Abstract:The data available in Electronic Health Records (EHRs) provides the opportunity to transform care, and the best way to provide better care for one patient is through learning from the data available on all other patients. Temporal modelling of a patient's medical history, which takes into account the sequence of past events, can be used to predict future events such as a diagnosis of a new disorder or complication of a previous or existing disorder. While most prediction approaches use mostly the structured data in EHRs or a subset of single-domain predictions and outcomes, we present MedGPT a novel transformer-based pipeline that uses Named Entity Recognition and Linking tools (i.e. MedCAT) to structure and organize the free text portion of EHRs and anticipate a range of future medical events (initially disorders). Since a large portion of EHR data is in text form, such an approach benefits from a granular and detailed view of a patient while introducing modest additional noise. MedGPT effectively deals with the noise and the added granularity, and achieves a precision of 0.344, 0.552 and 0.640 (vs LSTM 0.329, 0.538 and 0.633) when predicting the top 1, 3 and 5 candidate future disorders on real world hospital data from King's College Hospital, London, UK (\textasciitilde600k patients). We also show that our model captures medical knowledge by testing it on an experimental medical multiple choice question answering task, and by examining the attentional focus of the model using gradient-based saliency methods.
Foresight -- Generative Pretrained Transformer (GPT) for Modelling of Patient Timelines using EHRs

Abstract:Background: Electronic Health Records hold detailed longitudinal information about each patient's health status and general clinical history, a large portion of which is stored within the unstructured text. Existing approaches focus mostly on structured data and a subset of single-domain outcomes. We explore how temporal modelling of patients from free text and structured data, using deep generative transformers can be used to forecast a wide range of future disorders, substances, procedures or findings. Methods: We present Foresight, a novel transformer-based pipeline that uses named entity recognition and linking tools to convert document text into structured, coded concepts, followed by providing probabilistic forecasts for future medical events such as disorders, substances, procedures and findings. We processed the entire free-text portion from three different hospital datasets totalling 811336 patients covering both physical and mental health. Findings: On tests in two UK hospitals (King's College Hospital, South London and Maudsley) and the US MIMIC-III dataset precision@10 0.68, 0.76 and 0.88 was achieved for forecasting the next disorder in a patient timeline, while precision@10 of 0.80, 0.81 and 0.91 was achieved for forecasting the next biomedical concept. Foresight was also validated on 34 synthetic patient timelines by five clinicians and achieved relevancy of 97% for the top forecasted candidate disorder. As a generative model, it can forecast follow-on biomedical concepts for as many steps as required. Interpretation: Foresight is a general-purpose model for biomedical concept modelling that can be used for real-world risk forecasting, virtual trials and clinical research to study the progression of disorders, simulate interventions and counterfactuals, and educational purposes.

Abstract:In recent years, with the better availability of medical data such as Electronic Health Records (EHR), more and more data mining models have been developed to explore the data-driven insights for better human health. However, there are many challenges for analyzing EHR such as high-dimensionality, temporality, sparsity, etc., which make the data-driven models less reliable. Medical knowledge graph (MKG), which encodes comprehensive knowledge about the medical concepts and relationships extracted from medical literature, holds great promise to regularize the data-driven models as prior knowledge. Nonetheless, the MKGs are typically not complete, which limits its utility in helping with the data mining process. In this paper, we propose a mutual enhancement framework MendMKG for predictive modeling of clinical events with both EHR and MKG. In particular, MendMKG first conducts a self-supervised learning strategy to simultaneously pre-train a graph attention network for embedding nodes and complete the MKG. It iteratively performs (1) an embedding-based knowledge graph completion module to derive missing edges, (2) and a reconstruction module of unlabeled EHR data to select high-quality ones from these edges, which would be further appended to the MKG to update the embedding model. Through the iterations, the two modules mutually benefit each other. Then, MendMKG uses the pre-trained graph attention network and the updated MKG to generate the visit embeddings to represent patient’s historical visits, and predict the diagnosis in future visit, through a fine-tuning approach. Abstract:Recently, Electronic Health Records (EHR) have become valuable for enhancing medical decision making, as well as online disease detection and monitoring. Meanwhile, deep learning-based methods have achieved great success in health risk prediction and diagnosis prediction based on EHR. Nevertheless, deep learning-based models usually require high volumes of data due to the vast amount of parameters. In addition, a considerable proportion of medical codes appear rarely in the EHR data which poses huge difficulties for clinical applications. Hence, some works propose to adopt medical ontologies to enhance the prediction performance and provide interpretable prediction results. However, these medical ontologies are often small-scale and coarse-grained, most of diagnoses and medical concepts are not included, lacking many diagnoses and medical concepts, let alone various relationships between these concepts. To overcome this limitation, we propose to incorporate existing large-scale medical knowledge graphs (KGs) into diagnosis prediction and devise a Stage-aware Hierarchical Attentive Relational Network, named HAR. Specifically, for each visit, a personalized sub-KG is extracted from the existing medical KG, on which HAR conducts relation-specific message passing and hierarchical message aggregation to refine representations of nodes that correspond to medical codes in visits. HAR takes the specific stage of a patient's disease progression into consideration, which participates in the computation of relation-level and node-level attention. Extensive experiments on two public datasets demonstrate the effectiveness of HAR in improving both the visit-level precision and code-level accuracy of the diagnosis prediction task.

