Abstract:BACKGROUND:Short-term fall prediction models that use electronic health records (EHRs) may enable the implementation of dynamic care practices that specifically address changes in individualized fall risk within senior care facilities.OBJECTIVE:The aim of this study is to implement machine learning (ML) algorithms that use EHR data to predict a 3-month fall risk in residents from a variety of senior care facilities providing different levels of care.METHODS:This retrospective study obtained EHR data (2007-2021) from Juniper Communities' proprietary database of 2785 individuals primarily residing in skilled nursing facilities, independent living facilities, and assisted living facilities across the United States. We assessed the performance of 3 ML-based fall prediction models and the Juniper Communities' fall risk assessment. Additional analyses were conducted to examine how changes in the input features, training data sets, and prediction windows affected the performance of these models.RESULTS:The Extreme Gradient Boosting model exhibited the highest performance, with an area under the receiver operating characteristic curve of 0.846 (95% CI 0.794-0.894), specificity of 0.848, diagnostic odds ratio of 13.40, and sensitivity of 0.706, while achieving the best trade-off in balancing true positive and negative rates. The number of active medications was the most significant feature associated with fall risk, followed by a resident's number of active diseases and several variables associated with vital signs, including diastolic blood pressure and changes in weight and respiratory rates. The combination of vital signs with traditional risk factors as input features achieved higher prediction accuracy than using either group of features alone.CONCLUSIONS:This study shows that the Extreme Gradient Boosting technique can use a large number of features from EHR data to make short-term fall predictions with a better performance than that of conventional fall risk assessments and other ML models. The integration of routinely collected EHR data, particularly vital signs, into fall prediction models may generate more accurate fall risk surveillance than models without vital signs. Our data support the use of ML models for dynamic, cost-effective, and automated fall predictions in different types of senior care facilities.

Abstract:Backgrounds Falls are currently one of the important safety issues of elderly inpatients. Falls can lead to their injury, reduced mobility and comorbidity. In hospitals, it may cause medical disputes and staff guilty feelings and anxiety. We aimed to predict fall risks among hospitalized elderly patients using an approach of artificial intelligence. Materials and methods Our working hypothesis was that if hospitalized elderly patients have multiple risk factors, their incidence of falls is higher. Artificial intelligence was then used to predict the incidence of falls of these patients. We enrolled those elderly patients aged >65 years old and were admitted to the geriatric ward during 2018 and 2019, at a single medical center in central Taiwan. We collected 21 physiological and clinical data of these patients from their electronic health records (EHR) with their comprehensive geriatric assessment (CGA). Data included demographic information, vital signs, visual ability, hearing ability, previous medication, and activity of daily living. We separated data from a total of 1,101 patients into 3 datasets: (a) training dataset, (b) testing dataset and (c) validation dataset. To predict incidence of falls, we applied 6 models: (a) Deep neural network (DNN), (b) machine learning algorithm extreme Gradient Boosting (XGBoost), (c) Light Gradient Boosting Machine (LightGBM), (d) Random Forest, (e) Stochastic Gradient Descent (SGD) and (f) logistic regression. Results From modeling data of 1,101 elderly patients, we found that machine learning algorithm XGBoost, LightGBM, Random forest, SGD and logistic regression were successfully trained. Finally, machine learning algorithm XGBoost achieved 73.2% accuracy. Conclusion This is the first machine-learning based study using both EHR and CGA to predict fall risks of elderly. Multiple risk factors of falls in hospitalized elderly patients can be put into a machine learning model to predict future falls for early planned actions. Future studies should be focused on the model fitting and accuracy of data analysis.

Development and External Validation of a Machine Learning-based Fall Prediction Model for Nursing Home Residents: A Prospective Cohort Study

