Abstract:A 67-year-old physician reported that he had experienced interm ittent hoarseness for many years . During the previous several month s, his voice had become dysphonic most of the time. His voice was soft and hoarse, but he denied any throat pain. He had no systemic compl aints. He was unaware of any symptoms of gas troesophagea l refIux disease, but he admitted to excessive use of Tabasco sauee. He did not smoke. Strobovideolaryngoscopy revea led a thickened, irregu lar, partiaIly leukoplakic lesion involving the anterior two-third s of the left musculomembranous vocal fold. The mass extended onto the medial surface of the vibratory margin. The region of the abnormality was markedly hypodynamic under strobosco pic light. There were varicosi ties on the superior surface of the lesion and prominent irregular vesseIs lateral to it. These findings were confirmed at the time of surgery (figu re). A submucosal infusion with saIine and epinephrine was used during excisional biopsy . Although the lesion was stiff, it was not fixed to the vocal ligament. The lesion was resected in its entirety without any disturba nce to the surrounding uninvolved tissue . Histopathologic analysis revealed the presence of hyperkeratosis with intraepi thelial atypia (dysp lasia) . He healed weIl and is being foIlowed closely.
Update on ketosis in dairy cattle with major emphasis on subclinical ketosis and abdominal adiposity

Abstract:The metabolic changes that occur during the early post-partum period in dairy cows can indeed lead to an imbalance in energy utilization, resulting in the production of excessive ketone bodies. This can have detrimental effects on the cow's health and milk production, leading to economic losses for dairy producers. The release of non-esterified fatty acids into the blood due to increased lipolysis is a key factor in the development of ketosis. Abdominal adiposity is a key factor on these outcomes in modern dairy cows. The redirection of energy and glucose towards lactose synthesis and milk yield leaves a deficit of gluconeogenic precursors, leading to the conversion of acetyl-CoA into ketone bodies instead of entering the Krebs cycle. These ketone bodies, including acetone, acetoacetate and β-hydroxybutyrate, accumulate in the blood and can be detected in various bodily fluids, such as urine, blood and milk, allowing for diagnostic testing. Prevention is indeed crucial in managing ketosis in dairy cattle. Supplementation of propylene glycol in the diet or the use of monensin, either in the diet or in the form of a slow-release bolus, can help prevent the occurrence of ketosis. However, avoiding high body condition (subcutaneous fat) and excessive abdominal adiposity during the dry period and parturition plus an adequate cow comfort are fundamental tasks to avoid ketosis and related disorders. These interventions aim to provide additional energy sources or enhance the cow's ability to utilize energy efficiently, thus reducing the reliance on excessive lipolysis and ketone body production.

Abstract:Introduction Ketosis in dairy cows is characterized by increased levels of circulating ketone bodies, particularly of beta-hydroxybutyrate (BHB), as a result of the inability of the liver to metabolize an excess of non-esterified fatty acids (NEFA) which is typically observed shortly after calving. In this period the energy intake of the cow increases more slowly than the milk energy output does ultimately resulting in a negative energy balance (NEB) and a stimulated lipolysis. Thus, the p.p. lipomobilization syndrome is accompanied by hyperketonaemia and hepatolipidosis (e.g. [1]). Based on the absence or presence of clinical symptoms in combination with the levels of BHB in blood circulation the hyperketonaemia is classified as either subclinical or clinical ketosis irrespective of pathogenesis. Most authors define a subclinical ketosis at BHB concentrations between ≥1.2 and ≤2.0 mMol/L although these thresholds should be considered a convention (e.g. [2-4]). Clinical signs are often paralleled by BHB concentrations of >2.0 mMol/L although they might be absent even at higher BHB levels. Considering this classification it becomes clear that a medical treatment is mostly confined to clinical cases while subclinical cases usually are not discovered and consequently not treated. Preventive measures need to be implemented in view of the significant incidence of subclinical ketosis, its adverse impact on dry matter intake and milk yield, and its association with clinical ketosis and other productions diseases, such as displaced abomasum, metritis and mastitis [2] but also with vaccination success ([5]). Based on the fact that a subclinical ketosis is at high risk to derail into a clinical ketosis prevention of the subclinical form concurrently reduces the incidence of clinical cases and should be the method of choice.

