Abstract:Background: The rate of preoperative misdiagnosis between abdominal tuberculosis and ovarian cancer is high because the symptoms are non-specific, clinical findings can be misleading, and biomarkers commonly linked with other diseases also make appropriate diagnosis difficult. Objective: To evaluate the clinical and diagnostic features in women with abdominal tuberculosis that simulated advanced ovarian malignancy. Materials and Methods: A retrospective review of 11 cases of abdominal tuberculosis managed at the Gynaecological Oncology unit of the University of Port Harcourt Teaching Hospital with suspected advanced ovarian cancer between January 1, 2016, and December 31, 2021. Clinical characteristics, laboratory results, including serum tumour markers, radiological findings, and ascitic fluid evaluation were extracted from theatre records and case notes and entered into a proforma. SPSS 25 was used for data analysis, and the results are displayed as frequency tables. Results: More than half 6 (55%) of the cases of abdominal TB were found in patients aged 20 to 40 years, with many patients 8 (72.7%) being nulliparous. Abdominal distension, Abdominal pain, and weight loss were the most common presenting symptoms. All patients had clinical evidence of ascites, along with thickening or ill-defined nodularities in the pouch of Douglas and/or in the adnexal areas on pelvic examination. Serum CA-125 levels were elevated in all 11 patients (100%), with a mean value of 327.8 U/mL. Pleural effusion and increased infiltration were seen in 5 (45.5%) patients. Ultrasound scan and computed tomography studies showed ascites in all 11 (100%) patients. Debritic ascites with peritoneal thickening, mesenteric and omental stranding were observed in all 11 patients (100%), and all 11 (100%) patients had laparotomy. All patients received standard antitubercular therapy. Conclusion: In relatively young females with nonspecific symptoms, a diagnosis of abdominal TB should be considered. Ultrasound-guided biopsy, laparoscopy, or a mini-laparotomy may be the most direct and least-invasive approach for a diagnosis, avoiding unnecessary extended surgeries in these patients.

Abstract:This study aimed to summarise the clinical experience of peritoneal tuberculosis (TB). Clinical data of 26 patients admitted and diagnosed with peritoneal TB mimicking advanced ovarian cancer or peritoneal carcinoma between June 2008 and June 2017, was evaluated. The mean age was 35.92 ±15.30 years. The most common findings were abdominal mass (n=21, 80.77%) and elevated CA 125 levels (n=19, 73.08%). One patient underwent PET-CT and was diagnosed with peritoneal TB. Laparotomy was performed in 14 patients (70%) and laparoscopy in six patients (30%), out of whom two patients were converted to laparotomy due to severe adhesions. Meanwhile, mass puncture biopsy or small incision biopsy was the preferred method for diagnosing peritoneal TB, while PET-CT was considered as a good diagnostic method.

Abstract:Tuberculosis (TB) is a serious infection that can involve any organ system and present in various forms. About one-third of the world's population are carriers of latent TB. Although most cases are from a pulmonary origin, there is a rising prevalence of abdominal TB. Patients with pulmonary or extrapulmonary TB are treated similarly through the use of pharmacological therapy. Nonspecific clinical manifestations of TB have made it difficult for clinicians to diagnose. Peritoneal tuberculosis (PTB) is a serious concern as its symptoms overlap with that of many other chronic conditions, especially in those who are immunocompromised. The lack of highly sensitive and specific testing methods has made early intervention difficult, therefore a high index of suspicion is crucial in the progression of the disease. Here, we present a case of a 71-year-old female with a history of abdominal pain, fever, and weakness. Initial investigation with computed tomography (CT) imaging revealed omental fat stranding that pointed towards peritoneal carcinomatosis (PC) from possible recurrence of her ovarian cancer. Further investigation with a peritoneal biopsy was remarkable for caseating granulomas with fat necrosis confirming extrapulmonary TB. This report highlights a rare case of PTB mimicking PC in an elderly patient who is immunocompromised from the use of long-term corticosteroids who continued to decline after pharmacological treatment of the disease.

