Abstract:Advances in computer-aided design (CAD) / computer-aided manufacturing (CAM) technologies and their ease of application enabled the development of novel treatment concepts for modern prosthodontics. This recent paradigm shift in fixed prosthodontics from traditional to minimally invasive treatment approaches is evidenced by the clinical long-term success of bonded CAD/CAM glass-ceramic restorations. Today, defect-oriented restorations, such as inlays, onlays, and posterior crowns, are predominately fabricated from glass-ceramics in monolithic application. The variety of CAD/CAM ceramic restorative systems is constantly evolving to meet the increased demands for highly aesthetic, biocompatible, and long-lasting restorations. Recently introduced polymer-infiltrated ceramic network CAD/CAM blocks add innovative treatment options in CAD/CAM chairside 1-visit restorations. The material-specific high-edge stability enables the CAD/CAM machinability of thin restoration margins. Full-contour zirconia restorations are constantly gaining market share at the expense of bilayered systems. Advancements in material science and bonding protocols foster the development of novel material combinations or fabrication techniques of proven high-strength zirconia ceramics. CAD/CAM applications offer a standardized manufacturing process resulting in a reliable, predictable, and economic workflow for individual and complex teeth-supported restorations. More evidence from long-term clinical studies is needed to verify the clinical performance of monolithic polymer-infiltrated ceramic network and zirconia teeth-supported minimally invasive and extensive restorations.

Abstract:For the last ten years, the application of high-technology processes to dental ceramics allowed for the development of new materials such as heat-pressed, injection-molded, and slip-cast ceramics and glass-ceramics. The purpose of the present paper is to review advances in new materials and processes available for making all-ceramic dental restorations. Concepts on the structure and strengthening mechanisms of dental ceramics are provided. Major developments in materials for all-ceramic restorations are addressed. These advances include improved processing techniques and greater mechanical properties. An overview of the processing techniques available for all-ceramic materials is given, including sintering, casting, machining, slip-casting, and heat-pressing. The most recent ceramic materials are reviewed with respect to their principal crystalline phases, including leucite, alumina, forsterite, zirconia, mica, hydroxyapatite, lithium disilicate, sanidine, and spinel. Finally, a summary of flexural strength data available for all-ceramic materials is included.

Abstract:With a quasar sample designed for studying orientation effects, we investigate the orientation dependence of characteristics of the C IV 1549 broad emission line in approximately 50 Type 1 quasars with z=0.1-1.4. Orientation is measured for the sample via radio core dominance. In our analysis we include measurements of the continuum luminosity and the optical-to-X-ray spectral slope, spectral properties commonly included in the suite known as "Eigenvector 1", and the full-width at half maximum, full-width at one-quarter-maximum, shape, blueshift, and equivalent width of the C IV broad emission line. We also investigate a new prescription that we recently developed for predicting the velocity line width of the H-beta broad emission line based on the velocity line width of the C IV line and the ratio of continuum subtracted peak fluxes of Si IV + O IV] at 1400 A to C IV. In addition to a correlation analysis of the ultraviolet spectral properties and radio core dominance, we provide composite spectra of edge-on and face-on sources for this sample. In particular, we highlight the orientation dependence of the velocity line width predicted for H-beta. We find that this predicted line width depends on orientation in a manner similar to the true velocity line width of H-beta, where no such dependence is observed for C IV. This is an indication that orientation information concerning the line emitting regions can be extracted from ultraviolet spectra. [abridged]
Alternative crown systems. Is the metal-ceramic crown always the restoration of choice?

