Abstract:OBJECTIVE: Knee abduction moment (KAM) during landing predicts non-contact anterior cruciate ligament (ACL) injury risk with high sensitivity and specificity in female athletes. The purpose of this study was to employ sensitive laboratory (lab-based) tools to determine predictive mechanisms that underlie increased KAM during landing.METHODS: Female basketball and soccer players (N=744) from a single county public school district were recruited to participate in testing of anthropometrics, maturation, laxity/flexibility, strength and landing biomechanics. Linear regression was used to model KAM, and logistic regression was used to examine high (>25.25 Nm of KAM) versus low KAM as surrogate for ACL injury risk.RESULTS: The most parsimonious model included independent predictors (β±1 SE) (1) peak knee abduction angle (1.78±0.05; p<0.001), (2) peak knee extensor moment (0.17±0.01; p<0.001), (3) knee flexion range of motion (0.15±0.03; p<0.01), (4) body mass index (BMI) Z-score (-1.67±0.36; p<0.001) and (5) tibia length (-0.50±0.14; p<0.001) and accounted for 78% of the variance in KAM during landing. The logistic regression model that employed these same variables predicted high KAM status with 85% sensitivity and 93% specificity and a C-statistic of 0.96.CONCLUSIONS: Increased knee abduction angle, quadriceps recruitment, tibia length and BMI with decreased knee flexion account for 80% of the measured variance in KAM during a drop vertical jump.CLINICAL RELEVANCE: Females who demonstrate increased KAM are more responsive and more likely to benefit from neuromuscular training. These findings should significantly enhance the identification of those at increased risk and facilitate neuromuscular training targeted to this important risk factor (high KAM) for ACL injury.

Abstract:Background: The anterior cruciate ligament (ACL) is one of the most injurious parts of the knee in the biomechanical environment during landing actions. The purpose of this study was to compare the lower limb differences in movement patterns, muscle forces and ACL forces during drop landing (DL), drop vertical jump (DVJ) and forward vertical jump (FVJ). Methods: Eleven basketball and volleyball female college athletes (Division II and I) were recruited. Landing actions of DL, DVJ and FVJ, kinematics and dynamics data were collected synchronously using a motion capture system. OpenSim was used to calculate the ACL load, knee joint angle and moment, and muscle force. Results: At initial contact, different landing movements influenced knee flexion angle; DL action was significantly less than FVJ action ( p = 0.046). Different landing actions affected quadriceps femoris forces; FVJ was significantly greater than DL and DVJ actions ( p = 0.002 and p = 0.037, respectively). However, different landing movements had no significant effects on other variables (knee extension moment, knee valgus angle and moment, hamstring and gastrocnemius muscle forces, and ACL forces) ( p > 0.050). Conclusion: There was no significant difference in the knee valgus, knee valgus moment, and the ACL forces between the three landing actions. However, knee flexion angle, knee extension moments sagittal factors, and quadriceps and gastrocnemius forces are critical factors for ACL injury. The DL action had a significantly smaller knee flexion angle, which may increase the risk of ACL injury, and not recommended to assess the risk of ACL injuries. The FVJ action had a larger knee flexion angle and higher quadriceps femoris forces that were more in line with daily training and competition needs. Therefore, it is recommended to use FVJ action in future studies on risk assessment of ACL injuries and injury prevention in female college athletes.

