Abstract:This retrospective study evaluates the dosimetric benefits of adaptive radiotherapy for head and neck cancer patients. Five patients with node-positive oropharyngeal squamous cell carcinoma were treated with MR-guided radiotherapy using the Elekta Unity MR-Linac, undergoing 3-4 offline adaptive plan modifications during their treatment. This study compared the dose delivered to organs at risk (OARs) in a full offline adaptive approach versus an approach accounting only for daily setup. Results demonstrated a reduction in mean dose to critical structures in the offline adaptive arm. For example, the pharynx avoidance structure showed mean dose reductions ranging from 1.4 Gy to 3.6 Gy, and the left parotid gland exhibited reductions from 1.5 Gy to 1.9 Gy. Overall, offline adaptive radiotherapy reduced the mean cumulative dose to OARs in 19 of the 23 evaluated structures. Despite some instances of higher doses, the offline adaptive approach generally resulted in lower cumulative doses, emphasizing its potential to mitigate radiation-induced side effects. These findings suggest that offline adaptive radiotherapy has the potential to enhance treatment efficacy by better accommodating anatomical changes during therapy, thereby improving patient outcomes and reducing treatment-related morbidity.

Abstract:Background/Objectives: Advancements in radiotherapy technology now enable the delivery of ablative doses to targets in the upper urinary tract, including primary renal cell carcinoma (RCC) or upper tract urothelial carcinomas (UTUC), and secondary involvement by other histologies. Magnetic resonance imaging-guided linear accelerators (MR-Linacs) have shown promise to further improve the precision and adaptability of stereotactic body radiotherapy (SBRT). Methods: This single-institution retrospective study analyzed 34 patients (31 with upper urinary tract non-metastatic primaries [RCC or UTUC] and 3 with metastases of non-genitourinary histology) who received SBRT from August 2020 through September 2024 using a 1.5 Tesla MR-Linac system. Treatment plans were adjusted by using [online settings] for "adapt-to-position" (ATP) and "adapt-to-shape" (ATS) strategies for anatomic changes that developed during treatment; compression belts were used for motion management. Results: The median duration of treatment was 56 min overall and was significantly shorter using the adapt-to-position (ATP) (median 54 min, range 38–97 min) in comparison with adapt-to-shape (ATS) option (median 80, range 53–235 min). Most patients (77%) experienced self-resolving grade 1–2 acute radiation-induced toxicity; none had grade ≥ 3. Three participants (9%) experienced late grade 1–2 toxicity, potentially attributable to SBRT, with one (3%) experiencing grade 3. Conclusions: We conclude that MR-Linac-based SBRT, supported by online plan adaptation, is a feasible, safe, and highly precise treatment modality for the definitive management of select upper urinary tract lesions.

