Abstract:ACCORDING TO THE UNITED NATIONS PROGRAMME on HIV/AIDS (UNAIDS) global report on human immunodeficiency virus (HIV), in 2005 more than 2.3 million children were living with HIV infection, and 380 000 children with HIV had died. Most of the children who die from HIV have severe manifestations of common childhood illnesses, such as diarrhea, malnutrition, acute respiratory tract infection, and tuberculosis. These children often die before the diagnosis of HIV infection can be made, and mortality is often attributed to a background high rate of early childhood mortality. The United States is home for a diminishing proportion of HIV-infected children, with an estimated 11 000 cases in 2005. There are 2 reasons for this decreasing population of HIV-infected children in the developed world. First, pregnant women are increasingly being offered HIV testing, and HIV-infected women are receiving antiretroviral medications sufficient to reduce transmission to a rate of approximately 1% to 2%. Second, the population of HIV-infected children is diminishing because those children who had been infected are now growing up and becoming adults, following the introduction of potent new drugs in 1996. According to a longitudinal study by the Pediatric AIDS Clinical Trials Group (PACTG 219), the mortality rate for HIVinfected children and adolescents decreased from more than 5% in 1996 to less than 1% in 1998. Similar survival benefits have been observed among adults. The impact of highly active antiretroviral therapy (HAART) on survival and quality of life among patients affected by HIV is hardly disputable, but concise objective evidence of the benefit of treatment on infectious complications in children has been lacking in the medical literature. Before the availability of HAART, opportunistic infections such as Pneumocystis jiroveci pneumonia, bacteremia, bacterial pneumonia, and fungal sepsis were significant fatal diseases in patients infected with HIV and remain so among patients not receiving therapy. In this issue of JAMA, Gona and colleagues provide objective evidence to substantiate the benefit of HAART or, more correctly, the association of HAART and opportunistic infection prophylaxis with incidence of opportunistic infections before and after the advent of HAART. In a retrospective cohort study, Gona et al compared the incidence of 29 opportunistic infections in HIV-infected children who were treated in the HAART era with the incidence among those who were treated before the availability of HAART. They found that the incidence of P jiroveci pneumonia had decreased by 14-fold, disseminated Mycobacterium avium complex by 12-fold, bacteremia by 10-fold, and bacterial pneumonia by 5-fold. These reductions in opportunistic infections occurred despite a concomitant reduction in the use of specific prophylaxis medications, suggesting that immune reconstitution rather than improved prophylaxis was primarily responsible for better outcomes. The size and diversity of the population studied allows for generalizability of the results to the US population of children with HIV infection. There were approximately 3000 children in the cohort, which constitutes about 22% of children and adolescents living with HIV in the United States. Given the diminishing population of HIV-infected children, it is unlikely that additional data will become available in this country. There were limitations with the retrospective cohort study design, which evaluated available data from a database. Some events and clinical practices changed over time and could have affected case ascertainment and overall disease incidence. Although the incidence of bacterial pneumonia was reported, the diagnosis of this problem in children is difficult, because no consistent clinical or radiographic features reliably distinguish different etiologies of pneumonia. Radiographic pneumonia may or may not be associated with bacteremia. When such an association occurs, the relationship is not always a causal one. Specific etiologic diagnosis of bacterial pneumonia can be determined by diagnostic techniques, such as lung aspirates or bronchoalveolar lavage. These procedures are highly sensitive but are seldom used because they are considered too invasive.

Abstract:Purpose of Review Development of artificial intelligence (AI) models, particularly the application of machine learning (ML) and deep learning (DL) architectures to enhance the current diagnostic armoury for the detection of cardiovascular diseases (CVD) has expanded exponentially over the past five years. We review the current AI landscape in CVD and the impact of applying AI models to the electrocardiogram (ECG). Recent Findings Researchers continue to explore innovative methods utilising ML and DL to automate ECG diagnosis and gain insights into underlying cardiovascular physiology. The results from numerous studies including two randomised control trials (EAGLE and BEAGLE) demonstrates the wide-ranging potential for the application of AI models to both 12-lead and single lead ECG recordings. This allows a fixed timepoint recording to effectively be extrapolated into a continuous monitoring device to predict outcomes for CVD diseases such as left ventricular systolic dysfunction (LVSD), atrial fibrillation (AF), hypertrophic obstructive cardiomyopathy (HOCM), cardiac amyloidosis, valvular heart disease, pulmonary hypertension and channelopathies. A significant proportion of models developed utilise DL architectures such as convolutional neural networks (CNN). The area under the curve (AUC) for published studies range from 0.66–0.99 with sensitivities and specificities between 26.29—95% and 79.5—96.6% respectively. These results suggest a possible role for AI-ECG as a screening tool for cardiovascular pathology. Summary Initial research is promising and demonstrates the potential transformative impact of AI to streamline workflow, aid clinical decision making and prognosticate future cardiovascular outcomes. Future randomised controlled trials are needed to provide an evidence base for the implementation of AI tools into the clinician's workflow.

