Abstract:Although haemodialysis is an established treatment for patients with end-stage kidney disease, sustaining life for more than 2 million patients world-wide, the mortality of dialysis patients remains high and is greater than that for some of the more common solid organ cancers. As such, the question arises as to whether more efficient clearance of the waste products of metabolism which accumulate would improve outcomes. Recent reports of an association between improved patient survivals with higher-volume on-line haemodiafiltration exchanges would support this hypothesis. This has led to both the development of newer dialyser designs based on microfluidics using convective clearances to increase middle-molecule clearances and also a generation of superflux dialysers designed to remove larger-molecular-weight azotaemic toxins which have yet to be studied in large randomised prospective clinical trials. However, haemodiafiltration and superflux dialysers do not affectively clear protein-bound azotaemic toxins, and there is accumulating evidence that some of these toxins increase cardiovascular morbidity and mortality. This has led to resurgence in the interest of developing adsorption devices, using activated carbon technology, and the development of composite dialyser membranes by either adding carbons or other biomaterials to increase adsorption capacity to the standard dialyser. While anaphylactoid reactions used to be a recognised complication of haemodialysis, improvements in dialyser membrane bioincompatibility and changing sterilisation techniques have markedly reduced these reactions. Organic chemicals can leach out from the plastics in the blood lines and dialyser, and attention is required to adequately rinse the extracorporeal circuit to reduce patient exposure.

Abstract:A hallmark of chronic kidney disease is the retention of solutes that normally are eliminated by the kidneys. The current classification defines uremic toxins based on molecular weight and protein affinity. The retention of solutes is already detected in the early stages of the disease when patients are pauci-symptomatic or asymptomatic but the role of therapies to retard the loss of kidney function in patients with chronic kidney disease (e.g., modulators of the renin-angiotensin-aldosterone system, sodium-glucose cotransporter inhibitors) in reducing uremic toxins is poorly understood. Most of the research evaluating the impact of therapies to lower serum concentrations of those toxic compounds is carried out in patients with kidney failure already undergoing kidney replacement therapy. The removal of those molecules relies in physicochemical mass transfer phenomena, i.e., adsorption, diffusion, and convection. In the past 2 decades, the rise and broad adoption of blood purification strategies with enhanced convective properties, such as high-volume online hemodiafiltration and expanded hemodialysis, considerably amplified the ability to mechanically extract middle molecules (molecular weight #x3e;0.5 kDa) from the blood compartment. Nonetheless, the classification of uremic toxins has not evolved in parallel with dialysis advancements. Mounting evidence demonstrates the link between middle molecules with uremic symptoms, cardiovascular and mortality risks. An urgent need for updating the classification exists. Defining the causative relationship between specific solutes and specific clinical outcomes will promote the development of targeted therapies. In parallel, the inclusion of new pertinent dimensions to the classification like the influence of new dialysis membranes, sorbents, and intestinal chelators in the concentration of uremic toxins would improve the understanding of the pathogenesis of chronic kidney disease, setting the pace for future research in nephrology. Fluid Shear Stress-Induced Changes in Megalin Trafficking Enhance Endocytic Capacity in Proximal Tubule Cells

Abstract:Proximal tubule (PT) cells maintain a high-capacity apical endocytic pathway to recover essentially all proteins that escape the glomerular filtration barrier. The multiligand receptors megalin and cubilin play pivotal roles in the endocytic uptake of normally filtered proteins in PT cells but also contribute to the uptake of nephrotoxic drugs, including aminoglycosides. We previously demonstrated that opossum kidney (OK) cells cultured under continuous fluid shear stress (FSS) are superior to cells cultured under static conditions in recapitulating essential functional properties of PT cells . To identify drivers of the high-capacity, efficient endocytic pathway in the PT, we compared FSS-cultured OK cells with less endocytically active static-cultured OK cells. Megalin and cubilin expression are increased, and endocytic uptake of albumin in FSS-cultured cells is >5-fold higher compared with cells cultured under static conditions. To understand how differences in receptor expression, distribution, and trafficking rates contribute to increased uptake, we used biochemical, morphological, and mathematical modeling approaches to compare megalin traffic in FSS-versus static-cultured OK cells. Our model predicts that culturing cells under FSS increases the rates of all steps in megalin trafficking. Importantly, the model explains why, despite seemingly counterintuitive observations (a reduced fraction of megalin at the cell surface, higher colocalization with lysosomes, and a shorter half-life of surface-tagged megalin in FSS-cultured cells), uptake of albumin is dramatically increased compared with static-grown cells. We also show that FSS-cultured OK cells more accurately exhibit the mechanisms that mediate uptake of nephrotoxic drugs compared with static-grown cells. This culture model thus provides a useful platform to understand drug uptake mechanisms, with implications for developing interventions in nephrotoxic injury prevention.

