Abstract:In 2010, under the guidance of the DGAI (German Society of Anaesthesiology and Intensive Care Medicine) and DIVI (German Interdisciplinary Association for Intensive Care and Emergency Medicine), twelve German medical societies published the “Evidence- and Consensus-based Guidelines on the Management of Analgesia, Sedation and Delirium in Intensive Care”. Since then, several new studies and publications have considerably increased the body of evidence, including the new recommendations from the American College of Critical Care Medicine (ACCM) in conjunction with Society of Critical Care Medicine (SCCM) and American Society of Health-System Pharmacists (ASHP) from 2013. For this update, a major restructuring and extension of the guidelines were needed in order to cover new aspects of treatment, such as sleep and anxiety management. The literature was systematically searched and evaluated using the criteria of the Oxford Center of Evidence Based Medicine. The body of evidence used to formulate these recommendations was reviewed and approved by representatives of 17 national societies. Three grades of recommendation were used as follows: Grade “A” (strong recommendation), Grade “B” (recommendation) and Grade “0” (open recommendation). The result is a comprehensive, interdisciplinary, evidence and consensus-based set of level 3 guidelines. This publication was designed for all ICU professionals, and takes into account all critically ill patient populations. It represents a guide to symptom-oriented prevention, diagnosis, and treatment of delirium, anxiety, stress, and protocol-based analgesia, sedation, and sleep-management in intensive care medicine.

Abstract:Innate immune memory is an emerging area of research. However, innate immune memory at major mucosal sites remains poorly understood. Here, we show that respiratory viral infection induces long-lasting memory alveolar macrophages (AMs). Memory AMs are programed to express high MHC II, a defense-ready gene signature, and increased glycolytic metabolism, and produce, upon re-stimulation, neutrophil chemokines. Using a multitude of approaches, we reveal that the priming, but not maintenance, of memory AMs requires the help from effector CD8 T cells. T cells jump-start this process via IFN-γ production. We further find that formation and maintenance of memory AMs are independent of monocytes or bone marrow progenitors. Finally, we demonstrate that memory AMs are poised for robust trained immunity against bacterial infection in the lung via rapid induction of chemokines and neutrophilia. Our study thus establishes a new paradigm of immunological memory formation whereby adaptive T-lymphocytes render innate memory of mucosal-associated macrophages.

Abstract:CD4 tissue-resident memory T (CD4 TRM) cells are crucial adaptive immune components involved in preventing influenza A virus (IAV) infection. Despite their importance, their physiological role in the upper respiratory tract, the first site of contact with IAV, remains unclear. Here, we find that, after IAV infection, antigen-specific CD4 TRM persist in the nasal tissue (NT) compartment and play a prime role in local viral clearance. Single cell RNA sequencing analysis reveals that NT CD4 TRM are heterogeneous and transcriptionally distinct as compared to their lung counterparts. Mechanistically, we demonstrate that the CXCR6-CXCL16 axis is involved in CD4 TRM residency in the NT. Furthermore, we show that the NT of mice and humans contains a high frequency of IL-17-producing CD4 TRM that aid in local viral clearance and in reducing tissue damage. Collectively, our results support a robust physiological role for nasal tissue CD4 TRM in preventing IAV infection.

Abstract:Pathogen encounter can result in epigenetic remodeling that shapes disease caused by heterologous pathogens. Here, we examined innate immune memory in the context of commonly circulating respiratory viruses. Single-cell analyses of airway-resident immune cells in a disease-relevant murine model of SARS-CoV-2 recovery revealed epigenetic reprogramming in alveolar macrophages following infection. Post-COVID-19 human monocytes exhibited similar epigenetic signatures. In airway-resident macrophages, past SARS-CoV-2 infection increased activity of type I interferon (IFN-I)-related transcription factors and epigenetic poising of antiviral genes. Viral pattern recognition and canonical IFN-I signaling were required for the establishment of this innate immune memory and augmented secondary antiviral responses. Antiviral innate immune memory mounted by airway-resident macrophages post-SARS-CoV-2 was necessary and sufficient to ameliorate secondary disease caused by influenza A virus and curtailed hyperinflammatory dysregulation and mortality. Our findings provide insights into antiviral innate immune memory in the airway that may facilitate the development of broadly effective therapeutic strategies.

