Abstract:he AJR’s Centennial celebration recognizes “Percutaneous RF Interstitial Thermal Ablation in the Treatment of Hepatic Cancer” [1] as one of the most influential articles published in the AJR’s first 100 years. This article helped establish an entirely new field in radiology: percutaneous imaging-guided ablation therapy for liver malignancies. This extremely important article, written by Rossi and colleagues [1] who have established themselves as leaders in the area of percutaneous imaging-guided tumor ablation, was critical in setting the stage for expansion and advances of imaging-guided radiofrequency ablation of tumors. Earlier articles published by these authors gave preliminary results for using radiofrequency ablation in the treatment of inoperable liver tumors [2, 3]. However, this article was particularly important, hence the numerous citations, for several reasons. First, the authors showed the efficacy of this approach by systematically confirming the diagnosis of malignancy in each patient treated. Each treated lesion was a biopsyproven malignancy. This proof of malignancy validated the effectiveness of the technique in achieving oncologic control. Second, the authors are to be applauded for their careful and meticulous follow-up of patients treated with radiofrequency ablation for liver cancer in their series. This study spanned a 7-year experience using imaging-guided radiofrequency ablation for the treatment of liver tumors. The average patient follow-up was almost 2 years, a period adequate to critically compare this technique with surgical resection, which was previously the gold standard. Abstract:Hydatidosis is an endemic disease in certain areas in the world particularly in the Mediterranean, the Middle East, and South America, caused by a cestode known as Echinococcus granulosus. Humans are the accidental intermediate hosts. The liver and the lungs are the most commonly involved organ. If the parasite passes through the pulmonary capillary bed, the hydatid cyst may develop at any site in the body like bone, pancreas, brain, kidney, and orbit. Isolated spleen hydatid cyst is very rare. We hereby report one observation of isolated hydatid cyst of the spleen in a patient living in non-endemic area and without any potential risk.

Spleen salvation: A successful case of spleen-preserving surgery for hydatid cyst

Abstract:Introduction and significance: Hydatid disease is a parasitic infection caused by tapeworm larvae that can affect different organs, including the spleen. While isolated hydatid cysts in the spleen are rare, they are usually asymptomatic and are frequently discovered incidentally. Treatment options depend on several factors, such as cyst location, size, and number. Case presentation: We present the case of a 9-year-old child who lives in rural areas. He was suffering from pain in the left flank for 3 months. An ultrasound and CT scan of the abdomen were performed. It was found that there was a solitary hydatid cyst in the spleen, about half the size of the spleen. Surgical intervention was performed and the cyst was completely removed without opening the cyst and with preservation of normal splenic tissue. Clinical discussion: In cases of splenic cysts, the spleen must be preserved and the cyst must be removed with caution so as not to rupture it. Therefore, protecting the child from the possibility of hydatid cyst spread and anaphylactic shock. Conclusion: In conclusion, this article underscores the importance of differential diagnosis of a hydatid cyst in the spleen and taking the necessary preventive measures before and during surgery to avoid the rupture of the cyst.

Abstract:Background: Splenic cysts are quite rare and asymptomatic. They may result from infection by a parasite, especially Echinococcus granulosus (hydatid cyst), or from non-parasitic causes. Since primary splenic cysts are not common, simple cysts can be misdiagnosed with a hydatid cyst in endemic areas. Case presentation: We reported a 14-year-old Iranian girl initially presented with a vague abdominal pain, which progressed to left shoulder pain, fullness, early satiety, and shortness of breath and remained undiagnosed for 7 months despite seeking medical attention. Finally, imaging revealed a massive splenic cyst measuring 220 mm × 150 mm × 160 mm raising concern for a hydatid cyst due to regional endemicity. Consequently, the patient underwent total splenectomy. However, histopathological examination surprisingly revealed a simple non-parasitic cyst. Conclusions: Detecting rare simple spleen cysts requires early ultrasonography (US) and careful reassessment of diagnoses for non-responsive or worsening symptoms. Distinguishing them from splenic hydatidosis, especially in endemic areas, demands thorough paraclinical evaluations and patient history regarding potential parasitic exposure. While total splenectomy is the primary treatment for these huge cysts, the optimal surgical approach should be tailored case by case. These insights emphasize a comprehensive diagnostic approach to enhance accuracy and optimize patient care for these uncommon cysts.

