Abstract:Purpose Radiofrequency ablation along the posterior wall of the left atrium may lead to atrioesophageal fistula due to esophageal thermal injury. The purpose of our study was to prospectively investigate whether ablation guided by soluble contrast esophageal visualization (SCEV) reduces injury during atrial fibrillation (AF) ablation. Methods Seventy-eight patients with paroxysmal AF undergoing circumferential pulmonary vein isolation (PVI) were randomized to a SCEV group ( n = 39) and control group without visualization ( n = 39). Cine imaging of the esophagus was performed during soluble contrast swallowing at the beginning of ablation, after adjacent ipsilateral PVI and at the end of the procedure. The ablation lesion set was modified to avoid radiofrequency delivery within the contrast esophagram boundaries. In the control group, a single final ingestion was performed at the end of the procedure. Esophageal injury was assessed by esophagogastroscopy within 24 h in all patients. Results In the control group, the ablation lesion crossed over the esophagus in 46.2% of patients, whereas in SCEV group, the ablation line violated the boundaries of the esophagus unavoidably in 15.4% of patients (confidence interval (CI); 1.61–13.98, p = 0.003). The incidence of esophageal injury was significantly lower in patients that underwent ablation with SCEV (5.1% vs. 20.5%, CI; 0.04–1.06, p = 0.042). Regardless of randomization group, patients who received ablation which overlapped the esophagus had a higher incidence of esophageal injury compared with those without overlap (37.5 vs. 1.9%, CI; 3.73–271.37, p = 0.000). Conclusions Esophageal contrast visualization helps to reduce the potential for esophageal injury during paroxysmal AF ablation. This simple procedural adjunct has important implications to improve safety of paroxysmal AF ablation procedures globally.

Abstract:The feasibility and safety of intracardiac echocardiography (ICE)-guided catheter ablation for atrial fibrillation (AF) using a minimal/zero-fluoroscopy approach have recently been reported. This approach helps to reduce ionizing radiation exposure and orthopedic complications resulting from using lead aprons. The objectives of this planned prospective, multicenter randomized controlled trial (RCT) (paroxysmal AF (PAF)-ICE trial; ChiCTR2000033624) are to evaluate the efficacy and safety of ICE-guided minimal-fluoroscopy ablation in patients with PAF and the impact on occupational hazards among lab staff.Patients will be randomized in a 1:1 ratio to 2 groups: minimal fluoroscopy group ( n = 216) and traditional approach group ( n = 216). In the minimal fluoroscopy group, an ICE catheter will be used for geometry/anatomic construction, transseptal puncture, catheter tracking, and effusion monitoring. Pulmonary vein isolation (PVI) will be performed using an open-irrigated radiofrequency SmartTouch Surround Flow or SmartTouch catheter (Biosense Webster, Diamond Bar, California, USA), and confirmed by a multipolar Lasso or PentaRay catheter (Biosense Webster). In the traditional approach group, an ICE catheter will not be used. Transseptal puncture will be performed under fluoroscopic guidance, with all geometries constructed by mapping the catheters. The primary efficacy endpoint is freedom from AF recurrence (without antiarrhythmic medications) at 12 months after ablation. Other endpoints include duration of lead apron use, measures of intra-procedural efficiency, and peri-procedural complications. This RCT will evaluate the efficacy and safety of ICE-guided minimal-fluoroscopy ablation in patients with PAF, also evaluate the benefits to lab staff (regarding reducing occupational hazards) related to this "minimal/zero-fluoroscopy" and "leadless" mode.