Abstract:The nasal mucosa constitutes the primary entry site for respiratory viruses, including severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). While the imbalanced innate immune response of end-stage coronavirus disease 2019 (COVID-19) has been extensively studied, the earliest stages of SARS-CoV-2 infection at the mucosal entry site have remained unexplored. Here, we employed SARS-CoV-2 and influenza virus infection in native multi-cell-type human nasal turbinate and lung tissues ex vivo, coupled with genome-wide transcriptional analysis, to investigate viral susceptibility and early patterns of local mucosal innate immune response in the authentic milieu of the human respiratory tract. SARS-CoV-2 productively infected the nasal turbinate tissues, predominantly targeting respiratory epithelial cells, with a rapid increase in tissue-associated viral subgenomic mRNA and secretion of infectious viral progeny. Importantly, SARS-CoV-2 infection triggered robust antiviral and inflammatory innate immune responses in the nasal mucosa. The upregulation of interferon-stimulated genes, cytokines, and chemokines, related to interferon signaling and immune-cell activation pathways, was broader than that triggered by influenza virus infection. Conversely, lung tissues exhibited a restricted innate immune response to SARS-CoV-2, with a conspicuous lack of type I and III interferon upregulation, contrasting with their vigorous innate immune response to influenza virus. Our findings reveal differential tissue-specific innate immune responses in the upper and lower respiratory tracts that are specific to SARS-CoV-2. The studies shed light on the role of the nasal mucosa in active viral transmission and immune defense, implying a window of opportunity for early interventions, whereas the restricted innate immune response in early-SARS-CoV-2-infected lung tissues could underlie the unique uncontrolled late-phase lung damage of advanced COVID-19. IMPORTANCE In order to reduce the late-phase morbidity and mortality of COVID-19, there is a need to better understand and target the earliest stages of SARS-CoV-2 infection in the human respiratory tract. Here, we have studied the initial steps of SARS-CoV-2 infection and the consequent innate immune responses within the natural multicellular complexity of human nasal mucosal and lung tissues. Comparing the global innate response patterns of nasal and lung tissues infected in parallel with SARS-CoV-2 and influenza virus, we found distinct virus-host interactions in the upper and lower respiratory tract, which could determine the outcome and unique pathogenesis of SARS-CoV-2 infection. Studies in the nasal mucosal infection model can be employed to assess the impact of viral evolutionary changes and evaluate new therapeutic and preventive measures against SARS-CoV-2 and other human respiratory pathogens.

