Abstract:Purpose: To assess the effect of obese patientˈs body mass index (BMI) on organ doses from CT and PET exams. Methods: A set of 5 computational phantoms with different BMIs were created for PET and CT dose calculations using the Monte Carlo N‐Particle Extended (MCNPX) code. The phantoms represent patients having a BMI ranging from 23 (normal weight) to 44 (morbidly obese) with increasing amounts of subcutaneous and visceral fat. The external CT dose calculations assumed a whole‐body, diagnostic scan with a GE LightSpeed 16 scanner operating at a tube voltage of 120 kVp. The internal PET dose was based on S‐values for F‐18 and the ICRP‐106 FDG biokinetic model. Results: For the same tube current, ratios of the morbidly obese phantom CT organ doses compared to the normal weight phantom ranged from 0.46∼1 with an average of 0.7 because of shielding by the extra fat. This effect was greatest for deep organs in the abdomen (e.g. colon, lungs, stomach, liver, urinary bladder). For the same injected radioactivity, the average PET organ dose ratios were 0.95 and 0.65 for source organs (e.g. brain, heart, liver, urinary bladder) and organs in the remainder, respectively (e.g. colon, stomach, spleen, etc.). These PET dose differences are due primarily to the obese phantomˈs smaller remainder source S‐values which arise from the addition of the extra fat to the remainder compartment. Conclusions: After adjusting for the increased tube current and tube voltage, the results suggest that obese patients can receive higher absorbed dose to some organs, depending on the protocols. This work, as part of the VirtualDose software development, demonstrates the ability to improve the estimate of organ doses from PET and CT scans for obese patients and will be useful to on‐going efforts to optimize image quality and imaging doses.
Low-Dose Computed Tomography with Adaptive Statistical Iterative Reconstruction and Low Tube Voltage in Craniocervical Computed Tomographic Angiography: Impact of Body Mass Index

Abstract:Objectives To assess radiation dose and image quality using Adaptive Statistical Iterative Reconstruction (ASIR) in craniocervical computed tomographic angiography and to further evaluate the impact of body mass index (BMI) on image quality.Methods A total of 178 consecutive patients (112 men, 66 women; age range, 25-79 years) were enrolled in this prospective study and randomly divided into 2 groups: group A (conventional group): 120 kV, filtered back-projection reconstruction, and group B (low-dose group): 100 kV, 40% ASIR reconstruction. Radiation dose and image quality between groups A and B were compared. Correlation coefficients were calculated between quantitative image quality measurement and BMI, and between radiation dose and BMI using Pearson correlation. Two experienced radiologists independently evaluated the image quality with 4-point scores, and interrater reliability was calculated using analysis.Results The volume CT dose index, dose-length product, and effective dose of group B were lower than those of group A (each P < 0.01), with decreases of 23.99%, 25.15% and 25.47% respectively. Positive correlations existed between radiation dose and BMI for both groups A and B (each P < 0.01). Group B had lower image noise, higher attenuation, higher signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR), and higher subjective score than did group A (each P < 0.01). Computed tomographic values had negative correlations with BMI for the head, neck, and shoulders in both groups A and B (each P < 0.01). Image noise, SNR, and CNR had no correlations with BMI for the head and neck in both groups (each P > 0.01). On the contrary, image noise had a positive correlation, and SNR and CNR had a negative correlations with BMI for the shoulders in group A (each P < 0.01), but in contrast, no such correlations were found in group B (each P > 0.01).Conclusions In craniocervical CTA, ASIR can improve the image quality and reduce radiation dose in patients. Furthermore, ASIR reduced the variances of image quality of different body sizes in the shoulders.