Abstract:The metal-ceramic crown system still is selected the most frequently because of its strength and versatility. The ability to select metals for color or strength for single units or fixed partial dentures gives great flexibility, but when esthetics of the anterior region are a prime concern, the all-ceramic crown is still an excellent choice. Choice of which all-ceramic system to use is dependent on the strength demands, esthetic needs, amount of tooth structure that can be preserved, and laboratory support available. Where good tooth structure remains but some color, contour, or incisal length changes are desired, the porcelain laminate veneer is an outstanding esthetic and restorative choice. When good labial tooth structure remains but lingual structure is inadequate, a partial veneer gold crown can be an excellent esthetic choice. If moderate tooth structure is lost or moderate staining is present, the Dicor crown is a superb choice. In those instances in which heavy staining is present, a foil or core system should be considered to completely block out the background colors. As the occlusal forces become more of a factor, selection of a restorative system will depend more on strength than esthetic demands. The aluminous porcelain jacket crown still offers great strength and esthetics at a reasonable price. When most of the color is on the surface of the teeth, or when there is a high translucency to the teeth, Dicor can provide very esthetic results, and the Dicor Plus crown offers the opportunity to develop intrinsic shading. When greater strength is required, selection of a foil and core system is suggested, as might be a system that provides a stronger core material, like Alceram or Inceram. These stronger core materials will render improved flexural and compressive strengths, but some increases in brightness may occur with the increased alumina content of the cores. The future in ceramic restorative dentistry may be in the computer-generated crown if ways to develop internal coloring and layered building can be developed and cost can be controlled. Ceramic crowns are best limited to the anterior region of the mouth where occlusal forces normally are less than those found in the posterior region. In selected cases, in which there is no evidence of parafunctional habits, and when the dentist and technician are well versed in functional occlusion and have maximum restorative control of the occlusion, posterior ceramic restorations will be successful. It is important to remember in these cases that porcelain is harder than enamel and very strong in compression.(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract:We studied blood transfusion variables and cytomegalovirus (CMV) infection in 385 infants admitted to the Duke University Medical Center, Durham, NC, neonatal intensive care unit over 14 months. Cytomegalovirus antibody titers were measured at birth and monthly thereafter. Urine cultures for CMV were performed regularly. Infants admitted in the first six months (n = 197) received conventionally prepared blood. Infants admitted in the remaining eight months (n = 188) were given frozen, deglycerolized blood. Of the 105 infants weighing 1250 g or less (low birth weight [LBW]), 90 (86%) received transfusions. Two hundred eighty infants weighed more than 1250 g (non-LBW), and 111 (40%) of these were given blood. In the first six months of the study, three infants had CMV viruria. One case was congenital; two were acquired. Both infants who acquired infection were antibody-positive at birth and received multiple transfusions. In the remaining eight months, five infants had CMV viruria. Two cases were congenital; three were acquired. The three infants who acquired infection were antibody-positive at birth and received multiple transfusions. Our study demonstrates that infants with an LBW are more likely to receive blood transfusion and to be given significantly more blood than non-LBW infants. There was no difference in the number of infants acquiring CMV in the two periods despite the use of different preparations of blood.

Abstract:Objectives To give a comprehensive review of advancement in dental ceramics, fabrication methods, and the challenges associated with clinical application. Data, sources and study selection Researches on chemical composition, biomechanical behaviors, optical properties, bonding strategies and fabrication methods were included. The search of articles was independently conducted by two authors in the PubMed, Scopus, Medline and Web of Science. Conclusions Dental ceramics have shown significant advancements in terms of esthetics and function. However, improving fracture toughness without compromising optical properties remains a challenge. Repairing fractured zirconia or glass-matrix ceramic prostheses with the same material is difficult due to the sintering process. Developing innovative bonding techniques that provide strong and long-lasting bonding strength between ceramics and tooth structures poses a recurring obstacle. Clinical significance Despite the emergence of dental ceramics and fabrication techniques, certain limitations such as susceptibility to brittleness and fracture still exist. Therefore, the current review provided valuable information around the advanced dental ceramics in tooth repair. The laboratory test data and the clinical outcome are also presented in details, aiming to guide clinicians in making informed decisions regarding ceramic restorations.

