Abstract:ObjectivesThis study was designed to compare the detection of subtle lesions (calcification clusters or masses) when using the combination of digital breast tomosynthesis (DBT) and synthetic mammography (SM) with digital mammography (DM) alone or combined with DBT.MethodsA set of 166 cases without cancer was acquired on a DBT mammography system. Realistic subtle calcification clusters and masses in the DM images and DBT planes were digitally inserted into 104 of the acquired cases. Three study arms were created: DM alone, DM with DBT and SM with DBT. Five mammographic readers located the centre of any lesion within the images that should be recalled for further investigation and graded their suspiciousness. A JAFROC figure of merit (FoM) and lesion detection fraction (LDF) were calculated for each study arm. The visibility of the lesions in the DBT images was compared with SM and DM images.ResultsFor calcification clusters, there were no significant differences (p > 0.075) in FoM or LDF. For masses, the FoM and LDF were significantly improved in the arms using DBT compared to DM alone (p < 0.001). On average, both calcification clusters and masses were more visible on DBT than on DM and SM images.ConclusionsThis study demonstrated that masses were detected better with DBT than with DM alone and there was no significant difference (p = 0.075) in LDF between DM&DBT and SM&DBT for calcifications clusters. Our results support previous studies that it may be acceptable to not acquire digital mammography alongside tomosynthesis for subtle calcification clusters and ill-defined masses.Key Points• The detection of masses was significantly better using DBT than with digital mammography alone.• The detection of calcification clusters was not significantly different between digital mammography and synthetic 2D images combined with tomosynthesis.• Our results support previous studies that it may be acceptable to not acquire digital mammography alongside tomosynthesis for subtle calcification clusters and ill-defined masses for the imaging technology used.

Abstract:A three-dimensional (3D) linear model for digital breast tomosynthesis (DBT) was developed to investigate the effects of different imaging system parameters on the reconstructed image quality. In the present work, experimental validation of the model was performed on a prototype DBT system equipped with an amorphous selenium (a-Se) digital mammography detector and filtered back-projection (FBP) reconstruction methods. The detector can be operated in either full resolution with 85 mu m pixel size or 2 x 1 pixel binning mode to reduce acquisition time. Twenty-five projection images were acquired with a nominal angular range of +/- 20 degrees. The images were reconstructed using a slice thickness of 1 mm with 0.085 x 0.085 mm in-plane pixel dimension. The imaging performance was characterized by spatial frequency-dependent parameters including a 3D noise power spectrum (NPS) and in-plane modulation transfer function (MTF). Scatter-free uniform x-ray images were acquired at four different exposure levels for noise analysis. An aluminum (Al) edge phantom with 0.2 mm thickness was imaged to measure the in-plane presampling MTF. The measured in-plane MTF and 3D NPS were both in good agreement with the model. The dependence of DBT image quality on reconstruction filters was investigated. It was found that the slice thickness (ST) filter, a Hanning window to limit the high-frequency components in the slice thickness direction, reduces noise aliasing and improves 3D DQE. An ACR phantom was imaged to investigate the effects of angular range and detector operational modes on reconstructed image quality. It was found that increasing the angular range improves the MTF at low frequencies, resulting in better detection of large-area, low-contrast mass lesions in the phantom. There is a trade-off between noise and resolution for pixel binning and full resolution modes, and the choice of detector mode will depend on radiation dose and the targeted lesion. Abstract:OBJECTIVES Because past research has shown that benefits of cochlear implantation may include a significant decrease in psychological and emotional difficulties, this study examined whether persons seeking cochlear implants in recent years differed psychologically from those referred in the early 1980s. A second objective was to explore mechanisms by which profound deafness could contribute to psychological and emotional difficulties for implant candidates and their spouses. METHODS 178 cochlear implant candidates referred from 1981 to 1998 at the University of Iowa Hospitals completed a standard battery of psychological tests and questionnaires. The sample was divided into six 3-year cohorts and compared on standardized measures of psychological and emotional adjustment, and in participation in social and non-social activities. Spouses of implant candidates completed a similar assessment. RESULTS The sample was characterized by elevations in depression, social introversion, suspiciousness, and social anxiety and loneliness. There were no significant differences among cohorts across time except for an increase in expectations for implant success. Spouses also evidenced elevated levels of psychological distress. Hearing status was associated with significant differences in social activity participation. A paradoxical interaction was found between marital status and deafness. CONCLUSIONS There was no evidence that the psychological status of implant candidates is changing across time, suggesting continued psychological benefit for persons receiving cochlear implants. Both candidates and their spouses participated in fewer social activities than normal controls. Findings underscore the complex relation between marital status, deafness, and engagement and participation in positive activities.

