Abstract:Sleep is a major health issue for young adults. While sleep hygiene interventions have small-to-moderate effects, better outcomes might be achieved by delivering interventions individually to those reporting sleep difficulties, or in the presence of a supportive other or companion. To examine the feasibility, acceptability, and initial effects of an adapted sleep hygiene intervention for young adults with poor sleep, we conducted an ORBIT Phase IIb pilot randomized controlled trial (n = 51). Young adults with poor sleep were randomized to receive a sleep hygiene intervention individually or with a companion. Sleep patterns were measured at baseline and 4 weeks. Participants completed daily dairies and wore wearable devices for the month following the intervention. Attrition at follow-up was minimal (3.9%), and missing rates were low for diary data (8%), and in line with prior studies for wearable devices (14%). Participants expressed high levels of satisfaction with the study, with no differences between study arms. Significant effects were found postintervention for all participants, indicating improved sleep quality (t = 6.14, P < .001), greater sleep duration (t = 3.8, P < .001), and reduced daytime sleepiness (t = 2.13, P = .039). There were no significant differences in sleep efficiency pre and postintervention, or between study arms for any sleep outcome based on self-reported and objective data. Results from this pilot provide evidence for the feasibility and acceptability of a sleep hygiene intervention for young adults and lay the groundwork for a larger, fully powered study.

Abstract:Abstract Introduction Differences between objectively and subjectively measured sleep can vary widely between participants and these differences may depend on individual characteristics, including mental health. The severity of mental health symptoms may be beneficial in assessing the magnitude of discrepancies between participants’ objective and subjective data. This secondary data analysis examined the predictivity of validated measures of post-traumatic stress symptoms, anxiety symptoms, and depressive symptoms on differences between sleep electroencephalogram (EEG) and actigraphy with sleep diary data. Methods Adults in the community (N=80; Mage=32.65 years, 63% female, 88.8% White) completed the Post-Traumatic Stress Disorder Checklist, State-Trait Anxiety Inventory, Quick Inventory of Depressive Symptomatology, and seven days of sleep assessment via sleep diaries, actigraphy, and EEG. The mean absolute value of differences between the sleep diary, actigraphy, and EEG data was calculated for total sleep time, time in bed (TIB), sleep efficiency, sleep onset latency (SOL), wakefulness after sleep onset, terminal wakefulness, and number of awakenings. Stepwise linear regression was used to examine whether the anxiety, post-traumatic stress, and depressive symptom scores were significant predictors of the objective-subjective differences between these sleep parameters. Results Depressive symptoms significantly predicted differences between EEG and sleep diary data for SOL, F(1,72)=9.958, p=.002, R2=0.121. Anxiety symptoms significantly predicted differences between actigraphy and sleep diary data for SOL, F(1,69)=6.335, p=.014, R2=0.084. Anxiety and post-traumatic stress symptoms significantly predicted differences between actigraphy and sleep diary data for TIB, F(2,70)=10.355, p<.001, R2=.228, βQIDS=0.62, βPCL=-.42. Conclusion Anxiety, post-traumatic stress, and depressive symptoms significantly predicted EEG and actigraphy objective-subjective differences for SOL and TIB. To better assess whether these variations are due to measurement type or if there are individual characteristics responsible for the discrepancies (i.e., sleep state misperception, symptom of mental health), a larger sample with more longitudinal data is needed. Additionally, future studies may focus on clinical samples. For instance, SOL and TIB variations may be indicative of mental health concerns (i.e., hyperarousal, anxiety, depression). Abstract:Abstract Introduction Short sleep and sedentary behavior (SB) are both associated with negative health outcomes including increased risk of metabolic syndrome and obesity. Adolescence is a period characterized by decreased physical activity (PA), increased SB, and chronic short sleep. Literature about the impact of sleep duration on PA in adolescents is limited. The goal of this study was to examine changes in the amount and timing of PA and SB following a one-week sleep extension manipulation. Methods The sleep, PA, and SB of 34 short-sleeping (≤7h on school days) and habitually sedentary (< 3h of physical activity per week) healthy-weight adolescents (age=16.4±1.1 years; 68% female; 73% non-Hispanic White) were objectively measured in a randomized cross-over study during the academic year. Participants completed one week of Typical Sleep (usual school schedule) and one week of Sleep Extension (≥1h time in bed each school night). Sleep was estimated using wrist-worn actigraphy; PA and SB were measured using a thigh-worn accelerometer. Paired t-tests compared total minutes and time blocks of PA and SB during each condition. Results Total SB decreased from 750.4±109.08 to 699.32±91.76 min/day during Sleep Extension compared to Typical Sleep (p= 0.018). Specifically, SB significantly decreased during Sleep Extension in the 6PM-12AM (Typical Sleep=223.1±66.6, Sleep Extension=193.8±46.2 min; p=0.04) and 12AM-6AM (Typical Sleep=22.2±36.9, Sleep Extension=5.8±9.5 min; p=0.02) time frames. There were no significant changes in light, moderate, or vigorous PA between conditions (p>0.05). Conclusion Adolescents significantly decreased time spent in SB by nearly 1h per day during Sleep Extension with no change in PA, similar to prior research. Notably, the decrease in SB occurred during the 6PM-6AM time frame, suggesting that the additional sleep displaced time otherwise spent in SB. Reallocation of time spent in SB to PA is associated with positive health outcomes, but no research has examined reallocation to sleep time. Additional studies are needed to identify the related health effects of increased sleep duration and reduced SB in adolescents. Abstract:BACKGROUND AND PURPOSE: To measure the effects of prolonged sleep extension on daytime alertness, vigilance, and mood in healthy young adults. Little research has documented the effects of increased sleep on daytime function despite a high prevalence of daytime fatigue and sleepiness in the adult population. Past extension studies report conflicting results with regard to Multiple Sleep Latency Test (MSLT) scores, vigilance, and mood ratings. No study has challenged subjects to maximum sleep extension, defined by an MSLT score of 20.PATIENTS AND METHODS: Fifteen healthy college students reporting minimal daytime sleepiness were allowed to sleep as much as possible during a sleep extension period. MSLT scores, psychomotor vigilance task (PVT) reaction times, and profile of mood states (POMS) ratings were measured at baseline, mid-extension, and end-extension.RESULTS: There was a significant increase in both journal and actigraphy sleep totals during all extension segments (P<0.01). MSLT scores increased significantly from baseline to both mid- and end-extension (P<0.01). Five of eight tabulated PVT measures also improved significantly at mid- and end-extension with respect to baseline (P<0.05). POMS vigor and fatigue scores showed a similar improvement (P<0.01). Seven subjects achieved an MSLT score of 20. Six subjects showed substantial improvements while two subjects obtained relatively little extra sleep and showed little or no MSLT improvement. The maximum extension group displayed exceptional improvements in vigilance and POMS ratings.CONCLUSIONS: Extended sleep leads to substantial improvements in daytime alertness, reaction time, and mood.