Abstract:Objectives: This study sought to describe worldwide variations in the use of myocardial perfusion imaging hardware, software, and imaging protocols and their impact on radiation effective dose (ED). Background: Concerns about long-term effects of ionizing radiation have prompted efforts to identify strategies for dose optimization in myocardial perfusion scintigraphy. Studies have increasingly shown opportunities for dose reduction using newer technologies and optimized protocols. Methods: Data were submitted voluntarily to the INCAPS (International Atomic Energy Agency Nuclear Cardiology Protocols Study) registry, a multinational, cross-sectional study comprising 7,911 imaging studies from 308 labs in 65 countries. The study compared regional use of camera technologies, advanced post-processing software, and protocol characteristics and analyzed the influence of each factor on ED. Results: Cadmium-zinc-telluride and positron emission tomography (PET) cameras were used in 10% (regional range 0% to 26%) and 6% (regional range 0% to 17%) of studies worldwide. Attenuation correction was used in 26% of cases (range 10% to 57%), and advanced post-processing software was used in 38% of cases (range 26% to 64%). Stress-first single-photon emission computed tomography (SPECT) imaging comprised nearly 20% of cases from all world regions, except North America, where it was used in just 7% of cases. Factors associated with lower ED and odds ratio for achieving radiation dose ≤9 mSv included use of cadmium-zinc-telluride, PET, advanced post-processing software, and stress- or rest-only imaging. Overall, 39% of all studies (97% PET and 35% SPECT) were ≤9 mSv, while just 6% of all studies (32% PET and 4% SPECT) achieved a dose ≤3 mSv. Conclusions: Newer-technology cameras, advanced software, and stress-only protocols were associated with reduced ED, but worldwide adoption of these practices was generally low and varied significantly between regions. The implementation of dose-optimizing technologies and protocols offers an opportunity to reduce patient radiation exposure across all world regions.

Abstract:AbstractIn this report we have evaluated radiation effective dose received by patients during ECG-gated CCTA examinations based on gender, heart rate, tube voltage protocol and body mass index (BMI). A total of 1,824 patients were retrospectively recruited (1,139 men and 685 women) and they were divided into Group 1 (CCTA with calcium scoring), Group 2 (CCTA without calcium scoring) and Group 3 (only calcium scoring), where the association between gender, heart rate, tube voltage protocol and body mass index (BMI) were analysed. Examinations were performed using a retrospective ECG-gated CCTA protocol and the effective doses were calculated from the dose length product with a conversion coefficient of 0.026 mSv.mGy−1cm−1. No significant differences were observed in the mean effective dose between gender in all groups. The mean estimated dose was significantly higher when the heart rate was lower in Group 1 (p < 0.001) and Group 2 (p = 0.002). There were also significant differences between the mean effective dose in tube voltage protocol and BMI among the three groups. The mean effective dose was positively correlated with BMI (p < 0.001), but inversely related to the heart rate. This study supported the theory that a high heart rate, low tube voltage and low BMI could significantly reduce radiation dose exposure.

Abstract:Abstract Purpose Accurate measurement of renal mass size is crucial in the management of renal cancer. With the burdensome cost of imaging yet its need for management, a better understanding of the variability among patients when determining mass size remains of urgent importance. Current guidelines on optimal imaging are limited, especially with respect to body mass index (BMI). The aim of this study is to discern which modalities accurately measure renal mass size and whether BMI influences such accuracy. Methods A multi-institutional chart review was performed for adult patients undergoing partial or radical nephrectomy between 2018 and 2021, with 236 patients ultimately included. Patients were categorized by BMI (BMI 1: 18.5–24.9, BMI 2: 25–29.9, BMI 3: 30–34.9, and BMI 4: ≥ 35). The greatest mass lengths were compared between the pathology report and the following: computerized tomography (CT), renal ultrasound, and magnetic resonance imaging (MRI). Results The difference between greatest length on CT with contrast and MRI were significantly different when compared to pathologic measurement. BMI groups 3 and 4 were found to have a significant difference in size estimates compared to BMI 2 for CT with contrast. No difference was found between size estimates by BMI group for any other imaging modality. Conclusion CT with contrast becomes less accurate at estimating mass size for patients with BMI > 30. While contrast-enhanced CT remains a vital imaging modality for tissue enhancement in the context of unknown renal masses, caution must be used for mass size estimation in the obese population.

