Abstract:Objectives: Health economic evaluations (HEEs) help healthcare decision makers understand the value of new technologies. Artificial intelligence (AI) is increasingly being used in healthcare interventions. We sought to review the conduct and reporting of published HEEs for AI-based health interventions. Methods: We conducted a systematic literature review with a 15-month search window (April 2021 to June 2022) on 17 th June 2022 to identify HEEs of AI health interventions and update a previous review. Records were identified from 3 databases (Medline, Embase, and Cochrane Central). Two reviewers screened papers against predefined study selection criteria. Data were extracted from included studies using prespecified data extraction tables. Included studies were quality assessed using the National Institute for Health and Care Excellence (NICE) checklist. Results were synthesized narratively. Results: A total of 21 studies were included. The most common type of AI intervention was automated image analysis (9/21, 43%) mainly used for screening or diagnosis in general medicine and oncology. Nearly all were cost-utility (10/21, 48%) or cost-effectiveness analyses (8/21, 38%) that took a healthcare system or payer perspective. Decision-analytic models were used in 16/21 (76%) studies, mostly Markov models and decision trees. Three (3/16, 19%) used a short-term decision tree followed by a longer-term Markov component. Thirteen studies (13/21, 62%) reported the AI intervention to be cost effective or dominant. Limitations tended to result from the input data, authorship conflicts of interest, and a lack of transparent reporting, especially regarding the AI nature of the intervention. Conclusion: Published HEEs of AI-based health interventions are rapidly increasing in number. Despite the potentially innovative nature of AI, most have used traditional methods like Markov models or decision trees. Most attempted to assess the impact on quality of life to present the cost per QALY gained. However, studies have not been comprehensively reported. Specific reporting standards for the economic evaluation of AI interventions would help improve transparency and promote their usefulness for decision making. This is fundamental for reimbursement decisions, which in turn will generate the necessary data to develop flexible models better suited to capturing the potentially dynamic nature of AI interventions.

Abstract:Artificial Intelligence (AI) has become a focus of research in the rapidly evolving field of ophthalmology. Nevertheless, there is a lack of systematic studies on the health economics of AI in this field. This review examines studies from the PubMed, Google Scholar, and Web of Science databases that employed quantitative analysis, retrieved up to July 2023. Most of the studies indicate that AI leads to cost savings and improved efficiency in ophthalmology. On the other hand, some studies suggest that using AI in healthcare may raise costs for patients, especially when taking into account factors such as labor costs, infrastructure, and patient adherence. Future research should cover a wider range of ophthalmic diseases beyond common eye conditions. Moreover, conducting extensive health economic research, designed to collect data relevant to its own context, is imperative for China to facilitate the clinical implementation of AI within the country.

Abstract:Artificial intelligence (AI) is an emerging technology with numerous healthcare applications. AI could prove particularly useful in the cardiac intensive care unit (CICU) where its capacity to analyze large datasets in real-time would assist clinicians in making more informed decisions. This systematic review aimed to explore current research on AI as it pertains to the CICU. A PRISMA search strategy was carried out to identify the pertinent literature on topics including vascular access, heart failure care, circulatory support, cardiogenic shock, ultrasound, and mechanical ventilation. Thirty-eight studies were included. Although AI is still in its early stages of development, this review illustrates its potential to yield numerous benefits in the CICU.

Abstract:Artificial intelligence (AI) is a technique that attempts to replicate human intelligence, analytical behavior, and decision-making ability. This includes machine learning, which involves the use of algorithms and statistical techniques to enhance the computer's ability to make decisions more accurately. Due to AI's ability to analyze, comprehend, and interpret considerable volumes of data, it has been increasingly used in the field of healthcare. In critical care medicine, where most of the patient load requires timely interventions due to the perilous nature of the condition, AI's ability to monitor, analyze, and predict unfavorable outcomes is an invaluable asset. It can significantly improve timely interventions and prevent unfavorable outcomes, which, otherwise, is not always achievable owing to the constrained human ability to multitask with optimum efficiency. AI has been implicated in intensive care units over the past many years. In addition to its advantageous applications, this article discusses its disadvantages, prospects, and the changes needed to train future critical care professionals. A comprehensive search of electronic databases was performed using relevant keywords. Data from articles pertinent to the topic was assimilated into this review article.