Adsorption of suppressor cell activity on mouse macrophage monolayers.

Abstract:Suppressor cell activity is high in the spleen of C57BL/6 mice sensitized to the allogeneic P-815 mastocytoma. The suppressor cells inhibit proliferation of normal mouse spleen cells in mixed lymphocyte cultures. Preincubation of tumor-allosensitized spleen cells on monolayers of syngeneic or allogeneic macrophages reduces suppressor activity. Preincubation on monolayers of spleen cells does not affect suppressor activity. Suppressor cells can be retrieved from the macrophage monolayers used for adsorption. Heat-killed macrophages fail to adsorb out suppressor cell activity. Incubation of tumor-allosensitized spleen cells on syngeneic macrophage monolayers does not reduce cytotoxic cell activity. Removal of suppressor cell activity by macrophage adsorption restores helper activity of tumor-allosensitized spleen cells. These results demonstrate that suppressor cell activity can be reduced by contact with macrophages and point to an additional role for the macrophage in regulating the immune response to tumors.
Observation of Serum Albumin Distribution on Renal Proximal Tubules under Various Hemodynamic Conditions in Mice by "in Vivo Cryotechnique"

Abstract:Objective To visualize the topographical changes of serum albumin passing through mouse glomerular capillary loops and its reabsorption renal proximal tubules by immunohistochemistry combination with in vivo cryotechnique.Methods The in vivo cryotechnique was used to treat the left kidneys of mice under normotensive,experimentally acute hypertensive and heart-arrest conditions. The cryofixed tissues were routinely processed for freeze-substitution. Serial deparaffinized sections were stained with hematoxylin-eosine and immunostained with anti-mouse albumin antibody. Results Under the normotensive and heart-arrest conditions,albumin was clearly immunolocalized the blood vessels and slightly the apical cytoplasmic parts of some proximal tubules. Under the acute hypertensive condition,the albumin was strongly immunolocalized the apical cytoplasm of almost all proximal tubules.Conclusion The in vivo cryotechnique combined with freeze-substitution can transiently capture the reabsorption of albumin living functional renal proximal tubules and can also visualize the functional morphology living animal tissues and cells situ.
Upscaling of a living membrane for bioartificial kidney device

Abstract:The limited removal of metabolic waste products in dialyzed kidney patients leads to high morbidity and mortality. One powerful solution for a more complete removal of those metabolites might be offered by a bioartificial kidney device (BAK), which contains a hybrid "living membrane" with functional proximal tubule epithelial cells (PTEC). These cells are supported by an artificial functionalized hollow fiber membrane (HFM) and are able to actively remove the waste products. In our earlier studies, conditionally immortalized human PTEC (ciPTEC) showed to express functional organic cationic transporter 2 (OCT2) when seeded on small size flat or hollow fiber polyethersulfone (PES) membranes. Here, an upscaled "living membrane" is presented. We developed and assessed the functionality of modules containing three commercially available MicroPES HFM supporting ciPTEC. The HFM were optimally coated with L-Dopa and collagen IV to support a uniform and tight monolayer formation of matured ciPTEC under static culturing conditions. Both abundant expression of zonula occludens-1 (ZO-1) protein and limited diffusion of FITC-inulin confirm a clear barrier function of the monolayer. Furthermore, the uptake of 4-(4-(dimethylamino)styryl)-N-methylpyridinium iodide (ASP+), a fluorescent OCT2 substrate, was studied in absence and presence of known OCT inhibitors, such as cimetidine and a cationic uremic solutes mixture. The ASP+ uptake by the living upscaled membrane was decreased by 60% in the presence of either inhibitor, proving the active function of OCT2. In conclusion, this study presents a successful upscaling of a living membrane with active organic cation transport as a support for BAK device.

Despite current limitations, on-line kilo-voltage cone-beam computed tomography (CBCT) also has the potential to identify changes in target volume geometry and the location of important organs at risk. These capabilities could be enhanced by further technical improvements in CBCT, including image quality.