Abstract:Introduction: Pulsed-field ablation (PFA) and fluoroless ablation (FA) are emerging techniques in contemporary in electrophysiology. With widespread use of 3D electroanatomic mapping systems and advanced intracardiac echo (ICE) imaging, fluoroless ablation has become more widely adopted. However, with the importance of tissue contact for lesion durability, initial PFA has been used with fluoroscopic guidance, but both ICE and electroanatomic mapping make fluoroless PFA feasible. The objective of this study is to demonstrate that PFA can be done safely and effectively without fluoroscopy. Methods: At a single center, consecutive patients undergoing ablation with a pentaspline PFA catheter using a fluoroless approach are described. The standard 3D anatomic map settings were adjusted with changes in interior and exterior projection, respiratory compensation, and interpolation. In addition, projection map lesions were used to confirm adequate circumferential ablation lesions. ICE was used extensively for wire guidance and evaluation of contact with tissue. Results: Beginning on March 15, 2024, 50 consecutive subjects (19 female/31 male) aged 68.0 (± 13.7) underwent PFA ablation. The average CHA2DS2-VA2Sc score was 3.0 (± 1.9). The average LVEF was 57.3% (± 10.0) and the average LA size was 3.9 cm (± 1.2). Projection lesions were placed with every application of PFA. An average of 41.7 (± 8.5) PFA applications were placed. In 100% (50/50) of subjects, acute isolation of the pulmonary veins was achieved. Eighteen subjects also underwent concomitant posterior wall isolation and in 100% of these subjects, posterior isolation was achieved. There were zero complications in this cohort. In 50/50 subjects (100%), fluoroscopy was not used. In comparison to the control cohort, the LA dwell time of the ablation catheter was similar (p = 0.34). Conclusion: In comparison to the traditional PFA with fluoroscopy, this proof-of-concept study shows fluoroless PFA ablation can be performed safely and with similar acute success rates as with use of fluoroscopy.

Abstract:Introduction Radiofrequency (RF) ablation of parahisian accessory pathways (PHAPs) is a serious challenge, which is associated with high recurrence and fluoroscopy exposure. According to previous studies and case reports, catheter ablation of PHAPs is associated with a high post-ablation recurrence. The aim of this study is to assess the efficacy, safety and long-term outcome of radiofrequency ablation in patients with recurrent PHAPs after prior ablations with zero or extremely low fluoroscopy. Methods and Results A total of 58 patients (43 male, 75.4%) with PHAPs that had one or more previously unsuccessful RF ablations were enrolled. RF ablation procedures were performed under a three-dimensional guiding system (EnSite NavX, St. Jude Medical, MN) with extremely low exposure of fluoroscopy to femoral venous or arterial routes. The acute success rate was 96.6% (56/58). Two procedures were terminated during ablation because of high risk of complete AVB. In addition to a case of transient complete AVB during RF energy delivery, no minor or major complications were recorded during the procedure or long-term follow up. The mean fluoroscopy time was 98 seconds (0-129), and the mean radiation dose was 1.98 mGy (0-3.38) in all 58 cases. Fourteen patients underwent the EPS and RF ablation with zero X-ray exposure. The long-term success rate was 98.2% with a median follow-up period of 30 (12-45) months. Conclusions With a near zero X-ray exposure, RF ablation of recurrent PHAPs can be performed with a high acute success rate and convincing safety during long-term follow-up.

Abstract:Aims: Atrial fibrillation (AF) ablation and left atrial appendage occlusion (LAAO) are increasingly performed as individual procedures. Pulsed field ablation (PFA) has significantly reduced procedure duration and may be advantageous for the combined approach. Methods and results: We have launched a programme for simultaneous AF ablation using PFA and LAAO for patients qualifying for both treatments and excluding those with a complex anatomy. We compare procedure duration and fluoroscopy time against individual procedures (either AF ablation or LAAO alone), all performed by the same operators and using consistent technologies. We performed the combined procedure in 10 patients (50% males; median age 70 years) and excluded 2 patients (17%) because of a complex left atrial appendage anatomy. No death, stroke, or major bleeding events, including pericardial effusion, occurred. For single-procedure comparison, 207 AF ablation procedures and 61 LAAO procedures were available. The total median procedure duration was 79 min (range 60-125) for the combined procedure, 71 min (25-241) for individual AF ablation (51 min without and 78 min with 3-dimensional electroanatomic mapping), and 47 min (15-162) for individual LAAO. The respective fluoroscopy times were 21 (15-26), 15 (5-44), and 10 (3-50) min. For the combined procedure, femoral vein access to last PFA application lasted 49 min (34-93) and LAAO added 20 min (15-37). Conclusion: Simultaneous PFA-based AF ablation and LAAO in carefully selected patients is feasible and safe and can be executed within a short overall procedure duration.

