Abstract:There have been many major developments in the use of augmented reality (AR), virtual reality (VR), and mixed reality (MR) technologies in the context of global surgical research, yet few reports on the trends in this field have been published to date. This study was therefore designed to explore these worldwide trends in this clinically important field. Relevant studies published from 1 January 2009 through 13 October 2020 were retrieved from the Science Citation Index-Expanded (SCI-E) tool of the Web of Science database. Bibliometric techniques were then used to analyze the resultant data, with visual bibliographic coupling, co-authorship, co-citation, co-occurrence, and publication trend analyses subsequently being conducted with GraphPad Prism 8 and with the visualization of similarities (VOS) software tool. There is no patient and public involved. In total, 6221 relevant studies were incorporated into this analysis. At a high level, clear global annual increases in the number of publications in this field were observed. The USA made the greatest contributions to this field over the studied period, with the highest H-index value, the most citations, and the greatest total link strength for analyzed publications. The country with the highest number of average citations per publication was Scotland. The Surgical Endoscopy And Other Interventional Techniques journal contributed the greatest number of publications in this field. The University of London was the institution that produced the greatest volume of research in this field. Overall, studies could be broadly classified into five clusters: Neurological Research, Surgical Techniques, Technological Products, Rehabilitative Medicine, and Clinical Therapy. The trends detected in the present analysis suggest that the number of global publications pertaining to the use of AR, VR, and MR techniques in surgical research is likely to increase in the coming years. Particular attention should be paid to emerging trends in related fields including MR, extended reality, head-mounted displays, navigation, and holographic images.

Insight into the history and trends of surgical simulation training in education: a bibliometric analysis

Abstract:Background: Surgical simulation training enables surgeons to acquire clinical experience or skills from the operating room to the simulation environment. Historically, it has changed with advances in science and technology. Moreover, no previous study has analyzed this field from the bibliometric analysis dimension. The study aimed to review changes in surgical simulation training worldwide using bibliometric software. Materials and methods: Two searches were performed on the core collection database, Web of Science, regarding data from 1991 to the end of 2020 using three topic words (surgery, training, and simulation). From 1 January 2000, to 15 May 2022, the keyword 'robotic' was added for the hotspot exploration. The data were chiefly analyzed by publication date, country, author(s), and keywords using bibliometric software. Results: A total of 5285 articles were initially analyzed, from which it was clear that laparoscopic skill, three-dimensional printing, and virtual reality were the main focuses during those study periods. Subsequently, 348 publications on robotic surgery training were identified. Conclusion: This study systematically summarizes the current status in the field of surgical simulation training and provides insights into the research focuses and future hotspot in a global context.

Abstract:Simulation-based robotic surgery training may help surgeons gain operative skills and experience in the simulation environment. This bibliometric analysis examined the development of simulation-based training for robotic surgical education. Articles pertaining to robotic surgical simulation training that were included in the Web of Science Core Collection up to April 25, 2024, were included. The temporal patterns in published paper numbers were evaluated using Microsoft Excel software, and the data regarding co-authorship and keyword co-occurrence were analyzed and visualized using the VOSviewer and SCImago Graphica tools. A total of 594 papers on simulation-based training for robotic surgical education were evaluated in this study. The United States and United Kingdom were the leading contributors in this field. The most published authors were Professor Ahmed Kamran (23 publications) and Prokar Dasgupta (22 publications). The highest number of papers was published in the journal titled "Surgical Endoscopy and Other Interventional Techniques." The most common keywords were "virtual reality," "curriculum," "robotic surgery simulator," "assessment," and "learning curve." Our study offers a detailed overview of international research on simulation-based training for robotic surgical education, including the publishing countries, institutions, authors, journals, and research hotspots. It also methodically summarizes the state of knowledge in the area, and provides definite directions and concepts for further in-depth analysis.
Does Extended Reality Simulation Improve Surgical/Procedural Learning and Patient Outcomes When Compared With Standard Training Methods?