Abstract:Introduction and importance: A primary congenital splenic epidermoid cyst is an immensely rare pathology with mostly unknown epidemiological parameters. Misdiagnosis can easily happen and this results in life-threatening ramifications for patients. Considering this pathology as a potential differential diagnosis allows for the required surgical intervention to be timely accomplished. In this case, the authors are documenting this pathology and presenting how it was successfully managed via proper and informed preoperative analysis and meticulous intraoperative technique. Case presentation: Hereby, we portray the exceptionally rare case of a 7-year-old male who presented to our surgical clinic complaining of a sudden left hypochondriac pain with early satiety for 1 month's duration. The preoperative radiological assessment displayed numerous splenic cystic lesions throughout the splenic parenchyma. Clinical discussion: Resection of the cysts was accomplished via total splenectomy. The ensuing histopathological analysis via Hematoxylin and Eosin of the resected specimens established the diagnosis of a congested hemorrhagic spleen with multiple primary congenital splenic epidermoid cysts. Conclusion: Primary congenital splenic epidermoid cysts are an extremely rare type of splenic pathology. There is profound scarcity amidst the published literature regarding it. This merits in-depth study and apt documentation to raise awareness regarding this pathology as a potential differential diagnosis in cases of abdominal pain. Documentation allows us to set up proper and innovative clinical and surgical protocols for these patients. Based on our conclusive review of the published literature, the authors conclude that ours is the first ever documented case from our country of a primary congenital splenic epidermoid cyst.

Abstract:Introduction and importance: Hydatid cysts, caused by Echinococcus granulosus, are a significant public health problem in regions such as Tunisia and Morocco. These cysts primarily affect the liver and lungs but can also involve the spleen, kidneys, bones, and brain. Peritoneal hydatidosis, involving the formation of hydatid cysts in the peritoneal cavity, is a rare manifestation of this disease. This case report aims to highlight the presentation, diagnostic process, and surgical management of a primary peritoneal hydatid cyst. Case presentation: A 73-year-old asymptomatic woman with no significant medical history was found to have a hypoechoic mass with a calcified wall in the liver segment IV and multiple multiloculated cystic masses in the peritoneum on an incidental abdominal ultrasound. Further evaluation with a CT scan revealed a hydatid cyst in liver segments IV and V and additional cystic formations in the peritoneum. Surgical exploration via midline incision identified and treated cysts in the omentum, liver, spleen, and pouch of Douglas. A total pericystectomy and other relevant procedures were performed. The patient's postoperative course was uncomplicated, and she recovered well. Discussion: Peritoneal hydatidosis is typically secondary to hepatic hydatid cysts, caused by the parasite Echinococcus granulosus. The diagnosis is made primarily through imaging techniques such as CT and ultrasonography, which help to delineate the cysts and their relationships with adjacent structures. Treatment involves both medical and surgical approaches, with surgery being the primary intervention to prevent complications and recurrence. Scolicidal solutions are essential during surgery to prevent the dissemination of scolices. Conclusions: Primary peritoneal hydatidosis is a rare condition that is usually secondary to liver involvement. It is diagnosed by imaging and treated primarily through surgical intervention. Accurate diagnosis and timely management are crucial to prevent complications and ensure a favourable outcome.