Reducing radiation dose in 64-row spiral CT coronary angiography: study based on individualized scan dosage protocol

Abstract:Objective To investigate the methods of reducing radiation dose in CT coronary angiography through optimizing individualized scan dosage protocol.Methods Two hundred patients (group A)underwent coronary CTA examination which was performed with fixed 120 kV and variable mA according to their BMI.The mA was set as 150-300 mA(BMI ＜ 18.5 kg/m2),300-500 mA (18.5 kg/m2 ≤ BMI ＜ 25.0 kg/m2),and 500-800 mA(BMI ≥ 25.0 kg/m2).When all examinations were finished,a linear regression was employed to analyze the correlation between mA and BMI,body surface(Suf),image noise(SD)respectively.The results of the analysis were used to formulate a regression equation,which was further used to establish a table list for quick search on how much mA that individualized coronary CTA scan would need.Another 200 patients(group B)enrolled for the individualized scan were scanned under new protocol that previous study established.The tube voltage was 100 and 120 kV.The tube current was variable according to the data in the table list.One-way ANOVA and Kruskal-wallis H test were used for statistics.Results Regression equation between mA and BMI,Suf,SD was:mA =17.984 × BMI + 169.149 × Suf-2.282 × SD-361.039.The SD(group A:32.08 ± 5.80,group B:28.60±4.47),dose index volume(CTDIvol)[group A:(41.97 ± 11.37)mGy,group B:(33.18±10.07)mGy],effective dose(ED)[group A:(10.91 ±3.07)mSy,group B:(8.83 ±2.72)mSv]had significant differences between the two groups(F =43.45,63.71,49.07 respectively,P ＜0.01 for all).The SD and ED results obtained in group B were better than those in group A.Conclusion Better performances were obtained when BMI combined Suf was used as a new individualized protocol than when BMI was used only,which means good image quality and lower radiation dosage in coronary CTA examination.

Abstract:Cone-beam computed tomography (CBCT) is a widely used imaging technique in interventional radiology. Although CBCT offers great advantages in terms of improving comprehension of complex angioarchitectures and guiding therapeutic decisions, its additional degree of radiation exposure has also aroused considerable concern. In this study, we aimed to assess radiation exposure and its influential factors in patients undergoing CBCT scans of the head and abdomen during interventional procedures. A total of 752 patients were included in this retrospective study. Dose area product (DAP) and reference air kerma (RAK) were used as measures of patient dose. The results showed that the median values of DAP were 53.8 (50.5–64.4) Gy⋅cm2 for head CBCT and 47.4 (39.6–54.3) Gy⋅cm2 for that of the abdomen. Male gender and body mass index (BMI) were characterized by increased DAP and RAK values in both head and abdominal CBCT scans. Larger FOV size was associated with a higher DAP but a lower RAK value, especially in head CBCT scans. Exposure parameters under automatic exposure control (AEC) also varied according to patient BMI and gender. In conclusion, the patients received slightly higher radiation doses from head CBCT scans than from those applied to the abdomen. BMI, gender, and FOV size were the key factors that influenced the radiation dose administered to the patients during CBCT scans. Our results may help to define and minimize patients’ exposure to radiation.