Effects of a 1-hour per night week-long sleep extension in college students on cardiometabolic parameters, hydration status, and physical activity: A pilot study

Abstract:Objectives: Short sleep duration is associated with poor physical health in college students. Few studies examine the effects of sleep extension on physical health in this population, who are susceptible to sleep loss. We examined health effects of a 1-week, 1-hour nightly sleep extension in college students. Methods: Twelve healthy undergraduate college students (83% female; age 20.2 ± 1.5years) completed a study consisting of sleeping typically for 1week ("Habitual"), then extending sleep by ≥1 hour/night during the second week ("Extension"). Sleep and physical activity actigraphy were collected throughout. Following each week, participants completed cardiometabolic assessments including a meal response and provided a urine sample for markers of hydration. Results: In Extension compared to Habitual, average sleep duration increased (mean change±SEM, +42.6 ± 15.1 minutes; p = .005), while subjective sleepiness (-1.8 ± 0.8 units; p = .040), systolic blood pressure (-6.6 ± 2.8 mmHg; p = .037), postprandial glucose area under the curve (-26.5 ± 10.2 mg/dL × h; p = .025) and time to baseline (-83.0 ± 46.4 minutes; p = .031) after the meal response, sedentary time (-44.3 ± 15.7 minutes; p = .018), and percentage of wake in moderate-to-vigorous activity (-0.89% ± 0.35%; p = .030) decreased. Participants who increased average sleep duration by ≥20 minutes (n = 9) were better hydrated according to urine osmolality (-187.0 ± 68.4 mOsm/kg; p = .026) and specific gravity (-0.01 ± 0.002 g/mL; p = .012) and had reduced odds of dehydration according to urine osmolality (≥800 mOsm/kg; -67%; OR=0.03; p = .035). Conclusions: This pilot study's findings suggest that sleep extension may improve cardiometabolic functioning and hydration, and alter sedentary behavior and physical activity, in college students. Sleep extension may be employed to improve multiple aspects of health in this sleep-deprived population.