Abstract:Contemporary methods used to interpret the electrocardiogram (ECG) signal for diagnosis or monitoring are based on expert knowledge and rule-centered algorithms. In recent years, with the advancement of artificial intelligence, more and more researchers are using deep learning (ML) and deep learning (DL) with ECG data to detect different types of cardiac issues as well as other health problems such as respiration rate, sleep apnea, and blood pressure, etc. This study presents an extensive literature review based on research performed in the last few years where ML and DL have been applied with ECG data for many diagnoses. However, the review found that, in published work, the results showed promise. However, some significant limitations kept that technique from implementation in reality and being used for medical decisions; examples of such limitations are imbalanced and the absence of standardized dataset for evaluation, lack of interpretability of the model, inconsistency of performance while using a new dataset, security, and privacy of health data and lack of collaboration with physicians, etc. AI using ECG data accompanied by modern wearable biosensor technologies has the potential to allow for health monitoring and early diagnosis within reach of larger populations. However, researchers should focus on resolving the limitations.

Abstract:Cardiovascular diseases (CVDs) present a significant global health challenge and remain a primary cause of death. Early detection and intervention are crucial for improved outcomes in acute coronary syndrome (ACS), particularly acute myocardial infarction (AMI) cases. Artificial intelligence (AI) can detect heart disease early by analyzing patient information and electrocardiogram (ECG) data, providing invaluable insights into this critical health issue. However, the imbalanced nature of ECG and patient data presents challenges for traditional machine learning (ML) algorithms in performing unbiasedly. Investigators have proposed various data-level and algorithm-level solutions to overcome these challenges. In this study, we used a systematic literature review (SLR) approach to give an overview of the current literature and to highlight the difficulties of utilizing ML, deep learning (DL), and AI algorithms in predicting, diagnosing, and prognosis of heart diseases. We reviewed 181 articles from reputable journals published between 2013 and June 15, 2023, focusing on eight selected papers for in-depth analysis. The analysis considered factors such as heart disease type, algorithms used, applications, and proposed solutions and compared the benefits of algorithms combined with clinicians versus clinicians alone. This systematic review revealed that the current ML-based diagnostic approaches face several open problems and issues when implementing ML, DL, and AI in real-life settings. Although these algorithms show higher sensitivities, specificities, and accuracies in detecting heart disease, we must address the ethical concerns while implementing these models into clinical practice. The transparency of how these algorithms operate remains a challenge. Nevertheless, further exploration and research in ML, DL, and AI are necessary to overcome these challenges and fully harness their potential to improve health outcomes for patients with AMI.

Abstract:Leveraging artificial intelligence (AI) for the analysis of electrocardiograms (ECGs) has the potential to transform diagnosis and estimate the prognosis of not only cardiac but, increasingly, noncardiac conditions. In this review, we summarize clinical studies and AI-enhanced ECG-based clinical applications in the early detection, diagnosis, and estimating prognosis of cardiovascular diseases in the past 5 years (2019-2023). With advancements in deep learning and the rapid increased use of ECG technologies, a large number of clinical studies have been published. However, most of these studies are single-centre, retrospective, proof-of-concept studies that lack external validation. Prospective studies that progress from development toward deployment in clinical settings account for < 15% of the studies. Successful implementations of ECG-based AI applications that have received approval from the Food and Drug Administration have been developed through commercial collaborations, with approximately half of them being for mobile or wearable devices. The field is in its early stages, and overcoming several obstacles is essential, such as prospective validation in multicentre large data sets, addressing technical issues, bias, privacy, data security, model generalizability, and global scalability. This review concludes with a discussion of these challenges and potential solutions. By providing a holistic view of the state of AI in ECG analysis, this review aims to set a foundation for future research directions, emphasizing the need for comprehensive, clinically integrated, and globally deployable AI solutions in cardiovascular disease management.

