Abstract:Background: Dementia is a neurological condition characterized by deterioration in cognitive, behavioral, social, and emotional functions. Pharmacological interventions are available but have limited effect in treating many of the disease's features. Several studies have proposed therapy with music as a possible strategy to slow down cognitive decline and behavioral changes associated with aging in combination with the pharmacological therapy. Objective: We performed a systematic review and subsequent meta-analysis to check whether the application of music therapy in people living with dementia has an effect on cognitive function, quality of life, and/or depressive state. Methods: The databases used were Medline, PubMed Central, Embase, PsycINFO, and the Cochrane Library. The search was made up of all the literature until present. For the search, key terms, such as "music," "brain," "dementia," or "clinical trial," were used. Results: Finally, a total of eight studies were included. All the studies have an acceptable quality based on the score on the Physiotherapy Evidence Database (PEDro) and Critical Appraisal Skills Program (CASP) scales. After meta-analysis, it was shown that the intervention with music improves cognitive function in people living with dementia, as well as quality of life after the intervention and long-term depression. Nevertheless, no evidence was shown of improvement of quality of life in long-term and short-term depression. Conclusion: Based on our results, music could be a powerful treatment strategy. However, it is necessary to develop clinical trials aimed to design standardized protocols depending on the nature or stage of dementia so that they can be applied together with current cognitive-behavioral and pharmacological therapies.

Abstract:Background: Alzheimer's disease (AD) is a complex condition that affects various aspects of a patient's life. Music therapy may be considered a beneficial supplementary tool to traditional therapies, that not fully address the range of AD manifestations. Objective: The purpose of this systematic review is to investigate whether music therapy can have a positive impact on AD patients and on which symptoms. Methods: The main research databases employed have been PubMed and Cochrane, using the keywords "dementia", "music therapy", "Alzheimer", "fMRI", "music", and "EEG". Results: After removing duplicates and irrelevant studies, 23 were screened using set criteria, resulting in the final inclusion of 15 studies. The total number of participants included in these studies has been of 1,196 patients. For the fMRI analysis the search resulted in 28 studies on PubMed, two of which were included in the research; the total number of participants was of 124 individuals. The studies conducted with EEG were found using PubMed. The initial search resulted in 15 studies, but after a more accurate evaluation only 2 have been included in the analysis. Conclusions: Even though the data currently available is not sufficient to draw conclusions supported by robust statistical power, the impact of music therapy on AD neuropsychiatric symptoms deserves great interest. Further research should be ushered, possibly multicentric studies, led with neuroimaging and other recent techniques, which can eventually open views on the music role in improving the cognitive status in AD.

Abstract:IntroductionMusic therapy is one of the types of active ageing programmes which are offered to elderly people. The usefulness of this programme in the field of dementia is beginning to be recognised by the scientific community, since studies have reported physical, cognitive, and psychological benefits. Further studies detailing the changes resulting from the use of music therapy with Alzheimer patients are needed.ObjectivesTo determine the clinical improvement profile of Alzheimer patients who have undergone music therapy.Patients and methodsForty-two patients with mild to moderate Alzheimer disease (AD) underwent music therapy for 6 weeks. The changes in results on the Mini-Mental State Examination, Neuropsychiatric Inventory, Hospital Anxiety and Depression Scale and Barthel Index scores were studied. We also analysed whether or not these changes were influenced by the degree of dementia severity.ResultsSignificant improvement was observed in memory, orientation, depression and anxiety (HAD scale) in both mild and moderate cases; in anxiety (NPI scale) in mild cases; and in delirium, hallucinations, agitation, irritability, and language disorders in the group with moderate AD. The effect on cognitive measures was appreciable after only 4 music therapy sessions.ConclusionsIn the sample studied, music therapy improved some cognitive, psychological, and behavioural alterations in patients with AD. Combining music therapy with dance therapy to improve motor and functional impairment would be an interesting line of research.

