Abstract:It is common to argue that water infrastructure innovations improve life expectancy. Yet the benefits of clean water depend on a mechanism to dispose of waste water. We draw on the historical experience of a large industrial city to estimate the impact of the spread of the sewer system. Using a longitudinal data set on mortality and rents for each of Paris’ 80 neighborhoods we show that sanitation contributed several years to life expectancy. These results point out the multiplicity of infrastructure needed to help decrease mortality.

Abstract:We explore the first period of sustained decline in child mortality in the U.S. and provide estimates of the independent and combined effects of clean water and effective sewerage systems on under-five mortality. Our case is Massachusetts, 1880 to 1920, when authorities developed a sewerage and water district in the Boston area. We find the two interventions were complementary and together account for approximately one-third of the decline in log child mortality during the 41 years. Our findings are relevant to the developing world and suggest that a piecemeal approach to infrastructure investments is unlikely to significantly improve child health.

The effects of water supply on infant and childhood mortality: a review of historical evidence

Abstract:The provision of clean water is mentioned as an important factor in many studies dealing with the decline of mortality in Europe during the late nineteenth and early twentieth centuries. In developing countries too, improved water supply is assumed to have a strong impact on mortality. When studying the effect of water supply on public health, researchers are confronted with many methodological problems. Most of these also apply to historical studies of the subject. We review the evidence from this historical research, taking into account the methodological problems observed in contemporary impact evaluation studies, and we use more refined data from the Dutch city of Tilburg, enabling us to overcome many of these shortcomings. Finally, we discuss some factors which may explain why we failed to discover an effect of the availability of piped water on the level of childhood mortality.

Abstract:This survey sheds light on the causes and consequences of the global sanitary revolution that resulted in the spread of waterworks and sewerage projects since the middle of the 19th century, by drawing on research from the fields of economic history, economics, and history. I begin with a discussion of the construction of these infrastructures during the period ca. 1850–1950 showing that their spread was relatively similar in major urban cities across the globe, while diffusion within and between countries, as well as within cities themselves, was markedly unequal. Second, I review research estimating the mortality impact of access to clean water and sanitation. Following the provision of these services, infant mortality declined between ca. 10 and 30 percent. Lastly, I examine the drivers of the sanitary revolution with a new framework that distinguishes between proximate factors (e.g., physical capital) and ultimate factors (e.g., institutions). I argue that the state of knowledge in the literature is insufficient to explain between‐ and within country differences in access to sanitary services and that more attention should be devoted to the interaction of political and economic factors within broader institutional, cultural and biogeographic contexts.

Abstract:In 2020, 46% of the global population lacked safely managed sanitation services. Poor sanitation may lead to health problems and restrict economic activities. This study is the first to focus on the government-subsided Toilet Revolution in rural China and empirically investigates the causal effects of sanitation coverage increase on labor supply in developing countries. Using the China Health and Nutrition Survey data, we exploit the temporal and regional variation during the revolution and employ a difference-in-differences methodology. Our study finds that rural households with access to clean flush toilets significantly increase their work time by 0.253–0.431 h/day, and the effects are more prominent among women than men. As for the mechanism, clean toilets significantly decrease housework related to cleaning the house and raising livestock and poultry, allowing people to reallocate their time and invest more into work, which is more salient for women. Our findings highlight the importance of sanitation coverage in improving the economic outcomes in the rural areas of China. For policymakers concerned with the provision of sanitary facilities and the supply of labor force in developing countries, this study indicates that improved sanitation facilities also unexpectedly increase labor supply, particularly for women.

Abstract:Public health benefits are often a key political driver of urban sanitation investment in developing countries, however, pathogen flows are rarely taken systematically into account in sanitation investment choices. While several tools and approaches on sanitation and health risks have recently been developed, this research identified gaps in their ability to predict faecal pathogen flows, to relate exposure risks to the existing sanitation services, and to compare expected impacts of improvements. This paper outlines a conceptual approach that links faecal waste discharge patterns with potential pathogen exposure pathways to quantitatively compare urban sanitation improvement options. An illustrative application of the approach is presented, using a spreadsheet-based model to compare the relative effect on disability-adjusted life years of six sanitation improvement options for a hypothetical urban situation. The approach includes consideration of the persistence or removal of different pathogen classes in different environments; recognition of multiple interconnected sludge and effluent pathways, and of multiple potential sites for exposure; and use of quantitative microbial risk assessment to support prediction of relative health risks for each option. This research provides a step forward in applying current knowledge to better consider public health, alongside environmental and other objectives, in urban sanitation decision making. Further empirical research in specific locations is now required to refine the approach and address data gaps.

