Abstract:[Objective]To analyze the risk factors for respiratory failure in patients with esophageal cancer after resection.[Methods] A total of 120 cases with respiratory failure after esophageal carcinoma resection were selected from May 2014 to May 2017 in the department of thoracic surgery of our hospital,60 cases in the observation group (respiratory failure) and 60 cases in the control group (no respiratory failure).Multivariate Logistic regression analysis and single factor analysis were carried out on the respiratory failure after esophageal carcinoma resection.[Results] The results of univariate analysis showed that there were no significant differences between the two groups in gender,tumor size,TNM stage,diabetes,anemia,tuberculosis,smoking history and preoperative blood pressure (P＞0.05),while there were significant differences between the two groups in lung ftmction,intraoperative fluid input,complication rate of preoperative chronic obstructive pulmonary disease (COPD),age,incision number,operation time,the rate of severe complications and the incidence of second operation (P＜0.05).Theresults of logistic regression analysis indicated that the operation time [OR=1.650,95％CI (1.041,2.616),P=0.033],advanced age [OR=1.582,95％CI (1.019,2.457),P=0.041],second operation [OR=5.631,95％CI (1.096,28.935),P=0.039],severe pulmonary dysfunction [OR=2.539,95％CI (1.056,6.107),P=0.037],serious complications [OR=2.189 (1.048,95％CI (1.048,4.573),P=0.037],2 incisions [OR=6.524,95％CI (2.603,16.350),P=0.000],3 surgical incisions [OR=8.028,95％CI (1.534,42.002,P=0.014) were entered in the regression equation.[Conclusion] The main risk factors for respiratory failure after esophageal cancer resection are as follows:long operation time,serious complications,preoperative severe pulmonary dysfunction,more intraoperative incisions and second operation.

Abstract:BACKGROUND: Timely detection of modifiable risk factors for postoperative pulmonary complications (PPCs) could inform ventilation strategies that attenuate lung injury. We sought to develop, validate, and internally test machine learning models that use intraoperative respiratory features to predict PPCs.METHODS: We analysed perioperative data from a cohort comprising patients aged 65 yr and older at an academic medical centre from 2019 to 2023. Two linear and four nonlinear learning models were developed and compared with the current gold-standard risk assessment tool ARISCAT (Assess Respiratory Risk in Surgical Patients in Catalonia Tool). The Shapley additive explanation of artificial intelligence was utilised to interpret feature importance and interactions.RESULTS: Perioperative data were obtained from 10 284 patients who underwent 10 484 operations (mean age [range] 71 [65-98] yr; 42% female). An optimised XGBoost model that used preoperative variables and intraoperative respiratory variables had area under the receiver operating characteristic curves (AUROCs) of 0.878 (0.866-0.891) and 0.881 (0.879-0.883) in the validation and prospective cohorts, respectively. These models outperformed ARISCAT (AUROC: 0.496-0.533). The intraoperative dynamic features of respiratory dynamic system compliance, mechanical power, and driving pressure were identified as key modifiable contributors to PPCs. A simplified model based on XGBoost including 20 variables generated an AUROC of 0.864 (0.852-0.875) in an internal testing cohort. This has been developed into a web-based tool for further external validation (https://aorm.wchscu.cn/).CONCLUSIONS: These findings suggest that real-time identification of surgical patients' risk of postoperative pulmonary complications could help personalise intraoperative ventilatory strategies and reduce postoperative pulmonary complications.

A Clinical Prediction Model for Postoperative Pneumonia After Lung Cancer Surgery

