Abstract:Background Refractory system lupus erythematosus (SLE) still relies on aggressive treatment with high-dose glucocorticoids and immunosuppressive agents,but a significant proportion of patients persist with activity or relapse and show serious side-effects by the treatment.However, the imbalance of Th17 cells and regulatory T (Treg) cells in peripheral blood of patients with SLE may be an important factor in the pathogenesis of SLE.Because low-dose IL-2 can selectively enhance Treg function while avoiding the activation of effector T cells and rapamycin can promote the proliferation of Treg cells1–2,the combination of the low-dose IL-2 and rapamycin has been considered to treat refractory SLE for the purpose of remission. Objectives To observe the effect of low-dose IL-2 combined with rapamycin on the balance of Th17/Treg cells in patients with refractory SLE. Methods Eighty-two refractory SLE patients (80 women and 2 men),with a mean duration of 72.41±37.52 months and mean age of 36.22±12.48 years, were enrolled.They are in line with the standard of ACR in 1997,who are treated with glucocorticoid and immunosuppressant for more than one year, but the subjects continues to rise to a peak.After the eligible patients are given IL-2 and rapamycin in combination therapy with conventional therapy at 0, 6, 12, 24 week respectively after medication.The clinical symptoms, blood routine, urine routine, ESR, Th17 cells, Treg cells, Th17/Treg cells and the dosage of corticosteroids and immunosuppressant are registered one by one. Results At 24 week after treatment,28.9% patients with refractory SLE were relieved.Low-dose IL-2 combined with rapamycin leaded to an increase in the absolute counts of Treg cells in refractory SLE patients,from a median of 12.98 cells/ul (at week 0) to 22.1 cells/ul (at week 24) (P=0.002).The ratio of Th17/Treg cells shows a reduction from a median of 0.44 at week 0 to 0.29 at week 24 (P=0.029).No significant difference was observed in the absolute counts of Th17 after combined treatment.At week 24, the mean dosage of prednisone which refractory SLE patients were receiving decreased from 17.20 mg/d to 8.87 mg/d.And the categories of DMARDs use were also reduced (P<0.05). Conclusions Our results suggest that refractory SLE is major associated with the decreased number of Treg but not that of Th17.The Th17 and Treg cells in the peripheral blood of patients with refractory SLE tends to balance due to the significantly increase the number of Treg cells after low-dose IL-2 combined with rapamycin treatment.Low-dose IL-2 combined with rapamycin treatment can reduce the dosage of glucocorticoid and DMARDs. Abstract:Background The imbalance of T help 17 cells (Th17)/regulatory T cells (Tregs) is considered to be a pivotal cause of autoimmune diseases1, including systemic lupus erythematosus (SLE). However, previous reports1,2 describing the respective changes of Tregs and Th17 cells in SLE patients were controversial because a few samples or diverse markers were used to identify Tregs with little consensus. Objectives To clarify the status of Tregs and Th17 in SLE, we investigated the frequencies of Tregs and Th17 cells on a large scale and whether those defects can be corrected by the supplementation of low-dose human recombinant interleukin-2 (IL-2). Methods Two hundred and thirty-five SLE patients (219 women and 16 men), with mean age of 37.80±14.00 years, were enrolled. The disease activity using European League Against Rheumatism (EULAR) criteria was judged for SLE patients with erythrocyte sedimentation rate (ESR) and SLEDAI scores. The frequencies of CD3+CD4+FOXP3+Treg cells and Th17 cells in peripheral blood from these patients were measured by flow cytometry. And low-dose IL-2 was used among 127 patients at a dosage of fifty WIU every day for five days. Immunological and clinical assessments were performed again at the end of IL-2 treatment. Ninety healthy volunteers, matched for patients9 age and gender, were also included for the estimation of CD4+ T cell subsets. Results As compared to healthy controls (median of Treg cells: 33.09 cells/ul), the frequencies of circulating CD4+CD25+FOXP3+Treg cells were significantly decreased in SLE patients (median: 15.49 cells/ul, P<0.001). The median ratios of Th17/Tregs cells in patients were greatly higher than those of healthy volunteers [0.42 (0.19, 0.88) vs. 0.21 (0.15, 0.34), P<0.001]. There was not significantly different in circulating Th17 cell between two groups. Moreover, CD4+CD25+FOXP3+Treg cells were negatively correlated with ESR and SLEDAI score (r=-0.198, P=0.01; r=-0.25, P=0.002).While no obvious correlation was seen between Th17 cells and SLEDAI score. After IL-2 therapy in SLE, there was a four-fold increase in circulating CD4+CD25+FOXP3+Treg cells [43.73 (24.08,74.22) vs. 11.95 (7.51,20.34),P<0.001], whereas Th17 cells were increased slightly. The ratio of Th17/Tregs was decreased significantly in patients with IL-2 treatment [0.19 (0.09,0.41) vs. 0.52 (0.23,0.95),P<0.001], tended to balance and had no difference with healthy individual (P=0.275). Conclusions With the increase of disease activity,CD4+CD25+FOXP3+Treg cells were gradually reducing, while Th17 cells did not show a significant change, indicating that the reduction of Tregs but not the elevation of Th17 cells may be the major reason for imbalance of Th17/Tregs. It is speculated that SLE is an autoimmune disease triggered by the defect of immunotolerance. More importantly, low-dose IL-2 selectively modulated the abundance of Tregs, which effectively induced autoimmune tolerance and further improved clinical symptoms. Sustained Low‐dose Interleukin‐2 Therapy Alleviates Pathogenic Humoral Immunity Via Elevating the Tfr/Tfh Ratio in Lupus