Abstract:ABSTRACT: Objective: Short sleep and evening phase preference associate with impaired self-control, yet few studies have assessed the efficacy of sleep extension for improving this behavioral domain. Thus, this secondary analysis of a behavioral sleep intervention measured whether an intervention that enhanced children's sleep also affected self-control. Differences by chronotype were also explored. Methods: Sixty-seven children (8–11 yr), who reportedly slept <9.5 hr/d, were randomized to either a control or sleep intervention condition (i.e., 4-session behavioral intervention to enhance sleep by 1–1.5 hr/night). Chronotype was assessed using the Child Chronotype Questionnaire at baseline, and self-control was assessed using the Self-Control Rating Scale (SCRS, a caregiver report) at baseline and 8 weeks postrandomization. Total sleep time (TST) was measured using wrist actigraphy for 1 week at both baseline and 8 weeks postrandomization. Partial correlations and mixed-model ANOVAs were used for statistical analyses, with age as a covariate. Results: At baseline, children with shorter TST (r = −0.29, p = 0.02) and an evening preference (r = 0.26, p = 0.049) were perceived as having lower self-control by their caregivers. Significant condition*time interaction effects were found for TST ( p < 0.001) and SCRS score ( p = 0.046): From baseline to follow-up, children randomized to the sleep intervention exhibited a significant increase in TST and were perceived as having greater self-control by their caregiver; children randomized to the control condition exhibited no change in TST or in SCRS score. The condition*chronotype*time interaction effect was not significant. Conclusion: A brief sleep intervention that enhanced TST also resulted in enhanced caregiver reported self-control in school-age children. Results add to the growing evidence for the importance of sleep health in children.

Abstract:IntroductionCollege students tend to struggle with managing healthy sleep habits; these unhealthy behaviors can lead to poor sleep and impact their overall mental and physical health. More specifically, sleep is intimately connected to psychological and physiological factors such as anxiety, depression, and blood pressure. The focus of the current study was to examine habitual sleep habits in college students, provide a brief educational intervention, and investigate potential changes in psychological and physiological health.MethodsParticipants included 14 undergraduate students (6 men, average age M=20.64 years, SD=2.13) who wore wrist actigraphs to measure their typical sleep habits. After one week, participants completed questionnaires about sleep (Pittsburgh Sleep Quality Index, PSQI), sleepiness (Epworth Sleepiness Scale, ESS), fatigue (Multidimensional Assessment of Fatigue Scale, MAF), and psychological symptoms (i.e., depression, anxiety, and stress; Depression Anxiety Stress Scales, DASS-21). Blood pressure and heart rate were measured using a wrist device. Subjects participated in a short lecture about healthy sleep hygiene habits and the importance of sleep and then repeated the one-week observational study.ResultsPaired sample t-tests revealed a significant increase from baseline average sleep duration (M=5.83 hours) to post-intervention sleep duration (6.64 hours; t(13)=-2.532, p=.013). Sleep efficiency (actigraphy) and quality (PSQI) did not improve significantly. ESS scores decreased significantly (t(13)=3.76, p=.002 (pre M=9.29; post M=5.43) and MAF scores decreased significantly (t(13)=2.19, p=.047 (pre M=20.48; post M=15.60). No significant differences were found in depressive, anxiety, or stress symptoms when comparing DASS-21 scores pre- vs post-intervention. Baseline systolic blood pressure (M=114.88) significantly decreased compared to post-intervention recordings (M=108.21). Diastolic blood pressure and heart rate did not differ significantly.ConclusionThe results of this study suggest that one educational lecture about sleep hygiene may be a start to improving sleep in college students. Even a 48-minute increase resulted in decreased sleepiness and fatigue. However, no improvements were found in sleep quality or efficiency. Although a slight improvement was found in systolic blood pressure, no other physiological or psychological benefits were noted. More research should be conducted on how to improve sleep habits in college students beyond an educational approach.Support (If Any)None.

