Abstract:Background: The antimicrobial activity exhibited by the plant extracts against various pathogens has previously been demonstrated to a limited extent. However, the antibacterial effects of most of them have not yet been clarified in endodontics. Aim: This study aimed to determine the antimicrobial activity of some medicinal plants' methanolic extracts against some root canal microorganisms. Methods and Materials: In this in vitro study, after gathering the five plants (pomegranate peel, clove, Zhumeria majdae, Eucalyptus galbie and green tea), their methanolic extracts were obtained by the maceration method, and the antimicrobial activity was determined using micro- and macro-dilution methods. The microorganisms tested in this study were Enterococcus (E.) faecalis, Porphyromonas (P.) gingivalis and Fusobacterium (F.) nucleatum . Non-ready-to-use calcium hydroxide (CH), ready-to-use Calcipex II CH and methanol were used as control materials. Statistical Analysis: The nonparametric Kruskal–Wallis and Friedman tests were used to analyse and evaluate the data and variables. Results: In all concentrations and time intervals studied, eucalyptus extract showed the highest antimicrobial activity against E. faecalis . All the extracts showed growth-inhibitory effects against P. gingivalis and F. nucleatum . There were no significant differences between the anti-enterococcus effects of eucalyptus extracts and non-ready-to-use CH (P > 0.05). Conclusion: There was a greater efficacy of Eucalyptus galbie extract than other extracts.

Abstract:Candida albicans is one of the most common and virulent representatives of Candida fungi which causes fungal lesions of the mucous membranes predominantly in people with compromised immune system. Candida resistance to classical antifungals such as polyenes, azoles, allylamines is a serious practical problem. Imidazole and triazole are the most commonly used antifungal agents for candidiasis today. Their mechanism of action lies in the inhibition of ergosterol biosynthesis which is indispensable for maintaining the structural integrity of fungal cell membranes. Activation of elimination systems is often associated with changes in the structure of fungal membranes, leading to a decrease in the supply of azoles into the fungal cell Screening of antifungal activity and synergism of antifungal activity of 166 aqueous-ethanol plant extracts with Fluconazole in relation to 2 clinical strains of Candida fungi (C.) (C. albicans and C. tropicalis) with different mechanisms of azole resistance isolated from patients with prosthetic stomatitis was conducted by means of agar diffusion method. The expressed direct antifungal activity of extracts of Calendula officinalis L. inflorescences, flowers of Limonium platyphyllum Lincz., flowers of Grataegus monogyna Jacq., roots of Potentilla repens L., the grass of Melilotus albus Medik, leaves of Peucedanum ruthenicum Bieb, leaves of Rhus coriaria L. and aerial part of Polythridum commune Hedw was determined in reference to the tested strains. Synergism of antifungal action with 1/4, 1/8 and 1/32 of Fluconazole MIC (minimal inhibiting concentration) in relation to both test strains of yeast-like Candida fungi was demonstrated by the flowers of Calendula officinalis L, aerial part of Melilotus albus Medik. The main purpose of the screening was to discriminate extracts in order to select those that would be promising for further testing by a more accurate сheckerboard titration method. The efflux mechanism of resistance to triazoles (in C. tropicalis test strain) is more susceptible to modification by biologically active compounds of medicinal plants. Regarding C. albicans test strain which combines the efflux mechanism of resistance with mutation(s) of ergosterol biosynthesis enzymes, the modifying effect of plant extracts is much less frequent and less pronounced. Therefore, it may be assumed that mutations of ergosterol biosynthesis enzymes provide high stability of the phenotypic manifestation of Candida azole resistance. Should that be the case, determining the azole resistance mechanism of Candida clinical isolates may be important for clinical practice. Combination therapy is more beneficial than monotherapy because it may provide more effective destruction or suppression of pathogens. The combined action of synergistic agents is quite easy to achieve in case of localized superficial lesions (which occur on the mucous membranes of the prosthetic bed in candidal prosthetic stomatitis): for example, using a systemic antifungal agent and a topical agent that contains a modifier of resistance of plant origin fungi. Synergistic interactions will increase the therapeutic efficacy of drugs, reduce the likelihood of resistance occurrence or further development, and will reduce dose-related toxicity.
Antifungal Effectiveness of Various Intracanal Medicaments against Candida albicans: An In Vitro Study

