Abstract:BACKGROUND:Bifurcation angles may have an impact on the clinical outcomes of crush stenting. We sought to compare high (> or = 60 degrees ) with low (< 60 degrees ) bifurcation angle in patients who underwent either classical or double kissing (DK) crush stenting for bifurcation lesions from the DKCRUSH-1 data base.METHODS:There were 212 patients with 220 lesions, some with low-angle (n = 138) and some with high-angle (n = 74). Angiography was indexed at 8-month after procedure. Primary endpoint was the occurrence of major adverse cardiac events (MACEs), defined as cardiac death, myocardial infarction and target lesion revascularization (TLR). Secondary endpoint included late lumen loss, the rate of restenosis, and final kissing balloon inflation (FKBI).RESULTS:At 8 months, clinical follow-up was 100%; angiographic follow-up was 75% in the low-angle group and 83.3% in the high-angle group. There were no significant differences in the FKBI between the high-angle group (91.43%) and the low-angle group (82.39%). In the high angle group, there was a significant difference in contrast volume used (P = 0.005) but no significant difference in acute gain, minimum lumen diameter (MLD), late loss and diameter stenosis in the pre-bifurcation segment, post-bifurcation segment or side branch. When lesions were assigned into with-(n = 133) and without-FKBI (n = 42), significant side-branch late loss was seen in the group without-FKBI ((0.65 +/- 0.49) mm vs (0.47 +/- 0.62) mm, P = 0.02), with a resultant greater restenosis rate (37.68% vs 18.32%, P = 0.001). No difference was detected in the MACE free survival rate between the high and low angle groups (82.39% vs 82.36%, P = 0.84). The rate of stent thrombosis tended to be higher in the lower-angle group although there was no significant difference (P = 0.38). The TLR free survival rate was 87.2% in the with-FKBI group vs 73.5% in the without-FKBI group (P = 0.001). Cox regression analysis showed that the independent predictors for target vessel revascularization were the side branch stent MLD post stenting (hazard ratios (HR) 1.028, 95% CI 2.357 - 16.233, P = 0.002), lack of FKBI (HR 4.910, 95% CI 4.706 - 8.459, P = 0.001) and unsatisfactory kissing (HR 3.120, 95% CI 2.975 - 5.431, P = 0.001).CONCLUSIONS:Bifurcation angles do not influence the clinical outcome of crush stenting. Successful final kissing balloon inflation, regardless of bifurcation angles, can predict TLR.

Abstract:Objective To investigate the safety and efficacy of side branch treatment with drug coated balloon (DCB) in coronary bifurcation lesions.Methods 31 patients with bifurcation lesions in Department of Cardiology of our hospital from November 2014 to July 2016 were selected in this study (main branches stenosis ＜ 50％,branch stenosis ＞ 70％;cahber of branch vessel≥2mm) and treated with simple DCB in branch vessel.The success rate of operation,blood flow of the branch vessels and incidence of comphcations were observed.Quantitifying coronary angiography (QCA) analysis was done after 9 months and the major adverse cardiovascular events (MACE) were recorded during 9 months' follow-up as well.Results Of 33 coronary bifurcation lesions in 31 patients,57.58％ were located in left anterior descending artery/diagonal branches,30.30％ were located in circumflex aaery/obtuse marginal branches,and 12.12％ were located in right coronary artery/posterior descending artery or posterior branch of left ventricle.31 patiems successfully completed the imervemional treatment of bifurcation lesions with 100％ success rate.All the procedure was transradial approach.Branch vascular dissection was found only in two cases (one with TIMI flow grade ＜ 2) and drug-eluting stents were used as the remedial measure.During perioperative and postoperative clinical visit (outpatient or telephone follow-up),no MCAE such as death or acute myocardial infarction (AMI) occurred.Angiographic follow-up was finished in 74.19％ of the patients.There was only one patient underwem target lesion revascularization (TLR).Late lumen loss (LLL) was (0.13 ± 0.25) mm.Condusions It is a safe and effective strategy to use DCB as a standalone procedure for side branch in patients with coronary bifurcation lesions.

