Abstract:Myocarditis refers to inflammation of the heart muscle and may occur individually or together with pericarditis, which refers to inflammation of the saclike tissue layer that surrounds the heart. They may have infectious or non-infectious etiologies. Campylobacter jejuni, a major cause of gastroenteritis worldwide, may also cause myocarditis in rare situations. We present two cases highlighting this rare complication of diarrheal disease caused by Campylobacter jejuni infection and subsequent development of myocarditis. Both patients presented with chest pain and multiple episodes of watery diarrhea, with initial EKGs showing ST segment changes, as well as elevated inflammatory markers and elevated troponins. GI panels for both patients were positive for Campylobacter jejuni. Based on their presentations and investigative findings, they were diagnosed with myocarditis secondary to Campylobacter infection, and their symptoms subsided with appropriate management. It is unclear if the myocardial damage, in this case, is a direct effect of the toxin on cardiac myocytes or secondary to an immunologic phenomenon. Regardless, Campylobacter jejuni-associated myocarditis remains a rare phenomenon and needs to be considered in the differential of patients presenting with concurrent chest pain and diarrheal symptoms.
A Rare Case of Caecum Perforation Secondary to Campylobacter jejuni Infection

Abstract:Campylobacter is a common cause of bacterial gastroenteritis worldwide. This report presents a rare case of a 44-year-old female who developed a caecum perforation following an initial diagnosis of Campylobacter jejuni infection. The patient initially presented with several episodes of diarrhoea, vomiting, and abdominal pain. Initial investigations confirmed an uncomplicated C. jejuni infection, which was treated with antibiotics and supportive measures. Despite treatment, the patient's symptoms persisted and worsened, and caecum perforation was confirmed on the abdomen and pelvis computed tomography. The patient underwent an emergency right hemicolectomy with an end ileostomy and was discharged home on postoperative day 14 after she made a full recovery. Healthcare professionals should be vigilant for possible severe complications in patients with C. jejuni infection. Frequent abdominal examinations with radiological investigations should be considered when symptoms are worsening to promptly identify any potentially life-threatening complications similar to those in the presenting case.

Abstract:Essential thrombocythemia (ET) was first described in 1934 [1], classified as a myeloproliferative disorder (MPD) in 1951 [2], accepted as a distinct clinicopathologic entity in 1960 [3], and characterized as a clonal stem cell disorder in 1981 [4]. Formal diagnostic criteria were first established in the 1970s by the Polycythemia Vera Study Group (PVSG) [5]. These original criteria were recently revised by the World Health Organization (WHO) system for classification of myeloid neoplasms [6]. WHO-based ET diagnosis requires both positive (megakaryocytic hyperplasia with increased number of enlarged, mature megakaryocytes) and negative (absence of collagen fibrosis, more than minimal reticulin fibrosis, and granulocytic dysplasia) information from bone marrow histology. In addition, cytogenetic studies are required to exclude the possibility of both chronic myeloid leukemia (CML) and atypical myelodysplastic syndrome (MDS). Most recently, in an elegant reappraisal of the WHO classification system for MPD, Thiele and Kvasnicka [7] argued that detailed analysis of bone marrow histopathology provides added information that is both diagnostically and prognostically relevant: (i) arbitrarily set cut-off limits for platelet count and hematocrit undermine recognition of latent/early cases of ET and PV, respectively; (ii) currently available molecular markers for BCR/ABL-negative MPD cannot distinguish one subtype from another; (iii) reticulin fiber content in CML carries a significant impact on survival; and (iv) ‘cellular phase’ myelofibrosis with myeloid metaplasia (MMM) might mimic ET in its clinical presentation and peripheral blood picture [7]. In the current issue of the journal, Gianelli et al. [8] provide additional evidence for the histopathologic heterogeneity of PVSG-defined ET. In a blinded fashion, a group of four experienced pathologists reclassified a series of 116 patients with PVSG-defined ET into ‘true’ ET (19%), ‘cellular phase’ MMM (21%), fibrotic MMM (49%), latent PV (8%) and otherwise unclassifiable (3%). With regard to the histologic distinction between ET and cellular phase MMM, the authors found megakaryocyte morphology and pattern of megakaryocyte clusters to be the most helpful. The major issue with this particular study is the rather high frequency of alleged MMM (70%) in the authors’ cohort of ET patients that raises concerns regarding the validity of the original diagnosis and/or reproducibility of the revised diagnoses. Where do we go from here? It could be argued that bone marrow examination might not be necessary for ET diagnosis in routine clinical practice regardless of whether or not bone marrow histology might suggest an alternative diagnosis such as cellular phase MMM. The rationale for this argument includes: (i) MPD-relevant cytogenetic and molecular abnormalities can be detected in peripheral blood cells using modern technology that includes fluorescent in situ hybridization; (ii) peripheral blood smear examination provides reliable clues for the presence of both bone marrow fibrosis and MDS; and (iii) information regarding histologic variants that is not otherwise flagged by peripheral blood examination might not be therapeutically relevant [9]. This contention is further fueled by the recent discovery of the JAK2 mutation, which is found in approximately 50% of patients with ET but not in reactive thrombocytosis [10]. Bowel Ischemia Secondary to Campylobacter Enterocolitis: A Case Series and Review of the Literature