Abstract:The human gut microbiota constitutes a complex community of microorganisms residing within the gastrointestinal tract, encompassing a vast array of species that play crucial roles in health and disease. The disease processes involved in chronic kidney disease (CKD) and end-stage kidney disease (ESKD) are now increasingly established to result in dysregulation of gut microbiota composition and function. Gut microbiota dysbiosis has been associated with poor clinical outcomes and all-cause mortality in patients with ESKD, particularly individuals receiving dialysis. Prior studies highlighted various factors that affect gut microbiota dysbiosis in CKD and ESKD. These include, but are not limited to, uraemic toxin accumulation, chronic inflammation, immune dysfunction, medications, and dietary restrictions and nutritional status. There is a lack of studies at present that focus on the evaluation of gut microbiota dysbiosis in the context of dialysis. Knowledge on gut microbiota changes in this context is important for determining their impact on dialysis-specific and overall outcomes for this patient cohort. More importantly, evaluating gut microbiota composition can provide information into potential targets for therapeutic intervention. Identification of specific microbial signatures may result in further development of personalised treatments to improve patient outcomes and mitigate complications during dialysis. Optimising gut microbiota through various therapeutic approaches, including dietary adjustments, probiotics, prebiotics, medications, and faecal transplantation, have previously demonstrated potential in multiple medical conditions. It remains to be seen whether these therapeutic approaches are effective within the dialysis setting. Our review aims to evaluate evidence relating to alterations in the gut microbiota of patients undergoing dialysis. A growing body of evidence pointing to the complex yet significant relationship which surrounds gut microbiota and kidney health emphasises the importance of gut microbial balance to improve outcomes for individuals receiving dialysis.

Abstract:The p53 binding protein 2 (53BP2) has been identified as the interacting protein to p53, Bcl‐2, and p65 subunit of nuclear factor κB (NF‐κB). The TP53BP2 gene encodes two splicing variants, 53BP2S and 53BP2L, previously known as apoptosis stimulating protein 2 of p53 (ASPP2). We found that these 53BP2 proteins are located predominantly in the cytoplasm and induce apoptosis as demonstrated by cleavage of poly ADP ribose polymerase (PARP) and annexin V staining. Furthermore, we demonstrate that 53BP2 is located in the mitochondria and induces apoptosis associated with depression of the mitochondrial trans‐membrane potential (ΔΨm) and activation of caspase‐9. From these findings we conclude that 53BP2 induces apoptosis through the mitochondrial death pathway.
The bioartificial kidney and bioengineered membranes in acute kidney injury.

Abstract:The treatment of severe acute kidney injury (AKI) with dialysis or hemofiltration remains suboptimal with high levels of morbidity and mortality. Current renal replacement therapies substitute for the small solute clearance function of the kidney but do not replace the lost reclamation, metabolic and endocrine functions of this organ. Cell therapy and tissue engineering offer hope of fuller replacement of kidney function in renal failure patients. A renal tubule assist device (RAD) that includes a conventional hemodialysis filter and a bioreactor containing living renal proximal tubule cells has been successfully engineered. Differentiated activity of these cells and survival advantages have been demonstrated in large-animal models of sepsis and AKI. Data from phase I/II and phase II clinical studies have shown that the addition of renal tubule cell therapy to conventional continuous renal replacement therapy (CRRT) treatment resulted in a significant clinical impact on survival, and that RAD treatment demonstrated an acceptable safety profile. Another substantive advance for the treatment of AKI will be the development of nanofabrication technology to further improve the clearance function of the kidney to replicate glomerular permselectivity while retaining high rates of hydraulic permeability. New developments in this translational research area will improve the unmet medical needs of patients with renal failure. Copyright (c) 2008 S. Karger AG, Basel.

Abstract:ABSTRACT In most biological or industrial (including medical) separation processes, a membrane is a semipermeable barrier that allows or achieves selective transport between given compartments. In haemodialysis (HD), the semipermeable membrane is in a tubular geometry in the form of miniscule pipes (hollow fibres) and separation processes between compartments involve a complex array of scientific principles and factors that influence the quality of therapy a patient receives. Several conditions need to be met to accomplish the selective and desired removal of substances from blood in the inner cavity (lumen) of the hollow fibres and across the membrane wall into the larger open space surrounding each fibre. Current HD membranes have evolved and improved beyond measure from the experimental membranes available in the early developmental periods of dialysis. Today, the key functional determinants of dialysis membranes have been identified both in terms of their potential to remove uraemic retention solutes (termed ‘uraemic toxins’) as well subsidiary criteria they must additionally fulfill to avoid undesirable patient reactions or to ensure safety. The production of hundreds of millions of kilometres of hollow fibre membranes is truly a technological achievement to marvel, particularly in ensuring that the fibre dimensions of wall thickness and inner lumen diameter and controlled porosity—all so vital to core solute removal and detoxification functions of dialysis—are maintained for every centimetre length of the fragile fibres. Production of membranes will increase in parallel with the increase in the number of chronic kidney disease (CKD) patients expected to require HD therapies in the future. The provision of high-quality care entails detailed consideration of all aspects of dialysis membranes, as quality cannot in any way be compromised for the life-sustaining—like the natural membranes within all living organisms—function artificial dialysis membranes serve.

