Abstract:Crimean-Congo hemorrhagic fever virus (CCHFV) is the causative agent of Crimean-Congo hemorrhagic fever (CCHF), a highly contagious and potentially fatal emerging disease. We assessed CCHFV seroprevalence by conducting a serological survey of two cohorts from Brazzaville, Congo and Bamako, Mali. We retrospectively screened 581 sera samples, including 352 from monitoring centers for people living with HIV (PLWH) in Brazzaville and 229 provided by the Blood Transfusion Center at Gabriel Touré Hospital in Bamako. An ELISA kit (ID Screen® CCHF Double Antigen Multi-species, Innovative Diagnostics) was used to detect total anti-CCHFV antibodies in serum. CCHFV seroprevalence was 0.6% in the PLWH cohort in Brazzaville, all in a peri‑urban area near livestock/agriculture, and 1.75% in a cohort of blood donors in Bamako, half living in a peri‑urban area near livestock/agriculture and the others performing risk-exposure activities, such as working as a butcher or with frequent rural travels. PLWH from Brazzaville were mostly female, older, and more highly educated, with a tertiary sector activity and living in an urban biotope without livestock/agricultural activities in the surroundings, in contrast to the blood donors of Bamako, who were younger and more likely to live in peri‑urban/rural areas with livestock/agricultural activities in the surroundings. Despite a low CCHFV seroprevalence, our study indicates human contact with CCHFV in sub-urban areas of the capital cities of Congo and Mali associated with previously described CCHFV risk factors.

Abstract:Objectives Healthcare workers (HCWs) are on the frontline of combating COVID-19, hence are at elevated risk of contracting an infection with SARS-CoV-2. The present study aims to measure the impact of SARS-CoV-2 on HCWs in central sub-Saharan Africa. Setting A cross-sectional serological study was conducted at six urban and five rural hospitals during the first pandemic wave in the South Kivu province, Democratic Republic of the Congo (DRC). Participants Serum specimens from 1029 HCWs employed during the first pandemic wave were collected between August and October 2020, and data on demographics and work-related factors were recorded during structured interviews. Primary and secondary outcome measures The presence of IgG antibodies against SARS-CoV-2 was examined by ELISA. Positive specimens were further tested using a micro-neutralisation assay. Factors driving SARS-CoV-2 seropositivity were assessed by multivariable analysis. Results Overall SARS-CoV-2 seroprevalence was high among HCWs (33.1%), and significantly higher in urban (41.5%) compared with rural (19.8%) hospitals. Having had presented with COVID-19-like symptoms before was a strong predictor of seropositivity (31.5%). Personal protective equipment (PPE, 88.1% and 11.9%) and alcohol-based hand sanitizer (71.1% and 28.9%) were more often available, and hand hygiene was more often reported after patient contact (63.0% and 37.0%) in urban compared with rural hospitals, respectively. This may suggest that higher exposure during non-work times in high incidence urban areas counteracts higher work protection levels of HCWs. Conclusions High SARS-CoV-2 seropositivity indicates widespread transmission of the virus in this region of DRC. Given the absence of publicly reported cases during the same time period at the rural sites, serological studies are very relevant in revealing infection dynamics especially in regions with low diagnostic capacities. This, and discrepancies in the application of PPE between urban and rural sites, should be considered in future pandemic response programmes.

Abstract:Background: The Banalia health zone in the Democratic Republic of Congo reported a meningitis epidemic in 2021 that evolved outside the epidemic season. We assessed the effects of the meningitis epidemic response. Methods: The standard case definition was used to identify cases. Care was provided to 2651 in-patients, with 8% of them laboratory tested, and reactive vaccination was conducted. To assess the effects of reactive vaccination and treatment with ceftriaxone, a statistical analysis was performed. Results: Overall, 2662 suspected cases of meningitis with 205 deaths were reported. The highest number of cases occurred in the 30–39 years age group (927; 38.5%). Ceftriaxone contributed to preventing deaths with a case fatality rate that decreased from 70.4% before to 7.7% after ceftriaxone was introduced (p = 0.001). Neisseria meningitidis W was isolated, accounting for 47/57 (82%), of which 92% of the strains belonged to the clonal complex 11. Reactive vaccination of individuals in Banalia aged 1–19 years with a meningococcal multivalent conjugate (ACWY) vaccine (Menactra®) coverage of 104.6% resulted in an 82% decline in suspected meningitis cases (incidence rate ratio, 0.18; 95% confidence interval, 0.02–0.80; p = 0.041). Conclusion: Despite late detection (two months) and reactive vaccination four months after crossing the epidemic threshold, interventions implemented in Banalia contributed to the control of the epidemic.

