Abstract:Objective Although Pap smear (PS) is considered the best standard in detecting cervical cancer, adherence to timely and regular PS is often lower than global standards in developing countries. Thus, the present study aimed to identify barriers and facilitators of adherence to cervical cancer screening in Mashhad, Iran. Design A qualitative content analysis was done from July to December 2022 using semistructured in-depth interviews. Setting The study was conducted in health centres in Mashhad, Iran. Participants A sample of 36 married women aged 18–70 years was selected using a purposive sampling, with maximum diversity. Results The mean age of participants was 42.8±7.6 years. Among all, 66.7% had the PS test at least once; only 8.3% regularly had the test. The qualitative content analysis led to the extraction of four major themes: (1) individual challenges, (2) environmental limitations, (3) individual motivators, and (4) supportive and efficient environments. The most significant barriers were psychological stress, unhealthy attitudes, insufficient information, cultural issues and insufficient healthcare services. Facilitators included an active and efficient healthcare system, advice and support of important others, and positive emotions and individual beliefs. Conclusions Participants' perceptions showed that the main factors influencing the PS testing were supportive environments, individual motivators, individual challenges and environmental limitations. To encourage women to take the PS and reduce barriers, it may be necessary to revise current health system policies, promote individual and public awareness, reduce psychological stress and correct negative beliefs and attitudes.

Abstract:Background and Objectives: Cervical cancer is considered as a preventable cancer in women. Pap smear test is an effective screening program for diagnosing cervical cancer, but for some reasons its use is low. The purpose of this study was to determine the factors affecting the women's participation in the Pap smear test based on precaution adoption process model and effect of educational program based on model on doing the Pap test with a Mixed Method approach. Materials and Methods: This study was a Mixed Method approach. Thirty women voluntarily participated in this study in sarvabad city of Iran. The sampling began purposefully and continued until saturation. Semi-structured interviews were the primary method of data collection. Data were analyzed using qualitative content analysis and continuous comparisons. Quantitative study was conducted implementing a quasi-experimental method. Participants were divided into two groups as experimental and control groups using Precaution Adoption Process Model (PAPM) among 180 rural females in Sarvabad. In treating the experimental group, the methods of lecturing with question and answer, playing videos, specialized consulting and pamphlet were used. The questionnaire including a demographic characteristic and variables that affecting the stages of the Precaution Adoption Process Model (PAPM). Data analysis was performed using the Mann-Whitney, chi-square and descriptive statistical methods. P <0.05 was considered as the significance level. Results: In qualitative study Model-based themes included, awareness, perceived susceptibility, perceived severity, social norms. Women`s awareness about cervical cancer and Pap smear test is not enough, but, they perceived the severity of the disease. In quantitative study in comparison of the means of the scores of psychological variables affecting the decision process in Pap smear test, there was a significant relationship between the experimental and control groups based on Mann-Whitney test and control groups based on variables, perceived susceptibility, awareness and social norms. But the perceived severity was not statistically significant. Conclusion: Findings of this study will help health managers and health planners to plan and train ways to facilitate the participation of women in the Pap smear test.

Abstract:Abstract Objective Cervical cancer is considered treatable as long as it is detected early and managed effectively. Pap smear test is a screening tool that plays an important role in the early detection, prevention and can prevent any early cervical cell changes from becoming cancer. This study aims to survey the effect of educational programs based on beliefs, subjective norms, and perceived behavior control on doing the pap-smear test in a sample of Iranian women. Materials and methods This experimental interventional study was performed on 300 women admitted to Fasa City, Fars Province, Iran health centers in 2018–2019. A questionnaire consisting of demographic information, knowledge, Health Belief Model, and Theory of Planned Behavior constructs were used to measure on doing of Pap smear test in women before and after 6 months’ educational intervention. Results The results revealed that 6 months after the intervention, 108 women (72%) in the experimental group and only 9 women (6%) in the control group received the Pap smear test. Conclusions The current research results revealed that education based on the combination of the health Belief model and theory of planned behavior might be promoting participation and an increasing rate of receiving Pap smear tests in women.
Breast Cancer Screening Barriers from the Womans Perspective: a Meta-synthesis