Abstract:Campylobacter is the most common cause of diarrheal illness worldwide and is generally self-limiting. We present two cases of Campylobacter enterocolitis complicated by bowel ischemia in a 79-year-old male and a 53-year-old male, both presenting with abdominal pain and diarrhea and elevated lactate and C-reactive protein (CRP) levels. CT demonstrated common findings of pneumatosis intestinalis (PI) and portal venous gas. Exploratory laparotomy performed on the former demonstrated extensive small bowel infarction that was noncompatible with life and he was palliated postoperatively. The latter improved clinically after resection of the ischemic section of the small bowel with primary stapled anastomosis and closure. Clinicians need to be aware of the potentially fatal complications of Campylobacter-associated enterocolitis and should maintain a high clinical index of suspicion so that early surgical intervention can be considered in this patient population.

From the Gut to the Heart: A Rare Case of Salmonella dublin Pericarditis

Abstract:BACKGROUND Salmonella infections manifest typically as self-limiting gastroenteritis after the consumption of contaminated food. Extra-intestinal manifestations of Salmonella infections such as pericarditis are rare and are usually seen in severely immunocompromised individuals. Prior case reports suggest high rates of morbidity and mortality associated with Salmonella pericarditis. Here, we present a rare case of Salmonella dublin pericarditis. CASE REPORT A 45-year-old man presented to the Emergency Department reporting chest pressure and shortness of breath. An echocardiogram showed a large pericardial effusion without tamponade physiology. Pericardial window was performed, with removal of 700 cubic centimeters of bloody fluid, with presence of fibrinous debris in the pericardial cavity. A pericardial biopsy showed chronic pericarditis, and a lymph node biopsy was negative for malignancy. Antinuclear antibody (ANA), Lyme antibodies, and human immunodeficiency virus (HIV) testing were negative. Tissue culture revealed Salmonella species. Subsequent blood cultures grew Salmonella spp. Further history-taking revealed frequent travel and recent treatment with steroids for suspected Bell's palsy. Initially, the patient was treated with ceftriaxone, which was switched to ciprofloxacin after susceptibility testing revealed ceftriaxone resistance. Final identification of the organism revealed Salmonella dublin. The patient was discharged on colchicine, ibuprofen, and a 4-week course of ciprofloxacin. Outpatient follow-up showed improvement in inflammatory markers and symptoms. CONCLUSIONS This case illustrates the rarity of Salmonella-associated pericarditis, the importance of assessing a patient's risk factors, and obtaining an extensive history when searching for an etiology of pericarditis. Investigation into why a patient was susceptible to an infection with this organism should include medication assessment and age-appropriate cancer screening. Prompt identification and treatment of the offending organism can help prevent mortality.