Abstract:Abstract Introduction Short sleep duration and inconsistent timing have been linked to poor glycemic target achievement and greater comorbidities in individuals with type 1 diabetes (T1D), particularly adults aged 18-26 years who achieve glycemic targets at the lowest rates (only 14%). Preliminary evidence supports cognitive behavioral sleep interventions (sleep extension + timing consistency) in improving the achievement of sleep and glycemic targets. The purpose of this pilot randomized controlled trial (RCT) was to determine the preliminary effects of a cognitive behavioral sleep health self-management intervention (CB-Sleep Health - sleep extension + timing consistency) on self-report and objectively derived sleep health dimensions (satisfaction, alertness, timing, efficiency, and duration) in young adults with T1D. Methods Young adults with T1D for at least 6 months with a hemoglobin A1C ≥ 7% or ≤ 80% time in glucose range were randomly assigned 1:1 stratified by sex at birth to a 12-week CB-Sleep Health (n = 21) or time balanced attention control (AC) condition (n = 18). Participants completed the Pittsburgh Sleep Quality Index (sleep satisfaction) and trail making test (daytime alertness) while concurrently wearing continuous glucose monitors and research grade actigraphy (timing, efficiency, and duration) for 14 days at baseline, post-intervention, and a 3-month follow up. Results Thirty-nine young adults (mean age 21, BMI 25.9 kg/m2, A1C 8.5% and 44% time in glucose range, 59% male, 26% Non-White - 18% Black/8% other race) participated. Baseline sleep health dimension values (mean + sd) were 5.4 + 0.7 vs. 5.3 + 0.5 score (satisfaction), 18.4 + 1.4 vs. 20.3 + 1.3 seconds (daytime alertness), 16:07 + 0.4 vs. 15:95 + 0.4 hours (timing), 85.6 + 4.1% vs. 84.7 + 4.5% (efficiency), and 6.4 + 1.0, vs. 6.9 + 1.0 hours (duration) for CB-Sleep Health vs. AC respectively. Sleep duration and alertness improved for CB-Sleep Health compared to AC post intervention (+18 minute vs. -25.8 minutes, p = .01 and -3.92 seconds vs. +0.71, p = .005) and were sustained at the 3-month follow up. Conclusion Preliminary effects of a CB-Sleep Health intervention on sleep duration and alertness without impairing efficiency for young adults with T1D are encouraging. Abstract:OBJECTIVE: Sleep problems are common and persistent during adolescence and can have negative effects on adolescents' mood. To date, studies that investigate the effects of sleep extension on adolescents' sleep and depressive symptoms are still lacking. This study aims to investigate the effects of gradual sleep extension combined with sleep hygiene advice in adolescents with chronic sleep reduction on objectively measured sleep, self-reported sleep problems and depressive symptoms.METHODS: Fifty-five adolescents with chronic sleep reduction (mean age: 15.44 years; 85.5% females) were included in the study. Participants were randomly assigned to either a sleep extension group (gradual sleep extension by advancing bedtimes in the evening and receiving sleep hygiene advice) or to a control group (no instruction). Sleep was measured with actigraphy during three weeks, the first week was the baseline week, and the last two weeks were the experimental weeks during which sleep was extended. Other outcome variables were self-reported sleep problems (daytime sleepiness, symptoms of insomnia and circadian rhythm sleep disorder) and depressive symptoms, which were assessed before and after the experimental manipulation.RESULTS: During the third week of the experiment, adolescents in the sleep extension group had earlier bedtimes, earlier sleep onsets, spent more time in bed and slept longer than adolescents in the control group. Their chronic sleep reduction, insomnia symptoms and depressive symptoms diminished significantly. In addition, there was a trend of improved circadian rhythm sleep disorder symptoms and sleep quality.CONCLUSION: Gradual sleep extension combined with sleep hygiene advice seems to have beneficial effects on sleep, self-reported sleep problems and depressive symptoms of adolescents with chronic sleep reduction. Although we cannot distinguish between the effects of sleep extension and sleep hygiene advice, the results suggest that advancing bedtimes can extend sleep and improve depressive symptoms.

