Abstract:In 5000 BC, South American tribes digged the bones in the living head to seek ways to communicate with the gods, which was primitive trephination and may be the first neurosurgical behavior. In 2600 BC, Imhotep in ancient Egypt took the brain out of the head from the nose, for a better preserve of the mummy, which was a prototype of modern transsphenoidal surgery. And the development of anatomy in ancient Greek laid a solid foundation for neurosurgery. From 500 to 1500 AD, the rise of religion and the occurrence of war, prompted a large number of craniocerebral trauma, which contributed greatly to the early development of neurosurgery as a distinct specialty. In 1861, Brocca astutely localized the language function to the third left frontal convolution in a series of studies, which was considered to be of landmark importance in the understanding of cerebral localization. In 1878, William Macewen performed a successful surgery to remove an en plaque meningioma with intrathecal anesthesia, representing the first modern neurosurgical operation. However, the contributions of the Americans, starting with Harvey Cushing, exerted a definitive force. Portuguese Moritz performed the first cerebral angiogram on a living schizophrenia patient in 1926. And he established the Moniz-Lima prefrontal leucotomy for the treatment of schizophrenia, for which he won the Nobel Prize in Physiology and Medicine in 1949. In 1968, the Swiss scholar Yassagir firstly carried out neurosurgical surgeries under the microscope. China's neurosurgery was founded by Zhao Yicheng in 1952 in Tianjin, and the gap in neurosurgery between China and the world gradually narrowed after 60 years of development.
Meta‐chlorophenylpiperazine attenuates formalin‐induced nociceptive responses through 5‐HT1/2 receptors in both normal and diabetic mice

Abstract:1 This study was designed to investigate the effect of meta‐chlorophenylpiperazine (m‐CPP; a 5‐hydroxytryptamine (5‐HT) receptor agonist) on the formalin‐induced nociceptive responses in normal, insulin‐dependent streptozotocin (STZ) diabetic and non‐insulin dependent genetically diabetic (db/db) mice. 2 A subcutaneous injection of diluted formalin (1% formaldehyde in 0.9% saline, 10 μm) under the plantar surface of the left hindpaw induced biphasic nociceptive responses, the first and second phases considered to represent acute and chronic pain, respectively. The former response in db/db mice was significantly lower than those in normal mice, and the latter responses in STZ and db/db mice were significantly lower than those in normal mice. 3 In normal mice, m‐CPP (0.32‐3.2 mg ml−1, p.o.) exhibited potent antinociceptive activity, dose‐dependently attenuating the frst and second phase; the ID50 value of the second phase was 0.4 mg kg−1. m‐CPP (0.32‐3.2 mg kg−1, p.o.) also dose‐dependently attenuated the formalin‐induced nociceptive responses in STZ‐induced diabetic mice and genetically diabetic db/db mice, and the activities were comparable to those in normal mice. 4 The antinociceptive activities of m‐CPP (1 mg kg−1, p.o.) were significantly inhibited by pretreatment with pindolol (a 5‐HT1‐receptor antagonist, 1 mg kg−1, i.p.) or ketanserin (a 5‐HT2 receptor antagonist, 1mg kg−1, i.p.) but were hardly affected by ICS205‐930 (a 5‐HT3 receptor antagonist, 1 mg kg−1, i.p.). 5 These results suggest that m‐CPP inhibits not only acute but also chronic pain transmission through 5‐HT1 and 5‐HT2 receptors, and that the 5‐hydroxytryptaminergic antinociceptive pathways are little affected by diabetes.