Abstract:The objective of this research was to learn about the effects of music as a non-pharmacological therapeutic intervention applied to Alzheimer’s disease. To this aim, we evaluated its results regarding symptomatology and caregiver burden. Methods: This systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria. We searched Pubmed/Medline (NLM), Web of Science, Scopus, Cochrane and Google Scholar, including articles from 1 January 2011–20 July 2021, using the keywords “Alzheimer’s disease”, “Music therapy”, “Caregivers overload”, “Amnesia retrograde” and “Clinical evolution”. To select the articles our criteria included complete text availability, quantitative research of an experimental nature and studies which, at least, figured in SJR or in JCR. Results: We found a relationship between the application of music therapy in patients with Alzheimer’s disease and an improvement regarding symptomatology, as it smoothed down the progress of the disease. Using music therapy in Alzheimer’s patients also led to a decrease in caregivers’ burden and an increase in their well-being. Conclusions: Our results showed the benefits of music therapy, as it improved both patients’ symptomatology and caregivers’ burden.

Abstract:Alzheimer's disease (AD) is a progressive neurodegenerative disease associated with cognitive decline. Memory problems are typically among the first signs of cognitive impairment in AD, and they worsen considerably as the disease progresses. However, musical memory is partially spared in patients with AD, despite severe deficits in episodic (and partly semantic) memory. AD patients can learn new songs, encode novel verbal information, and react emotionally to music. These effects of music have encouraged the use and development of music therapy (MT) for AD management. MT is easy to implement and well‐tolerated by most patients and their caregivers. Effects of MT in patients with AD include improved mood, reduced depressive scores and trait anxiety, enhanced autobiographical recall, verbal fluency, and cognition. Here, we review musical memory in AD, therapeutic effects of studies using MT on AD, and potential mechanisms underlying those therapeutic effects. We argue that, because AD begins decades before the presentation of clinical symptoms, music interventions might be a promising means to delay and decelerate the neurodegeneration in individuals at risk for AD, such as individuals with genetic risk or subjective cognitive decline.

Abstract:Introduction Alzheimer's disease affects over 55 million people globally, projected to double by 2030 and triple by 2050. It ranks among the top 10 causes of death, necessitating urgent treatment and cures. Music therapy positively impacts brain development and plasticity, particularly in the medial temporal lobe. It shows promise as a therapeutic approach for enhancing neuroplasticity in the hippocampus, benefiting individuals with Alzheimer's disease. Materials and Methods This review explores music therapy as a cost-effective and side-effect-free avenue for patients to alleviate neurodegenerative symptoms based on the analysis of 21 other articles published by renowned authors. Discussion Alzheimer's disease causes hippocampal abnormalities and cognitive decline. Music, with musical mnemonics and enhanced working memory, shows promise for memory improvement. Music therapy induces neuroplasticity, potentially treating hippocampal neuron loss. Vocal training improves psychomotor speed and reduces dementia symptoms. Further exploration of music therapy may offer novel strategies for alleviating Alzheimer's symptoms and benefiting neurological disorders. Mechanisms underlying therapeutic benefits require further investigation. Music therapy holds inherent potential for Alzheimer's and related neurodegenerative diseases. Conclusion Empirical evidence shows musical training enhances hippocampal plasticity and gray matter volume, suggesting music therapy may treat hippocampal neuron loss in Alzheimer's disease, improving working memory. It holds promise for Alzheimer's and related neurodegenerative disorders by enhancing hippocampal function, but further research on underlying mechanisms is needed.
Effects of music therapy on cognition, quality of life, and neuropsychiatric symptoms of patients with dementia: A systematic review and meta-analysis of randomized controlled trials

Abstract:Dementia is a major cause of disability and dependency. Pharmacological interventions are commonly provided to patients with dementia to delay the deterioration of cognitive functions but cannot alter the course of disease. Nonpharmacological interventions are now attracting increasing scholarly interest. In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement, we aim to assess the effectiveness of music-based therapies on the cognition, quality of life (QoL), and neuropsychiatric symptoms of patients with dementia through a systematic review and meta-analysis of randomized controlled trials (RCTs). The PubMed, Embase, and Cochrane databases were searched for reports of RCTs examining the effectiveness of music-based therapies for dementia published as of April 2023. A total of 674 articles were screened, and 22 trials from 21 studies (1780 patients) met the eligibility criteria. In 15 trials, music-based therapies significantly improved the cognition of patients with dementia compared with non-music therapies. In 11 trials, music-based therapies also significantly improved the QoL of patients with dementia compared with non-music therapies. In six trials, music-based therapies significantly improved patients' neuropsychiatric symptoms compared with non-music therapies. In conclusion, music-based therapy is recognized as a safe and effective alternative approach for patients with dementia.
The Effects of Music Therapy on Cognition, Psychiatric Symptoms, and Activities of Daily Living in Patients with Alzheimer's Disease