Abstract:Background For the intensivists, accurate assessment of the ideal timing for successful weaning from the mechanical ventilation (MV) in the intensive care unit (ICU) is very challenging. Purpose Using artificial intelligence (AI) approach to build two-stage predictive models, namely, the try-weaning stage and weaning MV stage to determine the optimal timing of weaning from MV for ICU intubated patients, and implement into practice for assisting clinical decision making. Methods AI and machine learning (ML) technologies were used to establish the predictive models in the stages. Each stage comprised 11 prediction time points with 11 prediction models. Twenty-five features were used for the first-stage models while 20 features were used for the second-stage models. The optimal models for each time point were selected for further practical implementation in a digital dashboard style. Seven machine learning algorithms including Logistic Regression (LR), Random Forest (RF), Support Vector Machines (SVM), K Nearest Neighbor (KNN), lightGBM, XGBoost, and Multilayer Perception (MLP) were used. The electronic medical records of the intubated ICU patients of Chi Mei Medical Center (CMMC) from 2016 to 2019 were included for modeling. Models with the highest area under the receiver operating characteristic curve (AUC) were regarded as optimal models and used to develop the prediction system accordingly. Results A total of 5,873 cases were included in machine learning modeling for Stage 1 with the AUCs of optimal models ranging from 0.843 to 0.953. Further, 4,172 cases were included for Stage 2 with the AUCs of optimal models ranging from 0.889 to 0.944. A prediction system (dashboard) with the optimal models of the two stages was developed and deployed in the ICU setting. Respiratory care members expressed high recognition of the AI dashboard assisting ventilator weaning decisions. Also, the impact analysis of with- and without-AI assistance revealed that our AI models could shorten the patients’ intubation time by 21 hours, besides gaining the benefit of substantial consistency between these two decision-making strategies. Conclusion We noticed that the two-stage AI prediction models could effectively and precisely predict the optimal timing to wean intubated patients in the ICU from ventilator use. This could reduce patient discomfort, improve medical quality, and lower medical costs. This AI-assisted prediction system is beneficial for clinicians to cope with a high demand for ventilators during the COVID-19 pandemic.

Abstract:The management of mechanical ventilation (MV) remains a challenge in intensive care units (ICUs). The digitalization of healthcare and the implementation of artificial intelligence (AI) and machine learning (ML) has significantly influenced medical decision-making capabilities, potentially enhancing patient outcomes. Acute respiratory distress syndrome, an overwhelming inflammatory lung disease, is common in ICUs. Most patients require MV. Prolonged MV is associated with an increased length of stay, morbidity, and mortality. Shortening the MV duration has both clinical and economic benefits and emphasizes the need for better MV weaning management. AI and ML models can assist the physician in weaning patients from MV by providing predictive tools based on big data. Many ML models have been developed in recent years, dealing with this unmet need. Such models provide an important prediction regarding the success of the individual patient's MV weaning. Some AI models have shown a notable impact on clinical outcomes. However, there are challenges in integrating AI models into clinical practice due to the unfamiliar nature of AI for many physicians and the complexity of some AI models. Our review explores the evolution of weaning methods up to and including AI and ML as weaning aids.

Human-AI Teaming in Critical Care: A Comparative Analysis of Data Scientists' and Clinicians' Perspectives on AI Augmentation and Automation