Abstract:Background Double kissing crush (DKC) and nano‐crush (NC) techniques are frequently used, but the comparison for both techniques is still lacking. The goal of this multicenter study was to retrospectively assess the midterm clinical results of DKC and NC stenting in patients with complex bifurcation lesions (CBLs). Methods A total of 324 consecutive patients [male: 245 (75.6%), mean age: 60.73 ± 10.21 years] who underwent bifurcation percutaneous coronary intervention between January 2019 and May 2023 were included. The primary endpoint defined as the major cardiovascular events (MACE) included cardiac death, target vessel myocardial infarction (TVMI), or clinically driven target lesion revascularization (TLR). Inverse probability weighting (IPW) was performed to reduce treatment selection bias. This is the first report comparing the clinical outcomes of DKC and NC stenting in patients with CBL. Results The initial revascularization strategy was DKC in 216 (66.7%) cases and NC in 108 (33.3%) patients. SYNTAX scores [25.5 ± 6.73 vs. 23.32 ± 6.22, p = 0.005] were notably higher in the NC group than the DKC group. The procedure time (76.98 ± 25.1 vs. 57.5 ± 22.99 min, p = 0.001) was notably higher in the DKC group. The incidence of MACE (18.5 vs. 9.7%, p = 0.025), clinically driven TLR (14.8 vs. 6%, p = 0.009), and TVMI (10.2 vs. 4.2%, p = 0.048) were notably higher in the NC group than in the DKC group. The midterm MACE rate in the overall population notably differed between the NC group and the DKC group (adjusted HR (IPW): 2.712, [95% CI: 1.407−5.228], p = 0.003). Conclusion In patients with CBLs, applying the DKC technique for bifurcation treatment had better ischemia‐driven outcomes than the NC technique.

Double-Kissing Nanocrush for Bifurcation Lesions: Development, Bioengineering, Fluid Dynamics, and Initial Clinical Testing

Abstract:Background: When possible, a single-stent technique to treat coronary bifurcation disease is preferable. However, when 2 stents are required, there is scope to improve on existing techniques. The crush technique has already been improved with the introduction of double-kissing (DK) and minicrush. We sought to refine and simplify the minicrush technique, retaining its advantages while avoiding its disadvantages, by developing a DK nanocrush technique. Methods: The DK nanocrush method allows complete lesion coverage of a bifurcation lesion without excessive metal layers. This is achieved by positioning the side branch (SB) stent with minimal protrusion into the main branch (MB), implantation of the SB stent with an undeployed balloon in the MB, immediate kissing-balloon inflation with formation of a minimal neocarina, stenting the MB, recrossing the proximal part of the SB without crossing a double metal layer, and final kissing. We demonstrate this technique with benchtop implantation, microscopic computed tomographic reconstruction, computational fluid dynamics (CFD) modelling, and clinically with the use of angiographic and intravascular imaging. Results: The DK nanocrush was practically feasible and resulted in full ostial coverage. CFD analysis demonstrated minimally disturbed blood flow. The technique was successfully utilised in 9 patients with bifurcation lesions with excellent angiographic outcomes and no adverse events over 12 months. Conclusions: The DK nanocrush technique may represent the ultimate refinement of the original crush technique with a number of practical and theoretical advantages. It remains to be tested against other bifurcation techniques in prospective trials.