Abstract:Objectives: Cirrhosis affects all structures of the kidney, in particular the tubules which are responsible for secretion of protein-bound metabolites and electrolyte/water homeostasis. Yet, prevailing assessments of kidney function focus solely on glomerular filtration rate (GFR) which may incompletely reflect these processes. We sought to characterize markers of tubular function, injury, and viability in patients with and without cirrhosis. Methods: We recruited outpatients undergoing liver transplantation evaluation for a collection of plasma and 24-hour urine, matching by GFR to control participants without cirrhosis. We measured urinary kidney injury molecule-1 (KIM-1), a marker of proximal tubular injury, as well as epidermal growth factor (EGF), a marker of viability necessary for tubular epithelial cell proliferation after injury. We also estimated secretory clearance by measuring several highly secreted endogenous metabolites in urine and plasma. Results: We recruited 39 patients with cirrhosis (mean MELD-Na 17±4, Child-Pugh 8±2, eGFR 66±20 ml/min/1.73m 2 ) and 58 GFR-matched controls without cirrhosis (eGFR 66±21 ml/min/1.73m 2 ). Urinary KIM-1 was 4.4-fold higher than controls (95% CI:2.9-6.5), and EGF averaged 7.41-fold higher than controls (95% CI:2.15-25.53). We found that of 8 solutes, 5 had significantly greater kidney clearance in cirrhosis (1.3-2.1-fold higher): indoxyl sulfate, p-cresol sulfate, pyridoxic acid, tiglylglycine, and xanthosine. Conclusions: Cirrhosis was characterized by molecular signs of tubular injury in stable outpatients without acute kidney injury, accompanied by largely preserved tubular secretory clearance and greater signs of tubular viability. Within the limitations of the study, this suggests a phenotype of chronic ischemic injury but with initial preservation of tubular function in cirrhosis.

A Selective Cytopheretic Inhibitory Device to Treat the Immunological Dysregulation of Acute and Chronic Renal Failure.

Abstract:BACKGROUND:The poor outcomes in patients with acute kidney injury (AKI) and end-stage renal disease (ESRD) on chronic dialysis are due to immune dysregulation associated with these disorders. Evolving evidence suggests that the kidney, and specifically renal epithelial cells, plays an important role in the immunological response of leukocytes under disease states.METHOD:In this regard, the development of two therapeutic approaches utilizing renal epithelial cells and 'smart' immunomodulatory membranes has been tested in preclinical animal models and clinical trials.RESULTS:These two approaches have been demonstrated in phase II human trials to improve the survival of intensive care unit patients with AKI and multiorgan failure. The use of a 'smart' immunomodulatory membrane is also being evaluated in a small exploratory clinical trial to assess its effects on immunoregulation in ESRD patients requiring chronic hemodialysis.CONCLUSIONS:The use of renal progenitor/stem cell therapy and/or cytopheretic membranes may result in effective treatments to alter the dysregulated immunological state of acute or chronic renal failure and improve the outcomes of these diseases.
CLINICO-MICROBIOLOGICAL SPECTRUM OF HEMODIALYSIS CATHETER-RELATED BLOOD STREAM INFECTIONS: A CONCISE REVIEW