Pericardial Synovial Sarcoma in a Dyspnoeic Female with Tuberculous Pericarditis: A Case Report.

Abstract:Synovial sarcomas of the pericardium are extremely rare and associated with poor survival rate. The current case report describes a 45-year-old female who presented with dyspnea upon exertion, a paroxysmal cough, night sweats and recurrent pericardial effusion. The patient was diagnosed with tuberculous pericarditis and received antituberculous drug therapy. Echocardiography and magnetic resonance imaging (MRI) revealed a pericardial mass lying predominantly over the right atrium. The patient was treated by surgical excision and a subsequent histological analysis confirmed the diagnosis of a pericardial synovial sarcoma. Under high power examination, a characteristic biphasic appearance consisting of hypercellular spindled cell sheets was observed. Immunohistochemistry demonstrated positive staining for epithelial membrane antigen (EMA), vimentin and Bcl 2. The patient was then referred for adjuvant chemotherapy with a combination of adriamycin and ifosfamide. The patient has since remained clinically free of disease for 32 months.

Abstract:Objective To investigate the reasons and prevention measures of female pelvic tuberculosis misdiagnosed as ovarian cancer.Methods Eleven cases with pelvic tuberculosis misdiagnosed as ovarian cancer were included in this retrospective study between December 1994 and January 2011.Results Abdominal distension, anorexia, weight loss and menstrual disorders were chief complaints of female pelvic tuberculosis.Tuberculin tests were negative in 10 cases; Acid-fast bacilli in ascites were negative in all cases; CA125 level was above the normal in 6 cases; Pelvic ultrasound prompted to a large number of effusion, and pelvic mass can be explored in 9 cases; CT showed cystic masses of annex in 6 cases.Misdiagnosed as ovarian cancer, pelvic tuberculosis was confirmed by laparoscopy or laparotomy and biopsy.Conclusion Combination with history, comprehensive use of various auxiliary examination methods, and if necessary, diagnostic laparoscopy or laparotomy, the diagnostic accuracy of pelvic tuberculosis can be improved.

Abstract:Introduction and importance: 1-3% of tuberculosis cases are extrapulmonary, which 11-16% are abdominal. In recent years, a progressive increase in the number of peritoneal tuberculosis cases has been observed. Peritoneal tuberculosis accounts for only 1-2% of all cases of peritonitis. Historically it is known as "the great mimicker" since it can resemble a neoplastic, infectious or inflammatory intestinal disease. The most common triad of presentation is fever, weight loss and abdominal pain; ascites is also usually present. For diagnosis, computed tomography is the most sensitive technique and is complemented by elevation of CA 125 and Adenosine deaminase (ADA) but is confirmed by pathology study. Treatment is the same as the pulmonary presentation. Case presentation: A 33-year-old man presented with acute complication appendicitis. A limited right hemicolectomy was performed due to the nature of the patient advanced disease. Multiple small tumor resembling peritoneal implant and a granuloma were identified has sampled during the patient's surgical procedure. Pathologic examination revealed peritoneal tuberculosis and medical therapy was initiated. Laboratory assays including CA 125 and adenosine deaminase (ADA) can be useful serum markers to follow during treatment if they are positive. Discussion: The most common presentation of peritoneal tuberculosis is fever, weight loss and abdominal pain. Our patient did not have any of these symptoms, we founded granulomas in surgery of intestinal restitution. Methods: The work has been reported in line with the SCARE criteria. Conclusions: This pathology should always be kept in mind as a differential diagnosis when faced with unexpected findings in surgery.