Technology Assisted Behavior Intervention to Extend Sleep Among Adults With Short Sleep Duration and Prehypertension/Stage 1 Hypertension: A Randomized Pilot Feasibility Study.

Abstract:STUDY OBJECTIVES Short sleep duration contributes to hypertension, yet few behavioral sleep extension interventions have been developed. The goal of our study was to evaluate the feasibility and preliminary efficacy of a technology assisted sleep extension intervention among individuals with prehypertension/stage 1 hypertension on sleep, blood pressure and patient reported outcomes. METHODS Adults aged 30-65 with 24h ambulatory blood pressure (ABP) > 120/80 mmHg and average weekday sleep duration < 7 h/night were randomized 2:1 to a 6-week technology assisted intervention versus a self-management control group. The intervention included a wearable sleep tracker, smartphone application, weekly didactic lessons and brief telephone coaching. The control group was instructed to maintain their current sleep schedule. Data were analyzed using descriptive statistics and nonparametric statistics to evaluate differences in between groups as well as prepost changes within each group. We also conducted bivariate correlations to evaluate predictors of change in sleep and ABP. RESULTS A total of 16 adults were randomized into the study (11 intervention, 5 control group, 8 women, mean age 45.8 years, standard deviation 9.8 years.) Results at 6-week follow-up demonstrated greater improvement in the intervention group for total sleep time (P = .027), reductions in 24-hour systolic blood pressure (P = .013) and diastolic blood pressure (P = .026), improvements in sleep disturbance (P = .003) and sleep-related impairment (P = .008). Participants in the intervention group completed 90% of the coaching sessions and rated the enjoyment of the intervention as 4 or 5 out of 5. CONCLUSIONS Technology assisted sleep extension intervention is feasible and well liked in this population. Results demonstrate the potential for this intervention to improve sleep duration, quality and 24-hour ABP.

Predictors of interindividual differences in vulnerability to neurobehavioral consequences of chronic partial sleep restriction

Abstract:Interindividual differences in the neurobehavioral response to sleep loss are largely unexplained and phenotypic in nature. Numerous factors have been examined as predictors of differential response to sleep loss, but none have yielded a comprehensive view of the phenomenon. The present study examines the impact of baseline factors, habitual sleep-wake patterns, and homeostatic response to sleep loss on accrued deficits in psychomotor vigilance during chronic partial sleep restriction (SR), in a total of 306 healthy adults that participated in one of three independent laboratory studies. Findings indicate no significant impact of personality, academic intelligence, subjective reports of chronotype, sleepiness and fatigue, performance on working memory, and demographic factors such as sex, ethnicity, and body mass index, on neurobehavioral vulnerability to the negative effects of sleep loss. Only superior baseline performance on the psychomotor vigilance test and ability to sustain wakefulness on the maintenance of wakefulness test were associated with relative resilience to decrements in vigilant attention during SR. Interindividual differences in vulnerability to the effects of sleep loss were not accounted for by prior sleep history, habitual sleep patterns outside of the laboratory, baseline sleep architecture, or homeostatic sleep response during chronic partial SR. A recent theoretical model proposed that sleep-wake modulation may be influenced by competing internal and external demands which may promote wakefulness despite homeostatic and circadian signals for sleep under the right circumstances. Further research is warranted to examine the possibility of interindividual differences in the ability to prioritize external demands for wakefulness in the face of mounting pressure to sleep.
Determinants of sleep behavior in adolescents: A pilot study

Abstract:Purpose: To identify determinants of sleep behavior in adolescents of middle to high socioeconomic status. Methods: Eighteen students (9 boys, 9 girls) with a mean age of 14.9±1.5years were interviewed using a standardized script inspired by the theory of planned behavior. Interviews were audio-recorded and anonymously transcribed verbatim. In addition, participants completed a survey assessing demographic and health characteristics and their sleep duration was measured using actigraphy. Results: Adolescents listed a few positive benefits of healthy sleep and described a large number of immediate negative consequences caused by sleep deprivation. Strong positive/negative emotions were barriers to healthy sleep. The use of electronic devices at bedtime and sports participation were described as both barriers to and facilitators of healthy sleep. Participants indicated their intention to sleep more and to use relaxing activities at bedtime. Some intended to advance their bedtime, but none intended to stop using electronic devices at bedtime. Most participants stated that it would be easy to turn off phones but difficult to turn off movies at bedtime, and many believed that their parents and peers were against engaging in risky sleep behavior. Conclusions: Interventions that seek to alter adolescents' ability to regulate affect and address their beliefs regarding the use of electronic devices at bedtime, combined with information regarding the long-term impact of sleep health, may improve the effectiveness of sleep promotion programs for adolescents.

