Abstract:Background: Generative artificial intelligence (AI) tools like ChatGPT have emerged as potentially valuable technologies to augment human expertise in healthcare. However, uncertainties remain regarding appropriate clinical applications and limitations. This review synthesizes current evidence on using generative AI for clinical decision support, patient data processing, and medical education. Methods: A systematic search of Web of Science, Scopus, and ProQuest databases identified 33 relevant studies published in 2023 examining ChatGPT for healthcare uses. Two reviewers extracted data on study characteristics, AI system details, key results, and authors' conclusions. Evidence was synthesized qualitatively using a comparative analysis approach. Results: Supervised use of ChatGPT-generated simulations appeared beneficial for clinical training, but oversight was critical. Numerous studies found risks in relying on ChatGPT's clinical suggestions given frequent factual errors, outdated recommendations, and inappropriate advice. However, ChatGPT demonstrated potential for enhancing workflows via medical documentation automation. Conclusions: While showing promise for constrained uses like supervised education and documentation, findings caution against open-ended ChatGPT integration in clinical practice currently. Additional large-scale comparative effectiveness research is imperative to establish evidence-based implementation guidance. Responsible translation requires governance, validation against literature, and focus on human-AI collaboration versus replacement. Further inquiry can illuminate best practices for balancing innovation and safety.

Abstract:Background Generative artificial intelligence (GAI) is revolutionizing healthcare with solutions for complex challenges, enhancing diagnosis, treatment, and care through new data and insights. However, its integration raises questions about applications, benefits, and challenges. Our study explores these aspects, offering an overview of GAI's applications and future prospects in healthcare. Methods This scoping review searched Web of Science, PubMed, and Scopus using a data extraction form. The selection of studies involved screening titles, reviewing abstracts, and examining full texts, adhering to the PRISMA-ScR guidelines throughout the process. Results From 1406 articles across three databases, 109 met inclusion criteria after screening and deduplication. Nine GAI models were utilized in healthcare, with ChatGPT (n = 102, 74 %), Google Bard (Gemini) (n = 16, 11 %), and Microsoft Bing AI (n = 10, 7 %) being the most frequently employed. A total of 24 different applications of GAI in healthcare were identified, with the most common being "offering insights and information on health conditions through answering questions" (n = 41) and "diagnosis and prediction of diseases" (n = 17). In total, 606 benefits and challenges were identified, which were condensed to 48 benefits and 61 challenges after consolidation. The predominant benefits included "Providing rapid access to information and valuable insights" and "Improving prediction and diagnosis accuracy", while the primary challenges comprised "generating inaccurate or fictional content", "unknown source of information and fake references for texts", and "lower accuracy in answering questions". Conclusion This scoping review identified the applications, benefits, and challenges of GAI. This synthesis offers a crucial overview of GAI's potential to revolutionize healthcare, emphasizing the imperative to address its limitations.

Advancing the Democratization of Generative Artificial Intelligence in Healthcare: a Narrative Review

