Abstract:Objective: To understand nursing team professionals' strategies to include the family in painful procedures performed on hospitalized children. Method: An exploratory-descriptive, qualitative study, carried out with nursing professionals. Data were collected through semi-structured interviews, guided by a script of topics, transcribed and submitted to thematic content analysis, in the light of Symbolic Interactionism, discussed considering the Family-Centered Care philosophy assumptions. Results: Two central categories emerged, "Theoretical perspective: the family as a care agent in painful procedures" and "Practical perspective: experiences, challenges and strategies in painful procedures for family inclusion", with their respective subcategories. Conclusion: Nursing professionals have theoretical knowledge about family inclusion in painful procedures based on the assumptions: Family-Centered Care: dignity and respect; information sharing; joint participation; and family collaboration. However, knowledge is not applied in clinical practice; consequence of the interaction between beliefs and attitudes unfavorable to family presence.

Abstract:Background Despite being a core component of family-centered and compassionate care, children's pain is often undertreated in Canadian hospitals. Nurses’ and other healthcare professionals’ (HCPs) ability to understand and respond to a child and their family's pain care needs is integral to improving this care in a family-centered manner. Purpose To understand nurses’ and other HCPs’ perceptions of child and family needs to make care more collaborative and patient- and family-centered. Methods Eighteen participants were recruited and represented the specialties of nursing ( n = 8), psychology ( n = 1), child life services ( n = 2), medicine/surgery ( n = 3), and administration/leadership ( n = 4); 3 of the administrators had a nursing background. Transcripts were analysed using a semantic, inductive approach with two coders using a codebook to ensure reliability. Results Participants felt that pain care was important, but that it needs to take greater priority in the hospital. In our analysis, we identified four core needs that nurses and other HCPs have to provide better pain care: 1. Better acknowledgement of child and family experiences; 2. Better visual and written knowledge translation tools for patients and families; 3. Better provision of verbal pain education to children and families by nurses and other HCPs; and 4. Help for patients and families to advocate for better pain care when they feel their needs are not being met. Conclusions Nurses and other HCPs value patient- and family-centered pain care, and wish to empower families to advocate for it when it is sub-optimal.

Some social factors related to control of diabetes mellitus in adult Jamaican patients.

Abstract:A population of 103 adult diabetic patients was interviewed at the Outpatient Clinic for diabetic patients of the University Hospital of the West Indies to examine the effects of severity of the illness and social variables, such as facilities at home, education attained, employment status, informal medication, and understanding of the illness in relation to its control. Patients were classified as severe if there were clinical evidence of vascular or neurologic complications of diabetes and as mild if there were no complications. Control of diabetes was regarded as good if the patients were free from or had less than 2+ glycosuria and as poor if they had had 2+ or more glycosuria during the six months before the study. We found that the quality of control was mainly determined by the severity of the disease. In the mild diabetic, proper diabetic control was not influenced by any social variable examined. In the severe diabetic the quality of control was associated with social amenities, educational status, employment status, and understanding of the disease.

Abstract:Background Nurses play an essential role in pain management in the PICU. However, their perception regarding pediatric pain and current practice of pain assessment in the Japanese PICU remains uncertain. Methods In January 2021, we conducted a multicenter cross‐sectional survey across 35 PICUs in Japan. A structured questionnaire which focused on nurses' perceptions of pediatric pain and pain assessment was developed, pilot‐tested, and was revised. Twenty copies of the questionnaire were sent to each institution and were distributed to the nursing staff. Results A total of 356 nurses from 22 institutions responded. Median age of the respondents was 33 years old and 84.6% were female. Median length of nursing experience and PICU experience were ten years and four years respectively. Use of pain scales for assessing pain in children who can self‐report pain, preverbal children, and children unable to self‐report pain due to cognitive impairment were 90.7%, 55.9%, and 50.0%, respectively. Nurses' satisfaction regarding pain management in their PICU was 31.9% and their confidence in pain assessment and management were 32.6% and 44.9% respectively. Lack of knowledge (95.8%), difficulty assessing pain in children (95.2%), and delay in physician's action (91.8%) were the most perceived barriers to optimal pain management. Conclusions The use of pain scales is insufficient, and nurses' satisfaction proved to be extremely low in the Japanese PICUs. Substantial effort is required to enhance the level of current pain management and improve outcomes.