Abstract:Campylobacter spp has become one of the most important foodborne pathogens. Moreover, Campylobacter can cause an economic burden on the human population since it can cause about 8.4% of diarrheal cases worldwide. In addition, Campylobacteriosis outbreaks have been reported sporadically in association with untreated drinking water. Water that does not take from a licensed water supplier is considered the main cause of water contamination. The most common risk factor for Campylobacteriosis transmission to humans is raw undercooked chicken. Contamination among poultry may be the result of the environmental conditions inside the farms or chicken factories that could be spread very fast among others. This infection is usually self-limited with no signs or symptoms but it can also present with severe symptoms, including diarrhea that can last for more than one week and can ultimately lead to dehydration, fever, and abdominal pain. However, the main recognized sequelae are Guillain-Barré Syndrome (GBS), Reactive Arthritis (REA), Irritable Bowel Syndrome (IBS), and rarely Bacteremia. Recently, many cases of Campylobacter spp show important resistance to various antibiotics such as tetracyclines and fluoroquinolones. Thus, the prevention and monitoring of this infection play an essential role. Campylobacteriosis is self-limiting, and most of the cases does not need to be treated. Some medical interventions such as electrolytes replacement, as well as hydration may be followed to treat immunocompromised patients, patients suffering from severe symptoms, pregnant women, and the elderly.
A Rare Case Of Salmonella Perimyocarditis In A Young Healthy Male

Abstract:Background Perimyocarditis is acute inflammation of the pericardium and the underlying myocardium. The most common cause reported is viral etiology, with a 0.2-1.5 % prevalence secondary to bacterial pathogens. Case reports describe salmonella-associated myocarditis in immunocompromised patients. We present a case of salmonella perimyocarditis in a young healthy male. Case Description A 37-year-old male patient with acute onset of abdominal pain, diarrhea and fever developed chest pain three days later, which prompted ED visit. This pain was midsternal, radiating to jaw with postural changes. He reported no known contaminated food exposure. He was febrile on admission with a max temperature of a 100.5F and tachycardic. A detailed infectious and cardiac work-up was done. Investigations were notable for leukocytosis with a left shift, acute kidney injury, significantly up trending troponin, elevated CRP and procalcitonin. Stool pathogen panel resulted positive for Salmonella species. EKG showed sinus tachycardia, early repolarization, and PR depression consistent with pericarditis. Transthoracic echocardiogram showed regional wall motion abnormality with reduced LVEF of 40-44%. Cardiac catheterization ruled out coronary artery disease. A subsequent cardiac MRI showed findings of diffuse sub epicardial enhancement (Figure) of the distal to basal lateral wall and inferior wall, patchy involvement of the anterior and septal walls, mild pericardial enhancement, which is consistent with perimyocarditis. He was managed with ibuprofen taper transitioned to as needed dosing, oral colchicine for three months, oral ciprofloxacin for two weeks, and guideline directed medical therapy for reduced LVEF. A 3-month follow up echocardiogram revealed normalization of LVEF and no regional wall motion abnormality. Discussion In this young healthy male, salmonella perimyocarditis was diagnosed and the patient had eventual complete recovery of cardiac function with medical treatment. Extra-gastrointestinal manifestations of Salmonella have a reported incidence of 5%, seen mainly in immunocompromised individuals. Salmonella species leading to cardiac involvement, mostly because of bacteremia or sepsis, have been cited only in case reports. Cardiac involvement mainly consists of endocarditis, with rare myocardial involvement. A systematic review found 24 reported cases of non-typhoid Salmonella myocarditis. Conclusion In conclusion, we present a case of salmonella perimyocarditis in a young healthy male. The true incidence of Salmonella myopericarditis is not known, mainly because of lack of suspicion and possible self-limited course of illness. Prompt investigative work-up and future studies may lead to increased reported prevalence.