Abstract:AbstractObjectivesLow‐dose interleukin‐2 (IL‐2) has shown promising clinical benefits in the treatment of systemic lupus erythematosus (SLE), but how this therapy alleviates pathogenic humoral immunity remains not well understood. The dilemma is that IL‐2 can suppress both follicular helper and regulatory T (Tfh and Tfr) cells, which counteract each other in regulating autoantibody production.MethodsFemale NZB/W F1 mice received recombinant human IL‐2 (3 × 104 IU/dose) in three treatment regimens: (1) short, daily for 7 days; (2) medium, daily for 14 days, and (3) long, every second day for 28 days. Tfh (Foxp3−CXCR5+Bcl6+), Tfr (Foxp3+CXCR5+Bcl6+), germinal centre (GC, B220+GL‐7+Fas+) and antibody‐secreting cell (ASC, B220−CD138+TACI+) were analysed by flow cytometry. Serum anti‐dsDNA level was determined by ELISA. Kidney pathology was evaluated by H&E and immunofluorescence staining. Circulating Tfh and Tfr cells in SLE patients treated with low‐dose IL‐2 from a previous clinical trial (NCT02084238) was analysed.ResultsLow‐dose IL‐2 treatment consistently increased Tfr/Tfh ratio in mice and SLE patients, because of a stronger suppression on Tfh cells than Tfr cells. Three treatment regimens revealed distinct immunological features. Tfh suppression was observed in all regimens, but Tfr suppression and GC reduction were recorded in just medium and long regimens. Only the long treatment regimen resulted in inhibited ASC differentiation in spleen, which was accompanied by reduced anti‐dsDNA titres and ameliorated kidney pathology.ConclusionLow‐dose IL‐2 therapy increases the Tfr/Tfh ratio, and a less frequent and prolonged treatment can alleviate pathogenic humoral immunity and improve renal function.

Abstract:Treatment of lupus nephritis (LN) with cyclophosphamide (CYC) is effective but retains a certain severe adverse effect. Tacrolimus (TAC) may be a suitable treatment for LN. Forty patients with diffuse proliferative or membranous LN were recruited for this non-randomized open-label study - 67.5% (27/40) had nephrotic proteinuria (>3.5 g/day) and 50.0% (20/40) had low estimated glomerular filtration rate (eGFR) (<60 mL/min/1.73m(2)). We compared the efficacy and adverse effects of TAC (0.04-0.08 mg/kg/d)/prednisone for 12 months (TAC group, n = 20) with intravenous CYC (750 mg/m(2) per month)/prednisone for six months followed by azathioprine (Aza) (100 mg/day)/prednisone for six months (CYC group, n = 20). The TAC target concentration was 6-8 ng/mL or 4-6 ng/mL, respectively, when induction or maintenance therapy was required and 4.0 ng/mL for patient with renal insufficiency. In the TAC group, mean urinary protein excretion decreased significantly from 5.00 ± 1.91 g/day at baseline to 2.54 ± 1.68 g/day after two weeks of therapy (P < 0.001), compared with the CYC group (4.9 ± 19.4 g/day), P = 0.001, and 65.0% (13/20) achieved partial remission at one month, compared with the CYC group (0/20), P < 0.001. The incidence of complete remission (CR) was significantly higher in the TAC group than in the CYC group (55.0% vs.15.0% by five months, P = 0.008, and 75.0% vs.40.0% by 12 months, P = 0.025, respectively). The significant improvement in serum anti-dsDNA and systemic lupus erythematosus (SLE) disease activity index (DAI) was in the TAC group relative to the CYC group at 12 months (P = 0.031, P = 0.003, respectively). The eGFR improved in the TAC group from 59.90 ± 23.64 mL/min/1.73m(2) at baseline to 93.75 ± 28.52 mL/min/1.73m(2) after 12 months, P = 0.001. In the CYC group, two patients developed end-stage renal disease (ESRD), three patients experienced serious pneumonia, and one patient died. Our preliminary study showed TAC is a safe and effective treatment for LN with severe renal disease, and with less-severe adverse events compared with CYC followed Aza therapy. Further larger sample studies are needed to confirm our conclusion.