Abstract:Heart disease is one of the leading causes of mortality throughout the world. Among the different heart diagnosis techniques, an electrocardiogram (ECG) is the least expensive non-invasive procedure. However, the following are challenges: the scarcity of medical experts, the complexity of ECG interpretations, the manifestation similarities of heart disease in ECG signals, and heart disease comorbidity. Machine learning algorithms are viable alternatives to the traditional diagnoses of heart disease from ECG signals. However, the black box nature of complex machine learning algorithms and the difficulty in explaining a model's outcomes are obstacles for medical practitioners in having confidence in machine learning models. This observation paves the way for interpretable machine learning (IML) models as diagnostic tools that can build a physician's trust and provide evidence-based diagnoses. Therefore, in this systematic literature review, we studied and analyzed the research landscape in interpretable machine learning techniques by focusing on heart disease diagnosis from an ECG signal. In this regard, the contribution of our work is manifold; first, we present an elaborate discussion on interpretable machine learning techniques. In addition, we identify and characterize ECG signal recording datasets that are readily available for machine learning-based tasks. Furthermore, we identify the progress that has been achieved in ECG signal interpretation using IML techniques. Finally, we discuss the limitations and challenges of IML techniques in interpreting ECG signals.

Abstract:In recent years, cardiovascular diseases (CVDs) have become one of the leading causes of mortality globally. At early stages, CVDs appear with minor symptoms and progressively get worse. The majority of people experience symptoms such as exhaustion, shortness of breath, ankle swelling, fluid retention, and other symptoms when starting CVD. Coronary artery disease (CAD), arrhythmia, cardiomyopathy, congenital heart defect (CHD), mitral regurgitation, and angina are the most common CVDs. Clinical methods such as blood tests, electrocardiography (ECG) signals, and medical imaging are the most effective methods used for the detection of CVDs. Among the diagnostic methods, cardiac magnetic resonance imaging (CMR) is increasingly used to diagnose, monitor the disease, plan treatment and predict CVDs. Coupled with all the advantages of CMR data, CVDs diagnosis is challenging for physicians due to many slices of data, low contrast, etc. To address these issues, deep learning (DL) techniques have been employed in the diagnosis of CVDs using CMR data, and much research is currently being conducted in this field. This review provides an overview of the studies performed in CVDs detection using CMR images and DL techniques. The introduction section examined CVDs types, diagnostic methods, and the most important medical imaging techniques. The following presents research to detect CVDs using CMR images and the most significant DL methods. Another section discussed the challenges in diagnosing CVDs from CMR data. Next, the discussion section discusses the results of this review, and future work in CVDs diagnosis from CMR images and DL techniques are outlined. Finally, the most important findings of this study are presented in the conclusion section.

Abstract:The electrocardiogram (ECG) remains a fundamental tool in cardiac diagnostics, yet its interpretation traditionally reliant on the expertise of cardiologists. The emergence of deep learning has heralded a revolutionary era in medical data analysis, particularly in the domain of ECG diagnostics. However, inter-patient variability prohibit the generalibility of ECG-AI model trained on a population dataset, hence degrade the performance of ECG-AI on specific patient or patient group. Many studies have address this challenge using different deep learning technologies. This comprehensive review systematically synthesizes research from a wide range of studies to provide an in-depth examination of cutting-edge deep-learning techniques in personalized ECG diagnosis. The review outlines a rigorous methodology for the selection of pertinent scholarly articles and offers a comprehensive overview of deep learning approaches applied to personalized ECG diagnostics. Moreover, the challenges these methods encounter are investigated, along with future research directions, culminating in insights into how the integration of deep learning can transform personalized ECG diagnosis and enhance cardiac care. By emphasizing both the strengths and limitations of current methodologies, this review underscores the immense potential of deep learning to refine and redefine ECG analysis in clinical practice, paving the way for more accurate, efficient, and personalized cardiac diagnostics.