Abstract:Introduction: Tuberculosis can involve any part of the gastrointestinal tract or the peritoneum and is the sixth most frequent site of extrapulmonary involvement. The pseudotumoral form is relatively rare and several predisposing factors are mentioned, a significant differential diagnostic problem with malignant tumor pathology is exposed , often considered first when encountering an abdominal or pelvic mass in the context of overall deterioration. Presentation of case: We report a rare case of a huge retroperitoneal TB abscess mimicking an abdomino-pelvic cystic mass. Discussion: Abdominal tuberculosis represents 1 to 2% of all locations and 31 to 58% of abdominal locations. The pseudotumoral form is relatively rare and are uncommon in immunocompetent patients. Tuberculosis abscess can present with variable radiologic features. It is difficult to differentiate it from malignant pathologies. Invasive explorations, such as laparoscopy or laparotomy, play a crucial role in confirming the diagnosis. Peritoneal cavity exploration may reveal whitish granulations, peritoneal nodules, peritoneal hyperemia, filamentous peritoneal adhesions, and intestinal adhesions. Histological examination of biopsies are the key to the diagnosis. The treatment of peritoneal tuberculosis relies on a combined therapy of antitubercular drugs . Conclusion: As TB remains endemic in our country and with the continued increase in multidrug-resistant TB infections, it is important to know various presentations and diagnosis forms features of extrapulmonary TB including TB abscess as the treatment remains basically the same combining anti-tuberculosis drugs and the recourse to surgery.
Tuberculosis Peritonitis Resulting in Small Bowel Obstruction

Abstract:Tuberculosis (TB) peritonitis resulting in a small bowel obstruction is uncommon and can be a challenging infectious disease to diagnose. It often has an insidious onset with non-specific symptoms. Today we report a rare case of a 30-year-old woman who recently traveled to Vietnam and presented with worsening upper and lower gastrointestinal symptoms. CT scan revealed an ill-defined mass in the terminal ileum with prominent mucosal enhancement and wall thickening, which ultimately led to subsequent colonoscopy and Quantiferon Gold testing, revealing a positive result. Biopsy of the mass demonstrated noncaseating granulomatous colitis with rare acid-fast positive bacillus consistent with mycobacterial infection. As a result, the patient was ultimately initiated on antituberculosis therapy. Shortly thereafter, she was readmitted with clinical features suggestive of a bowel obstruction. The patient was managed with supportive care and did not require surgical intervention. However, approximately two months following the readmission, she presented to the emergency department once again with a mechanical bowel obstruction that ultimately required surgery. This case underscores the importance of TB testing in patients with insidious, worsening gastrointestinal symptoms and highlights the potential complications of TB peritonitis, even in those undergoing antituberculosis treatment.

Abstract:Creation of artificial ball lightning (BL) in gas discharges has been considered. Explosion of the experimental fireballs can be explained by an action of vapors inside them on the cover. Using these results, a model of a natural BL has been presented. The BL in it consists of a core filled by a vapor and a solid cover. Estimates of energy stored in the vapor of the melted material proved to be about those observed in high-energy BL.

Abstract:Peritoneal lymphomatosis is a rare presentation of lymphoma that can mimic peritoneal tuberculosis. The computed tomography findings in both conditions include omental caking, thickening, and nodularity. We report the case of a 41-year-old man who presented with intermittent abdominal pain and distension. Abdominal CT initially suggested peritoneal tuberculosis due to the thickening of the peritoneum and greater omentum with multiple nodules. However, 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) images showed diffuse metabolic activity increase in the thickened peritoneum, omentum, and mesentery. An omental biopsy was performed under ultrasonography guidance, and histopathological examination revealed a high-grade Burkitt lymphoma. It is crucial to distinguish peritoneal lymphomatosis from tuberculosis, as the prognosis and management of the two conditions are vastly different.