Abstract:Transition period between late pregnancy and early lactation (also called the periparturient period) certainly is the most interesting stage of the lactation cycle. Classically length of transition period has been defined as the last 3 weeks before parturition to 3 weeks after parturition.1,2 Most important infectious diseases and metabolic disorders occur during this time. Ketosis, retained fetal membranes, milk fever, merits, and displaced abomasums primarily impact cows during the periparturient period. Susceptibility to infectious diseases in this period occurs due to immune suppression during the periparturient period.1‒3 Ketosis (acetonemia) is an increase of ketone bodies (acetone, acetoacetate, and β-hydroxybutyrate, subsequently referred to as ketones) in blood until they eventually begin to spill over into urine and/or milk. In dairy cows, ketosis is a lactation disorder usually associated with intense milk production and negative energy balance.4 Ketosis occurs at the most productive stage of the lactation and hence can be extremely costly, as reported by different researchers.5‒7

Abstract:Many dairy cows experience a high incidence of health problems during the transition period (TP). The TP is an intermediate stage of various digestive, metabolic and reproductive functions which determine the general health status at the time of calving and during the first weeks postpartum. Negative energy balance due to increased energy demand at parturition and significantly reduced dry matter intake relative to demand is an important determinant. Consequently, substantial lipid mobilization from adipose tissue, increased oxidative stress and impaired immunity are associated with higher incidences of periparturient health problems including ketosis or subclinical ketosis (SCK), which have tremendous economic impact on dairy productivity. SCK is defined as the presence of increased blood ketone bodies (BHBA: betahydroxybutyric acid, acetone, acetoacetic acid) without clinical ketosis signs. Varying blood and milk cut-off values have been reported for BHBA concentrations defining SCK, but the most commonly accepted values are ≥ 1.2 mmol/L and ≥ 200 μmol/L respectively. This underestimated disease can impact dairy cow productivity through decreased milk production in the order of roughly 300 kg/lactation and increases the risk of metabolic and reproductive diseases such as displaced abomasum, retained placenta, metritis, mastitis, prolong oestrus interval and reduces conception rates. SCK also referred to as ‘profit robber or killer’ can cause productivity and economic losses of between $200-290 per dairy cow annually. Options for the control and prevention of SCK include controlled-release monensin capsules, and the injectable combination butaphosphan and cyanocobalamin and oral propylene glycol. SCK is easy to detect in early lactation using cow-side validated BHBA analysers with high specificity and sensitivity.

Abstract:The water complexes of 4-fluorophenylacetylene and 2-fluorophenylacetylene were investigated using IR-UV double resonance spectroscopy. Both 4-fluoro- and 2-fluorophenylacetylenes form a cyclic complex with water incorporating C-H...O and O-H...pi hydrogen bonds. These structures are similar to the phenylacetylene-water complex, implying that the fluorine substitution on phenylacetylene does not alter the intermolecular structure. Further, the presence of fluorine enhances the interaction of water with the acetylenic pi electron density. This behavior of fluorophenylacetylenes is dramatically different from that of fluorobenzene and fluorostyrene. A second water complex was also observed in the case of 2-fluorophenylacetylene in which water interacts with fluorine atom and acetylenic C-C triple bond in a double-donor fashion. Additionally, two distinct 2-fluorophenylacetylene-(water)(2) complexes were also observed. The first is a cyclic complex in which two water molecules bridge the hydrogen bond donor and acceptor sites present in 2-fluorophenylacetylene. The second is a kinetically trapped higher energy structure in which one water molecule acts as a double-acceptor.
[Overview on the relationship between changes in activity and ketosis in dairy cows]

