Abstract:Purpose: The requirement for anatomic venous reconstruction in digit replantation is an ongoing area of research. In this study, we evaluated our institutional experience to study whether replantation success is affected by the presence or absence of vein repair, stratified by the level of injury. Methods: A retrospective review was performed at an urban, level-1 trauma center of all single-digit replantations performed in adults from 2012 to 2021. Patient demographics, injury mechanism, level of injury, whether a vein was repaired, and replant survival were recorded. Results: Sixty-seven single replanted digits were included. Patients were, on average, 38 years old, and 94% were men. The most common mechanism of injury was a sharp laceration (81%). The overall survival rates for all replantations were 68.7% (46/67) and 60% (12/20) for distal finger replantation. Patients with digital replantations at Tamai zone III or more proximal exhibited a 1.8 times increase in survival rates when one vein was repaired versus zero veins (84.4% vs 46.7%). Patients with digital replantations at Tamai zones I and II exhibited similar survival rates. Conclusions: Replantations at or proximal to the middle phalanx should be repaired with at least one artery and vein to maximize the chance for success. However, for distal finger replantations, artery-only replantation is a viable option when vein anastomosis is not achievable. Typo of study/level of evidence: Therapeutic IV.

Abstract:Journal of Hand Surgery (European Volume), Ahead of Print. Distal fingertip replantation is associated with being a technically demanding procedure and dubious outcomes, although it is now performed more frequently across the world. However, the technique and outcomes remain controversial with disagreement among replantation surgeons due to lack of consensus about the indications, intraoperative strategy and postoperative regimes. In this article, we asked six experienced hand surgeons several pertinent questions that every replantation surgeon performing distal fingertip replantation would face in their clinical practice. The article summarizes their responses, which might provide valuable insight to every replantation surgeon in different parts of their career while managing these injuries.

Abstract:A 36-year-old healthy male patient was presented to the emergency room 3 h after experiencing a laceration to the left foot caused by a porcelain shard. The defect measured 7.5 × 6.0 × 0.8 cm, and the composite amputated tissue consisted of skin and subcutaneous layers. The terminal branch of the lateral calcaneal artery was first anastomosed end-to-end to the corresponding artery in the wound defect. The lateral calcaneal nerve was anastomosed after blood flow was restored. The two lateral veins were anastomosed end-to-end to the corresponding veins in the wound defect. Postoperatively, 1.0 × 1.5 cm area of skin necrosis was present at the distal end of the tissue, which healed smoothly after two weeks of dressing changes. The patient had retained excellent aesthetic and functionality by the 37 month follow up. Although such isolated amputation is relatively rare, microsurgical replantation is, thus, a feasible option in the management of heel amputation.
Tamai Zone 1 Fingertip Amputation: Reconstruction Using a Digital Artery Flap Compared With Microsurgical Replantation

Abstract:Purpose: The surgical treatment of fingertip amputations is controversial. This study was designed to compare the clinical results of 2 surgical procedures for fingertip amputation: reconstruction with a digital artery flap and microsurgical replantation. Methods: Between 2003 and 2015, 37 patients with Tamai zone 1 fingertip amputation of the index or middle finger were treated by reconstruction with a digital artery flap (n = 23) or microsurgical replantation (n = 14). Data for these patients were evaluated retrospectively. Nerve suture was not conducted in microsurgical replantation because spontaneous sensory recovery is expected in zone 1 replantation. Primary outcomes included hand dexterity (Purdue Pegboard Test), and disability of the upper extremity (Disabilities of the Arm, Shoulder, and Hand score). Secondary outcomes included strength (key pinch), digital sensitivity (Semmes-Weinstein test), and finger mobility (% total active motion). Results: The average follow-up period was 34 months. There was no significant difference in the primary outcomes between the 2 groups. The reconstruction group showed significantly better results for the secondary outcomes. Conclusions: This study suggests that the 2 procedures were comparable regarding postoperative activities of daily living and hand performance, but reconstruction using a digital artery flap gave better objective functional outcomes than microsurgical replantation. Success Rates of Finger Revascularization and Replantation

