Abstract:Objective To explore CT diagnostic the worth of intussusception in adults.Methods CT manifest of 15 cases intussusception in adults which were confirmed by pathology and operation were analysled retrospectively.Rusults The cause of 15 cases included colonic carcinomas(n=3);lymphoma(n=2);hemangioma(n=2)polylipoma(n=4);metastatic tumors(n=1); leiomyoma(n=2),colonic polypus(n=1).There were ileocolonic intussusceptions (n=5), small bowel intussusceptions (n=3) and colocolonic intussusception(n=7).Direct signs such as "target sign"(n=12) and "comet tail(n=7) and kidney sign"(n=4).Indirect sign of intussusception like increased bowel wall thickness(n=7),dilataltion of proximal intestintal cavity (n=9),intestintal obstruction(n=6) and mesentery infiltration(n=1) were observed.8 primary lesions(8/15)were found on CT.Conclusion CT has important diagnostic worth and it provide main reference in pathogenic diagnosis of intussusception in adult before operation.
Colonic torsion and volvulus in dogs is associated with a low mortality rate and good long-term outcome

Abstract:Objective: To describe clinical findings, complications, and short- and long-term outcomes associated with colonic torsion and volvulus in dogs. Animals: 28 client-owned dogs. Clinical presentation: Multi-institutional, retrospective study. Medical records were searched for dogs of any age, body weight, sex, and breed that underwent surgery for colonic torsion and volvulus. Collected data included signalment, previous history, preoperative findings, time until surgery, intraoperative findings, postoperative complications, length of hospitalization, survival to discharge, and outcomes. Results: 28 dogs were included. Thirteen of 28 dogs (46.4%) had preexisting gastrointestinal conditions. Nine of 28 dogs (32.1%) had a gastropexy performed prior to presentation. Ten dogs (35.7%) were found to have a resolution of colonic torsion and volvulus at the time of the surgery. All but 1 dog (27 of 28 [96.4%]) survived to discharge. Two dogs died during the postoperative period, yielding a mortality rate of 7.1%. Postoperative complications were noted in 9 dogs (9 of 28 [32.1%]). Long-term follow-up information was available in 16 of 28 dogs (57%). Among 16 dogs with at least 6 months' follow-up, all dogs (16 of 16 [100%]) were alive at 6 months postoperatively. Two dogs developed mesenteric torsion after the initial surgery. Clinical relevance: Dogs with colonic torsion and volvulus undergoing surgery can have an excellent survival-to-discharge ratio with a low mortality rate. Surgeons should not be prompted to euthanize or assume a guarded prognosis solely on the basis of the intraoperative appearance of the bowel and should consider all factors prior to making decisions. Owners should be informed of the risk of developing further torsional diseases after surgery.

Abstract:Objective: To describe the clinical, laboratory and ultrasonographic findings, treatment, and outcome of cattle with small intestinal incarceration (SII) through internal and external hernias. Methods: The medical records of 85 cattle with SII admitted between January 1, 1987, and December 31, 2019, were retrospectively reviewed. The long-term outcome was determined 2 years after discharge. Results: 85 cattle had herniation of the small intestine through congenital or acquired openings in mesentery or omentum (internal herniation; n = 60) or the abdominal wall (external herniation; 25). The most common findings were little or no feces in the rectum (77 of 85 [90.6%]), reduced or absent intestinal motility (76 of 85 [89.4%]), and hypocalcemia (36 of 44 [81.8%]). Thirteen (15.3%) cattle died or were euthanized without surgery. Of the remaining 72 (84.7%) cattle that underwent surgery, 42 survived the procedure. Overall, 52 of 85 cattle (61.2%; 95% CI, 50% to 72%) did not survive to hospital discharge and 33 (38.8%; 95% CI, 28% to 50%) were discharged alive. Of these, 11 (33.3%; 6 with and 5 without hernias closed completely) were still productive in their respective herds 2 years later. Conclusions: The diagnosis of an incarcerated external hernia is usually straightforward, whereas internal SII necessitates laparotomy or postmortem examination for a definitive diagnosis. Clinical relevance: Internal herniation should be part of the differential diagnosis in cattle with signs of ileus. Immediate surgical treatment is paramount in cattle with SII.
Activation of MAP kinase in Swiss 3T3 fibroblasts by insulin-like growth factor-I.