Abstract:Background: Studies on the biomechanical mechanisms of an anterior cruciate ligament (ACL) injury have been extensively studied, but studies on the neuromuscular control-related risk factors for an ACL injury in specific maneuvers have not been reported for badminton players. Study design: Controlled laboratory study. Methods: Sixteen badminton players (8 male, 8 female) performed a single-leg badminton ball landing task, and lower limb muscle activity, kinematic data, and ground reaction force were measured during this procedure using marker-based movement analysis, force plates, and electromyography (EMG). Gender differences in the lower limb kinematic data, mean values of normalized lower limb muscle activation (MVC%), and co-contraction values during the landing preparation phase (100 ms before initial contact) were analyzed using MANOVA. Results: In the badminton landing task, the knee valgus angle was greater in females than in males (6.27 ± 2.75 vs. 1.72 ± 3.20) in the pre-landing preparation position. Compared to male badminton players, females exhibited greater gluteus maximus (44.92 ± 18.00 vs. 20.34 ± 11.64), rectus femoris (41.56 ± 9.84 vs. 26.14 ± 10.46), and medial gastrocnemius (37.39 ± 17.31 vs. 19.11 ± 11.17) lateral gastrocnemius (36.86 ± 17.82 vs. 13.59 ± 2.71) muscle activity (MVC%). Conclusion: Female badminton players exhibit neuromuscular control strategies that may be inadequate for ACL protection and may be a potential risk factor for a high incidence of ACL injury In the future, when devising injury prevention plans for female badminton players, optimizing neuromuscular control during the pre-landing phase can be targeted.

Abstract:Introduction Incidence rates in anterior cruciate ligament (ACL) injuries, which are linked to multiple factors, are higher in females than males (Sutton & Bullock, 2013). However, the main contributors to the difference in risk are biomechanical factors such as increased knee valgus (KV), knee internal rotation (KIR), or decreased hip flexion (HF; Seyedahmadi et al., 2022). Returning back to sports after an injury requires extensive rehabilitation as well as training of dual-task situations in order to be physically and cognitively prepared. Exergaming represents a dual task including a cognitive and a motor component and may be used as training modality during rehabilitation. It is known that performing a dual-task leads to altered landing movement patterns in healthy athletes (Dai et al., 2018). Before an exergame for rehabilitation after ACL injury can be implemented and tailored to the specific needs of female and male athletes, the movement patterns of males and females need to be understood. Therefore, the aim of this study was to identify biomechanical differences between males and females when performing a high-intensive exergame. Methods Using 3D-motion capture (Vicon) kinematics were measured in 18 healthy athletes (9 male, 9 female) during a 25-min exergame (Sphery Racer, ExerCube). The exergame included nine different exercises of which 8 were used for analysis. The exercises were separated into neutral (squat, jump, burpee) and side-specific exercises (low-touch, mid-touch, high-touch, punch, lunge). For analysis, the maximal KV, maximal KIR, and the minimal HF were extracted during 10-30° knee flexion. These maximal values were then compared between the sexes, the exercises, and both legs with a linear mixed model, for each the neutral and the side-specific exercises. Results A main effect of sex was only found for side-specific exercises in KIR (F(1, 16) = 6.1, p = .02) with females exhibiting higher values. For KV and HF in side-specific exercises as well as all variables in neutral exercises, no significant main effect for sex was found. However, there was a main effect of exercise in KV; KIR, and HF in neutral (p < .001) as well as in side-specific exercises (p < .001). Discussion/Conclusion Differences in KIR between males and females primarily exist during touches and punches. Abstract:Modifiable (biomechanical and neuromuscular) anterior cruciate ligament (ACL) injury risk factors have been identified in laboratory settings. These risk factors were subsequently used in ACL injury prevention measures. Due to the lack of ecological validity, the use of on-field data in the ACL injury risk screening is increasingly advocated. Though, the kinematic differences between laboratory and on-field settings have never been investigated. The aim of the present study was to investigate the lower-limb kinematics of female footballers during agility movements performed both in laboratory and football field environments. Twenty-eight healthy young female talented football (soccer) players (14.9 ± 0.9 years) participated. Lower-limb joint kinematics was collected through wearable inertial sensors (Xsens Link) in three conditions: (1) laboratory setting during unanticipated sidestep cutting at 40-50°; on the football pitch (2) football-specific exercises (F-EX) and (3) football games (F-GAME). A hierarchical two-level random effect model in Statistical Parametric Mapping was used to compare joint kinematics among the conditions. Waveform consistency was investigated through Pearson's correlation coefficient and standardized z-score vector. In-lab kinematics differed from the on-field ones, while the latter were similar in overall shape and peaks. Lower sagittal plane range of motion, greater ankle eversion, and pelvic rotation were found for on-field kinematics (p < 0.044). The largest differences were found during landing and weight acceptance. The biomechanical differences between lab and field settings suggest the application of context-related adaptations in female footballers and have implications in ACL injury prevention strategies. Talented youth female football players showed kinematical differences between the lab condition and the on-field ones, thus adopting a context-related motor strategy.Lower sagittal plane range of motion, greater ankle eversion, and pelvic rotation were found on the field. Such differences pertain to the ACL injury mechanism and prevention strategies.Preventative training should support the adoption of non-linear motor learning to stimulate greater self-organization and adaptability.It is recommended to test football players in an ecological environment to improve subsequent primary ACL injury prevention programmes.