Abstract:Objectives Mesenchymal stromal cells (MSCs) and low-dose interleukin-2 (IL-2) both have demonstrated efficacy in treating systemic lupus erythematosus (SLE). The aim of this study is to conduct a head-to-head comparison between the 2 treatments and provide insights for clinical applications. Methods Lupus-prone mice were treated with umbilical cord-derived MSCs (UC-MSCs), IL-2, or a combination of UC-MSCs and IL-2, respectively. The lupus-like symptoms, renal pathology, and T-cell response were assessed 1 or 4 weeks later. Modulation of IL-2 production by MSCs on immune cells was investigated by the coculture assay. Disease activity and serum IL-2 of SLE patients were determined before and after receiving UC-MSCs. Results Both UC-MSCs and IL-2 improved lupus symptoms in lupus-prone mice 1 week after treatment, while the effects of UC-MSCs lasted up to 4 weeks. Moreover, the UC-MSC-treated group showed better renal pathology improvement. Importantly, UC-MSCs combined with IL-2 did not provide better efficacy than UC-MSCs alone. Consistent with this, UC-MSCs alone and UC-MSCs + IL-2 resulted in similar levels of serum IL-2 and frequencies of Tregs. Neutralization of IL-2 partly reduced the promotion of Tregs by UC-MSCs, suggesting that IL-2 was involved in the upregulation of Tregs by UC-MSCs. Lastly, an increase in serum IL-2 positively correlated with the reduction of disease activity of SLE patients by UC-MSCs. Conclusion Both the single injection of UC-MSCs and repeated IL-2 administration exerted comparable efficacy in alleviating SLE manifestations, but UC-MSCs provided sustained alleviation and showed better improvement in renal pathology.

Abstract:Background mTOR signaling is proved to be one of the most important pathway in the pathogenesis in SLE. However, in patients with SLE, whether mTOR pathway can be activated by low-dose IL-2 remained unclear. Objectives To clarify the effects of low-dose IL-2 therapy on mTOR signaling in the treatment of SLE. Methods Eight patients with active SLE were treated with 1 million IU IL-2. Phophrylation of S6 ribosomal protein (S6RP), AKT and pSTAT5 were measured before and after the first 2 week of low-dose rhIL-2 administration. C57BL/6 mice (male, 8–12 weeks old) were intraperitoneally immunized with SRB and followed by administration of different doses (low:10,000 IU and hight:300,000 IU) of rhIL-2 or PBS from day 3 to day 9. The ratio of Th1, Th2, Tfh, Th17, Tfh and Treg as well as the level of S6RP, AKT and pSTAT5 were assayed by flow cytometry Results Low-dose IL-2 was efficient and well tolerated in active SLE, and was associated with expansion of Treg cells (p<0.001) and reductions of Tfh and Th17 cells (p≤0.001). No significant change of pS6RP and pAKT was observed. On the other hand, there was a signfciant induction of the activation of STAT5. In mouse studies, low-dose IL-2 inhibited the differentiation of Th17 cells and Tfh cells. Comparing with high dose IL-2 group, there was no significantly increased mTOR activity after low-dose IL-2 administration. Low-Dose Interleukin-2 As an Alternative Therapy for Refractory Lupus Nephritis

Abstract:Low-dose interleukin-2 (IL-2) selectively restores disturbances of regulatory T cells (Treg) and conventional T cells, resulting in the induction of remission in patients with systemic lupus erythematosus. However, to date no research has been carried out on the efficacy of low-dose IL-2 in the treatment of refractory lupus nephritis (LN). The aim of the study reported here was to investigate the renal response to low-dose IL-2 in patients with refractory LN. The study population comprised ten patients with refractory LN who failed to achieve complete response or who had relapsed while being treated with at least two conventional immunosuppressive agents. One treatment cycle consisted of IL-2 at a dose of 1 million IU administered subcutaneously every other day for 2 weeks followed by a 2-week break. All patients received three cycles of IL-2 and were then followed up for another 12 weeks without any increase in the dose of previous immunosuppressive agents and steroids. Of the ten patients enrolled in the study, seven (70%) achieved ≥ 50% improvement in proteinuria at 12 weeks after initiating treatment with IL-2. Median proteinuria was significantly reduced by 50.3% at week 12, from 1.83 (interquartile range [IQR] 1.23–3.21) g/24 h at baseline to 0.91 (IQR 0.52–1.60) g/24 h at 12 weeks (P = 0.005). This was accompanied by a 71% reduction in urine erythrocytes, from 64/µl (IQR 24–102/µl) at baseline to 18/µl (IQR 2–20/µl) at 12 weeks (P = 0.018). Anti-dsDNA was decreased from 27.9 (IQR 7.6–40.28) IU/ml at baseline to 14.1 (IQR 7.3–20.12) IU/ml (P = 0.021) at week 12, while complements C3 and C4 were slightly increased (P = 0.445, P = 0.241, respectively). A significant expansion of Treg cells, from 9.3% at baseline to 16.6% at 12 weeks, was also found (P < 0.05). No serious adverse events occurred during the treatment period. Low-dose IL-2 therapy may have a promising role in the treatment of refractory LN as an alternative and safe therapeutic approach. It may be used as multi-target combination therapy in clinical practice.