Abstract:Aim and objective: To evaluate the antifungal efficiency of various intracanal medicaments against Candida albicans. Materials and methods: One-hundred and forty extracted human mandibular premolar teeth were decoronated, and the biomechanical preparation was done in crown-down technique. 10 μL culture suspension of C. albicans was placed into the prepared root canal space of all the teeth. After 21 days of incubation, all the teeth were randomly divided into 7 groups with 20 teeth per each group. Group I: triple antibiotic powder (TAP) mixed with 3% chitosan solution; group II: TAP mixed with macrogol-propylene (MP) glycol; group III: chlorhexidine-guttapercha (CHX-GP); group IV: Vitapex; group V: 2% chlorhexidine gel; group VI: calcium hydroxide paste; group VII: normal saline with cotton (positive control) were used as intracanal medicaments, and the samples were incubated for 14 days. Intracanal medicaments were then completely removed using the canal brush. Dentinal chips were harvested from the walls of the root canal space in all samples using Gates-Glidden drills, were transferred into test tubes containing saline, and were serially diluted and placed in 140 Sabouraud dextrose agar plates, incubated at 37°C for 48 hours. Colony forming units (CFUs) of C. albicans were then counted using the digital colony counter. Results: One-way ANOVA test showed statistically significant difference among the seven groups, as the p value was < 0.001. Tukey's post hoc test showed intergroup comparison between group I and group V; group II and group III were statistically nonsignificant as p value was >0.05. Conclusion: 2% chlorhexidine gel and TAP mixed with 3% chitosan solution showed superior antifungal efficiency against C. albicans. Clinical significance: Chitosan solution's inherent antifungal efficiency and slow and controlled drug release make it as an effective alternate carrier in mixing it with TAP instead of mixing TAP with MP.

Abstract:Background: Candida albicans is one of the organisms living on the human body symbiotically, but, in hosts with low immunity it becomes one of the most pathogenic fungal organisms. Combretum zeyheri has been reported to have antifungal, antibacterial and antioxidant activities. Medicinal plants are believed to be non-toxic by the general public. Toxicity studies, however, have indicated that they are capable of causing numerous side effects, therefore, evaluation of safety is required. The objective of this study was to determine the toxicity of the antifungal constituents of Combretum zeyheri on mammalian cells. Methods: Alkaloids, saponins, flavonoids-enriched extracts and crude ethanol extracts were prepared from the leaves of Combretum zeyheri. The broth microdilution method was used to investigate for antifungal activity, with miconazole used as the positive control. The MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) assay was used to determine cell viability of the Candida albicans cells. The most potent extracts; the ethanol extract, alkaloids and saponins respectively, were further tested for their toxicity on sheep erythrocytes, mouse peritoneal macrophages and Jurkat T cells. Results: All Combretum zeyheri extracts displayed a dose-dependent antifungal activity and had IC50 values ranging from 16 μg/ml to 159 μg/ml for Candida albicans. The alkaloids, saponins and ethanol extracts were found to be non-toxic towards mouse peritoneal cells and Jurkat T cells. In the haemolysis assay, all extracts were haemolytic at varying degrees and showed their greatest haemolytic activity at the highest concentration of 5 mg/ml. The saponins were the least haemolytic, followed by the ethanol extracts and the alkaloids respectively. Although these extracts were haemolytic to some extent, they may considered safe at therapeutic concentrations since there was a large difference between the antifungal IC50 and haemolysis EC50 values, hence a large therapeutic window. Conclusions: Combretum zeyheri antifungal constituents are, therefore, a potential source of lead compounds which can be developed into antifungal drugs of natural origin owing to Combretum zeyheri's effective antifungal activity and low toxicity to mammalian cells.