Abstract:Introduction Neurosurgery, and in particular academic neurosurgery, has, despite the tremendous progress in the last 100 years, not found its clear and solidly anchored home within the medical specialties. This stems from its dichotomic origin. Complicating the picture, these origins are different on different continents. In the USA, neurosurgery was established as a new specialty by Harvey Cushing—a surgeon—and neurosurgery hence for a long time was a surgical subspecialty dwelling on the technical aspects of neurosurgery and, to be fair on some pathophysiology, but not really on central nervous system (CNS) biology. In Europe, neurosurgery also branched off neurology with Otfried Foerster being an eminent neurologist, before turning to the surgical armamentarium to treat neurological disease. Ironically, Cushing and Foerster visited each other and showed admiration for their respective work. Its growth and technological changes have influenced many other subdisciplines of our field. As a matter of fact, of the numerous evolutionary and revolutionary advances in the practice of neurological surgery, most have arisen from this need to treat and cure vascular disease of the brain and spinal cord. Evaluating innovation. Part 2: Development in neurosurgery

Abstract:OBJECT Patients, practitioners, payers, and regulators are advocating for reform in how medical advances are evaluated. Because surgery does not adhere to a standardized developmental pathway, how the medical community accepts a procedure remains unclear. The authors developed a new model, using publication data and patterns, that quantifies this process. Using this technique, the authors identified common archetypes and influences on neurosurgical progress from idea inception to acceptance. METHODS Seven neurosurgical procedures developed in the past 15-25 years were used as developmental case studies (endovascular coil, deep brain stimulation, vagus nerve stimulation, 1,3-bis(2-chloroethyl)-l-nitrosourea wafer, and 3 radiosurgery procedures), and the literature on each topic was evaluated. A new metric the authors termed "progressive scholarly acceptance" (PSA) was used as an end point for community acceptance. PSA was reached when the number of investigations that refine or improve a procedure eclipsed the total number of reports assessing initial efficacy. Report characteristics, including the number of patients studied, study design, and number of authoring groups from the first report to the point of PSA, were assessed. RESULTS Publication data implicated factors that had an outsized influence on acceptance. First, procedural accessibility to investigators was found to influence the number of reports, number of patients studied, and number of authoring groups contributing. Barriers to accessibility included target disease rarity, regulatory restrictions, and cost. Second, the ease or difficulty in applying a randomized controlled trial had an impact on study design. Based on these 2 factors, 3 developmental archetypes were characterized to generally describe the development of surgery. CONCLUSIONS Common surgical development archetypes can be described based on factors that impact investigative methods, data accumulation, and ultimate acceptance by society. The approach and proposed terminologies in this report could inform future procedural development as well as any attempts to regulate surgical innovation.
Comparison of hydromorphone and meperidine for ureteral colic.

Abstract:OBJECTIVE To compare the efficacies of meperidine and hydromorphone in the treatment for ureteral colic in the emergency department (ED). METHODS A prospective, double-blind, randomized clinical trial was conducted over six months at a tertiary referral center with 93,000 annual ED visits. Seventy-three patients completed the study. The patients received either 1 mg of hydromorphone or 50 mg of meperidine IV at t = 0. Pain intensity was determined using a 10-cm visual analog scale at t = 0, 15, 30, 60, and 120 minutes. A second dose of the study drug could be given between t = 15 and t = 120 minutes when the clinician believed the initial dose was ineffective. Patients requiring more than one additional dose of analgesia were treated as nonresponders and were removed from the study. RESULTS Thirty-six patients received hydromorphone and 37 received meperidine. The initial pain intensities (hydromorphone group = 8.4 +/- 1.5; meperidine group = 8.5 +/- 2.1), age distributions, sex distributions, and side effects of the two groups were comparable. Pain relief was better (p < 0.05) with hydromorphone at t = 15, 30, 60, and 120 minutes. The hydromorphone group required rescue analgesia less often (31% vs 68%, p < 0.01), had fewer IV pyelographies (IVPs) (28% vs 54%, p < 0.05), and had a lower proportion of hospital admissions (25% vs 49%, p = 0.08). CONCLUSIONS For the fixed doses used in this study, the adult ureteral colic patients receiving hydromorphone achieved more pain relief, required less rescue medication, underwent fewer IVPs, and avoided hospital admission more frequently than did those receiving meperidine.