Abstract:Non-used clinical information has negative implications on healthcare quality. Clinicians pay priority attention to clinical information relevant to their specialties during routine clinical practices but may be insensitive or less concerned about information showing disease risks beyond their specialties, resulting in delayed and missed diagnoses or improper management. In this study, we introduced an electronic health record (EHR)-oriented knowledge graph system to efficiently utilize non-used information buried in EHRs. EHR data were transformed into a semantic patient-centralized information model under the ontology structure of a knowledge graph. The knowledge graph then creates an EHR data trajectory and performs reasoning through semantic rules to identify important clinical findings within EHR data. A graphical reasoning pathway illustrates the reasoning footage and explains the clinical significance for clinicians to better understand the neglected information. An application study was performed to evaluate unconsidered chronic kidney disease (CKD) reminding for non-nephrology clinicians to identify important neglected information. The study covered 71,679 patients in non-nephrology departments. The system identified 2,774 patients meeting CKD diagnosis criteria and 10,377 patients requiring high attention. A follow-up study of 5,439 patients showed that 82.1% of patients who met the diagnosis criteria and 61.4% of patients requiring high attention were confirmed to be CKD positive during follow-up research. The application demonstrated that the proposed approach is feasible and effective in clinical information utilization. Additionally, it's valuable as an explainable artificial intelligence to provide interpretable recommendations for specialist physicians to understand the importance of non-used data and make comprehensive decisions.

Abstract:Accurate and explainable health event predictions are becoming crucial for healthcare providers to develop care plans for patients. The availability of electronic health records (EHR) has enabled machine learning advances in providing these predictions. However, many deep learning based methods are not satisfactory in solving several key challenges: 1) effectively utilizing disease domain knowledge; 2) collaboratively learning representations of patients and diseases; and 3) incorporating unstructured text. To address these issues, we propose a collaborative graph learning model to explore patient-disease interactions and medical domain knowledge. Our solution is able to capture structural features of both patients and diseases. The proposed model also utilizes unstructured text data by employing an attention regulation strategy and then integrates attentive text features into a sequential learning process. We conduct extensive experiments on two important healthcare problems to show the competitive prediction performance of the proposed method compared with various state-of-the-art models. We also confirm the effectiveness of learned representations and model interpretability by a set of ablation and case studies.

Abstract:In this paper, RotatE model is trained by medical knowledge base to realize triplet extraction and inference of complex medical knowledge respectively. Based on these two models, this paper constructs a disease-centered medical knowledge map. At the same time, an intelligent medical system is developed based on the constructed knowledge graph. The system can diagnose diseases and deduce complications. The intelligent medical system extracts the triplet of symptoms input by patients through the TPLinker model, and standardizes the corresponding text. In the process of disease diagnosis, based on the RotatE model, the system deduces the relationship between multiple “disease” entities in the medical knowledge graph, which can not only predict the disease suffered by patients, but also deduce the possible complications caused by the disease. This provides theoretical methods and practical examples for the realization of intelligent medicine.