Abstract:Virtual clinical trials (VCT) have been developed by a number of groups to study breast imaging applications, with the focus on digital breast tomosynthesis (DBT) imaging. In this review, the main components of these simulation platforms are compared, along with the validation steps, a number of practical applications and some of the limitations associated with this method. VCT platforms simulate, up to a certain level of detail, the main components of the imaging chain: the x-ray beam, system geometry including the antiscatter grid and the x-ray detector. In building VCT platforms, groups use a number of techniques, including x-ray spectrum modelling, Monte Carlo (MC) simulation for x-ray imaging and scatter estimation, ray tracing, breast phantom models and modelling of the detector. The incorporation of different anthropomorphic breast models is described, together with the lesions needed to simulate clinical studies and to study detection performance. A step by step comparison highlights the need for transparency when describing the simulation frameworks. Current simulation bottlenecks include resolution and memory constraints when generating high resolution breast phantoms, difficulties in accessing/applying relevant, vendor specific image processing and reconstruction methods, while the imaging tasks considered are generally detection tasks without search, evaluated by computational observers. A number of applications are described along with some future avenues for research.

Abstract:Objectives Clinical experience regarding the use of dedicated photon-counting breast CT (PC-BCT) for diagnosis of breast microcalcifications is scarce. This study systematically compares the detection and classification of breast microcalcifications using a dedicated breast photon-counting CT, especially designed for examining the breast, in comparison with digital breast tomosynthesis (DBT). Materials and Methods This is a prospective intraindividual study on women with DBT screening–detected BI-RADS-4/-5 microcalcifications who underwent PC-BCT before biopsy. PC-BCT images were reconstructed with a noninterpolated spatial resolution of 0.15 × 0.15 × 0.15 mm (reconstruction mode 1 [RM-1]) and with 0.3 × 0.3 × 0.3 mm (reconstruction mode 2 [RM-2]), plus thin-slab maximum intensity projection (MIP) reconstructions. Two radiologists independently rated the detection of microcalcifications in direct comparison with DBT on a 5-point scale. The distribution and morphology of microcalcifications were then rated according to BI-RADS. The size of the smallest discernible microcalcification particle was measured. For PC-BCT, the average glandular dose was determined by Monte Carlo simulations; for DBT, the information provided by the DBT system was used. Results Between September 2022 and July 2023, 22 participants (mean age, 61; range, 42–85 years) with microcalcifications (16 malignant; 6 benign) were included. In 2/22 with microcalcifications in the posterior region, microcalcifications were not detectable on PC-BCT, likely because they were not included in the PC-BCT volume. In the remaining 20 participants, microcalcifications were detectable. With high between-reader agreement (κ > 0.8), conspicuity of microcalcifications was rated similar for DBT and MIPs of RM-1 (mean, 4.83 ± 0.38 vs 4.86 ± 0.35) ( P = 0.66), but was significantly lower ( P < 0.05) for the remaining PC-BCT reconstructions: 2.11 ± 0.92 (RM-2), 2.64 ± 0.80 (MIPs of RM-2), and 3.50 ± 1.23 (RM-1). Identical distribution qualifiers were assigned for PC-BCT and DBT in 18/20 participants, with excellent agreement (κ = 0.91), whereas identical morphologic qualifiers were assigned in only 5/20, with poor agreement (κ = 0.44). The median size of smallest discernible microcalcification particle was 0.2 versus 0.6 versus 1.1 mm in DBT versus RM-1 versus RM-2 ( P < 0.001), likely due to blooming effects. Average glandular dose was 7.04 mGy (PC-BCT) versus 6.88 mGy (DBT) ( P = 0.67). Conclusions PC-BCT allows reliable detection of in-breast microcalcifications as long as they are not located in the posterior part of the breast and allows assessment of their distribution, but not of their individual morphology.

