Abstract:Objective:To explore the diagnosis and treatment of malignant eccrine spiradenoma.Methods:We reported a case of malignant eccrine spiradenoma,and reviewed literature about the diagnosis and treatment of malignant eccrine spiradenoma.Results:We reported a case of a 67-year-old woman,who found an enlarging nodule in her right lower limb and underwent surgical resection of the mass in June 2015,the postoperative pathology showed malignant eccrine spiradenoma.After local resection,multiple metastases to the right iliac artery and right inguinal lymph nodes were confirmed by postoperative positron emission tomography/computed tomography of the whole body.She agreed to a trial of chemotherapy of "mFOLFOX",but the patient had a tumor recurrence and did operation twice.After surgery,the patient did not have antitumor therapy due to physically weakness.She was given the best symptomatic and supportive treatment and died on July 21st,2016.Conclusion:The clinical manifestation of MES lacks the specificity,and MES should be confirmed by pathology,the first choice for treatment of MES is complete resection covering enough tumor-free margins.There is still no generally accepted chemoradiotherapy,the prognosis of MES is poor and it has short survival time.
Surgical management of facial malignant tumors

Abstract:Objective To investigate the performance of surgical management in facial skin malignancies.Methods From January 2000 to December 2006,65 patients with facial skin malignancies,including47 cases of basal cell carcinoma.10 cases of squamous cell carcinoma,3 cases of dermatofibrosarocoma protuberans,2 cases of malignant melanomas,and one case of malignant acanthoma,hemangioendotheliosar-coma and sebaceous carcinoma,respectively,were collected and managed with wide resection followed by reconstruction.In order to achieve a thorough resection,frozen sections were prepared and subjected to pathological examination during the operation process to ensure the margins of resection were free of malignancy.Reconstruction was carried out by direct closure,or with local random flaps,extended flaps,free skin grafts.Resuits All defects were managed by one-stage reconstruction.The survival rate of skin flaps/grafts was 100%,and a satisfactory appearance and function was achieved.During the follow-up from 6 months to 5 years,local relapse was observed in one patient with basal cell carcinoma and one with squamous eell carcinoma,lymphatic metastasis in one with squamous cell carcinoma.Distant metastasis occurred in a patient with malignant melanoma.who died consequently.Conclusions Thorough resection is the key to prevent relapse of facial skin malignancies after surgery.Appropriate reconstruction may favor the restoration of facial appearance,and local random flaps appear to be the best reconstruction strategy.
Malignant transformation in eccrine spiradenoma: a case report and literature review

Abstract:One case of malignant transformation in eccrine spiradenoma was analyzed on image observation,morphological analysis with review of the literature.A 58-year-old female patient suffered from a cutaneous mass on her left lower abdominal wall for 30 years.Ultrasonography revealed a demarcated heterogeneous-echoic solid mass containing cystic change.It was encapsulated with a 2.1 cm × 1.8 cm nodule attached in the cystic wall.Histologically,the tumor was located in deep dermis.In its solid part,a classic benign spiradenoma was identified.The malignant transformation,however,was recognized in focal area.Those cancer cells were atypical and with high N/C ratio.Immunohistochemically,decrease or loss of myoepithelial and highly expression of Ki-67 were the key to diagnosis.Eccrine spiradenoma with malignant transformation is rare.Familiarity with its histologic characteristics is helpful to making correct diagnosis.
Clinicopathologic Analysis of Spiradenoma: 25 Case Reports

Abstract:Objective To analyze clinicopathologic features and malignant transformation of spiradenoma (SA).Methods Clinicopathologic data on 25 cases of SA were retrospectively analyzed.An immunohistochemical study was conducted on 5 of the 25 patients.Clinicopathologic features of SA were summarized based on clinicopathologic data,immunohistochemical results and relevant literature.Results The average age of these patients was 40 years.Lesions were acquired and solitary in 24 cases (96％,24/25),and occurred most frequently in the extremities.The diameter of tumors was less than 2 cm in 20 cases (87％,20/23).Twenty-three cases were benign SA with various histological manifestations,including 15 cases (65％,15/23) of typical SA,8 cases (35％,8/23) of cylindroma.VarTing numbers of mitotic figures were observed in different cases.The immunohistochemical study showed epithelial and myoepithelial differentiation.Two cases of SA experienced malignant transformation into poorlydifferentiated basal cell adenocarcinoma and well-differentiated adenocarcinoma respectively.Lymphocytes decreased significantly in number or disappeared in the malignant area,and both the malignant tumors exceeded 2 cm in diameter.Conclusions SA is a kind of neoplasm with distinct histological characteristics and nonspecific clinical manifestations.Most SA cases have a benign clinical course,but malignant transformation should be considered in some cases with longterm clinical course and large size.
A rare case of angiosarcoma with skull masses and erythropenia and thrombocytopenia: A case report and review of literature.