Abstract:OBJECTIVES:The aim of this study was to compare clinical outcomes of different bifurcation percutaneous coronary intervention (PCI) techniques. BACKGROUND:Despite several randomized trials, the optimal PCI technique for bifurcation lesions remains a matter of debate. Provisional stenting has been recommended as the default technique for most bifurcation lesions. Emerging data support double-kissing crush (DK-crush) as a 2-stent technique. METHODS:PubMed and Scopus were searched for randomized controlled trials comparing PCI bifurcation techniques for coronary bifurcation lesions. Outcomes of interest were major adverse cardiovascular events (MACE). Secondary outcomes of interest were cardiac death, myocardial infarction, target vessel or lesion revascularization, and stent thrombosis. Summary odds ratios (ORs) were estimated using Bayesian network meta-analysis. RESULTS:Twenty-one randomized controlled trials including 5,711 patients treated using 5 bifurcation PCI techniques were included. Investigated techniques were provisional stenting, T stenting/T and protrusion, crush, culotte, and DK-crush. Median follow-up duration was 12 months (interquartile range: 9 to 36 months). When all techniques were considered, patients treated using the DK-crush technique had less occurrence of MACE (OR: 0.39; 95% credible interval: 0.26 to 0.55) compared with those treated using provisional stenting, driven by a reduction in target lesion revascularization (OR: 0.36; 95% credible interval: 0.22 to 0.57). No differences were found in cardiac death, myocardial infarction, or stent thrombosis among analyzed PCI techniques. No differences in MACE were observed among provisional stenting, culotte, T stenting/T and protrusion, and crush. In non-left main bifurcations, DK-crush reduced MACE (OR: 0.42; 95% credible interval: 0.24 to 0.66). CONCLUSIONS:In this network meta-analysis, DK-crush was associated with fewer MACE, driven by lower rates of repeat revascularization, whereas no significant differences among techniques were observed for cardiac death, myocardial infarction, and stent thrombosis. A clinical benefit of 2-stent techniques was observed over provisional stenting in bifurcation with side branch lesion length ≥10 mm.

Abstract:Background: Data on the relevance of the location of coronary bifurcation lesions treated by crush stenting with outcomes were limited.Hypothesis: We hypothesized that the location of the bifurcation lesion correlated with clinical outcome.Method: A total of 212 patients with 230 true bifurcation lesions treated by crush stenting with drug-eluting stents (DES) were assessed prospectively. Surveillance quantitative angiographies were indexed at 8 months after procedure. Primary endpoint was major adverse cardiac events (MACE), defined as cardiac death, myocardial infarction, and target lesion revascularization (TLR).Results: Patients in the distal right coronary artery (RCAd) group were characterized by higher proportions of prior myocardial infarction and very tortuous lesions. However, lesions in the RCAd group, compared to those of other groups, had the lowest late lumen loss, with resultant lowest incidence of MACE at a mean follow-up of 268 +/- 35 days. Independent predictors of MACE included unsatisfied kissing (KUS; hazard ratio [HR]: 12.14, 95% confidence interval [CI]: 4.01-12.10, P = .001) and non-RCA lesion (HR: 20.69, 95% CI: 5.05-22.38, P = .001), while those of TLR were KUS (HR: 10.21, 95% CI: 0.01-0.34, P = .002), bifurcation angle (HR: 4.728, 95% CI: 2.541-4.109, P = .001), and non-RCA lesion (HR: 16.05, 95% CI: 1.01-4.83, P = .001).Conclusions: Classical crush stenting with drug-eluting stents is associated with significantly better outcomes in RCAd. Quality of kissing inflation is mandatory to improve outcome.
Study on DK crush versus provisional stenting technique for treatment of coronary artery bifurcation lesions