Abstract:Background: The principal aim of health service providers in the field of breast cancer is to detect and treat lesions at an appropriate time. Therefore, identification of barriers to screening can be very helpful. The present study aimed to systematically review the qualitative studies for extracting and reporting the barriers of screening for breast cancer from the womans perspective. Materials and methods: In this systematic review; Pubmed, Google Scholar, Ovid Scopus, Cochrane Library, Iranmedex, and SID were searched using the keywords: screening barriers, cancer, qualitative studies, breast and their Persian equivalents, and the needed data were extracted and analyzed using an extraction table. To assess the quality of the studies, the Critical Appraisal Skills Programme (CASP) tool was used. Results: From 2,134 related articles that were found, 21 articles were eventually included in the study. The most important barriers from the point of view of 1,084 women were lack of knowledge, access barriers (financial, geographical, cultural), fear (of results and pain), performance of service providers, women's beliefs, procrastination of screening, embarrassment, long wait for getting an appointment, language problems, and previous negative experiences. Articles' assessment score was 68.9. Conclusions: Increasing women's knowledge, reducing the costs of screening services, cultural promotion for screening, presenting less painful methods, changing beliefs of health service providers, provision of privacy for giving service, decreasing the waiting time, and providing high quality services in a respectful manner can be effective ways to increase breast cancer screening.
Predicting cervical cancer screening participation using self-care behaviors among women in Iran

Abstract:Background: Cervical cancer screening is an effective and accessible method for preventing this cancer. However, low participation rates among women have been reported. Self-care is one of the solutions to improve access to health services. This study was conducted to determine the prediction of cervical cancer screening participation using self-care behaviors among women in Iran. Materials and methods: This cross-sectional study was conducted on 310 eligible women who were referred to comprehensive health centers and women's clinics in teaching hospitals in Isfahan, Iran, from November 2020 to April 2021. Participants were enrolled using convenience sampling. The data collection tool included researcher-made questionnaires on personal and fertility characteristics, participation in cervical cancer screening, and self-care behaviors related to cervical cancer and its screening. Descriptive and inferential statistical methods were used for data analysis using the Statistical Package for the Social Sciences (SPSS) version 22 software. Results: The results showed that the intention to undergo screening was low among individuals who had not undergone screening. Lack of awareness and not having enough time were the most common barriers to screening. The results of logistic regression analysis indicated that self-efficacy was the significant predictor of cervical cancer screening. With an increase in the self-care score, the 12% chance of doing a Pap smear increases significantly (P = 0.002). Furthermore, the results of multiple regression showed that with an increase in the self-care score, the chance of women who refer to screening every year, every 2-3 years, and every 4-5 years is increased to 25% (P = 0.001), 34% (P < 0.001), and 11% (P = 0.032), respectively, compared with non-referral. Discussion: According to the results, self-care was a predictor of performing a Pap smear, and it was related to its regular performance of Pap smear too. Therefore, designing and implementing necessary interventions to increase self-care behaviors can improve women's participation in cervical cancer screening and its regularity.

Abstract:BackgroundDespite increasing numbers of Iranian-American physicians practicing in the United States, little is known about the barriers that may impact them as providers of sexual health care. This is an important topic as discussions of sexual topics are generally considered a taboo among Iranians. We aimed to identify barriers experienced by Iranian-American physicians that inhibit their willingness to engage in discussions of sexual health care with patients.MethodsIn 2013, a self-administrated questionnaire was sent to 1,550 Iranian-American physicians in California. Questions included demographics of the physicians as well as their perception of challenges in discussing various sexual health topics with their patients. Factor analysis: Principal components approach with a Varimax rotation was used to detect latent factors within the data that may help explain possible barriers to discussion of sexual health among physicians. The analysis was performed on 11 items, specifically focused on possible barriers, to detect a possible relationship between correlated variables within the data to produce a set of uncorrelated variables (factors).ResultsThe overall response rate was 23 %. Data revealed specific barriers regarding sexual history taking, discussing STIs and sexual dysfunctions with patients based on their gender, and age. Three factors were identified as internally consistent (Cronbach’s alpha = 0.82 to 0.91): (i) embarrassment, (ii) cultural and religious, (iii) lack of time and financial constraint. Significant associations were found between these 3 factors and some variables such as: country of medical graduation, religious affiliation, birthplace, age, and gender.ConclusionsOur findings are the first to identify possible barriers among Iranian-American physicians in delivering effective sexual health care to patients. Additional studies from Iranian-American physicians as well as from other foreign-born/subpopulation of US physicians populations and mainstream US physicians are needed to assess the extent of such barriers, and changes over time. Effective strategies to better engage such physicians in these studies are needed. If confirmed from other studies, our findings could have implications for the training of US medical graduates.