Abstract:The expansion of T follicular helper (Tfh) cells correlates with disease progression in human and murine systemic lupus erythematosus (SLE). Unfortunately, there are no therapies to deplete Tfh cells. Importantly, low-dose rIL-2-based immunotherapy shows potent immunosuppressive effects in SLE patients and lupus-prone mice, primarily attributed to the expansion of regulatory T cells (Tregs). However, IL-2 can also inhibit Tfh cell differentiation. In this study, we investigate the potential of low-dose rIL-2 to deplete Tfh cells and prevent autoantibody responses in SLE. Our data demonstrate that low-dose rIL-2 efficiently depletes autoreactive Tfh cells and prevents autoantibody responses in lupus-prone mice. Importantly, this immunosuppressive effect was independent of the presence of Tregs. The therapeutic potential of eliminating Tfh cells was confirmed by selectively deleting Tfh cells in lupus-prone mice. Our findings demonstrate the critical role of Tfh cells in promoting autoantibody responses and unveil, (to our knowledge), a novel Treg-independent immunosuppressive function of IL-2 in SLE.
Interleukin-2 Deficiency Associated with Renal Impairment in Systemic Lupus Erythematosus

Abstract:Impaired interleukin-2 (IL-2) production was reported in systemic lupus erythematosus (SLE). The aim of this study was to investigate the clinical relevance of serum IL-2 and therapeutic effects of recombinant IL-2 (rIL-2) in SLE, especially in lupus nephritis (LN). Decreased serum IL-2 was found in patients with active LN (P = 0.014) and correlated with 24-h urine protein excretion (r = -0.281, P = 0.026). Compared with LN patients with decreased levels of serum IL-2, patients with increased levels had better remission rate (P = 0.041). Furthermore, patients with exogenous low-dose IL-2 supplement demonstrated better improved nephritis and higher remission rate (55.56%, P = 0.058) than those with conventional therapy. In addition, the percentages of regulator T (Treg) cells expanded in LN patients with low-dose recombinant human IL-2 treatment (P = 0.007), especially in LN patients achieving remission (P = 0.010). IL-2 deficiency is associated with renal impairment that can be improved by exogenous IL-2 supplement.

Abstract:Background Systemic sclerosis (SSc) is a chronic inflammatory disease with complex pathogenesis. The role of regulatory T cells (Tregs) in the development of SSc has started being studied during the last decade with new aspects being disclosed continuously. Although there is a general agreement in the medical literature regarding the decreased functional capacity of circulating Tregs in SSc, the alteration of absolute number of Treg cells as well as orther CD4 +T lymphocyte subsets in SSc is still unclear. Objectives The aim of the present study was to explore the absolute number status of CD4 +T subsets in peripheral blood of patients with SSc using our modified flow cryometric method and investigate whether the imbalance of Th17 and Treg cells can be corrected by supplementing low dose interleukin −2 (IL-2). Methods The peripheral CD4 +T subsets from 54 patients with SSc and 30 healthy control subjects were analysed. The patients were divided into the untreated group (n=29) and treated group (n=25). The patients were also divided into group A (n=28) who only used low-dose glucocorticoids (GCS), and group B (n=26) who also used immunosuppressant cyclophosphamide (CYC), methotrexate (MTX), leflunomide (LEF) or mycophenolate mofetil (MMF). In addition, 21 patients from 54 patients, given a small dose of IL-2 (50WIU) treatment for a 5 day course, divided into pre-treatment group and post-treatment group. Directly using the results from flow cytometry combined with internal standard beads, absolute number of peripheral CD4 +T subsets from the subjects in each group were calculated. Results Although there were some changes among CD4 +T cell subsets in peripheral blood from these SSc patients, the major alteration was the reductions of Treg cell absolute number. Compared with the normal controls, the number of CD4+CD25+FOXP3+Treg cells were significantly decreased in all patients (p=0.001), in untreated group (p=0.029), in treated group (p=0.029), in group A (p=0.006) and in group B (p=0.004). The ratio of Th17/Treg in total patients (p=0.008) and group B (p=0.001) were significantly higher than that in normal control group, and the group B was significantly higher than the group A (p=0.032). Moreover, the number of Th17 cells in group B was significantly higher than that in group A (p=0.023). After IL-2 treatment, the absolute number of CD4+CD25+FOXP3+Treg cells (p 0.05). Since Th17 cells increases significantly lower than Treg cells, so their ratio decreases significantly (p=0.001) to get re-balance. Conclusions The absolute number of peripheral CD4 +CD25+FOXP3+Treg cells decreased in untreated patients, indicating that this reduction arising from the disease itself. Our findings suggest that SSc progression is associated with the absolute decrease of peripheral Treg cells. Although there was no statistically significant different, absolute number of the Treg cells trended to decrease in patients treated with immunosuppressive agents. The use of IL-2 can effectively up-regulate Treg cells as well as increase Th17 to a certain extent, which restores the balance of Th17/Treg cells, but the long-term effect needs further investigation. Lipocalin-2 Exacerbates Lupus Nephritis by Promoting Th1 Cell Differentiation