Abstract:Background: Whether single or double stent implantation is the optimal stenting strategy in coronary artery bifurcation lesions is unknown. Objective: To compare the efficacy and safety of DK crush (DK) versus provisional stenting technique (PT) for the treatment of coronary artery bifurcation lesions. Design, time and setting: Non-randomized, controlled and prospective experiment was performed at the Nanjing First Hospital between August 2006 and April 2007. Participants: Sixty-two patients with true coronary bifurcation lesions (branch diameter > 2.0 mm by visual estimation) were enrolled. Thirty-two patients were treated by PT and the other patients undergoing DK. Methods: Only Rapamycin eluting-stents were allowed to be implanted. T stent, Culotte stent or Reverse crush stent could be used if there was still more than 50% residual stenosis or dissection that could affect blood flow in PT group following routine implantation. DK group only underwent routine stent implantation. Main outcome measures: Quantifying coronary angiography dada were analyzed before and immediately after percutaneous coronary intervention as well as at 6-9 months follow-up. Major adverse cardiovascular events were recorded by telephone call after 1 year. Results: There were no significant differences between two groups in terms of baseline characteristics (P > 0.05). Patients in the DK group compared with those in the PT group were characterized by larger minimum lumen diameter at branch ostium (P < 0.01) and smaller residual stenosis (P < 0.01). At 6-9 months follow-up, no statistically significant differences were identified in minimum lumen diameter (P < 0.01), residual stenosis (P < 0.01) and late lumen loss in main vessel, but it tented to be higher in PT group compared with DK group (P > 0.05). There were no major adverse cardiovascular events developed during hospitalization in both groups. At 1 year follow-up, 1 major adverse cardiovascular event occurred in DK group and 4 developed in PT group (P > 0.05). Conclusion: Both DK crush and provisional stenting techniques are effective and safe for the treatment of coronary bifurcation lesions. But DK crush shows superior over provisional stenting because it can effectively reduce incidence of restenosis and major adverse cardiovascular events.

Abstract:OBJECTIVES The purpose of this study was to evaluate predictors of an adverse outcome after "crush" bifurcation stenting. BACKGROUND The "crush" technique is a recently introduced strategy with limited data regarding long-term outcomes.METHODS We identified 231 consecutive patients treated with drug-eluting stent implantation with the "crush" technique for 241 de novo bifurcation lesions. Clinical follow-up was obtained in 99.6%.RESULTS The in-hospital major adverse cardiac event (MACE) rate was 5.2%. At 9 months, 10 (4.3%) patients had an event consistent with possible post-procedural stent thrombosis. Survival free of target lesion revascularization (TLR) was 90.3%; the only independent predictor of TLR was left main stem (LMS) therapy (odds ratio [OR] 4.97; 95% confidence interval [Cl] 2.00 to 12.37, p = 0.001). Survival free of MACE was 83.5% and independent predictors of MACE were LMS therapy (OR 3.79; 95% CI 1.76 to 8.14, p = 0.001) and treatment of patients with multivessel disease (OR 4.21; 95% CI 0.95 to 18.56, p = 0.058). Angiographic follow-up was obtained in 77% of lesions at 8.3 +/- 3.7months. The mean late loss of the main vessel and side branch were 0.30 +/- 0.64 mm and 0.41 +/- 0.67 mm, respectively, with binary restenosis rates of 9.1% and 25.3%. Kissing balloon post-dilation significantly reduced the side branch late lumen loss (0.24 +/- 0.50 mm vs. 0.58 +/- 0.77 mm, p < 0.001).CONCLUSIONS The crush technique of bifurcation stenting with drug-eluting stents is associated with favorable outcomes for most lesions; however, efficacy appears significantly reduced in LMS bifurcations, and further research is needed before the technique can be routinely recommended in this group. Furthermore, the incidence of possible stent thrombosis is of concern and requires further investigation. Kissing balloon post-dilatation is mandatory to reduce side branch restenosis. Abstract:OBJECTIVES:This study sought to compare the outcomes of fractional flow reserve (FFR)-guided and angiography (Angio)-guided provisional side-branch (SB) stenting for true coronary bifurcation lesions. BACKGROUND:Angio-guided provisional SB stenting after stenting of the main vessel provides favorable outcomes for the majority of coronary bifurcation lesions. Whether an FFR-guided provisional stenting approach is superior has not been studied. METHODS:A total of 320 patients with single Medina 1,1,1 and 0,1,1 coronary bifurcation lesions undergoing stenting with a provisional SB approach were randomly assigned 1:1 to Angio-guided and FFR-guided groups. SB stenting was performed for Thrombolysis In Myocardial Infarction flow grade<3, ostial SB stenosis>70%, or greater than type A dissection after main vessel stenting in the Angio-guided group and for SB-FFR<0.80 in the FFR-guided group. The primary endpoint was the 1-year composite rate of major adverse cardiac events (cardiac death, myocardial infarction, and clinically driven target vessel revascularization). RESULTS:Comparing the Angio-guided and FFR-guided groups, treatment of the SB (balloon or stenting) was performed in 63.1% and 56.3% of lesions respectively (p=0.07); stenting of the SB was attempted in 38.1% and 25.9%, respectively (p=0.01); and, when attempted, stenting was successful in 83.6% and 73.3% of SBs, respectively (p=0.01). The 1-year composite major adverse cardiac event rate was 18.1% in both groups (hazard ratio: 0.91, 95% confidence interval: 0.48 to 1.88; p=1.00). The 1-year target vessel revascularization and stent thrombosis rates were 6.9% and 5.6% (p=0.82) and 1.3% and 0.6% (p=0.56) in the Angio-guided and FFR-guided groups, respectively. CONCLUSIONS:In this multicenter, randomized trial, angiographic and FFR guidance of provisional SB stenting of true coronary bifurcation lesions provided similar 1-year clinical outcomes. Coronary Bifurcation Angle from 3-D Predicts Clinical Outcomes after Stenting Bifurcation Lesions

