Abstract:Background: Graph analysis detects psychosis and literacy acquisition. Bronze Age literature has been proposed to contain childish or psychotic features, which would only have matured during the Axial Age (∼800-200 BC), a putative boundary for contemporary mentality. Method: Graph analysis of literary texts spanning ∼4,500 years shows remarkable asymptotic changes over time. Results: While lexical diversity, long-range recurrence and graph length increase away from randomness, short-range recurrence declines towards random levels. Bronze Age texts are structurally similar to oral reports from literate typical children and literate psychotic adults, but distinct from poetry, and from narratives by preliterate preschoolers or Amerindians. Text structure reconstitutes the "arrow-of-time", converging to educated adult levels at the Axial Age onset. Conclusion: The educational pathways of oral and literate traditions are structurally divergent, with a decreasing range of recurrence in the former, and an increasing range of recurrence in the latter. Education is seemingly the driving force underlying discourse maturation.

The concept of neuropsychiatry: a historical overview

Abstract:The elusive nature of the social practice called neuropsychiatry is the most important obstacle for writing its history. At different times, and in different countries, the term has been used to name different ideological and professional packages. Choosing any of these as the prototype would, however, bias and regionalize the historical account. One solution is to identify an ideological marker or common denominator. To spin its yarn, this paper has chosen the foundational claim (putatively common to all practitioners of 'neuropsychiatry') that all 'mental disorders are disorders of the brain'. Three among its results are worth noting. One is that the meaning and exercise of 'neuropsychiatry' will continue to depend on social, economic and political factors; this augurs future instability. The second is that each time that the foundational claim has been uttered in history it has meant something different. Further research is needed to clarify whether this is due to the way in which the claim becomes 'inscribed' in different cultural niches; what is clear is that by using the foundational claim as a marker it is not possible to establish a continuity in the progress of neuropsychiatry. The third finding is that users of the foundational claim require the use of a concept of matter and in this paper 'plain' and 'baroque' types of matter have been identified. To explain the origin of mind, those using the plain notion need to resort to external ingredients and their narratives incorporate bits of the real world. Users of the baroque definition do not need such aids. It is not for the historian to judge which of these two definitions is more felicitous and more conducive to the moral and aesthetic edification of psychiatry and her patients.

[Which way ahead for psychiatry? Identity, competence and cooperation--challenges of the 21st century]

Abstract:In the past decades some disadvantageous changes have been occurred in almost each field of medicine, especially neurology and psychiatry. Medical students and young doctors are less and less able to utilize their neurological knowledge in the everyday clinical practice and the number of doctors specialized in neurosciences is decreasing. Recent problems of psychiatry originate from special development, controversial process of detachment, particular societal and cultural roots, differences from medical model, and problems of identity, self-determination and competence, which may cause many difficulties in clinical practice. The author - near the review of the theoretical background - mentions several practical topics as well, for example: the difficulties of diagnosing, and classification of mental disorders, the diagnostic and treating problems being in the background of psychopathological symptoms. In the article it is emphasized that the present situation results in several serious problems for the psychiatrists like emotional overburden, somatic and mental disorders and burnout, moreover psychiatry became one of the least popular medical professions, which causes huge difficulties in the aftergrowth. Furthermore the author emphasises, that this profession has to actualize the humanistic view in clinical practice based on the holistic bio-psycho-socio-spiritual approach. The (neuro)scientific and consultative psychiatric viewpoints, the constructive intra- and interdisciplinary communication, the more effective value-based advocacy and the modern, evidence-based, interactive and personal education together have a key role in the solution of the above mentioned problems.