Abstract:Purpose: The authors develop a practical, iterative algorithm for image-reconstruction in under-sampled tomographic systems, such as digital breast tomosynthesis (DBT).Methods: The algorithm controls image regularity by minimizing the image total p variation (TpV), a function that reduces to the total variation when p=1.0 or the image roughness when p=2.0. Constraints on the image, such as image positivity and estimated projection-data tolerance, are enforced by projection onto convex sets. The fact that the tomographic system is undersampled translates to the mathematical property that many widely varied resultant volumes may correspond to a given data tolerance. Thus the application of image regularity serves two purposes: (1) Reduction in the number of resultant volumes out of those allowed by fixing the data tolerance, finding the minimum image TpV for fixed data tolerance, and (2) traditional regularization, sacrificing data fidelity for higher image regularity. The present algorithm allows for this dual role of image regularity in undersampled tomography.Results: The proposed image-reconstruction algorithm is applied to three clinical DBT data sets. The DBT cases include one with microcalcifications and two with masses.Conclusions: Results indicate that there may be a substantial advantage in using the present image-reconstruction algorithm for microcalcification imaging. Abstract:A preliminary investigation of image-reconstruction algorithm and scanning angular-range impact on μcalc imaging in digital breast tomosynthesis (DBT) is presented. The total-variation (TV) minimization and expectation-maximization (EM) algorithms are compared on synthetic data derived from a breast phantom. These algorithms are compared for 11-view data sets covering 30°, 60°, and 90°. For images resulting from truncating the iteration number at 10, it appears that TV-minimization for the 30° scan yields the highest contrast for μcalc imaging.
The Differential Diagnostic Value of Digital Breast Tomosynthesis in Combination with Synthesized Two-Dimensional Images in Breast Disease

Abstract:Objective To investigate the diagnostic value and characteristics of breast lesion in digital breast tomosynthesis (DBT) in combination with synthesized two-dimensional (2D) images. Methods The prospective study enrolled women older than 18 years with clinically suspected breast lesion.One hundred and sixty seven patients with 177 lesions confirmed by biopsy or surgery were included. All the patients underwent full-field digital mammography(FFDM)and DBT examinations,and synthesized 2D images(V-preview)were reconstructed from DBT.The images of FFDM,V-preview images and DBT plus FFDM, V-preview images were analyzed. The breast imaging reporting and data system (BI-RADS) and characteristic for predicting benign and malignant lesion were also evaluated.The average glandular dose for a single mammographic view between FFDM and DBT was recorded.The Nonparametric Z test was used to compare the differences among four different mammographic images in BI-RADS.The differential diagnostic performance among different mammography was evaluated by ROC analysis.Independent t test was used to compare the average glandular dose between FFDM and DBT.Results One hundred three benign lesions and 74 malignant lesions were confirmed. Compared with FFDM images alone or plus DBT,the diagnostic values of V-preview images alone/or plus DBT were not significantly different(Z=0.187 and 0.226,P=0.851 and 0.821), but compared with V-preview, the diagnostic values of V-preview images plus DBT revealed significant difference(Z=3.546,P<0.01).The area under ROC for V-preview plus DBT were 0.899,and the sensitivity,specificity,and accuracy were 90.5%,89.3%,and 89.3%,separately.For the average glandular dose, there was no significant difference between FFDM (1.48 ± 0.52) mGy and DBT (1.56 ± 0.39) mGy examination(t=1.714,P=0.087).Conclusion Synthesized 2D images from DBT,which may eliminate the need for FFDM,in combination with DBT can improve the diagnostic efficiency.
Optimizing angular range in digital breast tomosynthesis: A phantom study investigating lesion detection across varied breast density and thickness