Abstract:The study was aimed to evaluate the image quality and radiation dose during female chest computed tomography (CT) screening using organ-based tube current modulation technology X-ray combined applications to reduce exposure (XCARE).Five hundred sixty female individuals undergoing chest CT scan were prospectively enrolled and divided into 4 groups based on body mass index (BMI). Then they were randomly and equally divided into control and experimental subgroup and respectively accepted conventional low-dose and XCARE technology spiral CT scan with same parameters. Signal-to-noise ratio and contrast-to-noise ratio were calculated. The quality of the images was evaluated by 2 radiologists using a 5-point scale.Among experimental subgroups of the 4 BMI groups, Signal-to-noise ratios, CT dose index of volume, dose-length product, effective dose, and contrast-to-noise ratio all displayed significant differences, as well as in control subgroups (P < .001). Both the experimental and control subgroups showed an increasing trend in radiation dose with the increasing of BMI. Parameters of image quality and radiation dose displayed no significant differences between control and experimental subgroups in the 4 groups. In multiple linear regression analysis, age and scanning protocol were not associated with radiation dose (P > .05), while BMI was significantly associated with increased CT dose index of volume (P < .05). The display of the lesions for the patients in the control and experimental subgroups of the 4 groups with different BMIs exhibited no statistically significant difference.The same image quality and radiation dose can be obtained using XCARE technology compared to conventional chest CT scans, which can be used regularly in female patients.Advances in knowledge: Using automatic tube current modulation technology to reduce exposure in breast. Abstract:Objective To assess the image quality of coronary CT angiography(CCTA) by using high-pitch spiral CT under 80 kV in patients with a normal body mass index(BMI), and to investigate the feasibility of low radiation dose.Methods 60 patients,who suspicious coronary artery diseases and BMI between 18.6-25.0 kg/m2 underwent CCTA in our hospital.All patients were randomly divided into group A and group B(each with 30).Patients in group A were given 100 kV CCTA and filter back-projection algorithm (FBP), and patients in group B were given 80 kV CCTA and iterative reconstruction(IR).CT values of aortic root, right coronary artery (RCA), left anterior descending artery (LAD), left circumflex (LCX) and standard deviation (SD) were measured.Then signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated.All images were given quality subjective scoring by using 3-score method.The CT dose index was recorded and effective radiation dose (ED) was calculated.Results ED of group A and group B was 0.8 mSv and 0.41 mSv, respectively.Statistically significant difference was found between these two groups.In the study, SD was (28±6) HU in group A and (46±9) HU in group B, and the difference was statistically significant between the two groups.The CT values of aortic root, RCA, LAD and LCX under 80 kV were significantly increased, and the difference was statistically significant between the two groups.There was no statistically significant difference between two groups in RCA-SNR, LAD-SNR and RCA-CNR.But in LCX-SNR, LAD-CNR, LCX-CNR, there was statistically significant difference between two groups.There was no significant difference between the two groups in coronary artery segments which could be used for diagnosis.Conclusion For patients with a normal BMI, CCTA using Flash spiral CT under 80 kV could diagnose coronary artery disease and significantly reduce the radiation dose.

Abstract:[Abstract ] Objective CT coronary angiography (CTCA) is now generally used in clinical practice, but its high radiation has been drawing constant attention .This study was to assess the feasibility of body mass index (BMI)-adapted tube voltage prospec-tively ECG-triggered high-pitch CTCA . Methods A total of 102 patients with or suspected with coronary heart disease undergoing CTCA were assigned to an experimental group (n=72) and a control group (n=30), the former with BMI-adapted tube voltage (70 kV for BMI<23 kg/m2 , 80 kV for BMI 23-25 kg/m2 , and automated tube voltage for BMI >25 kg/m2 ) while the latter with 120 kV prospectively ECG-triggered sequential technique .Objective and subjective image quality and radiation dose were evaluated and com -pared between the two groups . Results Compared with the control group , the experimental group showed significant increases in the CT values of the right coronary artery ([399.6 ±61.3] vs [562.2 ±152.3] HU), left main coronary artery ([380.3 ±46.2] vs [571.1 ±159.5] HU), left anterior descending artery ([356.0 ±39.7] vs [559.4 ±140.0] HU), and left circumflex artery <br> ([348.9 ±52.3] vs [553.0 ±168.6] HU) (all P <0.01).No statistically significant differences were found between the two groups in the contrast-to-noise ratio of the right coronary artery (26.6 ±13.9 vs 27.2 ±12.6), let main artery (24.7 ±11.6 vs 27.5 ±12.4), left anterior descending artery (22.8 ±10.9 vs 27.2 ±12.5), and left circumflex artery (22.1 ±10.8 vs 26.8 ±12.5) (all P>0.05), or in the subjective image quality ( 3.2 ±0.8 vs 3.1 ±0.8, P = <br> 0.604).The effective radiation dose was remarkably lower in the experimental group than in the control ([0.5 ±0.5] vs [4.5 ±1.3] mSv, P<0.01), with an 88.9% reduction in 120 kV prospectively ECG-triggered sequential CTCA. Conclusion BMI-adapted tube voltage prospectively ECG-triggered high-pitch CTCA is feasible .