Abstract:Background: Soccer is one of the most popular sports worldwide, which subsequently increases the number of injuries experienced by players. Furthermore, a large percentage of all anterior cruciate ligament injuries occur while playing soccer. In order to more clearly understand injury mechanisms, it is important to make the testing environment as real-life as possible. Inclusion of an external focus and secondary task, such as heading a soccer ball, may increase joint loading during landing. The purpose of this study was to investigate the effect of a forward heading motion on lower extremity kinetics and kinematics between sexes during a stop-jump task and a jump-heading task. Methods: Ten male and ten female soccer players performed stop-jumps with no soccer ball present and jump-headings with a soccer ball present. Three-dimensional kinematics and kinetics were collected and analyzed during the landing. 2 × 2 mixed design analysis of variances (ANOVA) were performed to examine sex × jump task interactions and determine the main effects of sex and jump task. Results: Results indicated jump-heading yields greater peak vertical ground reaction forces, an 8% increase in peak knee extension moments, a reduced initial knee flexion angle by approximately 5°, and an increased initial hip flexion angle by approximately 7°. Additionally, females exhibited 5.6° greater peak knee abduction angles compared to men, regardless of task. Conclusions: Inclusion of an overhead target may have distracted the athletes from focusing on frontal plane knee control when landing, and could potentially lead to increased ACL stress.

Abstract:Anterior cruciate ligament (ACL) injuries in sports often occur with non-contact mechanisms, such as landing and cutting. Previous studies explored the ACL injury biomechanical risk factors through drop-jumps combined with secondary jumps. This study aimed to investigate the effect of the secondary jump direction on first landing kinematic temporal series. Fifty-seven participants (29 males, 28 females) performed three single-leg drop-jumps followed by secondary jumps in vertical (single-planar), 45°-medial and 45°-lateral direction (multi-planar). Lower limb and trunk landing kinematics was recorded using a 9-camera motion capture system and analysed with a One-way ANOVA through Statistical Parametric Mapping (SPM), from initial contact to maximum knee flexion. All variables were affected by the secondary jump direction, except trunk rotation. In sagittal plane, kinematic main differences were found between single- and multi-planar tasks. The latter elicited higher trunk, hip, and knee flexion. Frontal plane kinematics was more influenced by medio-lateral components of secondary jumps. Our results could underline how a single task may be insufficient for ACL injury risk assessment. Single- and multi-planar tasks including a secondary jump should be considered for more comprehensive evaluations in prevention and rehabilitation programs, but caution should be used when comparing results of studies adopting different tasks.

What are gender differences in lower limb muscle activity during jump–landing tasks? A systematic review and meta-analysis