Abstract:Artificial intelligence-enabled electrocardiogram (ECG) algorithms are gaining prominence for the early detection of cardiovascular (CV) conditions, including those not traditionally associated with conventional ECG measures or expert interpretation. This study develops and validates such models for simultaneous prediction of 15 different common CV diagnoses at the population level. We conducted a retrospective study that included 1,605,268 ECGs of 244,077 adult patients presenting to 84 emergency departments or hospitals, who underwent at least one 12-lead ECG from February 2007 to April 2020 in Alberta, Canada, and considered 15 CV diagnoses, as identified by International Classification of Diseases, 10th revision (ICD-10) codes: atrial fibrillation (AF), supraventricular tachycardia (SVT), ventricular tachycardia (VT), cardiac arrest (CA), atrioventricular block (AVB), unstable angina (UA), ST-elevation myocardial infarction (STEMI), non-STEMI (NSTEMI), pulmonary embolism (PE), hypertrophic cardiomyopathy (HCM), aortic stenosis (AS), mitral valve prolapse (MVP), mitral valve stenosis (MS), pulmonary hypertension (PHTN), and heart failure (HF). We employed ResNet-based deep learning (DL) using ECG tracings and extreme gradient boosting (XGB) using ECG measurements. When evaluated on the first ECGs per episode of 97,631 holdout patients, the DL models had an area under the receiver operating characteristic curve (AUROC) of 90% for 4 diagnoses (AVB, HCM, MS, STEMI). DL models outperformed XGB models with about 5% higher AUROC on average. Overall, ECG-based prediction models demonstrated good-to-excellent prediction performance in diagnosing common CV conditions.

Abstract:Cardiovascular disease (CVD) is the world's leading cause of mortality. There is significant interest in using Artificial Intelligence (AI) to analyse data from novel sensors such as wearables to provide an earlier and more accurate prediction and diagnosis of heart disease. Digital health technologies that fuse AI and sensing devices may help disease prevention and reduce the substantial morbidity and mortality caused by CVD worldwide. In this review, we identify and describe recent developments in the application of digital health for CVD, focusing on AI approaches for CVD detection, diagnosis, and prediction through AI models driven by data collected from wearables. We summarise the literature on the use of wearables and AI in cardiovascular disease diagnosis, followed by a detailed description of the dominant AI approaches applied for modelling and prediction using data acquired from sensors such as wearables. We discuss the AI algorithms and models and clinical applications and find that AI and machine-learning-based approaches are superior to traditional or conventional statistical methods for predicting cardiovascular events. However, further studies evaluating the applicability of such algorithms in the real world are needed. In addition, improvements in wearable device data accuracy and better management of their application are required. Lastly, we discuss the challenges that the introduction of such technologies into routine healthcare may face.

Abstract:Advances in Machine Learning (ML) algorithms, computing and Artificial Intelligence (AI)-based systems have been gradually finding applications in several domains including medical and health care systems. By using big data analytics and machine learning methodologies, AI has become a promising tool in the diagnosis and treatment of cardiovascular diseases. AI-ML based applications enhance our understanding of different parameters and phenotypes of heart diseases and lead to newer therapeutic strategies to tackle different types of cardiovascular ailments, a newer approach to cardiovascular drug therapy and a post-marketing survey of prescription drugs. Although AI has wide range of applications, it is in infant stage and has certain limitations in the clinical use of results and their interpretations such as data privacy, selection bias etc, which may result in wrong conclusions. Thus, AI-ML is a transformative technology and has immense potential in health care systems. This review covers various aspects of cardiovascular diseases (CVDs) and illustrate AI and ML based methods including supervised, unsupervised and deep learning and their applications in cardiovascular imaging, cardiovascular risk prediction and newer drug targets.
Nodulation of Soybeans as Affected by Half-root Infection with Heterodera glycines.

Abstract:A split-root technique was applied to soybean, Glycine max (L.) Merr. cv. Lee 68, to characterize the nature of the nodulation suppression by race 1 of the soybean cyst nematode (SCN), Heterodera glycines. Root-halves of each split-root plant were inoculated with Rhizobium japonicum, and one root-half only was inoculated with various numbers of SCN eggs. Nodulation (indicated by nodule number, nodule weights, and ratio of nodule weight to root weight) and nitrogen-fixing capacity (indicated by rate of acetylene reduction) were systemically and variously suppressed on both root-halves of the split-root plant 5 weeks after half-root inoculation with 12,500 SCN eggs. Inoculation with 500 eggs caused this suppression only on the SCN-infected (+NE) root-half; nodulation on the companion uninfected (-NE) root-half was stimulated slightly. The +NE root-halves inoculated with 5,000 eggs were excised at 2-week intervals; nodulation on the remaining -NE root-halves was not different from that of the noninoculated control when measured 6 weeks after the SCN inoculation. Thus, the systemic suppression of nodulation was reversible upon the removal of the SCN. Similarly, application of various levels of KNO to the -NE root-halves of the split-root plant did not alleviate the suppressed nodulation on the companion +NE root-halves, even though plant growth was much improved at certain levels of nitrogen (125 mug N/g soil). This indicated that the localized suppression of nodulation by SCN was caused by factors in addition to poor plant growth.

