Abstract:The increasing interest in innovative solutions for health and physiological monitoring has recently fostered the development of smaller biomedical devices. These devices are capable of recording an increasingly large number of biosignals simultaneously, while maximizing the user's comfort. In this study, we have designed and realized a novel wearable multisensor ring-shaped probe that enables synchronous, real-time acquisition of photoplethysmographic (PPG) and galvanic skin response (GSR) signals. The device integrates both the PPG and GSR sensors onto a single probe that can be easily placed on the finger, thereby minimizing the device footprint and overall size. The system enables the extraction of various physiological indices, including heart rate (HR) and its variability, oxygen saturation (SpO2), and GSR levels, as well as their dynamic changes over time, to facilitate the detection of different physiological states, e.g., rest and stress. After a preliminary SpO2 calibration procedure, measurements have been carried out in laboratory on healthy subjects to demonstrate the feasibility of using our system to detect rapid changes in HR, skin conductance, and SpO2 across various physiological conditions (i.e., rest, sudden stress-like situation and breath holding). The early findings encourage the use of the device in daily-life conditions for real-time monitoring of different physiological states.

Abstract:Bio-signals are being increasingly used for the assessment of pathophysiological conditions including pain, stress, fatigue, and anxiety. For some approaches, a single signal is not sufficient to provide a comprehensive diagnosis; however, there is a growing consensus that multimodal approaches allow higher sensitivity and specificity. For instance, in visceral pain subjects, the autonomic activation can be inferred using electrodermal activity (EDA) and heart rate variability derived from the electrocardiogram (ECG), but including the muscle activation detected from the surface electromyogram (sEMG) can better differentiate the disease that causes the pain. There is no wearable device commercially capable of collecting these three signals simultaneously. This paper presents the validation of a novel multimodal low profile wearable data acquisition device for the simultaneous collection of EDA, ECG, and sEMG signals. The device was validated by comparing its performance to laboratory-scale reference devices. N = 20 healthy subjects were recruited to participate in a four-stage study that exposed them to an array of cognitive, orthostatic, and muscular stimuli, ensuring the device is sensitive to a range of stressors. Time and frequency domain analyses for all three signals showed significant similarities between our device and the reference devices. Correlation of sEMG metrics ranged from 0.81 to 0.95 and EDA/ECG metrics showed few instances of significant difference in trends between our device and the references. With only minor observed differences, we demonstrated the ability of our device to collect EDA, sEMG, and ECG signals. This device will enable future practical and impactful advances in the field of chronic pain and stress measurement and can confidently be implemented in related studies.

Abstract:Smart devices are more and more present in every aspect of everyday life. From smartphones, which are now like mini-computers, through systems for monitoring sleep or fatigue, to specific sensors for the recording of vital parameters. A particular class of the latter regards health monitoring. Indeed, through the use of such devices, several vital parameters can be acquired and automatically monitored, even remotely. This paper presents the second generation of VITAL-ECG, a smart device designed to monitor the most important vital parameters as a “one touch” device, anywhere, at low cost. It is a wearable device that coupled with a mobile app can track bio-parameters such as: electrocardiogram, SpO2, skin temperature, and physical activity of the patient. Even if it not yet a medical device, a comprehensive comparison with a golden standard electrocardiograph is presented to demonstrate the quality of the recorded signals and the validity of the proposed approach.

Abstract:Wearable medical devices (WMDs) for healthcare applications have become ubiquitous, allowing remote, at-home, and real-time chronic monitoring that have significantly decongested clinics. These WMDs permitted the monitoring of several physiological parameters, such as heart and respiration rates, SPO2, temperature, and energy expenditure during activities of daily living (ADLs) or fitness activities. While the measurement of these parameters has become common, full noninvasive, unobtrusive, and real-time blood pressure (BP) monitoring remains elusive owing to BP's complex dynamics. To bring this into fruition, several works have been conducted combining different biosignals to indirectly extract BP by using PTT. Unlike previous works, we considered PTT variability by averaging it over discrete durations to account for BP variability for a more accurate estimation. PTTs were obtained using electrocardiograph (ECG) and reflective photoplethysmograph (rPPG) signals extracted by a wearable device attached to a single site on the upper arm. Our results show a significant correlation between average PTT and the BP measured using auscultation in a trial study. The developed system has potential for chronic, noninvasive, and cuff-less blood pressure monitors (BPMs) for localized and single-site implementations. Meanwhile, real-time data from the wearable device may be accessed via a remote desktop or a mobile phone application.