Abstract:Primary extra nodal lymphoma occurs in approximately 25% to 40% of patients and is more common in patients with NHL. Diffuse large B-cell lymphoma (DLBCL) is the dominant histological subtype. A 60-year-old gentleman presented to our hospital with pain abdomen associated with abdominal distension since a month, few constitutional symptoms with bilateral axillary and inguinal lymphadenopathy. Blood investigations showed high total count with significantly high LDH levels. Provisional diagnosis of tubercular Abdomen/Malignant Ascites with Unknown primary was made. CT Abdomen showed large bowel wall thickening with parietal peritoneal lesions with omental cake. Excision biopsy of inguinal lymph node showed lymphoid follicles with poorly defined attenuated mantle zones having centrocytes and centroblasts. Immunohistochemistry was positive for markers, which confirmed the diagnosis of Stage 4B DLBCL, germinal center B-cell type. Sepsis and acute kidney injury occurred and the patient expired, 6 days after diagnosis. To conclude, Peritoneal lymphomatosis is an extremely rare condition. This case highlights the need to have a high index of suspicion for malignancy, when a given case is clinically diagnosed with tuberculosis as and when new findings appear during investigation.

Abstract:A 60-year-old woman presented with vague abdominal pain for one week was referred to pancreatic tail carcinoma accompanied with splenic metastasizes. She came to our hospital for further treatment. Ultrasonography and abdominal computed tomography (CT) revealed a pancreatic tail tumor with splenic metastasizes. There was no history of tuberculosis. Laparotomy was performed because pancreatic tail carcinoma with splenic metastasizes was highly suspected. Indurated mass in the pancreatic tail and sporadic metastasizes in the spleen had been found during the surgery. The pancreatic tail and the spleen were removed and proved to be tuberculosis on histological examination of a frozen section. The patient was given antituberculosis therapy and is now getting well. Tuberculosis should be considered in the differential diagnosis of pancreatic masses. The response to antituberculosis treatment is very favorable.

Abstract:Introduction: Gastrointestinal tuberculosis (TB) is a rare condition, but it poses significant diagnostic and management challenges, especially in immunocompromised individuals. This case report discusses the diagnostic complexities and therapeutic strategies for managing gastrointestinal TB in an HIV-positive patient, emphasizing the importance of considering TB in differential diagnoses. Case presentation: A 33-year-old male with no significant medical history presented with a five-day history of severe abdominal pain, initially epigastric and later localized to the right lower quadrant (RLQ). Physical examination revealed RLQ tenderness, and elevated inflammatory markers were observed in laboratory tests. Imaging studies suggested splenomegaly and signs consistent with acute appendicitis. Laparotomy uncovered a perforated cecal mass and diffuse white lesions across the small intestine. Histopathological analysis confirmed necrotizing granulomatous colitis, and PCR identified Mycobacterium tuberculosis (MTB). During hospitalization, the patient was diagnosed with HIV. Clinical discussion: This case underscores the diagnostic challenge of abdominal TB, particularly in HIV-infected patients where clinical presentation can mimic other conditions like Crohn's disease or appendicitis. Effective management requires timely surgical intervention, followed by appropriate anti-tuberculous and antiretroviral therapies. The multidisciplinary approach ensures comprehensive care and better patient outcomes. Conclusion: Effective recognition and diagnosis of gastrointestinal TB in HIV-positive patients are critical for successful treatment. This report highlights the necessity for heightened clinical suspicion and a collaborative approach in managing such complex cases, ultimately improving patient prognosis and care.
"TB - Mindedness"-the only way to beat "the great mimicker": A case series with atypical presentation of female genital tuberculosis