Abstract:The problem of inadequate access to water, sanitation and hygiene (WASH) in less-developed nations has received much attention over the last several decades (most recently in the Millennium Development Goals), largely because diseases associated with such conditions contribute substantially to mortality in poor countries. We present country-level projections for WASH coverage and for WASH-related mortality in developing regions over a long time horizon (1975-2050) and provide dynamic estimates of the economic value of potential reductions in this WASH-related mortality, which go beyond the static results found in previous work. Over the historical period leading up to the present, our analysis shows steady and substantial improvements in WASH coverage and declining mortality rates across many developing regions, namely East Asia and the Pacific, Latin America and the Caribbean, Eastern Europe and the Middle East. The economic value of potential health gains from eliminating mortality attributable to poor water and sanitation has decreased substantially, and in the future will therefore be modest in these regions. Where WASH-related deaths remain high (in parts of South Asia and much of Sub-Saharan Africa), if current trends continue, it will be several decades before economic development and investments in improved water and sanitation will result in the capture of these economic benefits. The fact that health losses will likely remain high in these two regions over the medium term suggests that accelerated efforts are needed to improve access to water and sanitation, though the costs and benefits of such efforts in specific locations should be carefully assessed.

Abstract:Abstract Water, sanitation, and hygiene (WASH) investments are widely seen as essential for improving health in early childhood. However, the experimental literature on WASH interventions identifies inconsistent impacts on child health outcomes, with relatively robust impacts on diarrhea and other symptoms of infection but weak and varying impacts on child nutrition. In contrast, observational research exploiting cross-sectional variation in water and sanitation access is much more sanguine, finding strong associations with diarrhea prevalence, mortality, and stunting. In practice, both literatures suffer from significant methodological limitations. Experimental WASH evaluations are often subject to poor compliance, rural bias, and short duration of exposure, while cross-sectional observational evidence may be highly vulnerable to omitted variables bias. To overcome some of the limitations of both literatures, we construct a panel of 442 subnational regions in 59 countries with multiple Demographic Health Surveys. Using this large subnational panel, we implement difference-in-difference regressions that allow us to examine whether longer-term changes in water and sanitation at the subnational level predict improvements in child morbidity, mortality, and nutrition. We find results that are partially consistent with both literatures. Improved water access is statistically insignificantly associated with most outcomes, although water piped into the home predicts reductions in child stunting. Improvements in sanitation predict large reductions in diarrhea prevalence and child mortality but are not associated with changes in stunting or wasting. We estimate that sanitation improvements can account for just under 10 % of the decline in child mortality from 1990 to 2015.

Abstract:The decline in infant mortality played a crucial role in the health transition in the Western World. This decline among the vulnerable new-borns was however not an evenly dispersed process. Inequalities as a result of regional differences, cultural influences or socioeconomic status shaped the paths towards low mortality rates. The role of socioeconomic status in levels of infant mortality and its decline remain highly debated. In this article, we study the development of socioeconomic disparities in infant mortality in the Dutch town of Maastricht in the period 1864–1955. This study uses unique individual-level cause of death data in order to see when changes in disease patterns took place for different socioeconomic groups. The aim is to identify socioeconomic inequalities by mapping changing epidemiological patterns over time, according to age within the first year of life. By deploying a multinomial logistic regression analysis we can trace the different timing of changes in the epidemiological regime. The results reveal that for most infants the change in mortality patterns from water- and foodborne infectious diseases towards a predominance of airborne infectious diseases occurred simultaneously with the massive decline in infant mortality from the start of the First World War onwards. Infants from the upper classes, however, appeared to have gained an earlier advantage, followed by infants from unskilled workers. Finally, from qualitative data it becomes clear that the awareness of the problematic nature of infant mortality, knowledge on infant care, hygienic practices, breastfeeding practices and the economic situation of World War I were factors aiding to the uneven decline in infant mortality.

Abstract:Abstract I investigate the social costs imposed by poor implementation of public infrastructure. Focusing on the period from 2005 to 2015 in Peru, when the government embarked on a nationwide initiative to expand sewerage systems, I leverage quasirandom variation in initiation of the implementation phase. By combining several sources of administrative data, I find that infrastructure development increased infant and under-5 mortality. These effects are driven by health and safety hazards associated with construction work, leading to increased deaths from accidents and waterborne diseases. The severity of these effects is more pronounced in areas where construction activity was more intense.

Abstract:Between 1554 and 2021, the number of and mortality from epidemics in Amsterdam decreased sharply. The decrease in epidemic outbreaks, such as those of plague, smallpox and cholera, paralleled the decrease in chronic mortality from endemic ailments and diseases, such as tuberculosis, malaria and dysentery. There are several theories about the reason for these declines, which are not necessarily mutually exclusive: better nutrition, greater prosperity, increasing altruism, and a growing understanding of cause and effect with targeted medical and public health measures. In the powder keg of chronic poverty and poor public health, a social crisis, such as war, migration, and natural disaster, usually was the spark that led to epidemic outbreaks. The nature and extent of poverty and ill health have changed and improved over the centuries, but the threat of man-made crises is unfortunately unabated.