Abstract:Monitoring the effects of daily activities on the physiological responses of the body calls for wearable devices that can simultaneously track metabolic and haemodynamic parameters. Here we describe a non-invasive skin-worn device for the simultaneous monitoring of blood pressure and heart rate via ultrasonic transducers and of multiple biomarkers via electrochemical sensors. We optimized the integrated device so that it provides mechanical resiliency and flexibility while conforming to curved skin surfaces, and to ensure reliable sensing of glucose in interstitial fluid and of lactate, caffeine and alcohol in sweat, without crosstalk between the individual sensors. In human volunteers, the device captured physiological effects of food intake and exercise, in particular the production of glucose after food digestion, the consumption of glucose via glycolysis, and increases in blood pressure and heart rate compensating for oxygen depletion and lactate generation. Continuous and simultaneous acoustic and electrochemical sensing via integrated wearable devices should enrich the understanding of the body's response to daily activities, and could facilitate the early prediction of abnormal physiological changes.

Abstract:In recent years, due to increased attention of individual physical health condition, the demand on portable multi-vital signs monitoring devices has shown an explosive growth. This work designed a portable, multi-vital signs monitoring system, consisting of chest unit and wrist unit, with size of 32 mm x 32 mm and diameter of 30 mm, respectively. The designed system is capable of transmitting measured electrocardiograph (ECG), pulse wave and body temperature signal to the mobile devices wirelessly by Bluetooth low energy (BLE) technique. The heart rate, pulse oximetry (SpO(2)) and respiratory rate are extracted through time domain algorithm. The chest and wrist units can continuously monitor body vital signs for more than 12 and 24 hours, respectively. Besides, the experiment results reveal that the chest unit is able to monitor patients with heart rate abnormalities such as atrial fibrillation and premature beats. The typical heart diseases can be detected in real-time. Meanwhile, the wrist unit clearly detects the changes of the physical parameters of the subjects under different physiological conditions. The accuracy of the designed system is consistent with that of the commercial medical instrument counterpart, with further advantages of compact, portable and patient-friendly natures.

Abstract:The continuous and simultaneous monitoring of physiological parameters represents a key aspect in clinical environments, remote monitoring and occupational settings. In this regard, respiratory rate (RR) and heart rate (HR) are correlated with several physiological and pathological conditions of the patients/workers, and with environmental stressors. In this work, we present and validate a wearable device for the continuous monitoring of such parameters. The proposed system embeds four conductive sensors located on the user’s chest which allow retrieving the breathing activity through their deformation induced during cyclic expansion and contraction of the rib cage. For monitoring HR we used an embedded IMU located on the left side of the chest wall. We compared the proposed device in terms of estimating HR and RR against a reference system in three scenarios: sitting, standing and supine. The proposed system reliably estimated both RR and HR, showing low error averaged along subjects in all scenarios. This is the first study focused on the feasibility assessment of a wearable system based on a multi-sensor configuration (i.e., conductive sensors and IMU) for RR and HR monitoring. The promising results encourage the application of this approach in clinical and occupational settings.

Abstract:In the rapidly evolving landscape of continuous electrocardiogram (ECG) monitoring systems, there is a heightened demand for non-invasive sensors capable of measuring ECGs and detecting heart rate variability (HRV) in diverse populations, ranging from cardiovascular patients to sports enthusiasts. Challenges like device accuracy, patient privacy, signal noise, and long-term safety impede the use of wearable devices in clinical practice. This scoping review aims to assess the performance and safety of novel multi-channel, sensor-based biopotential wearable devices in adults. A comprehensive search strategy was employed on four databases, resulting in 143 records and the inclusion of 12 relevant studies. Most studies focused on healthy adult subjects (n = 6), with some examining controlled groups with atrial fibrillation (AF) (n = 3), long QT syndrome (n = 1), and sleep apnea (n = 1). The investigated bio-sensor devices included chest-worn belts (n = 2), wrist bands (n = 2), adhesive chest strips (n = 2), and wearable textile smart clothes (n = 4). The primary objective of the included studies was to evaluate device performance in terms of accuracy, signal quality, comparability, and visual assessment of ECGs. Safety findings, reported in five articles, indicated no major side effects for long-term/continuous monitoring, with only minor instances of skin irritation. Looking forward, there are ample opportunities to enhance and test these technologies across various physical activity intensities and clinical conditions.