Abstract:Background: X (previously known as "Twitter") serves as a platform for open discussions on mental health, providing an avenue for scrutinizing public perspectives regarding psychiatry, psychology and their associated professionals. Objective: To analyze the conversations happening on X about psychiatrists, psychologists, and their respective disciplines to understand how the public perception of these professionals and specialties has evolved over the last 15 years. Methods: We collected and analyzed all tweets posted in English or Spanish between 2007 and 2023 referring to psychiatry, psychology, neurology, mental health, psychiatrist, psychologist, or neurologist using advance topic modelling and sentiment analysis. Results: A total of 403,767 tweets were analyzed, 155,217 (38%) were in English and 248,550 (62%) in Spanish. Tweets about mental health and mental health professionals and disciplines showed a consistent volume between 2011 and 2016, followed by a gradual increase from 2016 through 2022. The proportion of tweets discussing mental health doubled from 2016 to 2022, increasing from 20% to 67% in Spanish and from 15% to 45% in English. Several differences were observed on the volume of tweets overtime depending on the language they were written. Users associated each term with varied topics, such as seeking for help and recommendation for therapy, self-help resources, medication and side effects, suicide prevention, mental health in times of crisis, among others. The number of tweets mentioning these topics increased by 5-10% from 2016 to 2022, indicating a growing interest among the population. Emotional analysis showed most of the topics were associated with fear and anger. Conclusion: The increasing trend in discussions about mental health and the related professionals and disciplines over time may signify an elevated collective awareness of mental health. Gaining insights into the topics around these matters and user's corresponding emotions towards them presents an opportunity to combat the stigma surrounding mental health more effectively.

Abstract:Our understanding of the history of psychiatry has been warped by the way history is remembered – collectively and personally; this is the point of departure from which Wynter, Wallis and Ellis have embarked upon this splendid essay anthology. The introduction provokes something of a liminal space by describing psychiatry as a specialty that is ever existing between 'the terrible past and the ideal future'. They argue that barbarity has become a cultural trope for psychiatry, especially when historical treatments are viewed through modern framing without the scaffolding of contemporaneous societal values. Progress seems to always be in response to wanting to heal this iniquity and provide a change in direction. The authors endeavour to show us that the actuality of psychiatry may exist somewhere between these two opposing poles, and furthermore it is interested in the idea that modern mental health services have arisen from within this in-between space. The essays loosely stick to a theme of memory and remembrance, and are arranged within five sections: governance; practitioners; case books; oral histories; and personal recollections. The various essays look at different aspects of psychiatry through different eras and across the globe, with very distinct voices and forms, but this thematic arrangement provides a sense of cohesion. However, there is no onus on the reader to read the whole collection all at once, with most of the pieces having enough strength to be considered in their own right. We see Britain's colonial footprint in the particularly provocative 'Madness, memory and delusion in late nineteenth-century Barbados', whereby Leonard Smith skilfully introduces readers to the cultural matrix of colonial Barbados, where many people were living in destitution, and then goes on to consider a series of nineteenth-century lunatic asylum case records held in the Barbados National Archive. A re-examination of these material impressions of the past acknowledges the colonial gaze through which patients were described, but also provides a new way of seeing patients and their problems, beyond the hermeneutic trouble entailed by the passage of time. A chapter about Bombay's lunatic asylums around the turn of the twentieth century readily relates to this one, but other works describing topics as far reaching as the remodelling of the Sigmund Freud Museum in Vienna and the nostalgia experienced by staff in the wake of an American psychiatric hospital ensure a significant breadth of content. The book ends with a devastatingly honest reflective essay by Allan Beveridge that focuses sharply upon his experience of psychiatric training in Edinburgh in the 1980s. He shares his personal sense of excitement upon embarking upon his career that gives way to unease as he faces the reality of clinical practice, and the discombobulating effect of competing explanations and opposing views that continue to persist, to a greater or lesser extent, to this day. The book grasps onto the Derridean idea that there is an ever-transforming inheritance of the past in light of changing contexts of the future, and that the key to history lies in how it is remembered. Through an exploration of remembrance, this work generates novel insights, which allow us to recast how we understand previous insights into psychiatry's past. This thought-provoking essay collection will generate enthusiasm for new ways of considering the past and reframing the present through troubling our interpretation of what has been written before. It is certainly worth reading to generate reflections on what the legacy of our own work may be.