Abstract:Introduction The use of extended reality (XR) technologies, including virtual, augmented, and mixed reality, has increased within surgical and procedural training programs. Few studies have assessed experiential learning- and patient-based outcomes using XR compared with standard training methods. Methods As a working group for the Society for Simulation in Healthcare, we used Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and a PICO strategy to perform a systematic review of 4238 articles to assess the effectiveness of XR technologies compared with standard training methods. Outcomes were grouped into knowledge, time-to-completion, technical proficiency, reactions, and patient outcomes. Because of study heterogeneity, a meta-analysis was not feasible. Results Thirty-two studies met eligibility criteria: 18 randomized controlled trials, 7 comparative studies, and 7 systematic reviews. Outcomes of most studies included Kirkpatrick levels of evidence I–III (reactions, knowledge, and behavior), while few reported level IV outcomes (patient). The overall risk of bias was low. With few exceptions, included studies showed XR technology to be more effective than standard training methods in improving objective skills and performance, shortening procedure time, and receiving more positive learner ratings. However, XR use did not show significant differences in gained knowledge. Conclusions Surgical or procedural XR training may improve technical skill development among trainees and is generally favored over standard training methods. However, there should be an additional focus on how skill development translates to clinically relevant outcomes. We recommend longitudinal studies to examine retention and transfer of training to clinical settings, methods to improve timely, adaptive feedback for deliberate practice, and cost analyses.

Abstract:Since the advent of virtual reality (VR), it has been implemented in medical education for surgical training and anatomy education so that the Internet of Medical Things (IoMT) could be further boosted. There have been rare studies on the research trends of the evaluation of endoscopic training through different XR modalities. Position-based dynamics (PBD) has been utilized to optimize the surgical thread simulation, This paper aims to quantitatively evaluate the training performance of each XR modality in general and in terms of the medical fields studied and outcomes measured. Sensors and devices are utilized to form the Internet of Medical Things for healthcare, where the data is uploaded to the cloud and then analyzed as follows before being fed back to the doctor so that he or she can understand his or her level of operation. Through subjective and objective evaluation, the potential promoting effects of vision and touch in module training were discussed.

Revolutionizing thoracic surgery education: a bibliometric analysis of the past decade's literature

Abstract:Objectives: Thoracic surgery is a complex field requiring advanced technical skills and critical decision-making. Surgical education must evolve to equip trainees with proficiency in new techniques and technologies. Methods: This bibliometric analysis systematically reviewed 113 articles on thoracic surgery skills training published over the past decade, retrieved from databases including Web of Science. Publication trends, citation analysis, author and journal productivity, and keyword frequencies were evaluated. Results: The United States contributed the most publications, led by pioneering institutions. Simulation training progressed from basic to sophisticated modalities and virtual reality emerged with transformative potential. Minimally invasive techniques posed unique learning challenges requiring integrated curricula. Conclusion: Ongoing investments in educational research and curriculum innovations are imperative to advance thoracic surgery training through multidisciplinary strategies. This study provides an evidentiary foundation to optimize training and address the complexities of modern thoracic surgery.

Abstract:Extended reality (XR) is a collective term for various technologies, including augmented reality (AR), virtual reality (VR), and mixed reality (MR). In recent years, XR has been applied in medical education and training, clinical therapy, and other fields. This scoping review aims to comprehensively summarize the current and potential use of XR in orthodontics. A systematic search was conducted to identify literature on the applications of XR in orthodontic education and training, doctor-patient communication, and orthodontic diagnosis and treatment published from 1999 to 2023. The search was conducted up to August 2023 to identify articles from Embase, PubMed, Web of Science, CNKI, and Google Scholar. A total of 185 studies were screened and 12 articles were reviewed in depth. The applications proposed for XR are educational purposes in 4 studies, and orthodontic diagnosis and treatment in 8 studies. A comparison of studies on different applications indicates that XR technology has varying degrees of positive effects in orthodontic education and treatment. However, XR in orthodontics is still in its early stages of development, and further studies with larger samples and longer clinical trials are needed to confirm the potential positive effects of XR.

Abstract:Surgical education demands extensive knowledge and skill acquisition within limited time frames, often limited by reduced training opportunities and high-pressure environments. This review evaluates the effectiveness of extended reality-based head-mounted display (ExR-HMD) technology in surgical education, examining its impact on educational outcomes and exploring its strengths and limitations. Data from PubMed, Cochrane Library, Web of Science, ScienceDirect, Scopus, ACM Digital Library, IEEE Xplore, WorldCat, and Google Scholar (Year: 2014-2024) were synthesized. After screening, 32 studies comparing ExR-HMD and traditional surgical training methods for medical students or residents were identified. Quality and bias were assessed using the Medical Education Research Study Quality Instrument, Newcastle-Ottawa Scale-Education, and Cochrane Risk of Bias Tools. Results indicate that ExR-HMD offers benefits such as increased immersion, spatial awareness, and interaction and supports motor skill acquisition theory and constructivist educational theories. However, challenges such as system fidelity, operational inconvenience, and physical discomfort were noted. Nearly half the studies reported outcomes comparable or superior to traditional methods, emphasizing the importance of social interaction. Limitations include study heterogeneity and English-only publications. ExR-HMD shows promise but needs educational theory integration and social interaction. Future research should address technical and economic barriers to global accessibility.

