Abstract:Arterial spin labeling (ASL) technology is a popular technique for evaluating and studying cerebral perfusion in recent years. It can sensitively detect the abnormal cerebral perfusion and effectively avoid the potential risks caused by exogenous contrast agents in other tests. Detection of the abnormal cerebral blood flow perfusion by ASL technology can early predict cognitive decline in normal elderly, explore the pathogeneses of mild cognitive dysfunction and Alzheimer's disease, and evaluate the cerebral blood flow around the ischemic focus and the establishment of collateral circulation in patients with aphasia after stroke. At the same time, it is verified that the decrease of cerebral blood flow perfusion is another precipitating factor of primary progressive aphasia except cortex atrophy. Although there are still some limitations, the ASL technology has a good prospect in the application of cognitive disorders. Abstract:BackgroundRecent works suggested the association between the dysfunctional glymphatic system and Alzheimer's Disease, which shows as alternated cerebrospinal fluid production. It is believed that the choroid plexus is the major site of CSF production conventionally and the choroid plexus blood flow is related to the production rate directly. Therefore, we investigated the choroid plexus perfusion in AD patients using arterial spin labeling (ASL) and MRI retrospectively.Method21 AD patients (15 males, 76.1±5.7 years), 36 MCI patients (20 males, 72.6±7.9 years), and 88 healthy controls (40 males, 74.8±8.0 years) from the ADNI3 were included in this study. The lateral choroid plexus regions were segmented automatically based on a 3D U‐net deep learning method, which has been demonstrated with higher accuracy compared to the Freesurfer. In addition, the gray matter mask was segmented from the T1 weighted images using SPM12, and it was used to transfer the ASL and M0 images to the T1 weighted image space. Relative cerebral blood flow was quantified by first dividing the ASL image with its smoothed M0 image (a Gaussian filter with the sigma of 4 pixels), and then normalizing the mean signal of the choroid plexus region with the mean of the gray matter region. The gray matter mask was chosen as the region with a probability higher than 0.5.ResultIn the nonparametric Mann‐Whitney U test, the choroid plexus volume of the AD patients (1.58±0.40ml) was significantly larger (p = 0.036) than that of the healthy controls (1.41±0.39). In addition, the AD patients (0.85±0.30) showed significantly higher (p = 0.017) relative perfusion of the choroid plexus regions than the healthy controls (0.70±0.20).ConclusionThe enlarged volume and increased blood flow of the choroid plexus may suggest the over‐compensation of CSF production in AD patients. However, further investigation is required to separate the blood flow and CSF signal in ASL at the choroid plexus region.
Aggregation of Vascular Risk Factors Modulates the Amplitude of Low-Frequency Fluctuation in Mild Cognitive Impairment Patients

Abstract:Background: Several vascular risk factors, including hypertension, diabetes, body mass index, and smoking status are found to be associated with cognitive decline and the risk of Alzheimer's disease (AD). We aimed to investigate whether an aggregation of vascular risk factors modulates the amplitude of low-frequency fluctuation (ALFF) in patients with mild cognitive impairment (MCI).Methods: Forty-three MCI patients and twenty-nine healthy controls (HCs) underwent resting-state functional MRI scans, and spontaneous brain activity was measured by the ALFF technique. The vascular risk profile was represented with the Framingham Heart Study general cardiovascular disease (FHS-CVD) risk score, and each group was further divided into high and low risk subgroups. Two-way ANOVA was performed to explore the main effects of diagnosis and vascular risk and their interaction on ALFF.Results: The main effect of diagnosis on ALFF was found in left middle temporal gyrus (LMTG) and left superior parietal gyrus (LSPG), and the main effect of risk on ALFF was detected in left fusiform gyrus (LFFG), left precuneus (LPCUN), and left cerebellum posterior lobe (LCPL). Patients with MCI exhibited increased ALFF in the LMTG and LSPG than HCs, and participants with high vascular risk showed increased ALFF in the LFFG and LCPL, while decreased ALFF in the LPCUN. An interaction between diagnosis (MCI vs. HC) and FHS-CVD risk (high vs. low) regarding ALFF was observed in the left hippocampus (LHIP). HCs with high vascular risk showed significantly increased ALFF in the LHIP than those with low vascular risk, while MCI patients with high vascular risk showed decreased ALFF in the LHIP than HCs with high vascular risk. Interestingly, the mean ALFF of LHIP positively correlated with word recall test in HCs with high vascular risk (rho = 0.630, P = 0.016), while negatively correlated with the same test in MCI patients with high vascular risk (rho = −0.607, P = 0.001).Conclusions: This study provides preliminary evidence highlighting that the aggregation of vascular risk factors modulates the spontaneous brain activity in MCI patients, and this may serve as a potential imaging mechanism underlying vascular contribution to AD.