Abstract:Background: Primary splenic hydatidosis, a rare manifestation of Echinococcus granulosus infection, presents unique diagnostic and therapeutic challenges. This study compares spleen-preserving surgeries with total splenectomy for treating primary splenic hydatid cysts, focusing on short- and long-term outcomes in the Jordanian context, a region particularly affected by this condition. Methods: This retrospective analysis was conducted on 18 patients diagnosed with primary splenic hydatid cysts at two Jordanian hospitals from January 2015 to June 2021. Selection criteria included confirmed diagnosis and complete medical records. Surgical approaches, including laparoscopic partial splenectomy, cystectomy, and cyst deroofing, supplemented by albendazole therapy, were compared based on patient demographics, symptoms, surgical details, complications, and recurrence rates. Results: The study group was composed of (n=7, 38.9%) male and (n=11, 61.1%) female patients, with an average age of 33.7 years. Most presented with left upper quadrant pain. Postoperative complications occurred in 22% of patients, with an 11% recurrence rate during follow-up. No significant statistical difference in recurrence rates was observed between spleen-preserving surgeries and total splenectomy. These findings highlight the efficacy of less invasive, spleen-preserving techniques in managing primary splenic hydatidosis, showing comparable outcomes to total splenectomy with minimal impact on recurrence rates. Conclusion: Spleen-preserving surgery offers a viable alternative to total splenectomy in treating primary splenic hydatid cysts. This approach maintains immune functionality and reduces septic risks, especially in pediatric patients. The study underscores the importance of individualized treatment approaches and suggests further research with larger cohorts for more comprehensive insights into managing this rare condition. The limitations of this study include its small sample size and retrospective nature.

Abstract:Background: Hydatid disease, also known as echinococcosis, is a zoonotic parasitic infection caused by the larvae of the Echinococcus tapeworm. It is endemic in various regions worldwide, particularly in rural areas of countries in southern South America, Central Asia, China, parts of Africa, the Mediterranean, and parts of the Middle East. The disease primarily affects the liver (60-70% of cases) and the lungs (10-25% of cases), but it can involve any organ, including the brain, bones, and rarely the pelvic region, as seen in our case report. Hydatid disease typically follows an asymptomatic course in the early stages of the primary infection and may remain so potentially for years or even permanently. If symptoms occur, they depend on various factors, such as the number, size, and location among other factors. Typically, hydatid disease presents with nonspecific symptoms. Common symptoms include abdominal pain, hepatomegaly, as well as anaphylaxis in case of cyst rupture. Extrahepatic intra-abdominal isolated hydatic cyst is a rare finding (6-11%). Case presentation: In our case, a 70 year-old Asian white male presented with right thigh pain radiating to the lower leg, which is an atypical presentation for an extrahepatic intraabdominal hydatid cyst. Primary intraabdominal hydatid cysts involving the pelvic region are relatively rare, and such cases pose diagnostic and management challenges. Conclusion: This case report underscores the challenges in diagnosing and managing extrahepatic intraabdominal hydatid cysts, particularly in atypical presentations. A combination of clinical evaluation, serological studies, and imaging techniques facilitates accurate diagnosis.

Abstract:Introduction and importance: Splenic epidermoid cysts are rare primary congenital cysts composed of an epithelial lining that represent a small proportion of nonparasitic splenic cysts. Despite their infrequency, there is a lack of uniform diagnostic and treatment guidelines for these cysts, emphasizing the need for further research and standardized reporting. Case presentation: A 45-year-old female presented with left upper quadrant abdominal pain, characterized by a pressing sensation. Physical examination revealed a palpable mass in the upper left quadrant. Laboratory test results were normal. Abdominal ultrasonography and CT tomography confirmed a large splenic unilocular cyst consistent with an epidermoid cyst. Surgical intervention involved aspiration of the cystic fluid, partial splenectomy, and pathological examination. The patient's postoperative recovery was uneventful. Clinical discussion: The origin of splenic epidermoid cysts remains unclear, with theories suggesting entrapment of mesothelial cells during embryonic development. The clinical presentation varies with cyst size, often causing abdominal pain and a palpable mass. Diagnostic modalities include ultrasonography and CT scans. Surgical intervention is recommended for symptomatic or suspicious cysts to prevent complications. The chosen approach depends on cyst characteristics and patient factors. This case highlights the challenges and considerations in managing splenic epidermoid the cysts and emphasizes the need for individualized treatment approaches. Conclusion: This case contributes to the understanding of splenic epidermoid cysts and demonstrates a successful subtotal splenectomy as a treatment approach. Further research and standardized guidelines are essential to improve the management of these rare lesions and to provide better insights into their etiology and optimal treatment strategies.