Abstract:Background: Revascularization surgery has been reported to have a higher success rate than replantation due to sufficient venous return. However, in complex cases, success depends on a wide range of indications. This study aimed to investigate success rates in cohorts that included severe cases. Methods: This single-center, noninterventional, retrospective cohort study included 292 patients (349 digits) who underwent revascularization or replantation at our institution between January 2000 and December 2022. Sex, age, smoking history, comorbidities, affected digit, amputation level, complete or incomplete amputation, type of fracture and mechanism, artery diameter, needle, vein anastomosis in the revascularization subgroup, vein grafting, warm ischemic time, and outcomes were investigated and compared between the revascularization and replantation subgroups of the distal and proximal amputation groups. Results: In the distal amputation group, the arterial diameter in the revascularization subgroup was larger than that in the replantation subgroup (P < 0.05). In the proximal amputation group, the revascularization subgroup had a lower frequency of multiple amputations than the replantation subgroup (P < 0.05). Vein grafts were more frequently used in both revascularization subgroups than in the replantation subgroups (P < 0.05). However, the other injury severity indices were similar, and the success rates were not significantly different between the subgroups. Conclusions: The revascularization success rate was similar to that of replantation. Vein anastomosis or vein grafting to the veins should be advocated for revascularization in severe cases where skin bridges may not have sufficient venous return.
Transplantation of a free fillet flap from discarded fingers for repair of a finger pulp skin defect: a case report

Abstract:Background Replantation represents a treatment option for patients with severed finger pulps. However, in some cases, replantation is a challenging task. Case presentation We report a successful case of finger pulp reconstruction of the ring finger using free flaps from a nonreplantable index finger in a spare-parts procedure. A 43-year-old worker accidentally injured the index, middle and ring fingers of his left hand on a machine turntable. The severed index and middle fingers and the distal pulp of the ring finger could not be replanted in situ due to extensive contusion of blood vessels and soft tissues. After vascular and nerve anastomosis, a free skin flap isolated from the nonreplantable index finger was transplanted to the wound of the distal pulpal defect of the ring finger. The flap survived completely postoperatively. Six months after the operation, only a slight deformity of the ring finger was observed. Moreover, sensation of the digit recovered well. Conclusions Spare-part surgery is a surgical approach that effectively saves and utilizes tissue that would otherwise be discarded in cases of severe limb trauma. This idea may be applied to treatment of severe injuries to multiple fingers. Additionally, in the process of tissue transplantation and repair, attention should be given to protecting the tissue in the recipient area to avoid damage to the original undamaged tissue structure, which can adversely affect healing and recovery of the tissue.

Abstract:Reconstruction of an amputated fingertip in a young child demands special techniques for success. We report a 2.5-year-old female patient with an amputated left index fingertip with the vascular defect being too severe to perform the usual replantation. Comparing several methods, we used the neighbouring digital artery as the feeding artery to perform foster replantation. Finally, the patient was satisfied with the appearance and function of her fingers. The clinical case, techniques, results are described and discussed. We consider it a useful technique, especially for those with a rather severe vascular defect.
Replantation of Severed Distal Segment of Fingers:A Report of 530 Cases

Abstract:Objective:To study and classify the severed distal segment of fingers and increase the survival rate of replantation.Methods:645 severed distal segments of fingers in 530 cases were studied. The distribution of blood vessels and nerves in the distal segment were observed under microscope using 36 mutilated fingers.Results:Severance of the distal segments could be classified into 3 types according to the level of amputation: type I, between the distal interphalangeal joint and nail root; type Ⅱ, between the nail root and the center of swirling fingerprint; and type Ⅲ , distal to the center of swirling fingerprint. 434 fingers of this series were categorized as type I. And there were 137 and 74 fingers classified into type Ⅱ and type Ⅲ respectively. In type Ⅰ and Ⅱ, the diameter of the arteries was around 0.3 to 0.6 mm, and that of the veins was 0.3 to 0.8 mm. In type Ⅲ, the artery was ramified, with diameter of 0.1 to 0.3 mm. The diameter of the veins was 0.15 to 0.3 mm. The survival rate after replantation was 97.7%, 95%, and 97% respectively. The replanted fingers had satisfactory sensation and appearance.Conclusions:The severed distal segment of fingers in type Ⅰ and type Ⅱ should be replanted under 10 × magnification. The vessels of the fingers in type Ⅲ should be anastomoses under 16 × magnification.

Abstract:Work in agriculture carries the risk of trauma as a result of mechanization (farm equipment and machinery), location of cultivated areas, and multi-functionality. In agriculture, the hands were the most affected parts of the body which sustained injury. Successful suturing of a finger initially disqualified from replantation is rare. The case is presented of a 55-year-old patient who had a non-work-related finger amputation. In a surgical centre not belonging to the reference ones in plastic surgery in Poland, a doctor with six months of surgical experience performed the replantation without vascular reconstruction (the amputated distal part of the finger served as a biological dressing). Despite the lack of vascular reconstruction and unfavourable prognosis, the amputated fragment of the finger maintained a partial blood supply. Subsequent flap surgery in the plastic surgery department, and rehabilitation of the patient due to finger contracture, led to functional recovery of the finger.