Comparative evaluation of anti-biofilm and anti- adherence potential of plant extracts against Streptococcus mutans: A therapeutic approach for oral health

Abstract:Dental caries predominantly attributed to the cariogenic nature of Streptococcus mutans, continue to pose a substantial global challenge to oral health. In response to this challenge, this study aimed to evaluate the effectiveness of leaf extracts (LEs) and essential oils (EOs) derived from different medicinal plants in inhibiting the growth of Streptococcus mutans biofilm. In vitro and in silico approaches were employed to identify active compounds and assess their inhibitory effects on S. mutans. Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) were measured to determine the anti-biofilm and anti-adherence activity against S. mutans. Biofilm viability (CFU/mL) and extracellular polymeric substance (EPS) concentration were quantified. GC-MS analysis was utilized to identify active compounds in the most effective plant extracts exhibiting anti-S. mutans activity. A high-throughput screening focused on the interaction between these compounds and the target enzyme SortaseA (SrtA) using molecular docking was performed. Results indicated that Cymbopogon citratus displayed the highest efficacy in reducing S. mutans biofilm formation and adhesion activity, achieving 90 % inhibition at an MIC value of 12 μg/mL. Among the 12 bioactive compounds identified, trans-Carvyl acetate exhibited the lowest binding energy with SrtA (-6.0 Kcal/mole). Trans-Carvyl acetate also displayed favorable pharmacokinetic properties. This study provides novel insights into the anti-S. mutans properties of C. citratus and suggests its potential as a therapeutic approach for oral health. Further research is needed to explore the combined effect of plant extracts for enhanced protection against dental caries.

Abstract:We report the case of a 51‐year‐old man with an asymptomatic indurated plaque on the chest wall that was surgically excised and submitted for histopathological examination. Microscopically, the dermis was occupied by a neoplastic process with spindle‐shaped cells arranged in longitudinal fascicles with cytologic atypia; the abundant sclerotic stroma was composed of hyaline material. Tumoral cells showed immunohistochemical reactivity to smooth muscle markers. The diagnosis was desmoplastic leiomyosarcoma of the skin. This unusual entity is identified and discussed, and we review the literature.

Abstract:The advancement of phytochemical and phytopharmacological sciences has enabled elucidation of the composition and biological activities of several medicinal plant products including plant extract and essential oils. These products have been widely used around the world since ancient times for the treatment of various disorders such as diabetes, hypertension, peptic ulcer disease, microbial infection, sexual disorder and many more. Its popularity in the modern system of medicine is mainly due to their availability and fewer adverse reactions compared to synthetic drugs. Various scientifific investigations have been conducted to look for the potential of the extract from the plant or isolated compounds for the continued use of these products in the treatment and prevention of various kinds of human diseases. It is evident from the available literature and scientifific investigations that many plant species possess potential for use as a benefificial therapeutic remedy with multiple pharmacological actions such as analgesic, anti-inflflammatory, antipyretic, hypoglycemic, hypotensive, antimicrobial, antiulcer and anticonvulsant activities. The present review aims to provide relevant updated information about certain plant products, its composition, preparation and its fungicidal or fungistatic effects on different species of fungus as evaluated by studies done in the past. It introduces six medicinal plants that have been studied for their antifungal property and are found to be effective. The overall objective is to provide comprehensive information about the use of plant extract and essential oil for treating fungal infections and to explore the evidence supporting its effectiveness in treating fungal diseases without causing any serious adverse reactions.

Studies on immunization against plague. III. Quantitative serological studies on an immunizing antigen of Pasteurella pestis.