Abstract:Background: The combination of highly localized impedance (LI) and contact force (CF) may improve tissue characterization and lesion prediction during radiofrequency (RF) pulmonary vein isolation (PVI) in patients with atrial fibrillation (AF). Objective: We report the outcomes of our acute and long-term clinical evaluation of CF-LI-guided PVI in consecutive AF ablation cases from an international multicenter clinical setting. Methods: Three hundred twenty-four consecutive patients from 20 European centers undergoing RF catheter ablation with the Stablepoint™ catheter were enrolled in the CHARISMA registry. Of these, 275 had a minimum follow-up of 1 year and were included in the primary analysis. Results: The mean procedure duration was 115 ± 47 min, and the mean fluoroscopy time was 9.9 ± 6 min. At the end of the procedures, all PVs had been successfully isolated in all study patients. Minor complications were reported in 12 patients (4.4%). At 1 year, 36 (13.1%) patients had had an AF recurrence, and freedom from antiarrhythmic drugs and AF recurrence was achieved in 228 (82.9%) patients. The recurrence rate was higher in patients with persistent AF (21/116, 18.1%) than in those with paroxysmal AF (15/159, 9.4%; p = 0.0459). On multivariate logistic analysis adjusted for baseline confounders, only time > 6 months from first diagnosis of AF to ablation (HR = 2.93, 95%CI 1.03 to 8.36, p = 0.0459) was independently associated with recurrences. Conclusion: An ablation strategy for PVI guided by CF-LI technology proved safe and effective and resulted in a low recurrence rate of AF over 1-year follow-up, irrespective of the underlying AF type. Abstract:Background Transseptal puncture is one of the most challenging steps in catheter ablation of atrial fibrillation (AF) because of the potential risk of complication. While single TSP reduces the puncture-related risk, double TSP simplifies the procedure (immediate signal visualization in pulmonary veins, avoidance of multiple changes of catheters through the single sheath). The purpose of this study was to analyze the impact of single versus double TSP during pulmonary vein isolation (PVI) with radiofrequency (RF) catheter ablation for AF on the fluoroscopy usage and procedure time. Methods Data reported from the international, multicenter prospective "Go For Zero Fluoroscopy Registry" performed in years 2018–2019 were analyzed. Overall 25 European electrophysiology centers from 14 countries and up to 5 operators from each center were enrolled. Only isolated PVI procedures were analyzed. Results A total data of 393 PVI RF procedures were collected from 25 centers. The median dose-area-product (DAP), fluoroscopy time and overall procedure time were: 810 (325 – 1839) cGy*cm2, 8 (4 – 14) minutes and 124 (92 – 168) minutes, respectively. Single TSP was associated with significantly higher DAP: 984 (459 – 1939) vs 667 (227 – 1469) cGy*cm2 (p=0.002), longer fluoroscopy time: 11 (6 – 19) vs 6 (3 – 11) minutes (p 0.05). However, single TSP was performed by operators with longer EP lab experience: 10 (4 – 15) vs 3 (0 – 10) years (p<0,001). Conclusions Double TSP is related with lower fluoroscopy usage and shorter overall procedure time of pulmonary vein isolation with radiofrequency catheter ablation for atrial fibrillation.