Abstract:Background: Alzheimer's disease (AD) is the most common type of dementia, affecting millions of older people worldwide. However, pharmacological therapies have not achieved desirable clinical efficacy in the past decades. Non-pharmacological therapies have been receiving increased attention to treat dementia in recent years. Objective: This study explores the effects of music therapy on cognitive function and mental wellbeing of patients with AD. Methods: A total number of 298 AD patients with mild, moderate, or severe dementia participated in the study. The participants with each grade of severity were randomly divided into three groups, which were a singing group, a lyric reading group, and a control group. These three groups received different interventions for three months. All participants underwent a series of tests on cognitive functions, neuropsychological symptoms, and activities of daily living at baseline, three months, and six months. Results: The analysis shows that music therapy is more effective for improving verbal fluency and for alleviating the psychiatric symptoms and caregiver distress than lyrics reading in patients with AD. Stratified analysis shows that music therapy is effective for enhancing memory and language ability in patients with mild AD and reducing the psychiatric symptoms and caregiver distress in patients with moderate or severe AD. However, no significant effect was found for activities of daily living in patients with mild, moderate, or severe AD. Conclusion: This study suggests that music therapy is effective in enhancing cognitive function and mental wellbeing and can be recommended as an alternative approach to manage AD associated symptoms.
Effects of Music Therapy on Patients with Dementia—A Systematic Review

Abstract:Dementia is an increasingly common syndrome and while pharmacotherapy is available, its potential benefit is limited, especially in non-cognitive outcomes. Non-pharmacotherapy such as music therapy is potentially associated with improved outcomes. We assessed the effects of music therapy on patients with dementia to evaluate its potential benefits on dementia. Two independent reviewers searched MEDLINE, EMBASE, CINAHL, CENTRAL, and ClinicalTrials.gov databases for clinical trials, using the keywords “music therapy” and “dementia”. Study outcomes included cognitive function, behavioral and psychological symptoms of dementia (BPSD), and quality of life. A total of 82 studies were included, of which 43 were interventional clinical trials, and 39 were systematic reviews or meta-analyses. Significant improvements in verbal fluency occurred after music therapy, with significant reductions in anxiety, depression, and apathy. There were no significant improvements in cognition or daily functioning, and the results on quality of life and agitation were ambiguous. Limitations of studies included low patient numbers, lack of standardized music therapy, and high heterogeneity in outcomes. More large-scale clinical trials would allow for clearer conclusions on the benefits of music therapy in patients with dementia.

Abstract:Alzheimer’s Disease (AD) is a global health issue given the increasing prevalence rate and the limitations of drug effects. As a consequent, non-pharmacological interventions are of importance. Music therapy (MT) is a non-pharmacological way with a long history of use and a fine usability for dementia patients. In this review, we will summarize different techniques, diverse clinical trials, and the mechanisms of MT as it is helpful to the cognition in AD, providing reference for future research. Many articles have demonstrated that MT can reduce cognitive decline especially in autobiographical and episodic memories, psychomotor speed, executive function domains, and global cognition. MT is a promising intervention for strategy of dementia especially of AD and it must be started as early as possible. However, more evidences with prospective, randomized, blinded, uniform and rigorous methodological investigations are needed. And we should consider to combine MT with other cognitive stimulations such as dance, physical exercise, video game, art and so on.