Abstract:Medical history was an important part of medicine in the West from antiquity, through the Middle Ages, and until the Renaissance. Hippocrates, Galen, and Avicenna were historical figures, but they dominated the medicine of the Western world at least until Renaissance. The medicine of the past, which did not become history, still remained an important part of present medicine. In the 19th century, medicine in the past is now relativized as an object of history. At the same time, the 'practicality' of medical science was emphasized. The practicality referred to here means that, unlike previous times, medicine in the past has been historicalized, but it can provide practical help to current medicine. In particular, in the era of positivism that dominated the late 19th century, this practicality was a core value of medical history. In the 20th century, the era of scientific medicine, the new role is given to medical history. It was to give a integrated view on contemporary medicine which was subdivided into many specialized fields. Along with this, medical history, once a main part of medicine, moves to the field of history. At the same time, the rise of medical humanities in medical education becomes an opportunity to redefine the role of medical history. Seeking productive cooperation with other humanities and social sciences that deal with medical issues, such as medical anthropology, medical sociology, and literature, will be a new task given to medical history today.

Abstract:This article offers an extensive review of the changing field of mental health therapies, charting a transformational path from traditional methods to modern breakthroughs and speculating on potential future developments. The story develops by investigating historical viewpoints while reflecting on the present and highlighting the lessons learned and their impact on contemporary practices. We have advanced from the stigmatized constraints of asylums to a paradigm that puts human rights, dignity, and individualized, culturally sensitive treatment first. Modern methods are much more varied and evidence-based, from cutting-edge technical advancements to evidence-based psychotherapies. The ethical considerations arising from the delicate balance of pharmacological therapies underline the responsibility of administering drugs that significantly affect mental health. Cultural factors become a pillar, highlighting how crucial cultural sensitivity is to promoting tolerance. By acknowledging how many facets of the human experience are interrelated, holistic methods help close the gap between the mind and body. Integrative medicine and alternative therapies represent a shift away from reductionist approaches and toward a holistic viewpoint. The delivery of mental health treatment is being reimagined by technological advancements, with virtual and digital environments opening up new access and support channels. These developments cut beyond regional boundaries, reinventing conventional therapy dynamics and paving the way for individualized therapies. Cultural concerns highlight the significance of cultural competency in navigating the complex mental health treatment system and adapting interventions to fit the particular requirements of various cultural contexts. With telepsychiatry, virtual reality, and artificial intelligence among the new technologies that promise to further revolutionize mental health therapies, the essay looks to the future. This review concludes by imagining a day when mental health is prioritized, therapies are available, and the diversity of human experience is valued. The path to a society that values, nurtures, and celebrates mental health continues.
Comparison of hydromorphone and meperidine for ureteral colic.

Abstract:OBJECTIVE To compare the efficacies of meperidine and hydromorphone in the treatment for ureteral colic in the emergency department (ED). METHODS A prospective, double-blind, randomized clinical trial was conducted over six months at a tertiary referral center with 93,000 annual ED visits. Seventy-three patients completed the study. The patients received either 1 mg of hydromorphone or 50 mg of meperidine IV at t = 0. Pain intensity was determined using a 10-cm visual analog scale at t = 0, 15, 30, 60, and 120 minutes. A second dose of the study drug could be given between t = 15 and t = 120 minutes when the clinician believed the initial dose was ineffective. Patients requiring more than one additional dose of analgesia were treated as nonresponders and were removed from the study. RESULTS Thirty-six patients received hydromorphone and 37 received meperidine. The initial pain intensities (hydromorphone group = 8.4 +/- 1.5; meperidine group = 8.5 +/- 2.1), age distributions, sex distributions, and side effects of the two groups were comparable. Pain relief was better (p < 0.05) with hydromorphone at t = 15, 30, 60, and 120 minutes. The hydromorphone group required rescue analgesia less often (31% vs 68%, p < 0.01), had fewer IV pyelographies (IVPs) (28% vs 54%, p < 0.05), and had a lower proportion of hospital admissions (25% vs 49%, p = 0.08). CONCLUSIONS For the fixed doses used in this study, the adult ureteral colic patients receiving hydromorphone achieved more pain relief, required less rescue medication, underwent fewer IVPs, and avoided hospital admission more frequently than did those receiving meperidine.