Abstract:Background Lipocalin-2 (LCN2) is an indicator of the severity of lupus nephritis (LN) and plays a pivotal role in immune responses, but it is not known if its effect on LN pathogenesis derives from regulating the immune imbalance of T lymphocyte subsets. Methods The expression of LCN2 in T cells and kidneys was assessed in renal biopsies from patients with LN. We investigated the relationship between LCN2 levels and development of LN and systemic illness by injecting anti-LCN2 antibodies into MRL/lprmice and analyzing pristane-treatedLCN2(-/-)mice. Results LCN2 is highly expressed in CD4(+)T cells and in renal tissues, and is associated with severe renal damage in patients with LN and in mice with experimental lupus. LCN2 promotes IFN-gamma overexpression in CD4(+)T cells through the IL-12/STAT4 pathway in an autocrine or paracrine manner. Both neutralization of LCN2 in MRL/lprmice and genetic depletion of LCN2 in pristane-induced lupus mice greatly ameliorate nephritis. The frequency and number of splenic and renal Th1 cells decrease in proportion to LN disease activity. Conversely, administration of LCN2 exacerbates the disease with significantly higher renal activity scores and increased numbers of Th1 cells. Conclusions LCN2 plays a crucial role in Th1 cell differentiation, and may present a potential therapeutic target for LN. Significance Statement Lipocalin-2 (LCN2) is an indicator of the severity of lupus nephritis (LN), but its exact role and the underlying mechanism of its association with the condition remain unclear. LCN2 is a key regulator of T helper type 1 (Th1) cell differentiation in the pathogenesis of LN, acting through the IL-12/signal transducer and activator of transcription 4 pathway in an autocrine or paracrine manner. Amelioration of nephritis in MRL/lprmice given LCN2-neutralizing antibodies and in pristane-treatedLCN2(-/-)mice, provides the first proof of concept that decreasing LCN2 protects against renal injury through dampening the Th1 response. LCN2 blockade may present a promising new strategy to attenuate LN.
Lactobacillus Acidophilus Supplementation Exerts A Synergistic Effect on Tacrolimus Efficacy by Restoring Th17/Treg Imbalance in Lupus-Prone Mice via the SIGNR3 Pathway

Abstract:Abstract Background: Systemic lupus erythematosus (SLE) is a systemic autoimmune disease characterised by tissue-binding autoantibodies and immune complexes. Tacrolimus (Tac), also known as FK506, is an immunosuppressant used in the treatment of lupus; however, it induces T-cell imbalances. Lactobacillus acidophilus (LA) is reported to have therapeutic efficacy in immune-mediated diseases via T-cell regulation. This study investigated whether a combination therapy of LA and Tac improves the therapeutic efficacy of Tac by ameliorating T-cell imbalance in an animal model of SLE. Eight-week-old MRL/Mp- Fas lpr (MRL/ lpr ) mice were orally administered with 5 mg/kg of Tac and/or 50 mg/kg of LA daily for 8 weeks. Caecal microbiota compositions, serum autoantibodies levels, the degree of proteinuria, histological changes in the kidney, and populations of various T-cell subsets in the spleen were analysed. Results: MRL/ lpr mice presented with significant gut microbiota imbalances, which were subsequently recovered by the combination treatment of Tac and LA. Double negative T-cells, a pathogenic subset of T-cells, in the peripheral blood and spleens of MRL/ lpr mice were significantly decreased by the combination therapy. The combination treatment also reduced serum levels of anti-double-stranded DNA antibodies and immunoglobulin G2a, and renal pathology scores were markedly alleviated. The combination therapy induced Treg cells and decreased Th17 cells both in vitro and in vivo . In vitro treatment with LA induced the production of indoleamine-2,3-dioxygenase, programmed death-ligand 1, and inerleukin-10, which was partially mediated by the induction of the specific intracellular adhesion molecule-3 grabbing non-integrin homolog-related 3 (SIGNR3) receptor. Conclusions: The present findings indicate that LA augments the therapeutic effect of Tac and restores Th17/Treg balance in a murine model of lupus. Accordingly, the combination treatment of Tac with LA could be a promising therapeutic candidate for lupus.