Abstract:The rapid growth of electronic health record (EHR) datasets opens up promising opportunities to understand human diseases in a systematic way. However, effective extraction of clinical knowledge from the EHR data has been hindered by its sparsity and noisy information. We present KG-ETM, an end-to-end knowledge graph-based multimodal embedded topic model. KG-ETM distills latent disease topics from EHR data by learning the embedding from the medical knowledge graphs. We applied KG-ETM to a large-scale EHR dataset consisting of over 1 million patients. We evaluated its performance based on EHR reconstruction and drug imputation. KG-ETM demonstrated superior performance over the alternative methods on both tasks. Moreover, our model learned clinically meaningful graph-informed embedding of the EHR codes. In additional, our model is also able to discover interpretable and accurate patient representations for patient stratification and drug recommendations.
Reasoning-Enhanced Healthcare Predictions with Knowledge Graph Community Retrieval

Abstract:Large language models (LLMs) have demonstrated significant potential in clinical decision support. Yet LLMs still suffer from hallucinations and lack fine-grained contextual medical knowledge, limiting their high-stake healthcare applications such as clinical diagnosis. Traditional retrieval-augmented generation (RAG) methods attempt to address these limitations but frequently retrieve sparse or irrelevant information, undermining prediction accuracy. We introduce KARE, a novel framework that integrates knowledge graph (KG) community-level retrieval with LLM reasoning to enhance healthcare predictions. KARE constructs a comprehensive multi-source KG by integrating biomedical databases, clinical literature, and LLM-generated insights, and organizes it using hierarchical graph community detection and summarization for precise and contextually relevant information retrieval. Our key innovations include: (1) a dense medical knowledge structuring approach enabling accurate retrieval of relevant information; (2) a dynamic knowledge retrieval mechanism that enriches patient contexts with focused, multi-faceted medical insights; and (3) a reasoning-enhanced prediction framework that leverages these enriched contexts to produce both accurate and interpretable clinical predictions. Extensive experiments demonstrate that KARE outperforms leading models by up to 10.8-15.0% on MIMIC-III and 12.6-12.7% on MIMIC-IV for mortality and readmission predictions. In addition to its impressive prediction accuracy, our framework leverages the reasoning capabilities of LLMs, enhancing the trustworthiness of clinical predictions.

Abstract:Electronic Health Records (EHRs) and routine documentation practices play a vital role in patients' daily care, providing a holistic record of health, diagnoses, and treatment. However, complex and verbose EHR narratives overload healthcare providers, risking diagnostic inaccuracies. While Large Language Models (LLMs) have showcased their potential in diverse language tasks, their application in the healthcare arena needs to ensure the minimization of diagnostic errors and the prevention of patient harm. In this paper, we outline an innovative approach for augmenting the proficiency of LLMs in the realm of automated diagnosis generation, achieved through the incorporation of a medical knowledge graph (KG) and a novel graph model: Dr.Knows, inspired by the clinical diagnostic reasoning process. We derive the KG from the National Library of Medicine's Unified Medical Language System (UMLS), a robust repository of biomedical knowledge. Our method negates the need for pre-training and instead leverages the KG as an auxiliary instrument aiding in the interpretation and summarization of complex medical concepts. Using real-world hospital datasets, our experimental results demonstrate that the proposed approach of combining LLMs with KG has the potential to improve the accuracy of automated diagnosis generation. More importantly, our approach offers an explainable diagnostic pathway, edging us closer to the realization of AI-augmented diagnostic decision support systems.

Abstract:Mental and physical disorders (MPD) are inextricably linked in many medical cases; psychosomatic diseases can be induced by mental concerns and psychological discomfort can ensue from physiological diseases. However, existing medical informatics studies focus on identifying mental or physical disorders from a unilateral perspective. Consequently, no existing domain knowledge base, corpus, or detection modeling approach considers mental as well as physical aspects concurrently. This paper proposes a joint modeling approach to detect MPD. First, we crawl through online medical consultation records of patients from websites and build an MPD knowledge ontology by extracting the core conceptual features of the text. Based on the ontology, an MPD knowledge graph containing 12,673 nodes and 82,195 relations is obtained using term matching with a domain thesaurus of each concept. Subsequently, an MPD corpus with fine-grained severities (None, Mild, Moderate, Severe, Dangerous) and 8909 records is constructed by formulating MPD classification criteria and a data annotation process under the guidance of domain experts. Taking the knowledge graph and corpus as the dataset, we design a multi-task learning model to detect the MPD severity, in which a knowledge graph attention network (KGAT) is embedded to better extract knowledge features. Experiments are performed to demonstrate the effectiveness of our model. Furthermore, we employ ontology-based and centrality-based methods to discover additional potential inferred knowledge, which can be captured by KGAT so as to improve the prediction performance and interpretability of our model. Our dataset has been made publicly available, so it can be further used as a medical informatics reference in the fields of psychosomatic medicine, psychiatrics, physical co-morbidity, and so on.
Time-aware Heterogeneous Graph Transformer with Adaptive Attention Merging for Health Event Prediction