Abstract:Absolute quantification of cerebral blood flow (CBF), cerebral blood volume (CBV), and mean transit time (MTT) are of great relevance for clinical applications. One of the widely used methods for quantification of these parameters is gamma-variate fitting. Traditional nonlinear regression methods for gamma-variate fitting are inaccurate and computationally demanding. In this study, we developed an adaptive total least square method (ATSSL) to fit a gamma-variate function to the delayed concentration-time course. For each concentration-time curve, the beginning and ending time point of the curve are adaptively determined online. After the curves were fitted, a robust method for automatically determination of arterial input function (AIF) from whole and region of interest (ROI) was developed. Using the obtained AIF and fitted gamma-variate concentration-time curve, the MTT, CBV, and CBF were calculated by utilizing singular value decomposition algorithm. Computer simulations show that the suggested method is adaptive, reliable, and insensitive to noise. Comparison with the traditional nonlinear regression method indicated that the presented method is more accurate and faster to determine the CBV, CBF and MTT.
Effects of Cerebral Blood Flow and White Matter Integrity on Cognition in CADASIL Patients

Abstract:Background: It remains unclear whether the degree of white matter tract damage or cerebral hypoperfusion can better predict global cognitive impairment in CADASIL. We sought to determine the independent effects of cerebral perfusion status and white matter integrity on the cognition. Methods: We reviewed prospectively collected clinical and imaging data from genetically-confirmed CADASIL patients who underwent both arterial spin labeling (ASL) perfusion MRI and diffusion tensor imaging (DTI). We analyzed the cerebral blood flow (CBF), mean diffusion (MD), and fractional anisotropy (FA) by dividing the brain tissue into white matter hyperintensity (WMH) and normal-appearing white matter (NAWM). Global cognitive function was evaluated by using Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment (MoCA). Results: Of the included 29 CADASIL patients, the mean age was 48.4 ± 7.9 years, and 17 (58.6%) were women. MD was significantly correlated with CBF in both WMH (r = -0.407, P = 0.035) and NAWM (r = -0.437, P = 0.023) after adjusting for age and WMH volume. A MoCA score was obtained in 13 patients and was significantly correlated with CBF in both WMH (r = 0.742, P = 0.004) and NAWM (r = 0.659, P = 0.014). Both CBF in WMH (area under the curve, 0.767; 95% CI, 0.586-0.947, P = 0.015) and MD in WMH (area under the curve, 0.740; 95% CI, 0.557-0.924, P = 0.028) were good predictors for cognitive impairment (MMSE score < 27). However, multiple linear regression analysis revealed that global cognitive function was independently associated with CBF in WMH only (standardized β = 0.485, P = 0.015), after adjusting for age, gender, WMH volume, the presence of subcortical infarcts and DTI metrics. Conclusions: Our findings suggested that cerebral hypoperfusion was more strongly associated with global cognitive dysfunction than the severity of brain microstructural damage, supporting that CBF assessed by ASL could serve as a candidate imaging indicator for monitoring alterations of global cognitive function in CADASIL.