Abstract:Ceramics can simulate the visual character of the tooth substance successfully and are biocompatible materials. However, a wide range of ceramic materials and systems on the market are available for use in dentistry. Therefore, it is the aim of this article to provide an overview of dental ceramics, their classifications, methods of construction, and clinically relevant aspects that enable the reader to select the most appropriate ceramic for a particular clinical situation.The PubMed (MEDLINE) search engine was used to gather the most recent information on dental ceramics. The search was restricted to a ten-year period (January 1, 2010–December 31, 2019) and only English-language studies. A Boolean search of the PubMed data set was implemented to combine a range of keywords: (ceramics OR all-ceramics OR dental porcelain OR polycrystalline OR porcelain fused to metal OR ceramometal OR procera OR e max OR zirconia OR In-ceram OR Inlays OR Onlays OR Overlays OR Endocrown) AND (survival rate OR success rate OR clinical outcomes OR classification) AND (humans). Studies were also obtained by manual searches and from Google Scholar.By using this process, 2173 articles and studies were obtained. More studies were also obtained by manual searches and from Google Scholar. The most relevant published studies were chosen and used in the current review.All-ceramic restoration use has increased in recent years. This increase has been attributed to patients' demand for good aesthetics and an improvement in the materials' mechanical and aesthetic properties as well as to required minimally invasive tooth preparation and the methods of fabrication. The success of ceramic restorations depends on several factors, such as selection of material, restoration design, occlusion, and cementation media.
In vitro performance and fracture resistance of novel CAD/CAM ceramic molar crowns loaded on implants and human teeth

Abstract:Purpose: To investigate the fatigue and fracture resistance of computer-aided design and computer-aided manufacturing (CAD/CAM) ceramic molar crowns on dental implants and human teeth. Materials and methods: Molar crowns (n=48; n=8/group) were fabricated of a lithium-disilicate-strengthened lithium aluminosilicate glass ceramic (N). Surfaces were polished (P) or glazed (G). Crowns were tested on human teeth (T) and implant-abutment analogues (I) simulating a chairside (C, crown bonded to abutment) or labside (L, screw channel) procedure for implant groups. Polished/glazed lithium disilicate (E) crowns (n=16) served as reference. Combined thermal cycling and mechanical loading (TC: 3000×5℃/3000×55℃; ML: 1.2×106 cycles, 50 N) with antagonistic human molars (groups T) and steatite spheres (groups I) was performed under a chewing simulator. TCML crowns were then analyzed for failures (optical microscopy, SEM) and fracture force was determined. Data were statistically analyzed (Kolmogorow-Smirnov, one-way-ANOVA, post-hoc Bonferroni, α=.05). Results: All crowns survived TCML and showed small traces of wear. In human teeth groups, fracture forces of N crowns varied between 1214±293 N (NPT) and 1324±498 N (NGT), differing significantly (P≤.003) from the polished reference EPT (2044±302 N). Fracture forces in implant groups varied between 934±154 N (NGI_L) and 1782±153 N (NPI_C), providing higher values for the respective chairside crowns. Differences between polishing and glazing were not significant (P≥.066) between crowns of identical materials and abutment support. Conclusion: Fracture resistance was influenced by the ceramic material, and partly by the tooth or implant situation and the clinical procedure (chairside/labside). Type of surface finish (polishing/glazing) had no significant influence. Clinical survival of the new glass ceramic may be comparable to lithium disilicate.

Abstract:Abstract Aim The aim of this randomized, prospective, and clinical multicenter study was to compare the overall clinical performance of two restorative options over a 5‐year period: individualized CAD/CAM abutments veneered with a hand‐layered ceramic, and prefabricated zirconium dioxide abutments veneered with press ceramic and inserted into a single edentulous gap in the anterior maxilla. Materials and Methods Forty subjects were recruited from two universities: 20 from the University of XX and 20 from the University of XY. Each subject received an implant to restore a single edentulous gap in the maxillary anterior region (14–24 FDI). 20 patients were randomized into each Group. Group A received a one‐piece single crown produced from a prefabricated zirconia abutment with pressed ceramic and Group B received an individualized CAD/CAM zirconia abutment with a hand‐layered technique. After 5 years, the aesthetic and radiographic parameters were assessed. Results Group A had four dropouts and one failure, resulting in a 95% survival rate and 95% success rate. Group B had two dropouts and two failures which resulted in a 90% survival rate and 90% success rate. No crestal bone level changes were observed, with a mean DIB of 0.06 mm in Group A and 0.09 mm in Group B. No statistically significant differences were present at baseline, 6 months, 1 year, 3 years, or 5 years for DIB values between time points and groups. Pink aesthetic score/white aesthetic score, Peri‐Implant and Crown Index, and Implant Crown Aesthetic Index values were stable over time at all five points for both groups. Conclusion Both implant‐supported restorative options represent a valuable treatment option for the restoration of implant crowns in the anterior maxilla. Clinical Significance In general, the use of ceramic abutments in the anterior zone represents a valuable treatment procedure with both standardized and CAD/CAM individualized abutments and following the recommendations from the respective manufacturer(s).