Abstract:Background Gender differences in muscle activity during landing have been proposed as a possible contributing factor to the greater incidence of anterior cruciate ligament injuries in women. Conflicting results among a few studies in this regard makes it impossible to reach correct conclusions. Objectives The aim of this study was systematic review and the meta-analysis of previous studies which have compared the electromyographic activity of lower limb muscles in gluteus muscles (maximus and medius), quadriceps (rectus femoris, vastus medialis and lateralis), hamstrings (biceps femoris and semimembranosus), and gastrocnemius in men and women in jump–landing task. Methods A systematic search of the PubMed, SCOPUS, Science Direct databases was performed for eligible articles in October 2020. Cross-sectional studies that compared the muscle activity of male and female athletes without a history of previous injury in the jump–landing task were included. Unisex and non-athlete's studies were extracted from the included studies. The data were synthesized using a fixed and random effects model. Results Eight studies involving 145 participants were included. All participants were people who participated in regular exercises. The meta-analysis of timing and muscle activity was performed in the feedforward (pre contact) and feedback (post contact) stages. There were no significant differences in the muscle activity of biceps femoris (MD = −12.01; 95% CI − 51.49 to 27.47; p = 0.55; I 2 = 87%), vastus medialis (MD = −53.46; 95% CI − 129.73 to 22.81; p = 0.17; I 2 = 91%), semimembranosus (MD = 1.81; 95% CI − 6.44 to 10.07; p = 0.67; I 2 = 0%), gluteus medius (MD = −3.14; 95% CI − 14.24 to 7.96; p = 0.58; I 2 = 48%), and rectus femoris (MD = −5.83; 95% CI − 14.57 to 2.92; p = 0.19; I 2 = 87%) in the pre contact phase between two sexes. There was a significant difference between men and women in the activity of vastus lateralis muscle in the post contact phase (MD = −34.90; 95% CI − 48.23 to − 21.57). No significant difference was observed between the men and women in the timing of semimembranosus (MD = 23.53; 95% CI − 14.49 to 61.54; p = 0.23; I 2 = 56%) and biceps femoris muscle activity (MD = −46.84; 95% CI − 97.50 to 3.83; p = 0.07; I 2 = 82%). Conclusion The results showed that in all lower limb muscles except vastus lateralis there were no significant differences between muscle activity and muscle contraction timing in both sexes before and after foot contact. Therefore, it can be concluded that the reason for the greater susceptibility of ACL injuries in women than men is maybe related to other factors such as biomechanical and hormonal. Additional good quality research in this regard is required to strengthen these conclusions.
Differences in ACL biomechanical risk factors between field hockey and lacrosse female athletes

Abstract:Purpose: Previous investigations have revealed a greater incidence of anterior cruciate ligament (ACL) injuries in female lacrosse versus field hockey players. Lacrosse is played in an upright posture with overhead throwing and catching, while field hockey is almost exclusively played in a crouched, forward-flexed position. Biomechanical factors, including decreased knee, hip, and trunk flexion angles, have been identified as risk factors for ACL injury. The purpose of this study was to assess ACL biomechanical risk factors in female field hockey and lacrosse players to determine whether sport-specific posture might contribute to the increased incidence of ACL injury observed in lacrosse athletes. Methods: Thirty-one Division I NCAA females from field hockey and lacrosse completed four tasks, three times per leg: bilateral drop jump, single-leg drop jump (SDJ), single-leg jump onto a Bosu ball (SDB), and a 45° anticipated cut. Kinematic and force plate data were used to evaluate knee flexion angle, knee adduction moment, hip flexion angle, and trunk flexion and sway angles. Muscle activity of the lateral hamstrings and vastus lateralis was used to estimate peak hamstring activity and the quadriceps/hamstring ratio at the time of peak quadriceps activity (co-contraction ratio). Results: During the SDJ and SDB, peak knee flexion angles were greater in field hockey compared with lacrosse. During cutting, field hockey players were more flexed at the trunk and had greater trunk sway, compared with the lacrosse players. No significant difference was observed for the co-contraction ratio for any of the tasks. Conclusions: Decreased knee flexion angle during landing, consistent with sport-specific playing postures, may contribute to the higher incidence of ACL injury in lacrosse players relative to field hockey. Sport-specific training injury prevention programmes may benefit from considering these differences between specialized athletes. Level of evidence: II.