Abstract:AbstractImaging and histopathological studies have demonstrated that structural changes of the retina affect subjects with Alzheimer’s disease (AD) or mild cognitive impairment (MCI). The aim of this study was to quantitatively investigate the retinal vessels in these disorders, using dynamic vessel analyzer (DVA) and optical coherence tomography angiography (OCTA) analysis. Twelve subjects with AD, 12 subjects with MCI, and 32 gender- and age-matched controls were prospectively enrolled. Mean ± SD age was 72.9 ± 7.2 years in the AD group, 76.3 ± 6.9 years in the MCI group, and 71.6 ± 5.9 years in the control group (p = 0.104). In the DVA dynamic analysis, the arterial dilation was decreased in the AD group (0.77 ± 2.06%), in the comparison with the control group (3.53 ± 1.25%, p = 0.002). The reaction amplitude was decreased both in AD (0.21 ± 1.80%, <0.0001) and MCI (2.29 ± 1.81%, p = 0.048) subjects, compared with controls (3.86 ± 1.94%). OCTA variables did not differ among groups. In the Pearson correlation analysis, amyloid β level in the cerebrospinal fluid was directly correlated with the arterial dilation (R = 0.441, p = 0.040) and reaction amplitude (R = 0.580, p = 0.005). This study demonstrate that Alzheimer’s and MCI subjects are characterized by a significant impairment of the retinal neurovascular coupling. This impairment is inversely correlated with the level of amyloid β in the cerebrospinal fluid.

Abstract:Early-onset Alzheimer's disease (EOAD) has different pathological and clinical patterns and disease course from late-onset AD (LOAD). Diffusion tensor imaging (DTI) studies have demonstrated white-matter (WM) and grey-matter (GM) microstructural changes that could be markers for AD progression. We therefore sought to demonstrate distinct patterns of brain microstructural in patients with early- and late-onset AD and mild cognitive impairment (MCI). 44 young healthy-controls (YHC, ≤65 years-old), 41 older HC (>65 years-old), 31 early-onset MCI (EOMCI), 59 late-onset MCI (LOMCI), 21 EOAD and 72 LOAD underwent T1 and diffusion MRI imaging. The free-water (FW) method was applied to derive individual FW (GM and WM) and tissue compartment fractional anisotropy (FAt) maps from DTI data. To assess age, stage, and interactions effects, we carried out ANCOVA analyses on WM-FW, WM-FAt, GM-thickness, and GM-FW images. We further tested the associations of brain-measures with Montreal Cognitive Assessment (MoCA) scores. In early-onset patients, minimal WM and GM microstructural changes were seen at the MCI stage, followed by steep damage from the MCI to AD stage. In late-onset patients, progressive microstructural alterations were demonstrated along the AD continuum (Fig. 1Band3B). Across all groups, older subjects had greater WM-FW and lower WM-tissue FAt than the younger counterparts (Fig.1). For GM-thickness, there were no age-related differences at pre-dementia stages, although EOAD had lower thickness than LOAD in the parietal regions (precuneus), while LOAD had lower thickness than EOAD in the precentral gyrus and superior-temporal gyrus (Fig. 2). In contrast to GM-thickness, GM-FW increases appeared early at the pre-dementia stage (LOMCI>EOMCI). EOAD had greater GM-FW increases in the parietal/precuneus and middle-temporal regions than LOAD (Fig. 3). Lastly, parietal GM-FW increases contributed to global cognitive impairment in MCI while parietal and temporal WM abnormalities and GM thinning related to lower MoCA in dementia (Table 1).
Pattern of cerebral hyperperfusion in Alzheimer's disease and amnestic mild cognitive impairment using voxel-based analysis of 3D arterial spin-labeling imaging: initial experience.