Abstract:Atrial fibrillation (AF) is one of the most common cardiac arrhythmias, which is harmful to human health and quality of life. AADs are limited in clinical because of their side effects, so in recent years, RFCA has become one of the effective treatments for AF, whose clinical application is showing a clear upward trend. In order to explore the efficacy and safety of catheter ablation for AF, we have followed up the patients with AF undergoing catheter ablation in this study. A cohort of 39 patients with AF after RFCA from 2008 to 2011 was studied, 25 male and 14 female; the average age was 61.08±10.17 (36–74) years old. Among them, there were 32 cases with paroxysmal AF, and seven cases with persistent AF. 19 cases were complicated with hypertension, six cases with coronary heart disease, seven cases with diabetes and two cases with cerebral infarction. All the patients suffered from obvious clinical discomfortable symptoms. Although treated by one to two kinds of AADs already, the effects were poor. CPVI was performed in the all of them (five cases recurred, 13.51%). The endpoint of the ablation was complete electronic isolation of all the pulmonary veins (PVs). The patients who were converted to sinus rhythm in the cause of ablation were validated by Lasso electrodes, and could not be evoked atrial arrhythmia again. However, those who were failed to be converted to sinus rhythm would be validated after electroversion. For those who weren't isolated PVs completely, the conducted gap was initially posited by the mapping catheter, and the earliest fragmented potential was targeted for ablation till the complete electronic isolation of all the PVs. (1) The results of catheter ablation: 37 cases were successfully achieved immediately the endpoint after the ablation. Surgery average time of operation was (174.89±35.05) min, average X-ray exposure time (33.62±16.44) min, average discharge time (40.78±11.61) min; No severe complications such as pulmonary vein stenosis, pericardial tamponade, cardio-esophagus fistula, stroke occurred during and after the procedure. One case with paroxysmal AF was converted to auricular flutter after ablation, who were failed to be converted to sinus rhythm by synchronised cardioversion, but converted after Marshall Ligament ablation; three cases with persistent AF undergoing ablation were turned to be sinus rhythm by cardioversion; one case with persistent AF undergoing ablation was converted to sinus rhythm by intravenous irapbullitt. Five cases developed atrial tachyarrhythmias in 5–7 days after ablation: four cases appeared paroxysmal AF with fast ventricular rate; one case appeared paroxysmal atrial tachycardia; another one was sinus bradycardia, junctional escape. All the patients didn't suffer dysponea, cough, haemoptysis and so on before discharge, moreover distention of jugular vein, pulsus paradoxus and pericardial rub were not found through physical examination. No severe complication happened during and after the procedure, and all of them maintained sinus rhythm before discharge. (2) Follow-up: 37 cases were followed-up after 6–12 months, average 6±2 months, only one case lost communication and one case died of congestive heart failure. Five cases recurred: one case with paroxysmal AF who recurred after simple CPVI refused re-ablation, decreased episode frequency and took amiodarone to maintain sinus rhythm. One patient who performed CPVI and atria sinistrum isthmus ablation, three cases with paroxysmal AF appeared atrial arrhythmia after RFCA, but they refused re-ablation temporarily because of decreasing episode frequency. 36 cases took warfarin according to monitoring INR periodically, while one case took warfarin only 2 months after ablation because of hemorrhinia. Dysponea, cough related to pulmonary vein stenosis, and thromboembolism complications didn't occur for all of the cases. The LAD of 29 cases recovered normal size (LAD≤35 mm) on cardiac ultrasound 3 months after ablation. The lower the preoperative LVEF is, the more obvious the improvement is after RFCA. Catheter ablation is safe and effective for treating AF.

A cost-consequence analysis comparing three cardiac ablation strategies for the treatment of paroxysmal atrial fibrillation

Abstract:Background and aims: Cardiac ablation is a well-established method for treating atrial fibrillation (AF). Pulsed field ablation (PFA) is a non-thermal therapeutic alternative to radiofrequency ablation (RFA) and cryoballoon ablation (CRYO). PFA uses high-voltage electric pulses to target cells. The present analysis aims to quantify the costs, outcomes, and resources associated with these three ablation strategies for paroxysmal AF. Methods: Real-world clinical data were prospectively collected during index hospitalization by three European medical centers (Belgium, Germany, the Netherlands) specialized in cardiac ablation. These data included procedure times (pre-procedural, skin-to-skin and post-procedural), resource use, and staff burden. Data regarding complications associated with each of the three treatment options and redo procedures were extracted from the literature. Costs were collected from hospital economic formularies and published cost databases. A cost-consequence model from the hospital perspective was built to estimate the impact of the three treatment options in terms of effectiveness and costs. Results: Across the three centers, N = 91 patients were included over a period of 12 months. A significant difference was seen in pre-procedural time (mean ± SD, PFA: 13.6 ± 3.7 min, CRYO: 18.8 ± 6.6 min, RFA: 20.4 ± 6.4 min; p < .001). Procedural time (skin-to-skin) was also different across alternatives (PFA: 50.9 ± 22.4 min, CRYO: 74.5 ± 24.5 min, RFA: 140.2 ± 82.4 min; p < .0001). The model reported an overall cost of €216,535 per 100 patients treated with PFA, €301,510 per 100 patients treated with CRYO and €346,594 per 100 patients treated with RFA. Overall, the cumulative savings associated with PFA (excluding kit costs) were €850 and €1,301 per patient compared to CRYO and RFA, respectively. Conclusion: PFA demonstrated shorter procedure time compared to CRYO and RFA. Model estimates indicate that these time savings result in cost savings for hospitals and reduce outlay on redo procedures. Clinical practice in individual hospitals varies and may impact the ability to transfer the results of this analysis to other settings.
Cost, Efficiency and Outcomes of Pulsed Field Ablation versus Thermal Ablation for Atrial Fibrillation: A Real World Study