Case Report: Campylobacter Bacteremia in India

Abstract:Gastroenteritis is the frequent clinical manifestation of infection by Campylobacter spp., whereas invasive infections such as bacteremia are less commonly reported. We report 12 cases of Campylobacter bacteremia from a tertiary-care hospital in India and describe their relatively unique characteristics compared with the previously published reports. These cases were detected after the introduction of new blood culture media. The most common symptoms were fever (eight cases, 67%), abdominal pain (six cases, 50%), and vomiting (four cases, 33%), whereas only two cases (17%) had diarrhea. Chronic liver disease was the common underlying condition affecting eight patients (75%). The median Child-Pugh and Model for End-Stage Liver Disease scores were 9 (interquartile range: 7-9.5) and 20 (interquartile range: 15-23) respectively. Campylobacter jejuni was isolated in six patients (50%), two of whom had preceding diarrhea and one patient who had culture evidence of C. jejuni in ascitic fluid. The in-hospital mortality was 8%, but this was due to subsequent Escherichia coli sepsis.
ENTEROHEMORRHAGIC ESCHERICHIA COLI LEADING TO HAEMOLYTIC UREMIC SYNDROME - CASE STUDY AND REVIEW

Abstract:Escherichia coli is a gram-negative bacillus and considered to be the normal pathogen of intestinal and extraintestinal manifestations depending upon the strain. A variety of strains exist that are responsible for causing myriads of clinical presentation. E.coli O157: H7 being the most common and severe bacterial pathogen is the leading cause of bloody diarrhea. EHEC (Enterohemorrhagic E.coli) is responsible for causing severe complications like HC (Hemorrhagic colitis). Herein, we present the case of a young girl with E.coli O157:H7 infection and review the related literature. A previously healthy 37-year-old female presented with bloody diarrhea, fever, headache, and lower abdominal pain. As per history she had eaten a hamburger, denied any recent travel and absence of inflammatory bowel disease or bloody stools in family history. Physical examination revealed normal vital signs and the physical findings were unremarkable except for severe abdominal pain. Her stool was hem-occult positive. The complete blood count was within normal limits except neutrophilia and leukocytosis. An abdominal ultrasound showed thickened bowel loops consistent with colitis. First week of her hospital course, she continued to have bloody diarrhea and severe abdominal pain. Her final stool submitted to the laboratory on day 7 was consistent with a blood clot, following her developed low urine output and hematuria, with a serum creatinine of 2.1 mg/dl on day 5. Her renal symptoms were treated with fluids. She was given supportive treatment, and her platelet count and hemoglobin were stabilized. In early stages of bloody diarrhea, parental hydration plays a major role in accelerating volume expansion. Rapid stool analysis for these bacteria can alert specialists to deal with severe complications like HUS.

Abstract:The case of 92-year-old male with a history of sepsis due to Clostridium septicum is described. Since the bacteria were also found at the site of joint aspiration, the patient underwent knee prosthesis removal. Due to the association between colorectal cancer and Clostridium septicum, a colonoscopy was performed. An adenocarcinoma was extracted in situ. At 5 months, the patient presented again with shortness of breath, acute retrosternal pain and fever. CT revealed a mycotic aneurysm of the descending aorta with Stanford B dissection and periaortic gas. Due to the fragility of the patient, conservative treatment was initiated.

Abstract:Introduction: There are very few documented cases of Escherichia coli endocarditis with cardiac abscesses in the literature. Here we describe a case presentation with diagnostic challenges and a multidisciplinary approach to management. Case description: This is a rare presentation of E. coli endocarditis in a patient with a prosthetic aortic valve. Initial tests were inconclusive and further investigation with transoesophageal echocardiography was required to make the diagnosis. Despite initial improvement, the patient deteriorated and ultimately died of complications related to the presentation. Discussion/conclusion: E. coli is a rare causative organism for endocarditis, which can itself be difficult to diagnose. A multidisciplinary approach to investigation and treatment is required when infective endocarditis is suspected. Transoesophageal echocardiography may be required to diagnose endocarditis when there is a strong clinical suspicion and risk factors present. Learning points: Infective endocarditis should be thoroughly investigated for in cases where there is a high clinical suspicion, but atypical organisms grown in blood cultures.A transoesophageal echocardiogram (TOE) may be a better imaging modality when endocarditis is strongly suspected, in comparison to a transthoracic echocardiogram (TTE).Escherichia coli endocarditis carries a high mortality rate, and early intervention is key in managing patients presenting with suspected endocarditis.
Pelvic Outlet Obstruction Secondary to Salmonella enterica serovar Bovismorbificans Abscess