Abstract:Purpose: A three-dimensional (3D) continuous pulse arterial spin labeling (ASL) technique was used to investigate cerebral blood flow (CBF) changes in patients with Alzheimer's disease (AD), amnestic mild cognitive impairment (aMCI), and age-and sex-matched healthy controls. Materials and methods: Three groups were recruited for comparison, 24 AD patients, 17 MCI patients, and 21 age-and sex-matched control subjects. Three-dimensional ASL scans covering the entire brain were acquired with a 3.0 T magnetic resonance scanner. Spatial processing was performed with statistical parametric mapping 8. A second-level one-way analysis of variance analysis (threshold at P<0.05) was performed on the preprocessed ASL data. An average whole-brain CBF for each subject was also included as group-level covariates for the perfusion data, to control for individual CBF variations. Results: Significantly increased CBF was detected in bilateral frontal lobes and right temporal subgyral regions in aMCI compared with controls. When comparing AD with aMCI, the major hyperperfusion regions were the right limbic lobe and basal ganglia regions, including the putamen, caudate, lentiform nucleus, and thalamus, and hypoperfusion was found in the left medial frontal lobe, parietal cortex, the right middle temporo-occipital lobe, and particularly, the left anterior cingulate gyrus. We also found decreased CBF in the bilateral temporo-parieto-occipital cortices and left limbic lobe in AD patients, relative to the control group. aMCI subjects showed decreased blood flow in the left occipital lobe, bilateral inferior temporal cortex, and right middle temporal cortex. Conclusion: Our results indicated that ASL provided useful perfusion information in AD disease and may be used as an appealing alternative for further pathologic and neuropsychological studies, especially of compensatory mechanisms for cerebral hypoperfusion.
Decoupling of Regional Cerebral Blood Flow and Brain Function along the Alzheimer's Disease Continuum.

Abstract:Background: Alzheimer's disease (AD) is accompanied with impaired neurovascular coupling. However, its early alteration remains elusive along the AD continuum. Objective: This study aimed to investigate the early disruption of neurovascular coupling in cognitively normal (CN) and mild cognitive impairment (MCI) elderly and its association with cognition and AD pathologies. Methods: We included 43 amyloid-beta-negative CN participants and 38 amyloid-beta-positive individuals (18 CN and 20 MCI) from the Alzheimer's Disease Neuroimaging Initiative dataset. Regional homogeneity (ReHo) map was used to represent neuronal activity and cerebral blood flow (CBF) map was used to represent cerebral blood perfusion. Neurovascular coupling was assessed by CBF/ReHo ratio at the voxel level. Analyses of covariance to detect the between-group differences and to further investigate the relations between CBF/ReHo ratio and AD biomarkers or cognition. In addition, the correlation of cerebral small vessel disease (SVD) burden and neurovascular coupling was assessed as well. Results: Related to amyloid-beta-negative CN group, amyloid-beta-positive groups showed decreased CBF/ReHo ratio mainly in the left medial and inferior temporal gyrus. Furthermore, lower CBF/ReHo ratio was associated with a lower Mini-Mental State Examination score as well as higher AD pathological burden. No association between CBF/ReHo ratio and SVD burden was observed. Conclusion: AD pathology is a major correlate of the disturbed neurovascular coupling along the AD continuum, independent of SVD pathology. The CBF/ReHo ratio may be an index for detecting neurovascular coupling abnormalities, which could be used for early diagnosis in the future.
Impaired Glymphatic Flow on Diffusion Tensor MRI as a Marker of Neurodegeneration in Alzheimer's Disease: Correlation with Gray Matter Volume Loss and Cognitive Decline Independent of Cerebral Amyloid Deposition