Abstract:The multi-faceted phenomenon known as globalization has a particular impact on the conceptual and practical development of mental health disciplines in general, and psychiatry in particular, across different world regions. To be theoretically and functionally effective, global psychiatry requires an integration of its different components. To such objective, and after a brief review of continental European and Anglo-Saxon contributions, this article examines the history, characteristics, and contributions of Latin/Iberian American and Spanish-speaking psychiatry, in order to substantiate its role in world psychiatry. The Latin American proper (including Portuguese-speaking Brazil), Spain, and U.S.-based Hispanic components are described, revealing an identity that is based on a humanistic tradition, a value-based, culturally-determined clinical approach to patient care, and a pragmatic adaptation of different treatment resources and techniques. These may constitute supportive elements of an instrumental inter-regional bond in the present and future of our discipline.

Abstract:Work at the intersection of philosophy and psychiatry has an extensive and influential history, and has received increased attention recently, with the emergence of professional associations and a growing literature. In this paper, we review key advances in work on philosophy and psychiatry, and their related clinical implications. First, in understanding and categorizing mental disorder, both naturalist and normativist considerations are now viewed as important – psychiatric constructs necessitate a consideration of both facts and values. At a conceptual level, this integrative view encourages moving away from strict scientism to soft naturalism, while in clinical practice this facilitates both evidence‐based and values‐based mental health care. Second, in considering the nature of psychiatric science, there is now increasing emphasis on a pluralist approach, including ontological, explanatory and value pluralism. Conceptually, a pluralist approach acknowledges the multi‐level causal interactions that give rise to psychopathology, while clinically it emphasizes the importance of a broad range of "difference‐makers", as well as a consideration of "lived experience" in both research and practice. Third, in considering a range of questions about the brain‐mind, and how both somatic and psychic factors contribute to the development and maintenance of mental disorders, conceptual and empirical work on embodied cognition provides an increasingly valuable approach. Viewing the brain‐mind as embodied, embedded and enactive offers a conceptual approach to the mind‐body problem that facilitates the clinical integration of advances in both cognitive‐affective neuroscience and phenomenological psychopathology.

Abstract:Dating back to ancient civilizations when records were carefully transcribed onto papyrus, clinical documentation has long served as a cornerstone of medical-and especially neurologic-education. From the case histories of Hippocrates to the diurnal patient logs used by trainees in the 18th and 19th centuries, clinical notes have an extended history as invaluable instruments of pedagogy, scholarly practice, and interprofessional communication. The novel paradigm introduced by Lawrence Weed in the 1950s, advocating for the problem-oriented medical record system, revolutionized the clinical note template and emphasized the need for physicians' carefully considered analyses of a patient's presentation to be clearly reflected in well-organized documentation. In the realm of medical records today, however, a profound shift is underway, largely propelled by the emergence of electronic medical records, the OpenNotes mandate of the federal 21st Century Cures Act, and, most recently, artificial intelligence (AI). Appropriately, patients now have full access to their medical records, but this raises critical questions. Should clinical notes now prioritize patient comprehension over their traditional role as educational instruments, aide-mémoire, and repositories of detailed assessments and insights? What role, if any, should AI have in the creation of physician notes and patient-facing clinical documents? These tensions underscore the delicate balance between transparency and the preservation of notes' clinical integrity and analytical depth. As we navigate the path forward, finding an equilibrium between openness and the continued utility of medical records as tools for education and professional communication will be imperative.