Abstract:Rationale: Primary splenic angiosarcoma (PSA) is a rare, fatal neoplasm originating from sinusoidal vascular endothelial cells, and usually metastasizes and almost always has a poor prognosis. Surgical excision is the main treatment of this highly malignant disease. Patient concerns: We reported a special case of a 68-year-old female who had a 6-month history of scalp masses. Diagnosis: The patient was found to have 2 skull masses on computed tomography (CT). Laboratory findings revealed erythropenia and thrombocytopenia. Enhanced abdomen magnetic resonance imaging (MRI) showed multiple masses in liver and spleen. The pathological result of the skull masses was revealed to be metastatic angiosarcoma. Interventions: The patient underwent surgical excision of skull masses, and no subsequent radiotherapy or chemotherapy was done. Outcomes: The patient died due to dyscrasia at August 12, 2015, with a survival of nearly 1 month. Lessons: We highlight the importance for clinicians to be aware of this rare neoplasm, and to consider it in the differential diagnosis when encountering a skull mass. Early confirmation and treatment may improve the prognosis.
Solitary fibrous tumor of the masticator space - Report of a rare case

Abstract:The solitary fibrous tumor (SFT) is a potentially malignant spindle cell neoplasm of the mesenchymal origin that was originally described as a thoracic lesion originating from the pleural tissue. Recently, numerous extrapleural sites of origin have been described, also affecting the head and neck region. SFTs are benign in most cases, but 10%-15% of extrapleural SFTs show malignant behavior in the form of recurrent or metastatic disease. We present the case of a 25-year-old female who presented with an asymptomatic left-sided facial swelling of over three years. She had a diffuse swelling in the left preauricular region, extending to the temporal region deep to the zygomatic arch. On magnetic resonance imaging, the vascular lobulated mass occupied the masticator space, infratemporal fossa, and parapharyngeal space, eroding the mandible. An ultrasound-guided fine-needle aspiration cytology was suggestive of SFT, positive for signal transducer and activator of transcription 6 and negative for TLE1. After preoperative embolization, the tumor was excised through a midline lip split approach with posterior segmental mandibulectomy and reconstruction with a titanium plate. Histopathological report was consistent with SFT. Due to high-risk features, she was advised adjuvant radiation therapy. SFTs of the head and neck are exceedingly rare and those with aggressive behavior even more so. To our knowledge, this is the only case of SFT arising in the masticator space. Diagnosis is often difficult and not definitive without immunohistochemistry. In most cases, complete surgical excision is the only treatment necessary. Regardless, all patients require close clinical follow-up for several years.
Successful surgical removal of a cutaneous lesion compatible with gigantic vascular eccrine spiradenoma: a rare case report

Abstract:Abstract Giant vascular eccrine spiradenoma (GVES) is an uncommon type of eccrine spiradenoma (ES). Compared to an ES, this is characterized by a greater degree of vascularity and a bigger size overall. In clinical practice, it is frequently mistaken for a vascular or malignant tumor. To achieve an accurate diagnosis of GVES via biopsy and successfully perform the surgical removal of a cutaneous lesion in the left upper abdomen compatible with GVES. We present a 61-year-old female with the lesion accompanied by on-and-off pain, bloody discharge and skin changes surrounding the mass treated surgically. However, there was no fever, weight loss, trauma or family history of malignancy or cancer managed by surgical excision. The patient recovered well postoperatively and was discharged on the same day with a 2-week follow-up. The wound got healed, the clips were removed on Day 7 postoperatively, and there was no requirement for further follow-up.
Malignant chondroid syringoma: An unusual scalp tumor