Abstract:Background The predictive value of bifurcation angle (BA) for worse events after stenting bifurcation lesions remains to be unknown. The present study was to investigate the dynamic change of BA and clinical relevance for patients with coronary bifurcation lesions treated by drug-eluting stent (DES).Methods BA was calculated by 3-D quantitative coronary analysis from 347 patients in DKCRUSH-II study. Primary endpoint was the occurrence of composite major adverse cardiac events (MACE) at 12-month, including cardiac death, myocardial infarction (MI) and target vessel revascularization (TVR). Secondary end points were the rate of binary restenosis and stent thrombosis at 12-month.Results Stenting was associated with the reduction of distal BA. The cut-off value of distal BA for predicting MACE was 60 degrees. Distal BA in <60 degrees group had less reduction after stenting ((-1.96 +/- 13.58)degrees vs. (-12.12 +/- 23.58)degrees, P <0.001); two-stent technique was associated with significant reduction of distal BA (Delta(-4.05 +/- 14.20)degrees), compared to single stent group (Delta+1.55 +/- 11.73, P=0.003); the target lesion revascularization (TLR), TVR and MACE rate was higher in one-stent group (16.5%, 19.0% and 21.5%), compared to two-stent group (3.8%, P=0.002; 7.5%, P=0.016; and 9.8%, P=0.024), respectively. Among patients in >= 60 degrees group, there were no significant differences in distal BA, stent thrombosis (ST), MI, MACE, death, TLR, TVR between one- and two-stent groups; after stenting procedure, there was only slight change of distal BA in left anterior descending (LAD)-left circumflex (LCX) subgroup (from (88.54 +/- 21.33)degrees at baseline to (82.44 +/- 31.72)degrees post-stenting), compared to either LAD-diagonal branch (Di), or LCX-obtuse marginal branch (OM), or RCA distal (RCAd) (all P <0.001).Conclusion Two-stent technique was associated with significant reduction of distal BA. Clinical Outcome of Double Kissing Crush Versus Provisional Stenting of Coronary Artery Bifurcation Lesions