Abstract:Innate immune memory provides antigen-independent cross-protection against heterologous pathogens. In Immunity , Lercher et al. determine the effects of innate immune memory in airway-resident immune cells on disease outcome of a second heterologous viral infection. The authors use a mouse-adapted SARS-CoV-2 strain for a murine infection model of SARS-CoV-2 recovery. They investigate the epigenetic and transcriptomics changes in airway-resident cells by single-nuclei combined assay of transposase-accessible chromatin with sequencing (snATAC-seq)/RNA sequencing (snRNA-seq) and find that alveolar macrophages present epigenetic imprinting after SARS-CoV-2 infection. Human monocytes from individuals after SARS-CoV-2 infection also retained epigenetic programs. Using an ex vivo culture of mouse primary airway-resident macrophages stimulated with different pattern recognition receptor agonists the authors show that the epigenetic imprinting associated with previous SARS-CoV-2 infection results in antiviral innate immune memory. The authors challenged mice that had recovered from SARS-CoV-2 with influenza A virus and observed milder pathology. Similar results were obtained after transferring airway-resident macrophages isolated from recovered mice into T cell-depleted naive recipient mice. Abstract:Viral respiratory infections are a significant global health burden. Human immune responses to infections caused by respiratory viruses are complex and dependent on the length of time the host has been infected. Human viral challenge studies provide a useful opportunity to understand the timing of human immune responses as samples can be obtained pre-inoculation and post-inoculation by defined virus inocula. Transcriptomic data is available from a large amount of publicly available viral challenge studies and genome-wide transcriptomic data is primarily derived from bulk samples containing mixed cell subpopulations. Traditional bulk transcriptomic analysis methods often utilize deconvolution techniques to study changes in cell proportions of major immune cell types. In this study, we digitally sorted the cellular diversity of 31 blood immune cell types over time in human challenge studies after inoculation by H3N2 influenza virus H1N1 influenza virus, and RSV using xCell, a digital cell separation method, on human bulk transcriptional repository data. We identified common inoculation-induced changes in cell subpopulations across studies of the three viruses with time kinetics. We observed a consistent elevation of myeloid cells and their subtypes post-inoculation across various viral inoculations. Lymphocytes and their subpopulations were reduced in symptomatic and infected (Sx/Inf) participants compared to asymptomatic and uninfected (Asx/Uninf) participants. H3N2 evoked the earliest transcriptional immune responses, followed by H1N1, RSV infection elicited a delayed transcriptional response compared to both influenza viruses likely due to slower replication kinetics. Our analysis also revealed unique immune cell composition changes caused by specific viruses. We found that pre-existing baseline memory T cells were associated with resistance to influenza viral infection and that the key cell type is effector memory (Tem) CD8 T cells. These findings identify specific target cell subpopulations which respond to respiratory virus inoculation over time, shedding light on mechanisms for eliciting a robust baseline level of immune memory against multiple viral strains. These studies highlight potential immune cell responses that can be utilized to enhance vaccine design.

Abstract:Background: Resident phenotypic memory CD8 + T cells are crucial for immune defense against pathogens. However, little is known about the potential transitions and regulation mechanisms of their function after influenza virus infection and reinfection. In this study, we utilized integrated transcriptome data and in vivo experiments to investigate the key characteristics behind it. Methods: Two single-cell RNA sequencing (scRNA-seq) datasets of lung CD8 + T cells and one RNA-seq dataset of lung tissue after infection or reinfection were included. After Seurat procedures classifying CD8 + T subsets, the scCODE algorithm was used to identify the differentially expressed genes for GSVA, GO, and KEGG pathway enrichment. Monocle 3 and CellChat were used to infer pseudotime cell trajectory and cell interactions. The ssGSEA method was used to estimate the relative proportions of immune cells. The findings were confirmed with a mouse model via flow cytometry and RT-PCR analysis. Results: Our study refined the landscape of CD8 + T-cell subsets in the lung, showing that CD8 + Trm cells accumulated in the lung within 14 days after influenza infection. The classical CD8 + Trm cells co-expressed a high level of CD49a and even maintained 90 days after primary infection. The ratio of CD8 + Trm cells decreased 1 day after influenza reinfection, which may be parallel with their potential transition into effector types, as observed in trajectory inference analysis. KEGG analysis suggested that PD-L1 expression and PD-1 checkpoint pathway were upregulated in CD8 + Trm cells on day 14 after infection. GO and GSVA analyses revealed that PI3K-Akt-mTOR and type I interferon signaling pathways were enriched in CD8 + Tem and Trm cells after reinfection. Additionally, CCL signaling pathways were involved in cell interaction between CD8 + Trm cells and other cells, with Ccl4-Ccr5 and Ccl5-Ccr5 ligand/receptor pairs being important between CD8 + Trm and other memory subsets after infection and reinfection. Conclusion: Our data suggest that resident memory CD8 + T cells with CD49a co-expression account for a large proportion after influenza infection, and they can be rapidly reactivated against reinfection. Function differences exist in CD8 + Trm and Tem cells after influenza infection and reinfection. Ccl5-Ccr5 ligand/receptor pair is important in cell interactions between CD8 + Trm and other subsets.