Abstract:A peak of cytosolic myelin basic protein kinase activity was observed at 2-5 min after addition of 10 nM insulin-like growth factor-I (IGF-I) to Swiss 3T3 fibroblasts. Analysis of the induced kinase activity by chromatography, immunodetection and in situ kinase assay suggests that this represents a 1.8- to 3.5-fold increase in the activity of p42 mitogen-activated protein kinase. Addition of insulin at 10 nM also resulted in an increased kinase activity in these respects, however, only to approximately 60% of that induced by IGF-I. A similar quantitative relation between the effects of insulin and IGF-I was found for induction of lipid and glycogen synthesis from [14C- (U)]-D-glucose. However, incorporation of 3H-L-leucine into protein was stimulated to the same extent by the two agents. The effect of 10 nM insulin on proliferation, measured as tetrazolium dye reduction, was only 18% of that induced by 10 nM IGF-I. The obtained data suggest that cytosolic mitogen-activated protein kinase activation may constitute a signalling pathway for glucose metabolism, and especially glycogen synthesis, induced by peptides of the insulin superfamily.

Abstract:OBJECTIVES: To review the literature, covering the most important aspects of management of acute poisoning.METHODS: The most relevant articles on the management of acute poisoning were selected.RESULTS: Acute poisoning is an important matter of public health, particularly for the child. Medicaments are the main responsible agents followed closely by accidents with poisonous animals and household chemical products, pesticides and industrial products. The management of the poisoned patient must proceed through the following sequential stages: initial clinical evaluation and stabilization of the patient, when life-threatening situations are identified and treated; recognition of the toxic syndrome and identification of the causal agent, when the careful analysis of the signs and symptoms help choosing the diagnostic tests; decontamination, with the careful use of the available methods; administration of antidotes, which are small in number and should never delay the other phases; enhancing the elimination of the absorbed toxic, which requires knowledge of the kinetics of the toxic substance; and supportive treatment.CONCLUSIONS: The recognition of the toxic syndrome and knowledge of the phases of management are basic conditions for the success of treatment.
TALP-UPC at TREC 2005: Experiments Using a Voting Scheme Among Three Heterogeneous QA Systems

Abstract:This paper describes the experiments of the TALPUPC group for factoid and ’other’ (definitional) questions at TREC 2005 Main Question Answering (QA) task. Our current approach for factoid questions is based on a voting scheme among three QA systems: TALP-QA (our previous QA system), Sibyl (a new QA system developed at DAMA-UPC and TALP-UPC), and Aranea (a web-based data-driven approach). For defitional questions, we used two different systems: the TALP-QA Definitional system and LCSUM (a Summarization-based system). Our results for factoid questions indicate that the voting strategy improves the accuracy from 7.5% to 17.1%. While these numbers are low (due to technical problems in the Answer Extraction phase of TALPQA system) they indicate that voting is a succesful approach for performance boosting of QA systems. The answer to definitional questions is produced by selecting phrases using set of patterns associated with definitions. Its results are 17.2% of F-score in the best configuration of TALP-QA Definitional system.