Abstract:In this paper we test two hypotheses about mortality in early modern England: (1) that market towns had more frequent and more severe mortality crises than rural parishes; and (2) that the underlying level of mortality in market towns was higher than that in rural parishes. The data consist of annual burial totals for ten pairs of parishes, each consisting of one market town and a nearby rural parish, drawn from counties in all parts of England between the sixteenth and the early nineteenth centuries. Mortality crises are identified and their severity measured using a Poisson model, which has the advantage that it can be applied both to small and large parishes without the need for ad hoc adjustments or rules. The results show clearly that mortality crises were more frequent and severe in market towns than in rural parishes, a pattern which would be predicted by epidemiological theory. The evidence that underlying mortality in market towns was higher than that in rural parishes is not as clear cut, though there is a tendency for market towns to record higher levels.

Abstract:This study explores the validity of chain effects of clean water, which are known as the "Mills-Reincke phenomenon," in early twentieth-century Japan. Recent studies have reported that water purifications systems are responsible for huge contributions to human capital. Although some studies have investigated the instantaneous effects of water-supply systems in pre-war Japan, little is known about the chain effects of these systems. By analyzing city-level cause-specific mortality data from 1922-1940, we find that a decline in typhoid deaths by one per 1,000 people decreased the risk of death due to non-waterborne diseases such as tuberculosis and pneumonia by 0.742-2.942 per 1,000 people. Our finding suggests that the observed Mills-Reincke phenomenon could have resulted in the relatively rapid decline in the mortality rate in early twentieth-century Japan.

Abstract:Background: Ghana's low investment in household sanitation is evident from the low rates of improved sanitation. This study analysed how land ownership, tenancy security and livelihood patterns are related to sanitation investments in three adjacent rural and peri-urban communities in a district close to Accra, Ghana's capital. Methods: Qualitative data was gathered for this comparative ethnographic study over seven months, (June, 2011-January, 2012) using an average of 43 (bi-weekly) participant observation per community and 56 in-depth interviews. Detailed observational data from study communities were triangulated with multiple interview material and contextual knowledge on social structures, history of settlement, land use, livelihoods, and access to and perceptions about sanitation. Results: This study shows that the history of settlement and land ownership issues are highly correlated with people's willingness and ability to invest in household sanitation across all communities. The status of being a stranger i.e. migrant in the area left some populations without rights over the land they occupied and with low incentives to invest in sanitation, while indigenous communities were challenged by the increasing appropriation of their land for commercial enterprises and for governmental development projects. Interview responses suggest that increasing migrant population and the high demand for housing in the face of limited available space has resulted in general unwillingness and inability to establish private sanitation facilities in the communities. The increasing population has also created high demand for cheap accommodation, pushing tenants to accept informal tenancy agreements that provided for poor sanitation facilities. In addition, poor knowledge of tenancy rights leaves tenants in no position to demand sanitation improvements and therefore landlords feel no obligation or motivation to provide and maintain domestic sanitation facilities. Conclusions: The study states that poor land rights, the history of settlements, in-migration and insecure tenancy are key components that are associated with local livelihoods and investments in private sanitation in rapidly changing rural and peri-urban communities of Ghana. Sanitation policy makers and programme managers must acknowledge that these profound local, ethnic and economic forces are shaping people's abilities and motivations for sanitation investments.
Rural/urban mortality differences in England and Wales and the effect of deprivation adjustment

Abstract:Perceptions that rural populations are inevitably healthier and live longer than urban populations are increasingly being challenged. But very few publications have investigated the extent to which these putative differences can be explained by variation in area composition. Existing publications have tended to use conventional deprivation measures, often thought to mask rural deprivation by favourable averages. Further, they have typically been based on large and variably-sized geographical units, or confined to studies of a single region or cause of death. This study examines differences in mortality between rural and urban areas in the entire population of England and Wales for 2002-2004. It uses the most up-to-date small geographical units of similar size and homogeneity of population together with the recently-introduced Rural and Urban Area Classification, and adjusts for five different deprivation measures (including modern composite indices). The causes of death investigated were all-cause mortality, cancer, lung cancer, respiratory disease, circulatory disease, suicide and accidents. Particular points of focus for the study were the potential for interaction between deprivation and rurality, and the importance of choice of deprivation measure in quantifying the relationships between mortality, rurality and deprivation. Choice of deprivation measure was not found to alter the substantive conclusions of any analysis, and little evidence for differential effects of deprivation in rural and urban areas was uncovered. Differences between rural and urban areas in all-cause, circulatory disease and cancer mortality could largely be accounted for by adjusting for deprivation. For these causes of death, therefore, rural populations were not found to be inherently healthier than their urban counterparts. However, substantial residual differences between rural and urban areas were found in comparisons of mortality from lung cancer and respiratory disease, mortality being lower in rural areas. Stronger relationships between rurality and mortality were found in 'village and dispersed' settlements.