Abstract:Background With the exponential growth of catheter ablation (CA) for atrial fibrillation (AF), there is increasing interest in associated healthcare costs. Pulsed field ablation (PFA) using a single-shot pentaspline multi-electrode catheter has been shown to be safe and effective for AF CA, but its cost efficiency compared to conventional thermal ablation modalities (cryoballoon [CB] or radiofrequency [RF]) has not been evaluated. Objective To compare cost, efficiency, effectiveness and safety between PFA, CB and RF for AF ablation. Methods We studied 707 consecutive patients (208 PFA, 325 CB, 174 RF) undergoing first-time AF ablation. Individual procedural costs were calculated, including equipment, lab utilisation and hospital stay, and compared between ablation modalities, as effectiveness and safety. Results Skin-to-skin times and catheter lab times were significantly shorter with PFA (68min/102min) vs CB (91min/122min) and RF (89min/123 min); p<0.001. General anaesthesia utilisation differed across modalities (PFA 100%, CB 10.2%, RF 61.5%); p<0.001. Major complications occurred in 1% of cases, with no significant differences between modalities. Shorter procedural times resulted in lower staffing and lab costs with PFA, but these savings were offset by substantially higher equipment costs, resulting in higher overall median costs with PFA (£10,010) vs CB (£8,106) and RF (£8,949); p<0.001. Conclusion In this contemporary real-world study of the three major AF CA modalities used concurrently, PFA had shorter skin-to-skin and catheter lab times than CB and RF, with similarly low rates of complications. However, PFA procedures were considerably more expensive, largely due to higher equipment cost.

Abstract:Atrial fibrillation (AF) affects around 33 million people worldwide, rendering it a common cardiac arrhythmia. Catheter ablation (CA) has evolved as a leading therapeutic intervention for symptomatic AF. This umbrella review systematically evaluates existing systematic reviews and meta-analyses to assess the safety, efficacy, and potential of high-power, short-duration (HPSD) ablation as an alternative therapy option for AF. A thorough exploration was undertaken across PubMed, the Cochrane Library, and Embase to identify pertinent studies for inclusion in this umbrella review. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method was employed to assess the overall certainty of the evidence comprehensively, and the quality of the incorporated reviews was meticulously evaluated through use of the AMSTAR 2 tool, the Cochrane Collaboration tool, and the Newcastle-Ottawa scale. In this study, we initially identified 35 systematic reviews and meta-analyses, narrowing them down to a final selection of 11 studies, which collectively integrated data from 6 randomized controlled trials and 26 observational studies. For primary efficacy outcomes, the HPSD approach led to a non-significant decrease in the risk of atrial tachyarrhythmia recurrence (risk ratio [RR], 0.88; 95% confidence interval [CI], 0.70-1.12; I 2 = 90%; P = .31) and a significantly reduced risk of AF recurrence (RR, 0.53; 95% CI, 0.42-0.67; I 2 = 0%; P < .00001) compared to the low-power, long-duration (LPLD) approach. In terms of primary safety outcomes, the HPSD approach significantly reduced the risk of esophageal thermal injury (ETI) (RR, 0.71; 95% CI, 0.61-0.83; I 2 = 0%; P < .00001) and facilitated a non-significant decrease in the risk of other major complications (RR, 0.87; 95% CI, 0.73-1.03; I 2 = 0%; P = .10). In conclusion, HPSD therapy is safer and more effective than LPLD therapy, facilitating decreased AF recurrence rates along with reductions in ETI, total procedure duration, ablation number, ablation time, fluoroscopy time, and acute pulmonary vein reconnection.