Abstract:The endocrown, a modified overlay or crown, is considered to have the advantages of adhesive retention and mechanical retention. Marginal adaptation and mechanical strength are common criteria for evaluating endocrowns. This review studied these aspects of endocrowns to guide further clinical application. Results from previous academic studies indicate that endocrowns made of different materials, such as glass-based ceramics, zirconia-based ceramics and resin-based ceramics, exhibit clinically acceptable marginal adaptation. Zirconia is a common dental material, but when used in endocrowns, it tends to present a risk of irreparable tooth root fractures. Compared with products manufactured with zirconia, endocrowns manufactured with resin-based ceramics and glass-based ceramics, such as lithium disilicate ceramics, do not tend to cause irreparable results, such as tooth root fracture, but their mechanical strengths are not as good as those of zirconia. At the same time, the tooth prepration design such as the types of endocrowns, the pulp chamber extension depth and angles, the endocrown thickness can influence the mechanical strength of endocrowns as well. Compared with traditional zirconia, self-glazed zirconia, a new type of material used to restore defective teeth, has the similar elasticity modulus（210 GPa）and better aesthetic advantages, and is suitable for short crown patients with insufficient occlusal space. Further study is warranted to improve the performance of endocrowns made from ceramic materials to prevent root fracture.

Abstract:We propose a restoration algorithm of images degraded by a motion blur and additive white noise using matrix operators. The present algorithm is preferable to all noisy motion degraded image data processing from viewpoints of reduction of memory sizes and computational time. Finally, simulation results of image restoration are illustrated to show the effectiveness of the proposed method.
A prospective, multi-center, practice-based cohort study on all-ceramic crowns

Abstract:Objectives: The aim of this prospective, multi-center, practice-based cohort study was to analyze factors associated with the success of all-ceramic crowns. Methods: All-ceramic crowns placed in a practice-based research network ([Ceramic Success Analysis, AG Keramik) were analyzed. Data from 1254 patients with (mostly in-office CAD/CAM) all-ceramic crowns placed by 101 dentists being followed up for more than 5 years were evaluated. At the last follow-up visit crowns were considered as successful (not failed) if they were sufficient, whereas crowns were considered as survived (not lost) if they were still in function. Multi-level Cox proportional hazards models were used to evaluate the association between a range of predictors and time of success or survival. Results: Within a mean follow-up period (SD) of 7.2(2)years [maximum:15years] 776 crowns were considered successful (annual failure rate[AFR]:8.4%) and 1041 crowns survived (AFR:4.9%). The presence of a post in endodontically treated teeth resulted in a risk for failure 2.7 times lower than that of restorations without a post (95%CI:1.4-5.0;p = 0.002). Regarding the restorative material and adhesive technique, hybrid composite ceramics and single-step adhesives showed a 3.4 and 2.2 times higher failure rate than feldspathic porcelain and multi-step adhesives, respectively (p < 0.001). Use of an oxygen-blocking gel as well as an EVA instrument resulted in a 1.5-1.8 times higher failure rate than their non-use (p ≤ 0.001). Significance: After up to 15years AFR were rather high for all-ceramic crowns. Operative factors, but no patient- or tooth-level factors were significantly associated with failure. The study was registered in the German Clinical Trials Register (DRKS-ID: DRKS00020271).