Abstract:Malignant gliomas, with an average survival time of 16-19 months after initial diagnosis, account for one of the most lethal tumours overall. Current standards in patient care provide only unsatisfying strategies in diagnostic and treatment for high-grade gliomas. Here we describe metabolic phenomena in the choline and glycine network associated with stem cell culture conditions in the classical glioma cell line U87. Using high-resolution proton magnetic resonance spectroscopy of cell culture metabolic extracts we compare the metabolic composition of U87 chronically propagated as adherent culture in medium supplemented with serum to serum-free neurosphere growth. We found that the switch to neurosphere growth, besides the increase of cells expressing the putative glioma stem cell marker CD133, modulated a number of intracellular metabolites including choline, creatine, glycine, and myo-inositol that have been previously reported as potential diagnostic markers in various tumours. These findings highlight the critical influence of culture conditions on glioma cell metabolism, and therefore particular caution should be drawn to the use of in vitro system research in order to investigate cancer metabolism.

Abstract:Abstract Previous studies focusing on the age disparity in COVID-19 severity have suggested that younger individuals mount a more robust innate immune response in the nasal mucosa after infection with SARS-CoV-2. However, it is unclear if this reflects increased immune activation or increased immune residence in the nasal mucosa. We hypothesized that immune residency in the nasal mucosa of healthy individuals may differ across the age range. We applied single-cell RNA-sequencing and measured the cellular composition and transcriptional profile of the nasal mucosa in 35 SARS-CoV-2 negative children and adults, ranging in age from 4 months to 65 years. We analyzed in total of ~ 30,000 immune and epithelial cells and found that age and immune cell proportion in the nasal mucosa are inversely correlated, with little evidence for structural changes in the transcriptional state of a given cell type across the age range. Orthogonal validation by epigenome sequencing indicate that it is especially cells of the innate immune system that underlie the age-association. Additionally, we characterize the predominate immune cell type in the nasal mucosa: a resident T cell like population with potent antiviral properties. These results demonstrate fundamental changes in the immune cell makeup of the uninfected nasal mucosa over the lifespan. The resource we generate here is an asset for future studies focusing on respiratory infection and immunization strategies.

Abstract:A set of 114 olfactory molecules divided into fruity, ethereal and camphoraceous compounds, was submitted to an analysis by Kohonen Neural Networks, also known as Self Organising Map (SOM). The compounds are represented in a hyperspace derived from their molecular descriptors and SOM gives a useful projection of this hyperspace onto a 2D map. Owing to the complexity of the olfaction mechanism, evidenced by the fact that one compound can exhibit simultaneously different properties, SOM alone is unable to take into account the olfaction diversity of the original 114 compounds, Then, a Fuzzy Partition method was applied on the Kohonen map previously developed. The obtained results allowed delineating different representative zones for the three odours, expressing more closely the olfactory richness. The ability of the Hybrid System combining SO and Fuzzy Partition to model the three odours was validated by dividing the 114 compounds into a training set and a test set, including 86 and 28 molecules, respectively. The most important olfactory characteristics were reproduced satisfactorily for the entire test set compounds.

Abstract:Early interactions of influenza A virus (IAV) with respiratory epithelium might determine the outcome of infection. The study of global cellular innate immune responses often masks multiple aspects of the mechanisms by which populations of cells work as organized and heterogeneous systems to defeat virus infection, and how the virus counteracts these systems. In this study, we experimentally dissected the dynamics of IAV and human epithelial respiratory cell interaction during early infection at the single-cell level. We found that the number of viruses infecting a cell (multiplicity of infection [MOI]) influences the magnitude of virus antagonism of the host innate antiviral response. Infections performed at high MOIs resulted in increased viral gene expression per cell and stronger antagonist effect than infections at low MOIs. In addition, single-cell patterns of expression of interferons (IFN) and IFN-stimulated genes (ISGs) provided important insights into the contributions of the infected and bystander cells to the innate immune responses during infection. Specifically, the expression of multiple ISGs was lower in infected than in bystander cells. In contrast with other IFNs, IFN lambda 1 (IFNL1) showed a widespread pattern of expression, suggesting a different cell-to-cell propagation mechanism more reliant on paracrine signaling. Finally, we measured the dynamics of the antiviral response in primary human epithelial cells, which highlighted the importance of early innate immune responses at inhibiting virus spread. IMPORTANCE Influenza A virus (IAV) is a respiratory pathogen of high importance to public health. Annual epidemics of seasonal IAV infections in humans are a significant public health and economic burden. IAV also causes sporadic pandemics, which can have devastating effects. The main target cells for IAV replication are epithelial cells in the respiratory epithelium. The cellular innate immune responses induced in these cells upon infection are critical for defense against the virus, and therefore, it is important to understand the complex interactions between the virus and the host cells. In this study, we investigated the innate immune response to IAV in the respiratory epithelium at the single-cell level, providing a better understanding on how a population of epithelial cells functions as a complex system to orchestrate the response to virus infection and how the virus counteracts this system.