Music therapy improves cognitive function and behavior in patients with moderate Alzheimer’s disease

Abstract:Alzheimer’s disease (AD) is in an increasingly common affliction among the world’s aging population, involving a steady decline in mental function and the onset of dementia. Music therapy has been demonstrated to provide health benefits when used as an adjuvant treatment. To determine whether music therapy can improve clinical outcomes of AD, we analyzed its effects on cognitive function and behavior in patients with mild AD in conjunction with conventional drug treatments. Sixty patients with mild AD were selected as the research subjects and randomly divided into an observation group and a control group. Patients in the control group were treated with drug therapy only, while the patients in the observation group were treated with music therapy and drug therapy. Scoring according to the mini-mental state examination (MMSE), the Montreal cognitive assessment (MoCA), and the neuropsychiatric inventory (NPI) was used to assess patients before beginning treatment (t0), at the end of treatment (t1), and three months after the completion of treatment (t2). We found that the MoCA and MMSE scores of patients in both groups improved over the course of treatment, but that patients in the music therapy group demonstrated a greater magnitude of improvement over patients in the control group. This finding suggests that music therapy can improve cognitive function and behavior in patients with AD, helping to delay the progression of the disease and improving prognosis.

Abstract:The etiology of depression is the degeneration of the brain cells involved in cognitive function before the other brain cells. It is characterized by a neurological condition that causes a reduction in terms of physical, social, and cognitive impairment and has no cure presently. These nonpharmacological approaches, such as music therapy, enhance living outcomes for those dealing with dementia and also reduce behavioral incidence. Among these strategies is music therapy, and individual or gap-time psychological and educational counseling. Many scientists believe in the advantages of music for the brain. The brain is affected by music function and enhances some cognitive abilities, including the mechanism of speech, alteration, memory, and learning. Music can activate the limbic system, subcortical circuits, and emotionally related systems, inducing the sensation of well-being. The music itself is quite effective at increasing cerebral plasticity. Music therapy has powerful stimulation for neuroplastic alterations in the adult and developing brain. Dementia can be cured by music therapy and music-based intervention (nonpharmacological intervention) rather than by medication. This study highlights dementia therapy utilizing the music therapy method.
Investigating Music Therapy's Cognitive Benefits in Patients with Dementia—A Network Meta-Analysis of Randomized Controlled Trials

Abstract:Numerous previous studies have shown the effectiveness of music therapy in enhancing cognitive functions in patients with dementia. Despite this, robust evidence in this field, especially concerning the comparison of different music therapy types, is lacking. Therefore, randomized controlled trials (RCTs) focusing on music therapy and cognitive functions in dementia patients, termed by "music" AND "dementia" OR "Alzheimer's disease" AND "cognitive", were identified from primary electronic databases to conduct this network meta-analysis (NMA). The primary outcome focused on the impact on cognitive functions, and the secondary outcome was the comparison of dropout rates between the intervention groups and the usual care control groups. Standardized mean difference (SMD) values and the corresponding 95% confidence intervals (CIs) were computed for effect evaluation. This study protocol has been registered in IPLASY (INPLASY202430082). A total of 14 RCTs with 1056 participants were enrolled, examining interventions including Active Music Therapy (AMT), Active Music Therapy with Singing (AMT + Sing), Rhythmic Music Therapy (RMT), Listening to Music (LtM), and Singing (Sing). The results indicated that RMT, AMT + Sing, and AMT all significantly improve cognitive functions in dementia patients, of which the SMD were 0.76 (95% CI = 0.32–1.21), 0.79 (95% CI = 0.03–1.49), and 0.57 (0.18–0.96), respectively. Compared with the control group (usual care), no music therapy type was associated with an increased dropout risk. In conclusion, music therapy can improve cognitive functions in patients with dementia without increasing the risk of dropout, particularly RMT, AMT + Sing, and AMT.

