Abstract:Background: Both sepsis-induced cardiomyopathy and worsening of preexisting cardiac disease can contribute to circulatory shock in septic patients. The early use of pulmonary artery catheter (PAC) could play a pivotal role in the management of sepsis-associated cardiogenic shock. In this study, we aimed to evaluate the impact of early invasive hemodynamic monitoring with PAC in patients with sepsis-associated cardiogenic shock. Method: We performed a retrospective study using the National Inpatient Sample data from January 2017 to December 2019. The early use of PAC was defined as the use of PAC within 2 days from the admission. We performed the multivariable logistic regression analysis to investigate the association between the early use of PAC and in-hospital mortality in patients with sepsis-associated cardiogenic shock and sepsis without cardiogenic shock, respectively. Results: There was no difference in in-hospital mortality between PAC and no PAC groups in sepsis without cardiogenic shock (adjusted odds ratio [aOR] = 1.05, 95% confidence interval [CI] = 0.82-1.35, P = 691). On the other hand, the early use of PAC was independently associated with lower in-hospital mortality in patients with sepsis-associated cardiogenic shock (aOR = 0.58, 95% confidence interval [CI] = 0.46-0.72, P < 0.001). The use of PAC was also associated with increased use of mechanical circulatory support in those with sepsis-associated cardiogenic shock (aOR = 12.26, 95% CI = 9.37-16.03, P < 0.001). For patients with sepsis-associated cardiogenic shock, the use of PAC after 2 days of admission was associated with significantly higher in-hospital mortality and decreased use of mechanical circulatory support. Conclusion: The use of pulmonary artery catheters in sepsis-associated cardiogenic shock was associated with significantly lower in-hospital mortality and increased use of mechanical circulatory supports in patients with sepsis-associated cardiogenic shock.

Abstract:Objective: To investigate the safety, efficacy and effects of emergent percutaneous coronary intervention (PCI) in patients from Second Affiliated Hospital, Medical College, Zhejiang University with cardiogenic shock (CS) complicating acute myocardial infarction (AMI). Method: Twenty-seven patients with CS complicating with AMI were treated by PCI with intraaortic balloon counterpulsation (IABP) support. The change of hemodynamics before and after IABP and PCI, the characteristics of PCI, the mortality during hospitalization, the major adverse cardiac events (MACE) and left ventricular ejection fraction at 30 - day follow - up were observed. Results: The hemodynamics were significantly improved after IABP. No patients died during PCI. Two patients died after PCI and the total mortality was 7.4% in hospital. During the period of 30 - day follow - up, one patient died of heart failure. The left ventricular ejection fraction greatly improved at 30 days after PCI. Conclusions: The data suggested that the use of IABP during PCI in patients with CS complicating AMI was safe, decreased mortality and improved prognosis.
Current Use and Impact of Pulmonary Artery Catheters on the Short-Term Outcomes in Patients With Cardiogenic Shock Treated With an Impella: Findings From the Japan Registry for Percutaneous Ventricular Assist Device (J-PVAD)

Abstract:Objectives: This study aimed to investigate the current use and impact of pulmonary artery catheters (PACs) in patients with cardiogenic shock (CS) who underwent Impella support. Design: This was a prospective multicenter observational study between January 2020 and December 2021 that registered all patients with drug-refractory acute heart failure and in whom the placement of an Impella 2.5, CP, or 5.0 pump was attempted or successful in Japan. Setting: Cardiac ICUs in Japan. Patients: Between January 2020 and December 2021, a total of 3112 patients treated with an Impella were prospectively enrolled in the Japan registry for percutaneous ventricular assist device (J-PVAD). Among them, 2063 patients with CS were divided into two groups according to the PAC use. Interventions: None. Measurements and Main Results: The primary endpoint was the 30-day mortality, and the secondary endpoints were hemolysis, acute kidney injury, sepsis, major bleeding unrelated to the Impella, and ventricular arrhythmias within 30 days. PACs were used in 1358 patients (65.8%) who underwent an Impella implantation. The use of venoarterial extracorporeal membrane oxygenation (VA-ECMO) was significantly higher in the patients with PACs than in those without. Factors associated with PAC use were the prevalence of hypertension, out-of-hospital cardiac arrest, New York Heart Association classification IV, the lesser prevalence of a heart rate less than 50, and the use of any catecholamine. The primary and secondary endpoints did not significantly differ according to the PAC use. Focusing on the patients with VA-ECMO use, the 30-day mortality and hemolysis were univariately lower in the patients with PACs. Conclusions: The J-PVAD findings indicated that PAC use did not have a significant impact on the short-term outcomes in CS patients undergoing Impella support. Further prospective studies are required to explore the clinical implications of PAC-guided intensive treatment strategies in these patients.