Abstract:Background and Objective: The emergence of ChatGPT-like generative artificial intelligence (GenAI) has dramatically transformed the healthcare landscape, bringing new hope for the democratization of artificial intelligence (AI) in healthcare-a topic that has not been comprehensively reviewed. This review aims to analyze the reasons propelling the democratization of healthcare GenAI, outline the initial evidence in the literature, and propose future directions to advance GenAI democratization. Methods: We conducted a deep literature search for GenAI studies using Google Scholar, PubMed, ChatGPT, Journal of the American Medical Association ( JAMA ), Nature, , Springer Link, and Journal of Medical Internet Research (JMIR). JMIR ). We performed an abstraction analysis on the nature of GenAI versus traditional AI and the applications of GenAI in medical education and clinical care. Key Content and Findings: (I) A detailed comparison of traditional and GenAI in healthcare reveals that large language model (LLM)-based GenAI's unprecedent general-purpose capabilities and natural language interaction ability, coupled with its free public availability, make GenAI ideal for democratization in healthcare. (II) We have identified plenty of initial evidence for GenAI democratization in medical education and clinical care, marking the start of the emerging trend of GenAI democratization in a host of impactful applications. Applications in medical education include medical exam preparation, medical teaching and training, and simulation. Applications in clinical care include diagnosis assistance, disease risk prediction, new generalist chatbots, treatment decision support, surgery support, medical image analysis, patient communication, physician communication, documentation automation, clinical trial automation, informatics tasks automation, and specialized or custom LLMs. (III) Responsible AI is essential for the future of healthcare GenAI. National initiatives and regulatory efforts are working to ensure safety, efficacy, accountability, equity, security and privacy are built into healthcare GenAI. Responsible GenAI requires a human-machine collaboration approach, where AI augments human expertise rather than replaces it. Conclusions: The democratization of GenAI in healthcare has just begun, driven by the nature of GenAI and guided by the principle of human-machine collaboration. To further advance GenAI democratization, we propose three key future directions: integrating GenAI in medical education curricula, democratizing GenAI clinical evaluation research, and building learning health systems (LHS) with GenAI for system- level enforcement of democratization. Democratizing GenAI in healthcare will revolutionize medicine and significantly impact care delivery and health policies.

Abstract:Introduction: The emergence and application of generative artificial intelligence/large language models (hereafter GenAI LLMs) have the potential for significant impact on the healthcare industry. However, there is currently a lack of systematic research on GenAI LLMs in healthcare based on reliable data. This article aims to conduct an exploratory study of the application of GenAI LLMs (i.e., ChatGPT) in healthcare from the perspective of digital media (i.e., online news), including the application scenarios, potential opportunities, and challenges. Methods: This research used thematic qualitative text analysis in five steps: firstly, developing main topical categories based on relevant articles; secondly, encoding the search keywords using these categories; thirdly, conducting searches for news articles via Google ; fourthly, encoding the sub-categories using the elaborate category system; and finally, conducting category-based analysis and presenting the results. Natural language processing techniques, including the TermRaider and AntConc tool, were applied in the aforementioned steps to assist in text qualitative analysis. Additionally, this study built a framework, using for analyzing the above three topics, from the perspective of five different stakeholders, including healthcare demanders and providers. Results: This study summarizes 26 applications (e.g., provide medical advice, provide diagnosis and triage recommendations, provide mental health support, etc.), 21 opportunities (e.g., make healthcare more accessible, reduce healthcare costs, improve patients care, etc.), and 17 challenges (e.g., generate inaccurate/misleading/wrong answers, raise privacy concerns, lack of transparency, etc.), and analyzes the reasons for the formation of these key items and the links between the three research topics. Conclusions: The application of GenAI LLMs in healthcare is primarily focused on transforming the way healthcare demanders access medical services (i.e., making it more intelligent, refined, and humane) and optimizing the processes through which healthcare providers offer medical services (i.e., simplifying, ensuring timeliness, and reducing errors). As the application becomes more widespread and deepens, GenAI LLMs is expected to have a revolutionary impact on traditional healthcare service models, but it also inevitably raises ethical and security concerns. Furthermore, GenAI LLMs applied in healthcare is still in the initial stage, which can be accelerated from a specific healthcare field (e.g., mental health) or a specific mechanism (e.g., GenAI LLMs' economic benefits allocation mechanism applied to healthcare) with empirical or clinical research.