Abstract:Since the beginning of 2023, the number of people with suspected monkeypox virus (MPXV) infection have sharply increased in the Democratic Republic of the Congo (DRC). We report near-to-complete MPXV genome sequences derived from six cases from the South Kivu province. Phylogenetic analyses reveal that the MPXV affecting the cases belongs to a novel Clade I sub-lineage. The outbreak strain genome lacks the target sequence of the probe and primers of a commonly used Clade I-specific real-time PCR.

Abstract:OBJECTIVES: The InVITE study, starting in August 2021, was designed to examine the immunogenicity of different vaccine regimens in several countries including the Democratic Republic of Congo, Guinea, Liberia, and Mali. Prevaccination baseline samples were used to obtain estimates of previous SARS-CoV-2 infection in the study population.METHODS: Adult participants were enrolled upon receipt of their initial COVID-19 vaccine from August 2021 to June 2022. Demographic and comorbidity data were collected at the time of baseline sample collection. SARS-CoV-2 serum anti-Spike and anti-Nucleocapsid antibody levels were measured.RESULTS: Samples tested included 1016, 375, 663, and 776, from DRC, Guinea, Liberia, and Mali, respectively. Only 0.8% of participants reported a prior positive SARS-CoV-2 test, while 83% and 68% had anti-Spike and anti-Nucleocapsid antibodies, respectively.CONCLUSIONS: Overall SARS-CoV-2 seroprevalence was 86% over the accrual period, suggesting a high prevalence of SARS-CoV-2 infection. Low rates of prior positive test results may be explained by asymptomatic infections, limited access to SARS-CoV-2 test kits and health care, and inadequate surveillance. These seroprevalence rates are from a convenience sample and may not be representative of the population in general, underscoring the need for timely, well-conducted surveillance as part of global pandemic preparedness.

Abstract:The Democratic Republic of the Congo (DRC) has a history of nonhuman primate (NHP) consumption and exposure to simian retroviruses yet little is known about the extent of zoonotic simian retroviral infections in DRC. We examined the prevalence of human T-lymphotropic viruses (HTLV), a retrovirus group of simian origin, in a large population of persons with frequent NHP exposures and a history of simian foamy virus infection. We screened plasma from 3,051 persons living in rural villages in central DRC using HTLV EIA and western blot (WB). PCR amplification of HTLV tax and LTR sequences from buffy coat DNA was used to confirm infection and to measure proviral loads (pVLs). We used phylogenetic analyses of LTR sequences to infer evolutionary histories and potential transmission clusters. Questionnaire data was analyzed in conjunction with serological and molecular data. A relatively high proportion of the study population (5.4%, n = 165) were WB seropositive: 128 HTLV-1-like, 3 HTLV-2-like, and 34 HTLV-positive but untypeable profiles. 85 persons had HTLV indeterminate WB profiles. HTLV seroreactivity was higher in females, wives, heads of households, and increased with age. HTLV-1 LTR sequences from 109 persons clustered strongly with HTLV-1 and STLV-1 subtype B from humans and simians from DRC, with most sequences more closely related to STLV-1 from Allenopithecus nigroviridis (Allen's swamp monkey). While 18 potential transmission clusters were identified, most were in different households, villages, and health zones. Three HTLV-1-infected persons were co-infected with simian foamy virus. The mean and median percentage of HTLV-1 pVLs were 5.72% and 1.53%, respectively, but were not associated with age, NHP exposure, village, or gender. We document high HTLV prevalence in DRC likely originating from STLV-1. We demonstrate regional spread of HTLV-1 in DRC with pVLs reported to be associated with HTLV disease, supporting local and national public health measures to prevent spread and morbidity.