Abstract:Abstract Aims The real‐world usage of intra‐aortic balloon pump (IABP) in various cardiogenic shocks (CS) and the association with outcomes are lacking. We aimed to investigate IABP adoption in CS in a nationwide registry in China. Methods and results We retrospectively retrieved data of 30 106 CS patients (age 67.1 ± 14.6 years, 37.6% female patients) in the Hospital Quality Monitoring System registry from 2013 to 2016. Ischaemic heart disease was the leading cause of CS (73.9%). Hypertension, cardiomyopathy, myocarditis, valvular, and congenital heart disease were seen in 36.0%, 7.5%, 2.6%, 7.3%, and 2.4% of the population. IABP was employed in 2320 (7.7%) subjects. The association between IABP usage and primary outcome of in‐hospital mortality and secondary outcomes of expenses and lengths of stay were investigated. The patients with IABP support had similar in‐hospital mortality to those without IABP (39.6% vs. 38.3%, P = 0.226), but longer hospital‐stay [8.0 (2.0–16.0) vs. 6.0 (2.0–13.0) days, P < 0.001] and higher expenses [7.1(4.4–11.1) vs. 2.3 (0.8–5.5) 10 000RMB, P < 0.001]. IABP support was not associated with reduced mortality in the overall CS population in multivariate regression analysis [odds ratio (OR) 1.05, 95% confidence interval (CI) 0.95–1.17], except for subgroups with myocarditis (OR 0.61, 95% CI 0.39–0.95, P for interaction = 0.010) and those who did not receive the early percutaneous coronary intervention (PCI) (OR 0.86, 95% CI 0.75–0.97, P for interaction < 0.001). Similar results were further confirmed in the propensity‐score‐matched population. Conclusions In this nationwide registry of CS patients, IABP was not noted with improved survival but increased healthcare consumption. However, IABP appears protective in those with myocarditis or who failed to receive early PCI.
[The Application of Pulmonary Artery Catheterization in Treatment of Septic Shock Patients].

Abstract:OBJECTIVE:To compare the differences of hemodynamic pattern and metabolic alteration between the survivors and nonsurvivors of septic shock so as to explore the value of pulmonary artery catheterization (PAC).METHODS:A total of 70 patients with septic shock in whom PAC was carried out after initial resuscitation were retrospectively studied during a 5-year sequential period from 2001 to 2006 in a 15-bed general intensive care unit of a teaching hospital in Beijing, China. Hemodynamic data, lactate concentration and APACHEII score were obtained at the beginning and 24 hours later of PAC after the initial resuscitation period. The patients' outcome were recorded at the 28th day of hospitalization after diagnosis.RESULTS:The overall mortality rate was 60%. There were no age and sex differences between the survivors and nonsurvivors. The nonsurvivor group had higher initial heart rate, APACHEII score and lactate concentration than the survivor group. After 24 hours, mean arterial pressure (MAP) was significantly lower, but central venous pressure (CVP), pulmonary arterial wedge pressure (PAWP), lactate concentration and APACHEII score of were significantly higher in the nonsurvivor group than those in the survivor group. There were no differences of the other hemodynamic date between the two groups in the beginning and 24 hours later. More patients received superhigh dosage of norepinephrine or dopamine in the nonsurvivor group. Achievement of superhigh concentration of oxygen delivery or early-goal directed therapy (EGDT) indices had no beneficial effect on survival. Multiple stepwise linear regression analysis showed 24-hour MAP, PAWP and initial heart rate were independent predictors of the outcome.CONCLUSION:PAC can not display the differences of hemodynamic patterns between the survivors and nonsurvivors of septic shock after initial resuscitation, but 24 hours later nonsurvivors had higher PAWP significantly. Whether there are differences of hemodynamic pattern between the survivors and nonsurvivors was not the basis of PAC. The value of PAC monitoring is to balance the relationship between the requirement of hyperdynamic circulation and cardiac suppression state.