Abstract:Heart disease is one of the significant challenges in today's world and one of the leading causes of many deaths worldwide. Recent advancement of machine learning (ML) application demonstrates that using electrocardiogram (ECG) and patients' data, detecting heart disease during the early stage is feasible. However, both ECG and patients' data are often imbalanced, which ultimately raises a challenge for the traditional ML to perform unbiasedly. Over the years, several data level and algorithm level solutions have been exposed by many researchers and practitioners. To provide a broader view of the existing literature, this study takes a systematic literature review (SLR) approach to uncover the challenges associated with imbalanced data in heart diseases predictions. Before that, we conducted a meta-analysis using 451 reference literature acquired from the reputed journals between 2012 and November 15, 2021. For in-depth analysis, 49 referenced literature has been considered and studied, taking into account the following factors: heart disease type, algorithms, applications, and solutions. Our SLR study revealed that the current approaches encounter various open problems/issues when dealing with imbalanced data, eventually hindering their practical applicability and functionality.In the diagnosis of heart disease, machine learning approaches help to improve data-driven decision-making. A metadata analysis of 451 articles and content analysis of 49 selected articles of heart disease diagnosis. Researchers primarily concentrated on enhancing the performance of the models while disregarding other issues such as the interpretability and explainability of Machine learning algorithms.

Abstract:Cardiovascular disease is the leading cause of global mortality and responsible for millions of deaths annually. The mortality rate and overall consequences of cardiac disease can be reduced with early disease detection. However, conventional diagnostic methods encounter various challenges, including delayed treatment and misdiagnoses, which can impede the course of treatment and raise healthcare costs. The application of artificial intelligence (AI) techniques, especially machine learning (ML) algorithms, offers a promising pathway to address these challenges. This paper emphasizes the central role of machine learning in cardiac health and focuses on precise cardiovascular disease prediction. In particular, this paper is driven by the urgent need to fully utilize the potential of machine learning to enhance cardiovascular disease prediction. In light of the continued progress in machine learning and the growing public health implications of cardiovascular disease, this paper aims to offer a comprehensive analysis of the topic. This review paper encompasses a wide range of topics, including the types of cardiovascular disease, the significance of machine learning, feature selection, the evaluation of machine learning models, data collection & preprocessing, evaluation metrics for cardiovascular disease prediction, and the recent trends & suggestion for future works. Abstract:Cardiovascular diseases remain the leading cause of global mortality, underscoring the critical need for accurate and timely diagnosis. This narrative review examines the current applications and future potential of artificial intelligence (AI) and machine learning (ML) in cardiovascular imaging. We discuss the integration of these technologies across various imaging modalities, including echocardiography, computed tomography, magnetic resonance imaging, and nuclear imaging techniques. The review explores AI-assisted diagnosis in key areas such as coronary artery disease detection, valve disorders assessment, cardiomyopathy classification, arrhythmia detection, and prediction of cardiovascular events. AI demonstrates promise in improving diagnostic accuracy, efficiency, and personalized care. However, significant challenges persist, including data quality standardization, model interpretability, regulatory considerations, and clinical workflow integration. We also address the limitations of current AI applications and the ethical implications of their implementation in clinical practice. Future directions point towards advanced AI architectures, multimodal imaging integration, and applications in precision medicine and population health management. The review emphasizes the need for ongoing collaboration between clinicians, data scientists, and policymakers to realize the full potential of AI in cardiovascular imaging while ensuring ethical and equitable implementation. As the field continues to evolve, addressing these challenges will be crucial for the successful integration of AI technologies into cardiovascular care, potentially revolutionizing diagnostic capabilities and improving patient outcomes.
Opportunities and Challenges of Deep Learning Methods for Electrocardiogram Data: A Systematic Review