Abstract:Background: The subamputation of fingers with vascular compromise presents a surgical challenge. Although tissue continuity may be considered a favourable prognostic element, in our experience, we noticed that there is not always a direct correlation between soft tissue involvement, radiographic appearance and final outcome. Methods: We included, in our study, all cases of vascular pedicle injury in which finger salvage was attempted with microsurgical revascularisation. Exclusion criteria were: integrity of both vascular pedicles, pedicle lesion without global circulatory compromise and patients treated immediately with amputation. Results: Between May 2018 and July 2023, 27 male patients with finger subamputation injuries were treated at our institution. In 11 cases of injured fingers, the only intact tissue was the flexor digitorum profundus (FDP) or flexor pollicis longus (FPL). Our global failure rate was 49%; whereas, in the subgroup of the 11 cases with continuity of the FDP or FPL, the failure rate rose to 73% and when the fingers showed flexor tendon integrity and radiographs demonstrated minimal bone damage, revascularisation failure was observed in all cases (100%). Conclusions: The results of the study show that subamputations with devascularisation, clinically presented with the combination of flexor tendon as the only element of tissue continuity and dislocation or minimal bone/articular injury, have a worse prognosis because of their trauma mechanism. We propose to add them to the Kay-Adani Classification as a subset of the poorest prognostic injuries group (III), to help surgeons to make decisions about the management of subamputation finger injuries.

Microvascular Ear Replantation: A Multicenter Study of 22 Patients with Complete Ear Amputation

Abstract:Background: We aimed to report our experience in treating ear amputations with microvascular replantation, with the largest sample to date. Methods: Twenty-two patients with complete ear amputation underwent microvascular ear replantation at three medical centers between May 2003 and May 2020. Arterial anastomoses, venous anastomoses, or vein graft were performed depending on different situations. Re-exploration was performed in four patients due to venous congestion (n = 3) or arterial compromise (n = 1). Results: Eleven patients had vascular complications (venous congestion: 10, arterial compromise: 1) and four of them required re-exploration. Three ears were completely salvaged (75%) and one case failed. Eighteen (81.8%) replanted ears survived completely, with 15 repaired ears demonstrating a good contour and 3 ears showing atrophy. Three replanted ears sustained partial loss, and one sustained total loss. Three extraordinary cases with the longest ischemic time, smallest tissue size, and youngest age reported thus far all survived and had cosmetically satisfactory appearances. Statistical analysis indicated no significant correlation between replanted ear survival and potentially influential factors, including ischemic time, number of arterial and venous anastomoses, presence of vein graft, and re-exploration. Conclusions: Microvascular replantation for ear amputations achieved excellent results. It may be considered the primary choice for surgeons with microsurgical skill.
Should natriuretic peptide testing be incorporated into emergency medicine practice?

Abstract:Shortness of breath is a common complaint among adults presenting to the emergency department (ED). The most frequent diagnosis made in this context is heart failure (HF). The prognosis for patients with HF remains poor, with only 45% to 60% of patients surviving 5 years after the diagnosis is made, a rate comparable to that of colorectal cancer. Given the high incidence of HF, its poor prognosis and the existence of therapies that improve the duration and quality of life, it is important that emergency physicians make the diagnosis as rapidly and accurately as possible. Point-of-care assays for brain natriuretic peptide (BNP) and its N-terminal pro-peptide (NT-proBNP) are now available. There is literature to support the utility of these assays for the ED diagnosis of HF. However, most publications have been based on post hoc subgroup analyses of a few large industry-sponsored prospective studies. In this issue of the Canadian Journal of Emergency Medicine, Murray and colleagues report the results of a small but innovative primary study that looked at the potential impact of incorporating NT-proBNP into emergency medicine practice. The results raise questions regarding the assumptions of previous research in this area and shed light on the challenges emergency physicians may face if they choose to adopt natriuretic peptide testing. Clinical uncertainty is the raison d’être for any new diagnostic test. If clinical uncertainty does not exist, additional diagnostic tests are not warranted. The degree of clinical certainty (or uncertainty) is determined by comparing the clinical impression with an independent validated criterion (gold) standard. Because there is no validated criterion standard for the diagnosis of HF, previous natriuretic peptide studies have compared the ED diagnosis with a retrospective diagnosis by 2 cardiologists who reviewed the medical records and were blinded to natriuretic peptide results and ED diagnoses. This unvalidated criterion standard has been assumed, although never proven, to be more accurate than the diagnostic impression of physicians in the ED. Ironically, the studies that used a retrospective criterion standard call into question the assumption that emergency physicians face diagnostic uncertainty for all patients presenting with shortness of breath. Although it is clear that emergency physicians are uncertain of the diagnosis in approximately 30% of dyspneic patients, they are rarely wrong when they rate the probability of HF as very high or very low. In a study of 1586 patients, the prevalence of disease in patients who were rated by emergency physicians as very likely to have HF was 95%. Conversely, the prevalence in patients who were rated as very unlikely to have HF was only 8%. At these extremes of pretest probability, it would be difficult for any diagnostic test to improve upon the clinical impression of emergency physicians. In fact, in a post hoc analysis of the BNP [Breathing Not Properly] Multinational Study Group data, the accuracy of BNP in very high and very low probability patients was found to be worse than clinical judgment alone. How do BNP or NTpro-BNP perform in clinically uncertain cases? All studies to date, including the one in this issue of CJEM, have evaluated diagnostic test performance in the entire spectrum of acutely dyspneic patients, including clinically certain ones. The appropriateness of extrapolating the findings from such studies to the subgroup of clinically uncertain patients is highly questionable. Successful replantation of amputated facial tissues by supermicrosurgery