Abstract:Objectives: To evaluate the relation between the coronary calcium score and the posterior choice of kilovoltage according to radiologists' criteria in a standard coronary CT angiography protocol to rule out coronary disease. To quantify the reduction in ionizing radiation after linking kilovoltage to patients' body mass index in a low-dose protocol with iterative model reconstruction. To evaluate the image quality and diagnostic performance of the low-dose protocol. Material and methods: We compared anthropometric characteristics, calcium score, kilovoltage levels, size-specific dose estimates (SSDE), and the dose-length product (DLP) between a group of 50 patients who were prospectively recruited to undergo coronary CT angiography with a low-dose protocol and a historical group of 50 patients who underwent coronary CT angiography with the standard protocol. We correlated these parameters, the number of coronary segments that could not be evaluated with and without temporal padding, the attenuation, and the signal-to-noise ratio in the ascending aorta in the low-dose protocol with excellent imaging quality according to a semiquantitative scale. To calculate the diagnostic performance per patient, we used 24-month clinical follow-up including all tests as the gold standard. Results: In the standard protocol, the presence of coronary calcium correlated with the selection of high kilovoltage (p = 0.02); this correlation was not found in the low-dose protocol (p = 0.47). Median values of SSDE and DLP were significantly (p < 0.001) lower and less dispersed in the low-dose protocol [9.22 mGy (IQR 7.84-12.1 mGy) vs. 26.5 mGy (IQR 21.3-36.3 mGy) in the standard protocol] and [97 mGy cm (IQR 78-134 mGy cm) vs. 253 mGy cm (IQR 216-404 mGy cm) in the standard protocol], respectively. The overall quality of the images obtained with the low-dose protocol was considered good or excellent in 96% of the studies. The parameters associated with image quality in a multivariable model (C statistic = 0.792) were heart rate (estimated coefficient, -0,12 [95% confidence interval: -0.2, -0.04]; p < 0.01) and the SSDE (estimated coefficient, -0,26 [95% confidence interval: -0.51, -0.01]; p < 0.05). The CAD-RADS modifier for a not fully evaluable or diagnostic study was used on two occasions (4%); the final measures for the diagnosis of coronary disease were sensitivity 100%, specificity 94%, and efficacy 94%. Conclusions: In the standard protocol, the radiologist selects higher kilovoltage for CT angiography studies for patients whose previous calcium score indicates the presence of coronary calcium. In the low-dose protocol, linking kilovoltage with body mass index enables the dose of radiation to be reduced by 65% while obtaining excellent or good image quality in 96% of studies and excellent diagnostic performance.
Coronary CT Angiography Radiation Dose Trends: A 10-Year Analysis to Develop Institutional Diagnostic Reference Levels

Abstract:PURPOSE:To develop institutional diagnostic reference levels (IDRL) for coronary CT angiography (CCTA) according to patient size by analyzing radiation dose changes over the past 10 years. MATERIALS AND METHODS:This IRB approved retrospective investigation analyzed radiation dose data from CCTA between 2007 and 2016 at our institution. Annual trends in radiation dose were described for each scanner type and scanning mode. Radiation levels were analyzed for normorhythmic patients, patients with prior coronary artery bypass grafting (CABG), arrhythmia, and according to patient size and tube voltage. Median, and quartile values for volume CT dose index (CTDIvol), dose-length product (DLP), and size-specific dose estimate (SSDE) were calculated. Wilcoxon rank-sum test and Kruskal Wallis test were performed to assess the significance of quantitative data. RESULTS:35,375 examinations from 33,317 patients (median age, 58 [50-66] years; male patients, 21,087 [58.7%]) were analyzed. CTDIvol, DLP, and SSDE significantly decreased by 9.0%, 30.8%, and 40.1% (all P < 0.05) for all examinations, respectively. All radiation dose metrics progressively decreased across scanning modes (especially retrospectively ECG-gated spiral and prospectively ECG-triggered high-pitch spiral acquisition mode), but did not significantly change across scanners in the last 6 years. CTDIvol and DLP increased with patient size when water-equivalent diameters were >19 cm for normorhythmic and CABG patients. In arrhythmic patients, CTDIvol increased progressively with water-equivalent diameters across all groups. CONCLUSION:CCTA radiation dose has progressively decreased in the past decade except in patients with prior CABG and arrhythmia. Size-specific IDRLs may optimize radiation utilization in these patients going forward.
Comparative effectiveness of positron emission tomography and single-photon emission computed tomography myocardial perfusion imaging for predicting risk in patients with cardiometabolic disease