Abstract:With a prevalence of up to 43 % subclinical ketosis is one of the most common diseases in dairy cows in their transition period. In itself, this may cause subsequent diseases such as clinical ketosis or lameness. Therefore, monitoring of animals in this stage is of importance. In addition to the measurement of β-hydroxybutyrate or acetoacetate in blood, milk, and urine as well as the observation of the animals, computer-assisted systems are suitable means of monitoring. Information such as animal identification and activity data are recorded on a data logger and transmitted to a computer. A change in activity may be an indication of an underlying disease days before the onset of additional clinical signs. In cases of ketosis, a decrease in activity may be observed 5 days before the clinical diagnosis is made. Thus, these data are a valuable contribution in monitoring the cattle herd's health status for both the farmer and the veterinarian. Activity measurement may also be employed for the detection of a beginning lameness. In the presence of lameness, the individual's activity decreases and periods of lying are longer. Activity measurement via transponder as a part of the herd monitoring provides important information on lameness prevalence in the herd. In the presence of a lameness a visual assessment should additionally be made. Lameness scores (Locomotion score, Gait score) have been developed for this purpose and add to determining the lameness status of the herd. This way the animals are divided into different lameness classes. Based on this classification those individuals in need of claw trimming or further treatment may be identified leading to amelioration or prevention of secondary diseases. Due to lameness and subsequent reduction of activity and feed intake, the animals may develop subclinical or clinical ketosis. Therefore, under consideration of both animal welfare and economic factors early disease detection and prophylaxis is desirable and should be a main objective of herd monitoring.

Abstract:According to the latest studies, the prevalence of subacute ruminal acidosis (SARA) is around 20% in early and mid- lactation dairy cows, generating annual losses in the United States of approximately USD 500 million to 1 billion. The diagnosis of SARA is still difficult due to lack of pathognomonic clues and the delayed appearance of certain clinical signs. Therefore, SARA remains neglected or even unrecognized in many dairy herds. SARA is characterized by daily episodes of low ruminal pH, when the pH remains in the range of 5.2 to 6 for a prolonged period due to the accumulation of short-chain fatty acids and insufficient rumen buffering. The causes of SARA are related to high-grain diets, such as feeding excessive amounts of non-structural carbohydrates and highly fermentable forages, and insufficient dietary coarse fibre. SARA is associated with the inflammation of several organs and tissues in dairy cows, and its main long-term health and economic consequences are the fluctuation of feed intake, reduced fibre digestion, depression of milk yield and milk fat content, gastrointestinal damage, diarrhoea, laminitis, liver abscesses, and lameness. The aim of this review is to summarize the information available on the physiological aspects, risk factors, prevalence and possible indicators of SARA in dairy cattle. Basedon the existing literature, rumenocentesis and the use of an oral stomach tube are reliable field techniques to detect SARA. Nowadays, improved field techniques allowing the continuous measurement of reticuloruminal pH are also available for better diagnosis of SARA. Wireless indwelling pH probes may become important tools for the continuous measurement of ruminal pH in the coming years.

Abstract:The peripartum period of a dairy cow is characterized by several physiological and behavioral changes in response to a rapid increase in nutrient demands, to support the final stages of fetal growth and the production of colostrum and milk. Traditionally, the transition period is defined as the period 3 weeks before and 3 weeks after parturition. However, several researchers have argued that the transition period begins at the time of dry-off (~60–50 days prior to calving) and extends beyond the first month post-calving in high producing dairy cows. Independent of the definition used, adequate adaptation to the physiological demands of this period is paramount for a successful lactation. Nonetheless, not all cows are successful in transitioning from late gestation to early lactation, leading to approximately one third of dairy cows having at least one clinical disease (metabolic and/or infectious) and more than half of the cows having at least one subclinical case of disease within the first 90 days of lactation. Thus, monitoring dairy cows during this period is essential to detect early disease signs, diagnose clinical and subclinical diseases, and initiate targeted health management to avoid health and production impairment. In this review, we discuss different strategies to monitor dairy cows to detected unintended disruptions in performance and management strategies that can be implemented to improve the metabolic health and performance of dairy cows during the transition period.

Differences in displacement of the proximal and distal ends of mid-upper thoracic esophageal squamous cell carcinoma.