Abstract:the advancement of radiotherapy in the past 50 years. Soon after the clinical introduction of computerized tomography (CT) it was employed in radiotherapy, in order to visualize internal organs and to calculate the volumes of the tumor target and internal organs close to the tumor which could be affected by the radiation dose (1). 3D imaging enabled the first studies of extracranial extreme hypofractionation, commonly called stereotactic body radiation therapy (SBRT) or stereotactic ablative radiation therapy (SABR). Timmerman et al pioneered SBRT in early stage lung cancer (2). The success of SABR for lung cancer was made possible by the ability to accurately identify the tumor on CT imaging of the thorax. Further incremental improvements came with realtime imaging on the radiotherapy (RT) treatment table (e.g cone beam CT) ensuring control and accurate delivery of SBRT plans.The application of SABR to mediastinal, abdominal and pelvic sites requires accurate and precise imaging of these areas as provided by magnetic resonance imaging (MRI). The first commercially available clinical MRI radiotherapy system began treating patients on a radioactive cobalt-based system in 2014 (3). Over the last 10 years there have been gradual developments in the delivery of MR guided RT, as the successful incorporation of Linac-based radiation into the ViewRay system and the development of rival machines such as the Elekta Unity. There are now over 100 installations of MR guided RT systems in the world. A PubMed search shows a steady increase in publications regarding MR Linac reaching over 200 per year in recent years. This volume is the second in a series of frontiers dedicated to advances in the clinical use of MR guided radiotherapy. The articles can be divided into 2 ovelapping broad categories, clinical experience and physics with quality assurance.A single institution series of pancreatic cancer patients receiving MRgSBRT to the pancreas with 5-10 Gy fractions shows the range of patients being treated today. The great majority have locally advanced inoperable disease. The series includes patients with locally recurrent disease following prior surgery or fractionated radiation and a few patients with oligo-progressive disease treated to both the primary and the oligometastasis. One of the difficulties of treating pancreatic tumors with MRgSBRT is the visualization of surrounding small bowel and real-time changes in organs at risk around the target tumor. Another paper reports on improved inter-observer contouring accuracy of pancreatic cancer MRI with the use of butylscopolamine prior to MRI.A major advantage of MRguided radiotherapy is the ability to have real time tumor tracking to ensure coverage of the target and avoid toxicity of adjacent organs. Two papers describe novel methods of tracking. A case report of a nodal recurrence of endometrial cancer used real time tracking of isodose lines to deliver daily isotoxic doses. A single institute series of intrathoracic tumors who received MRgSBRT due to proximity to organs at risk showed the importance of real time tracking. A further study looked at gating protocols for apical lung lesions. Cardiac lesions close to the right ventricles are especially difficult to treat with focal radiotherapy and in this series there is a report of 2 patients safely treated with a dose of 30 Gy in 3 fractions of 10 Gy each.A couple of papers described aspects in dosimetry of MR guided SBRT. One paper looked at techniques of delineation of gas packets and the impact of these gas packets using various dose and gas volume measurement protocols. Another paper reported on the benefits of online adaptation in the treatment of prostate cancer with MRgSBRT.The addition of non-proprietary software upgrades to MRgRT planning programs requires meticulous QA. An online QA program to evaluate such upgrades is described.In summary, the several papers in this volume describe a wide range of issues involved in implementing MRgSBRT and serve as an important addition to existing knowledge.

Abstract:MR-linac devices offer the potential for advancements in radiotherapy (RT) treatment of head and neck cancer (HNC) by using daily MR imaging performed at the time and setup of treatment delivery. This article aims to present a review of current adaptive RT (ART) methods on MR-Linac devices directed towards the sparing of organs at risk (OAR) and a view of future adaptive techniques seeking to improve the therapeutic ratio. This ratio expresses the relationship between the probability of tumor control and the probability of normal tissue damage and is thus an important conceptual metric of success in the sparing of OARs. Increasing spatial conformity of dose distributions to target volume and OARs is an initial step in achieving therapeutic improvements, followed by the use of imaging and clinical biomarkers to inform the clinical decision-making process in an ART paradigm. Pre-clinical and clinical findings support the incorporation of biomarkers into ART protocols and investment into further research to explore imaging biomarkers by taking advantage of the daily MR imaging workflow. A coherent understanding of this road map for RT in HNC is critical for directing future research efforts related to sparing OARs using image-guided radiotherapy (IGRT).
Dosimetric comparison of magnetic resonance-guided radiation therapy, intensity-modulated proton therapy and volumetric-modulated arc therapy for distal esophageal cancer