Abstract:Introduction: Postoperative pneumonia (POP) is a common complication following lung can-cer surgery and is associated with increased hospitalization costs and mortalities. We aimed to identify risk factors associated with POP and to develop a reliable predictive model.Methods: Patients who underwent lung cancer surgery between January 2015 and December 2021 in our hospital were enrolled. Least absolute shrinkage and selection operator regression analysis was used to select predictors of POP. Multivariable logistic regression was performed to construct the nomogram. Bootstrap resampling was conducted for in-ternal validation. The performance of the model was evaluated by discrimination and calibration.Results: A total of 5269 consecutive patients were enrolled. POP occurred in 1.7% of patients (92/5269). Five independent predictors were identified: age, predicted forced expiratory volume in 1 s, predicted diffusing capacity of the lungs for carbon monoxide, tuberculosis history, and surgery duration. The multivariable regression model showed good discrimi-nation (C-index: 0.821, 95% confidence interval, 0.783-0.859), which was well validated by internal validation. The calibration curve illustrated good agreement between the pre-dicted probability and observed probability of POP.Conclusions: Based on the easily available risk factors, our nomogram could predict the risk of POP with good discrimination and calibration. The model has good clinical practicability, enabling precise and targeted interventions to reduce the incidence of POP in high-risk patients.(c) 2022 Published by Elsevier Inc.
Prediction of Postoperative Cardiopulmonary Complications after Lung Resection in a Chinese Population: A Machine Learning-Based Study

Abstract:Background Approximately 20% of patients with lung cancer would experience postoperative cardiopulmonary complications after anatomic lung resection. Current prediction models for postoperative complications were not suitable for Chinese patients. This study aimed to develop and validate novel prediction models based on machine learning algorithms in a Chinese population. Methods Patients with lung cancer receiving anatomic lung resection and no neoadjuvant therapies from September 1, 2018 to August 31, 2019 were enrolled. The dataset was split into two cohorts at a 7:3 ratio. The logistic regression, random forest, and extreme gradient boosting were applied to construct models in the derivation cohort with 5-fold cross validation. The validation cohort accessed the model performance. The area under the curves measured the model discrimination, while the Spiegelhalter z test evaluated the model calibration. Results A total of 1085 patients were included, and 760 were assigned to the derivation cohort. 8.4% and 8.0% of patients experienced postoperative cardiopulmonary complications in the two cohorts. All baseline characteristics were balanced. The values of the area under the curve were 0.728, 0.721, and 0.767 for the logistic, random forest and extreme gradient boosting models, respectively. No significant differences existed among them. They all showed good calibration (p > 0.05). The logistic model consisted of male, arrhythmia, cerebrovascular disease, the percentage of predicted postoperative forced expiratory volume in one second, and the ratio of forced expiratory volume in one second to forced vital capacity. The last two variables, the percentage of forced vital capacity and age ranked in the top five important variables for novel machine learning models. A nomogram was plotted for the logistic model. Conclusion Three models were developed and validated for predicting postoperative cardiopulmonary complications among Chinese patients with lung cancer. They all exerted good discrimination and calibration. The percentage of predicted postoperative forced expiratory volume in one second and the ratio of forced expiratory volume in one second to forced vital capacity might be the most important variables. Further validation in different scenarios is still warranted.
Analysis of Risk Factors for Acute Lung Injury/acute Respiratory Distress Syndrome after Esophagectomy

Abstract:OBJECTIVE:To explore the incidence and risk factors for the acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) after resection of esophageal carcinoma.METHODS:We retrospectively analyzed 422 consecutive patients admitted to the Department of Critical Care Medicine with esophageal carcinoma undergoing esophagectomy from January 2010 to December 2016 in Peking University Cancer Hospital. ALI/ARDS were diagnosed, the patients were divided into ALI/ARDS group and control group without ALI/ARDS, the differences of clinical features were contrasted between the two groups, and the multivariate Logistic regression modeling was used to identify the independent risk factors for ALI/ARDS.RESULTS:In the study, 41 ALI/ARDS cases were diagnosed, making up 9.7% (41/422) of all the enrolled patients undergoing esophagectomy. Comparisons of the ALI/ARDS group and the control group indicated significant statistical differences in the average length of their hospital stay [(18.9±9.7) d vs. (14.8±3.6) d, P=0.011], the proportion of the patients who needed mechanical ventilation support [51.2% (21/41) vs. 9.4% (36/381), P<0.001] and in-hospital mortality [31.7% (13/41) vs. 5.0% (19/381), P<0.001]. Univariate analysis showed significant differences between the patients with ALI/ARDS and without ALI/ARDS in smoking history (P=0.064), preoperative forced expiratory volume in one second/forced vital capacity (FEV1/FVC) (P=0.020), diffusing capacity of the lung for carbon monoxide (DLCO) (P=0.011), body weight index (BMI) (P=0.044), American Society of Anesthesiologists (ASA) physical status classification (P=0.049) and one lung ventilation duration (P=0.008), while multivariate Logistic regression analysis indicated that preoperative FEV1/FVC (OR=1.053, P=0.016, 95%CI 1.010-1.098), ASA physical status classification (OR=2.392, P=0.033, 95%CI 1.073-5.335) and one lung ventilation duration (OR=0.994, P=0.028, 95%CI 0.989-0.999) were the independent risk factors for ALI/ARDS after esophagectomy.CONCLUSION:ALI/ARDS was a serious complication in patients undergoing esophagectomy associated with increment in length of hospital stay and in-hospital mortality. Multivariate Logistic regression analysis indicated that preoperative FEV1/FVC, ASA classification and one lung ventilation duration were the independent risk factors for ALI/ARDS after esophagectomy. Carefully assessing the patient before operation, shortening one lung ventilation duration were the key points in preventing ALI/ARDS after esophagectomy.