Abstract:Cervical cancer, along with other sexual and reproductive health and rights (SRHR) conditions, poses a significant burden in the Kingdom of Saudi Arabia (KSA). Despite the availability of effective preventive methods such as vaccinations, particularly against the Human Papillomavirus (HPV), awareness about such preventive methods and HPV vaccination remains alarmingly low in the KSA, even with governmental effort and support. While many women are aware of the risks, the uptake of the HPV vaccine remains below 10% (7.6%) at the country level. This highlights the urgent need for Knowledge, Attitude, and Practice (KAP) at the community level to raise awareness, dispel misconceptions, and empower women to embrace vaccinations. Additionally, there is a need to revitalize the cancer registry system to better track and monitor cervical cancer cases. This short communication aims to map these barriers while identifying opportunities for impactful research. Drawing from the scientific literature, government reports, and expert insights, we highlight the challenges surrounding the tackling of HPV. By exploring diverse sources of knowledge, this paper not only highlights current obstacles but also proposes actionable solutions for future interventions.

Abstract:The aim of this study was to explore issues that challenge menopausal women in discussions of their sexual problems with a physician. This was done from the perspective of healthcare providers. In a descriptive exploratory qualitative study, using a semi-structured interview and purposive sampling, a sample set of 12 midwives and 13 general practitioners aged 25-70 years were selected in order to elicit meaning behind their experiences about the subject under study. Data analysis was carried out using qualitative content analysis. Results were used to identify a number of obstacles that hindered women from seeking help for sexual problems from GPs and midwives. These obstacles included the following: (1) traditional and cultural beliefs; (2) religious belief; (3) individuals' beliefs and (4) access to services. More research is needed to explore effective strategies to overcome these problems. Impact statement Current knowledge on the subject: In the literature, many reasons have been identified for the unwillingness of Iranian women to discuss their sexual problems with health providers. These include lack of time, feelings of shame and an expectation that a doctor cannot help. However, no qualitative study has addressed barriers held by menopausal women for seeking treatment for sexual problems. The contribution made by the results of this study: The results of this study add to the growing body of research on reasons that determine why most postmenopausal women rarely visit a doctor unless they were in tremendous physical or emotional pain. Also, menopausal women thought that an unmarried health provider would be less understanding about sexual and marital problems and they felt guilty about sharing such issues with them. Patients' opinions on the nature of menopause (a pathological vs. physiological process) affect the way in which the symptoms of menopause and sexual problems are handled by patient. The implications are of these findings for clinical practice and further research: The results of this study can be used to formulate guidelines to cultural sensitivity appropriate to Iranian society.

Abstract:OBJECTIVES: In women, breast cancer is the most common cancer and the leading cause of cancer death. Screening tests are the basis for early diagnosis. In Iranian women, the mortality rate of breast cancer is high due to insufficient screening examinations and delayed visits for care. Therefore, this study aimed to determine the factors affecting breast self-examinations among Iranian women employed in medical careers.METHODS: This cross-sectional study included 501 women working in the medical professions at Hamadan University of Medical Sciences in western Iran in 2018. The subjects were selected by stratified random sampling. Data were collected using a researcher-developed, self-report questionnaire that contained demographic information and questions based on protection motivation theory and social support theory. Descriptive data analysis was conducted using SPSS version 23 and model fitting with PLS version 2.RESULTS: The mean age of the participants was 37.1±8.3 years, and most of the women (80.4%) were married. Most women had a bachelor's degree (67.5%). The findings of this study showed that the coping appraisal construct was a predictor of protection motivation (β=0.380, p<0.05), and protection motivation (β=0.604, p<0.05) was a predictor of breast self-examination behavior. Additionally, social support theory (β=0.202, p<0.05) had a significant positive effect on breast self-examination behavior.CONCLUSIONS: The frequency of practicing self-examinations among women employees in the medical sector was low; considering the influence of social support as a factor promoting screening, it is necessary to pay attention to influential people in women's lives when designing educational interventions.