Abstract:Introduction and importance: Splenic cysts are infrequent and most of them are related to parasitic diseases. Splenic epithelial cyst (SEC) is the most common type of primary non-parasitic splenic cysts. Its pathogenesis is not yet clear. Splenic cysts are usually asymptomatic and are detected incidentally during imaging exams or an exploratory laparotomy. Case presentation: Our case is about a primary SEC discovered incidentally. An 18-year-old male patient without any personal and family medical history, presented with symptoms of upper urinary tract infection. Renal ultrasound was performed and found incidentally a solitary cystic lesion in the spleen. A laparoscopic partial splenectomy was made then. The specimen was sent for pathological examination and a diagnosis of primary (epidermoid) epithelial splenic cyst was given. Clinical discussion: SEC is a rare pathology that could mimic other splenic cysts like hydatid cyst. The preoperative diagnosis of SEC can be suspected by ultrasonography, computed tomography or magnetic resonance imaging. However, histopathological examination is mandatory to confirm the diagnosis. A wide range of differential diagnoses is considered when we suspect clinically SEC because of its lower incidence. Conclusion: Actually, spleen-preserving surgery with minimally invasive methods such as laparoscopy is the gold standard for the treatment of SEC despite the risk of recurrence encountered with these techniques. However, different surgical modalities are discussed depending on the size and the location of the cyst and the patient's age.
A Large Splenic Epidermoid Cyst Initially Misdiagnosed as a Hydatid Cyst

Abstract:BACKGROUND Splenic cysts are classified as either parasitic or non-parasitic cysts, with both types sharing similarities in their clinical presentation and imaging findings. Most splenic cysts are of parasitic origin, while non-parasitic splenic cysts are less common. Splenic epidermoid cysts (SECs) are a rare subtype of non-parasitic cysts and compromise only around 10% of them. CASE REPORT In this paper we present a case of 22-year-old man with no significant past clinical history, who presented with non-specific, vague symptoms, including persistent left upper-quadrant pain and discomfort for the last 2 years. A physical examination and extensive laboratory tests were inconclusive. Subsequently, the patient underwent multiple imaging studies including ultrasonography and computed tomography (CT) scan of the abdomen. His ultrasonographic findings were consistent with the diagnosis of hydatid cyst, which was further emphasized by its frequent occurrence in clinical practice, as our country is considered an endemic region. In light of this, he underwent laparoscopic splenectomy following percutaneous cyst drainage. The consequent histopathological examination revealed the diagnosis of splenic epidermoid cysts. CONCLUSIONS When encountering splenic cysts in regions where parasitic infections are endemic, special attention is needed, as physical examination, laboratory tests, and imaging studies alone are insufficient to differentiate among the types of cysts. Histopathological examination remains the diagnostic tool of choice, particularly when imaging findings are inconclusive. Splenectomy, with either a laparoscopic or open approach, is the treatment of choice for splenic cysts to prevent recurrence as well as other potential catastrophic complications.

Abstract:In describing physiochemical properties of interfacial interactions, the interfacial tension is a major parameter. In the case of biological or body fluids, the presence of surface active substances influence the value of the surface tension. Thus, several proposals have been made for methods to investigate the changes in surface tension. Alterations of surface tension (compared to those of the normal values, e.g. of amniotic fluid) may indicate a pathophysiological status. This report presents data of systematically measured surface tensions of a few physiological fluids. Problems of methodology and data interpretation are discussed together with suggestions for new approaches, and comparisons are made with previously reported measurements.