Abstract:Background:The electrocardiogram (ECG) is one of the most commonly used diagnostic tools in medicine and healthcare. Deep learning methods have achieved promising results on predictive healthcare tasks using ECG signals. Objective:This paper presents a systematic review of deep learning methods for ECG data from both modeling and application perspectives. Methods:We extracted papers that applied deep learning (deep neural network) models to ECG data that were published between Jan. 1st of 2010 and Feb. 29th of 2020 from Google Scholar, PubMed, and the DBLP. We then analyzed each article according to three factors: tasks, models, and data. Finally, we discuss open challenges and unsolved problems in this area. Results: The total number of papers extracted was 191. Among these papers, 108 were published after 2019. Different deep learning architectures have been used in various ECG analytics tasks, such as disease detection/classification, annotation/localization, sleep staging, biometric human identification, and denoising. Conclusion: The number of works on deep learning for ECG data has grown explosively in recent years. Such works have achieved accuracy comparable to that of traditional feature-based approaches and ensembles of multiple approaches can achieve even better results. Specifically, we found that a hybrid architecture of a convolutional neural network and recurrent neural network ensemble using expert features yields the best results. However, there are some new challenges and problems related to interpretability, scalability, and efficiency that must be addressed. Furthermore, it is also worth investigating new applications from the perspectives of datasets and methods. Significance: This paper summarizes existing deep learning research using ECG data from multiple perspectives and highlights existing challenges and problems to identify potential future research directions.

Abstract:Electrocardiography (ECG), improved by artificial intelligence (AI), has become a potential technique for the precise diagnosis and treatment of cardiovascular disorders. The conventional ECG is a frequently used, inexpensive, and easily accessible test that offers important information about the physiological and anatomical state of the heart. However, the ECG can be interpreted differently by humans depending on the interpreter's level of training and experience, which could make diagnosis more difficult. Using AI, especially deep learning convolutional neural networks (CNNs), to look at single, continuous, and intermittent ECG leads that has led to fully automated AI models that can interpret the ECG like a human, possibly more accurately and consistently. These AI algorithms are effective non-invasive biomarkers for cardiovascular illnesses because they can identify subtle patterns and signals in the ECG that may not be readily apparent to human interpreters. The use of AI in ECG analysis has several benefits, including the quick and precise detection of problems like arrhythmias, silent cardiac illnesses, and left ventricular failure. It has the potential to help doctors with interpretation, diagnosis, risk assessment, and illness management. Aside from that, AI-enhanced ECGs have been demonstrated to boost the identification of heart failure and other cardiovascular disorders, particularly in emergency department settings, allowing for quicker and more precise treatment options. The use of AI in cardiology, however, has several limitations and obstacles, despite its potential. The effective implementation of AI-powered ECG analysis is limited by issues such as systematic bias. Biases based on age, gender, and race result from unbalanced datasets. A model's performance is impacted when diverse demographics are inadequately represented. Potentially disregarded age-related ECG variations may result from skewed age data in training sets. ECG patterns are affected by physiological differences between the sexes; a dataset that is inclined toward one sex may compromise the accuracy of the others. Genetic variations influence ECG readings, so racial diversity in datasets is significant. Furthermore, issues such as inadequate generalization, regulatory barriers, and interpretability concerns contribute to deployment difficulties. The lack of robustness in models when applied to disparate populations frequently hinders their practical applicability. The exhaustive validation required by regulatory requirements causes a delay in deployment. Difficult models that are not interpretable erode the confidence of clinicians. Diverse dataset curation, bias mitigation strategies, continuous validation across populations, and collaborative efforts for regulatory approval are essential for the successful deployment of AI ECG in clinical settings and must be undertaken to address these issues. To guarantee a safe and successful deployment in clinical practice, the use of AI in cardiology must be done with a thorough understanding of the algorithms and their limits. In summary, AI-enhanced electrocardiography has enormous potential to improve the management of cardiovascular illness by delivering precise and timely diagnostic insights, aiding clinicians, and enhancing patient outcomes. Further study and development are required to fully realize AI's promise for improving cardiology practices and patient care as technology continues to advance.