Abstract:Background: The epidemiology of coronary artery disease (CAD) has shifted, with increasing prevalence of cardiometabolic disease and decreasing findings of obstructive CAD on myocardial perfusion imaging (MPI). Coronary microvascular dysfunction (CMD), defined as impaired myocardial flow reserve (MFR) by positron emission tomography (PET), has emerged as a key mediator of risk. We aimed to assess whether PET MFR provides additive value for risk stratification of cardiometabolic disease patients compared with single-photon emission computed tomography (SPECT) MPI. Methods: We retrospectively followed patients referred for PET, exercise SPECT, or pharmacologic SPECT MPI with cardiometabolic disease (obesity, diabetes, or chronic kidney disease) and without known CAD. We compared rates and hazards of composite major adverse cardiovascular events (MACEs) (annualized cardiac mortality or acute myocardial infarction) among propensity-matched PET and SPECT patients using Poisson and Cox regression. Normal SPECT was defined as a total perfusion deficit (TPD) of <5%, reflecting the absence of obstructive CAD. Normal PET was defined as a TPD of <5% plus an MFR of ≥2.0. Results: Among 21,544 patients referred from 2006 to 2020, cardiometabolic disease was highly prevalent (PET: 2308 [67%], SPECT: 9984 [55%]) and higher among patients referred to PET (P < 0.001). Obstructive CAD findings (TPD > 5%) were uncommon (PET: 21% and SPECT: 11%). Conversely, impaired MFR on PET (<2.0) was common (62%). In a propensity-matched analysis over a median 6.4-year follow-up, normal PET identified low-risk (0.9%/year MACE) patients, and abnormal PET identified high-risk (4.2%/year MACE) patients with cardiometabolic disease; conversely, those with normal pharmacologic SPECT remained moderate-risk (1.6%/year, P < 0.001 compared to normal PET). Conclusions: Cardiometabolic disease is common among patients referred for MPI and is associated with a heterogenous level of risk. Compared with pharmacologic SPECT, PET with MFR can detect nonobstructive CAD including CMD and can more accurately discriminate low-risk from higher-risk individuals.

Abstract:Objective To explore the differences among volumetric CT dose index (CTDI vol ), body-specific dose assessment (SSDE ED ) based on effective diameter (ED), and SSDE WED based on water equivalent diameter (WED) in evaluating the radiation dose of adult thoracic and abdominal CT scanning. Methods From January 2021 to October 2021, enhanced chest CT scans of 100 patients and enhanced abdomen CT scans of another 100 patients were collected. According to the body mass index (BMI), they can be divided into groups A and D (BMI < 20 kg/m 2 ), groups B and E (20 kg/m 2 ≤ BMI ≤ 24.9 kg/m 2 ), and groups C and F (BMI > 24.9 kg/m 2 ). The CTDIvol, anteroposterior diameter (AP), and the left and rght diameter (LAT) of all the patients were recorded, and the ED, water equivalent diameter (WED), the conversion factor ( f size,ED ), ( f size, WED ), SSDE ED , and SSDE WED were calculated. The differences were compared between the different groups. Results The AP, LAT, ED, and WED of groups B, E, C, and F were higher than those of groups A and D, and those of groups C and F were higher than those of groups B and E ( P < 0.05). The f size,ED and f size, WED of groups B, E, C, and F are lower than those of groups A and D, and those of groups C and F are lower than those of groups B and E ( P < 0.05). CTDI vol , SSDE ED , and SSDE WED in groups B, E, C, and F are higher than those in groups A and D, and those in groups C and F are higher than those in groups B and E ( p < 0.05). In the same group, patients with chest- and abdomen-enhanced have higher SSDE WED and SSDE ED than CTDI vol , patients with chest-enhanced CT scans have higher SSDE WED than SSDE ED , and patients with abdomen-enhanced CT scans have higher SSDE ED than SSDE WED ( P < 0.05). Conclusion CTDIvol and ED-based SSDEED underestimated the radiation dose of the subject exposed, where the patient was actually exposed to a greater dose. However, SSDE WED based on WED considers better the difference in patient size and attenuation characteristics, and can more accurately evaluate the radiation dose received by patients of different sizes during the chest and abdomen CT scan.