Abstract:Purpose: To determine the optimal angular range (AR) for digital breast tomosynthesis (DBT) systems that provides highest lesion visibility across various breast densities and thicknesses. Method: A modular DBT phantom, consisting of tissue-equivalent adipose and glandular modules, along with a module embedded with test objects (speckles, masses, fibers), was used to create combinations simulating different breast thicknesses, densities, and lesion locations. A prototype DBT system operated at four ARs (AR±7.5°, AR±12.5°, AR±19°, and AR±25°) to acquire 11 projection images for each combination, with separate fixed doses for thin and thick combinations. Three blinded radiologists independently assessed lesion visibility in reconstructed images; assessments were averaged and compared using linear mixed models. Results: Speckle visibility was highest with AR±7.5° or AR±12.5°, decreasing with wider ARs in all density and thickness combinations. The difference between AR±7.5° and AR±12.5° was not statistically significant, except for the tube-side speckles in thin-fatty combinations (5.83 [AR±7.5°] vs. 5.39 [AR±12.5°], P = 0.019). Mass visibility was not affected by AR in thick combinations, while AR±12.5° exhibited the highest mass visibility for both thin-fatty and thin-dense combinations (P = 0.032 and 0.007, respectively). Different ARs provided highest fiber visibility for different combinations; however, AR±12.5° consistently provided highest or comparable visibility. AR±12.5° showed highest overall lesion visibility for all density and thickness combinations. Conclusions: AR±12.5° exhibited the highest overall lesion visibility across various phantom thicknesses and densities using a projection number of 11.
Examining the Influence of Digital Phantom Models in Virtual Imaging Trials for Tomographic Breast Imaging

Abstract:Purpose: Digital phantoms are one of the key components of virtual imaging trials (VITs) that aim to assess and optimize new medical imaging systems and algorithms. However, these phantoms vary in their voxel resolution, appearance, and structural details. This study aims to examine whether and how variations between digital phantoms influence system optimization with digital breast tomosynthesis (DBT) as a chosen modality. Methods: We selected widely used and open-access digital breast phantoms generated with different methods. For each phantom type, we created an ensemble of DBT images to test acquisition strategies. Human observer localization ROC (LROC) was used to assess observer performance studies for each case. Noise power spectrum (NPS) was estimated to compare the phantom structural components. Further, we computed several gaze metrics to quantify the gaze pattern when viewing images generated from different phantom types. Results: Our LROC results show that the arc samplings for peak performance were approximately 2.5 degrees and 6 degrees in Bakic and XCAT breast phantoms respectively for 3-mm lesion detection tasks and indicate that system optimization outcomes from VITs can vary with phantom types and structural frequency components. Additionally, a significant correlation (p= 0.01) between gaze metrics and diagnostic performance suggests that gaze analysis can be used to understand and evaluate task difficulty in VITs.

Assessing Digital Breast Tomosynthesis Impact on Early Cancer Detection: Insights from Consecutive Screening

Abstract:Background Analysis of how digital breast tomosynthesis (DBT) screening affects consecutive screening performance is important to estimate its future value in screening. Purpose To evaluate whether DBT contributes to early detection of breast cancer by assessing cancer detection rates (CDRs), including the fraction of invasive cancers and cancer subtypes in consecutive routine digital mammography (DM). Materials and Methods The paired prospective Malmö Breast Tomosynthesis Screening Trial (MBTST) was performed between January 2010 and February 2015. Participating women underwent one-view DBT and two-view DM at one screening occasion. In this secondary analysis, women were followed up through their first (DM1) and second (DM2) consecutive two-view DM screening rounds after MBTST participation. Cancer diagnoses were identified by referencing records. A logistic regression model, adjusted for age, was used to calculate the odds of luminal A-like cancers with use of the MBTST as reference. Results There were 14 848 final participants in the MBTST (median age, 57 years [IQR, 49-65 years]). Of those, 12 876 women were screened in DM1 (median age, 58 years [IQR, 50-66 years]) and 10 883 were screened in DM2 (median age, 59 years [IQR, 51-67 years]). Compared with CDRs in the trial of 6.5 of 1000 women (95% CI: 5.2, 7.9) for DM and 8.7 of 1000 women (95% CI: 7.3, 10.3) for DBT, the CDR was lower in DM1 (4.6 of 1000 women [95% CI: 3.6, 5.9]) and DM2 (5.3 of 1000 women [95% CI: 4.1, 6.9]). The proportion of invasive cancers was 84.9% (118 of 139 cancers) in the MBTST; the corresponding numbers were 66% (39 of 59 cancers) for DM1 and 83% (50 of 60 cancers) for DM2. The odds of luminal A-like cancers were lower in DM1 at 0.28 (95% CI: 0.12, 0.66 [P = .004]) but not in DM2 at 0.80 (95% CI: 0.40, 1.58 [P = .52]) versus screening in the MBTST. Conclusion CDR and the fraction of invasive cancers were lower in DM1 and then increased in DM2 following the MBTST, indicating earlier cancer detection mainly due to increased detection of luminal A-like cancers in DBT screening. MODIFYING EFFECT OF VITAMIN E ON ADRIAMYCIN AND CYCLOPHOSPHAMIDE INDUCED GENOTOXICITY AND ANTIOXIDANT STATUS IN RATS