Abstract:Context.—: Generative artificial intelligence (GAI) technologies are likely to dramatically impact health care workflows in clinical pathology (CP). Applications in CP include education, data mining, decision support, result summaries, and patient trend assessments. Objective.—: To review use cases of GAI in CP, with a particular focus on large language models. Specific examples are provided for the applications of GAI in the subspecialties of clinical chemistry, microbiology, hematopathology, and molecular diagnostics. Additionally, the review addresses potential pitfalls of GAI paradigms. Data sources.—: Current literature on GAI in health care was reviewed broadly. The use case scenarios for each CP subspecialty review common data sources generated in each subspecialty. The potential for utilization of CP data in the GAI context was subsequently assessed, focusing on issues such as future reporting paradigms, impact on quality metrics, and potential for translational research activities. Conclusions.—: GAI is a powerful tool with the potential to revolutionize health care for patients and practitioners alike. However, GAI must be implemented with much caution considering various shortcomings of the technology such as biases, hallucinations, practical challenges of implementing GAI in existing CP workflows, and end-user acceptance. Human-in-the-loop models of GAI implementation have the potential to revolutionize CP by delivering deeper, meaningful insights into patient outcomes both at an individual and population level.
Role of Artificial Intelligence Applications in Real-Life Clinical Practice: Systematic Review

Abstract:Background Artificial intelligence (AI) applications are growing at an unprecedented pace in health care, including disease diagnosis, triage or screening, risk analysis, surgical operations, and so forth. Despite a great deal of research in the development and validation of health care AI, only few applications have been actually implemented at the frontlines of clinical practice. Objective The objective of this study was to systematically review AI applications that have been implemented in real-life clinical practice. Methods We conducted a literature search in PubMed, Embase, Cochrane Central, and CINAHL to identify relevant articles published between January 2010 and May 2020. We also hand searched premier computer science journals and conferences as well as registered clinical trials. Studies were included if they reported AI applications that had been implemented in real-world clinical settings. Results We identified 51 relevant studies that reported the implementation and evaluation of AI applications in clinical practice, of which 13 adopted a randomized controlled trial design and eight adopted an experimental design. The AI applications targeted various clinical tasks, such as screening or triage (n=16), disease diagnosis (n=16), risk analysis (n=14), and treatment (n=7). The most commonly addressed diseases and conditions were sepsis (n=6), breast cancer (n=5), diabetic retinopathy (n=4), and polyp and adenoma (n=4). Regarding the evaluation outcomes, we found that 26 studies examined the performance of AI applications in clinical settings, 33 studies examined the effect of AI applications on clinician outcomes, 14 studies examined the effect on patient outcomes, and one study examined the economic impact associated with AI implementation. Conclusions This review indicates that research on the clinical implementation of AI applications is still at an early stage despite the great potential. More research needs to assess the benefits and challenges associated with clinical AI applications through a more rigorous methodology.

Abstract:Transplantation of organs continues to be a primary therapeutic modality for treatment of end-stage organ disease, and 1-year graft survival rates show increasing improvements for most organs. A number of transplant centers show 1-yr graft survival rates approaching 90% or more for the kidney, liver and pancreas. Rejection continues to be the major cause for loss of organs and there is still a major shortage of organs for transplantation. Additionally, many organs showed delayed graft function (or primary nonfunction) which may be related to either donor factors or preservation factors. The University of Wisconsin solution for organ preservation has increased the safe time of preservation for the liver, kidney, and pancreas and helped to increase the quality and number of organs available for transplantation. However, the long-range goal of organ preservation (unlimited preservation) is still far from being reached. In the past, preservation could accurately be categorized as an art and preservation solutions were developed based upon theoretical rationales about the mechanisms of organ injury at hypothermia and what agents would suppress injury. The utility and success of this approach is exemplified by the developments of Collins solution and the UW solution. However, further developments in methods to increase the quality and duration of preservation of all transplantable organs would appear to be dependent upon defining, systematically, how organs are injured and what can be done to suppress the injury.(ABSTRACT TRUNCATED AT 250 WORDS)

This emerging opportunity is accompanied by a need for decision makers and practitioners across healthcare systems to evaluate the safety and effectiveness of these interventions against the needs of their own setting. To meet this need, high-quality evidence regarding AI-enabled interventions must be made available, and decision makers in varying roles and settings must be empowered to evaluate that evidence within the context in which they work. This article summarizes good practices across four stages of evidence generation for AI health technologies: study design, study conduct, study reporting, and study appraisal.