Abstract:We present a case of a 30-year-old Hispanic male with pelvic outlet obstruction syndrome secondary to a large pelvic abscess caused by Salmonella enterica Bovismorbificans. This case demonstrates a potentially serious complication of a rare foodborne illness in the United States, in which an urgent surgical intervention was warranted. A computed tomography (CT) scan of the abdomen and pelvis demonstrated a large pelvic cystic mass causing near-total pelvic outlet obstruction of both gastrointestinal and genitourinary systems. A total of 1,250 mg of IV vancomycin and 3.375 mg of IV piperacillin-tazobactam were administered every eight hours, and an urgent decompressive transverse loop colostomy, Foley catheter placement, and percutaneous drainage were performed. Culture of the abscess fluid identified Salmonella enterica serotype Bovismorbificans, and the antibiotic regimen was changed to 1,000 mg IV ceftriaxone every 24 hours. Subsequent CT imaging displayed a reduction in abscess size. The patient was then discharged with a 14-day course of 500 mg of oral ciprofloxacin every 12 hours and 500 mg of oral metronidazole every eight hours. Imaging at three weeks post-discharge displayed resolution of the abscess, and the drain was removed. The patient had complete recovery and did well several months following treatment. While rare, Salmonella enterica serotype Bovismorbificans could potentially lead to serious complications such as giant pelvic abscess, in which a multidisciplinary team approach (i.e., medical, surgical, and interventional) is critical for a good outcome.
Survival in a Case of Emphysematous Cholecystitis With Sepsis Caused by Clostridium perfringens

Abstract:A 77-year-old man presented to the Department of Internal Medicine with a chief complaint of abdominal pain. During the outpatient examination, a computed tomography (CT) scan showed gallstones. The patient developed worsening abdominal pain and fever and was admitted to the emergency department. He was diagnosed with cholecystitis and hospitalized. Treatment with antimicrobial agents was initiated. On the second day of hospitalization, the patient developed a fever of 39°C, hypotension, and oliguria. An emergency CT scan was performed, which showed gas production in the gallbladder. He was diagnosed with emphysematous cholecystitis, and emergency percutaneous transhepatic gallbladder drainage was performed. The patient was transferred to the high-care unit, and intensive care was initiated. On the eighth day, a follow-up CT scan showed an abscess in the gallbladder bed, and drainage was performed percutaneously. His general condition gradually improved, and he was discharged from the hospital on day 24. The patient was readmitted for cholecystectomy three months after the initial admission. The prognosis of sepsis caused by Clostridium perfringens is extremely poor, with a mortality rate of 70%-100%. We present a case of emphysematous cholecystitis successfully treated with multimodal treatment despite the presence of sepsis due to Clostridium perfringens and discuss the possible prognostic factors by reviewing the literature.
Cardiac tamponade and constrictive pericarditis due to Actinomyces meyeri bacterial pericarditis: a case report

Abstract:Background: Purulent bacterial pericarditis (PBP) is a highly lethal infection of the pericardial space that arises as a complication of infective illnesses. Purulent bacterial pericarditis remains a diagnostic challenge given its non-specific clinical and investigative features and carries exceedingly high mortality rates due to fulminant sepsis and morbidity including constrictive pericarditis in survivors. We present our management of cardiac tamponade and subsequent constrictive pericarditis due to Actinomyces meyeri PBP. Case summary: A 53-year-old Caucasian male presented with acute New York Heart Association Class IV dyspnoea and chest discomfort, in the context of multiple hospital presentations over the preceding 8 weeks due to presumed recurrent viral pericarditis. On this admission, initial transthoracic echocardiography (TTE) demonstrated a large asymmetric pericardial effusion for which he underwent urgent pericardiocentesis. Serial TTE post-pericardiocentesis, however, demonstrated effusion re-accumulation and effusive-constrictive pericarditis, confirmed on cardiac magnetic resonance imaging. Fluid culture was positive for A. meyeri. He was diagnosed with PBP, but his condition deteriorated despite appropriate intravenous antibiotic therapy, necessitating semi-urgent surgical pericardiectomy. He recovered well and was discharged on Day 10 post-operatively. Discussion: Unlike uncomplicated acute viral or idiopathic pericarditis, PBP portends a very poor prognosis if unrecognized and untreated. Diagnostic challenges persist given its rarity in modern clinical practice; however, PBP should be considered in cases of seemingly recurrent pericarditis. Multi-modal cardiac imaging and careful analysis of pericardial fluid including cultures and lactate dehydrogenase/serum ratios may assist in earlier recognition. In this case, source control and symptom relief were achieved only with combined intravenous antibiotics, surgical evacuation, and pericardiectomy.
Salmonella Peritonitis Secondary to Gastroduodenal Perforation: An Unusual Presentation of Enteric Fever