Abstract:Background: Impaired glymphatic flow on the Alzheimer's disease (AD) spectrum may be evaluated using diffusion tensor image analysis along the perivascular space (DTI-ALPS). Objective: We aimed to validate impaired glymphatic flow and explore its association with gray matter volume, cognitive status, and cerebral amyloid deposition on the AD spectrum. Methods: 80 participants (mean age, 76.9±8.5 years; 57 women) with AD (n = 65) and cognitively normal (CN) (n = 15) who underwent 3T brain MRI including DTI and/or amyloid PET were included. After adjusting for age, sex, apolipoprotein E status, and burden of white matter hyperintensities, the ALPS-index was compared according to the AD spectrum. The association between the ALPS-index and gray matter volume, cognitive status, and quantitative amyloid from PET was assessed. Results: The ALPS-index in the AD was significantly lower (mean, 1.476; 95% CI, 1.395–1.556) than in the CN (1.784;1.615–1.952; p = 0.026). Volumes of the entorhinal cortex, hippocampus, temporal pole, and primary motor cortex showed significant associations with the ALPS-index (all, p < 0.05). There was a positive correlation between the ALPS-index and MMSE score (partial r = 0.435; p 0.05). Conclusions: Decreased glymphatic flow measured by DTI-ALPS in AD may serve as a marker of neurodegeneration correlating with structural atrophy and cognitive decline.

Disrupted Regional Cerebral Blood Flow, Functional Activity and Connectivity in Alzheimer’s Disease: A Combined ASL Perfusion and Resting State fMRI Study

Abstract:Recent studies have demonstrated a close relationship between regional cerebral blood flow (rCBF) and resting state functional connectivity changes in normal healthy people. However, little is known about the parameter changes in the most vulnerable regions in Alzheimer's disease (AD). Forty AD patients and 30 healthy controls participated in this study. The data of resting-state perfusion and functional magnetic resonance imaging (fMRI) was collected. By using voxel-wise arterial spin labeling (ASL) perfusion, we identified several regions of altered rCBF in AD patients. Then, by using resting state fMRI analysis, including amplitude low frequency fluctuation (ALFF) and seed-based functional connectivity, we investigated the changes of functional activity and connectivity among the identified rCBF regions. We extracted cognition-related parameters and searched for a sensitive biomarker to differentiate the AD patients from the normal controls (NC). Compared with controls, AD patients showed special disruptions in rCBF, which were mainly located in the left posterior cingulate cortex (PCC), the left and right dorsolateral prefrontal cortex (DLPFC), the left inferior parietal lobule (IPL), the right middle temporal gyrus (MTG), the left middle occipital gyrus (MOG), and the left precuneus (PCu). ALFF was performed based on the seven regions identified by the ASL method, and AD patients presented significantly decreased ALFF in the left PCC, left IPL, right MTG, left MOG, and left PCu and increased ALFF in the bilateral DLPFC. We constituted the network based on the seven regions and found that there was decreased connectivity among the identified regions in the AD patients, which predicted a disruption in the default mode network (DMN), executive control network (ECN) and visual network (VN). Furthermore, these abnormal parameters are closely associated with cognitive performances in AD patients. We combined the rCBF and ALFF value of PCC/PCu as a biomarker to differentiate the two groups and reached a sensitivity of 85.3% and a specificity of 88.5%. Our findings suggested that there was disrupted rCBF, functional activity and connectivity in specific cognition-related regions in Alzheimer's disease, which can be used as a valuable imaging biomarker for the diagnosis of AD.

Abstract:Objective: This study investigated cerebral small vessel disease (CSVD) damage patterns in early-onset and late-onset Alzheimer's disease (EOAD and LOAD) and their effects on cognitive function. Methods: This study included 93 participants, 45 AD patients (14 EOAD and 31 LOAD), and 48 normal controls (13 YNC and 35 ONC) from the ADNI database. All participants had diffusion tensor imaging data; some had amyloid PET and plasma p-tau(181) data. The study used peak width of skeletonized mean diffusivity (PSMD) to measure CSVD severity and compared PSMD between patients and age-matched controls. The effect of age on the relationship between PSMD and cognition was also examined. The study also repeated the analysis in amyloid-positive AD patients and amyloid-negative controls in another independent database (31 EOAD and 38 LOAD), and the merged database. Results: EOAD and LOAD showed similar cognitive function and disease severity. PSMD was validated as a reliable correlate of cognitive function. In the ADNI database, PSMD was significantly higher for LOAD and showed a tendency to increase for EOAD; in the independent and merged databases, PSMD was significantly higher for both LOAD and EOAD. The impact of PSMD on cognitive function was notably greater in the younger group (YNC and EOAD) than in the older group (ONC and LOAD), as supported by the ADNI and merged databases. Interpretation: EOAD has less CSVD burden than LOAD, but has a greater impact on cognition. Proactive cerebrovascular prevention strategies may have potential clinical value for younger older adults with cognitive decline.
Clinical application of brain perfusion SPECT/CT imaging with NeuroGamTM software in subjective cognitive decline, mild cognitive impairment and Alzheimer's disease