Abstract:Australasian Psychiatry, Ahead of Print. ObjectivePersonal narratives of lived experience with psychiatric illness and distress remain central in the epistemology of mental illness. We provide a commentary on this potential bridging of patient narrative-based epistemology, and medico-scientific epistemology used by psychiatrists used for diagnosis, formulation, prognosis and treatment.ConclusionDiscussion and planning of psychiatric care can be framed by understanding the narrative-based epistemology of a patient's illness as highlighted by five key questions to explore the patient's illness explanatory models. We propose five key questions for the psychiatrist's complementary consideration of medico-scientific epistemology that frame conceptual models of aetiology, pathophysiology, diagnosis, formulation, prognosis and treatment, which are embedded in the predominant socio-cultural environment. These questions assist in bridging patient narrative and medico-scientific explanatory models to facilitate more effective collaborative care planning.

The Health Benefits of Autobiographical Writing: An Interdisciplinary Perspective

Abstract:A large body of experimental evidence in the empirical sciences shows that writing about life experiences can be beneficial for mental and physical health. While empirical data regarding the health benefits of writing interventions have been collected in numerous studies in psychology and biomedicine, this literature has remained almost entirely disconnected from scholarship in the humanities and cognitive neuropsychology. In this paper, I review the literature from psychological and biomedical writing interventions, connect these findings to views from philosophy, cognitive neuropsychology and narratology and argue that examining established regularities in how narratives are structured can shed further light on the psychological processes engaged during writing interventions. In particular, I argue that the narratological concept of conflict can be applied to resolve patterns of seemingly conflicting empirical findings in psychological studies. More generally, I propose that an interdisciplinary perspective can provide a broader theoretical basis for understanding the psychological processes underlying the health benefits of autobiographical writing and provide directions for future research in psychology and biomedicine.

Abstract:Histories of psychiatry in the United States can shed light on current areas of need in mental health research and treatment. Often, however, these histories fail to represent accurately the distinct trajectories of psychiatric care among black and white populations, not only homogenizing the historical narrative but failing to account for current disparities in mental health care among these populations. The current paper explores two parallel histories of psychiatry in the United States and the way that these have come to influence current mental health practices. Juxtaposing the development of psychiatric care and understanding as it was provided for, and applied to, black and white populations, a picture of the theoretic foundations of mental health emerges, revealing the separate history that led to the current uneven state of psychiatric care.
A History of Medicine for Patient: Patient Subjectivity and the Change of Historiography of Medicine

Abstract:Abstract Patient’s voice has become louder in recent years. It is a statement of illness, disability, near-death experience, and a cry from the weak, which reveals rich and multiple aspects of patient's physical diseases and mental pain. The groans of patients should not only be treated as reference for decision-making in clinical diagnosis and treatment, but need to be marked down as a voice of the weak in the history of medicine. However, the reality is this voice often stays as physician’s medical records than writings in history of medicine. The mainstream of medical history focuses on how doctors cure diseases, while ignores the role of patients. At times in history of medicine, it was patient’s illness challenged traditional medical theories, provided new perspectives to diagnosis and treatment, and corrected mistakes and prejudice of physicians. Yet, patients benefited very little in the process. Although a few patients miraculously recovered, more became experimental subjects and victims of medicine (Perino L. Patients Zéro: Histoires inversées de la médecine (Chinese edition). France: La Découverte; 2020). Tracing the change of the understanding on patient subjectivity in history of medicine, this paper aims to call for a rise of history of medicine centered on patients.

Issues in Mental Health Nursing

Abstract:The history of mental health nursing has often been overshadowed by other histories. It has been overshadowed by nursing's subservience to medical men who were quick to lay claim to the ownership of psychiatry as a science to gain respectability and a professional identity. It has been overshadowed by general nursing history which was able to celebrate heroines such as Florence Nightingale. Without such exalted heroines, mental health nursing has often been relegated as an appendage to general nursing, and its history, and given little value as part of the history of health care. It has also been overshadowed by a lack of motivation to hear its history given its association with the anxieties that society has had about mental illness and the negative mythology associated with lunatic asylums of the past. Yet, its history is rich and deserving of acknowledgement. Understanding mental health nursing history encourages a collective memory, helps to forge a professional bond and identity and a way forward out of past circumstances and old mentalities.