Abstract:Objective To summarize the clinical characteristics of small intestinal stromal tumors(SIST), and evaluate the diagnostic values of various imaging or endoscope examinations for SIST. Methods From July 2004 to June 2009, 74 patients whose operation or endoscopy biopsy tissues pathologically confirmed SIST were collected. The clinical data, imageology including enteroclysis, abdominal ultrasound, spiral computered tomography (CT) and the double-balloon enteroscopy report of the patients were analyzed retrospectively. According to biological behavior, the SIST was divided into four risk degree such as extremely low risk, low risk, moderate risk and high risk. The correlation between pathologic characters and spiral CT feature was analyzed. Results The most predilection site of SIST was jejunum in 43 patients (58.1 %); secondarily duodenum in 17 cases (23%); and 10 cases (13.5%) in ileum. About 94.6 percent of patients (70/74) showed clinical signs, the most common symptom was gastrointestinal bleeding in 46 cases (67. 2 % ), abdominal pain in 23 cases (31.1%). Of various photogrammetry examinations and endoscopy, spiral CT has the highest diagnosis rate and diagnosis coincidence rate, which was 100% and 72. 1% respectively. Among the 74 SIST lesions, 14 cases were extremely low biological risk (18. 9 % ), 21 at low risk (28.4 % ), 15 at moderate risk (20. 3%) and 24 at high risk (32.4%). Spiral CT is helpful for the SIST risk diagnosis. Conclusions The onset of SIST was concealed and early diagnosis was very difficult. Spinal CT which could help to predict the tumor's risk degree and prognosis was noninvasive, convenient and reliable. Therefore, it could be the first choice for SIST examination at present. Abstract:We present a 26-year-old male patient who presented with complete bowel obstruction and peritonitis. His abdominal X-rays showed grossly distended large bowels with air-fluid levels. A differential diagnosis of sigmoid volvulus was entertained and the patient was taken for an exploratory laparotomy. Intraoperatively, we found a sigmoid volvulus and a concurrent transverse colon volvulus. A subtotal colectomy and colostomy was performed. The histopathology results showed mucosal and submucosal congestion, chronic inflammation, and no malignancy or dysplasia. Synchronous volvulus of the sigmoid and transverse colon is an extremely rare. Management includes endoscopic derotation and decompression followed by semi-elective surgery in non-complicated cases. When endoscopic decompression has failed or there is suspicion of necrosis or perforation, surgery is mandatory.

Serum neuropeptide concentrations in cows with intrapartum uterine torsion

Abstract:Intrapartum uterine torsion is a common cause of dystocia in dairy cows. To clarify the pathophysiology of this disease, the aim of the present study was to examine serum neuropeptide and cytokine concentrations of cows with intrapartum uterine torsion. Blood samples of 20 cows with uterine torsion, 36 healthy controls, and 15 intrapartum cows without uterine torsion were obtained. Concentrations for substance P (SP), vasoactive intestinal polypeptide (VIP), and interleukin-1β (IL-1β) were quantified by using commercially available ELISA kits. Significant differences between groups were observed for SP in cows with uterine torsion and cows calving normally (P < 0.01). Markedly greater SP concentrations were observed in calving cows than in cows with uterine torsion. Compared with healthy controls, there were greater SP concentrations during parturition (P < 0.01). No significant group differences were detected for VIP concentrations. Global differences were observed in IL-1β (P = 0.04). Large amounts of SP are released into the blood during parturition. Because SP is mainly present within the cervix and functions as a biomarker and mediator of pain, cows with uterine torsion are presumed to not experience as much pain as cows that are calving normally. Consistent with this, in cows with uterine torsion, there is a disturbance in the opening of the cervix, along with only mild signs of colic. Furthermore, significantly elevated IL-1β concentrations correlate with the inflammation that occurs in cases of uterine torsion. Further research is needed to support these findings and clarify the clinical relevance.
Strangulation of the duodenum by the uterus during late pregnancy in two cows

Abstract:Two Swiss Braunvieh cows in late pregnancy underwent surgery because of a rare form of ileus due to strangulation of the duodenum at its caudal flexure by the gravid uterus. The whole uterus had passed through a gap between the mesoduodenum and duodenum and with increasing weight had led to strangulation of the duodenum. This was possible since the mesoduodenum and both walls of the greater omentum adjacent to its caudal edge were not connected with the duodenum, probably due to a congenital inhibitory malformation. A transsection and an end-to-end anastomosis of the duodenum were necessary in both cases since it was impossible to retract the gravid uterus through the defect. Postoperative recovering was uneventful in both cows, which were discharged after seven and five days respectively and calved normally about two months later.