Abstract:In the present study, clips were used as markers to evaluate displacement differences between proximal and distal ends of esophageal tumors and to test whether their internal target volume (ITV) margins should be determined separately. A total of 23 patients with mid-upper thoracic esophageal squamous-cell carcinoma, a tumor length of ≤8 cm and an esophageal lumen suitable for endoscopic ultrasonography were recruited for the present study. Clips were implanted endoscopically at the proximal and distal ends of the esophageal tumor (upper and lower clips). In a further exploratory study on 16 of the patients, a third clip was placed at the distal esophagus 2 cm above the gastro-esophageal junction (GEJ) (cardiac clip). The clips were contoured for all 10 phases of the four-dimensional computed tomography and the maximum displacements of the clip centroids among different breathing phases in left-right (LR), superior-inferior (SI) and anterior-posterior (AP) directions were marked as x, y and z, respectively. The ITV margins that covered 95% of the LR, SI and AP motion were 2.89, 5.00 and 2.36 mm, respectively. Axial displacement (y) was greater than radial displacement (x, z; P<0.05). It was also revealed that LR(x), SI(y) and AP(z) displacement of cardiac clips was greater than that of upper or lower clips (P<0.05). Differences in the axial and radial displacement of the upper and lower clips indicated that axial and radial ITV margins should be determined separately. However, further study is required on patients in whom the distal tumor end is located in proximity to the GEJ.
Metabolic Stress in the Transition Period of Dairy Cows: Focusing on the Prepartum Period

Abstract:All modern, high-yielding dairy cows experience a certain degree of reduced insulin sensitivity, negative energy balance, and systemic inflammation during the transition period. Maladaptation to these changes may result in excessive fat mobilization, dysregulation of inflammation, immunosuppression, and, ultimately, metabolic or infectious disease in the postpartum period. Up to half of the clinical diseases in the lifespan of high-yielding dairy cows occur within 3 weeks of calving. Thus, the vast majority of prospective studies on transition dairy cows are focused on the postpartum period. However, predisposition to clinical disease and key (patho)physiological events such as a spontaneous reduction in feed intake, insulin resistance, fat mobilization, and systemic inflammation already occur in the prepartum period. This review focuses on metabolic, adaptive events occurring from drying off until calving in high-yielding cows and discusses determinants that may trigger (mal)adaptation to these events in the late prepartum period.

Subclinical Ketosis in Dairy Herds: Impact of Early Diagnosis and Treatment

Abstract:Clinical and subclinical ketosis (SCK) in dairy cows occurs during the lactation period frequently in many herds, causing a reduction in milk yield and alterations in milk quality with significant economic losses for farmers. SCK is defined as a preclinical stage of ketosis characterized by an elevated ketone body level without clinical signs. Often many cows develop an elevated ketone body level during the first weeks of lactation even though it never goes up to a critical point causing clinical signs. This study aimed to evaluate the prevalence of SCK in Sicily and assess the effect of a treatment with propylene glycol (PG) to control the SCK, thus, reducing the negative effect on milk quality yield. This cross-sectional study was carried out on 22 farms located south-east of Sicily and 1,588 cows in lactation. A total of 3,989 individual milk samples were collected from calving to 80 subsequently days to check the β-hydroxybutyrate (BHB) values in order to establish the SCK status by the Fourier Transform Infrared Spectroscopy. Moreover, the contents of fat, protein, lactose, casein, urea, somatic cell count and acetone were evaluated to identify a correlation between SCK and milk quality. A total of 1,100 cows showed BHB values higher than 0.10 mmol/L. These cows were considered SCK positive, were separated from the rest of the herd, and treated with PG (400 g/head per day), all SCK cows were treated with PG and cows without SCK were not treated. The results showed a prevalence of 41.5% of SCK-positive cows during the first 9 days of lactation. The comparison among the cure rate of treated cows shows that the treatment was most effective in the first 7 days of lactation (76.5% of treated cows) than in the following days. PG positively influenced the milk quality parameters, except for the fat proportion. Moreover, the animals treated with PG showed also an increase in milk yield, supporting the economical sustainability of treatment.
[The clinical outcomes of patients undergoing revascularization for acute coronary syndrome].