Abstract:Background The concept of Mixed Connective Tissue Disease (MCTD) was first proposed by Sharp and his colleagues in 1972.1 MCTD has overlapping features between at least two autoimmune diseases, such as SLE, SSc, PM/DM and RA. Regulatory T cells (Treg cells) play a critical role in the maintenance of immune tolerance and the regulation of immune responses. Treg cell dysfunction contributes to the initiation of pathogenesis and the maintenance of high disease activity in RA and SLE.2 Recently, some studies have indicated that the proportion of Treg cells was different from healthy controls in SSc patients.3 Therefore, MCTD may be associated with Treg cells. Objectives To explore the status of CD4+T cells in peripheral blood of patients with MCTD; to explore the effect of low dose interleukin -2 (IL-2) treatment on the balance of Treg cells and Th17 cells in patients with MCTD, and to observe the short-term curative effect. Methods CD4+T cells in the peripheral blood of 58 MCTD patients and 33 healthy controls were analyzed by flow cytometry. All of 58 cases were treated with standard therapy, including corticosteroids, NSAIDs, immunosuppressants, or combination therapy. Among them, 26 cases of MCTD without using immunosuppressant (CYC, MTX, LEF, MMF), were as untreated group, another 32 cases were as treated group. Then we compared the difference of CD4+T cells between two groups. In 58 patients with MCTD, a total of 27 patients were treated with low-dose IL-2 (50WIU) on the basis of standard treatment for 5 days. The difference of CD4+T cells before and after treatment with IL-2 was compared. Results The absolute count of CD4+CD25+FOXP3+Treg cells in peripheral blood of patients with MCTD was significantly lower than those in healthy controls. There was no significant difference in the absolute count of CD4+CD25+FOXP3+Treg cells in the peripheral blood between the treated group and the untreated group. The absolute count of CD4+CD25+FOXP3+Treg cells was significantly higher than those before treatment with IL-2. Conclusions The absolute count of CD4+CD25+FOXP3+Treg cells in peripheral blood of patients with MCTD were significantly reduced, which may be the mechanism of immune imbalance in MCTD patients. The decrease of the absolute count of CD4+CD25+FOXP3+Treg cells in peripheral blood of patients with MCTD was not associated with the use of immunosuppressive agents. Low dose and short-term IL-2 treatment may increase the absolute count of CD4+CD25+FOXP3+Treg cells in the peripheral blood in patients with MCTD, so as to restore the immune balance in patients with MCTD. Tacrolimus versus intravenous cyclophosphamide in the induction therapy of diffuse proliferative lupus nephritis

Abstract:Objective:To assess the efficacy and safety of tacrolimus (FK506) as the induction therapy in patients with diffuse proliferative lupus nephritis (LN) and to explore the dosing of FK506 and its suitable blood concentration. Methodology:Thirty-four female patients diagnosed class Ⅳ-G LN (according to ISN/ RPS 2003 classification criteria) were randomly assigned to either FK506 0.1 mg/(kg·d) for 6 months (n=17) or intravenous cyclophosphamide (IVC) 0.5-1.0 g/m~2 BSA monthly for 6 months (n=17) in addition to corticosteroids. The primary end point was complete remission (CR) at 6 months (defined as a value of proteinuria <0.4 g/24hr with normal urinary sediment, serum albumin ≥3.5 g/dl and a normal value of serum creatinine or no more than 15% above the base-line values). A secondary end point was partial remission (PR) and remission at 6 months. Results:1) Clinical efficacy: The remission occurred in 16 out of 17 patients (94.1%) in the FK506 group and 14 out of 17 patients (82.4%) in the IVC group. There were 64.7% in the FK506 group and 41.2% in the IVC group that achieved CR (P=0.303). The average time to reach PR in the FK506 group was significantly less than that in the IVC group (1.9±1.2 vs 3.2±1.8 months,P=0.034) while the time to reach CR was 4.0±1.3 and 5.0±1.2 months, respectively. The improvements in SLE-DAI, proteinuria, urine RBC, serum albumin, serum complement and anti-dsDNA were similar in both groups. 2) Dosage and trough blood level of FK506: The average dosage of FK506 was 0.08-0.09 mg/(kg[DK]·d) and trough blood level was 5.7-7.1 ng/ml. Those who achieved CR with high blood level of 8.1±3.9 ng/ml and those of PR were 5.2±2.7 ng/ml. 3 patients whose blood level less than 5.0 ng/ml also achieved CR. 3)Adverse effects: In the FK506 group, the adverse effects of infection, temporary GPT/GOT rise, leucopenia, irregular menstruation were lower than that in the IVC group. In contrast, elevated blood pressure, hyperglycemia, transient increase in SCr and alopecia were higher in the FK506 group with no statistics significance. Conclusion:FK506 was an effective immunosuppressor, which had quick response and less adverse effects, as the inducing treatment in patients with diffuse proliferative LN.

Abstract:When a fluticasone propionate (FP) diskhaler is used to administer inhaled corticosteroid, it has been reported that there is considerable drug residue remaining in the diskhaler after use. The internal structure of the diskhaler is complex, and it is possible that sufficient cleaning of the device is not achieved using the attached brush. In this study, the diskhaler cleaning method was examined using a patient questionnaire. In response to the question on cleaning, 56.3% of patients responded "Having done", and 66.7% responded to the question on the frequency of the cleaning, "When I use it". Furthermore, cleaning by a healthy volunteer was examined using Rotadisk for inhalation practice. When the group that did not perform cleaning was compared with the group that performed cleaning with the brush, the amount of the lactose adhesion was significantly lower in the cleaning group. When the no-cleaning group was compared with the group that shook off the excess residue from the tray and the main body of the diskhaler, the group that shook off the diskhaler components showed a significantly lower amount of lactose adhesion. It was confirmed that drug residue were able to accumulate, and the shaking off method appeared to have an effect equal to that of cleaning with the brush. It seems that providing patients with guidance not only about the method of inhaling with the diskhaler but also about cleaning of the device is an important area of pharmacy patient management.