Abstract:Schizophrenia is a persistent mental health condition that, while presenting challenges, underscores the dynamic nature of cognitive functions and encourages a unique perspective on how individuals engage with their surroundings. Social networks, as a means of communication of great importance at the present time, are for this type of people a way of interacting with their environment with a high level of security. The aim is to find out how schizophrenia is dealt with in different social networks and to differentiate between different types of articles dealing with the use of Facebook, X (former Twitter), YouTube, TikTok, Instagram, and Weibo. A total of 45 articles to i) Social networks used, ii) Country of analyzed users, iii) age of the users analyzed, iv) focus of the analyzed manuscript (mental health literacy, stigmatization, detection of patterns associated with schizophrenia, and Harmful substance use). It was observed that 45.45% of the studies analyzed were conducted in the USA population, followed by UK and China (13.64%). The most analyzed social networks were those based on audiovisual communication (60%). Furthermore, the two main foci addressed in these articles were: stigmatization of schizophrenia with 16 articles (35.55%), following by the prediction of schizophrenia-detecting patterns with 15 articles (33.33%) and the use of social networks to stigmatize people with schizophrenia (38%) and only 14 articles (31.11%) were focused on mental health literacy. Likewise, it was found that there is great potential in the use of the analysis of the content generated, as possible predictors of the presence of this disease, which would allow rapid detection and intervention for psychosis and schizophrenia.

'I think writing is everything': An exploration of the writing experiences of people with aphasia

Abstract:Background: Written communication has become an increasingly important part of everyday life in social, educational and professional spheres. The substantial increase in writing via the internet and mobile technologies provides both an opportunity for social engagement and distinct challenges for people with aphasia. Within the current literature there has been limited research into the lived experiences of people with aphasia of their writing difficulties and how these affect their ability to communicate. Aims: This qualitative study aimed to explore the experiences of people with aphasia of living with language-related writing difficulties and the impact of these on their lives. Methods & procedures: Eight people with post-stroke aphasia and writing difficulties took part in semi-structured interviews. The interviews were analysed using inductive reflexive thematic analysis. Outcomes & results: Two themes were found in the data. The first theme was a gradual and effortful improvement to writing: Participants described how writing had improved since their stroke due to strategies and support, but they still found writing to be difficult and frustrating and described many barriers to writing. The second theme was the importance of writing for fulfilling adult social roles: Participants found writing to be important for communicating with family, friends and organizations, but their participation in society and self-esteem and confidence were impacted by writing difficulties; reduced social roles meant reduced need for writing, but participants were still motivated to work towards writing goals. Conclusions & implications: The findings demonstrate the emerging importance of writing skills for people with aphasia with respect to communication, well-being, participation and inclusion in society, and carrying out social roles. They provide an insight into the process of improvement, including the difficulties, facilitators and barriers. Implications for speech and language therapy assessment and management are discussed. What this paper adds: What is already known on the subject People with aphasia have difficulties with writing that can affect their ability to communicate. A small body of qualitative research has provided insights into individuals' experiences of literacy difficulties. More research is needed to understand the writing experiences of people with aphasia to help design appropriate assessments and interventions. What this paper adds to existing knowledge Participants experienced gradual and effortful improvement since their stroke. They felt negative about aspects of their writing, including speed, accuracy and range of vocabulary. Writing was facilitated through assistive technologies, spelling practice and support from others; barriers included technology, lack of time, stroke-related symptoms and others' lack of awareness about aphasia. Participants considered writing skills to be important, particularly for communication, carrying out adult social roles and participating in society, and were therefore still working towards goals related to everyday writing activities. What are the potential or actual clinical implications of this work? This study suggests that speech and language therapy assessment should include interviewing participants about their activities, strengths, difficulties, facilitators and barriers in writing, and informal assessment of a range of functional writing tasks. Intervention should be tailored to the individual's needs. This should include meaningful activities that relate to functional everyday writing and, where appropriate, self-management, compensatory technologies and group approaches, while making use of existing strategies identified by the individual.