Abstract:Introduction Intra-aortic balloon pump (IABP) has been used since 1960s. Randomized controlled trials demonstrated neither benefit nor harm with IABP use in cardiogenic shock complicating acute coronary syndrome (ACS-CS). However, the clinical trajectory of non-ischemic cardiogenic shock (NI-CS) differs significantly from ACS-CS. There has been increasing interest in the use of IABP to stabilize patients with NI-CS. We aimed to study trends in outcomes of IABP use in NI-CS. Method This is a retrospective cohort analysis using 2016 to 2020 National Inpatient Sample. The International Classification of Diseases, Tenth Revision (ICD- 10) Clinical Modification codes and Procedure codes were used to identify adult patients (equal or >18 years) with NI-CS who were managed with IABP solely. We excluded patients with STEMI, NSTEMI and any revascularization procedures during their hospitalization as well as patients that received other percutaneous ventricular assist devices including Impella or extracorporeal membrane oxygenation. Pearson Chi-square and t-test were used for data analysis. Results We identified 5,533 hospitalizations with NI-CS that were managed with IABP solely during the study period. The rate of admissions for NI-CS managed with IABP increased from 17.9% in 2016 to 21.5% in 2020, p trend<0.001. Mean age was 64 ±14 years. The majority were men (66%). Racial distributions were White (66%), Black (16%), Hispanic (9%) and Asian (4%). The 3,689 (67%) hospitalizations had a Charlson comorbidity index (CCI) of equal or >3. The most common comorbidities found were chronic heart failure (76%), hypertension (62%), atrial fibrillation (42%), chronic kidney disease (40%) and diabetic mellitus (40%). Other comorbidities were chronic pulmonary disease (16%), obstructive sleep apnea (11%) and pulmonary hypertension (7%). Most NI-CS admissions received IABP at urban teaching hospitals, compared to rural hospitals (84% vs 3%, p<0.001). The in-hospital mortality rates decreased from 31.1% in 2016 to 21.7% in 2020, p trend<0.001, however, the overall mortality was still remarkably high (26%). There was no statistically significant difference in trends of length of stay (14 days in 2016 vs 16 days in 2020, p =0.194). Conclusion During the study period, there was decreasing trends in in-hospital mortality despite increased hospitalizations with NI-CS that were managed with IABP solely. Further studies are needed to evaluate the role of IABP use in NI-CS.

Abstract:Objective: It is still uncertain what effects pulmonary artery catheter (PAC)-guided resuscitation has on outcomes for patients with severe acute pancreatitis (SAP). Therefore, we aimed to investigate the effect of PAC on hospital mortality in patients with SAP. Methods: We collected the data of patients with a diagnosis of SAP from January 10, 2017, to July 30, 2019. Patients were divided into a PAC group and a control group. The primary outcome measured was the day-28 mortality. Secondary outcomes included day-90 mortality, duration of ICU and hospital stay, ventilation days, usage of renal support and vasoactive agents, incidences of acute abdominal compartment syndrome, infusion volumes, and fluid balance and hemodynamic characteristics measured by the PAC. Kaplan-Meier analysis was applied to estimate survival outcomes. Complications related to PAC were also analyzed. Results: There was no significant difference between the PAC group and the control group for day-28 mortality (22.7% vs. 30%, odds ratio, 0.69; 95% CI 0.31-1.52; P = 0.35). The duration of ICU stay in the PAC group was shorter (P = 0.00), and the rate of dependence on renal support treatment was lower in the PAC group than in the control group (P = 0.03). There was no difference in other secondary outcomes and no significant difference in the survival curve between the two groups (log-rank P = 0.72, X2 = 0.13). However, SAP patients inserted PAC within 24 h ICU admission showed that duration of renal support therapy in PAC patients within 24 h ICU admission (mean days, 1.60; standard deviation, 0.14) was shorter than those with 24-72 h ICU admission (mean days, 2.94; standard deviation, 0.73; P = 0.03). The organ failure rates (1 organ, 2 organs and 3 organs) were all lower in PAC patients within 24 h ICU admission than with 24-72 h ICU admission (P = 0.02, P = 0.02, P = 0.048, respectively). Conclusion: In patients with severe acute pancreatitis, PAC-guided fluid resuscitation shortened the duration of ICU stay, and patients in the PAC group had a lower rate of dependence on renal support, while no benefit in terms of mortality was observed. However, SAP patients inserted PAC within 24 h ICU admission showed shorter duration of renal support therapy and lower organ failure rates than those with 24-72 h ICU admission, indicating that early use of PAC, especially within 24 h, might be better for SAP patients.