Abstract:Purpose: Cancer patients often experience pain, which can greatly reduce their quality of life. It affects their emotions, cognitive function, and daily interactions. Healthcare providers need to understand the obstacles to pain management to create helpful programs for patients and families. This study focuses on Iranian nurses' views on pain management barriers in cancer patients. Methods: The study utilized a qualitative content analysis method with a purposive sampling approach, involving 27 nurses. Participants were selected to ensure a wide range of perspectives and experiences by considering factors such as gender, age, work experience, education levels, and positions until data saturation was achieved. Inclusion criteria specified a minimum of six months of oncology ward experience and a bachelor's degree or higher in nursing. Nurses with less than six months of oncology experience or lacking a nursing degree were excluded. Data collection was conducted through semi-structured interviews and analyzed using Lundman and Granheim's qualitative content analysis method. Results: One main theme, four main categories, and nine subcategories in the study reflected the nurses' experiences and viewpoints about barriers to pain management. These categories included the marginalization of complementary medicine, medical malpractice, inadequate organizational infrastructure, and personal barriers. Conclusion: The study demonstrated that the barriers to pain management in cancer patients were complex. To improve patients' comfort and quality of life, it is important to identify and address issues from different angles. It is crucial to train patients and healthcare providers in pain management and to address weaknesses in the healthcare system.
Molecular diversity of peanut‐nodulating rhizobia in soils of Argentina

Abstract:RSα sequencing is a valuable tool for identification of bacterial strains, and for evaluating the genetic structure of indigenous rhizobial populations. The purpose of this study was to evaluate, qualitatively, the presence or absence of RSα fragment in peanut‐nodulating strains isolated from plants grown at four sites in central Argentina. RSα fragment was found in only three of 26 indigenous strains, and in one of three inoculant strains analyzed. In contrast to results from studies of other symbiotic nitrogen‐fixing bacteria, such as soybean‐nodulating strains, no correlation was found between generation time and presence of RSα sequence. Phylogenetic analysis of the 16S rRNA gene sequence grouped peanut‐nodulating strains into two clusters, Bradyrhizobium japonicum vs. B. elkanii, and showed divergence among strains positive for RSα sequence. Our results confirm the genetic diversity previously reported for various peanut‐nodulating rhizobial strains, and indicate that the RSα fragment is not applicable as a marker or tool for competition assays at the field or ecological level. Abstract:Sialomucin complex (SMC, rat Muc4) is a heterodimeric glycoprotein composed of two subunits, the mucin component ascites sialoglycoprotein ASGP-1 and the transmembrane subunit ASGP-2, which is aberrantly expressed on the surfaces of a variety of tumor cells. Up-regulation of the Muc4/SMC gene in the 13762 sublines of the rat mammary adenocarcinoma correlates with the overexpression of transcription factor PEA3 and the receptor tyrosine kinase ErbB2. Here we report that PEA3 is capable of transactivating the Muc4/SMC promoter in a dose-dependent manner via direct attachment to a PEA3 binding site. ERM and ER81, the other two members of the PEA3 subfamily of transcription factors, could not transactivate the Muc4/SMC promoter. Transcriptional activation of Muc4/SMC by PEA3 is potentiated by Ras and MEKK1 kinases. These data suggest that expression of PEA3 in mammary tumors leads to up-regulation of Muc4/SMC transcription, the gene product of which may contribute to the metastatic potential of mammary tumors.

Abstract:This article examines the pivotal role of critical care nurses in managing pediatric pain, emphasizing the multifaceted nature of care. It covers the challenges and opportunities nurses face, stressing the importance of education and training. The background section underscores the global issue of inadequately managed pediatric pain and the importance of addressing social determinants of health while recognizing perceptions, beliefs, stereotypes, and biases on pain management. A multimodal approach in treatment is detailed in pharmacologic and non-pharmacologic approaches. Abstract:Introduction In critically ill children, pain management is complex owing to cognitive development and the nature of hospitalisation in paediatric intensive therapy units. Although there are many protocols and guidelines for pain control via pharmacological interventions, non-pharmacological practices should also be explored and disseminated for their potential benefit. Methods and analysis A systematic literature search will be performed using the following databases: Academic Search Premier, Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, Excerpta Medica Database, Virtual Health Library, Medical Literature Analysis and Retrieval System Online, ScienceDirect, Scopus, Web of Science Core Collection, Theses from Coordenacão de Aperfeicoamento de Pessoal de Nível Superior , Dart Europe, Open Access Theses and Dissertations and grey literature from Google Scholar. The research will consider quantitative and qualitative studies, mixed-method studies, systematic reviews, text articles, opinion articles, letters to editors and editorials in any language and from any database. The following will be eligible for inclusion: (1) newborns, infants, children and adolescents; and (2) non-pharmacological therapies used for pain in paediatric intensive care. Abstract:Objectives: To explore Chinese nurses’ behaviour, experience and opinion of postoperative paediatric pain assessment. Methods: A qualitative research was used and we conducted in-depth face-to-face interviews with 20 Chinese paediatric nurses. Results: The evidence demonstrated that nurses’ role position, expectation, capability and behaviour in assessing postoperative paediatric pain were influenced by many factors, including pain management education and policy, the relationship between nurses, doctors, children and their parents, and special clinical environment. The lack of detailed theoretical and practical support was the main barrier to initiate the professional strength of nurses’ contribution to the assessment of the postoperative pain for children and to reduce their pain. Conclusion: The current nursing educational curriculum and the administrative policy in pain assessment and management need to be re-evaluated about their relevance and accuracy according to nurses’ opinions and needs.