Abstract:Child temperament and parental control were studied as interacting predictors of behavior outcomes in 2 longitudinal samples. In Sample 1, data were ratings of resistant temperament and observed restrictive control in infancy-toddlerhood and ratings of externalizing behavior at ages 7 to 10 years; in Sample 2, data were retrospective ratings of temperament in infancy-toddlerhood, observed restrictive control at age 5 years, and ratings of externalizing behavior at ages 7 to 11 years. Resistance more strongly related to externalizing in low-restriction groups than in high-restriction groups. This was true in both samples and for both teacher- and mother-rated outcomes. Several Temperament x Environment interaction effects have been reported previously, but this is one of very few replicated effects.

Abstract:Aim: Splenic cysts are rare benign lesions of the spleen, usually asymptomatic and incidentally discovered at imaging. Material and methods: A case of huge epidermoid cyst of the spleen on a 16 years old female patient is presented. This case was symptomatic with fullness and palpable mass in the entire abdomen. Results: Initial evaluation with ultrasound and computed tomography established the diagnosis with the greater dimension of the cyst about 25cm. Because of the size and the relation with the hilum of the spleen, splenectomy was performed and any effort to rescue splenic parenchyma was impossible. Discussion-conclusions: Many techniques are reported as treatment options, but the standard of care for nonparasitic splenic cysts is splenectomy. Key word: Benign, Cyst, Epidermoid, Splenic.

Abstract:Introduction: Splenic cysts are rare. They are usually incidentally diagnosed and there is no harmonised treatment pathway. We report a case of a large splenic epidermoid type cyst without history of previous abdominal trauma. Presentation of case: A 23-year old male patient presented with symptoms of upper abdominal pain, nausea and vomiting. Except for a tenderness in the upper and lower left quadrant of the abdomen, the initial examination showed no extraordinary findings. A contrast enhanced computed tomography revealed a large singular splenic cyst displacing neighbouring structures. Echinococcus serology was tested negative. A laparoscopic fenestration of the superficially located splenic cyst was performed. Perioperative course was free of complications. Histopathological analysis of the excisate showed a squamous lining indicating the cyst as epidermoid type. Discussion: Non-parasitic cyst types include traumatic, neoplastic, degenerative and congenital cysts. Due to its considerable size, our patients splenic cyst was diagnosed after occurring symptoms lead to further examination (CT scan). Laparoscopic fenestration of the cyst was chosen as the optimal surgical approach because of the superficial location of the cyst and to preserve residual splenic parenchyma. In the present case, recurrence of the splenic cyst appeared, which left the patient with a total splenectomy as the final treatment choice. Conclusion: Due to the unspecific symptoms, the diagnosis of a splenic cyst can be prolonged. Choosing the adequate surgical technique to avoid complications is crucial. By deepening the understanding of the condition and surgical approaches, we can improve diagnostic and therapeutic management for affected patients.

Abstract:Background: Hydatid disease is a zoonotic infection caused by the species Echinococcus that typically affects the liver. Most liver hydatid cysts are asymptomatic at first, but as the cyst grows larger, symptoms, such as compression effects, start to appear. Ultrasonography and computed tomography scans are the widely used diagnostic tools, and surgery is considered the mainstay of treatment. Case presentation: We present an unusual case of a giant pedunculated hydatid cyst causing inferior vena cava syndrome in a 20-year-old male patient from the Oromo ethnic group from a rural area of the country. Abdominal ultrasound and computed tomography scan confirmed the diagnosis. Our patient underwent radical surgical resection of the cyst and had a good outcome. Conclusion: Hydatid liver cyst diagnosis needs a high index of suspicion for echinococcal etiology when dealing with a giant liver cyst as it results in grave complications without any manifestations.