Abstract:The cellular adaptive immune response plays a key role in resolving influenza infection. Experiments where individuals are successively infected with different strains within a short timeframe provide insight into the underlying viral dynamics and the role of a cross-reactive immune response in resolving an acute infection. We construct a mathematical model of within-host influenza viral dynamics including three possible factors which determine the strength of the cross-reactive cellular adaptive immune response: the initial naive T cell number, the avidity of the interaction between T cells and the epitopes presented by infected cells, and the epitope abundance per infected cell. Our model explains the experimentally observed shortening of a second infection when cross-reactivity is present, and shows that memory in the cellular adaptive immune response is necessary to protect against a second infection.

Abstract:Abstract Lung tissue-resident memory T cells (T RM) are critical for the local control of viral respiratory tract infections. However, the exact mechanisms of T RMstimulation and maintenance are not fully understood, rendering the induction of protective T RMresponses by vaccination an ambitious goal. Here, T RMpopulations induced by natural influenza A virus (IAV) infection or by mucosal immunization using adenoviral vector vaccines encoding the IAV antigens HA and NP as well as IL-1β as adjuvant were analyzed. Phenotypic and functional T RMresponses were assessed in mice over 200 days via flow cytometry. Interestingly, long-lasting T RMresponses with a less cytotoxic character were found scattered within the lung tissue after mucosal immunization. By contrast, T RMtriggered by prior IAV infection surrounded bronchus-associated B cell clusters and their quantity waned over time more rapidly. Furthermore, single cell RNA sequencing revealed different gene expression profiles of NP-specific T RMdepending on the priming condition. However, both T RMpopulations provided protection from heterologous challenge, resulting in an expansion of cognate antigen-specific T RM. Substantial differences in IAV-specific T-cell immunity after adenoviral vector immunization or prior infections could be confirmed in the Babraham pig model. Noticeably, increased T RMresponses did not correlate with protection against an infection with a heterologous IAV strain, indicating potentially different correlates of protection in pig and mice. In conclusion, mucosal immunization was associated with a prolonged tissue retention of functional T RM, opening up a promising line of enquiry for the design of future vaccines against respiratory viruses. Abstract:SARS-CoV-2 infection and COVID-19 disease vary with respect to viral variant and host vaccination status. However, how vaccines, emergent variants, and their intersection shift host responses in the human nasal mucosa remains uncharacterized. We and others have shown during the first SARS-CoV-2 wave that a muted nasal epithelial interferon response at the site of infection underlies severe COVID-19. We sought to further understand how upper airway cell subsets and states associate with COVID-19 phenotypes across viral variants and vaccination. Here, we integrated new single-cell RNA-sequencing (scRNA-seq) data from nasopharyngeal swabs collected from 67 adult participants during the Delta and Omicron waves with data from 45 participants collected during the original (Ancestral) wave in our prior study. By characterizing detailed cellular states during infection, we identified changes in epithelial and immune cells that are both unique and shared across variants and vaccination status. By defining SARS-CoV-2 RNA+ cells for each variant, we found that Delta samples had a marked increase in the abundance of viral RNA+ cells. Despite this dramatic increase in viral RNA+ cells in Delta cases, the nasal cellular compositions of Delta and Omicron exhibit greater similarity, driven partly by myeloid subsets, than the Ancestral landscapes associated with specialized epithelial subsets. We found that vaccination prior to infection was surprisingly associated with nasal macrophage recruitment and activation rather than adaptive immune cell signatures. While patients with severe disease caused by Ancestral or Delta variants had muted interferon responses, Omicron-infected patients had equivalent interferon responses regardless of disease severity. Our study defines the evolution of cellular targets and signatures of disease severity in the upper respiratory tract across SARS-CoV-2 variants, and suggests that intramuscular vaccines shape myeloid responses in the nasal mucosa upon SARS-CoV-2 infection.