Abstract:Introdução: a proposta dos Cuidados Paliativos é implementada para pacientes em situações de vulnerabilidade ante uma doença grave, limitante e que pode, inevitavelmente, se encaminhar para a terminalidade. Objetivo: conhecer as percepções e vivências dos profissionais de enfermagem frente ao cuidado à criança em cuidados paliativos em unidades pediátricas. Metodologia: estudo qualitativo, exploratório e descritivo, realizado em unidades pediátricas de um hospital universitário de grande porte do Sul do Brasil. Os dados foram obtidos mediante entrevistas semiestruturadas com nove participantes, enfermeiros e técnicos de enfermagem, submetidas à análise de conteúdo temática. Resultados: a partir da análise temática emergiram quatro temas: Criança em cuidados paliativos: olhar da enfermagem; Cuidado centrado na família; Vivências, sentimentos e percepções da equipe de enfermagem; e Enfrentamento da equipe de enfermagem: desafios para o cuidado. O estabelecimento dos princípios de cuidados paliativos pediátricos é fundamental para subsidiar o cuidado qualificado. Os profissionais de enfermagem revelam a necessidade de serem incluídos nas reuniões multidisciplinares realizadas para decidir condutas em relação ao paciente e sentem necessidade de apoio psicológico. Considerações finais: desvela-se a compreensão de que a filosofia de Cuidados Paliativos precisaser ampliada para uma rede integrada de atenção e respeito às decisões para o final da vida.
Neonatal Nurses' Perceptions of Pain Management: Survey of the United States and China.

Abstract:Despite growing knowledge, neonatal pain remains unrecognized, undertreated, and generally challenging. A cross-sectional survey study was conducted to investigate neonatal nurses' perceptions, knowledge, and practice of infant pain in the United States and China, including 343 neonatal nurses (American nurses [n = 237]; Chinese nurses [n = 106]). Nurses' responses regarding neonatal pain reflected adequate knowledge in general pain concepts, but knowledge deficits related to several topics were found (e.g., preterm infants are more sensitive to pain and long-term consequences of pain). Most reported regular use of pain assessment tools, but fewer agreed that the tool used was appropriate and accurate. More American nurses (83%) than Chinese nurses (58%) felt confident in the use of pain medications, while more Chinese nurses (78%) than American nurses (61%) acknowledged the effectiveness of nonpharmacologic interventions. About half reported that pain in their units was well managed (American: 44.3%; Chinese: 55.7%), and less than half felt that pain guidelines/protocols were research-based (American: 42.6%; Chinese: 34.9%). Nurses' perceptions of well-managed pain in their units were significantly correlated with adequate education/training, use of accurate tools, and use of research-based protocols. Barriers to effective pain management included resistance to change, lack of knowledge, lack of time, fear of side effects of pain medication, and lack of trust in the tools. The survey reflects concerns that pain has not been well managed in many neonatal intensive care units in the United States and China. Further actions are needed to solve the issues of inadequate training, lack of clinically feasible pain tools, and absence of evidence-based guidelines/protocols.

Abstract:In daily practice in the neonatal intensive care unit (NICU), newborns are subjected to a series of procedures and therapeutic interventions that cause pain; Pain management in the newborn is important within specialized care in neonatology (Gallegos-Martínez & Salazar-Juárez, 2010). Patricia Benner in her theory points out that, as the professional acquires this experience, clinical knowledge becomes a mixture of practical and theoretical knowledge. It is important to mention that nursing sees the human being as a holistic being in each of the stages of his life. Objective: Identify the level of knowledge about pain management in the Neonatal Intensive Care area by the nursing professional Methodology: A quantitative, descriptive study was carried out, its type of research is prospective, longitudinal. The subjects were nursing staff. With a universe of 35 nurses from all shifts and a sample of 20 participants at the convenience of the NICU area from different shifts (morning, afternoon, night and cumulative shift). An instrument was used to measure knowledge of neonatal pain by nursing professionals in neonatal intensive care units carried out by Edwin Millán Flores and validated by the Ana G. Méndez University System (SUAGM) in Puerto, Rico. Results: The level of knowledge about neonatal pain management education was 35.0% of the nursing staff had a good level of knowledge and 45.0% had an excellent level. Conclusions: Although we have the knowledge, the nursing professional Benner tells us that we must reach the experts, therefore, it is necessary to design tools or strategies that take us to the experts, to apply the knowledge and share it with the other personnel in the area.
Non-Pharmacological Pain and Stress Management (N-PPSM) in Pediatric Wards: A Nurses' Perspective