Abstract:Advances in radiotherapy (RT) technologies permit significant decreases in the dose delivered to organs at risk (OARs) for patients with esophageal cancer (EC). Novel RT modalities such as proton beam therapy (PBT) and magnetic resonance-guided radiotherapy (MRgRT), as well as motion management techniques including breath hold (BH) are expected to further improve the therapeutic ratio. However, to our knowledge, the dosimetric benefits of PBT vs MRgRT vs volumetric-modulated arc therapy (VMAT) have not been directly compared for EC. We performed a retrospective in silico evaluation using the images and datasets of nine distal EC patients who were treated at our institution with a 0.35-Tesla MR linac to 50.4 Gy in 28 fractions in mid-inspiration BH (BH-MRgRT). Comparison free-breathing (FB) intensity-modulated PBT (FB-IMPT) and FB-VMAT plans were retrospectively created using the same prescription dose, target volume coverage goals, and OAR constraints. A 5 mm setup margin was used for all plans. BH-IMPT and BH-VMAT plans were not evaluated as they would not reflect our institutional practice. Planners were blinded to the results of the treatment plans created using different radiation modalities. The primary objective was to compare plan quality, target volume coverage, and OAR doses. All treatment plans met pre-defined target volume coverage and OAR constraints. The median conformity and homogeneity indices between FB-IMPT, BH-MRgRT and FB-VMAT were 1.13, 1.25, and 1.43 (PITV) and 1.04, 1.15, 1.04 (HI), respectively. For FB-IMPT, BH-MRgRT and FB-VMAT the median heart dose metrics were 52.8, 79.3, 146.8 (V30Gy, cc), 35.5, 43.8, 77.5 (V40Gy, cc), 16.9, 16.9, 32.5 (V50Gy, cc) and 6.5, 14.9, 17.3 (mean, Gy), respectively. Lung dose metrics were 8.6, 7.9, 18.5 (V20Gy, %), and 4.3, 6.3, 11.2 (mean, Gy), respectively. The mean liver dose (Gy) was 6.5, 19.6, 22.2 respectively. Both FB-IMPT and BH-MRgRT achieve substantial reductions in heart, lung, and liver dose compared to FB-VMAT. We plan to evaluate dosimetric outcomes across these RT modalities assuming consistent use of BH.

Abstract:The treatment of central and ultracentral lung tumors with radiotherapy remains an ongoing clinical challenge. The risk of Grade 5 toxicity with ablative radiotherapy doses to these high-risk regions is significant as shown in recent prospective studies. Magnetic resonance (MR) image-guided adaptive radiotherapy (MRgART) is a new technology and may allow the delivery of ablative radiotherapy to these high-risk regions safely. MRgART is able to achieve this by utilizing small treatment margins, real-time gating/tracking and on-table plan adaptation to maintain dose to the tumor but limit dose to critical structures. The process of MRgART is complex and has nuances and challenges for the treatment of lung tumors. We outline the critical steps needed for appropriate delivery of MRgART for lung tumors safely and effectively.

Dosimetric Evaluations of Different Accelerator Based Treatment Modalities for Curative Intent Stereotactic Body Radiation Therapy of Isolated Thoracic Spinal Metastases

Abstract:To investigate the dosimetric characteristics of 4 linear accelerator (LINAC) based stereotactic body radiation therapy (SBRT) modalities, CyberKnife (CK, Accuray), helical tomotherapy (HT, Accuray TomoTherapy system), double-arc volumetric modulated arc therapy (VMAT) performed with RapidArc system (RA, Varian) and step-and-shoot intensity modulated radiation therapy (sIMRT, Elekta Synergy S system) on isolated thoracic spinal metastasis . CK, HT, RA and sIMRT plans were cross compared in 10 patients with 6 body type and 6 body + pedicle type isolated thoracic metastases, with curative-intent prescription dose of 33 Gy/3F and 40 Gy/5F to covering ≥90% of the target volume, respectively. Different dosimetric indices, beam-on-time, and MUs were evaluated in details In ensuring the dose-volume constraints for cord and esophagus of the premise, CK, HT and RA achieved a sharp gradient index (GI) and a small penumbra volume compared to sIMRT. RA achieved a better conformity index (CI) compared to CK, HT and sIMRT. CK had a heterogeneous dose distribution in the target volume. CK occupied the longest beam-on-time and the highest MUs, followed by HT, sIMRT and RA with shortest and the least MUs. For the body type metastasis, CK, HT and RA satisfied the target coverage of ≥90% in all the 6 cases, but only in 3 cases (50%) by sIMRT. For the body + pedicle type metastasis, HT satisfied the criterion of the target coverage criterion in 4 out of the 6 cases, and reached target coverage of 89.0% in another case. The criterion of the target coverage was satisfied in 2 cases by CK and RA, but only in 1 case by sIMRT. For curative-intent SBRT with isolated thoracic spinal metastasis, RA is the common choice for the body type metastasis with current LINAC systems, then followed by CK or HT techniques if available; HT is the preferential choice for the body + pedicle type metastasis. This study suggests further clinical investigations to differential individual benefits.