Abstract:BackgroundSubjective cognitive decline (SCD) and mild cognitive impairment (MCI) are a transitional state between normal aging and mild dementia, and most of them have potential Alzheimer's Disease (AD) pathology. Studying the pathophysiological characteristics of SCD and MCI is helpful to the early diagnosis of AD.PurposeThis study aims to investigate the brain perfusion differences among SCD, MCI and AD, and to evaluate the application value of brain perfusion SPECT/CT imaging combined with NeuroGamTM software in these diseases.MethodsFrom July 2020 to April 2021, 15 patients with SCD, 29 patients with MCI and 36 patients with AD were collected from several general hospitals in Shanghai. All subjects underwent neuropsychological test and 99mTc-ECD brain perfusion SPECT/CT imaging. The perfusion levels of Brodmann areas (BA) in the three groups were semi-quantitatively analyzed by NeuroGamTM, and compared with normal database. Student's t-test, one-way analysis of variance and rank sum test were used for the comparison of quantitative variables. Pearson's chi-squared test was used for the comparison of qualitative variables.ResultsThere is no significant difference in BA 4~11, 17~25, 28, 31~32, 36~40 and 44~47 between SCD group and MCI group. The mean perfusion values of BA 8-right(R), 20-left(L), 21R, 23L, 23R, 24L, 31L, 38L, 38R and 45R in SCD group are significantly higher than those in AD group (p<0.016 7). The mean perfusion values of BA 21R, 23R, 23R, 31L and 39L of MCI group are significantly higher than those in AD group (p<0.016 7). Compared with the normal database, SCD group show hypoperfusion in BA 24L, 24R, 28L and 28R whilst MCI group show hypoperfusion in BA 21L, 21R, 23R, 24L, 24R, 25L, 25R, 28L, 28R, 36L, 36R, 38R and 47L, and AD group show hypoperfusion in BA 21L, 21R, 22L, 23L, 23R, 24L, 24R, 25R, 28L, 28R, 32L, 32R, 36L, 36R, 47L.ConclusionsThere are differences in the degree and range of hypoperfusion in SCD, MCI and AD. Brain perfusion SPECT/CT imaging combined with NeuroGamTM software has certain application value in these diseases and can be used as an auxiliary diagnostic tool.