Abstract:Objective: Muscle fatigue contributes to anterior cruciate ligament (ACL) injuries, with increased knee and hip abduction observed during fatigue. However, there have been no reports revealing the differences between fatigue tasks or the duration of these changes. In this study, we conducted single-leg drop landings before and after hip and knee fatigue tasks to elucidate the changes in lower limb biomechanics over time. Methods: Twenty-two male participants performed single-leg drop landings before, immediately after, and 5, 10, and 15 min after fatigue tasks involving isokinetic hip abduction/adduction (hip fatigue task [HFT]) and knee extension/flexion (knee fatigue task [KFT]). Hip and knee kinematic and kinetic data were collected using a three-dimensional motion analysis device and two force plates. A two-way ANOVA was performed with both the fatigue task (HFT and KFT) and time point (Time 1 to Time 4) as factors, and the main effects and interactions were calculated. Results: The knee adduction angle after the HFT was significantly greater than that after KFT immediately following the fatigue task. The knee flexion moment was significantly lower in the KFT, whereas the knee adduction and internal rotation moments were significantly higher in the HFT immediately after the fatigue task. Conclusion: This study revealed distinct kinematic and kinetic changes specific to each fatigue task, particularly in the frontal plane for hip joint tasks and the sagittal plane for knee joint tasks. These findings could assist in the development of ACL injury prevention programs tailored to the functional improvement and exercise capacity of each joint.
Sex difference in frontal plane hip moment in response to lateral trunk obliquity during single-leg landing

Abstract:Abstract Background Lateral trunk obliquity during landing is a characteristic of anterior cruciate ligament (ACL) injuries in female athletes and affects their knee and hip kinetics and kinematics. However, it is unclear whether these effects differ between females and males. The purpose of this study was to compare the effects of lateral trunk obliquity on knee and hip kinetics and kinematics in females and males during single-leg landing. Methods Eighteen female (aged 22.1 ± 1.5 years) and 18 male participants (aged 21.8 ± 1.1 years) performed single-leg landings under two conditions: (1) without any instructions about trunk position (natural) and (2) with leaning their trunks laterally 15° from the vertical line (trunk obliquity). The kinetics and kinematics of their hip and knee were analyzed using a three-dimensional motion analysis with a force plate. Two-way repeated-measures ANOVA (sex × trunk obliquity) and Bonferroni pairwise comparisons were conducted. Results The trunk obliquity angle at initial contact was significantly greater in the trunk-obliquity landing condition than in the natural landing condition (natural 4.0 ± 2.2°, trunk-obliquity 15.1 ± 3.6°, P < 0.001) with no sex difference (95% CI − 1.2 to 2.2°, P = 0.555). The peak knee abduction moment was significantly larger in the trunk-obliquity landing condition than in the natural landing condition (trunk-obliquity, 0.09 ± 0.07 Nm/kg/m; natural, 0.04 ± 0.06 Nm/kg/m; P < 0.001), though there was no sex or interaction effect. A significant interaction between sex and landing condition was found for the peak hip abduction moment ( P = 0.021). Males showed a significantly larger peak hip abduction moment in the trunk-obliquity landing condition than in the natural landing condition (95% CI 0.05 to 0.13 Nm/kg/m, P < 0.001), while females showed no difference in the peak hip abduction moment between the two landing conditions (95% CI − 0.02 to 0.06 Nm/kg/m, P = 0.355). Conclusions The knee abduction moment increased with a laterally inclined trunk for both female and male participants, while the hip abduction moment increased in males but not in females. It may be beneficial for females to focus on frontal plane hip joint control under lateral trunk-obliquity conditions during single-leg landing.