Abstract:Introduction Magnetic resonance imaging (MRI)-guided radiation therapy has proven to provide many benefits such as real-time tracking, dose escalation, and the ability to perform online adaptive therapy. The objective of this study is to compare curative treatment plans for glioblastoma tumors on a low-field MR-guided linac vs a C-arm linac and evaluate if they are comparable in terms of coverage, organ at risk sparing, delivery time, and deliverability. Methods This is a retrospective study that consisted of 15 previously treated patients who received radiation therapy for glioblastoma on a C-arm linac. The CT simulation data used for the original clinical plans was imported into the MR-linac treatment planning system (TPS) and utilized for MR-linac plan generation. The plans were evaluated utilizing the dose volumetric histogram (DVH) and isodose lines, then compared in terms of plan quality consisting of PTV coverage, dose distributions, and OAR constraints. Statistical analysis was performed to compare differences between the two planning techniques. QA was performed on a subset of the plans to verify deliverability. Results Plans generated on the MR-linac were more heterogenous compared to C-arm linac plans. A statistically significant difference was found in the homogeneity index (HI) and the PTV V105% volume (cc) values. The volume of the normal brain receiving 30 Gy also showed a statistically significant ( p = 0.0479) difference, where on average an additional 41.5 cc's of the normal brain tissue received 30 Gy in the MR-linac plans. The maximum dose to the normal brain structure also increased in the MR-linac plans on average by 2.6 Gy ( p = 0.0002). Similarly, the average maximum dose to the scalp 4 mm structure was 6.5 Gy higher in the MR-linac plans compared to C-arm linac plans ( p = 0.0103). The total MU's were higher in the MR-linac plans compared to the C-arm linac plans ( p = 0.0015). Conclusions Both MR-linac and C-arm linac plans met constraints for PTV coverage and OAR sparing, were deliverable, and resulted to be clinically acceptable. However, our study showed that MR-linac plans were not as conformal or as homogenous as C-arm linac plans utilizing noncoplanar beams.

Abstract:The host-guest assembly of CB7 with a series of alkyl(trimethyl)ammonium (C(n)TA(+)) surfactants of different chain lengths (n=6-18) has been studied. The complexation behaviour was investigated by NMR spectroscopy, isothermal titration calorimetry and kinetics measurements. The combined results of these techniques provided evidence for the formation of 1:1 inclusion and 2:1 external complexes in the cases of C(n)TA(+) with n=12-18. The binding constants for the 1:1 complexes are independent of the alkyl chain length of the surfactant, whereas a relationship between K(2:1) and the chain length of the surfactant was found for the 2:1 complexes.
Inhibitors of cyclooxygenase augment serotonergic responsiveness in canine coronary arteries.

Abstract:Experiments were designed to determine the role of products of cyclooxygenase in contractions of coronary smooth muscle evoked by serotonin. Rings of canine coronary artery without endothelium were suspended in organ chambers filled with modified Krebs-Ringer bicarbonate solution. Serotonin caused concentration-dependent contractions followed by secondary relaxations at higher doses. Indomethacin and meclofenamate augmented both the contraction and the relaxation. Indomethacin did not affect contractions evoked by increasing concentrations of either phenylephrine, prostaglandin F2 alpha, or potassium chloride. Propranolol did not affect the concentration-response curve to serotonin under control conditions; it prevented the facilitated contraction to the monoamine but not the augmented secondary relaxation caused by the inhibitors of cyclooxygenase. These results suggest that endogenous prostanoids simultaneously inhibit the contractile process and brake relaxations induced by higher concentrations of serotonin. As a consequence, inhibitors of prostanoid formation facilitate the vasospastic component of the response to the monoamine in large coronary arteries. For unknown reasons, propranolol prevents this facilitation.
Magnetic resonance guided adaptive post prostatectomy radiotherapy: Accumulated dose comparison of different workflows