Abstract:Abstract Older adults in the early stages of Alzheimer’s and dementia deserve effective modalities that support their cognition, emotional well-being, and social engagement. Music has demonstrated potential to support these critical outcomes through its ability to simultaneously stimulate multiple areas of the brain and induce neuroplasticity. We reviewed randomized controlled trials for studies that specifically utilized active music-making interventions for older adults with early-stage cognitive decline to assess their effects on cognition, emotional well-being, and social engagement. Additionally, this review categorized the specific music activities employed by each intervention. We conducted searches on Medline (Ovid), APA PsycInfo (Ovid) CINAHL (Ebsco), and Embase (Elsevier). Our search yielded 285 potential studies. We analyzed 19 studies with 1,387 participants for potential effect sizes and intervention ingredients. Of the 19 studies, eight studies, recruiting a total of 460 participants, were used to conduct a random-effects meta-analysis to assess the effect of music on cognition. Meta-analytic aggregration of effect sizes showed that music had a modest positive effect on cognition compare with the control conditions (SMD= 0.26; P= 0.008; 95% confidence interval, 0.07, 0.45; I2= 5%). The musical activities of 1) singing and/or playing pre-composed songs and/or 2) creating music in the moment were utilized in the protocols. This research demonstrates that active music-making supports cognition for older adults with early-stage cognitive decline. Future music programs should consider inclusion of pre-composed songs, as well as music creation, to better understand the power for music to provide critical support for a rapidly growing segment of the population.

Abstract:Background: The use of music in cognitive interventions represents a possibility with potential worthy of further investigation in the field of aging, both in terms of prevention from dementia, in the phase of mild cognitive impairment, and in the treatment of overt dementia. Objectives: Currently, the types of music-based interventions proposed in the literature are characterized by wide heterogeneity, which is why it is necessary to clarify which interventions present more evidence of effectiveness in stimulating different cognitive domains. Method: The study was conducted in accordance with PRISMA guidelines for scoping reviews. By searching two different databases, PubMed and the Web of Science, all studies evaluating the cognitive effects of music-based interventions on people at early stages of cognitive decline (MCI or mild-to-moderate dementia) were selected. Results: The study selection included a total of 28 studies involving n = 1612 participants (mean age ranged from 69.45 to 85.3 years old). Most of the studies analyzed agree with the observation of an improvement, or at least maintenance, of global cognitive conditions (mainly represented by the results of the MMSE test) following music-based interventions, together with a series of other positive effects on verbal fluency, memory, and executive processes. Conclusions: The results of this review suggest the introduction of music-based interventions as complementary approaches to usual cognitive treatments. Also, the use of standardized and well-defined protocols, in addition to strong methodological research approaches, is suggested. Music-based interventions are recommended in the early stages of dementia, in MCI, and in a preventive sense in healthy older adults.

Abstract:Demographic aging is a worldwide phenomenon, cognitive and behavioral impairment is becoming global burden of nerve damage. However, the effect of pharmacological treatment is not satisfying. Therefore, we analyzed the efficacy of music therapy in elderly dementia patients, and if so, whether music therapy can be used as first-line non-pharmacological treatment. A comprehensive literature search was performed on PubMed, EMbase and the Cochrane Library from inception to September 2016. A total of 34 studies (42 analyses, 1757 subjects) were included; all of them had an acceptable quality based on the PEDro and CASP scale scores. Studies based on any type of dementia patient were combined and analyzed by subgroup. The standardized mean difference was -0.42 (-0.74 to -0.11) for disruptive behavior and 0.20 (-0.09 to 0.49) for cognitive function as primary outcomes in random effect models using controls as the comparator; the secondary outcomes were depressive score, anxiety and quality of life. No evidence of publication bias was found based on Begg's and Egger's test. The meta-analysis confirmed that the baseline differences between the two groups were balanced. Subgroup analyses showed that disease sub-type, intervention method, comparator, subject location, trial design, trial period and outcome measure instrument made little difference in outcomes. The meta-regression may have identified the causes of heterogeneity as the intervention method, comparator and trial design. Music therapy was effective when patients received interactive therapy with a compared group. There was positive evidence to support the use of music therapy to treat disruptive behavior and anxiety; there were positive trends supporting the use of music therapy for the treatment of cognitive function, depression and quality of life. Abstract:ObjectivesTo examine randomized controlled trials with active music-making interventions, in which older adults with probable mild cognitive impairment (MCI) or dementia physically participate in music, and their effects on cognitive functioning, emotional well-being, and social engagement. Participating in music-making is engaging and has shown diverse benefits. Additionally, this review categorized the music activities of each intervention. DesignSystematic review and meta-analysis. SettingLong-term care facilities, day centers, specialty outpatient units, and community. MethodsPublished randomized controlled trials of active music-making interventions to support older adults with probable MCI or dementia were identified (to March 15, 2021) using searches on Medline (Ovid), APA PsycInfo (Ovid), CINAHL (Ebsco), and Embase (Elsevier). The outcomes were cognitive functioning, emotional well-being, and social engagement, including self- and clinician-reported measures such as the Mini-Mental State Examination, Positive and Negative Affect Schedule, and the Beck Depression Inventory. Studies were critically appraised and studies with similar methodology were meta-analyzed. ParticipantsOlder adults with probable MCI and mild or moderate dementia. ResultsA total of 21 studies with 1472 participants were analyzed for potential effect sizes and intervention activities. Of the 21 studies, nine studies recruiting a total of 495 participants were used to produce a random-effects meta-analytic model for cognitive functioning. Music showed a small, positive effect on cognitive functioning; the combined standard mean difference for the experimental and control group was 0.31 (95% confidence interval [CI] 0.10, 0.52). There was low study heterogeneity, with an I2 of 25% (p = 0.004). Individual studies for emotional well-being in terms of quality of life and mood showed positive effect sizes, d = 1.08 and d = 1.74, respectively. ConclusionThis review shows that music-making has a small but statistically significant effect on cognitive functioning for older adults with probable MCI or dementia. Future music interventions can benefit from rigorous intervention protocols that isolate specific activities.