Abstract:Generative artificial intelligence (GenAI) has the potential to improve healthcare through automation that enhances the quality and safety of patient care. Powered by foundation models that have been pretrained and can generate complex content, GenAI represents a paradigm shift away from the more traditional focus on task-specific classifiers that have dominated the AI landscape thus far. We posit that the imminent application of GenAI in healthcare will be through well-defined, low risk, high value, and narrow applications that automate healthcare workflows at the point of care using smaller foundation models. These models will be finetuned for different capabilities and application specific scenarios and will have the ability to provide medical explanations, reference evidence within a retrieval augmented framework and utilizing external tools. We contrast this with a general, all-purpose AI model for end-to-end clinical decision making that improves clinician performance, including safety-critical diagnostic tasks, which will require greater research prior to implementation. We consider areas where 'human in the loop' Generative AI can improve healthcare quality and safety by automating mundane tasks. Using the principles of implementation science will be critical for integrating 'end to end' GenAI systems that will be accepted by healthcare teams.

A comparative vignette study: Evaluating the potential role of a generative AI model in enhancing clinical decision‐making in nursing

Abstract:Aim This study explores the potential of a generative artificial intelligence tool (ChatGPT) as clinical support for nurses. Specifically, we aim to assess whether ChatGPT can demonstrate clinical decision‐making equivalent to that of expert nurses and novice nursing students. This will be evaluated by comparing ChatGPT responses to clinical scenarios to those of nurses on different levels of experience. Design This is a cross‐sectional study. Methods Emergency room registered nurses (i.e. experts; n = 30) and nursing students (i.e. novices; n = 38) were recruited during March–April 2023. Clinical decision‐making was measured using three validated clinical scenarios involving an initial assessment and reevaluation. Clinical decision‐making aspects assessed were the accuracy of initial assessments, the appropriateness of recommended tests and resource use and the capacity to reevaluate decisions. Performance was also compared by timing response generations and word counts. Expert nurses and novice students completed online questionnaires (via Qualtrics), while ChatGPT responses were obtained from OpenAI. Results Concerning aspects of clinical decision‐making and compared to novices and experts: (1) ChatGPT exhibited indecisiveness in initial assessments; (2) ChatGPT tended to suggest unnecessary diagnostic tests; (3) When new information required re‐evaluation, ChatGPT responses demonstrated inaccurate understanding and inappropriate modifications. In terms of performance, the mean number of words utilized in ChatGPT answers was 27–41 times greater than that utilized by both experts and novices; and responses were provided approximately 4 times faster than those of novices and twice faster than expert nurses. ChatGPT responses maintained logical structure and clarity. Conclusions A generative AI tool demonstrated indecisiveness and a tendency towards over‐triage compared to human clinicians. Impact The study shows that it is important to approach the implementation of ChatGPT as a nurse's digital assistant with caution. More study is needed to optimize the model's training and algorithms to provide accurate healthcare support that aids clinical decision‐making. Reporting method This study adhered to relevant EQUATOR guidelines for reporting observational studies. Generative artificial intelligence models in clinical infectious disease consultations: a cross-sectional analysis among specialists and resident trainees