Abstract:Chondroid syringomas are similar to mixed parotid tumors and present in both benign and malignant forms. Malignant chondroid syringoma (MCS) is a rare skin tumor that has a predilection for extremities, particularly in young women. It is even rarer to present as a scalp tumor with very few reported cases in the literature. We present a middle-aged woman, with a history of increasing fatigability of her right arm for the past 3 months who, on examination, was found to have scalp swelling and matted right posterior triangle lymph nodes. The working diagnosis on her was a large sebaceous cyst with secondary in the neck from an occult primary/non-Hodgkin’s lymphoma. Preliminary fine-needle aspiration was inconclusive. Imaging followed by wide excision of the tumor and the nearby occipital node was done. The final histopathological diagnosis was MCS with secondary in the lymph node. These tumors are aggressive and metastasize early. Radical surgery is the only hope of cure as adjuvant treatment is yet to be standardized.

Abstract:A 40-year-old female presented with right mandibular swelling. A panoramic radiograph showed a unilocular radiolucency from the mandibular angle to tooth #46. Biopsy confirmed a schwannoma. Surgical resection was performed via a submandibular approach with a reconstruction plate. Teeth 46 and 47 were extracted. Surgery was complication-free, and histopathology confirmed the tumor's benign nature. The patient was discharged on the second postoperative day. At the 1-year follow-up, she had no paresthesia, normal mouth opening, and full mandibular motion. The reconstruction plate was intact. This case adds to the limited literature on intraosseous schwannomas, emphasizing early detection, thorough radiological assessment, and meticulous surgical planning.

Abstract:Objective To explore the pathological and morphological features,pathological diagnosis,differential diagnosis,therapy and prognosis of the spiradenoma(SA).Methods Eight cases of the spiradenoma were studied by using paraffin slice for HE staining and immunohischemistrical method(EnVision method).Results 8 patients aged from 28 to 66 years,averaging 40.1 years old,4 cases of each genda,showed small nodules beneath skin accompanied with pain.The size of nodules ranged from 0.3 cm to 1.5 cm.Under microscope,two distinct types of neoplasm epithelial cells could be seen,namely,the dark cells and the pale cells.The dark cells were basaloid cells and located at the periphery of the neoplasm cell nests,whereas,the pale cells were glandular epithelia cells and tended to situate in the center of the nests.Two of the 8 cases were followed-up for 2-3 years.No evidence of recurrence was found for the two cases.Conclusions Spiradenoma usually appeared as a solitary nodule with tenderness and located deeply in cutis at any region of the body.Spiradenoma is commonly benign.By immunochemistry stain,the basaloid cells are myoepithelium markers positive,and the glandular epithelia cells are glandular epithelium markers positive.

Abstract:Distribution System State Estimation (DSSE) stays in focus in all smart grid related topics. In Medium Voltage (MV) networks, in particular, and in distribution networks, in general, DSSE is a prerequisite for smart grid functionality. In this paper a new approach for a three-phase DSSE is presented. It utilizes the most types of analog real time measurements (including current magnitude, active and reactive power measurements). Loads are modeled based on historical or AMI/AMR information. Applying traditional state estimation on distribution networks, by using load data as pseudo measurement and zero injection constraints, causes state estimation formulations that have very large size. Approach presented in this paper is based on network reduction which reduces size of the problem. The estimation problem is solved by weighted least squares error method. The approach is very fast and provides feasible results

Abstract:PurposeDesmoid fibromatosis in head and neck is infrequent and poses a significant challenge to the clinicians due to its non-specific characteristics.MethodsThis case report focuses on a 69-year-old male who presented to a tertiary healthcare center in Karnataka, India with a swelling in the oral cavity.ResultsDespite initial suspicions of malignancy based on clinical examination and findings on computed tomography imaging, subsequent histopathology and immunohistochemistry revealed an unexpected finding.ConclusionThe case highlights the importance of clinical suspicion and histopathological evaluation as well as the need for greater awareness to facilitate early diagnosis and appropriate management of desmoid fibromatosis. We also present a literature review of varied presentations of desmoid tumors afflicting various subsites of the head and neck.