Abstract:The widespread application of Electronic Health Records (EHR) data in the medical field has led to early successes in disease risk prediction using deep learning methods. These methods typically require extensive data for training due to their large parameter sets. However, existing works do not exploit the full potential of EHR data. A significant challenge arises from the infrequent occurrence of many medical codes within EHR data, limiting their clinical applicability. Current research often lacks in critical areas: 1) incorporating disease domain knowledge; 2) heterogeneously learning disease representations with rich meanings; 3) capturing the temporal dynamics of disease progression. To overcome these limitations, we introduce a novel heterogeneous graph learning model designed to assimilate disease domain knowledge and elucidate the intricate relationships between drugs and diseases. This model innovatively incorporates temporal data into visit-level embeddings and leverages a time-aware transformer alongside an adaptive attention mechanism to produce patient representations. When evaluated on two healthcare datasets, our approach demonstrated notable enhancements in both prediction accuracy and interpretability over existing methodologies, signifying a substantial advancement towards personalized and proactive healthcare management.

Leveraging Representation Learning for the Construction and Application of a Knowledge Graph for Traditional Chinese Medicine: Framework Development Study

Abstract:Background: Knowledge discovery from treatment data records from Chinese physicians is a dramatic challenge in the application of artificial intelligence (AI) models to the research of traditional Chinese medicine (TCM). Objective: This paper aims to construct a TCM knowledge graph (KG) from Chinese physicians and apply it to the decision-making related to diagnosis and treatment in TCM. Methods: A new framework leveraging a representation learning method for TCM KG construction and application was designed. A transformer-based Contextualized Knowledge Graph Embedding (CoKE) model was applied to KG representation learning and knowledge distillation. Automatic identification and expansion of multihop relations were integrated with the CoKE model as a pipeline. Based on the framework, a TCM KG containing 59,882 entities (eg, diseases, symptoms, examinations, drugs), 17 relations, and 604,700 triples was constructed. The framework was validated through a link predication task. Results: Experiments showed that the framework outperforms a set of baseline models in the link prediction task using the standard metrics mean reciprocal rank (MRR) and Hits@N. The knowledge graph embedding (KGE) multitagged TCM discriminative diagnosis metrics also indicated the improvement of our framework compared with the baseline models. Conclusions: Experiments showed that the clinical KG representation learning and application framework is effective for knowledge discovery and decision-making assistance in diagnosis and treatment. Our framework shows superiority of application prospects in tasks such as KG-fused multimodal information diagnosis, KGE-based text classification, and knowledge inference–based medical question answering.

Knowledge-Induced Medicine Prescribing Network for Medication Recommendation

Abstract:Extensive adoption of electronic health records (EHRs) offers opportunities for their use in various downstream clinical analyses. To accomplish this purpose, enriching an EHR cohort with external knowledge (e.g., standardized medical ontology and wealthy semantics) could help us reveal more comprehensive insights via a spectrum of informative relations among medical codes. Nevertheless, harnessing those beneficial interconnections was scarcely exercised, especially in the medication recommendation task. This study proposes a novel Knowledge-Induced Medicine Prescribing Network (KindMed) to recommend medicines by inducing knowledge from myriad medical-related external sources upon the EHR cohort and rendering interconnected medical codes as medical knowledge graphs (KGs). On top of relation-aware graph representation learning to obtain an adequate embedding over such KGs, we leverage hierarchical sequence learning to discover and fuse temporal dynamics of clinical (i.e., diagnosis and procedures) and medicine streams across patients' historical admissions to foster personalized recommendations. Eventually, we employ attentive prescribing that accounts for three essential patient representations, i.e., a summary of joint historical medical records, clinical progression, and the current clinical state of patients. We validated the effectiveness of our KindMed on the augmented real-world EHR cohorts, achieving improved recommendation performances against a handful of graph-driven baselines.