Abstract:Background The potential of generative artificial intelligence (GenAI) to augment clinical consultation services in clinical microbiology and infectious diseases (ID) is being evaluated. Methods This cross-sectional study evaluated the performance of four GenAI chatbots (GPT-4.0, a Custom Chatbot based on GPT-4.0, Gemini Pro, and Claude 2) by analysing 40 unique clinical scenarios synthesised from real-life clinical notes. Six specialists and resident trainees from clinical microbiology or ID units conducted randomised, blinded evaluations across four key domains: factual consistency, comprehensiveness, coherence, and medical harmfulness. Results Analysis of 960 human evaluation entries by six clinicians, covering 160 AI-generated responses, showed that GPT-4.0 produced longer responses than Gemini Pro (p<0·001) and Claude 2 (p<0·001), averaging 577 ±81·19 words. GPT-4.0 achieved significantly higher mean composite scores compared to Gemini Pro [mean difference (MD)=0·2313, p=0·001] and Claude 2 (MD=0·2021, p=0·006). Specifically, GPT-4.0 outperformed Gemini Pro and Claude 2 in factual consistency (Gemini Pro, p=0·02 Claude 2, p=0·02), comprehensiveness (Gemini Pro, p=0·04; Claude 2, p=0·03), and the absence of medical harm (Gemini Pro, p=0·02; Claude 2, p=0·04). Within-group comparisons showed that specialists consistently awarded higher ratings than resident trainees across all assessed domains (p<0·001) and overall composite scores (p<0·001). Specialists were 9 times more likely to recognise responses with "Fully verified facts" and 5 times more likely to consider responses as "Harmless". However, post-hoc analysis revealed that specialists may inadvertently disregard conflicting or inaccurate information in their assessments, thereby erroneously assigning higher scores. Interpretation Clinical experience and domain expertise of individual clinicians significantly shaped the interpretation of AI-generated responses. In our analysis, we have demonstrated disconcerting human vulnerabilities in safeguarding against potentially harmful outputs. This fallibility seemed to be most apparent among experienced specialists and domain experts, revealing an unsettling paradox in the human evaluation and oversight of advanced AI systems. Stakeholders and developers must strive to control and mitigate user-specific and cognitive biases, thereby maximising the clinical impact and utility of AI technologies in healthcare delivery.

Abstract:Abstract Objectives This article aims to examine how generative artificial intelligence (AI) can be adopted with the most value in health systems, in response to the Executive Order on AI. Materials and Methods We reviewed how technology has historically been deployed in healthcare, and evaluated recent examples of deployments of both traditional AI and generative AI (GenAI) with a lens on value. Results Traditional AI and GenAI are different technologies in terms of their capability and modes of current deployment, which have implications on value in health systems. Discussion Traditional AI when applied with a framework top-down can realize value in healthcare. GenAI in the short term when applied top-down has unclear value, but encouraging more bottom-up adoption has the potential to provide more benefit to health systems and patients. Conclusion GenAI in healthcare can provide the most value for patients when health systems adapt culturally to grow with this new technology and its adoption patterns.

Abstract:Background: Generative artificial intelligence (AI) is rapidly transforming various aspects of healthcare, including critical care nephrology. Large language models (LLMs), a key technology in generative AI, show promise in enhancing patient care, streamlining workflows, and advancing research in this field. Summary: This review analyzes the current applications and future prospects of generative AI in critical care nephrology. Recent studies demonstrate the capabilities of LLMs in diagnostic accuracy, clinical reasoning, and continuous renal replacement therapy (CRRT) alarm troubleshooting. As we enter an era of multiagent models and automation, the integration of generative AI into critical care nephrology holds promise for improving patient care, optimizing clinical processes, and accelerating research. However, careful consideration of ethical implications and continued refinement of these technologies are essential for their responsible implementation in clinical practice. This review explores the current and potential applications of generative AI in nephrology, focusing on clinical decision support, patient education, research, and medical education. Additionally, we examine the challenges and limitations of AI implementation, such as privacy concerns, potential bias, and the necessity for human oversight. Key messages: (i) LLMs have shown potential in enhancing diagnostic accuracy, clinical reasoning, and CRRT alarm troubleshooting in critical care nephrology. (ii) Generative AI offers promising applications in patient education, literature review, and academic writing within the field of nephrology. Abstract:Generative artificial intelligence (GAI) can be broadly described as an artificial intelligence system capable of generating images, text, and other media types with human prompts. GAI models like ChatGPT, DALL-E, and Bard have recently caught the attention of industry and academia equally. GAI applications span various industries like art, gaming, fashion, and healthcare. In healthcare, GAI shows promise in medical research, diagnosis, treatment, and patient care and is already making strides in real-world deployments. There has yet to be any detailed study concerning the applications and scope of GAI in healthcare. Addressing this research gap, we explore several applications, real-world scenarios, and limitations of GAI in healthcare. We examine how GAI models like ChatGPT and DALL-E can be leveraged to aid in the applications of medical imaging, drug discovery, personalized patient treatment, medical simulation and training, clinical trial optimization, mental health support, healthcare operations and research, medical chatbots, human movement simulation, and a few more applications. Along with applications, we cover four real-world healthcare scenarios that employ GAI: visual snow syndrome diagnosis, molecular drug optimization, medical education, and dentistry. We also provide an elaborate discussion on seven healthcare-customized LLMs like Med-PaLM, BioGPT, DeepHealth, etc.,Since GAI is still evolving, it poses challenges like the lack of professional expertise in decision making, risk of patient data privacy, issues in integrating with existing healthcare systems, and the problem of data bias which are elaborated on in this work along with several other challenges. We also put forward multiple directions for future research in GAI for healthcare.