Abstract:Although NK cells are classified as innate immune cells, recent studies have demonstrated the transformation of NK cells into long-lived memory cells that contribute to secondary immune responses in certain mouse models. However, whether NK cells mount an Ag-specific memory response to acute influenza virus infection has not yet been examined. Here, we show that, consistent with previous studies, lung NK cells play an important role in controlling viral proliferation after primary influenza virus infection. However, although lung NK cells display a memory phenotype at the late stage of infection, these cells do not protect mice against secondary influenza virus infection. Interestingly, liver NK cells from influenza virus infected mice possess a memory phenotype and protect mice against secondary influenza virus infection. Memory-like liver NK cells display a CD49a(+)DX5(-) phenotype, and the adoptive transfer of purified liver CD49a(+)DX5(-) NK cells into naive mice followed by viral infection results in protective immunity and decreased viral titer. Moreover, we demonstrate that primary inactivated influenza virus induces memory NK cells residing in the liver of Rag1(-/-) mice. Collectively, these data suggest that liver CD49a(+)DX5(-) NK cells remember encountered Ag from influenza virus after primary infection and are more protective upon subsequent infection.

Abstract:Abstract For the first time we have defined naïve, central memory, effector memory and differentiated effector porcine CD8 T cells and analyzed their distribution in lymphoid and respiratory tissues after influenza infection or immunization, using peptide-MHC tetramers of three influenza nucleoprotein (NP) epitopes. The hierarchy of response to the three epitopes changes during the response in different tissues. Most NP-specific CD8 T cells in broncho-alveolar lavage (BAL) and lung are tissue resident memory cells (TRM) that express CD69 and downregulate CD45RA and CCR7. NP-specific cells isolated from BAL express genes characteristic of TRM, but gene expression differs at 7, 21 and 63 days post infection. In all tissues the frequency of NP-specific CD8 cells declines over 63 days almost to background levels but is best maintained in BAL. The kinetic of influenza specific memory CD8 T cell in this natural host species differs from that in small animal models.

Abstract:Immune memory represents the most efficient defense against invasion and transmission of infectious pathogens. In contrast to memory T and B cells, the roles of innate immunity in recall responses remain inconclusive. In this study, we identified a novel mouse spleen NK cell subset expressing NKp46 and NKG2A induced by intranasal influenza virus infection. These memory NK cells specifically recognize N-linked glycosylation sites on influenza hemagglutinin (HA) protein. Different from memory-like NK cells reported previously, these NKp46(+) NKG2A(+) memory NK cells exhibited HA-specific silence of cytotoxicity but increase of gamma interferon (IFN-gamma) response against influenza virus-infected cells, which could be reversed by pifithrin-mu, a p53-heat shock protein 70 (HSP70) signaling inhibitor. During recall responses, splenic NKp46(+) NKG2A(+) NK cells were recruited to infected lung and modulated viral clearance of virus and CD8(+) T cell distribution, resulting in improved clinical outcomes. This long-lived NK memory bridges innate and adaptive immune memory response and promotes the homeostasis of local environment during recall response. IMPORTANCE In this study, we demonstrate a novel hemagglutinin (HA)-specific NKp46(+) NKG2A(+) NK cell subset induced by influenza A virus infection. These memory NK cells show virus-specific decreased cytotoxicity and increased gamma interferon (IFN-gamma) on reencountering the same influenza virus antigen. In addition, they modulate host recall responses and CD8 T cell distribution, thus bridging the innate immune and adaptive immune responses during influenza virus infection.