Abstract:Background Since pulsed-field ablation (PFA) specifically targets myocardial tissue, it has become a cutting-edge substitute for radiofrequency (RF) and cryoablation. Purpose We aim to compare the safety and efficacy of PFA vs thermal ablation for AF ablation. Methods We conducted a systematic review and meta-analysis synthesizing randomized controlled trials (RCTs) and observational comparative studies, which were retrieved by systematically searching: PubMed, EMBASE, Web of Science, SCOPUS, and Cochrane through September 2023. RevMan version 5.4 software was used to pool dichotomous data using risk ratio (RR) and continuous data using mean difference (MD) with a 95% confidence interval (CI). Results Seventeen studies with a total of 2,255 patients were included. PFA was significantly associated with a decreased AF recurrence (RR: 0.65, 95% CI [0.51, 0.84], P= 0.0010) but not any arrhythmia recurrence (RR: 0.87, 95% CI [0.69, 1.09], P= 0.22). Also, PFA was significantly associated with decreased total procedure time (MD: -15.15, 95% CI [-20.23, -10.07], P< 0.00001) and heart rate (MD: -7.56, 95% CI [-12.78, -2.34], P= 0.005); however, there was no significant difference between PFA and thermal ablation in fluoroscopy time (MD: 2.83, 95% CI [-0.38, 6.04], P= 0.08). PFA decreased phrenic nerve palsy (RR: 0.38, 95% CI [0.15, 0.98], P= 0.05) and esophageal lesions (RR: 0.09, 95% CI [0.01, 0.69], P= 0.02); however, PFA was significantly associated with increased pericardial tamponade (RR: 6.14 with 95% CI [1.43, 26.33], P= 0.01). Conclusion PFA was significantly associated with decreased AF recurrence, heart rate, total procedure time, phrenic nerve palsy, esophageal lesion, and increased incidence of pericardial tamponade compared with thermal ablation. However, current evidence is mainly weighted by observational studies, requiring further large-scale randomized trials.Figure

Results: We included 17 studies with a total of 2255 patients. PFA was significantly associated with a decreased incidence of AF recurrence (RR: 0.66 with 95% CI [0.51, 0.87], p = .003). However, there was no significant difference between PFA and thermal ablation in arrhythmia recurrence (RR: 0.92 with 95% CI [0.74, 1.46], p = .42). PFA was significantly associated with decreased total procedure time (MD: -15.15 with 95% CI [-20.23, -10.07], p < .00001), decreased heart rate change (MD: -7.39 with 95% CI [-12.16, -2.62], p = .002), decreased phrenic nerve palsy (RR: 0.38 with 95% CI [0.15, 0.98], p = .05), and reduced esophageal lesions (RR: 0.09 with 95% CI [0.01, 0.69], p = .02). On the contrary, PFA was significantly associated with increased pericardial tamponade (RR: 6.14 with 95% CI [1.43, 26.33], p = .01). Abstract:Background: Radiofrequency ablation is a critical therapeutic method used in the management of atrial fibrillation (AF). This study systematically evaluates the effectiveness and safety of two catheter radio frequency ablation approaches: high-power short-duration (HPSD) and traditional low-power long-duration (LPLD), in treating AF. Methods: Four databases were searched for prospective studies (eight cohort studies and three randomised controlled trials) that evaluated the effect of HPSD treatment on AF recurrence, occurrence rate of complications and procedural time in patients with AF from the establishment of the databases to March 2023. We utilised RevMan 5.20 and Stata 11.0 statistical software to conduct a meta-analysis, and publication bias was assessed using funnel plots and Egger's test. The effect estimates were synthesised as relative risks (RRs) or standardised mean differences (SMDs) along with their corresponding 95% confidence intervals (CIs). Results: A total of 536 relevant studies were retrieved, and 11 prospective studies were collected. The combined value of the estimated effect of HPSD versus LPLD treatment on AF recurrence in patients with AF had an RR of 0.59 (95% CI: 0.45–0.78; p < 0.001), the effects of HPSD versus LPLD treatment on procedural time in patients with AF had an SMD of –1.17 (95% CI: –1.56– –0.77; p < 0.001), and the effect of HPSD versus LPLD treatment on oesophageal thermal injury in patients with AF had an RR of 0.84 (95% CI: 0.22–3.28; p = 0.80). Notably, the estimated combined effects of HPSD and LPLD on other major complications (steam pop) in patients with AF had an RR of 0.57 (95% CI: 0.22–1.47; p = 0.24). Conclusions: HPSD is more effective than traditional LPLD and has a lower AF recurrence rate after surgery. Meanwhile, HPSD treatment can improve surgical efficiency and has a shorter procedural time than LPLD treatment.