Abstract:Purpose: To describe the nurses' use of non-pharmacological pain and stress management (N-PPSM) in pediatric wards and their perceptions of the benefits and perceived barriers that encourage and limited their use of these methods. Methods: This was a descriptive cross-sectional study involving 125 nurses working in the pediatric ward of an Indonesian hospital. Data collection utilized modified non-pharmacological method questionnaires, and data analysis involved t-tests and ANOVA. Results: The most widely used N-PPSM by nurses are cognitive methods such as providing education (48%), physical methods by positioning (40%), emotional support (36%), environmental support (45%), and family support (60%). Nurses stated that N-PPSM had few side effects (50.4%), were inexpensive (49.6%), could be performed independently (51.2%), and were easy to use (52.8%). However, during its implementation, there were several obstacles including lack of experience (42.4%), lack of training (48%), lack of equipment (45.6%), nurse belief (36.8%), lack of time (44%), patient unwilling (47.2%), and patient belief (41.6%). Work experience influences the overall utilization of N-PPSM (p-value = .043). Conclusion: The findings from this research emphasize the significance of enhanced training for nurses working in the utilization of N-PPSM in pediatric wards. Clinical implications: Findings from this study can enhance nurses' knowledge and competence in using N-PPSM in pediatric wards, thus improving patient care. Hospitals may consider implementing new policies, education, training, and infrastructure to support N-PPSM. Educational institutions can also incorporate these methods into nursing curricula, increasing nurses' awareness and skills in using N-PPSM in pediatric patients.

Abstract:Background: Although the occurrence of pain among hospitalized children is inevitable, the way to assess and detect the intensity of pain is still challenging to pediatric professionals and may lead to increased morbidity and mortality rates if left untreated. The knowledge base of nurses regarding both pharmacological and non-pharmacological pain management among children plays a significant role in managing and making a management plan. The purpose of this study was to assess nursing students' knowledge and attitudes toward pediatric pain management. Methods: This study used a cross-sectional design to investigate the knowledge and attitudes regarding pain and its management among undergraduate nursing students at a university. A convenience sample of students from all levels was obtained. The pediatric nurses knowledge and attitudes survey (PNKAS) regarding pain was used. Data were analyzed by IBM SPSS Statistics for Windows, Version 22 (Released 2013; IBM Corp., Armonk, New York, USA). Results: In this study, 86.3% of participants (n = 69) scored within the satisfactory range on the PNKAS, suggesting adequate knowledge and attitudes about pediatric pain management. The results showed there was no signiﬁcant relationship between age and a total score p-value of 0.740, as well as between academic levels and the same p-value of 0.397. Conclusion: This study highlights the importance of undergraduate nursing education in pediatric pain management. The findings suggest that students may benefit from additional training in pain assessment and pharmacological interventions.

Abstract:Asbestosis is a form of silicosis, the asbestos fibres consisting mainly of a fibrous magnesium and calcium silicate. These fibres are found in the fine dust present in factories engaged in the manufacture of hcat-resisting materials, etc., from crude mineral asbestos. The lesions produced in the lungs by the inhalation of asbestos fibres take the form of a diffuse pulmonary fibrosis affecting particularly the lower lobes of the lungs. The characteristic "asbestos bodies" may be found in the sputum, but it is only when they are present in clumps or masses that they indicate parenchymal involvement (Beattie and Dickson, 1943). Emphysema and bronchiectasis are commonly present; the pleura is thickened and may be adherent to the diaphragm. ¶Pulmonary tuberculosis may be associated with asbestosis, although not so commonly as with silicosis. The following case is of interest in view of the lapse of time between exposure to industrial hazard and the diagnosis of the disease, and because of the isolation of " asbestos bodies" in the sputum 20 years after exposure.