Augmenting intensive care unit nursing practice with generative AI: A formative study of diagnostic synergies using simulation-based clinical cases

Abstract:Background: As generative artificial intelligence (GenAI) tools continue advancing, rigorous evaluations are needed to understand their capabilities relative to experienced clinicians and nurses. The aim of this study was to objectively compare the diagnostic accuracy and response formats of ICU nurses versus various GenAI models, with a qualitative interpretation of the quantitative results. Methods: This formative study utilized four written clinical scenarios representative of real ICU patient cases to simulate diagnostic challenges. The scenarios were developed by expert nurses and underwent validation against current literature. Seventy-four ICU nurses participated in a simulation-based assessment involving four written clinical scenarios. Simultaneously, we asked ChatGPT-4 and Claude-2.0 to provide initial assessments and treatment recommendations for the same scenarios. The responses from ChatGPT-4 and Claude-2.0 were then scored by certified ICU nurses for accuracy, completeness and response. Results: Nurses consistently achieved higher diagnostic accuracy than AI across open-ended scenarios, though certain models matched or exceeded human performance on standardized cases. Reaction times also diverged substantially. Qualitative response format differences emerged such as concision versus verbosity. Variations in GenAI models system performance across cases highlighted generalizability challenges. Conclusions: While GenAI demonstrated valuable skills, experienced nurses outperformed in open-ended domains requiring holistic judgement. Continued development to strengthen generalized decision-making abilities is warranted before autonomous clinical integration. Response format interfaces should consider leveraging distinct strengths. Rigorous mixed methods research involving diverse stakeholders can help iteratively inform safe, beneficial human-GenAI partnerships centred on experience-guided care augmentation. Relevance to clinical practice: This mixed-methods simulation study provides formative insights into optimizing collaborative models of GenAI and nursing knowledge to support patient assessment and decision-making in intensive care. The findings can help guide development of explainable GenAI decision support tailored for critical care environments. Despite this, there remains a notable gap in the existing literature synthesizing the comprehensive understanding of AI's utilization in nursing care. Objective: This systematic review aims to synthesize the available evidence to comprehensively understand the application of AI in nursing care. Methods: Studies published between January 2019 and December 2023, identified through CINAHL Plus with Full Text, Web of Science, PubMed, and Medline, were included in this review. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines guided the identification, screening, exclusion, and inclusion of articles. The convergent integrated analysis framework, as proposed by the Joanna Briggs Institute, was employed to synthesize data from the included studies for theme generation. Results: A total of 337 records were identified from databases. Among them, 35 duplicates were removed, and 302 records underwent eligibility screening. After applying inclusion and exclusion criteria, eleven studies were deemed eligible and included in this review. Through data synthesis of these studies, six themes pertaining to the use of AI in nursing care were identified: 1) Risk Identification, 2) Health Assessment, 3) Patient Classification, 4) Research Development, 5) Improved Care Delivery and Medical Records, and 6) Developing a Nursing Care Plan. Conclusion: This systematic review contributes valuable insights into the multifaceted applications of AI in nursing care. Through the synthesis of data from the included studies, six distinct themes emerged. These findings not only consolidate the current knowledge base but also underscore the diverse ways in which AI is shaping and improving nursing care practices. Current Clinical Applications of Artificial Intelligence in Radiology and Their Best Supporting Evidence