Abstract:The wildlife reservoir and spillover mechanisms of the Ebola virus remain elusive despite extensive research efforts in endemic areas. This study employed a One Health approach to examine the virus’ circulation in wildlife and the associated human exposure risks in the Tshuapa province of the Democratic Republic of the Congo. In 2021, we collected 1015 samples from 888 animals, predominantly small mammals, and 380 inhabitants of Inkanamongo village, the site of an Ebola virus disease outbreak in 2014. These samples were screened for evidence of current (RNA) or past (IgG antibodies) Ebola virus infections. We also conducted interviews with 167 individuals in the surrounding districts to assess their interactions with wildlife. While no Ebola virus RNA was detected in the wildlife samples, anti-orthoebolavirus IgG antibodies were found in 13 bats and 38 rodents. Among the human participants, 120 individuals had IgG antibodies against at least one orthoebolavirus antigen, with 12 showing seropositivity for two antigens of the same orthoebolavirus, despite not having a prior Ebola disease diagnosis. Furthermore, over 50% of respondents reported frequent visits to the forest to hunt a variety of wild animals, particularly ungulates and rodents, which could account for occasional viral spillovers. The absence of active Ebola virus circulation in wildlife may reflect seasonal patterns in reservoir ecology, like those observed in bats. Similarly, seasonal human activities, like hunting and foraging, may result in periodic exposure risks. These findings highlight the importance of continuous, multidisciplinary surveillance to monitor changes in seasonal spillover risks.

Abstract:Studies on the impact of the COVID‐19 pandemic in sub‐Saharan Africa have yielded varying results, although authors universally agree the real burden surpasses reported cases. The primary objective of this study was to determine SARS‐CoV‐2 seroprevalence among patients attending Monkole Hospital in Kinshasa (D.R. Congo). The secondary objective was to evaluate the analytic performance of two chemiluminescence platforms: Elecsys® (Roche) and VirClia® (Vircell) on dried blood spot samples (DBS). The study population (N = 373) was recruited in two stages: a mid‐2021 blood donor cohort (15.5% women) and a mid‐2022 women cohort. Crude global seroprevalence was 61% (53.9%–67.8%) pre‐Delta in 2021 and 90.2% (84.7%–94.2%) post‐Omicron in 2022. Anti‐spike (S) antibody levels significantly increased from 53.1 (31.8–131.3) U/mL in 2021 to 436.5 (219.3–950.5) U/mL in 2022 and were significantly higher above 45 years old in the 2022 population. Both platforms showed good analytic performance on DBS samples: sensitivity was 96.8% for IgG (antiN/S) (93.9%–98.5%) and 96.0% (93.0%–98.0%) for anti‐S quantification. These results provide additional support for the notion that exposure to SARS‐CoV‐2 is more widespread than indicated by case‐based surveillance and will be able to guide the pandemic response and strategy moving forward. Likewise, this study contributes evidence to the reliability of DBS as a tool for serological testing and diagnosis in resource‐limited settings.