Abstract:Objectives: To develop and externally validate a machine learning-based fall prediction model for ambulatory nursing home residents. The focus is on predicting fall occurrences within 6 months after baseline assessment through a binary classification task, aiming to provide staff with an effective and user-friendly fall-risk assessment tool. Design: Prospective cohort study. Setting and participants: A total of 864 older residents living in 4 nursing homes between May 2022 and March 2023 in China. Methods: Potential fall-risk predictors were collected through in-person interviews and assessments of anthropometric and physical function. Participants were followed for 6 months, with falls recorded by trained nurses. Seven machine learning algorithms, including Logistic Regression (LR), Gradient Boosting Machine (GBM), eXtreme Gradient Boosting (XGBoost), Random Forest (RF), Support Vector Machine (SVM), Neural Networks (NN), and Decision Tree (DT), were used to develop prediction models. Performance was evaluated using the area under the receiver operating characteristic curve (ROC-AUC) and Precision-Recall curve (PR-AUC), with calibration assessed via a calibration curve. Feature importance was visualized using SHapley Additive exPlanations (SHAP). Results: The 6 selected predictors were balance, grip strength, fatigue, fall history, age, and comorbidity. The ROC-AUC for the models ranged from 0.710 to 0.750, PR-AUC from 0.415 to 0.473, sensitivity from 0.704 to 0.914, and specificity from 0.511 to 0.687 in the validation cohort. The LR model was converted into a nomogram. Conclusions and implications: The machine learning-based fall-prediction models effectively identified nursing home residents at high risk of falls. The developed nomogram can be integrated into clinical practice to enhance fall risk assessment protocols, ultimately improving patient safety and care in nursing homes.

Abstract:Introduction: Falls have been acknowledged as a major public health issue around the world. Early detection of fall risk is pivotal for preventive measures. Traditional clinical assessments, although reliable, are resource-intensive and may not always be feasible. Methods: This study explores the efficacy of artificial intelligence (AI) in predicting fall risk, leveraging gait analysis through computer vision and machine learning techniques. Data was collected using the Timed Up and Go (TUG) test and JHFRAT assessment from MMU collaborators and augmented with a public dataset from Mendeley involving older adults. The study introduces a robust approach for extracting and analyzing gait features, such as stride time, step time, cadence, and stance time, to distinguish between fallers and non-fallers. Results: Two experimental setups were investigated: one considering separate gait features for each foot and another analyzing averaged features for both feet. Ultimately, the proposed solutions produce promising outcomes, greatly enhancing the model's ability to achieve high levels of accuracy. In particular, the LightGBM demonstrates a superior accuracy of 96% in the prediction task. Discussion: The findings demonstrate that simple machine learning models can successfully identify individuals at higher fall risk based on gait characteristics, with promising results that could potentially streamline fall risk assessment processes. However, several limitations were discovered throughout the experiment, including an insufficient dataset and data variation, limiting the model's generalizability. These issues are raised for future work consideration. Overall, this research contributes to the growing body of knowledge on fall risk prediction and underscores the potential of AI in enhancing public health strategies through the early identification of at-risk individuals.

Abstract:Background While many falls are preventable, they remain a leading cause of injury and death in older adults. Primary care clinics largely rely on screening questionnaires to identify people at risk of falls. Limitations of standard fall risk screening questionnaires include suboptimal accuracy, missing data, and non‐standard formats, which hinder early identification of risk and prevention of fall injury. We used machine learning methods to develop and evaluate electronic health record (EHR)‐based tools to identify older adults at risk of fall‐related injuries in a primary care population and compared this approach to standard fall screening questionnaires. Methods Using patient‐level clinical data from an integrated healthcare system consisting of 16‐member institutions, we conducted a case–control study to develop and evaluate prediction models for fall‐related injuries in older adults. Questionnaire‐derived prediction with three questions from a commonly used fall risk screening tool was evaluated. We then developed four temporal machine learning models using routinely available longitudinal EHR data to predict the future risk of fall injury. We also developed a fall injury‐prevention clinical decision support (CDS) implementation prototype to link preventative interventions to patient‐specific fall injury risk factors. Results Questionnaire‐based risk screening achieved area under the receiver operating characteristic curve (AUC) up to 0.59 with 23% to 33% similarity for each pair of three fall injury screening questions. EHR‐based machine learning risk screening showed significantly improved performance (best AUROC = 0.76), with similar prediction performance between 6‐month and one‐year prediction models. Conclusions The current method of questionnaire‐based fall risk screening of older adults is suboptimal with redundant items, inadequate precision, and no linkage to prevention. A machine learning fall injury prediction method can accurately predict risk with superior sensitivity while freeing up clinical time for initiating personalized fall prevention interventions. The developed algorithm and data science pipeline can impact routine primary care fall prevention practice.

