the method proposed in this paper achieves average Dice value 0.83 on the deep brain structures segmentation, which verified the effectiveness of the method.