Although adjuvant trastuzumab has dramatically reduced progression--making metastatic disease far less common today--newer treatment advances are also impacting this disease. Dual targeting with pertuzumab and trastuzumab can extend the survival of metastatic disease by more 16 months, but despite such success, resistance to HER2 targeting remains a challenge. Drugs in the pipeline, such as neratinib, may help meet this therapeutic demand. In addition to anti-HER2 agents, chemotherapy is beneficial to patients with tumors 1 cm or larger, but the optimal treatment of smaller tumors is still a work in progress.
Biomechanical Risk Assessment of Non-Contact Anterior Cruciate Ligament Injury in Taekwondo Athletes

Abstract:Non-contact anterior cruciate ligament (ACL) injury can occur in many sports. It is interrelated with gender, anatomy, biomechanics, and neuromuscular control. Taekwondo athletes have a higher incidence of ACL injury than athletes from other sports. Objective: This study aimed to determine the biomechanical gender differences and mechanism of taekwondo athletes with ACL injury. Methods: A total of 28 taekwondo athletes (aged 14–19 years) were randomly selected and grouped by gender. Feet high floor, one foot high floor, and single leg squat were analyzed by a Vicon motion analysis system and Kistler 3D force platform for action. The knee joint angle and ground force were evaluated. Results: Results demonstrated biomechanical differences in knee joint between male and female athletes. Conclusion: ACL injury in taekwondo female athletes indicated the biomechanical mechanism of the knee joint, and it can be prevented by neuromuscular control training.
Differences and correlations in knee and hip mechanics during single-leg landing, single-leg squat, double-leg landing, and double-leg squat tasks

Abstract:Landing and squat tasks have been utilized to assess lower extremity biomechanics associated with anterior cruciate ligament loading and injury risks. The purpose of this study was to identify the differences and correlations in knee and hip mechanics during a single-leg landing, a single-leg squat, a double-leg landing, and a double-leg squat. Seventeen male and 17 female recreational athletes performed landings and squats when kinematic and kinetic data were collected. ANOVAs showed significant differences (p < 0.00001) for maximum knee flexion angles, maximum hip flexion angles, maximum knee abduction angles, maximum hip adduction angles, and maximum external knee abduction moments among squats and landings. For maximum knee and hip flexion angles, significant correlations (r ≥ 0.5, p ≤ 0.003) were observed between the two landings and between the two squats. For maximum knee abduction and hip adduction angles and maximum external knee abduction moments, significant correlations were mostly found between the two landings, and between the single-leg squat and landings (r ≥ 0.54, p ≤ 0.001). Individuals are likely to demonstrate different profiles of injury risks when screened using different tasks. While a double-leg landing should be considered as a priority in screening, a single-leg squat may be used as a surrogate to assess frontal plane motion and loading.