High-Frequency Low-Tidal Volume Ventilation Improves Long-Term Outcomes in AF Ablation: A Multicenter Prospective Study

Abstract:Background: High-frequency, low-tidal-volume (HFLTV) ventilation is a safe and simple strategy to improve catheter stability and first-pass isolation during pulmonary vein (PV) isolation. However, the impact of this technique on long-term clinical outcomes has not been determined. Objectives: This study sought to assess acute and long-term outcomes of HFLTV ventilation compared with standard ventilation (SV) during radiofrequency (RF) ablation of paroxysmal atrial fibrillation (PAF). Methods: In this prospective multicenter registry (REAL-AF), patients undergoing PAF ablation using either HFLTV or SV were included. The primary outcome was freedom from all-atrial arrhythmia at 12 months. Secondary outcomes included procedural characteristics, AF-related symptoms, and hospitalizations at 12 months. Results: A total of 661 patients were included. Compared with those in the SV group, patients in the HFLTV group had shorter procedural (66 [IQR: 51-88] minutes vs 80 [IQR: 61-110] minutes; P < 0.001), total RF (13.5 [IQR: 10-19] minutes vs 19.9 [IQR: 14.7-26.9] minutes; P < 0.001), and PV RF (11.1 [IQR: 8.8-14] minutes vs 15.3 [IQR: 12.4-20.4] minutes; P < 0.001) times. First-pass PV isolation was higher in the HFLTV group (66.6% vs 63.8%; P = 0.036). At 12 months, 185 of 216 (85.6%) in the HFLTV group were free from all-atrial arrhythmia, compared with 353 of 445 (79.3%) patients in the SV group (P = 0.041). HLTV was associated with a 6.3% absolute reduction in all-atrial arrhythmia recurrence, lower rate of AF-related symptoms (12.5% vs 18.9%; P = 0.046), and hospitalizations (1.4% vs 4.7%; P = 0.043). There was no significant difference in the rate of complications. Conclusions: HFLTV ventilation during catheter ablation of PAF improved freedom from all-atrial arrhythmia recurrence, AF-related symptoms, and AF-related hospitalizations with shorter procedural times.