Abstract:Background: Artificial intelligence (AI) holds immense potential for enhancing clinical and administrative health care tasks. However, slow adoption and implementation challenges highlight the need to consider how humans can effectively collaborate with AI within broader socio-technical systems in health care. Objective: In the example of intensive care units (ICUs), we compare data scientists' and clinicians' assessments of the optimal utilization of human and AI capabilities by determining suitable levels of human-AI teaming for safely and meaningfully augmenting or automating 6 core tasks. The goal is to provide actionable recommendations for policy makers and health care practitioners regarding AI design and implementation. Methods: In this multimethod study, we combine a systematic task analysis across 6 ICUs with an international Delphi survey involving 19 health data scientists from the industry and academia and 61 ICU clinicians (25 physicians and 36 nurses) to define and assess optimal levels of human-AI teaming (level 1=no performance benefits; level 2=AI augments human performance; level 3=humans augment AI performance; level 4=AI performs without human input). Stakeholder groups also considered ethical and social implications. Results: Both stakeholder groups chose level 2 and 3 human-AI teaming for 4 out of 6 core tasks in the ICU. For one task (monitoring), level 4 was the preferred design choice. For the task of patient interactions, both data scientists and clinicians agreed that AI should not be used regardless of technological feasibility due to the importance of the physician-patient and nurse-patient relationship and ethical concerns. Human-AI design choices rely on interpretability, predictability, and control over AI systems. If these conditions are not met and AI performs below human-level reliability, a reduction to level 1 or shifting accountability away from human end users is advised. If AI performs at or beyond human-level reliability and these conditions are not met, shifting to level 4 automation should be considered to ensure safe and efficient human-AI teaming. Conclusions: By considering the sociotechnical system and determining appropriate levels of human-AI teaming, our study showcases the potential for improving the safety and effectiveness of AI usage in ICUs and broader health care settings. Regulatory measures should prioritize interpretability, predictability, and control if clinicians hold full accountability. Ethical and social implications must be carefully evaluated to ensure effective collaboration between humans and AI, particularly considering the most recent advancements in generative AI.

Abstract:Background: Little is known about the role of artificial intelligence (AI) as a decisive technology in the clinical management of COVID-19 patients. We aimed to systematically review and critically appraise the current evidence on AI applications for COVID-19 in intensive care and emergency settings. Methods: We systematically searched PubMed, Embase, Scopus, CINAHL, IEEE Xplore, and ACM Digital Library databases from inception to 1 October 2020, without language restrictions. We included peer-reviewed original studies that applied AI for COVID-19 patients, healthcare workers, or health systems in intensive care, emergency, or prehospital settings. We assessed predictive modelling studies and critically appraised the methodology and key findings of all other studies. Results: Of fourteen eligible studies, eleven developed prognostic or diagnostic AI predictive models, all of which were assessed to be at high risk of bias. Common pitfalls included inadequate sample sizes, poor handling of missing data, failure to account for censored participants, and weak validation of models. Conclusions: Current AI applications for COVID-19 are not ready for deployment in acute care settings, given their limited scope and poor quality. Our findings underscore the need for improvements to facilitate safe and effective clinical adoption of AI applications, for and beyond the COVID-19 pandemic.

Abstract:Neutron rem meters are routinely used for real-time field measurements of neutron dose equivalent where neutron spectra are unknown or poorly characterized. These meters are designed so that their response per unit fluence approximates an appropriate fluence-to-dose conversion function. Typically, a polyethylene moderator assembly surrounds a thermal neutron detector, such as a BF3 counter tube. Internal absorbers may also be used to further fine-tune the detector response to the shape of the desired fluence conversion function. Historical designs suffer from a number of limitations. Accuracy for some designs is poor at intermediate energies (50 keV-250 keV) critical for nuclear power plant dosimetry. The well-known Andersson-Braun design suffers from angular dependence because of its lack of spherical symmetry. Furthermore, all models using a pure polyethylene moderator have no useful high-energy response, which makes them inaccurate around high-energy accelerator facilities. This paper describes two new neutron rem meter designs with improved accuracy over the energy range from thermal to 5 GeV. The Wide Energy Neutron Detection Instrument (WENDI) makes use of both neutron generation and absorption to contour the detector response function. Tungsten or tungsten carbide (WC) powder is added to a polyethylene moderator with the expressed purpose of generating spallation neutrons in tungsten nuclei and thus enhance the high-energy response of the meter beyond 8 MeV. Tungsten's absorption resonance structure below several keV was also found to be useful in contouring the meter's response function. The WENDI rem meters were designed and optimized using the Los Alamos Monte Carlo codes MCNP, MCNPX, and LAHET. A first generation prototype (WENDI-I) was built in 1995 and its testing was completed in 1996. This design placed a BF3 counter in the center of a spherical moderator assembly, whose outer shell consisted of 30% by weight WC in a matrix of polyethylene. A borated silicone rubber (5% boron by weight) absorber covered an inner polyethylene sphere to control the meter's response at intermediate energies. A second generation design (WENDI-II) was finalized and tested in 1999. It further extended the high-energy response beyond 20 MeV, increased sensitivity, and greatly facilitated the manufacturing process. A 3He counter tube is located in the center of a cylindrical polyethylene moderator assembly. Tungsten powder surrounds the counter tube at an inner radius of 4 cm and performs the double duty of neutron generation above 8 MeV and absorption below several keV. WENDI-II is suitable for field use as a portable rem meter in a variety of work place environments, and has been recently commercialized under license by Eberline Instruments, Inc. and Ludlum Measurements, Inc. Sensitivity is about a factor of 12 higher than that of the Hankins Modified Sphere (Eberline NRD meter) in a bare 252Cf field. Additionally, the energy response for WENDI-II closely follows the contour of the Ambient Dose Equivalent per unit fluence function [H'(10)/phi] above 0.1 MeV. Its energy response at 500 MeV is approximately 15 times higher than that of the Hankins and Andersson-Braun meters. Measurements of the energy and directional response of the improved meter are presented and the measured response function is shown to agree closely with the predictions of the Monte Carlo simulations in the range from 0.144 MeV to 19 MeV.