Integrating Real-Time Location Systems with Electronic Medical Records: A Machine Learning Approach for In-Hospital Fall Risk Prediction

Abstract:Hospital falls are the most prevalent adverse event in healthcare, posing significant risks to patient health outcomes and institutional care quality. The effectiveness of several fall prediction models currently in use is limited by various clinical factors. This study explored the efficacy of merging real-time location system (RTLS) data with clinical information to enhance the accuracy of in-hospital fall predictions. The model performances were compared based on the clinical data, RTLS data, and a hybrid approach using various evaluation metrics. The RTLS and integrated clinical data were obtained from 22,201 patients between March 2020 and June 2022. From the initial cohort, 118 patients with falls and 443 patients without falls were included. Predictive models were developed using the XGBoost algorithm across three distinct frameworks: clinical model, RTLS model, and clinical + RTLS model. The model performance was evaluated using metrics, such as AUROC, AUPRC, accuracy, PPV, sensitivity, specificity, and F1 score. Shapley additive explanation values were used to enhance the model interpretability. The clinical model yielded an AUROC of 0.813 and AUPRC of 0.407. The RTLS model demonstrated superior fall prediction capabilities, with an AUROC of 0.842 and AUPRC of 0.480. The clinical + RTLS model excelled further, achieving an AUROC of 0.853 and AUPRC of 0.497. Feature importance analysis revealed that movement patterns of patients on the last day of their stay were significantly associated with falls, together with elevated RDW levels, sedative administration, age. This study underscored the advantages of combining RTLS data with clinical information to predict in-hospital falls more accurately. This innovative technology-driven approach may enhance early fall risk detection during hospitalization, potentially preventing falls, improving patient safety, and contributing to more efficient healthcare delivery.

Abstract:Bovine retinal pigmented epithelial (RPE) cells express an inducible nitric oxide synthase (NOS-II) after activation with interferon-gamma (IFN-gamma) and lipopolysaccharide (LPS). Experiments were performed to investigate the effects of tyrosine kinase inhibitors (genistein and herbimycin A) and antioxidants [pyrrolidine dithiocarbamate (PDTC) and butyl hydroxyanisol] on NOS-II induction. The LPS-IFN-gamma-induced nitrite release was inhibited in a concentration-dependent manner by these compounds. Analysis by Northern blot showed that this inhibitory effect correlated with a decrease in NOS-II mRNA accumulation. Analysis by electrophoretic mobility shift assay of the activation of the transcription factor nuclear factor-kappaB (NF-kappaB) involved in NOS-II induction demonstrated that LPS alone or combined with IFN-gamma induced NF-kappaB binding. NF-kappaB activation was not changed by the presence of tyrosine kinase inhibitors but was totally prevented by PDTC pretreatment. Immunocytochemistry experiments confirmed the reduction of the nuclear translocation of NF-kappaB only by PDTC. Our results demonstrated the existence in retinal pigmented epithelial cells of different intracellular signaling pathways in NOS-II induction, since tyrosine kinase inhibitors blocked NOS-II mRNA accumulation without inhibiting NF-kappaB activation. Furthermore, the LPS-IFN-gamma-induced NOS-II mRNA accumulation was sensitive to cycloheximide, suggesting that, in addition to NF-kappaB, transcriptional factors that require new protein synthesis are involved in NOS-II induction.

Abstract:Nowadays, it becomes of paramount societal importance to support many frail-prone groups in our society (elderly, patients with neurodegenerative diseases, etc.) to remain socially and physically active, maintain their quality of life, and avoid their loss of autonomy. Once older people enter the prefrail stage, they are already likely to experience falls whose consequences may accelerate the deterioration of their quality of life (injuries, fear of falling, reduction of physical activity). In that context, detecting frailty and high risk of fall at an early stage is the first line of defense against the detrimental consequences of fall. The second line of defense would be to develop original protocols to detect future fallers before any fall occur. This paper briefly summarizes the current advancements and perspectives that may arise from the combination of affordable and easy-to-use non-wearable systems (force platforms, 3D tracking motion systems), wearable systems (accelerometers, gyroscopes, inertial measurement units-IMUs) with appropriate machine learning analytics, as well as the efforts to address these challenges.
A Machine Learning-Based Prediction Model for the Probability of Fall Risk Among Chinese Community-Dwelling Older Adults