Abstract:Purpose The aim of this study was to assess the use of magnetic resonance guided adaptive radiotherapy (MRgART) in the post‐prostatectomy setting; comparing dose accumulation for our initial seven patients treated with fully adaptive workflow on the Unity MR‐Linac (MRL) and with non‐adaptive plans generated offline. Additionally, we analyzed toxicity in patients receiving treatment. Methods Seven patients were treated with MRgART. The prescription was 70–72 Gy in 35–36 fractions. Patients were treated with an adapt to shape (ATS) technique. For each clinically delivered plan, a non‐adaptive plan based upon the reference plan was generated and compared to the associated clinically delivered plan. A total of 468 plans were analyzed. Concordance Index of target and Organs at Risk (OARs) for each fraction with reference contours was analyzed. Acute toxicity was then assessed at six‐months following completion of treatment with Common Terminology for Adverse Events (CTCAE) Toxicity Criteria. Results A total of 246 fractions were clinically delivered to seven patients; 234 fractions were delivered via MRgART and 12 fractions delivered via a traditional linear accelerator due to machine issues. Pre‐treatment reference plans met CTV and OAR criteria. PTV coverage satisfaction was higher in the clinically delivered adaptive plans than non‐adaptive comparison plans; 42.93% versus 7.27% respectively. Six‐month CTCAE genitourinary and gastrointestinal toxicity was absent in most patients, and mild‐to‐moderate in a minority of patients (Grade 1 GU toxicity in one patient and Grade 2 GI toxicity in one patient). Conclusions Daily MRgART treatment consistently met planning criteria. Target volume variability in prostate bed treatment can be mitigated by using MRgART and deliver satisfactory coverage of CTV whilst minimizing dose to adjacent OARs and reducing toxicity

Combined Megavoltage and Contrast-Enhanced Radiotherapy as an Intrafraction Motion Management Strategy in Lung SBRT

Abstract:Using Monte Carlo simulation and a realistic patient model, it is shown that the volume of healthy tissue irradiated at therapeutic doses can be drastically reduced using a combination of standard megavoltage and kilovoltage X-ray beams with a contrast agent previously loaded into the tumor, without the need to reduce standard treatment margins. Four-dimensional computed tomography images of 2 patients with a centrally located and a peripherally located tumor were obtained from a public database and subsequently used to plan robotic stereotactic body radiotherapy treatments. Two modalities are assumed: conventional high-energy stereotactic body radiotherapy and a treatment with contrast agent loaded in the tumor and a kilovoltage X-ray beam replacing the megavoltage beam (contrast-enhanced radiotherapy). For each patient model, 2 planning target volumes were designed: one following the recommendations from either Radiation Therapy Oncology Group (RTOG) 0813 or RTOG 0915 task group depending on the patient model and another with a 2-mm uniform margin determined solely on beam penumbra considerations. The optimized treatments with RTOG margins were imparted to the moving phantom to model the dose distribution that would be obtained as a result of intrafraction motion. Treatment plans are then compared to the plan with the 2-mm uniform margin considered to be the ideal plan. It is shown that even for treatments in which only one-fifth of the total dose is imparted via the contrast-enhanced radiotherapy modality and with the use of standard treatment margins, the resultant absorbed dose distributions are such that the volume of healthy tissue irradiated to high doses is close to what is obtained under ideal conditions

Abstract:Purpose Long treatment sessions are a limitation within MRIgART. This work aims for significantly enhancing the delivery efficiency on the MR-linac by introducing dedicated optimization and delivery techniques for VMAT. VMAT plan and delivery quality during MRIgART is compared to step-and-shoot IMRT for prostate SBRT. Methods and Materials Ten prostate patients previously treated on a 1.5T MR-linac were retrospectively replanned to 36.25 Gy in five fractions using step-and-shoot IMRT and the clinical Hyperion optimizer within Monaco (Hyp-IMRT), the same optimizer with a VMAT technique (Hyp-VMAT), and a research-based optimizer with VMAT (OFL+PGD-VMAT). The plans were then adapted onto each daily MRI dataset using two different optimization strategies to evaluate the ATP workflow: "optimize weights" (IMRT-Weights and VMAT-Weights) and "optimize shapes" (IMRT-Shapes and VMAT-Shapes). Treatment efficiency was evaluated by measuring optimization time, delivery time, and total time (optimization + delivery). Plan quality was assessed by evaluating OAR sparing. Ten patient plans were measured using a modified linac control system to assess delivery accuracy via a gamma analysis (2%/2mm). Delivery efficiency was calculated as average dose rate divided by maximum dose rate. Results For Hyp-VMAT and OFL+PGD-VMAT the total time was reduced by 124 ± 140 seconds (p = .020) and 459 ± 110 seconds (p < 0.001), respectively as compared to the clinical Hyp-IMRT group. Speed enhancements were also measured for ATP with reductions in total time of 404 ± 55 (p<0.001) for VMAT-Weights as compared to the clinical IMRT-Shapes group. Bladder and rectum DVH points were within 1.3 % or 0.8 cc for each group. All VMAT plans had gamma passing rates greater than 96 %. The delivery efficiency of VMAT plans was 89.7 ± 2.7 % compared to 50.0 ± 2.2 % for clinical IMRT. Conclusions Incorporating VMAT into MRIgART will significantly reduce treatment session times while maintaining equivalent plan quality.