Scoping review of current state of the literature on the impact of music on symptoms of Alzheimer’s disease

Abstract:Across the globe, researchers, clinicians, and caregivers are continuing to search for solutions to the myriad problems caused by Alzheimer’s disease and related dementias (ADRDs). Music is an area of focus receiving considerable attention for its potential benefits to help manage the effects of neuropsychiatric symptoms upon individuals living with ADRDs. However, attention is lacking on the potential social value of music to positively influence quality of life for both individuals living with dementias and their surrounding communities. This scoping review compiles recent reports on this potential, highlighting the social nature of music and its various uses (e.g. listening, making) to change the often tense and reactive atmosphere surrounding both the individuals living with ADRDs and their surrounding communities into comparatively joyous living environments. Databases used included PubMed, EBSCOhost’s CINAHL and PsychINFO, Cochrane Library (sub‐search conducted using ALOIS, the Specialized Register of the Cochrane Dementia and Cognitive Improvement Group (CDCIG)), ProQuest’s Biological Science Collection, the Journal of Music Therapy and Nordic Journal of Music Therapy, and Google Scholar. After screening for eligibility and inclusion, 26 documents were identified, including review articles, experimental studies, recommendations, pilot studies, conference proceedings, book chapters, and executive summaries. Overall, this review concludes that music can facilitate prosocial engagement between persons living with ADRDs and their surrounding communities, due to a complex interplay of neurobiological mechanisms largely preserved in the ADRD population. More well designed and well controlled empirical studies are necessary to clarify contexts in which music and its various uses may be helpful to improve quality of life for both the individuals suffering from ADRDs and their surrounding communities. Such results would provide further evidence‐based support for the use of music to strengthen both formal and informal patient‐centered care models.

Abstract:Alzheimer's disease (AD) has become a worldwide health issue due to its increasing prevalence but limited pharmacologic effects. Besides traditional pharmacologic interventions, non-pharmacologic interventions are also very important. Music therapy (MT) is a long-established therapy with good applicability to people with dementia. This article mainly studies the cognitive and psychological influences of music on AD patients, mainly in listening to music, singing and doing rhythmic exercise. The advancement of music therapy has brought new treatment options to people who suffer from Alzheimer's disease, which is a positive development for patients and their families, making it more likely that AD can be cured. This paper will reflect on the role and comparisons from previous research. In the research results, listening to music, singing, and doing rhythmic exercise all have varying degrees of relief and improvement in depression, cognitive impairment, anxiety, and fugue. This research is helpful for scholars to understand the effects the music therapy has on patients.