Abstract:Fall is a common adverse event among older adults. This study aimed to identify essential fall factors and develop a machine learning-based prediction model to predict the fall risk category among community-dwelling older adults, leading to earlier intervention and better outcomes. Three prediction models (logistic regression, random forest, and naive Bayes) were constructed and evaluated. A total of 459 people were involved, including 156 participants (34.0%) with high fall risk. Seven independent predictors (frail status, age, smoking, heart attack, cerebrovascular disease, arthritis, and osteoporosis) were selected to develop the models. Among the three machine learning models, the logistic regression model had the best model fit, with the highest area under the curve (0.856) and accuracy (0.797) and sensitivity (0.735) in the test set. The logistic regression model had excellent discrimination, calibration, and clinical decision-making ability, which could aid in accurately identifying the high-risk groups and taking early intervention with the model.

Abstract:Falls are a common problem associated with significant morbidity, mortality, and economic costs. Current fall prevention policies in local healthcare settings are often guided by information provided by fall risk assessment tools, incident reporting, and coding data. This review was conducted with the aim of identifying studies which utilized natural language processing (NLP) for the automated detection and prediction of falls in the healthcare setting. The databases Ovid Medline, Ovid Embase, Ovid Emcare, PubMed, CINAHL, IEEE Xplore, and Ei Compendex were searched from 2012 until April 2023. Retrospective derivation, validation, and implementation studies wherein patients experienced falls within a healthcare setting were identified for inclusion. The initial search yielded 2611 publications for title and abstract screening. Full-text screening was conducted on 105 publications, resulting in 26 unique studies that underwent qualitative analyses. Studies applied NLP towards falls risk factor identification, known falls detection, future falls prediction, and falls severity stratification with reasonable success. The NLP pipeline was reviewed in detail between studies and models utilizing rule-based, machine learning (ML), deep learning (DL), and hybrid approaches were examined. With a growing literature surrounding falls prediction in both inpatient and outpatient environments, the absence of studies examining the impact of these models on patient and system outcomes highlights the need for further implementation studies. Through an exploration of the application of NLP techniques, it may be possible to develop models with higher performance in automated falls prediction and detection.
Development and internal validation of a risk prediction model for falls among older people using primary care electronic health records

Abstract:Abstract Background Currently used prediction tools have limited ability to identify community-dwelling older people at high risk for falls. Prediction models utilizing Electronic Heath Records (EHR) provide opportunities but up to now showed limited clinical value as risk stratification tool; because of among others the underestimation of falls prevalence. The aim of this study was to develop a fall prediction model for community-dwelling older people using a combination of structured data and free text of primary care EHR and to internally validate its predictive performance. Methods EHR data of individuals aged 65 or over. Age, sex, history of falls, medications and medical conditions were included as potential predictors. Falls were ascertained from the free text. We employed the Bootstrap-enhanced penalized logistic regression with the least absolute shrinkage and selection operator to develop the prediction model. We used 10-fold cross-validation to internally validate the prediction strategy. Model performance was assessed in terms of discrimination and calibration. Results Data of 36,470 eligible participants were extracted from the dataset. The number of participants who fell at least once was 4,778 (13.1%). The final prediction model included age, sex, history of falls, two medications and five medical conditions. The model had a median area under the receiver operating curve of 0.705 (IQR 0.700-0.714) . Conclusions Our prediction model to identify older people at high risk for falls achieved fair discrimination, and had reasonable calibration. It can be applied in clinical practice as it relies on routinely collected variables and does not require mobility assessment tests.
Predicting in‐hospital mortality among patients admitted with a diagnosis of heart failure: a machine learning approach