Abstract:Summary Background Although replantation of amputated facial segments remains challenging in reconstructive surgery, it offers excellent aesthetic and functional outcomes. Methods From May 2004 to October 2019, 12 patients underwent replantation of amputated facial tissues by supermicrosurgery. The case details, such as the rationale for replantation, the operation method, and postoperative therapy, are described. Four cases are discussed to demonstrate the replantation of different facial parts. Results Facial tissue replantation was successful in all 12 patients without secondary surgery. The cases included one nose, eight involving the ears, two of the lips, and one of the soft tissue segments surrounding the lower jaw. Venous congestion occurred in three patients who received a solitary arterial repair and were treated with bloodletting. All patients expressed satisfaction with the cosmetic and functional results at the final follow-up. Conclusions Supermicrosurgical facial tissue replantation is a promising and effective procedure for providing patients with the best aesthetic and functional outcomes.

Clinical analysis of digital replantation in 127 cases

Abstract:Objective To study the treatment experience of digital replantation and to take corresponding prevention countermeasures to the possible causes of the failure. MethodsTo retrospectively analyze the clinical data and to evaluate the postoperative curative effect of the 127 patients with finger replantation with 150 fingers cure in our hospital from 2009 to 2012.To observed the situation of necrosis and vascular crisis after operation.Results According to the evaluation trial standard of digital replantation function of Chinese Medical Association, the excellent and good rate was 88% with 80 excellent fingers,52 good fingers and 18 bad fingers.After the operation,the incidence rate of vascular crisis was 14.7% with 22 fingers,the survival rate was 92.1% with 10 necrosis.ConclusionThrough the corresponding prevention countermeasures to the unsuccessful factors in major factors affecting of postoperative survival of digital replantation,to improve the survival rate of digital replantation.

Abstract:Background: A continuous obstacle that has limited access to and implementation of finger replantation surgery is timeliness, as ischemia time is traditionally considered a crucial factor for success. However, claims that the vitality of amputated fingers decreases after 6 h of warm ischemia and 12 h of cold ischemia are mostly based on theoretical considerations. Methods: Here we present a case of multi-digit revascularization after 72 h of warm ischemia using the microsurgical arteriovenous bypass technique. Results: In the reported case, revascularization was performed after a long ischemic period and showed good recovery of motor and sensory function. Conclusions: We identified significant limitations in the literature supporting time limits of ischemia and recent evidence demonstrating the feasibility of delayed finger replantation. The current treatment approach for amputation injuries often requires transfers or nighttime emergency procedures, increasing costs and limiting the national availability of finger replantation. Changes to finger replantation protocols based on evidence could expand access to this service and improve the quality of care.