Abstract:Purpose: This study investigated the perioperative risk factors of postoperative pulmonary complications (PPCs) after minimally invasive anatomic resection for lung cancer. Patients and methods: We retrospectively reviewed the data from medical records of 729 lung cancer patients undergoing minimally invasive anatomic lung resections between January 2017 and December 2017. Univariate and binary logistic regression analyses were performed to select the independent risk factors for PPCs during the patient's postoperative hospitalization after surgery. Results: The incidence of PPCs was 24.8% (n=181/729). No patient died during the period of hospitalization. Logistic regression analysis revealed that body mass index (BMI) >= 24.0 kg/m(2) (vs <24.0 kg/m(2) : OR 1.514, 95% CI 1.057-2.167, P=0.024), single segmentectomy (vs single lobectomy: OR 2.115, 95% CI 1.150-3.891, P=0.016), bilobectomy or combined lobectomy and segmentectomy (vs single lobectomy: OR 2.731, 95% CI 1.013-7.361, P=0.047), and right lung lobe surgery (vs left lung lobe surgery: OR 1.519, 95% CI 1.046 2.205, P=0.028) were independent risk factors for PPCs in lung cancer patients who received minimally invasive anatomic lung resections. Conclusion: Individual factors such as BMI >= 24.0 kg/m(2) , single segmentectomy, bilobectomy or combined lobectomy and segmentectomy, and right lung lobe surgery were independent risk factors of PPCs, which should be helpful for risk stratification, patient counseling, and perioperative care for lung cancer patients.

Abstract:The widespread use of digital rectal examination (DRE) coupled with measurement of prostate-specific antigen (PSA) for prostate cancer screening has led to a dramatic increase in the number of men undergoing prostate biopsy guided by transrectal ultrasound (TRUS). Systematic parasagittal sextant biopsies have been widely adopted as the standard protocol for prostate biopsy. Significant numbers of men undergo repeat biopsy because of clinical indications (abnormal DRE, elevated PSA) or atypical findings on the initial biopsy. Increases in biopsy number and changes in distribution have recently been proposed in attempts to improve the diagnostic yield of initial biopsy.
Development and validation of a nomogram for predicting pulmonary complications after video-assisted thoracoscopic surgery in elderly patients with lung cancer