Abstract:Existing risk prediction models for hospitalized heart failure patients are limited. We identified patients hospitalized with a diagnosis of heart failure between 7 May 2013 and 26 April 2022 from a large academic, quaternary care medical centre (training cohort). Demographics, medical comorbidities, vitals, and labs were collected and were used to construct random forest machine learning models to predict in‐hospital mortality. Models were compared with logistic regression, and to commonly used heart failure risk scores. The models were subsequently validated in patients hospitalized with a diagnosis of heart failure from a second academic, community medical centre (validation cohort). The entire cohort comprised 21 802 patients, of which 14 539 were in the training cohort and 7263 were in the validation cohort. The median age (25th–75th percentile) was 70 (58–82) for the entire cohort, 43.2% were female, and 6.7% experienced inpatient mortality. In the overall cohort, 7621 (35.0%) patients had heart failure with reduced ejection fraction (EF ≤ 40%), 1271 (5.8%) had heart failure with mildly reduced EF (EF 41–49%), and 12 910 (59.2%) had heart failure with preserved EF (EF ≥ 50%). Random forest models in the validation cohort demonstrated a c‐statistic (95% confidence interval) of 0.96 (0.95–0.97), sensitivity (SN) of 87.3%, and specificity (SP) of 90.6% for the prediction of in‐hospital mortality. Models for those with HFrEF demonstrated a c‐statistic of 0.96 (0.94–0.98), SN 88.2%, and SP 91.0%, and those for patients with HFpEF showed a c‐statistic of 0.95 (0.93–0.97), SN 87.4%, and SP 89.5% for predicting in‐hospital mortality. The random forest model significantly outperformed logistic regression (c‐statistic 0.87, SN 75.9%, and SP 86.9%), and current existing risk scores including the Acute Decompensated Heart Failure National Registry risk score (c‐statistic of 0.70, SN 69%, and SP 62%), and the Get With the Guidelines‐Heart Failure risk score (c‐statistic 0.69, SN 67%, and SP 63%); P

Abstract:This paper reports our preliminary work on medical incident prediction in general, and fall risk prediction in specific, using machine learning. Data for the machine learning are generated only from the particular subset of the electronic medical records (EMR) at Osaka Medical and Pharmaceutical University Hospital. As a result of conducting three experiments such as (1) machine learning algorithm comparison, (2) handling imbalance, and (3) investigation of explanatory variable contribution to the fall incident prediction, we find the investigation of explanatory variables the most effective.

Abstract:ObjectiveTo develop and externally validate the STRAtifying Treatments In the multi-morbid Frail elderlY (STRATIFY)-Falls clinical prediction model to identify the risk of hospital admission or death from a fall in patients with an indication for antihypertensive treatment.DesignRetrospective cohort study.SettingPrimary care data from electronic health records contained within the UK Clinical Practice Research Datalink (CPRD).ParticipantsPatients aged 40 years or older with at least one blood pressure measurement between 130 mm Hg and 179 mm Hg.Main outcome measureFirst serious fall, defined as hospital admission or death with a primary diagnosis of a fall within 10 years of the index date (12 months after cohort entry). Model development was conducted using a Fine-Gray approach in data from CPRD GOLD, accounting for the competing risk of death from other causes, with subsequent recalibration at one, five, and 10 years using pseudo values. External validation was conducted using data from CPRD Aurum, with performance assessed through calibration curves and the observed to expected ratio, C statistic, and D statistic, pooled across general practices, and clinical utility using decision curve analysis at thresholds around 10%.ResultsAnalysis included 1 772 600 patients (experiencing 62 691 serious falls) from CPRD GOLD used in model development, and 3 805 366 (experiencing 206 956 serious falls) from CPRD Aurum in the external validation. The final model consisted of 24 predictors, including age, sex, ethnicity, alcohol consumption, living in an area of high social deprivation, a history of falls, multiple sclerosis, and prescriptions of antihypertensives, antidepressants, hypnotics, and anxiolytics. Upon external validation, the recalibrated model showed good discrimination, with pooled C statistics of 0.833 (95% confidence interval 0.831 to 0.835) and 0.843 (0.841 to 0.844) at five and 10 years, respectively. Original model calibration was poor on visual inspection and although this was improved with recalibration, under-prediction of risk remained (observed to expected ratio at 10 years 1.839, 95% confidence interval 1.811 to 1.865). Nevertheless, decision curve analysis suggests potential clinical utility, with net benefit larger than other strategies.ConclusionsThis prediction model uses commonly recorded clinical characteristics and distinguishes well between patients at high and low risk of falls in the next 1-10 years. Although miscalibration was evident on external validation, the model still had potential clinical utility around risk thresholds of 10% and so could be useful in routine clinical practice to help identify those at high risk of falls who might benefit from closer monitoring or early intervention to prevent future falls. Further studies are needed to explore the appropriate thresholds that maximise the model's clinical utility and cost effectiveness.