Cardiovascular Diagnosis and Therapy

Abstract:The global mortality rate is known to be the highest due to cardiovascular disease (CVD). Thus, preventive, and early CVD risk identification in a non-invasive manner is vital as healthcare cost is increasing day by day. Conventional methods for risk prediction of CVD lack robustness due to the non-linear relationship between risk factors and cardiovascular events in multi-ethnic cohorts. Few recently proposed machine learning-based risk stratification reviews without deep learning (DL) integration. The proposed study focuses on CVD risk stratification by the use of techniques mainly solo deep learning (SDL) and hybrid deep learning (HDL). Using a PRISMA model, 286 DL-based CVD studies were selected and analyzed. The databases included were Science Direct, IEEE Xplore, PubMed, and Google Scholar. This review is focused on different SDL and HDL architectures, their characteristics, applications, scientific and clinical validation, along with plaque tissue characterization for CVD/stroke risk stratification. Since signal processing methods are also crucial, the study further briefly presented Electrocardiogram (ECG)-based solutions. Finally, the study presented the risk due to bias in AI systems. The risk of bias tools used were (I) ranking method (RBS), (II) region-based map (RBM), (III) radial bias area (RBA), (IV) prediction model risk of bias assessment tool (PROBAST), and (V) risk of bias in non-randomized studies-of interventions (ROBINS-I). The surrogate carotid ultrasound image was mostly used in the UNet-based DL framework for arterial wall segmentation. Ground truth (GT) selection is vital for reducing the risk of bias (RoB) for CVD risk stratification. It was observed that the convolutional neural network (CNN) algorithms were widely used since the feature extraction process was automated. The ensemble-based DL techniques for risk stratification in CVD are likely to supersede the SDL and HDL paradigms. Due to the reliability, high accuracy, and faster execution on dedicated hardware, these DL methods for CVD risk assessment are powerful and promising. The risk of bias in DL methods can be best reduced by considering multicentre data collection and clinical evaluation.

Abstract:Hypertrophic Cardiomyopathy (HCM) presents a complex diagnostic and prognostic challenge due to its heterogeneous phenotype and clinical course. Artificial Intelligence (AI) and Machine Learning (ML) techniques hold promise in transforming the role of Electrocardiography (ECG) in HCM diagnosis, prognosis, and management. AI, including Deep Learning (DL), enables computers to learn patterns from data, allowing for the development of models capable of analyzing ECG signals. DL models, such as convolutional neural networks, have shown promise in accurately identifying HCM-related abnormalities in ECGs, surpassing traditional diagnostic methods. In diagnosing HCM, ML models have demonstrated high accuracy in distinguishing between HCM and other cardiac conditions, even in cases with normal ECG findings. Additionally, AI models have enhanced risk assessment by predicting arrhythmic events leading to sudden cardiac death and identifying patients at risk for atrial fibrillation and heart failure. These models incorporate clinical and imaging data, offering a comprehensive evaluation of patient risk profiles. Challenges remain, including the need for larger and more diverse datasets to improve model generalizability and address imbalances inherent in rare event prediction. Nevertheless, AI-driven approaches have the potential to revolutionize HCM management by providing timely and accurate diagnoses, prognoses, and personalized treatment strategies based on individual patient risk profiles. This review explores the current landscape of AI applications in ECG analysis for HCM, focusing on advancements in AI methodologies and their specific implementation in HCM care.

Abstract:Cardiovascular health-related problem is a rapidly increasing integrated field concerning the processing and fetching the information from cardiovascular systems for early detection and treatment of cardiovascular diseases. Artificial Intelligence (AI) techniques, especially machine and deep learning techniques are more impactful and powerful tools for upgrading the capabilities of an application, and they have been applied to medical data for analysis and disease detection purposes. This paper, represents a comprehensive view of AI-based computational modeling with the abilities of powerful AI techniques that can play a crucial role in developing smart and enhanced systems in a real-world application. The paper outlines the broad overview of AI-based modeling that can be utilized in various application domains. An electrocardiogram (ECG) plays a major role in biomedical applications to record the heartbeat activity. Regular monitoring of ECG through wearable devices like the band, watches, etc. can be done for early detection of cardiovascular diseases. The competency of each method discussed is related to ECG classification approaches that have been compared in terms of some parameters like accuracy, sensitivity, specificity, positive predictivity, and F-score. The noise affects the ECG signal which may deteriorate the features of the respective signal that leading to improper treatment. De-noising has been done by pre-processing of the signal, which enables the prediction of the heart condition. After detecting the positions of the P wave, QRS wave, and T wave, feature extraction has been done. The efficiency of ECG classification with different computational methods was evaluated with proposed algorithms by using different databases like MIT-BIH, PTB, and MIT-BIH Atrial Fibrillation test. It has been observed from the literature that Convolutional Neural Networks (CNN) are best suited for the classification and detection of arrhythmia. The challenges of existing techniques to analyze the ECG signal for the classification and detection of arrhythmia are summarized.