Abstract:Background: Postoperative pulmonary complications (PPCs) significantly increase the morbidity and mortality in elderly patients with lung cancer. Considering the adverse effects of PPCs, we aimed to derive and validate a nomogram to predict pulmonary complications after video-assisted thoracoscopic surgery in elderly patients with lung cancer and to assist surgeons in optimizing patient-centered treatment plans. Methods: The study enrolled 854 eligible elderly patients with lung cancer who underwent sub-lobectomy or lobectomy. A clinical prediction model for the probability of PPCs was developed using univariate and multivariate analyses. Furthermore, data from one center were used to derive the model, and data from another were used for external validation. The model's discriminatory capability, predictive accuracy, and clinical usefulness were assessed using the receiver operating characteristic (ROC) curve, calibration curve, and decision curve analysis, respectively. Results: Among the eligible elderly patients with lung cancer, 214 (25.06%) developed pulmonary complications after video-assisted thoracoscopic surgery. Age, chronic obstructive pulmonary disease, surgical procedure, operative time, forced expiratory volume in one second, and the carbon monoxide diffusing capacity of the lung were independent predictors of PPCs and were included in the final model. The areas under the ROC curves (AUC) of the training and validation sets were 0.844 and 0.796, respectively. Ten-fold cross-validation was used to evaluate the generalizability of the predictive model, with an average AUC value of 0.839. The calibration curve showed good consistency between the observed and predicted probabilities. The proposed nomogram showed good net benefit with a relatively wide range of threshold probabilities. Conclusion: A nomogram for elderly patients with lung cancer can be derived using preoperative and intraoperative variables. Our model can also be accessed using the online web server https://pulmonary-disease-predictor.shinyapps.io/dynnomapp/. Combining both may help surgeons as a clinically easy-to-use tool for minimizing the prevalence of pulmonary complications after lung resection in elderly patients.

Methods: Seven hundred and sixty-seven consecutive patients who underwent McKeown MIE via thoracoscopy and laparoscopy were retrospectively studied. Patient characteristics, perioperative data, and postoperative complications were analyzed. Results: The incidence of PPCs after MIE was 25.2% (193/767). Univariate analysis identified age (odds ratio [OR] 1.022, P = 0.044), male sex (OR 2.955, P < 0.001), pulmonary comorbidities (OR 1.746, P = 0.032), chronic obstructive pulmonary disease (COPD) (OR 2.821, P = 0.003), former smoking status (OR 1.880, P = 0.001), postoperative albumin concentration (OR 0.941, P = 0.007), postoperative creatinine concentration (OR 1.011, P = 0.019), and perioperative transfusion (OR 2.250, P = 0.001) as risk factors for PPCs. In multivariate analysis, the independent risk factors for PPCs were male sex (OR 3.135, P < 0.001), body mass index (BMI) (OR 1.088, P = 0.002), COPD (OR 2.480, P = 0.012), neoadjuvant chemoradiotherapy (OR 2.057, P = 0.035), postoperative albumin concentration (OR 0.929, P = 0.002), and perioperative transfusion (OR 1.939, P = 0.013). The area under the receiver operating characteristic curve for the predictive model generated by multivariate logistic regression analysis was 0.671 (95% confidence interval 0.628– 0.713). Conclusions: Male sex, BMI, COPD, neoadjuvant chemoradiotherapy, postoperative albumin concentration, and perioperative transfusion were independent predictors of PPCs after MIE. MIE is reportedly associated with significantly better short-term outcomes than open esophagectomy. 6,7 Studies have also found a lower incidence of PPCs in patients undergoing MIE compared with open surgery. 8–10 In a multi-center randomized controlled trial of patients undergoing esophagectomy, the incidence of a major pulmonary complication was 18% in the hybrid-procedure group and 30% in the open-procedure group. 9 Additionally, MIE reportedly decreases the risk of respiratory failure after esophagectomy. 11 However, PPCs remain a major concern after MIE, and the risk factors related to PPCs after MIE have not been fully investigated. Therefore, it would be beneficial for both patients and healthcare providers to identify the perioperative risk factors that influence the development of PPCs, especially regarding the best protective strategies during perioperative management of MIE to enhance clinical decision-making. The aim of this study was to assess the perioperative risk factors that influence the occurrence of PPCs after total MIE. This study was approved by the Ethics Committee of Peking University Cancer Hospital, Beijing, China. The need for patient consent was waived by the ethics committee due to the retrospective study design. Risk Factor Analysis and Nomogram Prediction Model Construction of Postoperative Complications of Thoracoscopic Non-Small Cell Lung Cancer