Abstract:Background: Monkeypox virus clade I is endemic in several central African countries and characterised by an increase in disease severity and mortality. Since October, 2023, a large-scale mpox outbreak has emerged in DR Congo, and in March, 2024, the first individuals with mpox were reported outside the endemic areas in Republic of the Congo. We aimed to provide insight into the epidemic by sequencing samples obtained from individuals with mpox in Republic of the Congo. Methods: In this cross-sectional, descriptive study, samples were collected from individuals with suspected mpox between Jan 15 and April 8, 2024, in Brazzaville, Pointe-Noire, Likouala, Cuvette-Centrale, and Plateaux (Republic of the Congo). Blood samples, skin or oropharyngeal swabs, or skin crusts were obtained for molecular diagnosis via real-time PCR. Monkeypox virus sequences were obtained and analysed using newly established nanopore sequencing methodology and bioinformatic pipeline. The sequences obtained were aligned and used to construct a maximum likelihood phylogenetic tree using IG-TREE. Findings: 61 samples were collected from individuals with suspected mpox, 31 of which were positive for monkeypox virus and were included in our analysis (four positive samples were excluded due to unavailability of epidemiological data or insufficient biological material). Individuals who tested positive for monkeypox virus were from Cuvette-Centrale (19 [61%] of 31), Likouala (eight [26%]), and Pointe-Noire (four [13%]). 20 (65%) were male and 11 (35%) were female. Phylogenetic analysis of sequences showed two major clusters within clade Ia. One cluster was made up of four sequences from this study clustering with two monkeypox virus sequences from the current DR Congo outbreak, three older sequences from Central African Republic sequenced between 2017 and 2018, and seven sequences from DR Congo sequenced in 2006-07 and 2022. The second cluster was made up of 16 sequences from this study clustering with sequences from the current DR Congo outbreak. In addition, sequences from Republic of the Congo show multiple phylogenetic positioning suggesting the occurrence of multiple co-circulating strains in the human population. Interpretation: Our findings suggest that multiple monkeypox virus strains are co-circulating in the human population, highlighting the need for implementation of expanded mpox surveillance, especially in countries bordering DR Congo and Republic of the Congo, in combination with control measures focused on containing the current outbreaks in these countries to prevent escalation into a larger-scale epidemic. Twenty-three provinces in the DRC have been affected, 15 of which border another country, including the Republic of the Congo (RoC). The RoC is bordered by five of these affected provinces, increasing the risk of cross-border spread. In the RoC, Mpox cases have historically occurred in the endemic areas of the Likouala department in the north. Recently, suspected cases have emerged outside this region, and it is unclear if these cases are recent spillover events from the DRC outbreaks. Therefore, we performed molecular analyses of these cases in the RoC. Methods: Samples were collected from suspected cases between January and April 29, 2024, in Likouala, Cuvette-Centralle, and Pointe-Noire. Blood samples, skin/oropharyngeal swabs or skin crusts were obtained for molecular diagnosis at the Laboratoire National de Sante Publique (LNSP), Brazzaville. MPXV sequences were obtained and analyzed using newly established Nanopore sequencing methodology and bioinformatic pipeline. Findings: Phylogenetic analysis of sequences shows the clustering of MPXV sequences obtained from cases in RoC with sequences from the ongoing Mpox outbreak in the DRC. In addition, sequences from the RoC show multiple phylogenetic positioning suggesting the occurrence of multiple co-circulating strains in the human population. Interpretation: The close genetic relatedness between sequences from RoC and those from DRC indicates a possible cross-border transmission of MPXV from DRC to RoC. These data highlight the need for implementation of expanded surveillance in countries bordering DRC and RoC, in combination with control measures focused at containing the current outbreaks in DRC and RoC to prevent escalation into a larger-scale epidemic.

Mapping and sequencing of cases from an ongoing outbreak of Clade Ib monkeypox virus in South Kivu, Eastern Democratic Republic of the Congo between September 2023 to June 2024