Abstract:In the present study, we have evaluated the protective effect of vitamin E (VE) against adriamycin (AD) and cyclophosphamide (CP) induced genotoxicity using chromosome aberration and micronucleus assays in Wistar rat bone marrow cells. The level of lipid peroxidation product malondialdehyde, the activity of antioxidant enzymes catalase and superoxide dismutase were measured in blood serum and erythrocyte hemolysate to evaluate the antioxidant status in rats. VE (250 mg/kg body weight, b.w.) was administered via gavage once a day for 3 consecutive days. Single dose of AD (5 mg/kg b.w.) or CP (30 mg/kg b.w.) was delivered by the intraperitoneal route. Pre-treatment of rats with VE was conducted for two days before AD or CP injection and concomitantly with AD or CP on the 3rd day. VE was administered concomitantly with AD or CP injection on the 1st day and for two consecutive days in the post-treated groups. No beneficial effect of VE on the antioxidant status of rats was determined. VE decreased drug induced bone marrow toxicity in all experimental groups. Protective effect of VE against ADand CP-induced genotoxicity was dependent on the treatment schedule (i.e. sequencing of VE treatment). VE was protective against AD-induced chromosome damage in animals pre-treated with VE (both chromosome aberration and micronucleus assays) and against CP-induced damage in animals post-treated with VE (micronucleus assay only).
Digital Breast Tomosynthesis: Systems, Characterization and Simulation

Abstract:Digital breast tomosynthesis (DBT) is an advanced imaging application for breast cancer detection, which makes use of a number of 2D X-ray projection images over a limited angular range to reconstruct quasi 3D reconstruction images. As such, first an introduction to digital X-ray tomosynthesis is given, after which existing limited angle tomosynthesis methods are presented. In the next section DBT geometry and its development to date is discussed. Details are given on geometries currently available from different manufacturers, and recent advances. Part of this section presents the available relevant reconstruction methods. Next, we look into the DBT detectors and their performance evaluation. Current DBT detectors are based on amorphous silicon (a-Si) and amorphous selenium (a-Se) thin film transistor (TFT) technology. However, complementary metal-oxide-semiconductor (CMOS) active pixel sensor (APS) digital X-ray detectors have the potential to replace a-Si:H TFT detectors in DBT in the near future, due to a smaller pixel pitch, low electronic noise, faster frame rate and negligible image lag. The performance of current DBT detectors is evaluated mainly by: automatic exposure control performance, response function, noise analysis, detector element failure, and system projection modulation transfer function (MTF). Following this we discuss image quality measurements, because they are essential for the evaluation and optimization of DBT systems. They should represent relevant clinical tasks, such as the detection of microcalcifications and masses in mammographic backgrounds. Currently the CDMAM phantom is used for contrast-detail analysis (i.e., the required threshold contrast to detect discs of various diameters) of the reconstructed images. The TOR MAM phantom can also be used to score the visualization of discs, filaments and specks for various contrast levels. The parameter Z-resolution is used to assess the inter-plane spread of reconstruction artifacts associated with 1 mm diameter aluminum spheres (contained in a specific three-dimensional phantom). Furthermore, the system MTF in the x–y plane is used to take into account all sources of blurring in the DBT system: detector MTF, additional sources of unsharpness and the reconstruction algorithm. The final part of the chapter describes image simulation methods for DBT optimization. Briefly, DBT is currently under consideration for its use in breast cancer screening in Europe, in combination with 2D mammography or alone. Several parameters (such as image acquisition parameters, detector response, system geometry, radiation dose, and image processing and reconstruction algorithms) are studied for their effect on image quality and the investigation of the optimum use of DBT. Traditionally, large clinical trials are required to evaluate these parameters over a large number of women. Such trials are time consuming, expensive and require irradiating asymptomatic women. Alternatively, several research groups use virtual clinical trials (based on image simulation methods) to optimize DBT parameters in fast, radiation-free, and cost-effective ways. This part of the chapter reviews several simulation methods in DBT and the applications in evaluating its effectiveness.
Clinical Feasibility of Dual-Layer CT With Virtual Monochromatic Image for Preoperative Staging in Patients With Breast Cancer: A Comparison With Breast MRI