Wrist-Worn Sensor Validation for Heart Rate Variability and Electrodermal Activity Detection in a Stressful Driving Environment

Abstract:Wearable sensors are widely used to gather psychophysiological data in the laboratory and real-world applications. However, the accuracy of these devices should be carefully assessed. The study focused on testing the accuracy of the Empatica 4 (E4) wristband for the detection of heart rate variability (HRV) and electrodermal activity (EDA) metrics in stress-inducing conditions and growing-risk driving scenarios. Fourteen healthy subjects were recruited for the experimental campaign, where HRV and EDA were recorded over six experimental conditions (Baseline, Video Clip, Scream, No-Risk Driving, Low-Risk Driving, and High-Risk Driving) and by means of two measurement systems: the E4 device and a gold standard system. The overall quality of the E4 data was investigated; agreement and reliability were assessed by performing a Bland-Altman analysis and by computing the Spearman's correlation coefficient. HRV time-domain parameters reported high reliability levels in Baseline (r > 0.72), Video Clip (r > 0.71), and No-Risk Driving (r > 0.67), while HRV frequency domain parameters were sufficient in Baseline (r > 0.58), Video Clip (r > 0.59), No-Risk (r > 0.51), and Low-Risk Driving (r > 0.52). As for the EDA parameters, no correlation was found. Further studies could enhance the HRV and EDA quality through further optimizations of the acquisition protocol and improvement of the processing algorithms.

Abstract:Bio-signals including electrocardiogram (ECG), respiration rate, motion-related signals, plantar pressure distribution are important for monitoring and assessing a person’s health status. Via some of the bio-signals, some diseases or abnormalities such as cardiovascular diseases or a human fall can be detected. However, the traditional health monitoring systems have limitations as they are expensive, not-easy-to-use, stationary, inconvenient and uncomfortable to use. It is required an advanced solution for dealing with the limitations while maintaining a high quality of services such as high-quality signals and accuracy. Therefore, we present a novel wireless wearable health monitoring system in this paper. The system consists of an upper limb device and a lower limb device using flexible materials such as polydimethylsiloxane-graphene compound and textile materials for collecting different ECG, EMG, respiration rate, motion-related signals, plantar pressure distribution. The complete system is implemented, tested and verified in a clinical environment in a hospital. The results show that the system can obtain high-quality bio-signals and can be a potential application for both home-care and hospital-care.
AI-Enabled Smart Wristband Providing Real-Time Vital Signs and Stress Monitoring

Abstract:This work introduces the design, architecture, implementation, and testing of a low-cost and machine-learning-enabled device to be worn on the wrist. The suggested wearable device has been developed for use during emergency incidents of large passenger ship evacuations, and enables the real-time monitoring of the passengers' physiological state, and stress detection. Based on a properly preprocessed PPG signal, the device provides essential biometric data (pulse rate and oxygen saturation level) and an efficient unimodal machine learning pipeline. The stress detecting machine learning pipeline is based on ultra-short-term pulse rate variability, and has been successfully integrated into the microcontroller of the developed embedded device. As a result, the presented smart wristband is able to provide real-time stress detection. The stress detection system has been trained with the use of the publicly available WESAD dataset, and its performance has been tested through a two-stage process. Initially, evaluation of the lightweight machine learning pipeline on a previously unseen subset of the WESAD dataset was performed, reaching an accuracy score equal to 91%. Subsequently, external validation was conducted, through a dedicated laboratory study of 15 volunteers subjected to well-acknowledged cognitive stressors while wearing the smart wristband, which yielded an accuracy score equal to 76%.