Abstract:The tested vaccines were all atoxic, except that after intracutaneous injection of 0.2 ml of 3.26 × 109 organisms/ml with adjuvant, a hard papule persisted for a few weeks. The potency ratio as tested in mice increased according to the number of organisms in the vaccine. The Al(OH)3 adsorbed vaccine appeared to be three times as effective as the aqueous one. In guinea pigs, the protective single dose is about 1.5 × 109 organisms. Given in two equal doses, 0.652 × 109 to 6.52 × 109 organisms effectively protect all the animals. However, the group given the large dose had better antibody titers and developed smaller buboes after infection. About 1.0 × 109 organisms, given in two basic immunization doses with 1 booster, also revealed a stronger immune reaction and smaller buboes. These findings suggest that the frequency of small doses of vaccine is likewise important in that the use of small doses can be compensated for by more frequent inoculations. One sensitizing dose for basic immunization is adequate to prepare for the booster effect, as demonstrated in both the guinea pig and primate experiments. A total dose of 4.89 × 109 organisms protected 90% of the guinea pigs when challenged by the intranasal route. In primates, a total dose of 4.89 × 109 and 8.15 × 109 organisms protected all animals from a challenge dose of 9.9 × 106 195/P organisms given intraperitoneally. In the animal experiments, the degree and duration of immunity was largely determined by the antigenic stimulation, enhanced by the aluminum adjuvant effect. The effective antigenic range of the tested vaccine preparations was relatively broad. When a good potential immunity exists in the host, an increase in antibody production can be obtained with a small booster dose.

Abstract:Introduction: A positive correlation has been observed between Streptococcus mutans, Candida albicans and periodontal diseases. Several substances including synthetic chemicals and natural agents have shown promising antimicrobial activity against the S. mutans- C. albicans complex. Extracts of Aloe Vera, Triphala, Tulsi, Pomegranate, and Acacia catechu have established therapeutic effects in medicine. The aim of the study was to evaluate the antimicrobial activity and the Minimum Inhibitory Concentration (MIC) of five herbal extracts in-vitro and to compare the antibacterial effects of 5 dentifrices containing the herbal extracts and a commercially available dentifrice on the plaque sample counts of S. mutans and C. albicans in-vivo Methodology: Extracts were obtained using a standardized method. Minimum inhibitory concentration was calculated for the test microorganisms using modified agar-well diffusion and macro broth dilution technique and the inhibition zone diameters were calculated. Dentifrices containing the extracts were formulated and the in vivo antimicrobial activity was evaluated by studying the CFU count from plaque samples of 420 participants. Results: Highest antimicrobial activity against S Mutans and C albicans was shown by Acacia catechu and Tulsi respectively and least was by pomegranate. Mean reduction in microbial counts (CFU/ml) by inoculating nutrient broth from baseline to 60 days was highest in Acacia catechu and Triphala for S Mutans and C albicans respectively. All herbal extracts performed better than the commercially available toothpaste. Conclusion: Dentifrices with herbal extracts of Aloe Vera, Triphala, Tulsi, Pomegranate, and Acacia catechu could serve as an alternative to commercially available dentifrices for treating oral diseases whose causative organisms are S. mutans and C. albicans.

AN OUTBREAK OF PESTE DES PETITS RUMINANTS IN GOATS AT DISTRICT LAHORE

Abstract:Peste des petits ruminants (PPR) is a highly contagious and economically important disease of small ruminants caused by a virus in the genus morbillivirus, family paramyxoviridae. This infection is responsible for high morbidity and mortality. In this study, an outbreak of PPR in goats was investigated in a well organized farm at district Lahore. Clinical signs, gross lesions and histopathological findings led to the suspicious of outbreak of PPR. Mesenteric lymph nodes and spleen samples collected from the dead animals were found positive for the PPR antigen by agar gel immunodiffusion (AGID) against hyperimmune anti-PPRV serum. Moreover, younger animals were more susceptible to PPR than adults.

Abstract:BACKGROUND: Complete microbial eradication from the root canal and 3-dimensional obturation of the canal space are necessary for an efficient root canal procedure.AIM: The current research was conducted to assess the antimicrobial effectiveness of herbal root canal irrigants and Chlorhexidine against Enterococcus faecalis.MATERIALS AND METHODS: The brain heart infusion (BHI) broth was used to grow the E. faecalis (ATCC) bacterial culture overnight before it was inoculated onto Mueller-Hinton agar plates. Agar-well diffusion was used to measure antibacterial inhibition. Respective propolis, Triphala, aloe vera, and chlorhexidine irrigants were added to the appropriate wells in agar plates and incubated for 24 hours at 37°C. Each well's bacterial inhibition zone was measured and recorded. Statistics were used to tabulate and analyze the results.RESULTS: Chlorhexidine indicated the maximum inhibitory zone against E. faecalis, subsequently propolis and Triphala, and the lowest by A. vera extract.CONCLUSION: Propolis, Triphala, and aloe vera were tested herbal remedies that demonstrated an inhibitory zone against E. faecalis. These irrigants are therefore, suitable for use as root canal irrigating solutions.
The inhibitory effects of carvacrol, nystatin, and their combination on oral candidiasis isolates