Abstract:Abstract Introduction Biological changes during adolescence naturally delay the circadian sleep-wake cycle. Adolescents struggle between the imposed timing of their sleep-wake cycle by social demands and their desired one, resulting in circadian misalignment. Circadian misalignment has been associated with a higher risk for mental health problems. We examined whether objective and subjective metrics of circadian misalignment are associated with internalizing symptoms (IS) and externalizing behaviors (EB) in adolescents. Methods We studied 376 adolescents from the Penn State Child Cohort (median 16 years; 47% female; 21% racial/ethnic minority) who had a minimum of 3-nights of at-home actigraphy (ACT), and 9-h in-lab polysomnography (PSG). ACT-measured sleep midpoint (SM) was calculated as the central point of the sleep period, while sleep regularity (SR) was calculated as the intra-individual standard deviation of the SM. Circadian preference (CP) was measured with the Morningness-Eveningness Questionnaire. IS and EB were measured by parent- or self-report on Achenbach System of Empirically Based Assessment using developmentally-appropriate forms and norms. Stepwise linear regression models adjusted for demographics (sex, race/ethnicity, age, BMI percentile), sleep disorders (insomnia, PSG-apnea/hypopnea index) and insufficient sleep (ACT-mean sleep duration, ACT-sleep duration variability). Results Both CP (r=-.118, p=.022) and SR (r=.116, p=.024) were associated with IS, however, they competed between each other in regression models resulting in marginally significant associations (β=-.094, p=.079 and β=.092, p=.084, respectively) that were further attenuated after adjusting individually for covariables (β=-.072, p=.190 and β=.076, p=.163, respectively). Both CP (r=-.230, p<.001) and SR (r=.140, p=.006) were associated with EB; however, only CP (β=-.153, p=.004) remained significantly associated with EB after adjusting for covariables (β=-.130, p=.016). Conclusion Assessing self-reported eveningness is a useful and inexpensive tool to identify adolescents in whom circadian misalignment contributes to mood and behavioral problems. However, it appears that its combination with objective assessments, particularly irregularity of the sleep-wake cycle, may yield the most predictive results. Abstract:Recent studies have suggested that Helicobacter pylori (H. pylori)-associated gastritis may play an important role in the pathogenesis of primary gastric lymphoma. Recently, triple therapy using proton pump inhibitor, amoxicillin, and clarithromycin, has been established for the eradication therapy of H. pylori infection, and is also recommended for the treatment of the superficial type of low-grade gastric MALT (mucosa-associated lymphoid tissue ) lymphoma. MALT lymphoma of the gastric stump is rare, and total resection or chemotherapy for MALT lymphoma of the gastric stump has been previously reported. Therefore, there is no evidence that eradication therapy is effective for low-grade MALT lymphoma of the gastric stump. Our case illustrates the remarkable efficacy of eradication of H. pylori for low-grade MALT lymphoma of the gastric stump without other modalities such as surgery and systemic chemotherapy.
Actigraphic sleep dimensions and associations with academic functioning among adolescents