Abstract:Background: A series of complications will inevitably occur after thoracoscopic pulmonary resection. How to avoid or reduce postoperative complications is an important research area in the perioperative treatment of thoracic surgery. This study analyzed the risk factors for thoracoscopic postoperative complications of non-small cell lung cancer (NSCLC) and established a nomogram prediction model in order to provide help for clinical decision-making. Methods: Patients with NSCLC who underwent thoracoscopic surgery from January 2017 to December 2021 were selected as study subjects. The relationship between patient characteristics, surgical factors, and postoperative complications was collected and analyzed. Based on the results of the statistical regression analysis, a nomogram model was constructed, and the predictive performance of the nomogram model was evaluated. Results: A total of 872 patients who met the study criteria were included in the study. A total of 171 patients had complications after thoracoscopic surgery, accounting for 19.6% of the study population. Logistic regression analysis showed that thoracic adhesion, history of respiratory disease, and lymphocyte-monocyte ratio (LMR) were independent risk factors for complications after thoracoscopic surgery (P<0.05). Variables with P<0.1 in logistic regression analysis were included in the nomogram model. The verification results showed that the area under curve (AUC) of the model was 0.734 [95% confidence interval (CI): 0.693-0.775], and the calibration curve showed that the model had good differentiation. The decision curve analysis (DCA) curve showed that this model has good clinical application value. In subgroup analysis of complications, gender, history of respiratory disease, body mass index (BMI), type of surgical procedure, thoracic adhesion, and Time of operation were identified as significant risk factors for prolonged air leak (PAL) after surgery. Tumor location and forced expiratory volume in the first second (FEV1) 1 ) were identified as important risk factors for postoperative pulmonary infection. N stage and thoracic adhesion were identified as significant risk factors for postoperative pleural effusion. The AUC for PAL was 0.823 (95% CI: 0.768-0.879). The AUC of postoperative pulmonary infection was 0.714 (95% CI: 0.627-0.801). The AUC of postoperative pleural effusion was 0.757 (95% CI: 0.650-0.864). The calibration curve and DCA curve indicated that the model had good predictive performance and clinical application value. Conclusions: This study analyzed the risk factors affecting the postoperative complications of NSCLC through thoracoscopic surgery, and the nomogram model built based on the influencing factors has certain significance for the identification and reduction of postoperative complications.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>When pulmonary complications occur, postlobectomy patients have a higher mortality rate, increased length of stay, and higher readmission rates. Because of a lack of high-quality consolidated clinical data, it is challenging to assess and recognize at-risk thoracic patients to avoid respiratory failure and standardize outcome measures.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The National (Nationwide) Inpatient Sample for 2015 was used to establish our model. We identified 417 respiratory failure from a total of 4,062 patients who underwent pulmonary lobectomy. Risk factors for respiratory failure were identified, analyzed, and used in novel machine learning models to predict respiratory failure.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Factors that contributed to increased odds of respiratory failure, such as preexisting chronic diseases, and intraoperative and postoperative events during hospitalization were identified. Two machine learning-based prediction models were generated and optimized by the knowledge accrued from the clinical course of postlobectomy patients. The first model, with high accuracy and specificity, is suited for performance evaluation, and the second model, with high sensitivity, is suited for clinical decision making.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>We identified risk factors for respiratory failure after lobectomy and introduced 2 machine learning-based techniques to predict respiratory failure for quality review and clinical decision-making settings. Such techniques can be used to not only provide targeted support but also standardize quality peer review measures.</p>

Abstract:Surgical treatment for elderly patients with lung cancer presents more challenges compared with general population. The aim of the study was to predict surgical outcome following pulmonary resection in the elderly with lung cancer by developing a clinical model. Clinical records of 525 patients aged over 70 years who underwent pulmonary resection for lung cancer in a single center were reviewed. Patients were divided into three ordered categories of surgical outcome according to the Clavien–Dindo classification. Using a development cohort of 401 patients, an ordinal logistic regression was performed to develop a prediction model for surgical outcome. The model was internally validated by bootstrap method and externally validated by another cohort of 124 patients. Two previous models were tested as benchmarks of our model. The model was developed based on five risk factors of morbidity: ASA classification (p<0.001), pulmonary disease (p=0.001), tumor size (p=0.011), tumor location (p=0.015) and surgical approach (p=0.036). C-statistics of the model was similar to bootstrapping one. Hosmer-Lemeshow test showed a good goodness-of-fit. Multicentre validation of a machine learning model for predicting respiratory failure after noncardiac surgery