Abstract:Background: Candida, a common oral microbiota, can cause opportunistic fungal infections. With rising Candida infections and limited effective antifungals, new treatments are needed. This study investigates carvacrol essential oil's effect on oral candidiasis, alone and with nystatin, compared to nystatin alone. Materials and methods: In this study, oral samples were collected from dental clinic patients, especially denture users. The presence of Candida was confirmed and cultured from these samples. Candidiasis was detected by observing Candida colonies. Drug sensitivity was tested on 100 positive samples. The minimum concentration of inhibition and lethality of each isolate was evaluated using nystatin and carvacrol. The results were compared using two-way analysis of variance. Finally, the minimum inhibitory concentration (MIC) of nystatin and carvacrol was calculated individually and in combination. Results: The present study found that Candida albicans and non-albicans species were equally prevalent. Carvacrol showed significant biological activity against all Candida species, with an average MTT of 50.01%. The average MIC value of carvacrol was 24.96 µg/ml, indicating its potential to inhibit Candida growth. The mean Minimum Fungicidal Concentration (MFC) value of carvacrol was 23.48 µg/ml, suggesting its effectiveness in killing the fungi. Conclusion: The study's findings reveal that the MIC of carvacrol was significantly lower than that of nystatin and the combination of nystatin and carvacrol. This suggests that carvacrol holds potential as an effective herbal remedy for candidiasis.

One major approach adopted to overcome antimicrobial resistance is the use of plant extracts individually and/or with combination of antibiotics with plant extracts, which may lead to new ways of treating infectious diseases and essentially representing a potential area for further future investigations. In this study, the antifungal activities of Azadirachta indica leaf and Catharanthus roseus flower extracts against fluconazole-resistant Candida albicans strains (isolated from pregnant women with vulvovaginal candidiasis) and anti-methicillin-resistant Staphylococcus aureus (MRSA) were evaluated by agar well diffusion, microdilution, and biofilm inhibition assays. Subsequently, the determination of the combined antimicrobial activity of the individual plant extracts with (fluconazole and voriconazole) and (ampicillin, tetracycline, and streptomycin) against C. albicans strains and MRSA, respectively, was evaluated by checkerboard microdilution assay. Results from the study showed that the antimicrobial activity of the two plant extracts determined by time-kill kinetics was fungistatic with their MICs ranging from 0.1 to 4 mg/mL. Interestingly, all extracts were proved as good biofilm inhibitors of resistant C. albicans and MRSA from 10.1 to 98.82%. Their combination interaction with fluconazole, voriconazole, ampicillin, tetracycline, and streptomycin ranged from synergy to antagonism as per the parameters used. Overall, these results showed that A. indica leaf and C. roseus flower extracts have significant antifungal property. Furthermore, A. indica leaf and C. roseus flower extracts alone or in combination with fluconazole and voriconazole could provide a promising approach to the management of candidiasis caused by drug-resistant strains as well as their interaction with the antibacterial agents to combat the common infections caused by MRSA.