Abstract:ABSTRACT The medical records of 12 cats presented with intussusception and diagnosed at the University of Sydney Veterinary Teaching Hospital and the Sylvania Veterinary Hospital were reviewed. Eleven cats were less than one year old at the time of diagnosis. No breed or sex predilections were determined. The most consistent clinical signs were anorexia (83 per cent), weight loss (83 per cent) and a palpable abdominal mass (92 per cent). Vomiting (58 per cent) and diarrhoea (50 per cent) were seen less frequently in these cats compared with the reported incidence of these signs in dogs. In 10 cats the intussusception was corrected surgically. Manual reduction was successful in three of the cats but six required end‐to‐end anastomosis following intestinal resection and in one cat a typhlectomy was performed. Postoperative ileus complicated recovery in three cases. Recurrence of the intussusception at a different site occurred in two of these cases. One cat with recurrence of intussusception died and the cat with ileus but not recurrence was euthanased one month after the original surgery.

Abstract:An eight-year-old, 35 kg, female spayed Doberman Pincher dog was presented with peracute abdominal distention and severe abdominal pain associated with vomiting. Radiographs showed segmental gas and fluid-distended small intestines and loss of serosal detail. Abdominal ultrasound revealed segmental hypomotile, fluid distended small intestines and high amount of gas in the intestinal lumen. CT showed signs consistent with a segmental mesenteric torsion. Exploratory laparotomy and en bloc enterectomy of the torsioned necrotic segment of the jejunum (50 per cent approximately) was performed. The patient was discharged 1 week after surgery with some sporadic vomiting and diarrhoea, which resolved with medical treatment within a week.
Emphysematous splenic torsion in a German shepherd dog

Abstract:A 9-year-old castrated male German shepherd dog was presented because of a 2-day history of lethargy and anorexia. Abdominal distention and free peritoneal fluid were noted on physical examination, which prompted radiographs revealing a large, soft tissue and stippled gas opaque structure in the right cranial abdomen. Computed tomography was used to further describe the structure and assess for comorbidities in the dog. Both CT and abdominal radiographs supported a diagnosis of emphysematous splenic torsion. The enlarged spleen was surgically extracted, along with sections of necrotic omentum. Four days after the procedure, the dog developed a portal vein thrombus and secondary abdominal effusion, which ultimately led to the decision for humane euthanasia. Veterinarians should know the different imaging modalities used to diagnose splenic torsion and the possible postoperative complications following treatment.
Comprehensive exploration unveiling the sonography and histopathology of uterine leiomyoma in a cow

Abstract:Genital tumours are rare among cattle, largely due to their relatively short lifespans. Leio-myoma, a smooth muscle tumour being more prevalent in dogs, appears only at a rate of 1.00 - 2.00% in cattle, affecting reproductive efficiency in cases of complete uterine obstruction. This case report involves an 8-year-old cow with repeated insemination attempts unveiled 5.00 cm intra-luminal uterine mass, obstructing the right uterine horn. Transrectal sonography (TRUS) revealed a highly vascularized mass with normal ovarian function. Confirmation of clinical condition, i.e., uterine leiomyoma, via uterine biopsy concluded the presence of neoplastic smooth muscle cells arranged in interlacing bundles showing mild pleomorphism, and special staining using Masson's trichrome revealed an unappreciable amount of connective tissue; subsequently right flank celiotomy was performed to remove the benign tumour. Forty-five days after celiotomy, TRUS examination confirmed an unobstructed uterine horn, and bilateral oviduct patency was adjudged with 2.50% methylene blue. Following treatment for chronic endometritis, artificial insemination led to conception nearly 90 days post-procedure. The TRUS aids preliminary diagnosis, while definitive identification demands necropsy and surgical methods. This case underscores the diagnostic significance of TRUS, histopathology and celiotomy for identifying and managing uterine leiomyoma in cattle.
Preoperative and Surgical Predictors of the Treatment Outcome of Dairy Cows with Right Abomasal Displacement-A Retrospective Study of 234 Cases