Abstract:Military neurosurgery has played a crucial role in the development of neurosurgery over time. Much of this progress is due to war-related experiences. Owing to the number and severity of war injuries and the limitations caused by war, surgeons have had to examine different methods and design special protocols for patient management. Given that in recent decades most wars have taken place in the Middle East, many lessons can be learned by reviewing the experiences of neurosurgeons in these wars. Wars in Iraq, Syria, Afghanistan, Lebanon, and Yemen have been the largest and longest conflicts in the Middle East since the beginning of the 21st century, and a number of studies reported the experiences of surgeons in these wars. In this study, we reviewed the experience of military surgeons in managing war neurosurgical injuries in these areas within the last 2 decades.

Abstract:Since the inception of their profession, neurosurgeons have defined themselves as physicians with a surgical practice. Throughout time, neurosurgery has always taken advantage of technological advances to provide better and safer care for patients. In the ongoing precision medicine surge that drives patient-centric healthcare, neurosurgery strives to effectively embrace the era of data-driven medicine. Neuro-oncology best illustrates this convergence between surgery and precision medicine with the advent of molecular profiling, imaging and data analytics. This convenient convergence paves the way for new preventive, diagnostic, prognostic and targeted therapeutic perspectives. The prominent advances in healthcare and big data forcefully challenge the medical community to deeply rethink current and future medical practice. This work provides a historical perspective on neurosurgery. It also discusses the impact of the conceptual shift of precision medicine on neurosurgery through the lens of neuro-oncology.

Abstract:Modern neurosurgery has a brief history of around 100 years. It is called “modern” because its development has been associated with the tremendous progress of modern science and technology. Sharp tools do good work. The introduction of the microscope brought neurosurgery into the era ofmicroneurosurgery, and use of the endoscope has further minimized surgical invasiveness in selected cases. Progress in neurology has made it possible to locate neurologic lesions more accurately. Not only the lesion itself, but also the surrounding vasculature, affected nerves, nucleus and tracts, and functional brain parenchyma are well mapped. The Visual Reality system, which employs three-dimensional image merging techniques, enables the neurosurgeon to make a detailed surgical plan. The neuronavigation system, a great innovation in neurosurgery, has helped neurosurgeons to perform operations with proper orientation. The introduction of intraoperative magnetic resonance imaging (MRI) has eliminated the worry about brain shift during operation. We have used 3.0 T intraoperative MRI for months and find it is of precious value in performing glioma resection to achieve “maximal safe resection.” Furtherprogress shouldbe focused on definitive mapping of brain functional areas, including the motor, sensory, hearing, and visual functional areas and the ways they interact. Resting MRI investigation is valuable in solving positional problems. Brain activity can be evaluated not only by BOLD (blood oxygen level-dependent) techniques but also by physical electric activity and neurotransmitter change, which cannot be well shown with images of high resolution. The deep roots of military service in neurological surgery: an academic genealogical analysis of the founding generation

Abstract:Throughout human history, advancements in medicine have evolved out of periods of war. The carnage of battlefield injuries provided wartime surgeons an unprecedented opportunity to study anatomy, develop novel techniques, and improve systems of care. As a specialty that was established and evolved during the first half of the 20th century, neurological surgery was heavily influenced by the experiences of its founders during the World Wars I and II. Utilizing the published Neurosurgery Tree, the authors conducted an academic genealogical analysis to systematically define the influence of wartime service on neurosurgery's earliest generations. Through review of the literature and military records, the authors determined that at least 60% of American neurosurgical founders and early leaders served during World Wars I and/or II. Inspired by the call to serve their nation as forces for good, these individuals were heralded as expert clinicians, innovative systems thinkers, and prolific researchers. Importantly, the service of these early leaders helped highlight the viability of neurosurgery as a distinct specialty and provided a framework for early neurosurgical education and expansion. The equipment, techniques, and guidelines that were developed during these wars, such as management of craniocerebral trauma, peripheral nerve repair, and hemostasis, set the foundation for modern neurosurgical practice.
Historical roots of modern neurosurgical cadaveric research practices: dissection, preservation, and vascular injection techniques