Abstract:PURPOSE: Volumetric modulated arc therapy (VMAT), a novel technique, employs a linear accelerator to conduct dynamic modulation rotation radiotherapy. The goal of this study was to compare VMAT with helical tomotherapy (HT) and step-and-shoot intensity-modulated radiation therapy (IMRT) for nasopharyngeal carcinoma (NPC) patients with regard to the sparing effect on organs at risk (OARs), dosimetric quality, and efficiency of delivery.MATERIALS AND METHODS: Twenty patients with NPC treated by HT were re-planned by VMAT (two arcs) and IMRT (7-9 fields) for dosimetric comparison. The target area received three dose levels (70, 60, and 54 Gy) in 33 fractions using simultaneous integrated boosts technique. The Philips Pinnacle Planning System 9.0 was adopted to design VMAT, using SmartArc as the planning algorithm. For a fair comparison, the planning target volume (PTV) coverage of the 3 plans was normalized to the same level. Dosimetric comparisons between VMAT, HT, and IMRT plans were analyzed to evaluate (1) coverage, homogeneity, and conformity of PTV, (2) sparing of OARs, (3) delivery time, and (4) monitor units (MUs).RESULTS: The VMAT, HT, and IMRT plans had similar PTV coverage with an average of 96%. There was no significant difference between VMAT and HT in homogeneity, while the homogeneity indices of VMAT (1.06) and HT (1.06) were better than IMRT plans (1.07, p<0.05). HT plans provided a better conformity index (1.17) than VMAT (1.28, p=0.01) and IMRT (1.36, p=0.02). When compared with IMRT, VMAT and HT had a better sparing effect on brain stem and spinal cord (p<0.05). The effect of parotid sparing was similar between VMAT (mean=26.3 Gy) and HT (mean=27.5 Gy), but better than IMRT (mean=31.3 Gy, p<0.01). The delivery time per fraction for VMAT (5.7 min) were much lower than for HT (9.5 min, p<0.01) and IMRT (9.2 min, p<0.01).CONCLUSIONS: Our results indicate that VMAT provides better sparing of normal tissue, homogeneity, and conformity than IMRT, and shorter delivery time than HT.

Abstract:In recent years, MRI-guided radiotherapy (MRgRT) has taken an increasingly important position in image-guided radiotherapy (IGRT). Magnetic resonance imaging (MRI) offers superior soft tissue contrast in anatomical imaging compared to computed tomography (CT), but also provides functional and dynamic information with selected sequences. Due to these benefits, in current clinical practice, MRI is already used for target delineation and response assessment in patients with head and neck squamous cell carcinoma (HNSCC). Because of the close proximity of target areas and radiosensitive organs at risk (OARs) during HNSCC treatment, MRgRT could provide a more accurate treatment in which OARs receive less radiation dose. With the introduction of several new radiotherapy techniques (i.e., adaptive MRgRT, proton therapy, adaptive cone beam computed tomography (CBCT) RT, (daily) adaptive radiotherapy ensures radiation dose is accurately delivered to the target areas. With the integration of a daily adaptive workflow, interfraction changes have become visible, which allows regular and fast adaptation of target areas. In proton therapy, adaptation is even more important in order to obtain high quality dosimetry, due to its susceptibility for density differences in relation to the range uncertainty of the protons. The question is which adaptations during radiotherapy treatment are oncology safe and at the same time provide better sparing of OARs. For an optimal use of all these new tools there is an urgent need for an update of the target definitions in case of adaptive treatment for HNSCC. This review will provide current state of evidence regarding adaptive target definition using MR during radiotherapy for HNSCC. Additionally, future perspectives for adaptive MR-guided radiotherapy will be discussed.