Abstract:Background Management of cardiogenic shock (CS) poses a formidable challenge in healthcare, necessitating innovative approaches and collaborative efforts, not only local, to improve outcomes. Purpose To demonstrate that CSWG international cooperation and research in CS can lower mortality, change practices, and improve outcomes. Methods From our institutional registry 28,054 patients, we selected 9,430 with CS in SCAI CWSG-score B-E 2022-23 CSWG incorporation (n=1112) against previous years (05-21, n=8318). The primary outcome was in-hospital mortality. Univariate and multivariate analyses were performed, and with PAC use as a a priori subgroup. Adjusted analysis was carried out with age, sex, DM, HTN, previous MI, PCI, CABG, or stroke; CKD, CS etiology, PAC, hemodialysis, number of vasoactives; the presence of MCS, use of mechanical ventilation, and SCAI-CSWG stage. Also, propensity score matching was used to explore differences using the a priori selected variables using K-nearest neighbors (K=2), radius of 1:2, caliper 0.2, and exact SCAI stage matching without replacement (Pre-CSWG n=2198, CSWG era n=1106). Results In the CSWG era, there's a increase in mean age (P=0.049) and a higher proportion of men (72.2 vs. 68%, p=0.005). Clinical risk factors show changes, including increased weight (P=0.037), higher smoking (53.3 vs. 48.6%, P=0.003), diabetes (40.1 vs. 34.8%, P=0.001), and hypertension (46.8 vs. 50%, p=0.043). Prior MI and CABG decreased significantly in the CSWG era. Therapeutic shifts are evident, with increased use of PAC (7.9 vs. 4.2%, P<0.001) and significant changes in mechanical circulatory support (p<0.001). Mortality substantially decreases in the CSWG era (15.3 vs. 22.3%, P<0.001). The unadjusted Cox Pre-CSWG era, the HR 1.41 (1.2-1.65, P< 0.001) a restricted mean survival time (RSMT) of 1.73 days (0.92-2.54, P<0.001). In PAC subgroup, the relationship between PAC use and outcomes appears to be consistent across the levels of the interacting variable (Pint=0.918), with a HR Pre-CSWG era 0.51 (0.44-0.59, P <0.001) and CSWG era HR 0.48 (0.33-0.71,P <0.001)(Fig1C) The adjusted analysis, the Pre-CSWG era had a HR 1.21 (1.03-1.42, P=0.019). PAC outcomes were consistent across eras (Pint= 0.923). In the Pre-CSWG era HR 0.71 (0.59-0.84,P<0.001), and similarly in the CSWG era 0.54 (0.34-0.87,P=0.01). (Fig1D) The PS-cohort had a Pre-CSWG era HR 1.21 (1.01-1.45, P=0.034) of the Pre-CSWG era vs CSWG era and a RSMT at 30 days of 1.07 (0.14-2.01 P=0.025). (Fig2) Conclusion Shifts in patient demographics, clinical risk factors, hemodynamic parameters, and therapeutic strategies were observed. Notably, mortality significantly decreased with the CSWG incorporation, with consistent benefits of PAC use in both eras. The PS cohort underscored the impact of CSWG incorporation on outcomes. Abstract:Background: Cardiogenic shock remains a major cause of morbidity and mortality in patients with ST-segment elevation myocardial infarction. We aimed to assess the current trends in cardiogenic shock management, looking specifically at the incidence, use of intra-aortic balloon pump therapy and outcomes in patients with ST-segment elevation myocardial infarction treated with primary percutaneous coronary intervention. Methods and results: We undertook an observational cohort study of 21,210 ST-segment elevation myocardial infarction patients treated between 2005–2015 at the eight Heart Attack Centres in London, UK. Patients’ details were recorded at the time of the procedure into local databases using the British Cardiac Intervention Society percutaneous coronary intervention dataset. There were 1890 patients who presented with cardiogenic shock. The primary outcome was all-cause mortality at a median follow-up of 4.1 years (interquartile range: 2.2–5.8 years). Increasing rates of cardiogenic shock were seen over the course of the study with consistently high mortality rates of 45–70%. A total of 685 patients underwent intra-aortic balloon pump insertion during primary percutaneous coronary intervention for cardiogenic shock with decreasing rates over time. Those patients undergoing intra-aortic balloon pump therapy were younger, more likely to have poor left ventricular function and less likely to have had previous percutaneous coronary intervention compared to the control group. Procedural success rates were similar (86.0% vs 87.1%, p=0.292) although crude, in-hospital major adverse cardiac event rates were higher (43.8% vs 33.7%, p<0.0001) in patients undergoing intra-aortic balloon pump therapy. Kaplan-Meier analysis demonstrated significantly higher mortality rates in patients receiving intra-aortic balloon pump therapy (50.9% intra-aortic balloon pump vs 39.9% control, p<0.0001) during the follow-up period. After multivariate Cox analysis (hazard ratio 1.04, 95% confidence interval 0.62–1.89) and the use of propensity matching (hazard ratio: 1.29, 95% confidence interval: 0.68–1.45) intra-aortic balloon pump therapy was not associated with mortality. Conclusion: Cardiogenic shock treated by percutaneous coronary intervention is increasing in incidence and remains a condition associated with high mortality and limited treatment options. Intra-aortic balloon pump therapy was not associated with a long-term survival benefit in this cohort and may be associated with increased early morbidity.