Abstract:Remote monitoring of vital body signals has drawn the attention of late, particularly with the burst of COVID-19. Wearable devices are started to be widely used for non-invasive health monitoring tasks. This work presents a wearable system for multiple body signal monitoring using only one biocompatible aluminum nitride piezoelectric sensor and a custom wireless electronic device for data acquisition and transmission. The proposed sensor has been customized for the suprasternal notch, where we can simultaneously extract heart rate, respiration rate, and deglutition events. These parameters are helpful for the remote diagnosis of multiple diseases like cardiac arrhythmia, asthma, and dysphagia. Moreover, heart sound components have been derived from the same signal, providing critical information about heart health and insight into possible heart diseases. The preliminary experimental results show that the proposed wearable system can be used for personalized healthcare applications and offers a promising solution for unobtrusive remote health monitoring.

Monitoring Cardiovascular Physiology Using Bio-Compatible AlN Piezoelectric Skin Sensors

Abstract:Arterial pulse waves contain a wealth of parameters indicative of cardiovascular disease. As such, monitoring them continuously and unobtrusively can provide health professionals with a steady stream of cardiovascular health indices, allowing for the development of efficient, individualized treatments and early cardiovascular disease diagnosis solutions. Blood pulsations in superficial arteries cause skin surface deformations, typically undetectable to the human eye; therefore, Microelectromechanical systems (MEMS) can be used to measure these deformations and thus create unobtrusive pulse wave monitoring devices. Miniaturized ultrathin and flexible Aluminium Nitride (AlN) piezoelectric MEMS are highly sensitive to minute mechanical deformations, making them suitable for detecting the skin deformations caused by cardiac events and consequently providing multiple biomarkers useful for monitoring cardiovascular health and assessing cardiovascular disease risk. Conventional wearable continuous pulse wave monitoring solutions are typically large and based on technologies limiting their versatility. Therefore, we propose the adoption of -thick biocompatible, skin-conforming devices on piezoelectric AlN to create versatile, multipurpose arterial pulse wave monitoring devices. In our initial trials, the devices are placed over arteries along the wrist (radial artery), neck (carotid artery), and suprasternal notch (on the chest wall and close to the ascending aorta). We also leverage the mechano-acoustic properties of the device to detect heart muscle vibrations corresponding to heart sounds S1 and S2 from the suprasternal notch measurement site. Finally, we characterize the piezoelectric device outputs observed with the cardiac cycle events using synchronized electrocardiogram (ECG) reference signals and provide information on heart rate, breathing rate, and heart sounds. The extracted parameters strongly agree with reference values as illustrated by minimum Pearson correlation coefficients (r) of 0.81 for pulse rate and 0.95 for breathing rate.

Abstract:Recently, the ever-growing interest in the continuous monitoring of heart function in out-of-laboratory settings for an early diagnosis of cardiovascular diseases has led to the investigation of innovative methods for cardiac monitoring. Among others, wearables recording seismic waves induced on the chest surface by the mechanical activity of the heart are becoming popular. For what concerns wearable-based methods, cardiac vibrations can be recorded from the thorax in the form of acceleration, angular velocity, and/or displacement by means of accelerometers, gyroscopes, and fiber optic sensors, respectively. The present paper reviews the currently available wearables for measuring precordial vibrations. The focus is on sensor technology and signal processing techniques for the extraction of the parameters of interest. Lastly, the explored application scenarios and experimental protocols with the relative influencing factors are discussed for each technique. The goal is to delve into these three fundamental aspects (i.e., wearable system, signal processing, and application scenario), which are mutually interrelated, to give a holistic view of the whole process, beyond the sensor aspect alone. The reader can gain a more complete picture of this context without disregarding any of these 3 aspects.

Abstract:Development of unconventional technologies for wireless collection, storage and analysis of quantitative, clinically relevant information on physiological status is of growing interest. Soft, biocompatible systems are widely regarded as important because they facilitate mounting on external (e.g. skin) and internal (e.g. heart, brain) surfaces of the body. Ultra-miniaturized, lightweight and battery-free devices have the potential to establish complementary options in bio-integration, where chronic interfaces (i.e. months) are possible on hard surfaces such as the fingernails and the teeth, with negligible risk for irritation or discomfort. Here we report materials and device concepts for flexible platforms that incorporate advanced optoelectronic functionality for applications in wireless capture and transmission of photoplethysmograms, including quantitative information on blood oxygenation, heart rate and heart rate variability. Specifically, reflectance pulse oximetry in conjunction with near-field communication (NFC) capabilities enables operation in thin, miniaturized flexible devices. Studies of the material aspects associated with the body interface, together with investigations of the radio frequency characteristics, the optoelectronic data acquisition approaches and the analysis methods capture all of the relevant engineering considerations. Demonstrations of operation on various locations of the body and quantitative comparisons to clinical gold standards establish the versatility and the measurement accuracy of these systems, respectively.