Abstract:Oxidative stress and apoptosis are the major mechanisms that induce dopaminergic cell death. Our study investigates the protective effects of atractylenolide-I (ATR-I) on 1-methyl-4-phenylpyridinium (MPP+)-induced cytotoxicity in human dopaminergic SH-SY5Y cells, as well as its underlying mechanism. Our experimental data indicates that ATR-I significantly inhibits the loss of cell viability induced by MPP+ in SH-SY5Y cells. To further unravel the mechanism, we examined the effect of ATR-I on MPP+-induced apoptotic cell death characterized by an increase in the Bax/Bcl-2 mRNA ratio, the release of cytochrome-c, and the activation of caspase-3 leading to elevated levels of cleaved poly(ADP-ribose) polymerase (PARP) resulting in SH-SY5Y cell death. Our results demonstrated that ATR-I decreases the level of pro-apoptotic proteins induced by MPP+ and also restored Bax/Bcl-2 mRNA levels, which are critical for inducing apoptosis. In addition, ATR-I demonstrated a significant increase in the protein expression of heme-oxygenase in MPP+-treated SH-SY5Y cells. These results suggest that the pharmacological effect of ATR-I may be, at least in part, caused by the reduction in pro-apoptotic signals and also by induction of anti-oxidant protein.

Abstract:Background Magnetic resonance image only (MRI‐only) simulation for head and neck (H&N) radiotherapy (RT) could allow for single‐image modality planning with excellent soft tissue contrast. In the MRI‐only simulation workflow, synthetic computed tomography (sCT) is generated from MRI to provide electron density information for dose calculation. Bone/air regions produce little MRI signal which could lead to electron density misclassification in sCT. Establishing the dosimetric impact of this error could inform quality assurance (QA) procedures using MRI‐only RT planning or compensatory methods for accurate dosimetric calculation. Purpose The aim of this study was to investigate if Hounsfield unit (HU) voxel misassignments from sCT images result in dosimetric errors in clinical treatment plans. Methods Fourteen H&N cancer patients undergoing same‐day CT and 3T MRI simulation were retrospectively identified. MRI was deformed to the CT using multimodal deformable image registration. sCTs were generated from T1w DIXON MRIs using a commercially available deep learning‐based generator (MRIplanner, Spectronic Medical AB, Helsingborg, Sweden). Tissue voxel assignment was quantified by creating a CT‐derived HU threshold contour. CT/sCT HU differences for anatomical/target contours and tissue classification regions including air ( 500 HU) were quantified. t‐test was used to determine if sCT/CT HU differences were significant. The frequency of structures that had a HU difference > 80 HU (the CT window‐width setting for intra‐cranial structures) was computed to establish structure classification accuracy. Clinical intensity modulated radiation therapy (IMRT) treatment plans created on CT were retrospectively recalculated on sCT images and compared using the gamma metric. Results The mean ratio of sCT HUs relative to CT for air, adipose tissue, soft tissue, spongy and cortical bone were 1.7 ± 0.3, 1.1 ± 0.1, 1.0 ± 0.1, 0.9 ± 0.1 and 0.8 ± 0.1 (value of 1 indicates perfect agreement). T‐tests (significance set at t = 0.05) identified differences in HU values for air, spongy and cortical bone in sCT images compared to CT. The structures with sCT/CT HU differences > 80 HU of note were the left and right (L/R) cochlea and mandible (>79% of the tested cohort), the oral cavity (for 57% of the tested cohort), the epiglottis (for 43% of the tested cohort) and the L/R TM joints (occurring > 29% of the cohort). In the case of the cochlea and TM joints, these structures contain dense bone/air interfaces. In the case of the oral cavity and mandible, these structures suffer the additional challenge of being positionally altered in CT versus MRI simulation (due to a non‐MR safe immobilizing bite block requiring absence of bite block in MR). Finally, the epiglottis HU assignment suffers from its small size and unstable positionality. Plans recalculated on sCT yielded global/local gamma pass rates of 95.5% ± 2% (3 mm, 3%) and 92.7% ± 2.1% (2 mm, 2%). The largest mean differences in D95, Dmean, D50 dose volume histogram (DVH) metrics for organ‐at‐risk (OAR) and planning tumor volumes (PTVs) were 2.3% ± 3.0% and 0.7% ± 1.9% respectively. Conclusions In this cohort, HU differences of CT and sCT were observed but did not translate into a reduction in gamma pass rates or differences in average PTV/OAR dose metrics greater than 3%. For sites such as the H&N where there are many tissue interfaces we did not observe large scale dose deviations but further studies using larger retrospective cohorts are merited to establish the variation in sCT dosimetric accuracy which could help to inform QA limits on clinical sCT usage.

