Abstract:Objective:To highlight the molecul ar biologic markers and techniques of four diseases studied currently in our lab oratory. Methods: Molecular genetic techniques such as PCR, RT -PCR, PCR-RFLP, TG GE, direct DNA sequencing and immunostaining are applicable to diagnostic surgi cal pathology. Results:(1)Synovial sarcoma: A characteristic S YT-SSX fusion gene resulting from chromosomal translocation t(X;18)(p11;q11) was detectable in sy n ovial sarcoma. SYT-SSX fusion transcripts were detected by the reverse-transcr ip tase polymerase chain reaction in paraffin-embedded synovial sarcoma tissues. I t was considered as a defining diagnostic marker of synovial sarcomas and the su bty pes of SYT-SSX fusion transcripts(SYT-SSX1 and SYT-SSX2)might yield prognosti c inf ormation.(2)Anaplastic large cell lymphoma(ALCL): Some ALCLs showed a unique chr omo somal abnormality t(2;5)(p23;q35) which caused ALCLs expressing the anaplastic l ymphoma kinase(ALK) protein fused with NPM. Immunostaining for ALK-NPM protein s howed both nuclear and cytoplasmic staining in these tumor cells carrying t(2;5). It is important to recognize the ALK + group among the ALCLs because of the e xc ellent prognosis.(3)Familial hypercholesterolemia(FH): Molecular genetic analysis of gene for the LDL receptor in affected families could contribute towards earl y assessment for the diagnosis of FH and thus to prevention of life threatening ca rdiovascular episodes in asymptomatic subjects. Promoter region and all of the 1 8 exons of LDLR gene including the intron-exon boundaries were amplified by pol y merase chain reaction(PCR)and were analyzed by temperature gradient gel electrop horesis(TGGE). TGGE combined with DNA sequencing can be used in the genetic diag nosis of FH.(4)Rapid detection of YMDD variants of HBV polymerase gene with long - term lamivudine therapy: long-term lamivudine therapy is associated with mutati o ns in the YMDD locus of the HBV polymerase gene, leading to lamivudine resistanc e. Detection of emerging HBV variants by direct sequencing of the HBV genome is excessibly time-consuming and high-cost for studying a large number of clinica l samples. It is essential to look for a rapid low-cost technique. HBV DNA of a c o nserved polymerase gene segment containing the YMDD locus, was amplified from se rum and liver biopsies by polymerase chain reaction(PCR) and sequenced. HBV geno type was analysed by means of restriction fragment length polymorphism(RFLP) of the PCR product. The PCR-RFLP assay used in this study, in perfect concordance with direct sequencing, is an accurate and simple method for genotyping lamivudi ne-resistant HBV variants. Conclusion: The practice of surgical pat hology tradit ionally relies on the evaluation of the gross and microscopic features of tissue specimens, integrated with the clinical history and other laboratory data. Hist ologic examination of biopsy specimens has for many decades played the central role in the determination and classification of diseases.Yet there are definite limitations to histologic diagnosis. For instance, histologic examination of ti ssues is only partly successful to predict prognosis. It is in the desire to ove rcome the limitations of histologic examination that molecular genetic analysis of tissue specimens has been introduced with the overall goal of classifying dis eases into biologically homogenous groups.