Abstract:Background— Provisional stenting is effective for anatomic simple bifurcation lesions. Double kissing crush stenting reduces the 1-year rate of target lesion revascularization. This study aimed to investigate the 5-year clinical results of the DKCRUSH-II study (Randomized Study on Double Kissing Crush Technique Versus Provisional Stenting Technique for Coronary Artery Bifurcation Lesions). Methods and Results— A total of 370 patients with coronary bifurcation lesions who were randomly assigned to either the double kissing crush or provisional stenting group in the DKCRUSH-II study were followed for 5 years. The primary end point was the occurrence of a major adverse cardiac event at 5 years. Patients were classified by simple and complex bifurcation lesions according to the DEFINITION criteria (Definitions and Impact of Complex Bifurcation Lesions on Clinical Outcomes After Percutaneous Coronary Intervention Using Drug-Eluting Stents). At 5 years, the major adverse cardiac event rate (23.8%) in the provisional stenting group was insignificantly different to that of the double kissing group (15.7%; P =0.051). However, the difference in the target lesion revascularization rate between 2 groups was sustained through the 5-year follow-up (16.2% versus 8.6%; P =0.027). The definite and probable stent thrombosis rate was 2.7% in each group ( P =1.0). Complex bifurcation was associated with a higher rate of target lesion revascularization (21.6%) at 5 years compared with 11.1% in patients with a simple bifurcation ( P =0.037), with an extremely high rate in the provisional stenting group (36.8% versus 12.5%, P =0.005) mainly because of final kissing balloon inflation (19.4% versus 5.2%; P =0.036). Conclusions— The double kissing crush stenting technique for coronary bifurcation lesions is associated with a lower rate of target lesion revascularization. The optimal stenting approach based on the lesions’ complexity may improve the revascularization for patients with complex bifurcations. Clinical Trial Registration— URL: http://www.chictr.org . Abstract:Abstract : Starting from the conventional definition of the state of a Fermi system containing a quasiparticle of momentum k, we construct a state with a quasiparticle wave packet (QPWP) by the usual superposition. We have studied the distributions of charges and currents in such a state to first order in the inter-particle interaction. The dressed QPWP state contains a charge e-bar which is less than the bare charge, e. (The charge e-bar is different from an incorrect value previously proposed on different grounds.) The remaining charges resides at infinity. In more dimensions than one, the charge density of the QPWP also contains anisotropic components that fall off as 1/r-cubed away from the center of the wave packet. Similar conclusions hold for the current density. We have verified that the continuity equation is satisfied at every point in space. The relationship of these results to the classical formal Landau theory is discussed. The Landau quasi particle distribution function, n(k,r), can not be interpreted in terms of the QPWP's defined here. Finally, experimental consequences of the work presented in this paper are briefly discussed.
Treatment Of Bifurcation Lesions With Two Stents: One Year Angiographic And Clinical Follow Up Of Crush Versus T Stenting

Abstract:Objectives: To compare long term outcomes of the crush versus the T technique in bifurcation lesions.Design: 182 consecutive patients were identified who underwent percutaneous coronary interventions for bifurcation lesions with drug eluting stents between April 2002 and January 2004. Two techniques were used according to the operator's discretion: crush ( group C, n = 121) or T ( group T, n = 61).Results: In-hospital outcome differed significantly between the two groups. Angiographic follow up was available for 142 (78%) patients. Groups C and T did not differ significantly regarding late loss (0.42 (0.39) mm v 0.34 (0.35) mm, p = 0.52) and rate of restenosis (16.2% v 13.0%, p = 0.80) in both the main and the side branch without final kissing balloon post-dilatation. However, when final kissing balloon post-dilatation was performed, group C had significantly lower late lumen loss (0.23 (0.21) mm v 0.37 (0.33) mm, p = 0.02) and restenosis rate (8.6% v 26.5%, p = 0.04) in the side branch. At one year's clinical follow up, group C compared with group T had lower rates of target lesion revascularisation (14.0% v 31.1%, p = 0.01) and target vessel revascularisation (16.5% v 32.8%, p = 0.02).Conclusions: In non-selected bifurcation lesions treated with drug eluting stents, the restenosis rate remains relatively high in the side branch. Compared with the T stenting technique, crush stenting with kissing balloon post-dilatation is associated with a reduced rate of restenosis in the side branch.