Abstract:There is a wide range of variation in the cephalic index in the full-term fetus. The index rises as birth weight increases. The correlation between birth weight and the cephalic index is significant (r = 0,65) at the 0,05 level. Increasing neurocranial size is associated with differential growth of the dimensions which determine endocranial capacity. A higher cephalic index is achieved by a disproportionately large increase in the biparietal diameter relative to the occipitofrontal dimension. This results in a more globular neurocranial form.

Abstract:Background: As digital technologies and especially artificial intelligence (AI) become increasingly important in health care, it is essential to determine whether and why potential users intend to use related health information systems (HIS). Several theories exist, but they focus mainly on aspects of health care or information systems, in addition to general psychological theories, and hence provide a small number of variables to explain future behavior. Thus, research that provides a larger number of variables by combining several theories from health care, information systems, and psychology is necessary. Objective: This study aims to investigate the intention to use new HIS for decisions concerning short- and long-term medical treatments using an integrated approach with several variables to explain future behavior. Methods: We developed an integrated theoretical model based on theories from health care, information systems, and psychology that allowed us to analyze the duality approach of adaptive and nonadaptive appraisals and their influence on the intention to use HIS. We applied the integrated theoretical model to the short-term treatment using AI-based HIS for surgery and the long-term treatment of diabetes tracking using survey data with structured equation modeling. To differentiate between certain levels of AI involvement, we used several scenarios that include treatments by physicians only, physicians with AI support, and AI only to understand how individuals perceive the influence of AI. Results: Our results showed that for short- and long-term treatments, the variables perceived threats, fear (disease), perceived efficacy, attitude (HIS), and perceived norms are important to consider when determining the intention to use AI-based HIS. Furthermore, the results revealed that perceived efficacy and attitude (HIS) are the most important variables to determine intention to use for all treatments and scenarios. In contrast, abilities (HIS) were important for short-term treatments only. For our 9 scenarios, adaptive and nonadaptive appraisals were both important to determine intention to use, depending on whether the treatment is known. Furthermore, we determined R² values that varied between 57.9% and 81.7% for our scenarios, which showed that the explanation power of our model is medium to good. Conclusions: We contribute to HIS literature by highlighting the importance of integrating disease- and technology-related factors and by providing an integrated theoretical model. As such, we show how adaptive and nonadaptive appraisals should be arranged to report on medical decisions in the future, especially in the short and long terms. Physicians and HIS developers can use our insights to identify promising rationale for HIS adoption concerning short- and long-term treatments and adapt and develop HIS accordingly. Specifically, HIS developers should ensure that future HIS act in terms of HIS functions, as our study shows that efficient HIS lead to a positive attitude toward the HIS and ultimately to a higher intention to use.