Abstract:Ceramic materials have rapidly become the material of choice for indirect restorations. There are a variety of material types available for use such as feldspathic ceramics, glass ceramics and many types of zirconia. Advances in digital dentistry led to a rapid switch from porcelain fused to metal restorations to all-ceramic restorations. Variations in composition, microstructure and processing affect mechanical properties and use of these materials. Having a better understanding of their differences is important for proper clinical selection. Ceramic materials may be classified several ways including by composition, microstructure, processing technique and clinical application. This article reviews the various types of ceramics based on structure and properties that relate to clinical selection. After reading this article, the reader should be able to: explain the types of ceramics in use in dentistry; understand clinical selection based on properties; and discuss the differences in zirconia-based ceramics.

Abstract:Objective: To evaluate the clinical performance of CAD/CAM zirconia all-ceramic crowns and fixed partial dentures(FPDs).Methods: 681 all-ceramic crowns or FPDs were fabricated for 267 patients.562 restorations of 205 patients were clinically followed for 2-6 years.Recalls were made 3 months after cementation.Porcelain fracture performance was compared between restorations in anterior and premolar and molar.Using modified US Public Health Service criteria,judgments were made on marginal integrity and shade and color.Loose or dislodgement,fracture and breakage were reviewed in the annually follow up.Results: Chipping rate is 3.31%,Chipping of restorations in molars were higher than the restorations in premolars and in anterior teeth(χ 2 test,P < 0.01).Different technicians had significant influence(χ 2 test,P < 0.001) on color match of crowns,while different color of abutments did not(χ 2 test,P > 0.05).The authors rated 90.8% as good or better in regard to color match,99.2% of the restorations were ranked as good or better on marginal integrity,Loose or dislodgement0.35%,0% for fracture of Zirconia-based.Conclusion: Zirconia-based all-ceramic crowns with CAD/CAM show good marginal integrity and shade compatibility with nature teeth,with a clinically acceptable chipping rate,This material can be used in conventional and esthetic prosthetic dentistry and especially is the preferred esthetic restorations in case of abutment teeth discoloration.Further studies are needed to reduce the chipping in molars and to improve the marginal integrity of long bridges.

Abstract:Ceramics are highly regarded in dental restorations owing to their favorable mechanical properties, chemical resistance, biocompatibility, and aesthetic features. Ceramic additive manufacturing (AM) technology has emerged as a promising solution that offers advantages over traditional techniques such as injection molding, die pressing, tape casting, and milling. Ceramic AM is, however, still under development, with new technologies and devices continuously emerging. This paper provides a comprehensive review of the latest research and applications of ceramic AM in dental restoration, focusing on the progress made within the past five years. Three perspectives are discussed: ceramic AM technologies, commonly used printable ceramic materials, and different types of dental restorations. Among these, vat photopolymerization is the most widely researched and promising AM technology for large-scale applications. ZrO 2 remains the primary material used in AM research, whereas crowns and bridges are the most frequently studied and are the closest to industrialized dental restorations. Currently, ceramic AM satisfies the clinical requirements of accuracy, mechanical performance, and biocompatibility. However, compared with traditional methods, it lacks significant advantages in terms of cost and manufacturing efficiency, limiting its large-scale application. Further improvements are necessary in all stages, including raw materials, equipment, post-processing, and standardization.

Abstract:Restorative material selection has become increasingly challenging due to the speed of new developments in the field of dental material science. The present narrative review gives an overview of the current indications for implant abutments and restoration materials for provisional and definitive implant‐supported fixed dental prostheses in partially edentulous patients. For single implant restorations, titanium base abutments for crowns are suggested as an alternative to the conventional stock‐ and customized abutments made out of metal or zirconia. They combine the mechanical stability of a metallic connection with the esthetic potential of ceramics. For multiple‐unit restorations, conical titanium bases especially designed for bridges are recommended, to compensate for deviating implant insertion axes and angulations. Even though titanium base abutments with different geometries and heights are available, certain clinical scenarios still benefit from customized titanium abutments. Indications for the definitive material in fixed implant restorations depend on the region of tooth replacement. In the posterior (not esthetically critical) zone, ceramics such as zirconia (3–5‐Ymol%) and lithium‐disilicate are recommended to be used in a monolithic fashion. In the anterior sector, ceramic restorations may be buccally micro‐veneered for an optimal esthetic appearance. Lithium‐disilicate is only recommended for single‐crowns, while zirconia (3–5‐Ymol%) is also recommended for multiple‐unit and cantilever restorations. Attention must be given to the specific mechanical properties of different types of zirconia, as some feature reduced mechanical strengths and are therefore not indicated for all regions and restoration span lengths. Metal‐ceramics remain an option, especially for cantilever restorations.