Abstract:OBJECTIVE To evaluate short-term and long-term prognosis of revascularization in patients with acute coronary syndrome. METHODS A total of 6005 patients who received coronary revascularization in our institution between July 2003 and September 2005 were enrolled. The patients were followed up in clinic or by telephone after discharge between September 2006 and November 2006. The clinical and prognosis data of all-cause mortality, neo-myocardial infarction, nonfatal stroke, and re-revascularization of ST-segment elevation myocardial infarction (STEMI), non ST-segment elevation myocardial infarction (NSTEMI) and major adverse cardiovascular and cerebrovascular events (MACCE) were analyzed. RESULTS Among 4865 acute coronary syndrome patients, 955 cases were STEMI; 263 cases were NSTEMI; and 3647 cases were unstable angina (UA) pectoris. There were no significant difference for in-hospital mortality and late mortality (18 month survival 96%, 98% and 98%) between patients with STEMI, NSTEMI and UA. Patients with UA had lower MACCE rate (18 month non-MACCE survival of STEMI, NSTEMI and UA group were 86%, 86%, and 89% respectively). CONCLUSIONS Despite different clinical characteristics, patients with STEMI, NSTEMI and UA undergoing revascularization had similar short-term and long-term mortality. Patients with UA had lower MACCE rate.

Abstract:Background: Ketosis is the most important metabolic disease with prevalence from 15 to 45%. Ketosis is diagnosed using a metabolic profile. Due to the high prevalence, it is necessary to determine a large number of metabolic profiles within farm, which represents an additional cost, so the implementation of pooled serum in assessing the metabolic status of cows was examined. The aim of this study was to validate and evaluate the influence of the relative position (Z-score) of the value of pooled sample metabolic parameters within the known reference value of healthy cows in the detection of ketosis in herd during early lactation.Materials, Methods & Results: The experiment has been carried out using blood samples collected by puncture of coccygeal vein from 50 ketotic and 50 healthy cows. Laboratory analysis includes determination of beta-hydroxybutyrate-BHB, non-esterified fatty acids-NEFA, cholesterol-CHOL, triglycerides-TGC, glucose-GLU, albumin-ALB, total protein-TPROT, UREA, Ca, P, total bilirubin-TBIL and aspartat aminotransferase-AST. The pooled serum was made from 10 individual samples originating from 10 different cows. A serum aliquot of 0.1 mL was taken from each sample, and a 1 mL volume of pooled serum was finally formed. Three types of serum pools were made: 1) 30 pooled sample were from ketosis; 2) 30 pooled sample were from healthy cows and 3) 60 pooled samples containing mixed sera of healthy cows and cows with ketosis were made as follows: 10 pools contain 10% to 60% of ketotic cows (1/10 to 6/10 samples). Statistical analysis includes: a) difference in metabolite concentration and Z-score in pooled sample and arithmetic mean individual sample in healthy and ketotic cow, b) correlation between Z-score of pooled sample and arithmetic mean of individual sample, c) ability of Z-score of metabolite to divide ketotic from healthy cow, d) correlation between Z-score and % of ketotic cow in pooled sample; and e) calculation of 95%CI of pooled sample Z-scores for each % of ketotic cow in pools. Z-score and all analysis were calculated for each metabolic parameter. The results of the study show that the mean values and Z-scores of the pool and the calculated average value of the individual samples participating in that pool differ significantly in healthy cows and cows in ketosis, except for TPROT and Ca. A higher value and a higher Z-score were found for BHB, NEFA, UREA, TBIL and AST, and a lower value and a lower Z-score for TGC, CHOL, GLU, ALB and P in ketotic cows compared to healthy cows. The value of the Z-score of the pooled sample and the calculated mean values of individual samples participating in the pool are highly correlated with each other (coefficient of determination over 99%). Z-score of metabolites in the pooled sample can be used to distinguish healthy from ketotic cows (ROC AUC= 0.711 to 0.989), except for TPROT and Ca. The Z-score value of the pooled sample shows a linear correlation with the percentage of ketotic cows in the pool and the reference ranges of Z-scores change significantly as a function of the percentage of ketosis cows.Discussion: Modern research on the metabolic profile in cows requires obtaining a large amount of information from as few samples as possible. The advantages of using the Z-score are reflected in the following: this score does not depend on the absolute value of the metabolite, but on the position within the known population reference value, Z-score of sample and the arithmetic mean of individual samples included in the same pool are almost identical, the Z-score of these 2 groups of results is ideally correlated, the Z-score significantly correlates with the % of ketosis samples in the pooled sample. The use of pooled sample Z-score can be a useful in a herd level assessment of metabolic status and detection of ketosis as most important metabolic disease in dairy cows.Keywords: dairy cattle, ketosis, metabolic disease, metabolic profile, pooled serum, z-score, diagnostics.