Abstract:Extended reality (XR) technology refers to any situation where real-world objects are enhanced with computer technology, including virtual reality, augmented reality, and mixed reality. Augmented reality and mixed reality technologies have been widely applied in orthopedic clinical practice, including in teaching, preoperative planning, intraoperative navigation, and surgical outcome evaluation. The primary goal of this narrative review is to summarize the effectiveness and superiority of XR-technology-assisted intraoperative navigation in the fields of trauma, joint, spine, and bone tumor surgery, as well as to discuss the current shortcomings in intraoperative navigation applications. We reviewed titles of more than 200 studies obtained from PubMed with the following search terms: extended reality, mixed reality, augmented reality, virtual reality, intraoperative navigation, and orthopedic surgery; of those 200 studies, 69 related papers were selected for abstract review. Finally, the full text of 55 studies was analyzed and reviewed. They were classified into four groups—trauma, joint, spine, and bone tumor surgery—according to their content. Most of studies that we reviewed showed that XR-technology-assisted intraoperative navigation can effectively improve the accuracy of implant placement, such as that of screws and prostheses, reduce postoperative complications caused by inaccurate implantation, facilitate the achievement of tumor-free surgical margins, shorten the surgical duration, reduce radiation exposure for patients and surgeons, minimize further damage caused by the need for visual exposure during surgery, and provide richer and more efficient intraoperative communication, thereby facilitating academic exchange, medical assistance, and the implementation of remote healthcare.

Abstract:Background The use of augmented reality (AR) is growing in medical education, in particular, in radiology and surgery. AR has the potential to become a strategic component of neurosurgical training courses. In fact, over the years, there has been a progressive increase in the application of AR in the various fields of neurosurgery. In this study, the authors aim to define the diffusion of these augmented reality systems in recent years. This study describes future trends in augmented reality for neurosurgeons. Methods A systematic review of the literature was conducted to identify research published from December 1st, 2011 to November 30th, 2021. Electronic databases (PubMed, PubMed Central, and Scopus) were screened. The methodological quality of studies and extracted data were assessed for “augmented reality” and “neurosurgery”. The data analysis focused on the geographical distribution, temporal evolution, and topic of augmented reality in neurosurgery. Results A total of 198 studies have been included. The number of augmented reality applications in the neurosurgical field has increased during the last 10 years. The main topics on which it is mostly applied are spine surgery, neuronavigation, and education. The geographical distribution shows extensive use of augmented reality in the USA, Germany, China, and Canada. North America is the continent that uses augmented reality the most in the training and education of medical students, residents, and surgeons, besides giving the greatest research contribution in spine surgery, brain oncology, and surgical planning. AR is also extensively used in Asia for intraoperative navigation. Nevertheless, augmented reality is still far from reaching Africa and other countries with limited facilities, as no publications could be retrieved from our search. Conclusions The use of AR is significantly increased in the last 10 years. Nowadays it is mainly used in spine surgery and for neurosurgical education, especially in North America, Europe and China. A continuous growth, also in other aspects of the specialty, is expected in the next future.

Global research status and trends in orthopaedic surgical robotics: a bibliometric and visualisation analysis study

Abstract:This study aimed to investigate and summarise the current state of research and trends in orthopaedic surgical robots. Data on publicly available publications related to orthopaedic surgical robots were extracted by abstract searches of the Web of Science Core Collection database. A bibliometric analysis and detailed reading of the publications were then carried out and the information was visualised using co-authorship analysis, coupling analysis, co-citation analysis and co-occurrence analysis in VOSviewer. With 436 publications included in the study from 1993 to 2022, the overall contribution of publications to the world showed an increasing trend year by year, with a particularly pronounced after 2017, and a geographical trend of predominantly East Asia, North America, and Western Europe. Of these, China was the largest contributor ( n = 128). Overall, the UK affiliates and their scholars were leaders in the field, with a high number of publications, total citations, average citations per article and H-index. The most published institutions and authors were Imperial College London ( n = 21) and Professor Fares Sahi Haddad of University College London ( n = 12), respectively. Journals with a high overall impact on robotic orthopaedic surgery were the Journal of Arthroplasty, Bone Joint Journal, International Journal of Medical Robotics and Computer Assisted Surgery. Keyword co-occurrence network analysis revealed four main clusters: robot-assisted knee, hip arthroplasty, spine surgery, and robotic technology research and development. The top three most common sites for robot-assisted surgery use were the knee, hip and spine. The most used robot types were the knee and spine, with Mako and Arobot being the most used robots for the knee and TiRobot for the spine. This study comprehensively reveals the current status and trends of global research on orthopaedic surgical robots, covering countries, institutions, authors, journals, research hotspots, robot types and surgical sites, providing directional guidance and research ideas for further research on the technological development and clinical evaluation of orthopaedic surgical robots.
Detecting the Interdisciplinary Nature and Topic Hotspots of Robotics in Surgery: Social Network Analysis and Bibliometric Study