Abstract:Long-term continuous systolic blood pressure (SBP) and heart rate (HR) monitors are of tremendous value to medical (cardiovascular, circulatory and cerebrovascular management), wellness (emotional and stress tracking) and fitness (performance monitoring) applications, but face several major impediments, such as poor wearability, lack of widely accepted robust SBP models and insufficient proofing of the generalization ability of calibrated models.

In this study, we demonstrated the acquisition of biophysical signals at one point on the body using a wearable all-gel-integrated multimodal sensor composed of four element sensors, inspired by the slow/rapid adapting functions of the skin sensory receptors. The gel-type sensors ensure flexibility, compactness, portability, adherence, and integrity. The wearable all-gel multimodal sensor was easily attached to the wrist and simultaneously gathered blood pressure (BP), electrocardiogram (ECG), electromyogram (EMG), and mechanomyogram (MMG) signals related to cardiac and muscle health. Human activity causes muscle contraction, which affects blood flow; therefore, the relationship between the muscle and heart is crucial for screening and predicting heart health. We monitored cardiac health by obtaining the two types of phase time differences (i.e., Δtbe: BP and ECG, Δtem: ECG and MMG) generated during muscle movement. The suggested multimodal sensor has potential applicability in monitoring biophysical conditions and diagnosing cardiac-related health problems.This article is protected by copyright. SensoRing: An Integrated Wearable System for Continuous Measurement of Physiological Biomarkers

Abstract:Advancements in miniaturized electronics and smart sensors combined with a broad platform of smart phones, big data, cloud service and wireless communication have not only empowered wearable technology, they have also increased users life expectancy. This is done through a range of applications including; tracking physical activity, personalized health care, and recommendations for enhancing user experience. However, the ability to measure one's emotional state using wearable biosensors in healthcare applications has been limited due to several challenges related to a lack in comfort, accuracy, reliability and low-power consumption. In this paper, we presented a novel wearable ring sensor for the continuous measurement of four SNS activities: Electrodermal activity (EDA), heart rate, skin temperature and locomotion. Detailed information is given regarding the development of the proposed ring sensor followed by a discussion of the evaluation that was done utilizing the biosensor on volunteers to gather data. Specifically, volunteers wore the ring sensor while being simultaneously monitored with real-time telemetry and while going through different emotional states. The experiment demonstrated accurate results, and data were collected from 43 participants of diverse age, body mass, height, and race. Additionally, to evaluate the performance of the developed ring sensor, we compared the results with a state-of-the-art open source device. This research aims to improve the worn biomedical sensor market, specifically when it comes to size and accuracy of worn sensors.
The Emerging Role of Wearable Technologies in Detection of Arrhythmia

Abstract:Over the past decade, there has been an explosion of consumer devices for the purposes of health and fitness tracking. The wearable technology market, composed of devices that monitor physiological parameters, such as heart rate and sleep pattern, is anticipated to grow to 929 million connected devices in 2021. These devices encompass wristbands, glasses, in-ear monitors, or electronic shirts, with varying capacity to monitor heart rate, heart rhythm, blood pressure, physical activity, respiratory rate, blood glucose, and sleep patterns. For heart-rate monitoring, most wearable devices use photoplethysmography (PPG) technology, meaning they are inherently less accurate than conventional electrocardiography monitoring techniques (reference standard). However, a growing body of evidence suggests that these technologies can be harnessed to facilitate arrhythmia detection in the appropriate context. Studies evaluating PPG-based wearables in conjunction with machine-learning algorithms have shown promise in detection of such arrhythmias, as atrial fibrillation. Limitations of wearable technologies include their accuracy and accessibility and the clinical implications of wearable-detected arrhythmias. Despite this, wearable technologies represent an important frontier in health evaluation. Future wearables will benefit from improved reliability and accuracy, collect additional health and fitness parameters, support management of chronic disease, and provide real-time connectivity and feedback that may supplant conventional medical monitoring. Wearables have the potential to become truly disruptive in our health care sector, with large segments of the population soon to have readily available health data that the physician must interpret.