Abstract:Background: Techniques for provisional and dual-stent left main bifurcation stenting require optimization. Aim: To identify technical variables influencing procedural outcomes and periprocedural myocardial infarction following left main bifurcation intervention. Methods: Procedural and outcome data were analyzed in 438 patients from the per-protocol cohort of the European Bifurcation Club Left Main Trial (EBC MAIN). These patients were randomized to the provisional strategy or a compatible dual-stent extension (T, T-and-protrude, or culotte). Results: Mean age was 71 years and 37.4% presented with an acute coronary syndrome. Transient reduction of side vessel thrombolysis in myocardial infarction flow occurred after initial stent placement in 5% of procedures but was not associated with periprocedural myocardial infarction. Failure to rewire a jailed vessel during any strategy was more common when jailed wires were not used (9.5% vs. 2.5%, odds ratio [OR]: 6.4, p = 0.002). In the provisional cohort, the use of the proximal optimization technique was associated with less subsequent side vessel intervention (23.3% vs. 41.9%, OR: 0.4, p = 0.048). Side vessel stenting was predominantly required for dissection, which occurred more often following side vessel preparation (15.3% vs. 4.4%, OR: 3.1, p = 0.040). Exclusive use of noncompliant balloons for kissing balloon inflation was associated with reduced need for side vessel intervention in provisional cases (20.5% vs. 38.5%, OR: 0.4, p = 0.013), and a reduced risk of periprocedural myocardial infarction across all strategies (2.9% vs. 7.7%, OR: 0.2, p = 0.020). Conclusion: When performing provisional or compatible dual-stent left main bifurcation intervention, jailed wire use is associated with successful jailed vessel rewiring. Side vessel preparation in provisional patients is linked to increased side vessel dissection requiring stenting. Use of the proximal optimization technique may reduce the need for additional side vessel intervention, and noncompliant balloon use for kissing balloon inflation is associated with a reduction in both side vessel stenting and periprocedural myocardial infarction. Cumulative 12-month major adverse cardiac event (MACE) was 22% in the whole STEMI group, whereas PS and DK crush groups were 23% versus 21%, respectively (P = NS). There were no differences in in-hospital, 6-month, and 12-month MACE in these two groups. At 6 and 12 months, there were two cardiac deaths in the PS group but without statistical significance when compared with the DK crush stenting group (7% vs. 0%, P = NS). Conclusion: Bifurcation stenting in patients with STEMI is safe and feasible. The immediate and midterm clinical outcomes were comparable between PS and DK crush stenting. A Bayesian meta-analysis of double kissing (DK) crush or provisional stenting for coronary artery bifurcation lesions

Abstract:Objective: Despite the development of dedicated, two-stent strategies, including the double kissing (DK) crush technique, the ideal technique for coronary artery bifurcation stenting has not been identified. We aimed to compare and determine the absolute risk difference (ARD) of the DK crush technique alone versus provisional stenting approaches for coronary bifurcation lesions, using the Bayesian technique. Method: We queried PubMed/MEDLINE to identify randomized controlled trials (RCTs) that compared DK crush technique with provisional stenting for bifurcation lesions, published till January 2023. We used Bayesian methods to calculate the ARD and 95% credible interval (CrI). Results: We included three RCTs, with 916 patients, in the final analysis. The ARD of cardiac death was centered at -0.01 (95% CrI: -0.04 to 0.02; Tau: 0.02, 85% probability of ARD of DK crush vs. provisional stenting <0). ARD for myocardial infarction was centered at -0.03 (95%CrI: -0.9 to 0.03; Tau: 0.05, 87% probability of ARD of DK crush vs. provisional stenting <0). ARD for stent thrombosis was centered at 0.00 (95% CrI: -0.04 to 0.03, Tau: 0.03, 51% probability of ARD for DK crush vs. provisional stenting <0). Finally, ARD for target lesion revascularization was centered at -0.05 (95% CrI: -0.08 to -0.03, Tau: 0.02, 99.97% probability of ARD for DK crush vs. provisional stenting <0). Conclusions: Bayesian analysis demonstrated a lower probability of cardiac death, myocardial infarction and target lesion revascularization, with DK crush compared with provisional stenting techniques, and a minimal probability of difference in stent thrombosis.
Stenting techniques for coronary bifurcation disease: a systematic review and network meta-analysis demonstrates superiority of double-kissing crush in complex lesions