Abstract:This article, written during the COVID-19 epidemic, provides a general introduction to the long-term history of infectious diseases, epidemics and the early phases of the spectacular long-term improvements in life expectancy since 1750, primarily with reference to English history. The story is a fundamentally optimistic one. In 2019 global life expectancy was approaching 73 years. In 1800 it was probably about 30. To understand the origins of this transition, we have to look at the historical sequence by which so many causes of premature death have been vanquished over time. In England that story begins much earlier than often supposed, in the years around 1600. The first two 'victories' were over famine and plague. However, economic changes with negative influences on mortality meant that, despite this, life expectancies were either falling or stable between the late sixteenth and mid eighteenth centuries. The late eighteenth and early nineteenth century saw major declines in deaths from smallpox, malaria and typhus and the beginnings of the long-run increases in life expectancy. The period also saw urban areas become capable of demographic growth without a constant stream of migrants from the countryside: a necessary precondition for the global urbanization of the last two centuries and for modern economic growth. Since 1840 the highest national life expectancy globally has increased by three years in every decade.

Abstract:Throughout its history, Cartagena de Indias, a seaport in the Colombian Caribbean, has been handicapped for not offering salubrious conditions to its people and visitors. The lack of an aqueduct and a sewerage system was an impairment to progress. For nearly forty years (1890-1930) these problems have caused a myriad of medical discourses formulated by scientists, technicians and politicians. Cartagena's contribution to solve the sanitation problem in cities has consisted in making use of engineers' knowledge. The construction of urban facilities in the beginning of the twentieth century required a more technical knowledge, one which would advance a comprehensive solution to the water problem, ensure sufficient supply and efficient drainage. Thus, in the last turn of the century, the medical doctor is no longer the only authoritative voice when it comes to the management of urban life. The construction works which require an engineer, involving him in public health, have drawn a distinction between "hygiene" and "sanitary science".

Abstract:We use data from the Irish census and exploit regional and temporal variation in infant mortality rates over the 20th century to examine effects of early life conditions on later life health. The urban mortality penalty collapsed in Ireland in the years right after World War II. Our main identification is public health interventions centered on improved sanitation and food safety, which we believed played a leading role in eliminating the Irish urban infant mortality penalty. Our estimates suggest that a unit decrease in mortality rates at time of birth reduces the probability of being disabled as an adult by about 12–18%.

Abstract:Improvement of water and sanitation conditions may reduce infant mortality, particularly in countries like India where open defecation is highly prevalent. We conducted a quasi-experimental study to investigate the association between the Swachh Bharat Mission (SBM)-a national sanitation program initiated in 2014-and infant (IMR) and under five mortality rates (U5MR) in India. We analyzed data from thirty-five Indian states and 640 districts spanning 10 years (2011-2020), with IMR and U5MR per thousand live births as the outcomes. Our main exposure was the district-level annual percentage of households that received a constructed toilet under SBM. We mapped changes in IMR and U5MR and toilet access at the district level over time. We fit two-way fixed effects regression models controlling for sociodemographic, wealth, and healthcare-related confounders at the district-level to estimate the association between toilets constructed and child mortality. Toilet access and child mortality have a historically robust inverse association in India. Toilets constructed increased dramatically across India following the implementation of SBM in 2014. Results from panel data regression models show that districts with > 30% toilets constructed under SBM corresponds with 5.3 lower IMR (p < 0.05), and 6.8 lower U5MR (p < 0.05). Placebo, falsification tests and robustness checks support our main findings. The post-SBM period in India exhibited accelerated reductions in infant and child mortality compared to the pre-SBM years. Based on our regression estimates, the provision of toilets at-scale may have contributed to averting approximately 60,000-70,000 infant deaths annually. Our findings show that the implementation of transformative sanitation programs can deliver population health benefits in low- and middle-income countries.

Abstract:Economic growth in Sweden during the early 20th Century was largely driven by industry. A significant contributor to this growth was the installation of different kinds of engines used to power factories. We use newly digitized data on engines and their energy source by industry sector, and combine this with municipality-level data of workers per industry sector to construct a new variable reflecting economic output using dirty engines. In turn, we assess the average externality of dirty output on mortality in the short-run, as defined by deaths over the population in the baseline year. Our results show substantial increases of up to 17% higher mortality in cities where large increases to dirty engine installations occurred, which is largely driven by the elderly. We also run a placebo test using clean powered industry and find no effect on mortality.