Abstract:To summarize the clinical effect of avulsed skin replantation of hand and foot via vacuum sealing drainage (VSD) combing low temperature technique.From March 2012 to October 2013,13 cases with avulsed skin replantation of hand foot using combined technique included 8 males and 5 females with an average age of 32 years old ranging from 18 to 62 years. The time from injury to hospital was 1 to 4 hours (2.4 hour in average). The reasons of injury included machine injury in 7 cases and rolling over by cars in 6 cases. The parts of injuried involved finger in 2 cases,back of the hand in 5 cases and dorsum of foot in 6 cases. The area of avulsed skin was 5 cm x 6 cm to 12 cm x 16 cm,tendon and bone exposure was found in 4 cases. VSD was operated in all patients and the avulsed skin was refrigerated in the temperature of -4 °C or -80 °C. After 4 days, the skin stored in the -4 °C was replanted to the wounded place in 5 cases and in 3 cases the skin was planted to the donor site of flap. The skin stored in the -80 °C was replanted in 4 cases after 7 or 8 days, 1 case after 45 days.Of the 13 cases, 1 case of degloved injury from lower leg to dorsal foot,the replanted skin was necrosis completely; 1 case of degloving injury with fourth finger,the skin which replanted after 45 days survived approximately 30%,cured after skin-graft many times. In the other cases, the survival area of replanted skin was more than 85%, all cured after dressing. According to the standard of skin survival area evaluation by Jia et al, 11 cases showed excellent, 1 showed medium and 1 showed inferior. There were no complication about grafted skin rupture after the skin survived in 11 patients,after 4 to 22 months follow-up, the resiliency of grafted skin showed good. Sensation recovery was measured by BMRC standard: 3 cases of S3, 5 cases of S3, 3 cases of S2.VSD combining lower temperature technique in skin replantation provides time and space for wound preparation and treatment plan for the patients who need second surgery, especially for the large area skin degloving,this method could utilize the degloved skin efficiently, decrease the donor site area, alleviate the pain and financial burden,reduce the scar formation of donor site and impediment.

Abstract:RATIONALE:Metacarpal and phalanx defects with soft tissue loss were suggested to be reconstructed by vascularized bone flap. The fibular osteocutaneous flap is a preferred method. Three-dimensional virtual planning has successfully applied in mandibular reconstruction with fibular free flap. We applied three-dimensional virtual planning in precise fibula flap harvest to maintain the continuity of the fibula and to achieve accurate metacarpal and phalanx reconstruction.PATIENT CONCERNS:A 35-year-old male presented with extensive soft tissue defects and first metacarpal defect involving the first metacarpophalangeal joint.DIAGNOSES:There were 4 cm of first metacarpal defect involving the first metacarpophalangeal joint and soft tissue defects of 5cm × 3cm + 3cm × 2cm.INTERVENTIONS:By combining three-dimensional virtual planning, we harvested a chimeric fibular flap. The precise fibula partial osteotomies were performed with cutting guides designed in virtual planning.OUTCOMES:All the chimeric flaps survived and no significant donor-site morbidity was noted. Michigan Hand Outcome Questionnaire scores indicated acceptable functional results.LESSONS:Our preliminary experience with the approach of three-dimensional virtual planning in precise chimeric fibula free flap is practical and efficient. Although more cases and follow-up are needed to evaluate it, this approach is expected to benefit patients.
Thumb dorsal skin composite tissue replantation: A case report

Abstract:The primary goal is to preserve thumb integrity and functionality to the greatest extent possible, even in cases involving very small amputated parts. In this article, we present a case in whom the seemingly non-replantable dorsal skin-nail composite tissue of the thumb could be successfully replanted with a single artery anastomosis. No additional procedures were required, and complete recovery was achieved. In conclusion, given the unique vascular structure of the thumb, all amputated parts should be carefully evaluated for replantation. Replanting a partially amputated finger yields superior functional and cosmetic outcomes compared to any reconstructive method.

Abstract:Avulsion of fingers are common industrial incidents and small skin flap injuries of one or more fingers are emergency room problems encountered daily [1]. Avulsion of skin from the underlying structures is usually a result of trauma, and has a dramatic presentation. Besides the possibility of injury to other structures, there may be a problem with viability of the skin involved. The degloving injury is more significant in the hand injury. Reconstruction of a single degloved finger can be performed with local flaps and tubed pedicle flaps [2,3]. However, it is not easy to wrap the entire circumference of the finger by using a local flap. Avulsions of multiple fingers is a severe injury that is more difficult to reconstruct. Some authors use the Millard ‘crane principle’ and bury the digits in the abdominal fat for 2 weeks with secondary skin grafting [4]. A tubed groin flap can also be used to reconstruct the injury [5]. However, it requires the hand to be attached to the groin for least 2 weeks, which is awkward and makes hand therapy difficult. Another possibility for reconstruction is the radial forearm flap, which is a fascia-only flap with skin graft on the fascia [6]. A degloved finger can also be reconstructed with a toe wrap-around flap [7] or an innervated arterialised venous flap from the dorsum of the foot [8]. We present a reconstruction of a traumatic degloving injury of the finger using a pedicled split-thickness skin graft from the abdominal wall to treat five cases (11 fingers) between June 2010 and September 2010, and the results were satisfactory.