Abstract:Background: Stiff landings with less knee flexion and high vertical ground-reaction forces have been shown to be associated with an increased risk of anterior cruciate ligament (ACL) injury. The literature on the association between other sagittal plane measures and the risk of ACL injuries with a prospective study design is lacking. Purpose: To investigate the relationship between selected sagittal plane hip, knee, and ankle biomechanics and the risk of ACL injury in young female team-sport athletes. Study Design: Case-control study; Level of evidence, 3. Methods: A total of 171 female basketball and floorball athletes (age range, 12-21 years) participated in a vertical drop jump test using 3-dimensional motion analysis. All new ACL injuries, as well as match and training exposure data, were recorded for 1 to 3 years. Biomechanical variables, including hip and ankle flexion at initial contact (IC), hip and ankle ranges of motion (ROMs), and peak external knee and hip flexion moments, were selected for analysis. Cox regression models were used to calculate hazard ratios (HRs) with 95% CIs. The combined sensitivity and specificity of significant test variables were assessed using a receiver operating characteristic (ROC) curve analysis. Results: A total of 15 noncontact ACL injuries were recorded during follow-up (0.2 injuries/1000 player-hours). Of the variables investigated, landing with less hip flexion ROM (HR for each 10° increase in hip ROM, 0.61 [95% CI, 0.38-0.99]; P < .05) and a greater knee flexion moment (HR for each 10-N·m increase in knee moment, 1.21 [95% CI, 1.04-1.40]; P = .01) was significantly associated with an increased risk of ACL injury. Hip flexion at IC, ankle flexion at IC, ankle flexion ROM, and peak external hip flexion moment were not significantly associated with the risk of ACL injury. ROC curve analysis for significant variables showed an area under the curve of 0.6, indicating a poor combined sensitivity and specificity of the test. Conclusion: Landing with less hip flexion ROM and a greater peak external knee flexion moment was associated with an increased risk of ACL injury in young female team-sport players. Studies with larger populations are needed to confirm these findings and to determine the role of ankle flexion ROM as a risk factor for ACL injury. Increasing knee and hip flexion ROMs to produce soft landings might reduce knee loading and risk of ACL injury in young female athletes.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 10, October 2024. Background:Biomechanics and anterior cruciate ligament injury mechanisms differ in males and females. There is a need for more data on between-limb biomechanical differences after anterior cruciate ligament reconstruction (ACLR) in females.Purpose:To explore biomechanical asymmetries throughout the kinetic chain during the single-legged (SL) and double-legged (DL) countermovement jump (CMJ) and drop jump (DJ) in female athletes after ACLR.Study Design:Descriptive laboratory study.Methods:Kinematic and kinetic between-limb differences were analyzed during the SL and DL CMJ and DJ in 67 female athletes 9 months after ACLR. Biomechanical and performance asymmetries between limbs during the jumps and isokinetic strength testing were analyzed with statistical parametric mapping. The entire stance phase was used for the paired t tests of the biomechanical variables, with Cohen d effect sizes of significant portions of the stance phase (reported as % of stance) calculated in a point-by-point manner.Results:Decreased vertical ground-reaction force, internal knee abduction moment, knee internal rotation angle, hip external rotation angle, internal ankle eversion, and external rotation moments were seen in the ACLR limb during all 4 vertical jump tests. The greatest number and highest value of differences were found during the DLDJ, with asymmetries having medium to large effect sizes. They tended to appear more frequently in the concentric phase (50% to 100% of stance) during the SLCMJ and DLCMJ and in the eccentric (0% to 49% of stance) and concentric (50% to 100% of stance) phase during the SLDJ and DLDJ. For the SLCMJ, SLDJ, and quadriceps strength, performance asymmetries of >15% were detected but not for change of direction.Conclusion:The findings suggest that return-to-play testing in female athletes should examine the entire stance phase and include assessments of kinetic and kinematic variables throughout the kinetic chain. Greater deficits were highlighted in the DJ than in the CMJ, and greater performance asymmetries were evident in the SL tasks, with greater kinetic and kinematic and compensatory strategies evident in the DL tests.Clinical Relevance:Biomechanical analysis focusing on contralateral compensation strategies and sex-specific interventions are necessary before return to play.

Abstract:This study aimed to explore the single-legged landing kinematics that could lead to increase or decrease in the risk of anterior cruciate ligament (ACL) injury. Immediate pre-impact kinematics at the single-legged landing from 33 healthy young female handball players were evaluated. Thereafter, two-year follow-up for ACL injury incidence was conducted, in which six new ACL injuries in non-dominant leg were registered. The evaluation of pre-impact kinematics across participants was performed first by the principal component analysis to decompose them into the kinematic components (KCs), and then by the linear discrimination analysis (LDA) for a set of KC-scores to obtain important KCs for discriminating injured and non-injured legs. The result of LDA showed that the combination of second major KC (knee flexion/extension angle and angular velocity) and some minor KCs such as torso medial/lateral leaning accurately discriminated the injured and non-injured legs with the error rate of 12.5%. To examine the mechanisms of this discriminative ability, we generated hypothetical pre-impact kinematics in the subspaces spanned by eigenvectors of multiple KCs, and examined relationships between pre-impact kinematics and the corresponding knee valgus torque predicted by the motion-equation-based model. The result showed that the second major KC and the minor KCs representing torso medial/lateral leaning and/or hip adduction/abduction angle, which contributed in LDA to discriminating injured legs, also significantly affected the frontal-plane knee loading patterns. These findings suggested that KC-based postural characterization of the pre-impact landing kinematics and the motion-equation-based knee stress quantification possibly explain the future ACL injury risks of female athletes.