Abstract:Abstract Study Objectives There is mixed evidence regarding associations of sleep duration with academic functioning in adolescents and a lack of research on other sleep dimensions, particularly using objective sleep measures. We examined associations of multiple actigraphic sleep dimensions with academic functioning among adolescents. Methods Data were from the sleep sub-study of the age 15 wave of the Future of Families and Child Wellbeing Study (n = 774–782; 52% female), a national, diverse sample of teens. Adolescents wore wrist-actigraphs for ~1 week and completed a survey reporting academic performance and school-related behavioral problems. Regression models assessed whether average sleep duration, timing, maintenance efficiency, and SD-variability were associated with self-reported academic functioning in cross-sectional analyses adjusted for demographic characteristics, depressive symptoms, and anxious symptoms. Results Later sleep timing (hours) and greater sleep variability (SD-hours) were associated with poorer academic outcomes, including sleep onset variability with higher odds of receiving a D or lower (OR = 1.29), sleep onset (β = −.07), sleep offset (β = −.08), and sleep duration variability (β = −.08) with fewer A grades, sleep offset with lower GPA (β = −.07), sleep offset (OR = 1.11), sleep duration variability (OR = 1.31), and sleep onset variability (OR = 1.42) with higher odds of being suspended or expelled in the past 2 years, and sleep duration variability with greater trouble at school (β = .13). Sleep duration, sleep maintenance efficiency, and sleep regularity index were not associated with academic functioning. Conclusions Later sleep timing and greater sleep variability are risk factors for certain academic problems among adolescents. Promoting sufficient, regular sleep timing across the week may improve adolescent academic functioning.

Abstract:Background: A third of adults in Western countries have impaired sleep quality. A possible solution involves distributing sleep aids through smartphone apps, but most empirical studies are limited to small pilot trials in distinct populations (eg, soldiers) or individuals with clinical sleep disorders; therefore, general population data are required. Furthermore, recent research shows that sleep app users desire a personalized approach, offering an individually tailored choice of techniques. One such aid is Peak Sleep, a smartphone app based on scientifically validated principles for improving sleep quality, such as mindfulness meditation and cognitive behavioral therapy. Objective: We aimed to test the impact of the smartphone app Peak Sleep on sleep quality and collect user experience data to allow for future app development. Methods: This was a 2-arm pilot randomized controlled trial. Participants were general population adults in the United Kingdom (aged ≥18 years) who were interested in improving their sleep quality and were not undergoing clinical treatment for sleep disorder or using sleep medication ≥1 per week. Participants were individually randomized to receive the intervention (3 months of app use) versus a no-treatment control. The intervention involved free access to Peak Sleep, an app that offered a choice of behavioral techniques to support better sleep (mindfulness, cognitive behavioral therapy, and acceptance commitment therapy). The primary outcome was sleep quality assessed using the Insomnia Severity Index at baseline and 1-, 2-, and 3-month follow-ups. Assessments were remote using web-based questionnaires. Objective sleep data collection using the Oura Ring (Ōura Health Oy) was planned; however, because the COVID-19 pandemic lockdowns began just after recruitment started, this plan could not be realized. Participant engagement with the app was assessed using the Digital Behavior Change Intervention Engagement Scale and qualitative telephone interviews with a subsample. Results: A total of 101 participants were enrolled in the trial, and 21 (21%) were qualitatively interviewed. Sleep quality improved in both groups over time, with Insomnia Severity Index scores of the intervention group improving by a mean of 2.5 and the control group by a mean of 1.6 (between-group mean difference 0.9, 95% CI -2.0 to 3.8), with was no significant effect of group (P=.91). App users' engagement was mixed, with qualitative interviews supporting the view of a polarized sample who either strongly liked or disliked the app. Conclusions: In this trial, self-reported sleep improved over time in both intervention and control arms, with no impact by group, suggesting no effect of the sleep app. Qualitative data suggested polarized views on liking or not liking the app, features that people engaged with, and areas for improvement. Future work could involve developing the app features and then testing the app using objective measures of sleep in a larger sample. Abstract:People who are grateful, resilient and flourishing in life show better health, including better sleep. This correlational finding is typically attributed to personality factors or to positive outlooks causing better sleep. We investigated the reverse causal interpretation: do sleep losses and sleep gains experimentally affect feelings and expressions of gratitude, resilience and flourishing? Young adults ( N = 90) were randomly assigned to sleep restriction, sleep extension or to sleep normally while wearing wristband actigraphy between Monday and Friday study sessions. State-level measures of flourishing, resilience and gratitude improved across the week with sleep extension and worsened with sleep restriction (η p 2 = .13 to .17). Trait-level measures showed modest or no changes. Sleep-extended participants wrote twice as many details on their gratitude list as the other two conditions (η p 2 = .11). Most effects persisted when accounting for mood. Therefore, subtle changes in nighttime sleep causally influence the components that underlie mental well-being.