Abstract:The displacement of the abomasum to the right (RDA) is a common condition regularly encountered in dairy cows, which requires urgent surgical correction. The survival of the patient primarily depends on early diagnosis and timely treatment, but other factors contributing to the outcome have been discussed in the literature. The objective of this study was to identify preoperative clinical, hematological, as well as intraoperative parameters that are associated with the prognosis of cows with RDA or abomasal volvulus (AV). This retrospective study included patients admitted to a veterinary teaching hospital over a period of 6 years with a diagnosis of RDA or AV. A total of 234 cows were included, of which 193 were discharged after treatment and thus classified as survivors. In contrast, 41 cases died or were euthanized during or after surgery and were categorized as non-survivors. Non-survivors showed more severe dehydration, higher heart rate, lower sodium, as well as higher L-lactate and phosphorus concentration in their blood prior to surgery compared with the survivors. During surgery, the abomasum of non-survivors was markedly dilated and twisted more frequently than in survivors. The results presented here can facilitate the early identification of animals with poor prognosis requiring more intensive peri- and postoperative care.
Atypical endoscopic finding of colocolic intussusception: case report and literature review

Abstract:Background: Adult-onset colonic intussusception is a rarely encountered condition that leads to large intestinal obstruction with time. Patients often present with a variety of symptoms that are non-specific making it challenging to arrive at a definitive diagnosis. This is worrying as diagnostic delay could lead to a significant increase in morbidity and mortality. We wish to present and describe a case of an atypical endoscopic finding of colocolic intussusception secondary to ascending colon cancer. Case description: Sixty-seven-year-old lady was referred for 1 month's duration of passing melenic stools with mucus followed by a week's complain of hematochezia. Clinical examination and other relevant blood results were unremarkable except for iron deficiency anemia. Initial colonoscopy revealed a large mass within the splenic flexure with inconclusive biopsies. A more detailed colonoscopy repeated the following day revealed a massive, black-to-yellowish lesion within the splenic flexure with no viable mucosa seen. Colonic bezoar was initially suspected, however various endoscopic retrieval methods proved futile. Switching to a slimmer diagnostic gastroscope, the colon was carefully negotiated until a large ulcer was found within the ascending colon, adjacent to the mass' origin. An emergency CT abdomen and subsequently extended right hemicolectomy performed revealed a colocolic intussusception with sealed perforation secondary to an ascending colonic mass acting as an intussusceptum. Histopathology evaluation confirmed an ascending colon adenocarcinoma (pT2N0M0) amidst a background of extensive ischemic changes. Conclusions: Endoscopic descriptions of colonic intussusception are unusual given their rarity. Furthermore, these lesions can mimic a colonic bezoar as a result of fecal accretion and this can ultimately lead to false diagnostic and therapeutic decisions. In such instances, clarification with a CT scan before management decision can potentially avert unnecessary endoscopic intervention and complications.

Abstract:OBJECTIVES: To review the histological findings in the intestine from dogs and cats with intussusception.METHODS: Medical records and histopathology reports of dogs and cats with intussusception were reviewed retrospectively.RESULTS: Fourty-nine animals (31 dogs and 18 cats) were identified for inclusion. Tissues examined com-prised the intussusception alone in 29 animals (16 dogs and 13 cats), and the intussusception with additional intestinal biopsies in 20 animals (15 dogs and 5 cats). Twenty-eight of 49 (57·1%) animals, comprising 19 of 31 (61·3%) dogs and 9 of 18 cats (50%) had abnormalities detected on histological examination of tissue. Eleven of 29 (46·9%) cases where only the intussusception was submitted achieved a histological diagnosis, compared to 17 of 20 (85%) where additional biopsies were submitted (P=0·003). Cats (median age 36 months, range 2 to 174) were significantly older than dogs (median age 7·5 months, range 1 to 125 months, P=0·010) and were significantly more likely to have underlying neoplasia (5 of 9; 55·6%) compared to dogs who were more likely to have inflammatory causes (17 of 19; 89·5%, P=0·020). There was no association between histological diagnosis and location of the intussusception (P=1·000).CLINICAL SIGNIFICANCE: Histological abnormalities were detected in more than half of the animals.