Abstract:Salmonella-induced peritonitis, secondary to spontaneous gastrointestinal perforation, is a rare but potentially life-threatening condition. We present a case of a 62-year-old female with a history of systemic hypertension, who presented with diffuse abdominal pain and altered bowel habits. Initial evaluation suggested acute gastroenteritis, but worsening symptoms led to emergent exploratory laparotomy, revealing a gastric/duodenal perforation. Peritoneal fluid analysis and culture confirmed Salmonella Paratyphi A infection. The patient underwent an emergency laparotomy with omental patch repair and received intravenous ceftriaxone, leading to a full recovery. This case underscores the importance of considering Salmonella infection in the differential diagnosis of peritonitis, prompt surgical intervention, and appropriate antimicrobial therapy for optimal management and outcomes. Further research on epidemiological trends, host-pathogen interactions, and antibiotic resistance should be explored. Clinical studies should refine diagnostic criteria and treatment protocols, while animal models can aid in understanding pathophysiology and vaccine development for Salmonella peritonitis. Public health interventions and environmental studies will enhance prevention and control strategies.
Campylobacter jejuni prosthetic joint infection in an ulcerative colitis patient in the absence of gastrointestinal symptoms

Abstract:73-year-old man with ulcerative colitis was diagnosed with Campylobacter jejuni prosthetic knee infection. No preceding gastrointestinal illness was reported. Joint aspirate and operative cultures were negative; however, blood cultures were positive for Campylobacter jejuni. The role of ulcerative colitis in inducing bacteremia and subsequent prosthetic joint infection is discussed.

Abstract:Emphysematous cholecystitis (EC) is the presence of gas in the lumen and wall of the gallbladder and adjacent structures. EC is rare and is associated with notably higher morbidity and mortality. Although the etiology of EC is unclear, it has been suggested to be associated with gallbladder infection with gas-producing bacteria such as Clostridium perfringens, Klebsiella, and Escherichia coli. Patients With a history of diabetes and/or immunosuppression have been found to be susceptible to EC. EC may be diagnosed via plain abdominal x-rays or abdominal contrast-enhanced computed tomography (CT).

Campylobacteriosis: A rising threat in foodborne illnesses

Abstract:Campylobacteriosis is a foodborne illness that is contracted by eating contaminated food, particularly animal products like meat from diseased animals or corpses tainted with harmful germs. The epidemiology of campylobacteriosis varies significantly between low-, middle-, and high-income countries. Campylobacter has a complicated and poorly known survival strategy for getting past host barriers and causing sickness in humans. The adaptability of Campylobacter to unfavorable environments and the host's immune system seems to be one of the most crucial elements of intestinal colonization. A Campylobacter infection may result in fever, nausea, vomiting, and mild to severe bloody diarrhea in humans. Effective and rapid diagnosis of Campylobacter species infections in animal hosts is essential for both individual treatment and disease management at the farm level. According to the most recent meta-analysis research, the main risk factor for campylobacteriosis is travel, which is followed by eating undercooked chicken, being exposed to the environment, and coming into close contact with livestock. Campylobacter jejuni, and occasionally Campylobacter coli, are the primary causes of Campylobacter gastroenteritis, the most significant Campylobacter infection in humans for public health. The best antibiotic medications for eradicating and decreasing Campylobacter in feces are erythromycin, clarithromycin, or azithromycin. The best strategy to reduce the number of human infections caused by Campylobacter is to restrict the amount of contamination of the poultry flock and its products, even if the majority of infections are contracted through handling or ingestion of chicken.