Abstract:Abstract Introduction Stress in interpersonal relationships has been related to adverse mental health outcomes, however, fewer studies have examined associations with physical health, particularly sleep. To date, most studies rely on retrospective reports of stress and sleep measured at a single timepoint, and do not examine multiple types of relationship stress. We tested whether daily stress with family, partner/spouse, and/or friends predicted shorter total sleep time (TST), lower sleep efficiency (SE), and/or later sleep midpoint via ecological momentary assessment (EMA) among young adults. Methods Participants were 293 young adults (21-30 y/o; 64% female; 5% Asian, 25% Black, 5% Biracial or Multiracial, 1% Other race, 64% White) reporting regular alcohol use from two studies with a shared assessment battery and up to 18-day EMA protocol. Participants reported daily sleep (TST [hours], SE, and midpoint, calculated from sleep diary reports), cannabis use (yes/no), alcohol use (total number of drinks), and stress with family, spouse/partner, and friends (coded as: 0=not at all, 1=somewhat, 2=moderately/very much). We fit generalized linear mixed effects models and estimated within-subject and between-subject associations between each stress variable and sleep outcome, respectively, adjusting for time-invariant (age, past-year household income, assigned sex) and time-variant covariates (weekend, cannabis use, alcohol use, pre- vs. post-COVID-19 pandemic). Results Participants completed a median of 17 EMA daily records (range: 1-18). Considering between-person effects, participants who reported feeling somewhat stressed with family [B(SE)=-.66(.32), p=0.04] or partner/spouse [B(SE)=-.63(.35), p=0.07] reported lower average TST over the EMA protocol. No associations emerged with experiencing moderately/very much stress in any relationship, nor for SE or midpoint outcomes. Results did not suggest a within-person effect of any type of daily relationship stress on sleep that night. Conclusion Among a well-characterized sample with up to 18 days of EMA data, reporting some stress in family and partner/spouse relationships predicted lower TST, relative to peers without stress in these relationships. Results may suggest that cumulative stress over time may be more detrimental for sleep than acute day-to-day stressors. When intervening on sleep, clinicians should also consider relationship stress, which is amenable to evidence-based interventions such as cognitive behavioral therapy. Abstract:Many American adolescents and adults report that they are not meeting sleep duration recommendations. Although insufficient sleep duration can occur due to factors outside an individual's direct control, many individuals choose to restrict their own sleep. The theory of planned behaviour (TPB) offers a framework to study this phenomenon. Recent research efforts have used the TPB to study sleep and have demonstrated success predicting sleep-related intentions and behaviour but have failed to consider volitional sleep behaviour or consider between- and within-person differences. The present study used an intensive longitudinal design to test how constructs of the TPB relate to nightly sleep opportunity. Healthy college students (N = 79) participated in a weeklong study in which they completed four ecological momentary assessment signals per day that measured their attitudes, subjective norms, perceived behavioural control, and intentions relating to their nocturnal sleep opportunity. Participants wore an actiwatch each night of the study to measure their sleep opportunity. Mixed linear models found that both intentions and perceived behavioural control were significant predictors of subsequent sleep opportunity, and that perceived behaviour control was the strongest within-day predictor of intentions. Results demonstrate that within-person changes in perceived behavioural control and intentions predict subsequent sleep opportunity and provide insight into the potential refinement of sleep promotion efforts.

Abstract:The protective nature of adequate sleep during adolescence is poorly understood, particularly regarding social relationships. We explored emotion and behaviour during two social interaction tasks among teens assigned to sleep extension (SE) or typical sleep (TS) conditions. Twenty healthy adolescents completed a baseline assessment (7 nights typical sleep) followed by assignment to SE or TS (5 nights). Participants returned to the lab to complete a frustrating computer game and two social interactions with a confederate peer (one naturalistic, one manipulated) during which adolescents were asked to up-regulate positive emotion. Following a frustrating task, the SE group reported increased positive emotions during the goal-directed interaction relative to both the naturalistic interaction and the TS group. Findings suggest another 30 minutes of sleep for five nights assisted adolescents in up-regulating positive emotional to meet socially relevant goals. Findings from this pilot study support the protective role of adequate sleep for social functioning and emotional health during adolescence.