Abstract:Background: Candida species, especially Candida albicans, cause oral candidiasis, also known as oral thrush. It affects the elderly, newborns, patients under antibiotics, chemotherapeutic agents, and patients with weaker immune systems. Although successful, traditional antifungal medications are available, they may have negative effects and do not prevent recurrence. Conventional antifungal medications have a restricted range of effectiveness; hence, the need for a novel antifungal agent arises. Chitosan, a natural polymer made from chitin found in crustaceans, kills fungal cells by damaging membranes. Excellent biocompatibility and biodegradability make it a promising medical material. Probiotics, live bacteria, compete with pathogenic microbes for adhesion sites and nutrients, produce antimicrobials such as lactic acid, hydrogen peroxide, and bacteriocins, and modulate the host's immunological response. Adding probiotics to chitosan nanoparticles (CSNPs) boosts their antifungal activity against Candida species and increases their stability and transport to the infection site. Methods: The antifungal activity was evaluated through in vitro experiments utilizing Candida strains that are significant to oral candidiasis. Different concentrations of CSNPs and formulations loaded with probiotics were examined to assess their effectiveness. The antifungal properties were assessed using microbiological assays, specifically agar diffusion and minimum inhibitory concentration (MIC). Results: The study demonstrated a notable fungicidal impact of CSNPs combined with probiotics against oral candidiasis. Furthermore, the MIC values were lower for the probiotic-loaded CSNPs than for chitosan alone or probiotics alone. Conclusion: The combination of CSNPs and probiotics shows potential in effectively combating oral candidiasis by inhibiting fungal growth. This novel method offers a promising strategy for creating therapeutic treatments for oral candidiasis by utilizing the combined benefits of chitosan and probiotics to combat fungal infections effectively.

Abstract:A BSTRACT Objectives: The aim of the study was to assess the effectiveness of ZOE-based, calcium hydroxide, and epoxy resin-based sealers on modification with three herbal extracts. Materials and Methods: Methanolic extracts of selected herbs were combined with ZOE-based, calcium hydroxide, and epoxy resin-based sealers. Cultures were prepared from E. faecalis and C. albicans and agar plates prepared. Prepared mixtures were inoculated in punched holes, and inhibitory zones were measured. Results: No statistical significance was obtained on comparing mean scores of test groups. Conclusion: None of the combinations used was found to be significantly better than others.

Abstract:Background: In the past few decades focus of research has been toward herbal medicines because of growing bacterial resistance and side effects of antimicrobial agents. The extract derived from the plants may increase the efficacy of antibiotics when used in combination against pathogenic bacteria. In the current study, the synergistic antibacterial efficacy of plant extracts in combination with antibiotics has been assessed on selected periodontal pathogens. Methods: Ethanolic extracts were prepared from Salvadora persica (Miswak) and Cinnamomum zeylanicum (Ceylon cinnamon), by the soxhalate method. Plaque samples were collected from clinical periodontitis patients to isolate and grow the periodontal pathobionts under favorable conditions. Susceptibility of bacteria to the extracts was assessed by gauging the diameter of the inhibition zones. Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of plant extracts were determined against each bacterium. Synergistic activity of plants extract in combination with antibiotics against the bacteria was also assessed by measuring the diameter of the inhibition zones. Results: Ethanolic extract of both the plants showed an inhibitory effect on the proliferation and growth of all four strains of periodontal pathobionts. Maximum antibacterial activity was exhibited by C. zeylanicum against Tannerella forsythia (MIC = 1.56 ± 0.24 mg/mL, MBC = 6.25 ± 0.68 mg/mL), whereas among all the studied groups the minimum activity was reported by C. zeylanicum against Aggregatibacter actinomycetemcomitans the (MIC = 12.5 ± 3.25 mg/mL, MBC = 75 ± 8.23 mg/mL). Combination of herbal extracts with different antibiotics revealed a synergistic antibacterial effect. The best synergism was exhibited by S. persica with metronidazole against A. actinomycetemcomitans (27 ± 1.78). Conclusions: Current in vitro study showed variable antibacterial activity by experimented herbal extracts against periodontal pathobionts. The synergistic test showed significant antibacterial activity when plant extracts were combined with antibiotics.

Abstract:The rise of phytotherapy has enabled the utilization of various plant species for medicinal purposes, such as Cymbopogon citratus (C. citratus), providing solutions for oral pathologies, such as oral candidiasis. The PubMed, Web of Science, Scopus, and SciELO databases were searched. In vivo and in vitro studies on the action of C. citratus against oral candidiasis were included, and ROBINS-I was used to determine study quality and risk of bias. The search yielded 1922 articles, of which 10 met the inclusion criteria. Limited scientific evidence exists regarding the use of C. citratus for oral candidiasis. However, studies have indicated its potent antifungal effects. Further studies, preferably clinical trials, are necessary to confirm this effect and to enable its clinical use as a therapeutic option.