Abstract:In a recent study [8] we reported the results for hepatitis-associated antigen (HAA) in 241 patients (46 SHl, 55 IHa and 140 sporadic cases of unknown acute viral hepatitis) at various stages of the disease. Patients were selected on the ground of classical epidemiological criteria. The results we obtained, divided weekly from the onset of the disease, are as follows: in the 1st week 62 of 65 cases were Australia positive, in the 2nd week 57 of 66, in the 3rd week 28 of 45 and in the 4th week only 30 of 60 (table I). The positivity percentage is shown in figure 1. It is seen that the HAA positivity decreases in relation t o the time after the onset of symptoms: from 95.4% in the 1st week to 46.1% in the 4th week and later. The immunological reaction has a great diagnostic value at the onset of the disease; the positivity index decreases rapidly when the test is performed late in the disease, and no difference in the frequency of detectable HAA was noted among 46 SH, 55 IH, and 140 sporadic cases of unknown acute viral hepatitis. Our positivity percentage is remarkably higher than that referred by many other authors [ 2 4 , 12, 14-16, 20, 211. We thought it could be due to different antisera wed and we compared our antisera to those used by other investigators by taking 40 cases of hepatitis and testing them with different antisera. One antiserum, that we commonly use, is able to detect 70% of the HAA, while the positivity percentage with other antisera is lower (from 30 t o 40%). Moreover, this antiserum is able t o detect the HAA where other antisera are not able to; the opposite never happened.

Abstract:Objectives This study aimed to show the advantages of each stereotactic radiosurgery (SRS) treatment option for single small brain metastasis among Gamma Knife (GK), Cone-based VMAT (Cone-VMAT), and MLC-based CRT (MLC-CRT) plans. Materials and Methods GK, Cone-VMAT, and MLC-CRT SRS plans were retrospectively generated for 11 patients with single small brain metastasis whose volume of gross tumor volume (GTV) ranged from 0.18 to 0.76 cc (median volume 0.60 cc). Dosimetric parameters, treatment efficiency, and biological parameters of the three techniques were compared and evaluated. The metric variation with the planning target volume (PTV) was also studied. Results The conformity index (CI) was similar in GK and MLC-CRT plans, higher than Cone-VMAT. Cone-VMAT achieved comparable volume covered by 12 Gy (V12) and gradient index (GI) as GK, lower than MLC-CRT. The heterogeneity index (HI) of GK, Cone-VMAT, and MLC-CRT decreased sequentially. GK gave the lowest volume covered by 3 Gy (V3) and 6 Gy (V6), while MLC-CRT got the highest. The beam-on time and treatment time of GK, Cone-VMAT, and MLC-CRT decreased in turn. Tumor control probability (TCP) of all three SRS plans was greater than 98%, and normal tissue complication probability (NTCP) of all organs at risk (OARs) was below 0.01%. GK and Cone-VMAT resulted in superior TCP and NTCP of the normal brain tissue than MLC-CRT. The relative value of Cone-VMAT and GK for all metrics hardly changed with the target volume. Except for the unchanged HI and TCP, the other results of MLC-CRT with respect to GK improved as the target volume increased. MLC-CRT could produce higher CI than GK and Cone-VMAT when the target volume increased above 2 and 1.44 cc, respectively. Conclusion For single small brain metastases, Cone-VMAT may be used as an alternative to GK-free centers. In addition to the advantage of short treatment time, MLC-CRT showed superiority in CI as the target volume increased. Treatment centers can choose appropriate SRS technique on a case-by-case basis according to institutional conditions and patients’ individual needs.