Abstract:Background: A Pap smear test is a microscopic examination of uterine cervix which can detect the precancerous changes in the cervix (cervical dysplasia). Presence of human papilloma virus (HPV) and presence of precancerous changes in the cervical cells confirms the diagnosis. So early detection of HPV (causative factor) and precancerous changes in the cervix (cervical dysplasia) can help in early treatment of cervical cancer and complete cure. Being asymptomatic at presentation it is important to screen all the women at risk of getting cervical cancer for early diagnosis and its secondary prevention. Knowledge is one of the important determinants predicting health behaviors and an effective factor in performing screening methods in the population. However, acquiring knowledge only is not adequate and success of a prevention programme depends on the positive preventive attitude and correct practice of the population. The present study was done to assess the knowledge, attitude and practice of the healthcare workers at MCH hospital, Al Ahsa Saudi Arabia towards the Pap smear screening test. Results: A total of 217 out of 258 staffs responded the online questionnaires making the response rate of 85%. The mean age of the participants was 32. 83 ± 7. 20 Std. Dev. (Range 21-61 years). Majority of the participants (N= 180, 82. 9%) were married. Seventy percent of the participants (N=152) were staff nurse. Sixty four percent of the participants were graduate (N=139). The mean knowledge of the participants was 6. 31 ± 3. 24 Std. Dev. (Range: 2 -10). More than fifty percent of the participant (N=109) did not have good knowledge about Pap smear test. The mean of positive attitude score was 3. 56 ±1. 19 Std. Dev. (Range: 00-5). Fifty eight percent of the participant (N=128) had positive attitude towards Pap smear test. The mean of practice score was 2. 21 ±1. 00 Std. Dev. (Range: 00 – 4). Sixty eight percent of the participants (N=191) did not have good practice towards Pap smear test. The attitude of staff nurses was significantly positive (p=0. 024) than those of other medical staffs. The practice of the nursing staffs towards the Pap smear test was significantly negative (P=0. 000) than those of other medical staff. There were no significant difference in the knowledge, attitude and practice score between different age groups and marital status of the participants. Conclusion: This study has demonstrated a moderate level of knowledge and attitude among the paramedical and nursing staffs of Maternity and Child Hospital towards Pap smear screening test. However good knowledge and attitude did not translate into good practice in most of them especially among the nursing staffs. The rate of uptake for Pap smear test was low. The major reason for low uptake of Pap smear screening among the respondents include, no recommendation by any doctors, embarrassment, lack of knowledge about the Pap smear, lack of time and fear of the test result.
Socioeconomic disparities in Papanicolaou test utilization in Western Iran

Abstract:Abstract Background Cervical cancer remains the fourth most frequently diagnosed cancer among women, and its impact is particularly significant in women residing in less developed countries. This study aims to assess socioeconomic inequality in using Papanicolaou tests, commonly known as Pap tests, which are crucial for detecting cervical cancer. The research also seeks to decompose this inequality, identifying its contributing factors. This investigation is conducted within a sizable population-based study focused on the Kurdish population, with an additional examination of potential variations between urban and rural areas. Method The study utilized baseline data from the Ravansar Non-Communicable Disease Cohort Study (RaNCD), involving 3,074 ever-married women aged 35–65. Asset data was employed to determine socioeconomic status (SES), and Principal Component Analysis was applied. The uptake of Papanicolaou tests was assessed for inequality using the Concentration Index (Cn). Additionally, decomposition analysis was conducted to identify and understand the factors contributing to socioeconomic inequality. Results The study found that overall, 86% of women reported having undergone cervical cancer screening at least once in their lifetime. The Concentration Index (Cn) for the total population was 0.21 ( p < 0.0001), indicating a higher concentration of Papanicolaou test uptake among wealthier groups. In urban areas, the Cn was 0.34 ( p < 0.0001), reflecting a significant concentration among the rich. However, in rural areas, the Cn was -0.10 ( p = 0.3006), suggesting no significant socioeconomic inequality. Factors such as socioeconomic status (SES), education, and age contributed to reducing inequality, explaining 62.7%, 36.0%, and 1.7% of the observed inequality, respectively. Interestingly, place of residence had a negative influence on inequality. Conclusion The uptake of Papanicolaou tests varies across different socioeconomic status levels, with a higher concentration among wealthier groups. The results enable health policymakers and researchers to tailor health intervention toward increasing public awareness, especially among women with lower levels of education women in economically deprived groups.

Abstract:Our aim in this cross-sectional study was to assess the cognitive determinants of Cervical Cancer Screening Behavior (CCSB) among housewife women in Islamabad County, Iran. Through multistage random sampling we recruited and interviewed 280 housewife women. The women who perceived more benefits of performing the Pap test (OR = 1.11), and perceived fewer barriers (OR = 0.915), and higher self-efficacy to perform the test (OR = 1.12) were more likely to have a CCSB in the previous three years. Our findings are informative for the development of targeted interventions to foster CCSB among housewife women.