Abstract:Background: Incidence of Tuberculosis (TB) cases in India reported in 2019 is 193 per 1 lakh population [National Tuberculosis Elimination Plan (NTEP)]. In India, annual extra pulmonary TB burden is 20-25%, of which 4% of cases are of urogenital origin (Revised National TB Control Programme, 2019; World Health Organization, 2019). The Ministry of Health and Family Welfare has made a target of eliminating tuberculosis by 2025 under the NTEP by the process of identification, notification and treatment of cases. Tuberculosis being a leading cause of infertility in developing countries, employing best clinical practices and being "TB-minded" will also save the patient of enormous anxiety and uncertainity and also decrease the time gap between clinical presentation and diagnosis to optimize fertility outcome. Methodology: A prospective cohort study of cases presenting with unusual findings and ultimately being diagnosed as genital tuberculosis was conducted in the gynaecology OPD, AIIMS, New Delhi, from November 2020 to November 2021 (1 year). Patients were investigated judiciously, diagnosis made and followed up for their response to anti tuberculosis therapy (ATT). Results: This data comprises of conglomerate of ten cases with unconventional exhibition of genital tuberculosis. 70% of the cases presented with pain lower abdomen not specifically related to menstrual cycle and often confused with IBD. Tubo ovarian mass (70%) mimicking as simple ovarian cyst, ovarian carcinoma or endometriosis was the most common clinical finding we came across.
Clinical Features and Diagnostic Approaches for Abdominal Tuberculosis: Five-Year Experience from a Non-Tuberculosis-designated Hospital in China

Abstract:Background and purpose: abdominal tuberculosis (TB) is a common form of extrapulmonary TB but it is still a diagnostic dilemma in clinical practice. This study aimed to highlight the clinical features and diagnostic approaches for abdominal TB. Methods: seventy cases of diagnosed abdominal TB were retrospectively collected between August 1st, 2015 and June 30th, 2020. They were classified as peritoneal TB, lymph node TB, gastrointestinal TB, visceral TB or mixed TB. Results: eighteen patients were diagnosed with peritoneal TB, nine with lymph node TB, five with gastrointestinal TB, two with visceral TB and 36 with mixed TB. More than 65 % of the patients had tuberculosis of other sites except the abdomen. The median diagnosis time was 60 days. Ascites (58.6 %), abdominal distension (48.6 %), weight loss (44.3 %) and fever (42.9 %) were the most common symptoms. The overall microbiological and histological detection rates were 70.0 % and 38.6 %, respectively. The non-ascite samples yielded a higher microbiological confirmation rate (63.6 %) than the total samples (40.8 %). Diagnosis was confirmed histologically in 18 patients (69.2 %). Forty-five cases (64.3 %) were clinically diagnosed. Invasive procedures such as surgery (6/7), percutaneous biopsy (7/7) and endoscopy in lymph node TB (4/5) had high confirmation rates. Conclusions: the diagnosis of abdominal TB should be reached by a combination of clinical, laboratory, radiological, microbiological and pathological findings.

Abstract:Background: Extrapulmonary tuberculosis, especially occurring during pregnancy, has been a challenge for clinicians, because of this condition's protean manifestations. Early diagnosis is important for preventing obstetrical morbidity and neonatal mortality. Case: The patient was a 38-year-old pregnant Chinese female, who presented with ascites, anorexia, and abdominal pain at 28 weeks of gestation. She was found to have abdominopelvic tuberculosis. She delivered her neonate via abdominal hysterotomy. Then, an excisional biopsy of impaired tissue taken during surgery (total abdominal hysterectomy, bilateral salpingectomy, and omentectomy) resulted in a histopathologic diagnosis of chronic granulomas with caseous necrosis. Results: This patient was given an antituberculosis chemotherapy regimen, with rifampicin (R), isoniazid (H), and pyrazinamide (Z), for 2 months and then R & H for an additional 4 months. At follow-up, she had completely recovered and returned to normal life. Conclusions: A high index of suspicion by health professionals is required to detect and manage tuberculosis during pregnancy in patients in both the developed and developing worlds. Abstract:Peritoneal tuberculosis (PTB) rare, manifesting in regions with endemic TB, often with risk factors that include living in overcrowded areas, lower socioeconomic means, and poor hygiene. Immunocompromised patients like those with HIV and alcohol-related liver disease are also thought to be at an increased risk for PTB.