Abstract:Background: With the widespread application of a robot to surgery, growing literature related to robotics in surgery (RS) documents widespread concerns from scientific researchers worldwide. Although such application is helpful to considerably improve the accuracy of surgery, we still lack the understanding of the multidiscipline-crossing status and topic distribution related to RS. Objective: The aim of this study was to detect the interdisciplinary nature and topic hotspots on RS by analyzing the current publication outputs related to RS. Methods: The authors collected publications related to RS in the last 21 years, indexed by the Web of Science Core Collection. Various bibliometric methods and tools were used, including literature distribution analysis at the country and institution level and interdisciplinary collaboration analysis in the different periods of time. Co-word analysis was performed based on the keywords with high frequency. The temporal visualization bar presented the evolution of topics over time. Results: A total of 7732 bibliographic records related to RS were identified. The United States plays a leading role in the publication output related to RS, followed by Italy and Germany. It should be noted that the Yonsei University in South Korea published the highest number of RS-related publications. Furthermore, the interdisciplinary collaboration is uneven; the number of disciplines involved in each paper dropped from the initial 1.60 to the current 1.31. Surgery; Engineering; Radiology, Nuclear Medicine, and Medical Imaging; and Neurosciences and Neurology are the 4 core disciplines in the field of RS, all of which have extensive cooperation with other disciplines. The distribution of topic hotspots is in imbalanced status, which can be categorized into 7 clusters. Moreover, 3 areas about the evolution of topic were identified, namely (1) the exploration of techniques that make RS implemented, (2) rapid development of robotic systems and related applications in surgery, and (3) application of a robot to excision of tissues or organs targeted at various specific diseases. Conclusions: This study provided important insights into the interdisciplinary nature related to RS, which indicates that the researchers with different disciplinary backgrounds should strengthen cooperation to publish a high-quality output. The research topic hotspots related to RS are relatively scattered, which has begun to turn to the application of RS targeted at specific diseases. Our study is helpful to provide a potential guide to the direction of the field of RS for future research in the field of RS.

Abstract:Background: Virtual reality (VR), widely used in the medical field, may affect future medical training and treatment. Therefore, this study examined VR's potential uses and research directions in medicine. Methods: Citation data were downloaded from the Web of Science Core Collection database (WoSCC) to evaluate VR in medicine in articles published between 1 January 2012 and 31 December 2023. These data were analyzed using CiteSpace 6.2. R2 software. Present limitations and future opportunities were summarized based on the data. Results: A total of 2143 related publications from 86 countries and regions were analyzed. The country with the highest number of publications is the USA, with 461 articles. The University of London has the most publications among institutions, with 43 articles. The burst keywords represent the research frontier from 2020 to 2023, such as "task analysis", "deep learning", and "machine learning". Conclusion: The number of publications on VR applications in the medical field has been steadily increasing year by year. The USA is the leading country in this area, while the University of London stands out as the most published, and most influential institution. Currently, there is a strong focus on integrating VR and AI to address complex issues such as medical education and training, rehabilitation, and surgical navigation. Looking ahead, the future trend involves integrating VR, augmented reality (AR), and mixed reality (MR) with the Internet of Things (IoT), wireless sensor networks (WSNs), big data analysis (BDA), and cloud computing (CC) technologies to develop intelligent healthcare systems within hospitals or medical centers.