Abstract:BACKGROUND: Participation of Iranian women with a family history of breast cancer in breast cancer screening programs is low. This study evaluates the compliance of women having a family history of breast cancer with clinical breast exam (CBE) according to the stage of transtheoretical model (TTM) and health belief model (HBM).MATERIALS AND METHODS: In this cross-sectional study, we used Persian version of champion's HBM scale to collect factors associated with TTM stages applied to screening from women over 20 years and older. The obtained data were analyzed by SPSS, using descriptive statistics, Chi-square test, independent t-test, and analysis of covariance.RESULTS: Final sample size was 162 women. Thirty-three percent were in action/maintenance stage. Older women, family history of breast cancer in first-degree relatives, personal history of breast disease, insurance coverage, and a history of breast self-examination were associated with action/maintenance stage. Furthermore, women in action/maintenance stages had significantly fewer perceived barriers in terms of CBE in comparison to women in other stages (P < 0.05). There was no significant difference in other HBM subscales scores between various stages of CBE screening behavior (P > 0.05).CONCLUSION: The finding indicates that the rate of women in action/maintenance stage of CBE is low. Moreover, results show a strong association between perceived barriers and having a regular CBE. These clarify the necessity of promoting national target programs for breast cancer screening, which should be considered as the first preference for reducing CBE barriers.

Creation of H-2 class I epitopes using synthetic peptides: recognition by alloreactive cytotoxic T lymphocytes.

Abstract:A major role that cytotoxic T lymphocytes (CTLs) play in the immune response is the specific destruction of viral-infected cells and tissue from foreign grafts. Class I molecules encoded within the major histocompatibility complex are the target structures for these CTLs. Recently, viral-restricted CTLs have been shown to recognize viral peptides in association with class I molecules, while several studies with cloned alloreactive CTLs have indicated that these T cells can recognize class I-derived peptides in association with class I molecules. Together, these observations suggest that peptide binding is an important function of class I molecules. In this paper, we show that the recognition of a particular class I molecule by a bulk population of alloreactive CTLs can be altered by incubating with it a peptide derived from another class I molecule. Specifically, we used the hybrid D/Ldm1 molecule as a target structure together with the peptide Ld61-85, and we have shown that their associative recognition by Ld-specific CTLs depends on sequence and configuration of the peptide and is specific for Ld using a cold-target inhibition assay. Our results are discussed in light of three possible models for the target structure(s) that can be recognized by alloreactive CTLs and in terms of the role peptides may play during allorecognition.
Evaluation of Health-Care Providers' Perception of Spiritual Care and the Obstacles to Its Implementation

Abstract:OBJECTIVE: Cancer patients face many health challenges, including spiritual issues. Therefore, an awareness of health-care providers' perspective on spiritual care provision is important. This study aimed to determine health-care providers' perception of spiritual care and to examine the individual barriers to its implementation in cancer patients.METHODS: The present descriptive study included 136 physicians and nurses. The Spiritual Care Survey was used as a research tool. Data were analyzed through descriptive statistics using IBM SPSS Statistics for Windows, version 20.0.RESULTS: In this study, 70.6% of the participants considered spiritual care to be influential in the patients' quality of life. However, 64.7% had received no spiritual care training, while 82.4% indicated a willingness to attend these courses. Regarding the obstacles to providing spiritual care, the highest and lowest scores, respectively, belonged to the lack of time and the person's reluctance to talk about spiritual issues.CONCLUSIONS: Spiritual care has not yet found its proper place in the care setting of Iran, and health-care team members do not have sufficient training to provide this kind of care despite their belief in its positive impact on patients' quality of life.

Abstract:Abstract Background The HPV vaccine prevents the most common cancer in women, which is cervical cancer. This study aimed to determine the factors affecting the uptake and non- uptake of HPV vaccine in the young adult in Tehran and strategies to increase their vaccine uptake. Methods This is the first qualitative study with the conventional content analysis approach in this context in Iran in 2018-2019. Participants, with a maximum variation in terms of age, sex, educational level, occupation, income, work experience, marital status, were selected purposefully to take part in a semi-structured interview. The data were recorded and analyzed according to the interview continued until data saturation. MAXQDA software version 10 was used for data management. Results The participants were 20 health professionals and 10 young adults from Tehran. Factors affecting HPV vaccine uptake and non- uptake identified as seven categories, including knowledge, health value, external stimuli, outcome expectations, perceived threat, healthcare services, and Contextual factors. Moreover, the strategies to increase vaccine uptake included Compulsory vaccination, vaccine availability, and implementing the educational process. Conclusions Considering the factors identified in this study, the authorities and practitioners must take steps to increase the HPV vaccine in young adults by considering the effective factors, as well as the strategies, such as Compulsory vaccination, increasing vaccine availability, and implementing educational interventions.
Challanges of meeting the palliative care needs of colorectal cancer patients in Iran: A qualitative research