Abstract:Background In September 2023, an mpox outbreak was reported in the eastern part, South Kivu Province, of Democratic Republic of the Congo. This outbreak is still ongoing and expanding to other regions and countries. Here, we describe the epidemiological and genomic evolution of the outbreak from September 2023 to June 2024. Methods Consenting patients with mpox-like symptoms admitted to the Kamituga and the Kamanyola hospitals were recruited to the study. Samples from throat, lesions, breast milk and placenta were collected for PCR testing and sequencing. For the patients from Kamituga hospital, data on place of residence and possible exposures were collected by interviews. The location and numbers of employees were collected for all bars with sex workers. Where possible, exposures were linked to the genomic sequencing data for cluster analysis. Findings In total, 670 (suspected) mpox cases were admitted to the Kamituga hospital. There were slightly more female than male cases (351/670 [52,4%] versus 319/670, [47,6%], and cases were reported from 17 different health areas. The majority of cases were reported in Mero (205/670 [30,6%]), followed by Kimbangu (115/670 [17,2%]), Kabukungu (105/670 [16,7%]), and Asuku (73/670 [10.9%]). During this period, 7 deaths occurred and 8 out of 14 women who were pregnant had fetal loss. Three healthcare workers acquired mpox infection when caring for patients. In depth case ascertainment showed that 83,4% of patients reported recent visits to bars for (professional) sexual interactions as a likely source of infection. Whole genome sequencing resulted in the generation of 58 genome sequences. Three main clusters characterized by specific mutations were identified and several miniclusters of 2 or more sequences with over two shared mutations. No clear link between sequence cluster, bar or health area was observed. The more recent sequences from Kamanyola were related to the sequences in Kamituga and confirmed to be Clade Ib. However, relatively long branches were observed and one of the sequences clustered with publicly released sequences from travelers in Kenya, Uganda, Sweden and Thailand, indicating more undocumented ongoing spread for cluster A than for the other clusters. Most observed mutations were APOBEC-3 related mutations indicative of ongoing human-to-human transmission. Interpretation These data suggests that the rapid transmission of monkeypox virus until June 2024 was mostly related to interactions with professional sex workers (PSW) within densely populated health areas. The expanding number of cases and the recent expansion to 29 other nearby health zones of South -Kivu as well as Rwanda, Burundi, Uganda and Kenya stresses the need for cross border surveillance and collaboration. Urgent enhanced response action is needed, including case finding, diagnostic capacity building, health education programmes focussing on sex workers, and possibly vaccination to limit further escalation and stop this outbreak.

Abstract:On 21 March 2023 the Tanzania's Ministry of Health reported the first Marburg virus disease (MVD) outbreak in Bukoba District reporting a total of eight cases and five fatalities including one health care worker with a case fatality ratio of 62.5%. MVD is a filoviral infection with an estimated incubation of 3-21 days and causes severe hemorrhagic fever in humans. Fruit bats are significant reservoir host leading to animal-to-human transmission and human-to-human transmission by direct contact of body fluids from an infected person. Symptoms and signs include fever, vomiting, diarrhea, body malaise, massive hemorrhage, and multiorgan failure. Currently, no definitive treatment or licensed vaccines are available to date but only supportive care. This outbreak is an alarming concern to the neighboring countries to contain the outbreak. Within 3 years from 2020 to 2023 Tanzania has already recorded one pandemic, which is the novel coronavirus disease 2019 and two epidemics, which are Cholera, Dengue, and now MVD. Tanzanian's Ministry of Health is drawing lessons from the previous health emergencies to contain this particular epidemic. To impede the MVD outbreak in Tanzania, the focus of this commentary is on highlighting the efforts performed and the significant recommendations provided to relevant organizations and the general public.

Abstract:Objectives This study aimed to investigate the prevalence of orthoebolavirus antibodies in Madina Oula, a non-epidemic rural area in Guinea, in 2022. Methods A cross-sectional study conducted from March 14 to April 3, 2022, involved recording household and socio-demographic characteristics, lifestyle data, and collecting dried blood spots (DBS) from 878 individuals in 235 households. DBS were tested using multiplex serology to detect antibodies to different orthoebolaviruses : Ebola (EBOV), Bundibugyo (BDBV), Sudan (SUDV), Reston (RESTV) and Bombali (BOMV). Seroprevalence was estimated with a 95% confidence interval (CI) and Z-test was performed to compare seropositivity between in children under 15 and those over 15 Household and participant characteristics were analyzed using descriptive statistic, and socio-historical conditions were discussed. Results Serological analysis conducted in 2022 on 878 participants revealed varying reactivity to orthoebolavirus antigens, notably with GP antigens, particularly glycoprotein SUDV (16%). Twenty-one samples exhibited reactivity with at least two antigens, with a median age of 27 years (interquartile range = 10.00 - 35.00) ranging from 2 to 80 years. There is no significant difference between seropositivity in children under 15 (2.86%) and those over 15 (2.14%). Antibody presence varied per village, with the highest prevalence observed in Ouassou and Dar-es-Salam. Conclusion Serological data in a region unaffected by recent Ebola outbreaks indicate possible orthoebolavirus endemicity, emphasizing the need for preparedness against known or novel orthoebolaviruses with potential cross-reactivity.