Abstract:Because of the complexity of the brain and its structures, anatomical knowledge is fundamental in neurosurgery. Anatomical dissection, body preservation, and vascular injection remain essential for training, teaching, and refining surgical techniques. This article explores the historical development of these practices and provides the contextual background of modern neurosurgical cadaveric brain models. Body preservation has ancient beginnings, evident in the Chinchorro mummifications and Egyptian embalming. However, brain preservation techniques for education were scarce until the beginning of the Renaissance in Europe. At the University of Bologna in the 13th century, occasional dissections were performed only in winter because of the lack of preservation techniques. Pope Sixtus IV's 1482 papal bull (official decree) formalized and expanded the use of dissection in medical education, leading to an explosion in anatomical studies. This surge brought advances in body preservation, such as soaking bodies in vinegar and distilled liquors. In subsequent centuries, Andreas Vesalius and Charles Bell advanced brain anatomical techniques and knowledge, combining novel illustrations and instruction. To better understand brain vasculature, Richard Lower developed vascular injection techniques using india ink and spirits of wine, leading to the 1664 description of the circle of Willis by Thomas Willis. In 1868, August Hofmann synthesized formaldehyde, markedly improving tissue preservation. Later, William Kruse introduced latex in 1939, and Sidney Sobin introduced silicone in 1965 for vascular studies. These advancements laid the foundation for modern neurosurgical cadaveric studies, many remaining relevant today.
Bibliometric Research in Neurosurgery: A Review of the 50 Most Cited Bibliometric Publications in the Neurosurgical Literature

Abstract:Bibliometry is a popular research method that is used to explore and analyze large volumes of data in an effort to highlight trends, patterns, and impacts within a specific field. This review aimed at highlighting the characteristics and citation patterns of the high-impact bibliometric research studies that were published in the neurosurgical literature. Using PubMed and Google Scholar, the 50 (52 due to identical citation numbers for the lowest three articles) most cited bibliometric research publications were identified and reviewed. Information relating to the articles' publication and bibliometric features were retrieved. The articles' citation numbers were collected. The median article age and journal impact factor (IF) were eight years and 2.76, respectively. The majority of studies were published in World Neurosurgery and the Journal of Neurosurgery, which were the publishing journals for 18 (35%) and 12 (23%) articles, respectively. Twenty-six (50%) articles were first authored by researchers from the United States of America (USA). The highest bibliometric component was science mapping, which was the theme in 30 (58%) articles. The majority of the bibliometric focus was clinical topics/fields (22 (42%) articles) and neurosurgeons/departments (21 (40%) articles). The most popular bibliometric metric was the h-index (±variants), which was employed in 22 (42%) articles. The median size of analyzed data was 188, and the most frequently utilized databases were Scopus (22 (42%) articles) and Web of Science (21 (40%) articles). The median (range) citation numbers were 52 (29-238). The citation analysis showed significantly higher citation numbers for older articles (aged ≥ 8 years) and studies published in the Journal of Neurosurgery. The citation rates were not influenced by the size of the data, the searched databases, or the bibliometric features. In conclusion, the most cited bibliometric research publications in the neurosurgical literature were predominantly descriptive analyses of clinical topics/fields and performance analyses of neurosurgeons/departments. Their citation numbers were relatively modest and were positively influenced by the publication's age and by a specific publishing journal but not by the bibliometric features of the study. Bibliometric research provides useful analytic tools that can be utilized in review studies and other practical purposes such as scholarly practices and policy decision-making.
The 100 Top-Cited Articles on Robotic Neurosurgery: A Bibliometric Analysis

Abstract:Background: Robotic neurosurgery is a rapidly advancing field with numerous applications in various subspecialties, including spine, functional, skull base, and cerebrovascular. This study aims to provide a comprehensive analysis of the most-cited articles on robotic neurosurgery. Methods: The Web of Science database was used to collect data, and bibliometric analysis was performed using VOSviewer and RStudio. Network analysis techniques such as co-occurrence, coauthorship, bibliographic coupling, and thematic mapping analyses were used to identify the top 100 most cited articles, major contributors, emerging trends, and noteworthy themes in the field. Results: The results showed that there has been a steady increase in the number of publications on robotic neurosurgery since 1991, with an exponential growth in the number of citations. The United States was the most common country of origin for articles, followed by Canada. The most productive authors in this field were Burton S.A. and Gerszten P.C., while the University of Pittsburgh was the most productive institution, and Neurosurgery was the most productive journal. Themes such as robotics, back pain, and prostate cancer, as well as trends in developing new technologies and improving the precision of surgical procedures, were identified. Conclusions: This study provides a comprehensive analysis of the most-cited articles on robotic neurosurgery. The broad range of topics and techniques explored emphasize the importance of continued innovation and investigation. Ultimately, the study's findings provide valuable guidance for future research and contribute to advancing our understanding of this critical area of study.