Abstract:A number of studies with tracers in animals have been done to investigate the metabolic pathways by which carbon of fatty acid is converted to carbohydrate. Analysis of distribution of isotope in the carbon positions of glucose (or glycogen) formed in vivo from position-labeled precursors provides insight into the metabolic routes traversed (2, 3). Such studies have generally upheld the concept that fatty acids are degraded to two-carbon units, then incorporated into the tricarboxylic acid cycle, from which three-carbon monocarboxylic acids of the glycolytic pathway and finally hexose derivatives are formed. In conformity with this scheme are the findings that fatty acids labeled in the carboxyl or one of the other odd-numbered carbon positions form glucose or glycogen with isotope almost exclusively in the 3 and 4 carbons, while fatty acids labeled in one of the even-numbered carbon atoms introduce isotope primarily and equally into the 1, 2, 5, and 6 carbons (3, 4). In order to gather evidence bearing on the utilization of these fundamental intermediary pathways in the human being and to provide data for comparison with pathological states, such as diabetes (1, 5), studies of this type have been commenced by the analysis of distribution of radioisotope in blood glucose after the administration of trace amounts of 1-C14-acetate to four cancer patients with presumably normal carbohydrate metabolism.

Abstract:Introduction Acute myocardial infarction is a well-known cause of cardiogenic shock for which early revascularization has shown to improve outcomes, However, recent data has suggested that cardiogenic shock is predominantly due to non-ischemic etiologies including decompensated heart failure, cardiac tamponade, myocarditis, arrhythmias, myocardial contusion and valvular heart disease. We aimed to study the trends and prognostic outcomes of ischemic cardiogenic shock (ICS) vs non-ischemic cardiogenic shock (NICS) using national-level database. Methods National Inpatient Sample 2016-2020 was used to conduct a retrospective analysis by identifying patients admitted for cardiogenic chock using ICD-10 codes. These cardiogenic shock patients were then divided into two cohorts: ischemic vs non-ischemic etiology. Primary outcome was the prognostic implications of ICS vs NICS in terms of mortality, use of mechanical circulatory support devices, pulmonary pressure monitoring and vasopressors. Multivariate logistic regression model was used for analysis. Results Out of 854685 patients admitted for cardiogenic shock, 40% patients had ischemic etiology (n=341165), and 60% had non-ischemic etiology (n=513520). Trends for the prevalence of ICS and NICS have been essentially stable with slight decrease in the prevalence of ICS from 2016 to 2020 i.e., 42% to 38.5% with the corresponding increase in the prevalence of NICS i.e., 58% to 61.5%. In terms of patient demographics, mean age of patients with ICS was greater than that of NICS (68 vs 64 years). Male white patients had higher prevalence of both ICS and NICS. Majority of the hospitalizations for both ICS and NICS were in large urban teaching hospitals in the southern US. On adjusted outcome analysis with multivariate logistic regression, we observed that ICS was associated with higher odds of mortality (OR 1.1, CI 1.08-1.14, p<0.001), MCSD placement including IABP and ECMO (OR 5.01, CI 4.86-5.2, p<0.001), and cardiac arrest (OR 1.24, CI 1.2-1.3, p<0.001) as compared to NICS. While we also reported that NICS was associated with higher odds of LVAD placement (OR 4.6, CI 4-5.3, p<0.001), pulmonary artery (PA) catheter monitoring (OR 1.2, CI 1.1-1.24, p<0.01), and vasopressor use (OR 1.1, CI 1.03-1.12, p<0.001) as compared to ICS. Mean length of stay was higher for NICS as compared to ICS (12.7 vs 9.7 days) while the difference in adjusted total charges was not statistically significant (p>0.5) Conclusion As compared to ICS, NICS has higher prevalence and it is associated with higher odds of LVAD placement, PA catheter monitoring and vasopressor use. While, ICS is associated with higher mortality, MCSD placement including IABP and ECMO, cardiac arrest, and longer length of stay as compared to NICS.