Abstract:Objective: Dual-layer CT (DLCT) can create virtual monochromatic images (VMIs) at various monochromatic X-ray energies, particularly at low keV levels, with high contrast-to-noise ratio. The purpose of this study was to assess the clinical feasibility of contrast-enhanced chest DLCT with a low keV VMI for preoperative breast cancer staging, in comparison to breast MRI. Materials and methods: A total of 152 patients with 155 index breast cancers were enrolled in the study. VMIs were generated from contrast-enhanced chest DLCT at 40 keV and maximum intensity projection (MIP) with three-dimensional (3D) reconstruction was performed for both bilateral breast areas. Two radiologists reviewed in consensus the 3D MIP images of the chest DLCT with VMI and breast MRI in separate sessions with a 3-month wash-out period. The detection rate and mean tumor size of the index cancer were compared between the chest DLCT with VMI and breast MRI. Additionally, the agreement of tumor size measurement between the two imaging modalities were evaluated. Results: Of all index cancers, 84.5% (131/155) were detected in the chest DLCT with VMI, while 88.4% (137/155) were detected in the breast MRI (P = 0.210). The Bland-Altman agreement between the chest DLCT with VMI and breast MRI was a mean difference of -0.05 cm with 95% limits of agreement of -1.29 to 1.19 cm. The tumor size in the chest DLCT with VMI (2.3 ± 1.7 cm) was not significantly different from that in the breast MRI (2.4 ± 1.6 cm) (P = 0.106). Conclusion: The feasibility of chest DLCT with VMI was demonstrated for preoperative tumor staging in breast cancer patients, showing comparable cancer detectability and good agreement in tumor size measurement compared to breast MRI. This suggests that chest DLCT with VMI can serve as a potential alternative for patients who have contraindications to breast MRI.

Abstract:PURPOSE:Digital breast tomosynthesis (DBT) offers poor image quality along the depth direction. This paper presents a new method that improves the image quality of DBT considerably through the a priori information from automated ultrasound (AUS) images.METHODS:DBT and AUS images of a complex breast-mimicking phantom are acquired by a DBT/AUS dual-modality system. The AUS images are taken in the same geometry as the DBT images and the gradient information of the in-slice AUS images is adopted into the new loss functional during the DBT reconstruction process. The additional data allow for new iterative equations through solving the optimization problem utilizing the gradient descent method. Both visual comparison and quantitative analysis are employed to evaluate the improvement on DBT images. Normalized line profiles of lesions are obtained to compare the edges of the DBT and AUS-corrected DBT images. Additionally, image quality metrics such as signal difference to noise ratio (SDNR) and artifact spread function (ASF) are calculated to quantify the effectiveness of the proposed method.RESULTS:In traditional DBT image reconstructions, serious artifacts can be found along the depth direction (Z direction), resulting in the blurring of lesion edges in the off-focus planes parallel to the detector. However, by applying the proposed method, the quality of the reconstructed DBT images is greatly improved. Visually, the AUS-corrected DBT images have much clearer borders in both in-focus and off-focus planes, fewer Z direction artifacts and reduced overlapping effect compared to the conventional DBT images. Quantitatively, the corrected DBT images have better ASF, indicating a great reduction in Z direction artifacts as well as better Z resolution. The sharper line profiles along the Y direction show enhancement on the edges. Besides, noise is also reduced, evidenced by the obviously improved SDNR values.CONCLUSIONS:The proposed method provides great improvement on the quality of DBT images. This improvement makes it easier to locate and to distinguish a lesion, which may help improve the accuracy of the diagnosis using DBT imaging.