Abstract:Abstract Introduction Radiofrequency (RF) ablation is performed in various arrhythmias. Still, arrhythmia recurrence is a challenging problem, not only in atrial fibrillation (AF). One reason for arrhythmia recurrence is suspected to be due to remaining gaps. Measurement of local impedance (LI) and change of LI during RF application can be used as a predictor and surrogate for successful ablation. Another tool, used for the last couple of years, estimating sufficient tissue contact is provided by catheters with real-time contact force (CF) measurement. The new INTELLANAV STABLEPOINTTM catheter by Boston Scientific was recently introduced combing both strategies. We sought to investigate the parallel measurement and correlation of LI and CF in real-time during left atrial (LA) and right atrial (RA) procedures. Methods We included the first n=20 patients who underwent LA ablation for AF or atypical atrial flutter and n=7 patients who underwent RA procedures for atrial flutter or atrial tachycardia and analysed retrospectively procedural data. For every RF delivery RF duration, CF, baseline LI, minimal LI, LI drop, as well as minimal LI and LI drop after 5 and 10 sec was documented. Only RF deliveries with stable contact (ablation without delay) were included. We then correlated baseline LI to LI drop (maximum, after 5 sec, after 10 sec) and to the documented CF at baseline. Results A total of 27 procedures was analysed. In total, 777 RF deliveries (663 in LA, 114 in RA) were documented with a mean RF duration of 21.7 sec. Mean baseline LI was 148.4 Ω, minimal LI 130.6Ω, LI after 5 sec 135.0 Ω and LI after 10sec 132.5 Ω. Mean complete LI drop was 17.8 Ω (13.4 Ω after 5 sec, 15.8 Ω after 10 sec). Mean CF was 15.5g. We could see a significant positive correlation between average CF and LI drop (p<0.01) (figure 1A), as well as between average CF and LI drop after 5 sec (p<0.01) and after 10 sec (p<0.01). Also, for baseline LI we could see a significant strong positive correlation to LI drop (p<0.001) (figure 1B), LI drop after 5 sec (p<0.001) and after 10 sec (p<0.001). Baseline LI did not correlate to average CF. Baseline LI was significantly higher in RA, when compared to LA (152.5Ω vs. 147.7Ω; p<0.05). LI drop did not differ between RA and LA procedures (17.8Ω vs. 17.7Ω), whereas mean RF duration per RF application was significantly longer in LA compared to RA procedures (22.8sec vs. 15.3sec; p<0.001) and applied CF was also higher in LA compared to RA procedures (16.0Ω vs. 12.8Ω; p<0.001). Conclusion Parallel real-time measurement of LI and CF is providing further insights into ablation biophysics. Based on these results the CF values might provide a prediction of subsequent lesion formation. A combination with LI measurement seems to be useful to prevent insufficient lesions, predisposing to gap formation and associated with arrhythmia recurrence, but also to potentially guarantee a higher safety by visualizing the applied CF and consecutive LI drop. This study aimed to evaluate the impact of contact force (CF) ablation with the porous tip on complications (congestive heart failure [CHF] and non‐CHF related), healthcare resource utilization, and procedural efficiency in patients undergoing de novo paroxysmal atrial fibrillation (PAF) ablations in a real‐world setting. Methods Consecutive de novo PAF ablations were performed between February 2014 and March 2019 by six operators at a single US academic center. The 6‐hole design was used through December 2016 with the 56‐hole porous tip adopted in October 2016. The outcomes of interest included symptomatic CHF presentation and CHF‐related complications. Abstract:Abstract Background/Introduction Pulmonary vein isolation (PVI) using cryoballoon (CB) ablation is associated with an increased radiation exposure compared to radiofrequency ablation. Previous studies showed that radiation exposure in CB PVI can be reduced by optimizing the fluoroscopy protocol without comprising acute efficacy and safety. Purpose We evaluated the mid-term outcome of an optimized fluoroscopy protocol in patients undergoing CB PVI. Methods The study population comprised 90 consecutive patients who underwent second generation CB-based PVI. The first 46 patients underwent CB PVI with conventional fluoroscopy settings (Group A). In the following 44 patients (Group B) an optimized fluoroscopy protocol was applied consisting of 1) using fluoroscopy instead of filming for determining the pulmonary vein occlusion grade; and 2) optimal collimation of the area of interest. Primary endpoints were the total dose area product (DAP), fluoroscopy time and freedom from documented recurrence of atrial fibrillation (AF) after a single procedure. Results Group B had a lower median DAP (1393 cGycm2 vs. 3232 cGycm2, P < 0.001) and median fluoroscopy time (20 min vs. 24 min, P < 0.001) as compared to group A. The 1-year freedom from documented recurrence of AF after a single procedure was similar among groups (74% in Group A vs. 77% in Group B, P = 0.71). There were no significant differences between both groups for the secondary endpoints, including procedure duration, proportion of patients with complete electrical isolation, and complications. Conclusion Using an optimized fluoroscopy protocol in CB PVI radiation exposure could be reduced with a similar 1-year clinical outcome.