Abstract:Background: The biological diagnosis criteria of the Alzheimer's disease (AD) suggests that previous work may misclassify the cognitive impairment caused by other factors into AD. Consequently, re-assessing the imaging profile of AD continuum is needed. Considering the high vulnerability of cortical association fibers, we aimed to elucidate the cortical demyelination process in the AD continuum biologically defined. Methods: According to the biological diagnosis criteria, we determined the positive amyloid status (A+) as cerebrospinal fluid (CSF) amyloid(1-42) < 192 pg/ml, Florbetapir Positron emission tomography (PET) composite standardized uptake value ratio (SUVR) > 1.11. Also, the positive Tau status (T+) was determined as p-Tau(181) > 23 pg/ml. Based on the cognitive characterization, we further categorized 252 subjects into 27 cognitively unimpaired with normal AD biomarkers (A-T-, controls), 49 preclinical AD (A+T+), 113 AD with mild cognitive impairment (MCI) (A+T+), and 63 AD dementia (A+T+). We estimated the intracortical myelin content used the T1- and T2-weighted (T1W/T2W) ratio mapping. To investigate the sensitivity of the ratio mapping, we also utilized well-validated AD imaging biomarkers as the reference, including gray matter volume and Fludeoxyglucose PET (FDG-PET). Based on the general linear model, we conducted the voxel-wise two-sample T-tests between the controls and each group in the AD continuum. Results: Compared to the controls, the results showed that the preclinical AD patients exhibited decreased T1W/T2W ratio value in the right inferior parietal lobule (IPL); as the disease progresses, the prodromal AD patients demonstrated lower ratio value in bilateral IPL, with hippocampus (HP) atrophy. Lastly, the AD dementia patients exhibited decreased ratio value in bilateral IPL and hippocampus; also, we observed the bilateral temporal cortices atrophy and widespread decreased metabolism in the AD dementia patients. After corrected with gray volume, the results remained mostly unchanged. Conclusion: Our study implied the decreased right IPL T1W/T2W ratio might represent early AD-related demyelination in disease continuum. Additionally, we demonstrated that the T1W/T2W ratio mapping is an easy-to-implement and sensitive metric to assess the intracortical myelin content in AD, particularly in the early stage.
Application of Cerebral MR Perfusion Imaging Using Pulsed Arterial Spin Labeling Technique in Patients with Amnestic-Type Mild Cognitive Impairment and Mild Alzheimer Disease

Abstract:Objective To study the value of relative cerebral blood flow(rCBF)changes in patients with amnestic-type mild cognitive impairment (aMCI)and mild Alzheimer disease(AD) using MRI pulsed arterial spin labeling(PASL).Methods A prospective study recruited 37 aMCI patients (aMCI group),30 mild AD patients(mild AD group) and 30 healthy volunteers (normal control group) from March 2011 to December 2013,MRI using PASL for cerebral perfusion imaging was performed and data of rCBF were collected.Taking age as covariate,analysis of variance (ANONA)was carried out to assess the difference of rCBF among all the three groups,then Bonferroni was done between every two groups.A follow-up examination using PASL was performed in the seventeen patients of the aMCI group.And paired t-test was used for comparing the longitudinal change of their rCBF data.Results Compared with the normal control group,the aMCI group showed significant increase of rCBF in bilateral posterior cingulate cortices and precuneus (cluster number 2 785,P＜0.05).While the mild AD group showed decrease of rCBF in the left inferior and superior parietal lobes,the angular,middle frontal lobe,as well as the right superior temporal lobe (cluster number 3 459-5 206,P＜0.05).When compared with the aMCI group,the mild AD group showed regional hypoperfusion in bilateral middle frontal lobes,the left precuneus,the right postcentral and inferior parietal lobe (cluster number 3 236-19 863,P＜0.05).In the longitudinal study of the 17 aMCI patients,an increased rCBF was found to coexist with reduced rCBF in the left inferior frontal and lateral occipital cortex,bilateral frontal poles and paracingulate gyrus,with hyperperfusion dominated.Increased rCBF was also detected in the left temporal lobe,the angular gyrus and precuneus,while decreased rCBF was present in the left putamen,the operculum and right corpus callosum (P＜0.05).Conclusions ASL perfusion imaging is a valuable method for dynamic monitoring of the cerebral perfusion changes in aMCI and AD patients.PASL will assist in finding a useful imaging biomarker for early diagnosis of AD.
Cerebral blood flow in mild cognitive impairment and Alzheimer’s disease: A systematic review and meta-analysis