Abstract:Abstract Aims Variation in diagnostic performance of single-photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI) has been observed, yet the impact of cardiac size has not been well characterized. We assessed whether low left ventricular volume influences SPECT MPI’s ability to detect obstructive coronary artery disease (CAD) and its interaction with age and sex. Methods and results A total of 2066 patients without known CAD (67% male, 64.7 ± 11.2 years) across nine institutions underwent SPECT MPI with solid-state scanners followed by coronary angiography as part of the REgistry of Fast Myocardial Perfusion Imaging with NExt Generation SPECT. Area under receiver-operating characteristic curve (AUC) analyses evaluated the performance of quantitative and visual assessments according to cardiac size [end-diastolic volume (EDV); <20th vs. ≥20th population or sex-specific percentiles], age (<75 vs. ≥75 years), and sex. Significantly decreased performance was observed in patients with low EDV compared with those without (AUC: population 0.72 vs. 0.78, P = 0.03; sex-specific 0.72 vs. 0.79, P = 0.01) and elderly patients compared with younger patients (AUC 0.72 vs. 0.78, P = 0.03), whereas males and females demonstrated similar AUC (0.77 vs. 0.76, P = 0.67). The reduction in accuracy attributed to lower volumes was primarily observed in males (sex-specific threshold: EDV 0.69 vs. 0.79, P = 0.01). Accordingly, a significant decrease in AUC, sensitivity, specificity, and negative predictive value for quantitative and visual assessments was noted in patients with at least two characteristics of low EDV, elderly age, or male sex. Conclusion Detection of CAD with SPECT MPI is negatively impacted by small cardiac size, most notably in elderly and male patients.

Abstract:Introduction: Deep knowledge of the properties and importance of the Exposure Index (EI) is crucial for delivering high-quality digital radiography images. This study aims to assess the EI on chest posterior anterior (PA) radiographic projection, demonstrating its correlation with parameters, such as body mass index (BMI), source-to-object distance (SOD), age, gender, and patient entrance skin dose (ESD). Methods: The study population included 805 patients who underwent a routine PA chest projection. All data were collected on an Agfa imaging system. Height and weight were also measured, and BMI was calculated. Descriptive statistics was used for data analysis, and inferential statistics was used for correlation analysis between EI, gender, age, ESD and BMI. Results: Over half (56.4%) of the study's participants were males; throughout the multivariate statistical correlation, increasing age was associated with increasing EI only in the female population (p=0.001). Ιn the lower BMI population subgroup, an increase in the SOD was associated with a decrease in the EI (p<0.001). Increasing BMI was associated with decreasing EI, while increasing ESD was linked to increased EI, except for those with lower or higher BMI, where no statistically significant relationship was observed (p=0.403 and p=0.445, respectively). Conclusion: Analysis of the intercorrelation between important parameters such as the EI, BMI, ESD, SOD, age and gender can prove useful for radiographers, offering the ability to deliver high-quality diagnostic chest images, whilst also establishing the EI as an important and highly significant factor.