Abstract:Cutaneous spindle cell tumors share the common feature of appearing as spindle-shaped cells on light microscopy. Their pathogenesis, presentation, and prognosis are highly variable, and numerous techniques for workup and treatment have been reported. We performed an analysis of the available scientific literature in order to codify the clinical, immunohistochemical, and biologic features of these tumors and to provide insight into the most effective practices for their management, with a focus on Mohs micrographic surgery (MMS). In this article, the clinical and histopathological characteristics of dermatofibrosarcoma protuberans, atypical fibroxanthoma, malignant fibrous histiocytoma, spindle cell squamous cell carcinoma, superficial leiomyosarcoma, desmoplastic melanoma, cutaneous angiosarcoma, and myofibrosarcoma are described, and methods for diagnosis, workup, treatment, and surveillance are evaluated. Cutaneous spindle cell neoplasms are diverse in origin, presentation, and behavior. Immunostaining assists in differentiating among the various types. Further workup is sometimes indicated to characterize local invasion or assess for metastatic disease. Surgery is typically the first-line treatment, and MMS is associated with low recurrence rates and a tissue-sparing advantage for many tumors. Adjuvant treatments, including radiation therapy, molecular-targeted therapy, and conventional chemotherapy, are sometimes indicated, and close clinical surveillance is required after treatment.

Abstract:Sclerosing polycystic adenoma (SPA) is a rare neoplastic salivary gland lesion with only about 100 cases reported worldwide so far. The lesion is confused with several malignant and other benign tumors such as apocrine intraductal carcinoma (IC), salivary duct carcinoma (SDC), chronic sclerosing sialadenitis, polycystic dysgenetic disease (PDD), pleomorphic adenoma (PA), acinic cell carcinoma (ACC), and mucoepidermoid carcinoma (MEC). We present a case of SPA for a 23-year-old male patient presenting with a slowly growing parotid mass. Fine needle aspiration (FNA) followed by total excision of the tumor was performed and the picture was consistent with SPA. We discuss the findings of the case and briefly review the literature on SPA.

Breast cancer with scalp metastases: a case report

Abstract:Background: While breast cancer is the most common cancer in women, cutaneous metastases are rare in breast cancer. Additionally, scalp involvement in breast cancer metastasis is extremely rare. That being said, scalp lesions should always be thoroughly investigated to distinguish metastatic lesions from other neoplasms. Case presentation: A 47-year-old female Middle-Eastern patient presented with metastatic breast cancer in the lungs, bone, liver, and brain with no signs of multiple organ failure, in addition to cutaneous metastases, including the scalp. Between 2017 and 2022, she was managed through modified radical mastectomy, radiotherapy, and several lines of chemotherapy. She presented in September of 2022 with enlarging scalp nodules, which started developing 2 months prior to her presentation. Physical examination revealed firm, non-tender, and immobile skin lesions. Magnetic resonance imaging scan of the head showed soft tissue nodules in different sequences. A punch biopsy was taken from the largest scalp lesion and showed metastatic invasive ductal carcinoma. A panel of immunohistochemistry stains was applied, because a single specific marker for differentiating primary cutaneous adnexal tumors or other malignant neoplasms from breast cancer has not yet been identified. The panel showed positive estrogen receptor 95%, progesterone receptor 5%, negative human epidermal growth factor receptor 2, positive GATA binding protein 3, positive cytokeratin-7, negative P63, and negative KIT (CD117). Conclusion: Breast cancer metastases to the scalp are extremely uncommon. When a scalp metastasis is present, it might be the only symptomatic sign of disease progression or widespread metastatic lesions. However, such lesions warrant a comprehensive radiologic and pathologic workup to rule out other possibilities of skin pathologies, such as sebaceous skin adenocarcinoma as it effects the management plan.

Abstract:Background: While statistically rare in comparison to other head and neck tumours, parotid gland swellings are often encountered in clinical practice where one of the primary goals of examination becomes distinction between benign and malignant lesions. Hallmarks of malignancy are characterized by a female preponderance, history of radiation exposure, a positive family history, and clinical features like heterogenous consistency, fixity to skin/underlying tissues and involvement of facial nerve. Case presentation: Here we present a case of parotid swelling in a 72-year old gentleman from south India that had a curious amalgamation of both benign and malignant features. Conclusions: While benign, the risk of malignant transformation and rare multicentric occurrence indicates a need to keep basal cell adenoma in mind in case of parotid swellings and their surgical management.
Clinical significance of bone marrow biopsy and plasma cell morphology in MM and MGUS.