Abstract:BACKGROUND: Reduced cerebral blood flow (CBF) contributes to the pathophysiology of Alzheimer's disease (AD). However, it is unclear whether there is a spatial-temporal-specific pattern of changed CBF in AD progression.METHODS: We systematically screened literature databases for cross-sectional and longitudinal studies reporting resting CBF or CBF velocity (CBFv) among patients with AD, mild cognitive impairment (MCI), and healthy controls (HCs). Standardised mean differences (SMDs) for CBF and mean differences (MDs) for CBFv were calculated. Quality assessments, meta-analysis, subgroup analysis, and meta-regression were subsequently performed (PROSPERO: CRD42020207548).RESULTS: Overall, 244 studies comprising 13,644 participants and 60 regions were included. Compared with HCs, AD subjects had decreased resting CBF throughout the brain (SMD range: -1.87 to -0.32), especially within the posterior cingulate and temporal-parietal regions. However, MCI subjects presented decreased CBF in ten regions with modest effects (SMD range: -0.86 to -0.25), especially in the precuneus. We identified the decreased CBF in the temporal, parietal, and hippocampal regions was associated with the lower AD Mini-Mental State Examination scores.CONCLUSIONS: Our findings suggest that the spatial-temporal pattern of CBF decreased from the precuneus, posterior cingulate and temporal-parietal regions to broader areas with progression from HC to MCI to AD, supporting the incorporation of CBF into the AD research framework.

Abstract:Mild cognitive impairment (MCI), a well-defined nonmotor manifestation of Parkinson's disease (PD), greatly impairs functioning and quality of life. However, the contribution of cerebral perfusion, quantified by arterial spin labeling (ASL), to MCI in PD remains poorly understood. The selection of an optimal delay time is difficult for single-delay ASL, a problem which is avoided by multidelay ASL. This study uses a multidelay multiparametric ASL to investigate cerebral perfusion including cerebral blood flow (CBF) and arterial transit time (ATT) in early stage PD patients exhibiting MCI using a voxel-based brain analysis. Magnetic resonance imaging data were acquired on a 3.0 T system at rest in 39 early stage PD patients either with MCI (PD-MCI, N = 22) or with normal cognition (PD-N, N = 17), and 36 age- and gender-matched healthy controls (HCs). CBF and ATT were compared among the three groups with SPM using analysis of variance followed by post hoc analyses to define regional differences and examine their relationship to clinical data. PD-MCI showed prolonged ATT in right thalamus compared to both PD-N and HC, and in right supramarginal gyrus compared to HC. PD-N showed shorter ATT in left superior frontal cortex compared to HC. Prolonged ATT in right thalamus was negatively correlated with the category fluency test (p = .027, r = -0.495) in the PD-MCI group. This study shows that ATT may be a more sensitive marker than CBF for the MCI, and highlights the potential role of thalamus and inferior parietal region for MCI in early stage PD.

Abstract:Purpose Early diagnosis of Alzheimer's disease is lack of objective imaging marker.This study evaluates characteristics of cerebral blood flow (CBF) and gray matter atrophy in patients with mild Alzheimer's disease (AD) by using 3D arterial spin labeling (3D ASL) and thin slice 3D T1 weighted images of voxel-based method (VBM).Materials and Methods Sixteen mild AD patients (mild AD group) and sixteen normal control subjects (control group) were recruited.3D ASL and T1WI SPGR sequences were performed.By using voxel-based method,the whole brain CBF and T1WI images were analyzed.CBF and volume of gray matter were compared between two groups,and correlation analysis was done.Results Compared with control group,CBF hypoperfusion was detected in bilateral precuneus,cunei,middle temporal cortex,superior temporalcortex,left parahippocampal gyrus,left superior temporal pole and right superioroccipital gyrus in mild AD group (t=3.84,Pcorrected＜0.05).Compared with control group,gray matter atrophy was found in bilateral hippocampi,amygdalae,superior temporal pole,left parahippocampal gyrus,left inferior temporal cortex in mild AD group (t=4.12,Pcorrected＜0.05).There was a correlation in left parahippocampal gyrus and left upper pole of the temporal between changes of CBF and volume of gray matter in mild AD patients (r=0.50,P＜0.05).Conclusion Voxel-based VBM and ASL can evaluate AD patients' cerebral atrophy and CBF change in early stage,and there is a correlation between changes of CBF and gray matter atrophy in some overlap areas.