Abstract:Background Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by loss of tolerance to nuclear self-antigens, production of pathogenic autoantibodies and damage to multiple organs. While corticosteroids and immunosuppressive agents have improved the outcome of patients, there remains a significant unmet need for safe and more effective treatments. Low-dose Interleukin-2 (IL-2) therapy has recently been shown effective to treat autoimmune disease. Objectives To assess the safety and efficacy of low-dose IL-2 therapy in active SLE. Methods We conducted a clinical trail on active SLE patients (NCT02084238). A total of 40 patients were enrolled. Patients with SLE Disease Activity Index (SLEDAI) scores ≥8 received three courses of low dose recombinant human IL-2 (1 million IU every second day for 2 weeks followed by a 2-week hiatus). The primary end point was the response rate at week 10. Both the safety and efficiency of IL-2 therapy were evaluated. Results Total 36 patients (36/40, 90%) achieved an SLE Responder Index (SRI) improvement at week 6. No patients demonstrated high grad adverse events; mild injection-site reaction was observed in 5 patients (5/40, 12.5%). Better response was seen in patients with skin involvement (erythema, photo sensibility, Rdynolds, vasculitis), hematologic abnormities (leukopenia, Thrombocytopenia and anemia) and disease-related fever. Patients showed the improvement of major laboratory indicators, including reduced anti-dsDNA autoantibody titres and 24-hour proteinuria, and increased levels of the complement proteins C3 and C4. Immunological analysis showed significant increase of Treg cells and decrease of effector helper T cells after the therapy. Conclusions Our results showed that low-dose IL-2 therapy in active SLE were safe and achieved satisfactory efficacy with increasing Treg and decreasing effector helper T cells. Abstract:Background Recent studies have shown that increased number of Th17 cells and decreased number of Treg cells in the peripheral blood contribute to ankylosing spondylitis (AS). However, current therapy for AS, including biological agents, immunosuppressant and glucocorticoid, can not correct the imbalance of Th17 and Regulatory T (Treg) cells in AS patients effectively. It has been reported that low dose IL-2 can selectively expand Treg cells and had a critical effect on homeostatic balance between the Th17 and Treg cells. Objectives The study is to explore the effect of low dose IL-2 therapy on the balance of Treg and Th17 cells in patients with AS and observe the efficiency and side effects of the therapy. Methods Seventeen patients, who met the 1984 modified New York criteria and had evidence of active inflammatory spondylitis (defined as Bath AS Disease Activity Index (BASDAI) score >4) despite of receiving standard therapy including glucocorticoid, immune-suppressants, biological agents or combination of them, were enrolled. These patients were not only given traditional treatment, but also injected subcutaneously low-dose IL-2 (50 WIU/day for 5 days). Clinical and laboratory indicators were compared before and after IL-2 treatment. The side effects were observed in the course of therapy. Results The number of Treg cells significantly increased after the treatment by 1 week (22.58±12.80 vs. 73.46±33.79, p<0.001). At the same time, there was a significantly decrease in the ratio of Th17/Treg cells (0.67±0.70 vs. 0.32±0.33, p=0.068). Besides, Th17 cells were also increased (12.83±9.24 vs. 19.26±13.24, p=0.054). Clinical manifestations were improved after the combination treatment of IL-2 and traditional drugs, especially BASDAI was decreased (4.57±0.61 vs. 1.98±0.83, P<0.001). No obvious adverse reactions were observed. Conclusions Low dose IL-2 therapy can restore and maintain the balance of Th17 and Treg cells in the active patients with AS. Manifestation improved after the combination therapy. The therapy is safe. Further research is needed to investigate long term benefits of low-dose IL-2 therapy. A Prospective Preliminary Study of Tacrolimus (FK506) Combined with Low-Dose Prednisone in Patients with Membranous Lupus Nephropathy