Abstract:Multiple myeloma (MM) is characterized by plasma cell infiltration in the bone marrow, suppression of normal haematopoiesis, destruction of bone, renal failure and recurrent infections. The extent of organ involvement, severity of complications, drug sensitivity and therefore the clinical course differ widely among patients. Specific and reliable laboratory examinations and especially morphology including both biopsy and aspirations are required to evaluate its aggressivity and extent. In this review of MM and its variants we emphasize the impact of morphology on the diagnostic and prognostic approach to MM and MGUS, and on the planning and monitoring of appropriate therapy.
Squamous Cell Carcinoma of the Submandibular Gland With Cutaneous Fistula: A Case Report and Literature Review

Abstract:Squamous cell carcinoma (SCC) of salivary glands, also referred to as epidermoid carcinoma, is a very rare neoplastic tumor. It occurs as metastasis of a cutaneous or mucosal squamous carcinoma of the head and neck or as a primary SCC. In the latter case, the most known risk factor is previous irradiation to the gland. Common clinical symptoms are represented by cervical swelling and hyposialia. The treatment is essentially surgical, most often supplemented by a radical neck dissection and postoperative radiation therapy. A 75-year-old male patient with a history of chronic smoking was consulted for a tumefaction in the right submandibular region evolving for three months. No cervical lymphadenopathy in the submandibular and superior jugulo-carotid areas was palpable. CT scan showed an enhancing heterogeneous process of the right cervical region, invading the mylohyoid and stylohyoid muscles. A biopsy-excision of the lesion has shown a keratinizing tumor with cytonuclear atypia, consistent with SCC. Radical resection of the mass was associated with the removal of the infiltrated skin. The cutaneous defect was repaired with a rhomboid flap. The patient was started on sessions of adjuvant radiotherapy and chemotherapy. Eight months postoperatively, the patient came for follow-up, with no signs of local disease. The EGFR protein is found in ~70% of salivary neoplasms and is considered as a factor of poor prognosis and rapid proliferation. PET CT is currently the best examination to detect the existence of a concomitant malignant lesion. Diagnosis of primary SCC (PSCC) of the submandibular gland is made on histopathology. Differential diagnoses include mucoepidermoid carcinoma, lymphoepithelial carcinoma and submandibular metaplasia. There is an increased prevalence of nodal involvement in the PSCC, which justifies neck dissection (regions I, II and III of the neck). The RAS mutation leading to resistance to anti-EGFR therapies may be assessed. This would allow for a treatment depending on molecular features for metastatic PSCCs. PSCC of major salivary glands is a very rare lesion with local and general aggressiveness. The diagnosis is based on a combination of clinical examination, MRI, fine needle aspiration and histological examination. Immunotherapy constitutes a ground of research to treat metastatic and advanced pathological cases.

Abstract:Meningiomas typically manifest as benign, slow-growing, and well-defined tumors on a macroscopic level and are usually asymptomatic. However, the mass effect caused by large meningiomas may lead to various neurological symptoms, commonly headaches and visual problems. Radiological imaging can establish the diagnosis, and a biopsy can provide a definitive diagnosis. Our case report describes the surgical intervention for bilateral parasagittal-falcine meningioma in a 57-year-old male who presented to the emergency department with a tonic-clonic seizure. On examination, he had a bifrontal longitudinal mass. Magnetic resonance imaging (MRI) revealed a large anterior superior falcine extra-axial mass, measuring about 5.7 x 5.3 x 3.1 cm, with surrounding vasogenic edema and superior sagittal sinus invasion. He underwent surgery for tumor resection involving the anterior third and middle third of the superior sagittal sinus without radiotherapy. He did not develop any intraoperative complications, and during the post-operative evaluation, he was symptom-free. A follow-up MRI with contrast performed three months later showed no neurological complications or recurrent tumor. To achieve better outcomes, surgical intervention for parasagittal and falcine meningiomas involving the superior sagittal sinus should aim to eliminate clinical signs, control tumor growth, and prevent neurological deterioration post-operatively.