Towards a Personal Health Knowledge Graph Framework for Patient Monitoring

Abstract:Healthcare providers face significant challenges with monitoring and managing patient data outside of clinics, particularly with insufficient resources and limited feedback on their patients' conditions. Effective management of these symptoms and exploration of larger bodies of data are vital for maintaining long-term quality of life and preventing late interventions. In this paper, we propose a framework for constructing personal health knowledge graphs from heterogeneous data sources. Our approach integrates clinical databases, relevant ontologies and standard healthcare guidelines to support alert generation, clinician interpretation and querying of patient data. Through a use case of monitoring Chronic Obstructive Pulmonary Disease (COPD) patients, we demonstrate that inference and reasoning on personal health knowledge graphs built with our framework can aid in patient monitoring and enhance the efficacy and accuracy of patient data queries.

Abstract:While large language models (LLMs) have showcased their potential in diverse language tasks, their application in the healthcare arena needs to ensure the minimization of diagnostic errors and the prevention of patient harm. A medical knowledge graph (KG) houses a wealth of structured medical concept relations sourced from authoritative references, such as UMLS, making it a valuable resource to ground LLM diagnostic process in knowledge. In this paper, we examine the synergistic potential of LLMs and medical KG in predicting diagnoses given electronic health records (EHR), under the framework of Retrieval-augmented generation (RAG). We proposed a novel graph model: DR.KNOWS, that selects the most relevant pathology knowledge paths based on the medical problem descriptions. In order to evaluate DR.KNOWS, we developed the first comprehensive human evaluation approach to assess the performance of LLMs for diagnosis prediction and examine the rationale behind their decision-making processes, aimed at improving diagnostic safety. Using real-world hospital datasets, our study serves to enrich the discourse on the role of medical KGs in grounding medical knowledge into LLMs, revealing both challenges and opportunities in harnessing external knowledge for explainable diagnostic pathway and the realization of AI-augmented diagnostic decision support systems.

Abstract:Predicting a patient’s future health condition by analyzing their Electronic Health Records (EHRs) is a trending subject in the intelligent medical field, which can help clinicians prescribe safely and effectively, and also make more accurate diagnoses. Benefiting from powerful feature extraction capabilities, graph representation learning can capture complex relationships and achieve promising performance in many clinical prediction tasks. However, existing works either exclusively consider single domain knowledge with an independent task or do not fully capitalize on domain knowledge that can provide more predictive signals in the code encoding stage. Moreover, the heterogeneous and high-dimensional nature of EHR data leads to a deficiency of hardly encoding implicit high-order correlations. To address these limitations, we proposed a knowledge-guided Multi-viEw hyperGrAph predictive framework (MEGACare) for diagnosis prediction and medication recommendation. Our MEGACare leveraged multi-faceted medical knowledge, including ontology structure, code description, and molecular information to enhance medical code presentations. Furthermore, we constructed an EHR hypergraph and a multi-view learning framework to capture the high-order correlation between patient visits and medical codes. Specifically, we propose three perspectives around the pairwise relationship between patient visits and medical codes to comprehensively learn patient representation and enhance the robustness of our framework. We evaluated our MEGACare framework against a set of state-of-the-art methods for two clinical outcome prediction tasks in the public MIMIC-III dataset, and the results showed that our proposed framework was superior to the baseline methods. 1 1 Our code and data are released at https://github.com/senticnet/MEGACare.

Robustly Extracting Medical Knowledge from EHRs: A Case Study of Learning a Health Knowledge Graph

Abstract:Increasingly large electronic health records (EHRs) provide an opportunity to algorithmically learn medical knowledge. In one prominent example, a causal health knowledge graph could learn relationships between diseases and symptoms and then serve as a diagnostic tool to be refined with additional clinical input. Prior research has demonstrated the ability to construct such a graph from over 270,000 emergency department patient visits. In this work, we describe methods to evaluate a health knowledge graph for robustness. Moving beyond precision and recall, we analyze for which diseases and for which patients the graph is most accurate. We identify sample size and unmeasured confounders as major sources of error in the health knowledge graph. We introduce a method to leverage non-linear functions in building the causal graph to better understand existing model assumptions. Finally, to assess model generalizability, we extend to a larger set of complete patient visits within a hospital system. We conclude with a discussion on how to robustly extract medical knowledge from EHRs.