Abstract:Aim Despite the importance of pain treatment, does nursing education devoted enough time educating the next generation of nursing? Do bedside nurses have an adequate perception of pain management and the appropriate confidence to treat pain effectively? Background People often seek treatment because of pain. If pain is not addressed or inadequately managed it can lead to immobility, decompensation and longer hospital stays. If acute pain is not treated it can lead to chronic pain with long-term physical and psychological effects on the individual (Sedighie et al., 2020). Despite the importance of pain treatment, does nursing education devoted enough time educating the next generation of nursing? Do bedside nurses have an adequate perception of pain management and the appropriate confidence to treat pain effectively? Information was gathered from a nurse residency project in an intensive care unit in tertiary care facility about nurse confidence and knowledge in a surgical and neurosurgical intensive care unit to deliver appropriate treatment to the patient population. Method Staff nurses in acute care settings were asked to evaluate their knowledge with pain medication, patient's assessment and interventions based on a survey tool modified from the Ferrell and McCaffrey, (2014), Knowledge and Attitudes Survey Regarding Pain. This tool is ubiquitous in pain nursing studies and has been established as valid and reliable. Pain skills of staff nurses have been studied in multiple areas. Results This study is in process with expected completion date in August 2023. Conclusions The results of this study will strive to improve nursing knowledge with pain medication, become more self aware of patient's assessment and interventions. This information will be incorporated in the initial staff nurse education for nursing on-boarding and will be a staple in continuing nursing education.

Abstract:Introduction Pain is the most common presenting feature within the ED, yet undertreatment of pain in the ED is a well-documented problem worldwide. Despite the development of interventions to address this problem, there is still limited understanding of how pain management can be improved within the ED. This systematic mixed studies review aims to identify and critically synthesise research exploring staff views of barriers and enablers to pain management to understand why pain continues to be undertreated in the ED. Methods We systematically searched five databases for qualitative, quantitative and mixed methods studies reporting ED staff views of barriers and enablers to pain management in the ED. Studies were quality assessed using the Mixed Methods Appraisal Tool. Data were extracted and qualitative themes were generated by deconstructing data then developing interpretative themes. Data were analysed using convergent qualitative synthesis design. Results We identified 15 297 articles for title/abstract review, reviewed 138 and included 24 in the results. Studies were not excluded due to low quality, although lower scoring studies contributed less data to the analysis. Quantitative surveys focused more on environmental factors (eg, high workload and bureaucratic restrictions), while qualitative studies revealed more insight about attitudes. We developed five interpretative themes from the thematic synthesis: (1) pain management is seen as important but not a clinical priority; (2) staff do not recognise the need to improve pain management; (3) the ED environment makes it difficult to improve pain management; (4) pain management is based on experience, not knowledge; and (5) staff lack trust in the patient’s ability to judge pain or manage it appropriately. Conclusions Overly focusing on environmental barriers as principal barriers to pain management may mask underlying beliefs that hinder improvements. Improving feedback on performance and addressing these beliefs may enable staff to understand how to prioritise pain management.

Abstract:Aim To understand nurses' knowledge, beliefs and experiences affect pain management practices in hospitalised persons living with dementia (PLWD). Design Naturalistic inquiry using qualitative descriptive design. Methods Semi‐structured interviews were conducted with 12 registered nurses who worked in one acute care hospital in Southern California from October to November 2022. Data were analysed using content analysis to identify themes. Results Two themes were developed: improvising pain assessment, which included how pain was documented, and administration hesitancy referring to nurse's concerns about PLWD's confusion. Nurses described the challenges of assessing pain in hospitalised PLWD particularly if they were non‐verbal and/or demonstrating responsive behaviours. Nurse's years of experience, dementia stigma, and their unconscious biases affected nurses' pain management practices. Conclusions The study findings highlight the complex challenges of pain management in hospitalised PLWD that are exacerbated by nurses' knowledge deficits, negative stereotypical beliefs, dementia stigma and unconscious biases towards older people that contributes to undermanaged pain in hospitalised PLWD. Implications A comprehensive strategy using an implementation framework is needed to address nurse's knowledge gaps, unconscious bias, dementia stigma and techniques that enhance communication skills is suggested. Building a foundation in these areas would improve pain management in hospitalised PLWD. Impact Improving pain management in hospitalised PLWD would improve the quality of life, decrease hospital length of stay, prevent readmissions, and improve nurse satisfaction. Reporting Method The study adhered to the Consolidated Criteria for Reporting Qualitative Research (COREQ). Patient Contributions Improving pain management in hospitalised PLWD would prevent long term confusion, episodes of delirium and improve quality of life as they recover from their acute illness for which they required hospital care.