Abstract:BACKGROUND: Postoperative respiratory failure is a serious complication that could benefit from early accurate identification of high-risk patients. We developed and validated a machine learning model to predict postoperative respiratory failure, defined as prolonged (>48 h) mechanical ventilation or reintubation after surgery.METHODS: Easily extractable electronic health record (EHR) variables that do not require subjective assessment by clinicians were used. From EHR data of 307,333 noncardiac surgical cases, the model, trained with a gradient boosting algorithm, utilised a derivation cohort of 99,025 cases from Seoul National University Hospital (2013-9). External validation was performed using three separate cohorts A-C from different hospitals comprising 208,308 cases. Model performance was assessed by area under the receiver operating characteristic (AUROC) curve and area under the precision-recall curve (AUPRC), a measure of sensitivity and precision at different thresholds.RESULTS: The model included eight variables: serum albumin, age, duration of anaesthesia, serum glucose, prothrombin time, serum creatinine, white blood cell count, and body mass index. Internally, the model achieved an AUROC of 0.912 (95% confidence interval [CI], 0.908-0.915) and AUPRC of 0.113. In external validation cohorts A, B, and C, the model achieved AUROCs of 0.879 (95% CI, 0.876-0.882), 0.872 (95% CI, 0.870-0.874), and 0.931 (95% CI, 0.925-0.936), and AUPRCs of 0.029, 0.083, and 0.124, respectively.CONCLUSIONS: Utilising just eight easily extractable variables, this machine learning model demonstrated excellent discrimination in both internal and external validation for predicting postoperative respiratory failure. The model enables personalised risk stratification and facilitates data-driven clinical decision-making.

Abstract:BACKGROUND:Esophagectomy is a very important method for the treatment of resectable esophageal cancer, which carries a high rate of morbidity and mortality. This study was undertaken to assess the predictive score proposed by Ferguson et al for pulmonary complications after esophagectomy for patients with cancer.METHODS:The data of patients who admitted to the intensive care unit after transthoracic esophagectomy at Cancer Hospital of Chinese Academy of Medical Sciences and Peking Union Medical College between September 2008 and October 2010 were retrospectively reviewed.RESULTS:Two hundred and seventeen patients were analyzed and 129 (59.4%) of them had postoperative pulmonary complications. Risk scores varied from 0 to 12 in all patients. The risk scores of patients with postoperative pulmonary complications were higher than those of patients without postoperative pulmonary complications (7.27±2.50 vs. 6.82±2.67; P=0.203). There was no significant difference in the incidence of postoperative pulmonary complications as well as in the increase of risk scores (χ (2)=5.477, P=0.242). The area under the curve of predictive score was 0.539±0.040 (95%CI 0.461 to 0.618; P=0.324) in predicting the risk of pulmonary complications in patients after esophagectomy.CONCLUSION:In this study, the predictive power of the risk score proposed by Ferguson et al was poor in discriminating whether there were postoperative pulmonary complications after esophagectomy for cancer patients.

Risk factors for postoperative pneumonia after cardiac surgery: a prediction model

Abstract:BACKGROUND: Postoperative pneumonia is the main infectious complication following cardiac surgery and is associated with significant increases in morbidity, mortality and health care costs. The aim of this study was to identify potential risk factors related to the occurrence of postoperative pneumonia in adult patients undergoing cardiac surgery and to develop a predictive system.METHODS: Adult patients who underwent open heart surgery in our institution between 2016 and 2019 were enrolled in this study. Preoperative and intraoperative variables were collected and analyzed. A multivariate prediction model for evaluating the risk of postoperative pneumonia was established using logistic regression analysis via forward stepwise selection, and points were assigned to significant risk factors based on their regression coefficient values.RESULTS: Postoperative pneumonia occurred in 530 of the 5,323 patients (9.96%). Prolonged stays in the postoperative intensive care unit (ICU) and hospital, as well as higher mortality (25.66% versus 0.65%), were observed in patients with postoperative pneumonia. Multivariate analysis identified 13 independent risk factors including patient demographics, comorbidities, cardiac function, cardiopulmonary bypass (CPB) duration, and blood transfusion. The prediction model showed good discrimination (C-statistic: 0.80) and was well calibrated (Hosmer-Lemeshow χ2=7.907, P value =0.443). A 32-point risk score was generated, and then three risk intervals were defined.CONCLUSIONS: We derived and validated a prediction model for postoperative pneumonia after cardiac surgery incorporating 13 easily discernible risk factors. The scoring system may be helpful for individualized risk estimations and clinical decision-making.