Abstract:The disease burden of chronic kidney disease is increasing worldwide, and with the growing population of patients requiring hemodialysis, more patients are placed on central venous catheters (CVCs). Vascular access infections significantly cause hospitalization and mortality among end-stage renal disease (ESRD) patients. As per the data from the United States, Gram-positive organisms account for almost 56% of the total cases of catheter-related bloodstream infection (CRBSI), in stark contrast to Indian studies showing a dominance of Gram-negative organisms. The prolonged use of non-tunnelled catheters, poor hygiene, lack of catheter care protocols, and the tropical climate pose unique challenges to control CRBSIs in resource-poor countries. The most effective strategy to prevent CRBSI is to minimize the duration of dialysis catheters. Other strategies include the introduction of strict catheter care protocols, using tunnelled cuffed catheters, anti-microbial lock solutions, and topical antimicrobials. Eradicating nasal carriage of Staphylococcus aureus also reduced the incidence of CRBSIs.Treatment of suspected CRBSI should be promptly initiated with empirical broad-spectrum antibiotics covering Gram-positive and Gram-negative organisms. Catheter removal is indicated only if the patient is clinically unstable, has metastatic infections, and those who do not respond to antibiotic therapy.
Effect of caffeine on intravenous adenosine-induced hyperemia in fractional flow reserve measurement.

Abstract:BACKGROUND The interaction between caffeine and adenosine is still a matter of debate. AIMS We examined whether caffeine attenuated intravenous adenosine-induced hyperemia in the measurement of fractional flow reserve (FFR) and whether an increased dose of adenosine overcame the caffeine antagonism. METHODS FFR was measured using different adenosine doses (140, 175, and 210 μg/kg/min) and papaverine as a reference standard in patients with intermediate coronary stenoses, who refrained from caffeine for >24 h (no-caffeine group; n = 14) and those who consumed caffeine (caffeine group; n = 28). RESULTS The median caffeine level in the caffeine group was 2.9 mg/L (interquartile range, 1.8-4.6 mg/L). In the no-caffeine group, FFR with adenosine did not decrease above the dose of 140 μg/kg/min (0.769, 0.771, and 0.770 at 140, 175, and 210 μg/kg/min, respectively) and was not significantly different from that with papaverine (0.765). In the caffeine group, adenosine overestimated FFR (140 μg/kg/min: 0.813, P<.001; 175 μg/kg/min: 0.806, P<.01; 210 μg/kg/min: 0.794, P=.01) compared with papaverine (0.779). The difference in FFR between papaverine and 140 μg/kg/min dose of adenosine was significantly greater in the caffeine group than in the no-caffeine group (0.034 vs 0.004; P<.05). CONCLUSION Caffeine attenuates intravenous adenosine-induced hyperemia in FFR measurement. Even increased adenosine doses up to 210 μg/kg/min cannot fully surmount the antagonism.

Abstract:This review highlights the complex membrane architectures and organelles observed along the renal tubular segments through careful review of ultrastructural and physiological studies published over the past several decades. We also showcase the vital role(s) played by the actin cytoskeleton and actin associated myosin motor proteins in regulating cell type-specific physiological functions within cells of the renal epithelium. The purpose of this review is to provide a fresh conceptual framework to explain the structure-function relationships that exist between the actin cytoskeleton, organelle structure, and cargo transport within the mammalian kidney. We believe that with recent advances in technologies to visualize the actin cytoskeleton and associated proteins within intact kidneys, it is imperative to reimagine the functional role(s) for these proteins in situ, which will provide a rationale for their unique, cell type specific function(s), necessary to build and maintain complex physiological processes.

Abstract:A critical barrier in the nephrology field is the lack of appropriate in vitro renal tubule models that allow manipulation of various mechanical factors, facilitating studies of disease pathophysiology and drug discovery. Here we report development of a novel in vitro assay system comprised of a renal tubule within an elasto-plastic extracellular matrix microenvironment. This in vitro tubule mimetic device consists of a container with two, pipette-accessible ports, filament-deposition (3D-) printed into 35 mm cell culture dishes. The container is filled with a hydrogel, such as a collagen I or fibrin gel, while a narrow masking tube is threaded through the ports. Following gelation, the masking material is pulled out leaving a tunnel within the gel. Seeding of the tunnels with M1 or MDCK renal epithelial cells through the side ports results in a monolayer with apical-basal polarity, such that laminin and fibronectin are present on the basal surface, while primary cilia project from the apical side of cells into the tubular lumen. The device is optically accessible, and can be live-imaged by phase contrast or epifluorescence microscopy. The lumen of the epithelial-lined tube can be connected through the side ports to a circulatory flow. We demonstrate that kidney epithelial cells are able to adjust the diameter of the model tubule by myosin-II dependent contractility. Furthermore, cells of the tubule are also able to remodel the surrounding hydrogel leading to budding from the main tubule. We propose that this versatile in vitro model system can be developed into a future pre-clinical tool to study pathophysiology of kidney diseases and identify therapeutic compounds.