Abstract:Background: Despite the high prevalence of Colorectal Cancer (CRC) in Iran and the need to pay more attention to the Palliative Care (PC) needs of patients with this disease, a few studies have previously examined the PC needs of them by gaining the patients', family members', and treatment team's views. This study aimed to investigate the challenges in the way of meeting the PC needs of CRC patients. Materials and Methods: This study was a qualitative content analysis study conducted on 43 participants (including 15 patients, 20 health care providers, and eight family members) through purposive sampling and semi-structured individual interviews in Omid Educational and Medical Center and Iranian Cancer "Control Center (MACSA) in Isfahan from May 2020 to December 2021. Data analysis was performed simultaneously with data collection using conventional qualitative content analysis with the Graneheim and Lundman approach. In order to assess the trustworthiness of the obtained data, credibility, dependability, confirmability, and transferability criteria were used. Results : Data analysis led to the production of 615 primary codes, 16 sub-categories, and five main categories. These five main categories included "Lack of facilities and equipment", "Lack of quality of services provided by the treatment team", "Insufficient financial support to the patient and family", "Insufficient psychological and emotional support from the patient and family", and "Inefficiency of the patient and community awareness process". Conclusions: To improve the quality of care and increase patients' satisfaction with the provided services, it is recommended that policymakers should meet the challenges of the PC.
Risk perception regarding social determinants of health among women with breast cancer in Iran: a qualitative study

Abstract:Objectives Breast cancer is the most common cancer among women all around the world. Today, in addition to factors including hormones and genetics that are involved in the occurrence of breast cancer, special attention is paid to the role of social and non-medical determinants of health. This study aims to explore the perception of Social Determinants of Health (SDH) in women with breast cancer. Design Qualitative study design with a conventional thematic analysis approach. Setting The study was conducted in Tehran, Iran, between December 2021 and February 2023. Participants 19 women with breast cancer were selected through purposeful and snowball sampling with maximum variation. Sampling continued until data saturation was reached. Primary and secondary outcome measures The study categorised the extracted codes from interviews into three main categories and 12 subcategories related to SDH in women with breast cancer. Results The study identified factors such as personal and family health records, health behaviours and lifestyles and medical screening and follow-up as key themes in the perception of SDH among women with breast cancer. The main categories were also categorised into 12 subcategories, 'including family history', 'environmental factors', 'hormonal and medicinal changes', 'metaphysical factors', 'traditional medicine-related factors', 'stress', 'body weight', 'physical activity', 'nutrition', 'smoking and tobacco use', 'screening', 'self-examination' and 'barriers to medical follow-up'. 'Near the telecommunications tower', 'Improper use of supplements', 'Being subjected to the evil eye regularly', 'Eating cold nature foods', 'Breast weight' and 'Being ashamed of the doctor' were some examples of the codes. Conclusions Participants identified a number of environmental, personal and cultural factors as contributing to the disease. Woman's screening behaviours were influenced by factors such as fear of disease and death, disbelief in getting sick and shame, depending on cultural context. The study suggests the need for further research to explore the impact of these factors on screening behaviours and outcomes in different cultural contexts.

Abstract:Background Electronic prescriptions represent a fundamental shift in service delivery, healthcare management, and associated costs, offering numerous advantages. However, akin to other electronic systems, they also present challenges. This study aimed to investigate patients’ understanding of the challenges associated with electronic prescriptions in Iran. Methods This study used a qualitative research design, utilizing individual and semi-structured interviews with patients referred to selected pharmacies across all 11 districts of Shiraz City. The data were analyzed using MAXQDA software (version 10), and descriptive statistics for demographic data were calculated using SPSS version 19. Results The study revealed that the participants generally demonstrated a certain level of familiarity with electronic prescribing systems. However, it was evident that many were unaware of the potential implications of such technology for their relationships with healthcare providers. This underscores the urgent need for patient understanding in the context of the electronic prescription system. While patients were relatively familiar with the functionality of electronic prescribing systems, they lacked a comprehensive understanding of how using these systems could affect their interactions with healthcare providers. Conclusion Patients are significant beneficiaries of the electronic prescribing system. By addressing their needs and concerns, they can develop a positive attitude toward this system. Their active engagement can pave the way for the system’s ease of use, increase its acceptance, and ultimately enhance the quality of healthcare services.