Abstract:Abstract Objective This study was aimed to compare different stenting techniques for coronary bifurcation disease (CBD). Background Percutaneous coronary intervention (PCI) remains controversial for CBD; over the years, several stent techniques for bifurcation lesions have been used. Current guidelines recommend a provisional single-stent strategy as the preferred method for coronary artery bifurcation lesions. However, several randomized controlled trials (RCT) indicated that two-stent techniques showed better clinical outcomes. Methods We systematically searched Embase, PubMed, and Web of Science to include RCTs. The primary endpoint was the major adverse cardiovascular event (MACE). Secondary outcomes were cardiac death, myocardial infarction (MI), target-lesion or target-vessel revascularization (TLR or TVR), and definite or probable stent thrombosis (ST). Finally, we used 26 RCTs and a total of 7257 individuals were randomly assigned to one of the 6 stent techniques and included in this network meta-analysis. Results In our network meta-analysis, double-kissing (DK) crush was significantly more superior to other 5 stent techniques in MACEs: OR vs. provisional 0.40 (95% CI 0.28–0.55); vs. culotte 0.40 (95% CI 0.26–0.60). DK crush ranked the most effective treatment for MACE (100%), MI (75%), ST (83%), and TLR (100%) in the rank probabilities analysis. In patients with complex bifurcation lesion defined by DEFINITION criteria, DK crush was notably more efficacious than provisional, culotte, and T-stenting/T-stenting and protrusion (TAP) in MACEs (OR vs. provisional 0.26, 95% CI 0.13–0.52) and TLR (OR vs. provisional 0.24, 95% CI 0.10–0.58). Conclusion Compared with other stenting techniques, DK crush had a lower incidence of MACEs in CBD. DK crush was significantly associated with a lower rate of MACEs in patients with complex bifurcation lesions defined by the DEFINITION criterion. Graphical abstract

Abstract:Objectives The unprotected left main coronary artery (ULMCA) bifurcation stenosis remains challenging for percutaneous coronary intervention (PCI). The aim of the study was to evaluate the effectiveness and safety of bifurcation technique on the treatment of ULMCA bifurcation stenosis in drug eluting stent era. Methods Between March 2004 and November 2011, a total of 224 patients (168 male) underwent bifurcation technique with drug eluting stent for true bifurcation lesions of ULMCA. Patients with cardiogenic shock were excluded from the analysis. We defined true bifurcations as Medina 1,1,1 or 1,0,1 or 0,1,1. The indexes of in-hospital, angiographic restenosis at 12-months and major adverse cardiac events (MACE) including cardiac death, myocardial infarction or any target lesion revascularisation were evaluated. Results The procedural was successful in all patients. Bifurcation techniques included T stenting in 78(34.8%), Cullotte stenting in 69(30.8%) and Crush stenting in 77(34.3%) patients. Final “kissing balloon” inflation was preferred in 196 cases (87.5%). Intravascular ultrasound (IVUS) was performed in 79 cases during procedural. The mean stent diameter was (3.4 ± 0.40) mm in ULMCA. The overall rate of MACE was 1.3% (3/224) during hospital stay, including one acute stent thrombosis during procedure and 2 patients underwent repeat angioplasty for subacute stent thrombosis after PCI procedure. One patient died for subacute stent thrombosis during hospital stay. All patients were followed-up for at least 12 months. Two patients died for congestive heart failure during follow-up. Angiographic follow-up was obtained in 96(42.9%) patients. Binary in-stent restenosis occurred in 15(6.7%) patients and all of them were treated with CABG (7/15) or repeat PCI (8/15). The rate of target vessel revascularisation was significantly lower in patients with final kissing inflation during the index procedure compared with those without final kissing (4.1% (8/196) vs 25.0% (7/28); P Conclusions Our experience indicated that bifurcation stenting for LMCA bifurcation lesions in elective patients could achieve a high technical success rate. Final kissing balloon might reduce the need for target vessel revascularisation.