Abstract:Objective To evaluate the therapeutic effect of intra-aortic balloon couterpulsation in percutaneous coronary in-tervention for patients with acute myocardial infarction combined with cardiogenic shock. Methods Fifty-two AMI patients combined with CS diagnosed were treated with a verdict by the IABP circulatory support emergency treatment,with the same period,48 patients of the same disease without IABP treatment as a control group to compare. Application of PCI treatment in both groups on the basis of conventional medication. Before and after IABP that were observed in patients with cardiac function,fatality after one week of PCI and the incidence of vascular re -occlusion. Results Neice radial artery diastolic pressure,MAP,CI,CO,SV and EF in the treatment group were significantly higher than that before operation ( P < 0. 05 or P < 0. 05),while PCWP was significantly lower than that before operation ( P < 0. 05),the mortality of treatment group was significantly lower than that in the control group ( P < 0. 05),the incidence rate of vessel re -occlusion in treatment group was significantly lower than that in the control group ( P < 0. 05). Conclusion IABP can significantly improve cardiac function and reduce mortality in patients with AMI complicated with CS and reduce the rate of vessel reocclusion.
Cardiogenic shock in general intensive care unit: a Nationwide prospective analysis of epidemiology and outcome

Abstract:Abstract Background Cardiogenic shock (CS) is a life-threatening disease burdened by a mortality up to 50%. The epidemiology has changed with non-ischemic aetiologies being predominant although data was mainly derived from patients admitted to dedicated acute cardiac care. We report the epidemiology and outcome of patients with CS admitted to general intensive care unit (ICU). Methods Prospective multicentric epidemiological study including 314 general ICU adhering to the GiViTI Nationwide registry from 2011 to 2018, excluding cardiac arrest. The primary endpoint of the study was mortality. The association between clinical factors and mortality was evaluated using a logistic regression model. The Odds Ratios of the covariates quantify their association with mortality during hospitalization. Results 11052 patients admitted to general ICU (incidence 2.17%; median age 72 (IQR [66-81]), 38.7% were women) with CS were included. Fourthy-seven percent of patients had more than 3 organ insufficiency at the time of admission. The most common CS aetiologies were: left heart failure LHF- 5247–47.5%), acute myocardial infarction (AMI - 3612–32.6%); right heart failure (RHF- 515–4.6%) and biventricular failure (532- 4.8%). 85.5% were mechanically ventilated during the ICU hospitalization. The overall ICU mortality was 44.8%, increasing to 53.4% during the hospitalization in the index hospital and to 54.3% at the latest hospital. RHF-CS patients exhibited the highest mortality risk (OR: 1.19 95% CI [0.94 − 1.50]; p < 0.001), followed by biventricular-CS OR 1.04 95% CI [0.82–1.32]. Respiratory failure (OR 1.13 [95%CI 1.08–1.19]), coagulation disorder (1.17 (95% CI 1.1–1.24), renal dysfunction (OR 1.55 [95% CI 1.50-1.61] and neurological alteration (OR 1.45 [95% CI 1.39–1.50]) were associated with worsen outcome along with severe hypotension (systolic blood pressure < 70 mmHg- OR 2.35 95% CI [2.06–2.67]), increasing age (OR 2.21 95% CI [2.01–2.42] and longer ICU stay prior to admission (2-fold increase for each 4.7 days). Conclusions In the general ICU the aetiology of CS, excluding cardiac arrest, remains characterized mostly by LHF with RHF-CS burdened by higher mortality. Multiorgan failure at admission and longer hospital stay before ICU admission predispose to worsen outcome.