Abstract:Despite the successes in wild-type polio eradication, poor vaccine coverage in the DRC has led to the occurrence of circulating vaccine-derived poliovirus outbreaks. This cross-sectional population-based survey provides an update to previous poliovirus-neutralizing antibody seroprevalence studies in the DRC and quantifies risk factors for under-immunization and parental knowledge that guide vaccine decision making. Among the 964 children between 6 and 35 months in our survey, 43.8% (95% CI: 40.6–47.0%), 41.1% (38.0–44.2%), and 38.0% (34.9–41.0%) had protective neutralizing titers to polio types 1, 2, and 3, respectively. We found that 60.7% of parents reported knowing about polio, yet 25.6% reported knowing how it spreads. Our data supported the conclusion that polio outreach efforts were successfully connecting with communities—79.4% of participants had someone come to their home with information about polio, and 88.5% had heard of a polio vaccination campaign. Additionally, the odds of seroreactivity to only serotype 2 were far greater in health zones that had a history of supplementary immunization activities (SIAs) compared to health zones that did not. While SIAs may be reaching under-vaccinated communities as a whole, these results are a continuation of the downward trend of seroprevalence rates in this region.

Abstract:Summary Background The rVSVΔG-ZEBOV-GP vaccine constitutes a valuable tool to control Ebola virus disease outbreaks. This retrospective cohort study aimed to assess the protective effect of the vaccine against death among patients with confirmed Ebola virus disease. Methods In this retrospective cohort analysis of patients with confirmed Ebola virus disease admitted to Ebola health facilities in the Democratic Republic of the Congo between July 27, 2018, and April 27, 2020, we performed univariate and multivariate analyses to assess case fatality risk and cycle threshold for nucleoprotein according to vaccination status, Ebola virus disease-specific treatments (eg, mAb114 and REGN-EB3), and other risk factors. Findings We analysed all 2279 patients with confirmed Ebola virus disease. Of these 2279 patients, 1300 (57%) were female and 979 (43%) were male. Vaccination significantly lowered case fatality risk (vaccinated: 25% [106/423] vs not vaccinated: 56% [570/1015]; p<0·0001). In adjusted analyses, vaccination significantly lowered the risk of death compared with no vaccination, with protection increasing as time elapsed from vaccination to symptom onset (vaccinated ≤2 days before onset: 27% [27/99], adjusted relative risk 0·56 [95% CI 0·36–0·82, p=0·0046]; 3–9 days before onset: 20% [28/139], 0·44 [0·29–0·65, p=0·0001]; ≥10 days before onset: 18% [12/68], 0·40 [0·21–0·69; p=0·0022]; vaccination date unknown: 33% [39/117], 0·69 [0·48–0·96; p=0·0341]; and vaccination status unknown: 52% [441/841], 0·80 [0·70–0·91, p=0·0011]). Longer time from symptom onset to admission significantly increased risk of death (49% [1117/2279], 1·03 [1·02–1·05; p<0·0001]). Cycle threshold values for nucleoprotein were significantly higher—indicating lower viraemia—among patients who were vaccinated compared with those who were not vaccinated; the highest difference was observed among those vaccinated 21 days or longer before symptom onset (median 30·0 cycles [IQR 24·6–33·7]) compared with patients who were not vaccinated (21·4 cycles [18·4–25·9], p<0·0001). Interpretation To our knowledge, this is the first observational study describing the protective effect of rVSVΔG-ZEBOV-GP vaccination against death among patients with confirmed Ebola virus disease admitted to an Ebola health facility. Vaccination was protective against death for all patients, even when adjusted for Ebola virus disease-specific treatment, age group, and time from symptom onset to admission.