Abstract:Motivation: The price of medical treatment continues to rise due to (i) an increasing population; (ii) an aging human growth; (iii) disease prevalence; (iv) a rise in the frequency of patients that utilize health care services; and (v) increase in the price. Objective: Artificial Intelligence (AI) is already well-known for its superiority in various healthcare applications, including the segmentation of lesions in images, speech recognition, smartphone personal assistants, navigation, ride-sharing apps, and many more. Our study is based on two hypotheses: (i) AI offers more economic solutions compared to conventional methods; (ii) AI treatment offers stronger economics compared to AI diagnosis. This novel study aims to evaluate AI technology in the context of healthcare costs, namely in the areas of diagnosis and treatment, and then compare it to the traditional or non-AI-based approaches. Methodology: PRISMA was used to select the best 200 studies for AI in healthcare with a primary focus on cost reduction, especially towards diagnosis and treatment. We defined the diagnosis and treatment architectures, investigated their characteristics, and categorized the roles that AI plays in the diagnostic and therapeutic paradigms. We experimented with various combinations of different assumptions by integrating AI and then comparing it against conventional costs. Lastly, we dwell on three powerful future concepts of AI, namely, pruning, bias, explainability, and regulatory approvals of AI systems. Conclusions: The model shows tremendous cost savings using AI tools in diagnosis and treatment. The economics of AI can be improved by incorporating pruning, reduction in AI bias, explainability, and regulatory approvals.
Use of Artificial Intelligence in Early Warning Score in Critical ill Patients: Scoping Review

Abstract:Early Warning Score (EWS) systems can identify critical patients through the application of artificial intelligence (AI). Physiological parameters like blood pressure, body temperature, heart rate, and respiration rate are encompassed in the EWS. One of AI's advantages is its capacity to recognize high-risk individuals who need emergency medical attention because they are at risk of organ failure, heart attack, or even death. The objective of this study is to review the body of research on the use of AI in EWS to accurately predict patients who will become critical. The analysis model of Arksey and O'Malley is employed in this study. Electronic databases such as ScienceDirect, Scopus, PubMed, and SpringerLink were utilized in a methodical search. Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA SR) guidelines were utilized in the creation and selection of the literature. This analysis included a total of 14 articles. This article summarizes the findings on several aspects: the usefulness of AI algorithms in EWS for critical patients, types of AI algorithm models, and the accuracy of AI in predicting the quality of life of patients in EWS. The results of this review show that the integration of AI into EWS can increase accuracy in predicting patients in critical condition, including cardiac arrest, sepsis, and ARDS events that cause inhalation until the patient dies. The AI models that are often used are machine learning and deep learning models because they are considered to perform better and achieve high accuracy. The importance of further research is to identify the application of AI with EWS in critical care patients by adding laboratory result parameters and pain scales to increase prediction accuracy to obtain optimal results.

Abstract:Artificial intelligence (AI) health technologies are increasingly available for use in real-world care. This emerging opportunity is accompanied by a need for decision makers and practitioners across healthcare systems to evaluate the safety and effectiveness of these interventions against the needs of their own setting. To meet this need, high-quality evidence regarding AI-enabled interventions must be made available, and decision makers in varying roles and settings must be empowered to evaluate that evidence within the context in which they work. This article summarizes good practices across four stages of evidence generation for AI health technologies: study design, study conduct, study reporting, and study appraisal.

Abstract:As exponential expansion of computing capacity converges with unsustainable health care spending, a hopeful opportunity has emerged: the use of artificial intelligence to enhance health care quality and safety. These computer-based algorithms can perform the intricate and extremely complex mathematical operations of classification or regression on immense amounts of data to detect intricate and potentially previously unknown patterns in that data, with the end result of creating predictive models that can be utilized in clinical practice. Such models are designed to distinguish relevant from irrelevant data regarding a particular patient; choose appropriate perioperative care, intervention or surgery; predict cost of care and reimbursement; and predict future outcomes on a variety of anchored measures. If and when one is brought to fruition, an artificial intelligence platform could serve as the first legitimate clinical decision-making tool in spine care, delivering on the value equation while serving as a source for improving physician performance and promoting appropriate, efficient care in this era of financial uncertainty in health care.