Abstract:Background Digital breast tomosynthesis (DBT) has outpaced digital mammography in clinical adoption in the United States; however, substantial technological limitations remain to image quality in DBT, including undersampling from a one‐dimensional (1D) scan geometry, x‐ray source motion during acquisition, and patient motion artifacts from long exam times. Purpose A thermionic cathode x‐ray system employing two‐dimensional (2D, planar) multiple x‐ray‐source arrays (MXA) is proposed to improve DBT image quality. Methods A 1D MXA, consisting of a linear array of thermionic cathodes was used to simulate a 2D MXA. The 1D MXA included 11 focal spots separated by a distance of Δd = 23 mm. The 11 cathodes were paired with 11 molybdenum 50 mm diameter anode disks, mounted on a rotating shaft within a single vacuum enclosure. Image quality was investigated as a function of MXA configuration by integrating the 1D MXA with a 200 × 250 mm2 flat panel detector at a source‐to‐detector distance of 630 mm, resulting in a 20° tomographic arc. To simulate a 2D MXA, the detector (with phantom) was translated orthogonally to the linear array by a distance (δ ) ranging from δ = 0 mm (conventional 1D) to δ = 57 mm. All sources operated at 30 kV with 80 mA and 4.5 mAs/pulse, yielding ∼100 mAs per DBT dataset. DBT reconstructions involved 22 projections and used filtered backprojection with a ramp and Hann apodization filter. Volumetric reconstructions for each source were weighted by sampling differences between sources, and averaged. Image quality was assessed in terms of contrast‐to‐noise ratio (CNR), background clutter noise and power spectrum, and slice sensitivity profile (SSP) using a set of physical phantoms, including: (i) contrast‐detail signals coupled to spherical clutter (PMMA in air); (ii) an SSP phantom; (iii) a commercial "breast" phantom (CIRS BR3D, Sun Nuclear, Norfolk, VA); and (iv) bovine muscle. Abstract:Abstract Objectives The randomized TOmosynthesis plus SYnthesized MAmmography (TOSYMA) screening trial has shown that digital breast tomosynthesis plus synthesized mammography (DBT + SM) is superior to digital mammography (DM) in invasive breast cancer detection varying with breast density. On the other hand, the overall average glandular dose (AGD) of DBT is higher than that of DM. Comparing the DBT + SM and DM trial arm, we analyzed here the mean AGD and their determinants per breast density category and related them to the respective invasive cancer detection rates (iCDR). Methods TOSYMA screened 99,689 women aged 50 to 69 years. Compression force, resulting breast thickness, the calculated AGD obtained from each mammography device, and previously published iCDR were used for comparisons across breast density categories in the two trial arms. Results There were 196,622 exposures of 49,227 women (DBT + SM) and 197,037 exposures of 49,132 women (DM) available for analyses. Mean breast thicknesses declined from breast density category A (fatty) to D (extremely dense) in both trial arms. However, while the mean AGD in the DBT + SM arm declined concomitantly from category A (2.41 mGy) to D (1.89 mGy), it remained almost unchanged in the DM arm (1.46 and 1.51 mGy, respectively). In relative terms, the AGD elevation in the DBT + SM arm (64.4% (A), by 44.5% (B), 27.8% (C), and 26.0% (D)) was lowest in dense breasts where, however, the highest iCDR were observed. Conclusion Women with dense breasts may specifically benefit from DBT + SM screening as high cancer detection is achieved with only moderate AGD elevations. Clinical relevance statement TOSYMA suggests a favorable constellation for screening with digital breast tomosynthesis plus synthesized mammography (DBT + SM) in dense breasts when weighing average glandular dose elevation against raised invasive breast cancer detection rates. There is potential for density-, i.e., risk-adapted population-wide breast cancer screening with DBT + SM. Key Points Breast thickness declines with visually increasing density in digital mammography (DM) and digital breast tomosynthesis (DBT). Average glandular doses of DBT decrease with increasing density; digital mammography shows lower and more constant values. With the smallest average glandular dose difference in dense breasts, DBT plus SM had the highest difference in invasive breast cancer detection rates.

Investigating Simulation-Based Metrics for Characterizing Linear Iterative Reconstruction in Digital Breast Tomosynthesis

Abstract:Simulation-based image quality metrics are adapted and investigated for characterizing the parameter dependences of linear iterative image reconstruction for DBT. Three metrics based on 2D DBT simulation are investigated: (1) a root-mean-square-error (RMSE) between the test phantom and reconstructed image, (2) a gradient RMSE where the comparison is made after taking a spatial gradient of both image and phantom, and (3) a region-of-interest (ROI) Hotelling observer (HO) for signal-known-exactly/background-known-exactly (SKE/BKE) and signal-known-exactly/background-known-statistically (SKE/BKS) detection tasks. Two simulation studies are performed using the aforementioned metrics, varying voxel aspect ratio and regularization strength for two types of Tikhonov regularized least-squares optimization. The RMSE metrics are applied to a 2D test phantom and the ROI-HO metric is applied to two tasks relevant to DBT: large, low contrast lesion detection and small, high contrast microcalcification detection. The RMSE metric trends are compared with visual assessment of the reconstructed test phantom. The ROI-HO metric trends are compared with 3D reconstructed images from ACR phantom data acquired with a Hologic Selenia Dimensions DBT system. Sensitivity of image RMSE to mean pixel value is found to limit its applicability to the assessment of DBT image reconstruction. Image gradient RMSE is insensitive to mean pixel value and appears to track better with subjective visualization of the reconstructed bar-pattern phantom. The ROI-HO metric shows an increasing trend with regularization strength for both forms of Tikhonov-regularized least-squares; however, this metric saturates at intermediate regularization strength indicating a point of diminishing returns for signal detection. Visualization with reconstructed ACR phantom images appears to show a similar dependence with regularization strength.