Abstract:With the rapid development of technology, virtual reality (VR) technology has gradually become an important branch of medical education, especially in the field of thoracic surgery skills training. This paper aims to explore the current application status of VR technology in thoracic surgery skills training, evaluate its effectiveness in surgical training, and discuss its potential impact on improving the quality of surgical education. Through a literature review, the basic principles and development history of VR technology, its specific applications in thoracic surgery education, as well as the challenges and trends it currently faces, are analyzed. This paper also discusses the role of VR technology in improving surgical training efficiency, promoting personalized learning, and potential economic benefits. Although VR technology has significant advantages in thoracic surgery training, attention needs to be paid to its practical application and effect verification in clinical practice. Finally, with the continuous advancement of technology, the future application of VR technology in thoracic surgery training is expected to be more extensive and in-depth, contributing to the cultivation of more high-quality thoracic surgeons.
Mortality in a Swedish rubber tire manufacturing plant: occupational risks or an "unhealthy worker" effect?

Abstract:BACKGROUND In this cohort study a classification of 12 work categories, with specified common exposures, is used to evaluate the mortality among workers in a Swedish rubber tire plant. METHODS Mortality among the cohort members is compared with expected values from national rates. Standardized mortality ratios (SMR) were calculated for the total cohort, for sub-cohorts and with the inclusion of a latency requirement. RESULTS Male workers had increased risks for total mortality, circulatory death, respiratory death, mental disorders, epilepsy, injuries/poisoning, and for alcohol-related death. Many risks were noted among workers exposed to dust, fumes, or vapor. No trend in risk with increasing duration of employment was seen. DISCUSSION Some established risks for rubber workers could be related to some specified work categories. The highest risks were noted among workers having a very short duration of employment indicating a possible "unhealthy worker effect" associated with life-style factors or to tasks offered to unskilled workers.

Abstract:OBJECTIVE Extended reality (XR) systems, including augmented reality (AR), virtual reality (VR), and mixed reality, have rapidly emerged as new technologies capable of changing the way neurosurgeons prepare for cases. Thus, the authors sought to evaluate the perspectives of neurosurgical trainees on the integration of these technologies into neurosurgical education. METHODS A 20-question cross-sectional survey was administered to neurosurgical residents and fellows to evaluate perceptions of the use of XR in neurosurgical training. Respondents evaluated each statement using a modified Likert scale (1–5). RESULTS One hundred sixteen responses were recorded, with 59.5% of participants completing more than 90% of the questions. Approximately 59% of participants reported having institutional access to XR technologies. The majority of XR users (72%) believed it was effective for simulating surgical situations, compared with only 41% for those who did not have access to XR. Most respondents (61%) agreed that XR could become a standard in neurosurgical education and a cost-effective training tool (60%). Creating patient-specific anatomical XR models was considered relatively easy by 56% of respondents. Those with XR access reported finding it easier to create intraoperative models (58%) than those without access. A significant percentage (79%) agreed on the need for technical skill training outside the operating room (OR), especially among those without XR access (82%). There was general agreement (60%) regarding the specific need for XR. XR was perceived as effectively simulating stress in the OR. Regarding clinical outcomes, 61% believed XR improved efficiency and safety and 48% agreed it enhanced resection margins. Major barriers to XR integration included lack of ample training hours and/or time to use XR amid daily clinical obligations (63%). CONCLUSIONS The data presented in this study indicate that there is broad agreement among neurosurgical trainees that XR holds potential as a training modality in neurosurgical education. Moreover, trainees who have access to XR technologies tend to hold more positive perceptions regarding the benefits of XR in their training. This finding suggests that the availability of XR resources can positively influence trainees' attitudes and beliefs regarding the utility of these technologies in their education and training. Abstract:Background: Robot-assisted gastrointestinal and liver surgery has been an important development direction in the field of surgery in recent years and it is also one of the fastest developing and most concerning fields in surgical operations. Aim: To illustrate the major areas of research and forward-looking directions over the past twenty-six years. Methods: Using the Web of Science Core Collection database, a comprehensive review of scholarly articles pertaining to robot-assisted gastrointestinal and liver surgery was researched out between 2000 and 2023. We used Citespace (Version 6.2.4) and Bibliometrix package (Version 4.3.0) to visualize the analysis of all publications including country, institutional affiliations, authors, and keywords. Results: In total, 346 articles were retrieved. Surgical Endoscopy had with the largest number of publications and was cited in this field. The United States was a core research country in this field. Yonsei University was the most productive institution. The current focus of this field is on rectal surgery, long-term prognosis, perioperative management, previous surgical experience, and the learning curve. Conclusion: The scientific interest in robot-assisted gastrointestinal and liver surgery has experienced a significant rise since 1997. This study provides new perspectives and ideas for future research in this field.