Abstract:Objective:Th2 cell and cytokine IL-10 play an important role in the pathogenesis of membranous lupus nephropathy. Tacrolimus (FK506), an inhibitor of calcineurin, can inhibit the synthesis of IL-10 potently. In this study, we investigated the efficacy and adverse effects of tacrolimus in patients with membranous lupus nephropathy (WHO classification, class Ⅴ lupus nephritis). Methodology:A prospective preliminary study was conducted in 11 patients with membranous lupus nephritis proven by renal biopsy. In all the patients, the daily excretion of urinary protein exceeded 2.0 g, and the serum creatinine (SCr) was less than up-lower limit of normal range (110 μmol/L). All the patients received a regimen of FK506 combined with steroids. The starting dosage of FK506 was 0.1 mg/(kg·d), combined with prednisone at 0.6 mg/(kg·d). The dosage of prednisone was tapered after 4 weeks until a maintenance dosage of 10mg/d. Complete remission (CR) referred to daily urinary protein excretion reduced to 0.4g or below,no active urinary sediment (urinary RBC10×10 4/ml, no cast nor urinary WBC), serum albumin over 35g/L, SCr in normal range, and negativity of serum anti-dsDNA antibody. Partial remission (PR) referred to daily urinary protein excretion reduced to a range of 0.4-2.0 g and the reduction exceeded 50% or more of the baseline level, serum albumin equal or more than 30g/L, and SCr level in normal range. No-response (NR) referred to daily urinary protein excretion still exceeded 2.0g or the reduction less than 50% of the baseline value, serum albumin below 30g/L, or increment of SCr to more than 50% of the baseline value, or transformation to Class Ⅳ LN in repeat renal biopsy. Results:All the patients tolerated well to the regimen of FK506 for 3-15months, with nobody withdrew from the study. After treatment for 3 months, 2 of the 11 patients (18.2%) achieved complete remission, 5 patients (45.5%) achieved partial remission. In the 10 patients treated with FK506 for more than 6 months, 4 patients (40%) got CR and 4 patients got PR (40%) respectively. The median time to achieve CR/PR was 3 months (1-7months). In 7 of the 10 patients with hypoalbuminemia, serum albumin increased to normal range after treatment for 3 months. The titer of serum ANA and anti-dsDNA decreased,and the level of serum complement component increased significantly after treatment. Repeated renal biopsy in 8 patients (3 patients with CR, 3 with PR and 2 with NR) demonstrated that the activity index decreased from 4.8±2.38 to 2.25±1.28 (p0.05). There was no significant exacerbation of interstitial fibrosis. The adverse effects included temporary increased SCr level in 6 patients, and diarrhea, hypertension, herpes zoster each in 1 patient respectively. Conclusion:This preliminary study demonstrated that the regime of FK506 combined with low-dose prednisone is effective for patients with membranous lupus nephropathy.

Abstract:Background: Treatment of lupus nephritis (LN) remains challenging.Objective: To assess the efficacy and safety of a multitarget therapy consisting of tacrolimus, mycophenolate mofetil, and steroid compared with intravenous cyclophosphamide and steroid as induction therapy for LN.Design: 24-week randomized, open-label, multicenter study. (ClinicalTrials.gov: NCT00876616)Setting: 26 renal centers in China.Patients: Adults (aged 18 to 65 years) with biopsy-proven LN.Intervention: Tacrolimus, 4 mg/d, and mycophenolate mofetil, 1.0 g/d, versus intravenous cyclophosphamide with a starting dose of 0.75 (adjusted to 0.5 to 1.0) g/m(2) of body surface area every 4 weeks for 6 months. Both groups received 3 days of pulse methylprednisolone followed by a tapering course of oral prednisone therapy.Measurements: The primary end point was complete remission at 24 weeks. Secondary end points included overall response (complete and partial remission), time to overall response, and adverse events.Results: After 24 weeks of therapy, more patients in the multitarget group (45.9%) than in the intravenous cyclophosphamide group (25.6%) showed complete remission (difference, 20.3 percentage points [95% CI, 10.0 to 30.6 percentage points]; P < 0.001). The overall response incidence was higher in the multitarget group than in the intravenous cyclophosphamide group (83.5% vs. 63.0%; difference, 20.4 percentage points [CI, 10.3 to 30.6 percentage points]; P < 0.001), and the median time to overall response was shorter in the multitarget group (difference, -4.1 weeks [CI, -7.9 to -2.1 weeks]). Incidence of adverse events did not differ between the multitarget and intravenous cyclophosphamide groups (50.3% [91 of 181] vs. 52.5% [95 of 181]).Limitation: The study was limited to 24 weeks of follow-up.Conclusion: Multitarget therapy provides superior efficacy compared with intravenous cyclophosphamide as induction therapy for LN.

Methods Fifty-eight DMARDs-naive RA patients were enrolled, thirty-eight were treated with Rapamycin (0.5 mg every 2 days, combined with IL-2 50WIU per day for 5 days), the others with Methotrexate (10mg per week) taken as control. Clinical improvement and immunological assessments were performed at baseline, 1 and 12 weeks. Treatment group assessed CD4+ T cell subsets by flow cytometry at baseline, 1 and 12 weeks. Results We enrolled 58 patients. At baseline, patients had a mean DAS28 of 3.34 (0.81). Rapamycin group and Methotrexate group included 38 and 20 patients, respectively, with no significant differences in baseline characteristics. At 1 week, the mean DAS28 after Rapamycin treatment (2.43 [0.77]) and Methotrexate (2.25 [0.86]) was not significantly different (P=0.43). Same as ESR (24.74 [24.53], 21.76 [24.27], P=0.66). The dose of glucocorticoid during hospitalization of rapamycin treatment group (720.8 [554.3]) was lower than Methotrexate (1202.3 [943.1], P=0.042). The length of hospital stay of Rapamycin (14.5 [3.9]) was lower than Methotrexate (21.0 [3.8], P Conclusions Rapamycin combined with the low-dose IL-2 appears to be a safe and effective therapy for RA, by a rapid increase of circulating Treg cells and a correction of the ratio of Th17/Treg cells, which has gotten a same response compared with Methotrexate.