Abstract:Early detection of high fall risk is an important process of fall prevention in hospitalized elderly patients. Hospitalized elderly patients can face several falling risks. Monitoring systems can be utilized to protect health and lives, and monitoring models can be less effective if the alarm is not invoked in real time. Therefore, in this paper we propose a monitoring prediction system that incorporates artificial intelligence. The proposed system utilizes a scalable clustering technique, namely the Catboost method, for binary classification. These techniques are executed on the Snowflake platform to rapidly predict safe and risky incidence for hospitalized elderly patients. A later stage employs a deep learning model (DNN) that is based on a convolutional neural network (CNN). Risky incidences are further classified into various monitoring alert types (falls, falls with broken bones, falls that lead to death). At this phase, the model employs adaptive sampling techniques to elucidate the unbalanced overfitting in the datasets. A performance study utilizes the benchmarks datasets, namely SERV-112 and SV-S2017 of the image sequences for assessing accuracy. The simulation depicts that the system has higher true positive counts in case of all health-related risk incidences. The proposed system depicts real-time classification speed with lower training time. The performance of the proposed multi-risk prediction is high at 87.4% in the SERV-112 dataset and 98.71% in the SV-S2017 dataset.

Abstract:Falls among the elderly population cause detrimental physical, mental, financial problems and, in the worst case, death. The increasing number of people entering the higher risk age-range has increased clinicians’ attention to intervene. Clinical tools, e.g., the Timed Up and Go (TUG) test, have been created for aiding clinicians in fall-risk assessment. Often simple to evaluate, these assessments are subject to a clinician’s judgment. Wearable sensor data with machine learning algorithms were introduced as an alternative to precisely quantify ambulatory kinematics and predict prospective falls. However, they require a long-term evaluation of large samples of subjects’ locomotion and complex feature engineering of sensor kinematics. Therefore, it is critical to build an objective fall-risk detection model that can efficiently measure biometric risk factors with minimal costs. We built and studied a sensor data-driven convolutional neural network model to predict older adults’ fall-risk status with relatively high sensitivity to geriatrician’s expert assessment. The sample in this study is representative of older patients with multiple co-morbidity seen in daily medical practice. Three non-intrusive wearable sensors were used to measure participants’ gait kinematics during the TUG test. This data collection ensured convenient capture of various gait impairment aspects at different body locations.

Abstract:The elderly population is growing rapidly in the world and falls are becoming a big problem for society. Currently, clinical assessments of gait and posture include functional evaluations, objective, and subjective scales. They are considered the gold standard to indicate optimal mobility and performance individually, but their sensitivity and specificity are not good enough to predict who is at higher risk of falling. An innovative approach for fall prediction is the machine learning. Machine learning is a computer-science area that uses statistics and optimization methods in a large amount of data to make outcome predictions. Thus, to assess the performance of machine learning algorithms in classify participants by age, number of falls and falls frequency based on features extracted from a public database of stabilometric assessments. 163 participants (116 women and 47 men) between 18 and 85 years old, 44.0 to 75.9 kg mass, 140.0 to 189.8 cm tall, and 17.2 to 31.9 kg/m 2 body mass index. Six different machine learning algorithms were tested for this classification, which included Logistic Regression, Linear Discriminant Analysis, K Nearest-neighbours, Decision Tree Classifier, Gaussian Naive Bayes and C-Support Vector Classification. The machine learning algorithms were applied in this database which has sociocultural, demographic, and health status information about participants. All algorithm models were able to classify the participants into young or old, but our main goal was not achieved, no model identified participants at high risk of falling. Our conclusion corroborates other works in the biomechanics field, arguing the static posturography, probably due to the low daily living activities specificity, does not have the desired effects in predicting the risk of falling. Further studies should focus on dynamic posturography to assess the risk of falls.

Abstract:Protein folding and binding are kindred processes. Many proteins in the cell are unfolded, so folding and function are coupled. This paper investigates how binding kinetics is influenced by the folding of a protein. We find that a relatively unstructured protein molecule can have a greater capture radius for a specific binding site than the folded state with its restricted conformational freedom. In this scenario of binding, the unfolded state binds weakly at a relatively large distance followed by folding as the protein approaches the binding site: the "fly-casting mechanism." We illustrate this scenario with the hypothetical kinetics of binding a single repressor molecule to a DNA site and find that the binding rate can be significantly enhanced over the rate of binding of a fully folded protein.