Abstract:To investigate whether digital breast tomosynthesis (DBT) and subsequently generated synthesized mammography (SM) images show a better performance than full-field digital mammography (FFDM) images for diagnosing malignant breast lesions. In addition, the radiation doses for SM using different procedures were compared. This prospective study enrolled 212 women (age ≥ 25 years) with clinically suspicious breast lesions. All participants underwent FFDM and DBT with the same breast compression. Finally, 222 lesions were confirmed by pathological analysis. The mammogram results were evaluated according to the BI-RADS criteria and compared with the pathological results. The diagnostic performances, morphological features and average glandular doses (AGDs) were compared. In total, 141 malignant lesions and 81 benign lesions were confirmed by pathological analysis. The overall AGD showed no significant difference between FFDM and DBT. Compared with 2D imaging, the AUC values of FFDM plus DBT and SM plus DBT were both significantly different overall (P = 0.0002) and remained significantly different in dense breasts (P < 0.0001). In terms of morphologic characteristics, lesions showed similar morphology between FFDM and SM, while the lesion characteristics were discordant from 2D imaging to DBT in 33 lesions in dense breasts. Compared to FFDM, 2D SM images generated from DBT had significantly improved diagnostic efficacy for detecting malignant breast lesions without increasing radiation doses. This new procedure is useful for characterizing breast lesions, particularly in dense breasts.

Abstract:What's new Digital breast tomosynthesis (DBT), when combined with full‐field digital mammogram (FFDM), exhibits relatively high accuracy in breast cancer diagnosis. The combination, however, increases radiation exposure. This review evaluated the performance of DBT plus synthesized 2D (S2D), derived from reconstructed tomosynthesis images, as an alternative to DBT + FFDM. DBT + S2D was found to have a higher cancer detection rate compared to FFDM alone and exhibited superior specificity relative to DBT + FFDM. CDR, however, was similar for DBT + S2D and DBT + FFDM. DBT and S2D perform similarly to DBT combined with FFDM, while mitigating increased radiation exposure, suggesting that it is a safe and effective alternative. Digital breast tomosynthesis (DBT) with full‐field digital mammography (FFDM) exposes women to a higher radiation dose. A synthetic 2D mammogram (S2D) is a two‐dimensional image constructed from DBT. We aim to evaluate the S2D performance when used alone or combined with DBT compared to FFDM alone or with DBT. Studies were included if they recruited screening participants and reported on S2D performance. Studies were excluded if they included symptomatic patients, imaging was for diagnostic purposes, or if participants had a breast cancer history. Meta‐analyses for cancer detection rates (CDR) and Specificities were conducted where available. Differences in the performance of imaging modalities were calculated within individual studies, and these were pooled by meta‐analysis. Out of 3241 records identified, 17 studies were included in the review and 13 in the meta‐analysis. The estimated combined difference in CDRs per thousand among individual studies that reported on DBT plus S2D vs. FFDM and those reporting on DBT plus S2D versus DBT plus FFDM was 2.03 (95% CI 0.81–3.25) and − 0.15 (95% CI −1.17 to 0.86), respectively. The estimated difference in percent specificities was 1.13 (95% CI −0.06 to 2.31) in studies comparing DBT plus S2D and FFDM. In studies comparing DBT plus S2D and DBT plus FFDM, the estimated difference in specificities was 1.08 (95% CI 0.59–1.56). DBT plus S2D showed comparable accuracy to FFDM plus DPT and improved cancer detection to FFDM alone. Integrating S2D with DBT in breast cancer screening is safe and preserves performance.