Abstract:Background Cardiogenic shock (CS) has high mortality and involves complex decision-making. Palliative care consultation (PCC) clarifies goals of care, aids in symptom management, and provides psychosocial support. However, the utilization of PCC in CS patients is not well described. Methods A retrospective cross-sectional observational analysis was performed on patients admitted between 2019-2021 who met hemodynamic criteria for CS or required pharmacological or mechanical circulatory support. Patients who received VAD, cardiac transplant, or died within 24h of admission were excluded. Patients were grouped based on the presence or absence of PCC. Fisher's exact test was used to compare clinical and demographic variables and outcomes. Results Of 148 patients, 61 (41.2%) received PCC. The mean time to PCC was 21.7±89.0 days. There were no significant differences between groups in age, gender, or race; or comorbid history, including shock, HF, MI, atrial arrhythmias, PCI, CABG, ICD, or stroke. Patients receiving PCC were more likely to have longer hospitalizations (usual care =12.8±10.5 days vs PCC =17.9±13.6 days, p=0.011) and CKD (21.8% vs 39.3%, p=0.021). Patients receiving PCC were also more likely to be in cardiogenic shock from heart failure (40.2% vs 59.0%, p=0.024) as opposed to STEMI (p= 0.56) or NSTEMI (p= 0.13). There was no significant association between PCC and pharmacologic and/or mechanical circulatory support use, comfort-measures-only transition, mortality, PCI, or in-hospital cardiac arrest. There was also no association between PCC and hospital complications such as cardiac tamponade, limb ischemia, vascular complication requiring surgery, or stroke. Patients who were discharged on home inotrope (1.2% vs 27.9%, p<0.001) or hospice (0% vs 9.8%, p=0.0028) were more likely to receive PCC. Of patients who were discharged, those receiving PCC were significantly more likely to have had a cardiac rehospitalization at 1-year follow-up (18.4% vs 24.6%, p=0.021). However, there was not a significant mortality difference. Conclusions Patients with cardiogenic shock due to heart failure, a history of CKD, were being discharged on Hospice or home inotropes, or who were perceived at high risk for readmission were more likely to receive PCC. No differences were detected in demographics or pharmacologic and/or mechanical circulatory support between groups. Differences were not detected for some variables, including comfort-measures-only transition, mortality, and in-hospital complications, possibly due to small sample size or confounding variables.

Abstract:Introduction Epidemiology and outcomes of cardiogenic shock (CS) in patients with end stage renal disease (ESRD) have not been well studied. The objective of this study is to analyze temporal trends of incidence and outcomes of cardiogenic shock and utilization of percutaneous left ventricular assist device (pLVAD) in patients with ESRD. Methods We identified all hospitalized patients with CS and ESRD between January 2006 to December 2018 utilizing the United States Renal Data System (USRDS) database. We analyzed trends in the incidence, use of mechanical circulatory support, in-hospital mortality, and 1-year all-cause mortality. Results There were a total of 38,679 patients with ESRD admitted for CS during the study period. Mean age was 67.8 years (IQR; 59.4, 75.9) and 59% were men. The annual incidence of CS in patients with ESRD increased from 0.27% to 0.57% from 2006 to 2018 (Ptrend<0.001) (Figure 1A). There was no significant change in in-hospital mortality (53.91% in 2006 and 41.41% in 2018; Ptrend =0.38) and 1-year all-cause mortality (28.91% in 2006 and 35.13% in 2017; Ptrend =0.1) during the study period (Figure 1B). There was decrease in the use of intra-aortic balloon pump from 17.86 % in 2006 to 8.25% in 2018 (Ptrend < 0.001) (Figure 1C) and a significant increasing trend in the use of Impella from 0.14% in 2006 to 4.8% in 2018 (Ptrend <0.001) (Figure 1D). On multivariable regression analysis, age, peripheral vascular disease (OR: 1.13, 95% CI: 1.08 to 1.18; p<0.0001), diabetes mellitus (OR: 1.11, 95% CI: 1.06 to 1.16; p<0.0001) and duration on dialysis are independent predictors of in-hospital mortality. The risk of CS increases with increasing duration on dialysis: time on dialysis 2-5 years (OR: 1.26, 95% CI: 1.20 to 1.32; p=0.02), 6-10 years (OR: 1.4, 95% CI: 1.32 to 1.5; p=0.003), and ≥11 years (OR: 1.69, 95% CI: 1.53 to 1.86; p<0.0001). Conclusion The incidence of CS in patients with ESRD has doubled between 2006 and 2018. There has been significant decrease in use of IABP with concomitant increase in utilization of Impella during this period but there was no significant change in in-hospital and 1-year all-cause mortality.