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>Despite tremendous gains from deep learning and the promise of artificial intelligence (AI) in medicine to improve diagnosis and save costs, there exists a large translational gap to implement and use AI products in real-world clinical situations. Adoption of standards such as Transparent Reporting of a Multivariable Prediction Model for Individual Prognosis or Diagnosis, Consolidated Standards of Reporting Trials, and the Checklist for Artificial Intelligence in Medical Imaging is increasing to improve the peer-review process and reporting of AI tools. However, no such standards exist for product-level review.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A review of clinical trials showed a paucity of evidence for radiology AI products; thus, the authors developed a 10-question assessment tool for reviewing AI products with an emphasis on their validation and result dissemination. The assessment tool was applied to commercial and open-source algorithms used for diagnosis to extract evidence on the clinical utility of the tools.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>There is limited technical information on methodologies for FDA-approved algorithms compared with open-source products, likely because of intellectual property concerns. Furthermore, FDA-approved products use much smaller data sets compared with open-source AI tools, because the terms of use of public data sets are limited to academic and noncommercial entities, which precludes their use in commercial products.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Overall, this study reveals a broad spectrum of maturity and clinical use of AI products, but a large gap exists in exploring actual performance of AI tools in clinical practice.</p>

Abstract:BACKGROUND Research on the integration of artificial intelligence (AI) into community-based primary health care (CBPHC) has highlighted several advantages and disadvantages in practice regarding, for example, facilitating diagnosis and disease management, as well as doubts concerning the unintended harmful effects of this integration. However, there is a lack of evidence about a comprehensive knowledge synthesis that could shed light on AI systems tested or implemented in CBPHC. OBJECTIVE We intended to identify and evaluate published studies that have tested or implemented AI in CBPHC settings. METHODS We conducted a systematic scoping review informed by an earlier study and the Joanna Briggs Institute (JBI) scoping review framework and reported the findings according to PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analysis-Scoping Reviews) reporting guidelines. An information specialist performed a comprehensive search from the date of inception until February 2020, in seven bibliographic databases: Cochrane Library, MEDLINE, EMBASE, Web of Science, Cumulative Index to Nursing and Allied Health Literature (CINAHL), ScienceDirect, and IEEE Xplore. The selected studies considered all populations who provide and receive care in CBPHC settings, AI interventions that had been implemented, tested, or both, and assessed outcomes related to patients, health care providers, or CBPHC systems. Risk of bias was assessed using the Prediction Model Risk of Bias Assessment Tool (PROBAST). Two authors independently screened the titles and abstracts of the identified records, read the selected full texts, and extracted data from the included studies using a validated extraction form. Disagreements were resolved by consensus, and if this was not possible, the opinion of a third reviewer was sought. A third reviewer also validated all the extracted data. RESULTS We retrieved 22,113 documents. After the removal of duplicates, 16,870 documents were screened, and 90 peer-reviewed publications met our inclusion criteria. Machine learning (ML) (41/90, 45%), natural language processing (NLP) (24/90, 27%), and expert systems (17/90, 19%) were the most commonly studied AI interventions. These were primarily implemented for diagnosis, detection, or surveillance purposes. Neural networks (ie, convolutional neural networks and abductive networks) demonstrated the highest accuracy, considering the given database for the given clinical task. The risk of bias in diagnosis or prognosis studies was the lowest in the participant category (4/49, 4%) and the highest in the outcome category (22/49, 45%). CONCLUSIONS We observed variabilities in reporting the participants, types of AI methods, analyses, and outcomes, and highlighted the large gap in the effective development and implementation of AI in CBPHC. Further studies are needed to efficiently guide the development and implementation of AI interventions in CBPHC settings.