An External-Validated Algorithm to Predict Postoperative Pneumonia Among Elderly Patients With Lung Cancer After Video-Assisted Thoracoscopic Surgery

Abstract:The aim of the study was to develop an algorithm to predict postoperative pneumonia among elderly patients with lung cancer after video-assisted thoracoscopic surgery. We analyzed 3,009 patients from the Thoracic Perioperative Database for Geriatrics in our hospital and finally enrolled 1,585 elderly patients (age≧65 years) with lung cancer treated with video-assisted thoracoscopic surgery. The included patients were randomly divided into a training group ( n = 793) and a validation group ( n = 792). Patients in the training group were used to develop the algorithm after screening up to 30 potential risk factors, and patients in the validation group were used to internally validate the algorithm. External validation of the algorithm was achieved in the external validation dataset after enrolling 165 elderly patients with lung cancer treated with video-assisted thoracoscopic surgery from two hospitals in China. Of all included patients, 9.15% (145/1,585) of patients suffered from postoperative pneumonia in the Thoracic Perioperative Database for Geriatrics, and 10.30% (17/165) of patients had postoperative pneumonia in the external validation dataset. The algorithm consisted of seven variables, including sex, smoking, history of chronic obstructive pulmonary disease (COPD), surgery duration, leukocyte count, intraoperative injection of colloid, and intraoperative injection of hormone. The C-index from the receiver operating characteristic curve (AUROC) was 0.70 in the training group, 0.67 in the internal validation group, and 0.71 in the external validation dataset, and the corresponding calibration slopes were 0.88 (95% confident interval [CI]: 0.37–1.39), 0.90 (95% CI: 0.46–1.34), and 1.03 (95% CI: 0.24–1.83), respectively. The actual probabilities of postoperative pneumonia were 5.14% (53/1031) in the low-risk group, 15.07% (71/471) in the medium-risk group, and 25.30% (21/83) in the high-risk group ( p < 0.001). The algorithm can be a useful prognostic tool to predict the risk of developing postoperative pneumonia among elderly patients with lung cancer after video-assisted thoracoscopic surgery.

Abstract:This study aimed to develop and validate a risk prediction model based on real-world data to assess the risk of delayed recovery from anesthesia in elderly lung adenocarcinoma patients underwent video-assisted thoracoscopic (VATS) radical resection. This study is a retrospective study of real-world data. A total of 257 elderly lung adenocarcinoma patients who underwent VATS radical resection from January 2022 to December 2023 in a tertiary hospital in Wuhan were selected. Patients were divided into delayed recovery ( n = 42) and non- delayed recovery group ( n = 215) according to whether delayed recovery occurred after anesthesia. Lasso regression was used to screen the independent variables. Logistic regression was used to analyze the risk factors of delayed recovery from anesthesia, and a nomogram model was established. Bootstrap method was used to internally verify the nomogram model. Delayed recovery from anesthesia occurred in 42 of 257 elderly lung adenocarcinoma patients underwent VATS radical resection (16.34%). Logistic regression analysis showed that anesthesia duration, intraoperative infusion volume, inhaled desflurane, preoperative respiratory tract infection, intraoperative hypothermia and diagnosed with hypertension were risk factors for delayed recovery from anesthesia in elderly lung adenocarcinoma patients underwent VATS radical resection ( P < 0.05). The area under receiver operating characteristic curve was 0.869, 95% CI (0.815 ~ 0.923). The optimal cutoff value was 0.198, the sensitivity was 0.738, and the specificity was 0.823. Hosmer-Lemeshow test showed that χ 2 = 7.346, P = 0.500. The decision curve analysis results have shown that the threshold probability is between 0.23 and 0.91, and the net benefit rate of the model is good. The risk prediction model constructed in this study can provide reference for medical staff to screen precisely high-risk of delayed recovery from anesthesia in elderly lung adenocarcinoma patients underwent VATS radical resection, which is of great significance.