Abstract:In the 1930s and 1940s, neurosurgeons and clinical neurologists engaged in a fierce exchange on the scope of their specialties. Neurosurgeon Wilder Penfield's rhetoric of therapeutic superiority had a strong impact both on the Rockefeller Foundation's support for his institute and on the self-fashioning of neurologists. Neurologists articulated their identity in spirited performances at the meetings of specialist societies, their response shifting from a combative approach to a focus on internal organization. In light of the neurosurgeons' discourse, by the 1950s a new generation of neurologists created a revisionist narrative that inaccurately portrayed the clinical neurologists of the past as having been uninterested in therapeutics.

Abstract:Despite their relatively low incidence globally, central nervous system (CNS) tumors remain amongst the most lethal cancers, with only a few other malignancies surpassing them in 5-year mortality rates. Treatment decisions for brain tumors heavily rely on histopathological analysis, particularly intraoperatively, to guide surgical interventions and optimize patient outcomes. Frozen sectioning has emerged as a vital intraoperative technique, allowing for highly accurate, rapid analysis of tissue samples, although it poses challenges regarding interpretive errors and tissue distortion. Raman histology, based on Raman spectroscopy, has shown great promise in providing label-free, molecular information for accurate intraoperative diagnosis, aiding in tumor resection and the identification of neurodegenerative disease. Techniques including Stimulated Raman Scattering (SRS), Coherent Anti-Stokes Raman Scattering (CARS), Surface-Enhanced Raman Scattering (SERS), and Tip-Enhanced Raman Scattering (TERS) have profoundly enhanced the speed and resolution of Raman imaging. Similarly, Confocal Laser Endomicroscopy (CLE) allows for real-time imaging and the rapid intraoperative histologic evaluation of specimens. While CLE is primarily utilized in gastrointestinal procedures, its application in neurosurgery is promising, particularly in the context of gliomas and meningiomas. This review focuses on discussing the immense progress in intraoperative histology within neurosurgery and provides insight into the impact of these advancements on enhancing patient outcomes.
Al-Zahrawi (936-1013AD): On the Surgical Treatment of Neurological Disorders by the Father of Operative Surgery

Abstract:Introduction Medical knowledge during the medieval ages flourished under the influence of great scholars of the Islamic Golden age such as Ibn Sina (Latinized as Avicenna), Abu Bakr al-Razi (Rhazes), and Abu al-Qasim Khalaf ibn al-Abbas al-Zahrawi, known as al-Zahrawi. Much has been written on al-Zahrawi's innovation in various disciplines of medicine and surgery. In this article, we focus for on the contributions of al-Zahrawi toward the treatment of neurological disorders in the surgical chapters of his medical encyclopedia, Kitab al-Tasrif ( The Method of Medicine ). Methods Excerpts from a modern copy of volume 30 of al-Zahrawi's Kitab al-Tasrif were reviewed and translated by the primary author from Arabic to English, to further provide specific details regarding his neurosurgical knowledge. In addition, a literature search was performed using PubMed and Google Scholar to review prior reports on al-Zahrawi's neurosurgical instructions. Results In addition to what is described in the literature of al-Zahrawi's teachings in cranial and spine surgery, we provide insight into his diagnosis and management of cranial and spinal trauma, the devices he used, and prognostication of various traumatic injuries. Conclusion Al-Zahrawi was a renowned physician during the Islamic Golden age who made significant contributions to the diagnosis and treatment of neurological conditions, particularly cranial and spinal cord injuries. He developed innovative surgical techniques for trephination and spinal traction, which are still used in modern neurosurgery. His insights make him worthy of recognition as an important figure in the history of neurological surgery.