Abstract:Metabolic disorders in animals are one of the most important problems in modern cattle production. The paper presents results characterising the functional state of the liver in dairy cows with clinical ketosis. The study was conducted on 17 Holstein cows (9 of them were clinically healthy with negative test for ketone bodies in urine and 8 with positive test) in their second to fourth lactation with milk yield above 8,000 kg per previous lactation. Conducted research has shown that cows affected with ketosis, had hypoglycaemia, dysproteinaemia, hypoalbuminaemia, increased liver enzymes and higher blood content of aromatic and sulfur-containing amino acids. Herewith the activity of cholinesterase and phospholipids were decreased. Albumins-globulins ratio and esterified to total cholesterol ratio was decreased in conditions associated with liver pathology due to clinical ketosis. Sick cows had signs of secondary osteodystrophy. Obtained results allowed us concluding that dairy cows ketosis caused significant changes in structural and functional state of liver, associated with abnormalities in of serum proteins synthesis, disturbed metabolism of amino acids, lipids and calcium.

Abstract:ABSTRACT Metabolic maladaptation in dairy cows after calving can lead to long-term elevation of ketones, such as β-hydroxybutyrate (BHB), representing the condition known as hyperketonemia, which greatly influences the health and production performance of cows during the lactation period. Although the gut microbiota is known to alter in dairy cows with hyperketonemia, the association of microbial metabolites with development of hyperketonemia remains unknown. In this study, we performed a multi-omics analysis to investigate the associations between fecal microbial community, fecal/plasma metabolites, and serum markers in hyperketonemic dairy cows during the transition period. Dynamic changes in the abundance of the phyla Verrucomicrobiota and Proteobacteria were detected in the gut microbiota of dairy cows, representing an adaptation to enhanced lipolysis and abnormal glucose metabolism after calving. Random forest and univariate analyses indicated that Frisingicoccus is a key bacterial genus in the gut of cows during the development of hyperketonemia, and its abundance was positively correlated with circulating branched-chain amino acid levels and the ketogenesis pathway. Taurodeoxycholic acid, belonging to the microbial metabolite, was strongly correlated with an increase in blood BHB level, and the levels of other secondary bile acid in the feces and plasma were altered in dairy cows prior to the diagnosis of hyperketonemia, which link the gut microbiota and hyperketonemia. Our results suggest that alterations in the gut microbiota and its metabolites contribute to excessive lipolysis and insulin insensitivity during the development of hyperketonemia, providing fundamental knowledge about manipulation of gut microbiome to improve metabolic adaptability in transition dairy cows. IMPORTANCE Accumulating evidence is pointing to an important association between gut microbiota-derived metabolites and metabolic disorders in humans and animals; however, this association in dairy cows from late gestation to early lactation is poorly understood. To address this gap, we integrated longitudinal gut microbial (feces) and metabolic (feces and plasma) profiles to characterize the phenotypic differences between healthy and hyperketonemic dairy cows from late gestation to early lactation. Our results demonstrate that cows underwent excessive lipid mobilization and insulin insensitivity before hyperketonemia was evident. The bile acids are functional readouts that link gut microbiota and host phenotypes in the development of hyperketonemia. Thus, this work provides new insight into the mechanisms involved in metabolic adaptation during the transition period to adjust to the high energy and metabolic demands after calving and during lactation, which can offer new strategies for livestock management involving intervention of the gut microbiome to facilitate metabolic adaptation.