Abstract:Objective This review aims to evaluate the current understanding and clinical applications of machinable ceramic‐reinforced composites (CRCs) and the emerging first generation of 3D‐printed CRCs in dental restorations. Overview Machinable CRCs, introduced over a decade ago, have shown moderate success in short‐ to medium‐term clinical applications, particularly in low‐stress areas. However, their long‐term durability limitations, such as increased wear and marginal deterioration, restrict their use in high‐stress situations and full crowns. The first generation of 3D‐printed CRCs offers customization advantages but is still in early development and exhibits lower mechanical strength and higher wear rates than CAD‐CAM CRCs and traditional ceramics. Additionally, the classification and definitions surrounding CRCs remain ambiguous, as ADA categorizations do not clearly differentiate CRCs from ceramics, complicating clinical indication, usage, and billing practices. Conclusion Machinable CAD‐CAM CRCs are moderately successful in low‐stress applications, while 3D‐printed CRCs show limitations in wear resistance and durability, raising concerns for their use in definitive restorations. Both require further research and clinical validation. Clinical Significance Machinable CAD‐CAM CRCs are best suited for low‐stress applications, while 3D‐printed CRCs may be more appropriate for provisional use. Until long‐term clinical data are available, ceramics should be preferred for high‐stress or full‐coverage restorations. Clearer definitions for porcelain/ceramic and comprehensive application guidelines are urgently needed to support clinician decision‐making and improve patient outcomes.

Abstract:With the development of ceramic technology, prosthodontic ceramics are becoming a useful option for improving esthetic outcomes in dentistry. In this paper, various ceramic materials were reviewed and evaluated, and their advantages and disadvantages and indications in oral prosthodontics were analyzed objectively. The properties of resin-based ceramics, polycrystalline ceramics, and silicate ceramics were compared and analyzed. Resin-based ceramics may replace other ceramic materials in the CAD/CAM field.
Clinical performance of resin-matrix ceramic partial coverage restorations: a systematic review

Abstract:Abstract Objective To evaluate clinical performance of the new CAD/CAM resin-matrix ceramics and compare it with ceramic partial coverage restorations. Materials and methods An electronic search of 3 databases (The National Library of Medicine (MEDLINE/PubMed), Scopus, and the Cochrane Central Register of Controlled Trials) was conducted. English clinical studies published between 2005 and September 2020 that evaluated the clinical performance of CAD/CAM resin-matrix ceramics inlays, onlays, or overlays were selected. The primary clinical question was applied according to PICOS strategy (Population, Intervention, Comparison, Outcome, Study design). The included studies were individually evaluated for risk of bias according to the modified Cochrane Collaboration tool criteria. Results A total of 7 studies were included according to the established inclusion and exclusion criteria. From the included studies, 6 were randomized clinical trials while one study was longitudinal observational study without control group. According to the results of the included studies, the success rate of CAD/CAM resin-based composite ranged from 85.7 to 100% whereas the success rate reported for ceramic partial coverage restorations ranged from 93.3 to 100%. Fractures and debondings are found to be the most common cause of restorations failure. Conclusion CAD/CAM resin-based composite can be considered a reliable material for partial coverage restorations with clinical performance similar to glass ceramic restorations. However, this result needs to be confirmed in long-term evaluations. Clinical relevance CAD/CAM resin-based composites provide a potential alternative to ceramic indirect restorations. However, clinicians must be aware of the lake of knowledge regarding long-term outcome.
